Total joint arthroplasty (TJA) is one of the commonest and most successful orthopaedic procedures used for the management of end-stage arthritis. With the recent introduction of robotic-assisted joint replacement, Computed tomography (CT) has become part of required pre-operative planning.

The aim of this study is to quantify and characterise incidental CT findings, their clinical significance, and their effect on the planned joint arthroplasty.

All consecutive patients undergoing an elective TJA (total joint arthroplasty; hip or knee) were retrospectively identified, over a 4-year period (December 2019 and November 2023). Data documented and analysed included patient demographics, type of joint arthroplasty, CT findings, their clinical significance, as well as potential delays to the planned arthroplasty because of these findings and subsequent further investigation.

A total of 987 patients (female: 514 patients (52.1%)) undergoing TJA were identified (THA: 444 patients (45.0%); TKA: 400 patients (40.5%); UKA: 143 patients (14.5%)). Incidental findings within imaged areas were identified in 227 patients (23.0%). Of these findings, 74 (7.5%) were significant, requiring further investigation or management, 40 (4.1%) of which represented potential malignancy and 4 (0.4%) resulting in a new cancer diagnosis. A single patient was found to have an aneurysm requiring urgent vascular intervention. Surgery was delayed for further investigation in 4 patients (0.4%). Significant findings were more frequent in THA patients (THA: 43 (9.7%) TKA/UKA: 31 (5.7%)

Within our cohort, 74 (7.5%) patients had significant incidental findings that required further investigations or management, with 4 (0.4%) having a previously undiagnosed malignancy. We strongly advocate that all robotic arthroplasty planning CTs are reviewed and reported by a specialist, to avoid missing undiagnosed malignancies and other significant diagnoses.

Pelvic osteotomies for hip dysplasia results can be variable and depend on the amount of preexisting arthritis. Delayed gadolinium-enhanced magnetic resonance imaging of cartilage (dGEMRIC) is a technique designed to measure early arthritis, and could be used to select hips that would benefit from a joint-preserving reconstructive procedure. Our objective was to investigate the role of preoperative dGEMRIC in predicting the success of PAO in patients 40 and above. We hypothesized that patients who failed had lower preoperative dGEMRIC index compared to those who did not.

Following IRB approval, patients 40 or older who underwent PAO between 1990–2013 and had preoperative dGEMRIC scan and minimum follow-up of 4 years were identified. Patients with prior hip surgeries or any pathologies were removed leading to a total of 70 patients (Age: 44.2 ± 2.9 years old, BMI: 25.7 ± 4.5 Kg/m2). We only included the first hip undergoing PAO for those with bilateral PAO. Out of 70, 19 had failure defined by the need for total hip replacement or WOMAC pain score of 10 and above within 10 years after index PAO surgery. Articular cartilage was segmented on the 3D pre-operative dGEMRIC scan. The average thickness and dGEMRIC index across the whole articular surface were analyzed.

Failed hips had a lower dGEMRIC index by 115 ± 20 ms (P<0.001). All but one failed hips had a dGMERIC index of 400 or less (range: 313 – 479 ms), while all survived hips had a dGMERIC index of greater than 400 (range: 403 – 691 ms). Similar trends were observed when comparing the dGEMRIC index within the 6 subgroups (P<0.01). There were no differences in cartilage thickness (combined femoral head and acetabular cartilage) between the failed and survived hips (p>0.2).

Patients with a high dGMERIC index (indicating high GAG content) may have a higher chance of successful outcomes following PAO. Current efforts are underway to develop a multi-modal predictive model to evaluate risk of failure after PAO.

Aims

Morphological abnormalities are present in patients with developmental dysplasia of the hip (DDH). We studied and compared the pelvic anatomy and morphology between the affected hemipelvis with the unaffected side in patients with unilateral Crowe type IV DDH using 3D imaging and analysis.

Methods

A total of 20 patients with unilateral Crowe-IV DDH were included in the study. The contralateral side was considered normal in all patients. A coordinate system based on the sacral base (SB) in a reconstructed pelvic model was established. The pelvic orientations (tilt, rotation, and obliquity) of the affected side were assessed by establishing a virtual anterior pelvic plane (APP). The bilateral coordinates of the anterior superior iliac spine (ASIS) and the centres of hip rotation were established, and parameters concerning size and volume were compared for both sides of the pelvis.

Background

Isolated fracture of the greater trochanter is an uncommon presentation of hip fracture. Traditional teaching has been to manage these injuries nonoperatively, but modern imaging techniques have made it possible to detect occult intertrochanteric extension of the fracture in up to 90% of cases.

This study aims to review the investigation and management of greater trochanter fractures in a single major trauma centre.

Methods

A retrospective review was completed of patients admitted with greater trochanter fractures. These were matched to cases with 2-part extracapsular fractures. Initial management and clinical outcome was established using electronic notes and radiographs. Mortality and length of stay was calculated for both groups.

Acetabular dysplasia (AD) can cause hip pain and early osteoarthritis. Lateral Centre Edge Angle (LCEA) and sourcil angle (AI) are plain radiographic measures of acetabular morphology, however there is little agreement as to what constitutes mild, moderate or severe dysplasia. This study aims to establish the correlation, if any, between two-dimensional (XR) and three-dimensional (CT) measurements of acetabular morphology and to establish the level of femoral head cover (CTFHC) for different levels of dysplasia.

Methods

Governance board approved retrospective study. 353 PAOs performed by the senior author between January 2014 and December 2017 were included. Exclusion criteria were inadequate pre-operative CT imaging and/or plain radiographs, previous pelvic/hip surgery, acetabular retroversion, or femoral head asphericity. Of the remainder, 84 had 3D analysis by clinical graphics giving measurements for CTFHC, LCEA at 1100, 1200, 1300 and sourcil angle (AI). XRLCEA, AI, posterior wall index (PWI), and anterior wall index (AWI), were measured from supine AP pelvis radiographs. Pearson correlation coefficient, and mean CTFHC for stratified LCEAs were calculated. A linear regression model to predict CTFHC from XRLCEA was validated against these.

Results

XRLCEA correlated very strongly with total femoral head coverage (Pearson=0.917, p<0.001). Mean CTFHC with XRLCEA between 15°-19.9° was 55% (range 51–59%). At 25° −29.9° mean CTFHC was 61%.

There was a linear relation of CTFHC with XR LCEA such that CTFHC = 41.5 + 0.78(XRLCEA). This linear regression model predicted CTFHC 55% (95%CI 54–56%) for XRLCEA of 17.5°, and CTFHC 63% (95%CI 62–64%) for XRLCEA 27.5°.

The femoral head receives its blood supply primarily from the medial femoral circumflex artery, with its deep branch being the most important.

In a previous study, we performed classical anatomical dissections of 16 hips. We have extended our investigation with a radiological study, in which we aimed to visualise the arteries supplying the femoral head in healthy individuals. We analysed 55 CT angiographic images of the hip.

Using 64-row CT angiography, we identified three main arteries supplying the femoral head: the deep branch of the medial femoral circumflex artery and the posterior inferior nutrient artery originating from the medial femoral circumflex artery, and the piriformis branch of the inferior gluteal artery. CT angiography is a good method for visualisation of the arteries supplying the femoral head. The current radiological studies will provide information for further investigation of vascularity after traumatic dislocation of the hip, using CT angiography.

The cam-type deformity in femoroacetabular impingement is a 3D deformity. Single measurements using radiographs, CT or MRI may not provide a true estimate of the magnitude of the deformity. We performed an analysis of the size and location of measurements of the alpha angle (α°) using a CT technique which could be applied to the 3D reconstructions of the hip. Analysis was undertaken in 42 patients (57 hips; 24 men and 18 women; mean age 38 years (16 to 58)) who had symptoms of femoroacetabular impingement related to a cam-type abnormality. An α° of > 50° was considered a significant indicator of cam-type impingement. Measurements of the α° were made at different points around the femoral head/neck junction at intervals of 30°: starting at the nine o’clock (posterior), ten, eleven and twelve o’clock (superior), one, two and ending at three o’clock (anterior) position.

The mean maximum increased α° was 64.6° (50.8° to 86°). The two o’clock position was the most common point to find an increased α° (53 hips; 93%), followed by one o’clock (48 hips; 84%). The largest α° for each hip was found most frequently at the two o’clock position (46%), followed by the one o’clock position (39%). Generally, raised α angles extend over three segments of the clock face.

Single measurements of the α°, whether pre- or post-operative, should be viewed with caution as they may not be representative of the true size of the deformity and not define whether adequate correction has been achieved following surgery.

The gold standard treatment of hip dysplasia is a peri-acetabular osteotomy (PAO). Labral tears are seen in the majority of patients presenting with hip dysplasia and diagnosed using Magnetic Resonance Imaging (MRI). The goal was to (1) evaluate utility/value of MRI in patients undergoing hip arthroscopy at time of PAO, and (2) determine whether MRI findings of labral pathology can predict outcome. A prospective randomized controlled trial was conducted at tertiary institutions, comparing patients with hip dysplasia treated with isolated PAO versus PAO with adjunct hip arthroscopy. This study was a subgroup analysis on 74 patients allocated to PAO and adjunct hip arthroscopy (age 26±8 years; 89.2% females). All patients underwent radiographic and MRI assessment using a 1.5-Tesla with or 3-Tesla MRI without arthrography to detect labral or cartilage pathology. Clinical outcome was assessed using international Hip Outcome Tool-33 (iHOT). 74% of patients (55/74) were pre-operatively diagnosed with a labral tear on MRI. Among these, 41 underwent labral treatment (74%); whilst among those without a labral tear on MRI, 42% underwent labral treatment (8/19). MRI had a high sensitivity (84%), but a low specificity (56%) for labral pathology (p=0.053). There was no difference in pre-operative (31.3±16.0 vs. 37.3±14.9; p=0.123) and post-operative iHOT (77.7±22.2 vs. 75.2±23.5; p=0.676) between patients with and without labral pathology on MRI. Value of MRI in the diagnostic work-up of a patient with hip dysplasia is limited. MRI had a high sensitivity (84%), but low specificity (44%) to identify labral pathology in patients with hip dysplasia. Consequently, standard clinical MRI had little value as a predictor of outcome with no differences in PROM scores between patients with and without a labral tear on MRI. Treatment of labral pathology in patients with hip dysplasia remains controversial. The results of this subgroup analysis of a prospective, multi-centre RCT do not show improved outcome among patients with dysplasia treated with labral repair

Introduction. The effect of spine-pelvis position and motion on hip arthroplasty function has been increasingly appreciated in the past several years. Some authors have stressed the importance of using precision technologies for component placement while others have advocated the use of dual mobility articulations or large bearings and lateralized liners in patients with fused lumbar spines. The current study assesses the prevalence of stiff and fused spines in an elective total hip arthroplasty population. Methods. One hundred and forty-nine patients undergoing elective total hip arthroplasty were assessed preoperatively with CT (computed tomography) and functional radiographs for the purpose of CT based planning and intraoperative navigation of total hip arthroplasty (HipXpert System, Surgical Planning Associates, Inc., Boston, MA). The functional radiographs included standing and sitting lateral images (EOS Imaging, SA, Paris, France). Patients were assessed for supine, standing and sitting pelvic tilt (PT) and change in sacral slope (SS). Spine stiffness was defined by a change in sacral slope (SS) of less than or equal to 10 degrees on the standing to sitting lateral radiographs according to Luthringer et al JOA 2019. Results. Of these 149 patients, 2 (1.5%) had been previously treated by instrumented lumbar fusion. Thirty-nine additional patients (26.1%) had stiff spines as defined by a change in sacral slope of less than 10 degrees from standing to sitting. The mean supine PT measured by CT scan was 3.46 degrees of anterior PT which is similar to previously described in the literature. The mean supine PT in stiff spine patients measured 1.5 degrees of anterior tilt which was not statistically significant. The mean standing pelvic tilt measured 0.0 degrees in the all patients and −4.3 degrees in stiff spine patients. The mean sitting pelvic tilt was −18.9 degrees in the entire cohort and −11.3 degrees in the stiff spine patients. The difference in pelvic tilt between these two groups was statistically significant with p-values of 0.002 and 0.006, respectively. Discussion and Conclusion. Although the incidence of formal instrumented spine fusion was low in this cohort (1.5%), the incidence of spine stiffness was very high at 27.6%. Given that hip instability has been decreasing owing to a variety of techniques including larger bearings, intraoperative radiography, and intraoperative precision technologies, advocacy for the use of dual mobility implants simply for a history of spine fusion does not appear to be logical given that most stiff spines have not had a surgical fusion

Aims

Leg length discrepancy (LLD) is a common pre- and postoperative issue in total hip arthroplasty (THA) patients. The conventional technique for measuring LLD has historically been on a non-weightbearing anteroposterior pelvic radiograph; however, this does not capture many potential sources of LLD. The aim of this study was to determine if long-limb EOS radiology can provide a more reproducible and holistic measurement of LLD.

Methods

In all, 93 patients who underwent a THA received a standardized preoperative EOS scan, anteroposterior (AP) radiograph, and clinical LLD assessment. Overall, 13 measurements were taken along both anatomical and functional axes and measured twice by an orthopaedic fellow and surgical planning engineer to calculate intraoperator reproducibility and correlations between measurements.

Aims

Femoroacetabular impingement (FAI) patients report exacerbation of hip pain in deep flexion. However, the exact impingement location in deep flexion is unknown. The aim was to investigate impingement-free maximal flexion, impingement location, and if cam deformity causes hip impingement in flexion in FAI patients.

Methods

A retrospective study involving 24 patients (37 hips) with FAI and femoral retroversion (femoral version (FV) < 5° per Murphy method) was performed. All patients were symptomatic (mean age 28 years (SD 9)) and had anterior hip/groin pain and a positive anterior impingement test. Cam- and pincer-type subgroups were analyzed. Patients were compared to an asymptomatic control group (26 hips). All patients underwent pelvic CT scans to generate personalized CT-based 3D models and validated software for patient-specific impingement simulation (equidistant method).

Recent studies about hip stability after total hip arthroplasties (THA) concerned differences regarding bearings: ceramic on ceramic (CoC) presenting less dislocations on the long term compared to metal or ceramic on polyethylene. The hypothesis is a difference in the healing process of periarticular tissues, with a stronger fibrous tissue for the first one, and more foreign body reaction, joint effusion with the others. NMR Imaging of the pelvis showing both hips using novel MR MAVRIC program for metal artefacts suppression, were performed in 10 patients, 15 THA and 2 non-pathological contralateral hips. Eight hips had CoC bearings, 3 of which were impacted cementless bulky ceramic implant, and 5 had a metal back. 7 hips had CoP bearings, 4 of which were cemented. Native capsules showed a mean thickness of 6.6mm. For CoC bearings, capsule thickness ranged from 7mm to 9.6 mm with a mean thickness of 8mm. For CoP bearings, capsule thickness ranged from 3mm to 8.4mm, with a mean thickness of 6.1mm. Neocapsule appeared clearly in all COC bearings observed, while for CoP, sometimes it was less dense with fatty aspect, 3 hips out of 7 having a very thin capsule under 4mm. It is possible to observe and quantify new capsule after THR and measure differences although not significant regarding bearings on limited number of samples. More patients might be included, but the tendencies observed here might explain better long term stability in vivo observed with Coc

Aims

The Birmingham Hip Resurfacing (BHR) arthroplasty has been used as a surgical treatment of coxarthrosis since 1997. We present 20-year results of 234 consecutive BHRs performed in our unit.

Methods

Between 1999 and 2001, there were 217 patients: 142 males (65.4%), mean age 52 years (18 to 68) who had 234 implants (17 bilateral). They had patient-reported outcome measures collected, imaging (radiograph and ultrasound), and serum metal ion assessment. Survivorship analysis was performed using Kaplan-Meier estimates. Revision for any cause was considered as an endpoint for the analysis.

Aims

United Classification System (UCS) B2 and B3 periprosthetic fractures in total hip arthroplasties (THAs) have been commonly managed with modular tapered stems. No study has evaluated the use of monoblock fluted tapered titanium stems for this indication. This study aimed to evaluate the effects of a monoblock stems on implant survivorship, postoperative outcomes, radiological outcomes, and osseointegration following treatment of THA UCS B2 and B3 periprosthetic fractures.

Methods

A retrospective review was conducted of all patients who underwent revision THA (rTHA) for periprosthetic UCS B2 and B3 periprosthetic fracture who received a single design monoblock fluted tapered titanium stem at two large, tertiary care, academic hospitals. A total of 72 patients met inclusion and exclusion criteria (68 UCS B2, and four UCS B3 fractures). Primary outcomes of interest were radiological stem subsidence (> 5 mm), radiological osseointegration, and fracture union. Sub-analysis was also done for 46 patients with minimum one-year follow-up.

Aims

A revision for periprosthetic joint infection (PJI) in total hip arthroplasty (THA) has a major effect on the patient’s quality of life, including walking capacity. The objective of this case control study was to investigate the histological and ultrastructural changes to the gluteus medius tendon (GMED) in patients revised due to a PJI, and to compare it with revision THAs without infection performed using the same lateral approach.

Methods

A group of eight patients revised due to a PJI with a previous lateral approach was compared with a group of 21 revised THAs without infection, performed using the same approach. The primary variables of the study were the fibril diameter, as seen in transmission electron microscopy (TEM), and the total degeneration score (TDS), as seen under the light microscope. An analysis of bacteriology, classification of infection, and antibiotic treatment was also performed.

Aims

Precise implant positioning, tailored to individual spinopelvic biomechanics and phenotype, is paramount for stability in total hip arthroplasty (THA). Despite a few studies on instability prediction, there is a notable gap in research utilizing artificial intelligence (AI). The objective of our pilot study was to evaluate the feasibility of developing an AI algorithm tailored to individual spinopelvic mechanics and patient phenotype for predicting impingement.

Methods

This international, multicentre prospective cohort study across two centres encompassed 157 adults undergoing primary robotic arm-assisted THA. Impingement during specific flexion and extension stances was identified using the virtual range of motion (ROM) tool of the robotic software. The primary AI model, the Light Gradient-Boosting Machine (LGBM), used tabular data to predict impingement presence, direction (flexion or extension), and type. A secondary model integrating tabular data with plain anteroposterior pelvis radiographs was evaluated to assess for any potential enhancement in prediction accuracy.

Aims

Successful cell therapy in hip osteonecrosis (ON) may help to avoid ON progression or total hip arthroplasty (THA), but the achieved bone regeneration is unclear. The aim of this study was to evaluate amount and location of bone regeneration obtained after surgical injection of expanded autologous mesenchymal stromal cells from the bone marrow (BM-hMSCs).

Methods

A total of 20 patients with small and medium-size symptomatic stage II femoral head ON treated with 140 million BM-hMSCs through percutaneous forage in the EudraCT 2012-002010-39 clinical trial were retrospectively evaluated through preoperative and postoperative (three and 12 months) MRI. Then, 3D reconstruction of the original lesion and the observed postoperative residual damage after bone regeneration were analyzed and compared per group based on treatment efficacy.

Aims

Spinopelvic pathology increases the risk for instability following total hip arthroplasty (THA), yet few studies have evaluated how pathology varies with age or sex. The aims of this study were: 1) to report differences in spinopelvic parameters with advancing age and between the sexes; and 2) to determine variation in the prevalence of THA instability risk factors with advancing age.

Methods

A multicentre database with preoperative imaging for 15,830 THA patients was reviewed. Spinopelvic parameter measurements were made by experienced engineers, including anterior pelvic plane tilt (APPT), spinopelvic tilt (SPT), sacral slope (SS), lumbar lordosis (LL), and pelvic incidence (PI). Lumbar flexion (LF), sagittal spinal deformity, and hip user index (HUI) were calculated using parameter measurements.

Aims

The aim of this study was to evaluate the reliability and validity of a patient-specific algorithm which we developed for predicting changes in sagittal pelvic tilt after total hip arthroplasty (THA).

Methods

This retrospective study included 143 patients who underwent 171 THAs between April 2019 and October 2020 and had full-body lateral radiographs preoperatively and at one year postoperatively. We measured the pelvic incidence (PI), the sagittal vertical axis (SVA), pelvic tilt, sacral slope (SS), lumbar lordosis (LL), and thoracic kyphosis to classify patients into types A, B1, B2, B3, and C. The change of pelvic tilt was predicted according to the normal range of SVA (0 mm to 50 mm) for types A, B1, B2, and B3, and based on the absolute value of one-third of the PI-LL mismatch for type C patients. The reliability of the classification of the patients and the prediction of the change of pelvic tilt were assessed using kappa values and intraclass correlation coefficients (ICCs), respectively. Validity was assessed using the overall mean error and mean absolute error (MAE) for the prediction of the change of pelvic tilt.

In this study, we aimed to visualize the spatial distribution characteristics of femoral head necrosis using a novel measurement method.

Methods

We retrospectively collected CT imaging data of 108 hips with non-traumatic osteonecrosis of the femoral head from 76 consecutive patients (mean age 34.3 years (SD 8.1), 56.58% male (n = 43)) in two clinical centres. The femoral head was divided into 288 standard units (based on the orientation of units within the femoral head, designated as N[Superior], S[Inferior], E[Anterior], and W[Posterior]) using a new measurement system called the longitude and latitude division system (LLDS). A computer-aided design (CAD) measurement tool was also developed to visualize the measurement of the spatial location of necrotic lesions in CT images. Two orthopaedic surgeons independently performed measurements, and the results were used to draw 2D and 3D heat maps of spatial distribution of necrotic lesions in the femoral head, and for statistical analysis.
