Low back pain in junior Australian Rules footballers has not been investigated, despite findings that adolescent back pain is a strong predictor for adult back pain. The aim of this study was to determine the prevalence, intensity, quality and frequency of low back pain in junior Australian Rules footballers. A cross-sectional survey of male non-elite junior (n = 60) and elite junior players (n = 102) was conducted along with a convenience sample of non-footballers (school children) (n = 100). Subjects completed a self-reported questionnaire on low back pain incorporating the Quadruple Visual Analogue Scale and McGill Pain Questionnaire (short form), along with additional questions adapted from an Australian epidemiological study. For current, average and best low back pain levels, elite junior players had higher pain levels (p < 0.001), with no difference noted between non-elite juniors and controls for average and best low back pain. For low back pain at worst, there were significant differences in the mean pain cores. The difference between elite juniors and non-elite juniors (p = 0.040) and between elite juniors and controls (p < 0.001) was significant, but not between non-elite juniors and controls. The chance of suffering low back pain increases from 45% for controls, through 55% for non-elite juniors to 66.7% for elite juniors. The chance that a pain sufferer experiences chronic pain is 16% for controls and 41% for non-elite junior and elite junior players. Elite junior players experienced low back pain more frequently (p = 0.002), with no difference in frequency noted between non-elite juniors and controls. Over 25% of elite junior and non-elite junior players reported that back pain impacted their performance some of the time or greater. This study demonstrated that when compared with non-elite junior players and non-footballers of a similar age, elite junior players experience back pain more severely and frequently and have higher prevalence and chronicity rates

To determine the number of patients with low back pain who have low serum Vitamin-D levels in our local population and the clinical efficacy of Vitamin-D supplementation on VAS and MODQ scores. This Prospective cohort study was conducted from 20th March 2016 to 19th March 2017. 600 patients were included in the study who met the inclusion criteria, i.e. patients presenting to the Out Patient Department (OPD) with low back pain for a duration of less than six months aged between 15 to 55 years. Venous blood withdrawn and serum levels of Vitamin-D measured. According to serum Vitamin-D levels, categorized as deficient, sufficient or excess. Those having deficient Vitamin-D levels (< 2 0 ng/dL) were given Vitamin-D supplementation as Oral 50,000 IU Vitamin-D3 daily for 05 days, then once weekly for 08 weeks while those having insufficient levels (20–30 ng/dL) given Oral 50,000 IU Vitamin-D3 once weekly for 08 weeks. Vitamin-D levels, Visual Analog Pain Scale (VAS) and Modified Oswestry Disability Questionnaire (MODQ) scoring done at baseline, 02, 03 and 06 months. Data analyzed using SPSS version 23. Mean age of patients included in the study 44.21 ± 11.92 years. Out of the total, 337 (56.17%) were males and 263 (43.83%) females. Out of the total, 20.67%, 26.17% and 28.83% had mild, moderate and severe Vitamin-D deficiency, respectively. Predominantly patients with severe Vitamin-D deficiency presented in winters (October – February) (17.16%) as compared to other seasons. The most pre-dominant risk factor in patients with low Vitamin-D levels was smoking (21.33%). Mean baseline Vitamin-D levels were 13.32 ± 6.10 ng/dL and after supplementation these levels improved to 37.18 ± 11.72 ng/dL. VAS score improved from a mean baseline value to 81 to 36 at 6 months (p < 0 .01). Likewise, MODQ score decreased from a baseline mean of 46 to 25 at 6 months (p < 0 .01). Vitamin D plays a crucial role in the musculoskeletal framework of the body. The deficiency is more prevalent in the youth due to sedentary lifestyle and indoor preference. The study used a qualitative methodology to explore the attitudes and beliefs of military physiotherapists and how these influenced the management of military patients presenting with chronic low back pain. Semi-structured interviews were undertaken with a sample of 16 military physiotherapists; the transcripts were analysed using a method of thematic content analysis. Analysis of semi-structured interviews undertaken resulted in the identification of six themes. These were: military culture, occupational issues, continuing professional development, clinical reasoning, need for cure and labelling the patient. The importance of understanding the occupational demands on their patients was considered highly significant by all of the military physiotherapists interviewed. However, there appeared generally poor knowledge of the biopsychosocial model in the management of low back pain and over-reliance on the medical model. Three-quarters of the military physiotherapists interviewed expressed frustration in their management of patients with low back pain. Similarly, the military physiotherapists displayed a poor awareness of current evidence-based clinical guidelines for the management of low back pain. The themes military culture and occupational issues were significant in influencing the military physiotherapist's clinical management. The highly physical and arduous nature of military occupations resulted in investigative procedures being requested at an earlier stage than is recommended in the current evidence-based guidelines. Justification for early investigations was provided on the basis of the unique occupational factors combined with requirement to optimise the number of military personnel able to deploy operationally. It was concluded that the management of low back pain in military personnel could be improved by increasing awareness of the current evidence-based guidelines. This would benefit both patients and the Armed Services, by reducing the disability caused by low back pain and increasing the number of operationally deployable service personnel

Low back pain is a common complaint and reason for patients to seek medical help. Studies have shown that 80% of people over 60 yrs of age will have suffered from it at one point in their lives. Low back pain, after the common cold, is the 2nd most common reason for patients to visit medical practitioners. Aim. The purpose of this survey was to establish a patient profile, risk factors, previous management and care strategies for patients presenting to the Orthopaedic Out Patient Department at an Academic Hospital with low back pain. Method. We reviewed 257 patients during a 6 month period from July 2010- December 2010.0. Patients were excluded if there were: XR deformities (eg. spondylolistheses, masses, cysts etc); associated neurology, incontinence or constitutional symptoms; previous vertebral column fracture or surgery or current malignancy. Patients referred for first visits were also excluded. Results. There were 206 females and 51 males. The average age of patients was 55 yrs. The average duration of symptoms was 6 months and the average duration of clinical follow-up at the institution was 8 months. The occupational setback due to this affliction was great: 23% were unemployed due to the pain and 21% receiving Disability Grants for it. The average BMI of these patients was 35. A smoking and alcohol history was not conspicuous and very few patients had a history of trauma. The investigative and treatment strategy and implementation in these patients was haphazard

Intervertebral discs (IVDs) degeneration is one of the major causes of back pain. Upon degeneration, the IVDs tissue become inflamed, and this inflammatory microenvironment may cause discogenic pain. Cellular senescence is a state of stable cell cycle arrest in response to a variety of cellular stresses including oxidative stress and adverse load. The accumulation of senescent IVDs cells in the tissue suggest a crucial role in the initiation and development of painful IVD degeneration. Senescent cells secrete an array of cytokines, chemokines, growth factors, and proteases known as the senescence-associated secretory phenotype (SASP). The SASP promote matrix catabolism and inflammation in IVDs thereby accelerating the process of degeneration. In this study, we quantified the level of senescence in degenerate and non-degenerate IVDs and we evaluated the potential of two natural compounds to remove senescent cells and promote overall matrix production of the remaining cells. Human IVDs were obtained from organ donors. Pellet or monolayer cultures were prepared from freshly isolated cells and cultured in the presence or absence of two natural compounds: Curcumin and its metabolite vanillin. Monolayer cultures were analyzed after four days and pellets after 21 days for the effect of senolysis. A cytotoxicity study was performed using Alamar blue assay. Following treatment, RNA was extracted, and gene expression of senescence and inflammatory markers was evaluated by real-time q-PCR using the comparative ΔΔCt method. Also, protein expression of p16, Ki-67 and Caspase-3 were evaluated in fixed pellets or monolayer cultures and total number of cells was counted on consecutive sections using DAPI and Hematoxylin. Proteoglycan content was evaluated using SafraninO staining or DMMB assay to measure sulfated glycosaminoglycan (sGAG) and antibodies were used to stain for collagen type II expression. We observed 40% higher level of senescent cells in degenerate compare to the non-degenerate discs form unrelated individuals and a 10% increase when we compare degenerate compare to the non-degenerate discs of the same individual. Using the optimal effective and safe doses, curcumin and vanillin cleared 15% of the senescent cells in monolayer and up to 80% in pellet cultures. Also, they increased the number of proliferating and apoptotic cells in both monolayer and pellets cultures. The increase in apoptotic cell number and caspase-3/7 activity was specific to degenerate cells. Following treatment, mRNA expression levels of SASP factors were decreased by four to 32-fold compared to the untreated groups. Senescent cell clearance decreased, protein expression of MMP-3 and −13 by 15 and 50% and proinflammatory cytokines levels of IL-1, IL-6 and IL-8 by 42, 63 and 58 %. Overall matrix content was increased following treatment as validated by an increase in proteoglycan content in pellet cultures and surrounding culture media. This work identifies novel senolytic drugs for the treatment of IVD degeneration. Senolytic drugs could provide therapeutic interventions that ultimately, decrease pain and provide a better quality of life of patients living with IVDs degeneration and low back pain

Most people experience low back pain (LBP) at least once in their lifetime. A minority goes on to develop persistent LBP causing significant socioeconomic costs. Aim of this study was to identify factors that influence the progression of acute to persistent LBP at an early stage (Hilfiker et al. 2007). Prospective inception cohort study of patients attending a health practitioner for their first episode of acute LBP or recurrent LBP after a pain free period of at least six months. Patients were assessed at baseline addressing occupational and psychological factors as well as pain, disability, quality of life and physical activity, and followed up over six months. Baseline and follow-up questionnaires were based on the recommendations of the Multinational Musculoskeletal Inception Cohort Study (MMICS) Statement (Pincus et al. 2008). Variables were combined to the three indices ‘working condition’, ‘depression and maladaptive cognitions’ and ‘pain and quality of life’. The index ‘depression and maladaptive cognitions’ comprising of depression, somatisation, a resigned attitude towards the job, fear-avoidance, catastrophizing and negative expectations on return to work was found to be a significant baseline predictor for persistent LBP up to six months (OR 5.1; 95%CI 1.04–25.1). The diagnostic accuracy of the predictor model had a sensitivity of 0.54 and a specificity of 0.90. Positive likelihood ratio was moderate with 5.3, negative likelihood ratio 0.5. Overall predictive accuracy of the model was 81%. The area under the curve in receiver operating characteristic (ROC) analysis of the index was 0.78 (CI95% 0.65–0.92), demonstrating a satisfactory quality of discrimination. Psychological factors in patients with acute LBP in a primary care setting correlated with a progression to persistent LBP up to six months. The benefit of including factors such as ‘depression and maladaptive cognition’ in screening tools is that these factors can be addressed in primary and secondary prevention

To investigate the effectiveness of surgical fusion for chronic low back pain (CLBP) compared to non-surgical intervention, databases were searched from 1966-2005. The meta-analysis was based on the mean difference in Oswestry Disability Index (ODI) change from baseline to follow-up. Four studies were eligible (634 patients). The pooled mean difference in ODI was 4.13 in favour of surgery (95% CI: -0.82-9.08; p=0.10; I2=44.4%). Surgery was associated with a 16% pooled rate of complication (95% CI: 12-20%, I2=0%). The cumulative evidence does not support surgical fusion for CLBP due to the marginal improvement in ODI which is of minimal clinical importance

Introduction. Numerous studies have been conducted to investigate the kinematics of the lumbar spine, and while many have documented its intricacies, few have analyzed the complex coupled out-of-plane rotations inherent in the low back. Some studies have suggested a possible relationship between patients having low back pain (LBP) or degenerative conditions in the lumbar region and various degrees of restricted, excessive, or poorly-controlled lumbar motion. Conversely, others in the orthopedic community maintain there has been no distinct correlation found between spinal mobility and clinical symptoms. The objective of this study was to evaluate both the in-plane and coupled out-of-plane rotational magnitudes about all three motion axes in both symptomatic and asymptomatic patients. Methods. Ten healthy, 10 LBP, and 10 degenerative patients were CT scanned and evaluated under fluoroscopic surveillance while performing flexion/extension of the lumbar spine. Three-dimensional, patient-specific bone models were created and registered to fluoroscopic images using a 3D-to-2D model fitting algorithm. In vivo kinematics were derived at specified increments and the overall in-plane flexion/extension and coupled out-of-plane rotations were analyzed using two techniques. The first method derived the maximal absolute rotational magnitude (MARM) at each level by subtracting the rotational motion in the increment exhibiting the most negative or least amount of rotation from the increment having the greatest amount of rotation. The second method was designed to isolate the path of rotation (POR) of the vertebrae at each level while performing the prescribed flexion/extension activity. By tracking the rotational path of the cephaled vertebrae as it articulated upon the more caudal vertebrae and summing the absolute rotation between each increment about each axis the POR was calculated over the entire flexion/extension activity. Results. Using both the MARM and POR methods, the average overall in-plane rotations between L1 and L5 were not significantly different among any of the groups, although the degenerative group did exhibit less in-plane range-of-motion compared to the healthy and LBP patients. At the L4–L5 level, patients in the healthy and LBP groups achieved 13.1° and 14.4° of rotation, respectively, compared to only 10.7° in the degenerative group. In addition, both of the symptomatic patient groups experienced less rotation during the extension phase of the activity. The coupled out-of-plane motions in both the LBP and degenerative subjects were significantly greater than those observed in healthy subjects (p=0.0199 and p<0.001, respectively). On average, LBP and degenerative patients achieved 5.5° and 7.1° more out-of plane rotational motion per level, respectively, compared to healthy subjects. Conclusions. These findings correlate with previous studies documenting paradoxical motions in the lumbar spine during an overall gross motion and support the idea of pain being a biological response to tissue injury which may result from excessive kinetic energy introduced into the biological system. Identification of these aberrant motion path magnitudes may aid in recognizing possible causes of pain in patients suffering from non-specific low back problems. Increased magnitudes of out-of-plane rotational paths observed in symptomatic patients may also be an indicator for progressive pathologies requiring surgical intervention in the lumbar spine region

Worldwide, most spine imaging is either “inappropriate” or “probably inappropriate”. The Choosing Wisely recommendation is “Do not perform imaging for lower back pain unless red flags are present.” There is currently no detailed breakdown of lower back pain diagnostic imaging performed in New Brunswick (NB) to inform future directions.

A registry of spine imaging performed in NB from 2011-2019 inclusive (n=410,000) was transferred to the secure platform of the NB Institute for Data, Training and Research (NB-IRDT). The pseudonymized data included linkable institute identifiers derived from an obfuscated Medicare number, as well as information on type of imaging, location of imaging, and date of imaging. The transferred data did not include the radiology report or the test requisition.

We included all lumbar, thoracic, and complete spine images. We excluded imaging related to the cervical spine, surgical or other procedures, out-of-province patients and imaging of patients under 19 years.

We verified categories of X-ray, Computed Tomography (CT), and Magnetic Resonance Imaging (MRI). Red flags were identified by ICD-10 code-related criteria set out by the Canadian Institute for Health Information.

We derived annual age- and sex-standardized rates of spine imaging per 100,000 population and examined regional variations in these rates in NB's two Regional Health Authorities (RHA-A and RHA-B). Age- and sex-standardized rates were derived for individuals with/without red flag conditions and by type of imaging. Healthcare utilization trends were reflected in hospital admissions and physician visits 2 years pre- and post-imaging. Rurality and socioeconomic status were derived using patients’ residences and income quintiles, respectively.

Overall spine imaging rates in NB decreased between 2012 and 2019 by about 20% to 7,885 images per 100,000 people per year. This value may be higher than the Canadian average.

Females had 23% higher average imaging rate than males. RHA-A had a 45% higher imaging rate than RHA-B. Imaging for red flag conditions accounted for about 20% of all imaging.

X-rays imaging accounted for 67% and 75% of all imaging for RHA-A and RHA-B respectively. The proportions were 20% and 8% for CT and 13% and 17% for MRI.

Two-year hospitalization rates and rates of physician visits were higher post-imaging. Females had higher age-standardized hospitalization and physician-visit rates, but the magnitude of increase was higher for males. Individuals with red flag conditions were associated with increased physician visits, regardless of the actual reason for the visit.

Imaging rates were higher for rural than urban patients by about 26%. Individuals in the lowest income quintiles had higher imaging rates than those in the highest income quintiles. Physicians in RHA-A consistently ordered more images than their counterparts at RHA-B.

We linked spine imaging data with population demographic data to look for variations in lumbar spine imaging patterns. In NB, as in other jurisdictions, imaging tests of the spine are occurring in large numbers. We determined that patterns of imaging far exceed the numbers expected for ‘red flag’ situations. Our findings will inform a focused approach in groups of interest. Implementing high value care recommendations pre-imaging ought to replace low-value routine imaging.

Low back pain is more common in women than men, yet most studies of intervertebral disc (IVD) degeneration do not address sex differences. In humans, there are sex differences in spinal anatomy and degenerative changes in biomechanics, and animal models of chronic pain have demonstrated sex differences in pain transduction. However, there are few studies investigating sex differences in annular puncture IVD degeneration models. IVD puncture is known to result in progressive biomechanical alterations, but whether these IVD changes correlate with pain is unknown. This study used a rat IVD injury model to determine if sex differences exist in mechanical allodynia, biomechanics, and the relationship between them, six weeks after IVD injury. Procedures were IACUC approved. 24 male & 24 female four-month-old Sprague-Dawley rats underwent a sham or annular puncture injury surgery (n=12 male, 12 female). In injury groups, three lumbar IVDs were each punctured three times with a needle, and injected with tumor necrosis factor-alpha. Mechanical allodynia was tested biweekly using von Frey filaments. Six weeks after IVD injury, rats were euthanized and motion segments were dissected for non-destructive axial tension-compression and torsional rotation biomechanical testing. Two-way ANOVA with Bonferroni corrections identified statistically significant differences (p < 0 .05) and correlations used Pearson's coefficient. Annular puncture injury induced a significant increase in mechanical allodynia compared to sham in male but not female rats up to six weeks after injury. There was a significant sex effect on both torque range and torsional stiffness, with males exhibiting greater stiffness and torque range than females. Tensile stiffness, compressive stiffness, and axial range of motion showed no sex difference. Males and females showed similar patterns of correlation between variables when sham and injury groups were analyzed together, but correlations were stronger in males. Most correlations were clustered within testing approach: axial biomechanics negatively correlated, torsional biomechanics positively correlated, and von Frey thresholds positively correlated. Surprisingly, mechanical allodynia did not correlate with any biomechanics after injury, and the axial and torsional biomechanics showed little correlation. This study demonstrates that males and females respond to IVD injury differently. Given the absence of correlation between pain and biomechanics, pain cannot be attributed completely to biomechanical changes. This may explain why spinal fusion surgery, an intervention limited to the spine, has produced inconsistent results and is controversial for patients with low back pain. Thus, in addressing low back pain, we must consider both spinal tissues and the nervous system. Further, the limited correlation between axial and torsional biomechanics indicates that IVD injury may have distinct effects on nucleus pulposus and annulus fibrosus. Biomechanics did not differ between sham and injury at week six, suggesting healing after injury. It remains possible that acute biomechanical changes may initiate chronic pain pathogenesis. We conclude that the observed sex differences demonstrate the need for inclusion of both males and females in IVD injury and pain studies, and suggest that males and females may require different treatments for conditions that appear similar

A retrospective analysis was carried out to determine the influence of pre-existing spinal pathology on the outcome of Total Knee Replacement surgery. Data was collected from 345 patients who had undergone Total Knee Replacement, at four centres in the UK, between 2000 and 2007. Oxford Knee Scores (OKS), American Knee Society Scores (AKSS) and SF-12 questionnaires were recorded prospectively. Data was collected pre-operatively and then post-operatively at 3 months, 1 year and 2 years. Patients were divided into those with (n=40) and without a history of low back pain (n=305). In addition to determining the influence of low back pain on outcome after Total Knee Replacement we also examined the influence of concomitant hip and ankle pathology in the same cohort of patients. OKS scores were significantly worse for patients with symptomatic low back pain at 3 (p=0.05), 12 (p=0.009) and 24 months (p=0.039) following surgery. SF-12 physical scores followed a comparable pattern with significance demonstrated at 3 (p=0.038), 12 (p=0.0002) and 24 months (p=0.016). AKSS followed a similar pattern, but significance was only reached at 1 year (p=0.013). The mental component of the SF-12 measure demonstrated a significant improvement in patients' mental health post-operatively for patients with no history of low back pain. In contrast patients with low back pain showed no improvement in mental health scores post-operatively. In contrast to low back pain, hip and ankle pathology had no statistically significant detrimental effect on the outcome of Total Knee Replacement surgery. This study demonstrates that low back pain significantly affects the functional outcome after Total Knee Replacement surgery and that patients with low back pain show no improvement in mental health post-operatively

7% of adolescent idiopathic scoliosis (AIS) patients also present with a pars defect. To date, there are no available data on the results of fusion ending proximal to a spondylolysis in the setting of AIS. The aim of this study was to analyze the outcomes of posterior spinal fusion (PSF) in this patient cohort, to investigate if maintaining the lytic segment unfused represents a safe option. Retrospective review of all patients who received PSF for AIS, presented with a spondylolysis or spondylolisthesis and had a min. 2-years follow-up. Demographic data, instrumented levels and preoperative radiographic data were collected. Mechanical complications, coronal or sagittal parameters, amount of slippage and pain levels were evaluated. Data from 22 patients were available (age 14.4 ± 2.5 years), 18 Lenke 1–2 and four Lenke 3–6. Five patients (24%) had an isthmic spondylolisthesis, all Meyerding I. The mean preoperative Cobb angle of the instrumented curves was 58 ± 13°. For 18 patients the lowest instrumented vertebra (LIV) was the last touched vertebra (LTV); for two LIV was distal to the LTV; for two, LIV was one level proximal to the LTV. The number of segments between the LIV and the lytic vertebra ranged from 1 to 6. At the last follow-up, no complications were observed. The residual curve below the instrumentation measured 8.5 ± 6.4°, the lordosis below the instrumented levels was 51.4 ± 13°. The magnitude of the isthmic spondylolisthesis remained constant for all included patients. Three patients reported minimal occasional low back pain. The LTV can be safely used as LIV when performing PSF for the management of AIS in patients with L5 spondylolysis

Low back pain is the single most common cause for disability in individuals aged 45 years or younger, it carries tremendous weight in socioeconomic considerations. Degenerative aging of the structural components of the spine can be associated with genetic aspects, lifetime of tissue exposure to mechanical stress & loads and environmental factors. Mechanical consequences of the disc degenerative include loss of disc height, segment instability and increase the load on facets joints. All these can lead to degenerative changes and osteophytes that can narrow the spinal canal. Surgery is indicated in patients with spinal stenosis who have intractable pain, altered quality of life, substantially diminished functional capacity, failed non-surgical treatment and are not candidates for non-surgical treatment. The aim was to determine the reasons for refusal of surgery in patients with established degenerative lumber spine pathology eligible for surgery. All patients meeting the study criteria, patients older than 18 years, patients with both clinical and radiological established symptomatic degenerative lumbar spine pathology and patients eligible for surgery but refusing it were recruited. Questionnaire used to investigate reasons why they are refusing surgery. Results 59 were recruited, fifty-one (86.4 %) females and eight (13.6 %) males. Twenty (33.8 %) were between the age of 51 and 60 years, followed by nineteen (32.2 %) between 61 and 70 years, and fourteen (23.7 %) between 71 and 80 years. 43 (72 %) patients had lumber spondylosis complicated by lumber spine stenosis, followed by nine (15.2 %) with lumbar spine spondylolisthesis and four (6.7 %) had adjacent level disease. 28 (47.4 %) were scared of surgery, fifteen (25.4 %) claimed that they are too old for surgery and nine (15.2 %) were not ready. Findings from this study outlined that patients lack information about the spinal surgery. Patients education about spine surgery is needed

Psoas tendinopathy is a potential cause of groin pain after primary total hip arthroplasty (THA). The direct anterior approach (DAA) is becoming increasingly popular as the standard approach for primary THA due to being a muscle preserving technique. It is unclear what the prevalence is for the development of psoas-related pain after DAA THA, how this can influence patient reported outcome, and which risk factors can be identified. This retrospective case control study of prospectively recorded data evaluated 1784 patients who underwent 2087 primary DAA THA procedures between January 2017 and September 2019. Psoas tendinopathy was defined as (1) persistence of groin pain after DAA THA and was triggered by active hip flexion, (2) exclusion of other causes such as dislocation, infection, implant loosening or (occult) fractures, and (3) a positive response to an image-guided injection with xylocaine and steroid into the psoas tendon sheath. Complication-, re-operation rates, and patient-reported outcome measures (PROMs) were measured. Forty-three patients (45 hips; 2.2%) were diagnosed with psoas tendinopathy according to the above-described criteria. The mean age of patients who developed psoas tendinopathy was 50.8±11.7 years, which was significantly lower than the mean age of patients without psoas pain (62.4±12.7y; p<0.001). Patients with primary hip osteoarthritis were significantly less likely to develop psoas tendinopathy (14/1207; 1.2%) in comparison to patients with secondary hip osteoarthritis to dysplasia (18/501; 3.6%) (p<0.001) or FAI (12/305; 3.9%) (p<0.001). Patients with psoas tendinopathy had significantly lower PROM scores at 6 weeks and 1 year follow-up. Psoas tendinopathy was present in 2.2% after DAA THA. Younger age and secondary osteoarthritis due to dysplasia or FAI were risk factors for the development of psoas tendinopathy. Post-operatively, patients with psoas tendinopathy often also presented with low back pain and lateral trochanteric pain. Psoas tendinopathy had an important influence on the evolution of PROM scores

Background. In 2009, NICE CG 88 guideline on the management of non-specific low back pain was published. We looked at whether the introduction of these guidelines has had an impact on the management of back pain within primary care. Methods. Patients with non-specific low back pain (> 6 weeks but < 12 months) attending spinal outpatient clinic in UHL between 2008 and 2011 were asked to complete questionnaires. Two groups were studied, the first prior to the publication of NICE guidelines, and the second afterwards. Patients with radicular, stenotic and red flag symptoms were excluded. Key audited treatment standards assessed included manual therapy, acupuncture, focused structured back exercise program, supervised group exercise program and lastly referral to a combined physical and psychological treatment program. Compliance with not using X-ray or MRI and treatment modalities such as injections, laser therapy, ultrasound therapy, lumbar supports, traction and TENS therapy was assessed. Secondary outcomes included VAS (back, leg pain), Oswestry Disability Index, MSP and MZD. Primary outcomes analyzed using 1-sided Fisher's exact test and secondary outcomes using two sample t tests. Results. 46 patients (pre-guidelines) and 34 patients (post-guidelines) were studied. Key findings showed significant deterioration in the institution of manual therapy in the post guidelines group (p value = 0.032) and an increase in use of MRI scan in post guidelines group (p value = 0.005). Deterioration in the mean presenting VAS for leg pain in post guidelines group noted. No significant difference between groups in the mean scores for VAS for back pain, ODI, MSP and MZDI. Conclusion. Our study suggests that the introduction of NICE guidelines on the management of low back pain has not yet influenced management in primary care. This may be due to lack of awareness of its implementation or due to adherence to local guidelines

Osteomyelitis is usually related to trauma, surgery, immunocompromised patients, IV drug abuse, poor vascular supply, diabetes, sickle cell disease or peripheral neuropathy. We report an unusual case of femur osteomyelitis without any of these risk factors. A 31 years old male, light smoker, presented at the Emergency Room for pain in the left thigh for about a month, without any previous event. He had 2 previous visits to the hospital with similar diffuse complaints interpreted as irradiated low back pain. He was pale, feverish but no signs of local inflamation. His left knee ROM was 30°-15°-0°. He had no neurologic deficits. Blood tests showed high WBC count and PCR (400mg/L). After contrasted CT showing an 1,2×6×2,5cm abcess the patient was taken to the OR for irrigation and debridment. The day after the patient did a MRI that showed extended femur osteomyelitis and adjacent myositis (images). He underwent new surgery for a more extensive irrigation and debridment, femur medular canal included, from where a large pus quantity erupted. The hemocultures and bone biopsys, revealed a Multissensible Strep. Alfa-hemolitic (S. anginosus) and appropriate antibiotherapy was implemented (Amoxicillin/Clavulanate). He slowed improve till 11th day when he showed rise of PCR and a new MRI showed the same inflammatory process. He underwent new surgery but no pus was visible. He gradually improved, started hyperbaric oxygen therapy and was discharged on the 28th day after the first surgery, continuing antibiotherapy at home for a total of 8 weeks. Transthoracic Ecography was normal, as well Brucella, HIV, and other serologies. Three weeks later, the patient again presented to the ER with fever and rise of inflammatory markers in blood, consistent with recurrence of the infection. The patient was taken to the OR for surgical debdridment and irrigation, but this time the cultures showed a multissensible Gemella haemolysans, possible contamination during the hyperbaric oxygen therapy sessions. This is the case of a healthy young adult with an idiopathic femur osteomyelitis, initially misinterpreted for irradiated low back pain, that recurred after the first medical and surgical successful treatment, with an unusual agent

Hypothesis. Lumbar spinal stenosis (LSS) is diagnosed by a history of claudication, clinical investigation, cross-sectional area (CSA) of the dural sac on MRI or CT, and walking distance on the treadmill test. As radiological findings do not always correlate with clinical symptoms, additional diagnostic signs are needed. In patients without LSS, we observe the sedimentation of lumbar nerve roots to the dorsal part of the dural sac on supine MRI scans. In patients with LSS, this sedimentation is rarely seen. We named this phenomenon ‘sedimentation sign’ and defined the absence of sedimenting nerve roots as positive sedimentation sign for the diagnosis of LSS. We hypothesised that the new sedimentation sign discriminates between non-specific low back pain (LBP) and LSS. Methods and analysis. This prospective case-control study included 200 patients in an orthopaedic in- and outpatient clinic. Patients in the LBP group (n=100) had low back pain, a cross-sectional area (CSA) of the dural sac >120mm. 2. , and a walking distance >1000m; patients in the LSS group (n=100) showed claudication, a CSA < 80mm. 2. , and a walking distance < 200m. Results. A positive sedimentation sign was identified in 94 patients in the LSS group but in no patient in the LBP group. There was no difference in the detection of the sign between segmental levels L1 - L5 in the LSS group. Conclusion. Our findings show that a positive sedimentation sign exclusively and reliably occurs in patients with LSS, suggesting its usefulness in clinical practice. If future accuracy studies confirm the sign's high specificity, a positive sedimentation sign can rule in LSS, and with a high sensitivity, a negative sedimentation sign can rule out LSS. The sedimentation sign is potentially a valuable tool to identify patients who will benefit from spinal surgery

There is conflicting information regarding the recommendations of bracing, physical therapy and cessation of sports for young athletes with symptomatic spondylolysis. The purpose of this study was to identify factors affecting the prognosis and to find the optimal method of non-operative treatment. The patients in our study were athletes who visited our children's hospital for low back pain with lumbar spondylolysis and were treated non-operatively from 1990 to 2002. Clinical and radiological outcomes were reviewed retrospectively. The effects of bracing, physical therapy, cessation of sports, duration of symptoms before the first hospital visit, lateralisation of spondylolysis, age, gender, onset of low back pain after lumbar trauma during sports, bone scan uptake, vertebral level of the lesion, associated scoliosis or spina bifida and radiological bony healing were analysed using univariate and multivariate analysis with logistic regression. The mean age of patients was 13 years (range 7 to 18 years). The mean follow-up was 4.2 years (range 1.2 to 12 years). Of 132 patients, 48 patients had excellent results with no pain during sports, 76 good, 6 fair, and 4 poor. Cessation of sports, early non-operative intervention, and a unilateral spondylolysis appeared to be factors associated with excellent outcomes. However, bracing, physical therapy, age, gender, level of lesion, history of trauma, increased uptake on bone scan, or associated scoliosis or spina bifida were not factors. Bony healing was not related to the clinical outcome. The non-operative treatment of spondylolysis in children can yield excellent clinical outcomes, and the absence of bony healing has no influence on clinical outcome. Factors in this study found to correlate with an excellent outcome include unilateral spondylolysis, acute spondylolysis, and treatment with cessation of sports for 12 weeks

Intervertebral disc (IVD) degeneration plays a major role in low back pain which is the leading cause of disability. Current treatments in severe cases require surgical intervention often leading to adjacent segment degeneration. Injectable hydrogels have received much attention in recent years as scaffolds for seeding cells to replenish disc cellularity and restore disc properties and function. However, they generally present poor mechanical properties. In this study, we investigated several novel thermosensitive chitosan hydrogels for their ability to mimic the mechanical properties of the nucleus pulposus (NP) while being able to sustain the viability of NP cells, and retain proteoglycans. CH hydrogels were prepared by mixing the acidic chitosan solution (2% w/v) with various combinations of three gelling agents: sodium hydrogen carbonate (SHC) and/or beta-glycerophosphate (BGP) and/or phosphate buffer (PB) (either BGP0.4M, SHC0.075M-BGP0.1M, SHC0.075M-PB0.02M or SHC0.075M-PB0.04M). The gelation speed was assessed by following rheological properties within 1h at 37°C (strain 5% and 1Hz). The mechanical properties were characterized and compared with that of human NP tissues. Elastic properties of the hydrogels were studied by evaluating the secant modulus in unconfined compression. Equilibrium modulus was also measured, using an incremental stress-relaxation test 24h after gelation in unconfined compression (5% strain at 5%/s followed by 5min relaxation, five steps). Cells from bovine IVD were encapsulated in CH-based gels and maintained in culture for 14 days. Cytocompatibility was assessed by measuring cell viability, metabolism and DNA content. Glycosaminoglycan (GAG) synthesis (retained in the gel and released) was determined using DMMB assay. Finally injectability was tested using human cadaveric discs. Unconfined compression confirmed drastically enhanced mechanical properties compared to conventional CH-BGP hydrogels (secant Young modulus of 105 kPa for SHC0.075PB0.02 versus 3–6 kPa for BGP0.04). More importantly, SHC0.075PB0.02 and SHC0.075BGP0.1 hydrogels exhibited mechanical properties very similar to NP tissue. For instance, equilibrium modulus was 5.2±0.6 KPa for SHC0.075PB0.02 and 8±0.8 KPa for SHC0.075BGP0.1 compared to 6.1±1.7 KPa for human NP tissue. Rheological properties and gelation time (G′=G″ after less than 15 s at 37°C, and rapid increase of G') of these hydrogels also appear to be adapted to this application. Cell survival was greater than 80% in SHC0.075BGP0.1 and SHC0.075PB0.02 hydrogels. Cells encapsulated in the new formulations also showed significantly higher metabolic activity and DNA content after 14 days of incubation compared to cells encapsulated in BGP0.4 hydrogel. Cells encapsulated in SHC0.075BGP0.1 and SHC0.075PB0.02 produced significantly higher amounts of glycosaminoglycans (GAG) compared to cells encapsulated in SHC0.075PB0.04 and BGP0.4 hydrogels. The total amount of GAG was higher in SHC0.075BGP0.1 hydrogel compared to SHC0.075PB0.02. Interestingly, both the SHC0.075BGP0.1 and SHC0.075PB0.02 hydrogels retained similar amounts of GAG. Injectability through a 25G syringe, filling of nuclear clefts and good retention in human degenerated discs was demonstrated for SHC0.075PB0.02 hydrogel. SHC0.075BGP0.1 appears to be a particularly promising injectable scaffold for IVD repair by providing suitable structural environment for cell survival, ECM production and mechanical properties very similar to that of NP tissue

Overall, hip and knee total joint replacement (TJR) patients experience marked benefit, with reported satisfaction rates of greater than 80% with regard to pain relief and improved function. However, many patients experience ‘nuisance’ symptoms, an annoyance which may cause discomfort, which can negatively impact postoperative satisfaction. The purpose of this study was to evaluate the prevalence of nuisance symptoms among TJR patients and impact on overall patient satisfaction. A prospective survey study to assess type and prevalence of primary hip/knee TJR related nuisance symptoms, and impact on patient satisfaction at six-months to one-year post-TJR was conducted. The survey was administered over a one-year period at one academic arthroplasty centre. Survey questions tapped occurrence of commonly reported nuisance symptoms (e.g. localized pain, swelling, stability, incision appearance/numbness, stiffness, clicking/noise, ability to perform activities of daily living), and impact of the symptom on overall hip/knee satisfaction rated on a 10-point visual analogue scale (VAS), (0=no impact, 10=to a great extent). Overall VAS satisfaction with TJR was also assessed (0=not at all satisfied, 10=extremely satisfied). Survey responses were analysed using descriptive statistics. The sample comprised of 974 primary TJR patients, including 590 knees (61%) and 384 hips 39%) who underwent surgery over a one-year period. Among knees, the most commonly reported nuisance symptoms and associated impact to satisfaction per mean VAS scores included: difficulty kneeling (78.2%, mean VAS 4.3, ±3.3), limited ability to run or jump (71.6%, VAS 3.3, ±3.3), numbness around incision (46.3%, VAS 3.8, ±3.3), clicking/noise from the knee (44.2%, VAS 2.7, ±2.7) and stiffness (43.3%, 3.3, ±2.7) following knee arthroplasty. Overall, 88.1% of knee patients surveyed experienced at least one self-reported nuisance symptom at one-year postoperative. Mean overall VAS satisfaction with knee TJR was reported as 9/10 (±1.7). Among hip TJR patients, the most commonly reported nuisance symptoms and associated impact to satisfaction per VAS scores were: limited ability to run or jump (68.6%, VAS 3.4, ±3.4), muscular pain in the thigh (44.8%, VAS 3 ±2.7), limp when walking (37.6%, VAS 4.1, ±3.2), hip stiffness (31%, VAS 3.1, ±2.4), and new or worsening low back pain (24.3%, VAS 2.9, ±2.5). Overall, 93.7% of patients experienced at least one self-reported nuisance symptom at one-year postoperative. Mean overall VAS satisfaction following total hip arthroplasty at one year was reported as 8.9/10 (±1.7). Nuisance symptoms following primary total hip and knee arthroplasty are very common. Despite the high prevalence of such symptoms, impact of individual symptoms to overall TJR satisfaction is minimal and overall TJR patient satisfaction remains high. Careful preoperative counselling regarding the prevalence of such symptoms is prudent and will help establish realistic expectations following primary hip and knee TJR
