Methods. This is a retrospective population-based cohort study using prospectively collected patient-level data between 1 January 2013 and 1 March 2020. The diagnosis of deep SSI was defined as per the Centers for Disease Control/National Healthcare Safety Network criteria. The Mann-Kendall Trend test was used to detect monotonic trends in annual rates of early-onset deep SSI over time. Multiple logistic regression was used to analyze the effect of different patient, surgical, and healthcare setting factors on the risk of developing a deep SSI within 90 days from surgery for patients with complete data. primary anterior cruciate ligament (ACL) reconstruction. A total
of 48 patients had undergone bilateral ACL reconstruction and
42 were available for review. These patients were matched to a unilateral
ACL reconstruction control group for gender, age, sport of primary
injury, meniscal status and graft type. At 15-year follow-up a telephone
interview with patients in both groups was performed. The incidence
of further ACL injury was identified through structured questions
and the two groups were compared for the variables of graft rupture
or further ACL injury, family history of ACL injury, International
Knee Documentation Committee (IKDC) subjective score and activity
level. There were 28 male and 14 female patients with a mean age of

Introduction. Overwhelming evidence has established obesity as a risk factor for osteoarthritis (OA) of the knee. Randomized clinical trials such as the Look AHEAD study have shown long term successful intentional weight loss with an intensive lifestyle intervention (ILI) in overweight and obese type 2 diabetics. Weight loss can also decrease knee pain in persons who have OA, but it is unknown if intentional weight loss can reduce the risk of TKR. To answer this question, data from the Look AHEAD study were examined to determine if intentional weight loss could reduce the risk of TKR. Methods. Look AHEAD is a multicenter, randomized trial which began in August 2001 and follow-up continued for a median of 11.3 years at 16 academic centers. 5145 persons aged 45–76 with diabetes were randomized to either an ILI with reduced calorie consumption and increased physical activity designed or to diabetes support and education intervention (DSE). TKR events were ascertained every 6 months. Retrospective data of reported knee pain was assessed using the WOMAC knee pain questionnaire. Participants with partial TKR or revisions were excluded. Cox proportional hazard models were used to relate baseline BMI category (obese, Class I, Class II, or Class III obesity), baseline knee pain, and treatment group with TKR. Weight change category (lost<=5%, stable, gained>=2%) from baseline to year 1 follow-up by treatment assignment was also examined as a predictor of TKR after excluding TKR occurring prior to year 1. Results. Out of the randomized participants, 2171 reported knee pain (43%) at the baseline visit (p=0.81). WOMAC knee pain score did not differ by random assignment (ILI:3.6±2.9, DSE:3.9±3.0, p=0.08). During follow up there were 631 TKRs reported by participants. TKR was more common in heavier (p<0.001), and older (p<0.001) participants and did not differ by randomization. Heterogeneity of treatment effect was observed with baseline knee pain (interaction p = 0.02), therefore analyses were stratified by presence or absence of knee pain at baseline. In persons without knee pain at baseline, there was a 29% reduced rate of TKR in the ILI group compared to the DSE group (HR[95%CI] 0.71[0.52,0.96]; Figure 1A). Whereas in persons with knee pain at baseline, there was a trend for the ILI to have an increased rate of TKR compared to DSE (1.11[0.92, 1.33]; Figure 1B). The purpose of this study was to assess the overall clinical and radiographic outcomes of unicompartmental knee arthroplasty (UKA) in the 2–10 year postoperative period. The secondary goal was to compare outcomes between fixed- (FB) and mobile-bearing (MB) implant designs. We performed a retrospective analysis of 237 consecutive primary medial UKAs from a single academic center. All cases were performed by high-volume fellowship-trained arthroplasty surgeons, though UKA comprised <10% of their overall knee arthroplasty practice (<20 medial UKAs per surgeon per year). Clinical outcomes included the Oxford Knee Scores (OKS) and revision rates. Femoral and tibial coronal and sagittal angles (FCA, FSA, TCA, TSA) were radiographically measured. FCA (>±10º deviation from the neutral axis), FSA (>15º flexion), TCA (>±5º deviation from the neutral axis), and TSA (>±5º deviation from 7º) outliers were defined. Pulmonary embolism (PE) complicates up to 1% of total joint arthroplasties (TJA). Many PE treatment guidelines call for immediate initiation of therapeutic anticoagulation. Options include Xa inhibitors, Enoxaparin, and Warfarin. Deciding between these is a balance of the efficacy and the risks. Little data exists regarding the risks of each of these treatment options for treating PE in arthroplasty patients. We examined the records of 29,270 patients who underwent a primary total joint arthroplasty (TJA), defined as a unilateral total knee arthroplasty (TKA) (18,987) or total hip arthroplasty (THA) (10,283), between 2/2016 and 12/2018 at our institution and identified 338 (242 TKA, 96 THA) patients who developed an in-hospital PE treated with therapeutic anticoagulation. The patients were treated with therapeutic doses of Xa inhibitors, enoxaparin or warfarin. The type and frequency of complications were determined and classified as major or minor. Major complication included: bleeding requiring surgery, GI bleed requiring treatment, >2 unit transfusion and mortality. Minor complications included wound drainage, bleeding not requiring surgery, and thrombocytopenia.Introduction

We have evaluated prospectively the
AIM. We aim to identify whether meniscal repair at the time of ACL reconstruction was associated with a better outcome than meniscectomy. Method. We prospectively collected data on 233 consecutive patients undergoing ACL reconstruction in our unit. A four strand autologous hamstring graft was used with suspensory femoral fixation, and a tibial interference screw. At surgery the presence and location of chondral and meniscal injuries was noted, and whether the meniscal lesion was resected or repaired. Patients were reviewed and scored by a specialist physiotherapist practitioner pre-operatively, and at 6, 12, and 24 months. Tegner, Lysholm, and Cincinatti knee scores were used. Results. At surgey 58% of patients had a meniscal injury, of which 51% were medial and 36% were lateral. 13% had injury to both medial and lateral menisci. 26% of these were repaired. 23% of patients had an associated chondral injury of which 55% involved the medial femoral condyle. The presence of a meniscal injury did not correlate with a worse outcome (Tegner, Lysholm and Cincinatti). There was no difference in outcome whether a meniscal tear was repaired or resected. Lateral meniscus injuries were associated with a worse outcome at 2 years than medial meniscus injuries (Lysholm p= 0.013). Chondral injury was associated with a worse outcome at 2 years. Two weeks before beginning basic training lasting for 14 weeks, 473 infantry recruits were enrolled into the study and underwent two-dimensional measurement of their subtalar joint displacement angle during walking on a treadmill. approximately 0.4% per year over the first 27 years. The most common
cause for revision surgery is related to polyethylene insert failure
and accounts for approximately 50% of re-operations. Late metastatic
infection is the next most frequent cause followed by patellar problems,
late instability and component loosening in decreasing frequency.

We have examined the results obtained with 72 NexGen legacy posterior stabilised-flex fixed total knee replacements in 47 patients implanted by a single surgeon between March 2003 and September 2004. Aseptic loosening of the femoral component was found in 27 (38%) of the replacements at a mean follow-up of 32 months (30 to 48) and 15 knees (21%) required revision at a mean of 23 months (11 to 45). We compared the radiologically-loose and revised knees with those which had remained well-fixed to identify the factors which had contributed to this high rate of aseptic loosening. Aims. This multicentre retrospective observational study’s aims were to investigate whether there are differences in the occurrence of radiolucent lines (RLLs) following total knee arthroplasty (TKA) between the conventional Attune baseplate and its successor, the novel Attune S+, independent from other potentially influencing factors; and whether tibial baseplate design and presence of RLLs are associated with differing risk of revision. Methods. A total of 780 patients (39% male; median age 70.7 years (IQR 62.0 to 77.2)) underwent cemented TKA using the Attune Knee System) at five centres, and with the latest radiograph available for the evaluation of RLL at between six and 36 months from surgery. Univariate and multivariate logistic regression models were performed to assess associations between patient and implant-associated factors on the presence of tibial and femoral RLLs. Differences in revision risk depending on RLLs and tibial baseplate design were investigated with the log-rank test. Results. The conventional and novel Attune baseplates were used in 349 (45%) and 431 (55%) patients, respectively. At a median follow-up of 14 months (IQR 11 to 25), RLLs were present in 29% (n = 228/777) and 15% (n = 116/776) of the tibial and femoral components, respectively, and were more common in the conventional compared to the novel baseplate.
