Climbers and mountaineers will present to arthroplasty surgeons in need of hip replacement surgeries. There is a lack of guidance for both parties with a paucity in the literature. Climbing is often considered a high-risk activity to perform with a total hip replacement, due to the positions the hip is weighted in, and the potential austere environment in which an injury may occur. The aim was to assess levels of climbing and mountaineering possible following hip arthroplasty, and any factors affecting these levels. Ethical approval was obtained from the University of Central Lancashire. An anonymous online questionnaire was disseminated via email, social media and word-of-mouth to include all climbers, hill-walkers or mountaineers across the UK. This was used to collect climbing and mountaineering ability at various timepoints, along with scores such as the Oxford Hip Score and UCLA Score. The Kappa statistic was used to assess for correlations. Of the responders, 28 had undergone right hip arthroplasty surgery, with 11 having left hips and 22 receiving bilateral hips. A total of 67 of the replaced hips were total hip replacements, with 16 having undergone hip resurfacing. There is a fair agreement in level of climbing ability 3 months pre- and 3 months post-operatively (kappa=0.287, p<0.001), and a substantial agreement between 1 year post-operatively and currently for both climbing (kappa=0.730, p<0.001) and mountaineering (kappa=0.684, p<0.001). Impressively, 17 participants are climbing at more than E1 trad or 6c sport at one or more time points post operatively, which is regarded as an advanced level within the climbing community. Out of those 17 participants, 8 were climbing at this level within 3 months post-operatively. The level of climbing possible following hip arthroplasty surgery is above what is expected and perhaps desired by the operating surgeon. It is essential to take the individual patient into account when planning an operative intervention such as arthroplasty. The one year post-operative time point is highly predictive of longer term outcomes for both climbing and mountaineering

Local anatomical abnormalities vary in congenital hip disease patients. Authors often present early to mid-term total hip arthroplasty clinical outcomes using different techniques and implants randomly on patients with different types of the disease, making same conclusions difficult. We report long term outcomes (13 to 23 years) of the treatment of low and high dislocation cases (separately) with total hip arthroplasty using TM technology acetabular cups (Implex initially and then Zimmer) and short fluted conical (Zimmer) femoral stems. From 2000 to 2010, 418 congenital hip disease hip joints were treated in our department with total hip arthroplasty. According to Hartofilakidis et al's classification, 230 hips had dysplasia, 101 low dislocation, (group A) and 87 high dislocation (group B). Pre-operative and post-operative values, at regular intervals, of HHS, SF-12, WOMAC, OHS and HOOS were available for all patients. Patient, surgeon and implant related failures and complications were recorded for all patients. In all cases an attempt was made to restore hip center of rotation. In group A the average lengthening was 2.8 cm (range: 1 to 4.2) and in group B 5.7 cm (range: 4.2 to 11). In both groups, no hips were revised due to aseptic loosening of either the acetabular cup or the femoral stem. In group A, a cumulative success rate of 95.6% (95% confidence interval, 92.7% – 97.4%) and in group B a cumulative success rate of 94.8% (95% confidence interval, 92.6%–96.9%) was recorded, at 20 years, with revision for any reason as an end point. No s.s. differences were found between groups when mean values of HHS, SF-12, WOMAC and OKS were compared. Satisfactory long-term clinical outcomes can be achieved in treating different types of congenital hip disease when appropriate surgical techniques combined with “game changing” implants are used

There is an increasing demand worldwide for total hip arthroplasty in patients over 80 years old. This study is the largest of its kind reporting long term outcomes and clinical survivorship of patients over 80 years old undergoing THR. 13171 patients 80 years or older who underwent THR between 2000 and 2019 were included. Demographic and operative data was collected including age, sex, laterality, date of surgery and operative technique. Presence and date of complications were collected. Data was also collected for the same time period on 80910 patients aged 51–79 years undergoing THR for comparison. 4103 (31.2%) male and 9068 female (68.8%) patients were included in the 80year old cohort. Median age was 83 (IQR 81–83, range 80–98). 32682 (40.4%) male and 48227 (59.6%) females were included in the 50–79year old cohort. Median age was 68 (IQR 62–73, range 50–79). The 80 cohort was more likely to sustain post operative complications in the 6 months following surgery including DVT (81/13171 vs 364/80910, P<0.05), myocardial infarction (177/13171 vs 341/80910, P<0.05), acute renal failure (371/12800 vs 812/80910 P<0.05). The 50–79year old cohort was over twice as likely to undergo revision surgery than the 80 year old cohort (HR 2.55, 95% CI 2.216–2.932, p<0.001). Of those requiring revision surgery, the elderly cohort were more likely to undergo earlier revision surgery (378days, 95%CI 236–519d vs 1586days, 95%CI 1471–1700d, p<0.001). In those undergoing revision surgery, a higher proportion were done for infection in the 80 year old cohort (39/219 (17.8%) vs 215/2809 (7.7%), p<0.05. This study demonstrates good outcomes in terms of medical complications and a low overall risk of requiring revision surgery in patients 80years old undergoing THR. Patients over the age of 80 should be counselled on the relatively increased risk of medical complications post operatively

Introduction. Femoroacetabular impingement(FAI) is a relatively common cause causes of hip pain and dysfunction in active young adults. The concept of FAI was popularized by Ganz et al in early 2000s. Surgical treatment for FAI has been widely employed over the last two decades. The long term outcome of femoroacetabular osteoplasty (FAO) and risk factors for long-term failure of FAO is less studied. The goal of this single surgeon series is to identify the long term outcome of FAO (minimum 10-year follow-up) and risk factors for surgical treatment failure in these patients. Methods. The prospective database on hip joint preservation identified 1,120 patients who have undergone FAO between January 2005-June 2019. Of these 164 patients (178 hips) have a minimum 10-year follow-up (range, 10–14). The database collects detailed information on patient demographics, clinical history, radiographic and cross sectional imaging findings, intraoperative findings (site and size of chondral lesion, labral tear, subchondral cyst, size of cam lesion, etc), and patient outcome using the modified HHS, SF-36, and UCLA activity score. Patients are contacted on an annual basis and the functional outcome instruments administered. Of 1,120 patients, 122 patients have been lost to follow-up. Results. The mean age of patients in the cohort is 34.3±10.4 years and 39.6% of patients are female. FAO resulted in an improvement in mean mHHS (58.2±3.9 to 86.4±3.2) and SF-36(60.4±4 to 85±4.1) in 89% of patients. At the latest follow up 8.1% of patients have undergone THA. The study found that older age, longer preoperative symptomatic period, higher preoperative alpha angle, presence of hip dysplasia and acetabular retroversion, inability to repair acetabular labrum, and full thickness acetabular chondral lesion were associated with higher risk of failure of treatment. Conclusion. Patients with symptomatic FAI who undergo surgery experience pain relief and functional improvement that appears to endure over a decade in the majority. This study on a large cohort with long term follow-up has also identified patients who are at higher risk of failure. The study has also shed light on some interesting findings such as labral tear being very common in patients without hip symptoms and others that will be presented. Background. The use of the direct anterior approach (DAA) for total hip arthroplasty (THA) has increased in recent years. This is in part due to the proposed benefits of a faster early recovery and a lower risk of dislocation. The purpose of this study is to understand the dislocation rate in a non-selective, consecutive cohort of patients undergoing THA via the DAA including those at high risk for instability due to spinopelvic pathology. Methods. We performed a retrospective review of a large prospectively collected single institution database assessing all patients undergoing THA via the DAA between 2011 and 2017. The primary outcome measure was dislocation at minimum two-year follow-up. We then stratified patients by known risk factors for dislocation including spinopelvic pathology and performed an in-depth analysis of those patients who had a dislocation event. Results. 2,831 hips in 2205 patients were included in the study. Mean age was 64.9 (24–96) and 1,595 (56.3%) were female. Mean BMI was 29.2 (15.1–53.8). There were 11 dislocations within one year of the index operation (0.38%) and 13 total dislocations at terminal follow-up (0.45%). Five dislocations required revision (38.4% of dislocations 0.17% overall). When stratified by experience the dislocation rate for surgeons who had completed their learning curve was 0.15% compared to 1.11% in those who hadn't. There were 666 patients with an established diagnosis of spinopelvic pathology or prior surgical instrumentation, only 2 (0.30%) dislocated and neither required revision. Conclusion. In a non-selective, consecutive cohort of patients undergoing THA via the DAA the risk of dislocation is low. Even amongst patients with lumbosacral stiffness secondary to spinal instrumentation or degenerative changes, the rate of dislocation is low following THA via the DAA. Our data suggests that utilizing the DAA in high risk patients may be protective against dislocation without the need for additional constraint or the use of newer bearing constructs that lack long term outcome studies. The inclusion of seven surgeons suggests that these results are generalizable

Introduction. Total Hip Arthroplasty has been shown to have excellent long term outcomes, yet early reoperation remains a risk. The current study assesses the incidence, causes, and cost associated with early revision following elective primary THA in the US Medicare population. Methods. The study used the Limited Data Set (LDS) from the Centers for Medicate and Medicaid Services (CMS) to identify all primary THA (DRG 469/470) performed in the US (excluding Maryland) during 2016. All cases were followed for one year after the original date of operation to create a database of readmissions after surgery. These data allowed for the determination of the 1-year incidence and type of reoperation, the timing of reoperation, the admitting diagnosis, hospital utilization, and total cost. Results. There were 164,050 THA performed on Medicare patients in the US in 2016. After surgery, there were 24,728 associated readmissions during the 1-year follow-up, or a readmission rate of 15.1%. There were 5,646 readmissions with a procedure performed on the hip; this represents 23% of the total 1-year readmissions after surgery, or a reoperation rate of 3.4%. Admission for reoperation was the most common reason for readmission following elective primary THA. When looking at reoperations, fracture was the most common (41.69%). 45% of reoperations occur within the first 30 days, 74% of reoperations occur within the first 90 days, and 26% of reoperations occur between days 90 and 360. For reoperations, a primary diagnosis of infection was associated with the highest average total cost ($104,024). Conclusions. Revision after THA is both a frequent occurance and expensive one. Fractures are the most common diagnoses that lead to revision, followed by dislocation. Increased focus to reduce complications such as fracture and dislocation would have the most beneficial impact on the patient, as well as on the healthcare system. Introduction. Rationing of orthopaedic services is increasingly being used by Care Commissioning Groups (CCG) within the United Kingdom to restrict the numbers of patients being referred for Total Hip Arthroplasty (THA). In Devon, only patients with an Oxford Hip Score (OHS) less than 20 are referred on for specialist Orthopaedic Review. The aim of this study was to look at long term outcomes after THA to see if this rationing has any rational base to justify its use. Methods. Consecutive patients undergoing THA in Exeter between 1996 and 2012 had OHS' collected prospectively pre-operatively and a minimum of 4 years post-operatively. These scores were analysed looking for trends in patient related outcome scores. Results. 2341 patients had an OHS at a minimum of 4 years' post-op (mean 4.97 years, SD 0.33, range 4.0–5.5). This accounted for just under 50% of patients operated on in this period. Average improvement in OHS post THA was 19.6 points (range 0–44). 45.7% of patients undergoing THA had a pre-operative OHS of greater than the CCG threshold of 20. Patients did have a significantly better increase in post-operative OHS when their starting score was less than 20 but patients above this threshold still had a significant benefit from THA. However, patients with a pre-operative OHS less than 32 seemed to have more benefit from THA compared to those with a pre-op OHS of greater than 32. Interesting 7.8% of patients did not achieve the mean detectable change of 5 points in OHS at a minimum of 4 years' post THA. Conclusion. Rationing has been introduced in many CCG's around the country. Little of the rationing decisions have their basis in scientific fact and reasoning. Our data would suggest that 92.2% of patients undergoing THA have excellent outcomes. If rationing were to be fully patient centred all patients with an OHS less than 32 should be considered for THA

Introduction. Open hip debridement surgery has been used for treatment of femoral acetabular impingement pain for over ten years in our unit. While literature has reported promising short-term outcomes, longer term outcomes are more sparsely reported. Patients/Materials & Methods. Patients who had undergone this surgery were identified on our database. Electronic, radiographic and paper records were reviewed. Demographic data, radiological and operative findings were recorded. Patients underwent ten-year review with standardised AP hip radiographs, questionnaire, non-arthritic hip (NAHS), Oxford hip (OHS) and SF-12 scores. Results. A total of 1626 operations were identified of which 183 had reached the tenth anniversary of their surgery. There were 91 females and 92 males. The average age was 39. Kaplan-Meier analysis indicated an overall 78% survival at ten years. However, once the learning curve was accounted for, by excluding the first year's cohort of patients, the survival rate was 84% at 10 years. Those who underwent hip arthroplasty were older at initial surgery and had a higher Tonnis grade than those who survived. The majority failed within the first 2 years. Labral repair, restoration of a normal centre edge angle and adequate CAM correction were associated with better survival. In those whose hip survived, the average NAHS and OHS were 87 and 39 respectively. 15% required further non-arthroplasty surgery (scope/revision) and 51% underwent trochanteric screw removal. 89% of survivors deemed their surgery to have been worthwhile at ten year follow up. Discussion. These results represent the first cohorts of our patients to reach ten year follow up. The results represent a learning curve in patient selection. It is unsurprising that those with more arthritic change were more likely to fail. Conclusion. In patients with clearly defined CAM impingement without joint space narrowing and where successful resection of the CAM and restoration of a normal centre edge angle can be achieved open hip debridement remains an operation with worthwhile outcomes at ten years

This study utilized data from the NJR dataset on all Corail/Pinnacle total hip replacements (THR) to determine (a) the level of unit variation of the Corail/Pinnacle 36mm Metal On Metal THR within England and Wales; (b) patient, implant and surgeon factors that may be associated with higher revision rates; (c) Account for the influence of the MHRA announcement in 2010. The national Revision Rate (RR) for the Corail / Pinnacle MOM THR was 10.77% (OR:1.46; CI:1.17–1.81). This was significantly greater than other articulation combinations (MOP 1.72%, COP 1.36%, COC 2.19%). The 2010 MHRA announcement did not increase rate of revision (X. 2. =1649.63, df=13, p<.001). Patient factors associated with significantly increased revision rates included, female gender (OR 1.38 (CI 1.17–1.63, p<.001) and younger age OR 0.99 (CI 0.98–0.99), p<.001). Implant factor analysis demonstrated an inverse relationship between cup size and revision. As head length increased RR increased – highest risk of revision +12.5 (OR 1.69 (CI 1.12–2.55), p=0.13). Coxa vara, high offset stems had a higher risk of revision compared to standard offset stems (OR:1.41 (CI 1.15–1.74; p<.001). As stem size increased risk of revision decreased (OR 0.89 (CI 0.85–0.93); p<.001). Surgeon grade did not influence RR. There was significant variation in RR between hospitals with 7 units (7/61 excluding low volume centres, <50 implants) identified as having significant higher rates of revision. However, for each of these units there was a greater proportion of higher risk patients (female, cup size 50–54, stem type). This study has provided insight into unit variation, risk factors and the long term outcome of the Corail/Pinnacle 36mm MOMTHR. Future aims are to use these results to develop a risk stratified algorithm for the long term follow of these patients to minimize patient inconvenience and excess use of limited NHS resources

Introduction. The Exeter Hip femoral component remains largely unchanged from the original design, introduced in 1970. It is a highly polished, modular, double tapered stem and has undergone various minor modifications to surface, modularity and most recently the taper; changed to the current V40. TM. design in 2000. The effect of any design modification cannot easily be foreseen and greater emphasis is now placed on ensuring appropriate monitoring for such implants. Methods. We present the results of the first 540 V40. TM. Exeter THAs performed in our Centre between December 2000 and May 2002. All patients were reviewed prospectively at 1, 5 and 10 years following surgery. Results. Ten years after surgery there are no revisions for aseptic loosening and no evidence of aseptic loosening in any hip. The fate of every implant is known and all patients remain under review. One hundred and forty-five patients (26.9%) died before 10 years and of the remaining 395 stems, 374 (94.7%) remain in situ, 11 were revised (2.7%). Three were revised for infection, one for recurrent dislocation and one implant failed at the neck of the prosthesis. Ten were exchanged using cement-in-cement techniques to aid acetabular revision. There were ten periprosthetic fractures: six requiring stem revision, two underwent ORIF and two had already healed by the time the patients presented for follow-up. There was no radiographic evidence of failure preceding the periprosthetic fractures. All radiographs and pain scores were analysed as part of long-term follow-up, and are discussed. Discussion. There were no revisions for aseptic loosening. Previous studies from this centre reviewing the Original and Universal series have demonstrated the stem to provide a predictable and reproducible long term outcome with 100% survival well beyond10 years for aseptic loosening. Conclusion. The polished V40. TM. Exeter stem continues to perform well and survival has remained comparable to the Universal series

All patients underwent ten year review with a standardised AP hips radiograph, questionnaire, non-arthritic hip score (NAHS), oxford hip score (OHS) and SF12 score. Results. A total of 1626 operations were identified of which 37 had reached the tenth anniversary of their surgery. There were 14 females and 22 males. The average age was 41. 10 (45%) had undergone subsequent hip arthroplasty. Those who underwent hip arthroplasty were older at initial surgery than those who survived. However, this was not statistically significant (p= 0.127). Those who failed had a higher Tonnis and UCLA grade at time of surgery. The majority failed within the first 2 years. In those whose hip survived, the average NAHS was 87. 100% of survivors deemed there surgery to have been worthwhile. Discussion. These results represent the first cohort of our patients to reach ten year follow up. The results represent a learning curve in patient selection. It is unsurprising that those with more arthritic change were more likely to fail. Conclusion. In patients with clearly defined CAM impingement without joint space narrowing, open hip debridement remains an operation with worthwhile outcomes at ten years

Aims

The aim of this study was to evaluate the accuracy of implant placement when using robotic assistance during total hip arthroplasty (THA).

Patients and Methods

A total of 20 patients underwent a planned THA using preoperative CT scans and robotic-assisted software. There were nine men and 11 women (n = 20 hips) with a mean age of 60.8 years (sd 6.0). Pelvic and femoral bone models were constructed by segmenting both preoperative and postoperative CT scan images. The preoperative anatomical landmarks using the robotic-assisted system were matched to the postoperative 3D reconstructions of the pelvis. Acetabular and femoral component positions as measured intraoperatively and postoperatively were evaluated and compared.

Aims

We report on the outcome of the Synergy cementless femoral stem with a minimum follow-up of 15 years (15 to 17).

Patients and Methods

A retrospective review was undertaken of a consecutive series of 112 routine primary cementless total hip arthroplasties (THAs) in 102 patients (112 hips). There were 60 female and 42 male patients with a mean age of 61 years (18 to 82) at the time of surgery. A total of 78 hips in the 69 patients remain in situ; nine hips in eight patients died before 15 years, and 16 hips in 16 patients were revised. Clinical outcome scores and radiographs were available for 94 hips in 85 patients.
