Aims. The prevalence of scoliosis is not known in patients with idiopathic short stature, and the impact of treatment with recombinant human growth hormone on those with scoliosis remains controversial. We investigated the prevalence of scoliosis radiologically in children with idiopathic short stature, and the impact of treatment with growth hormone in a cross-sectional and retrospective cohort study. Methods. A total of 2,053 children with idiopathic short stature and 4,106 age- and sex-matched (1:2) children without short stature with available whole-spine radiographs were enrolled in the cross-sectional study. Among them, 1,056 with idiopathic short stature and 790 controls who had radiographs more than twice were recruited to assess the development and progression of scoliosis, and the need for bracing and surgery. Results. In the cross-sectional study, there was an unexpectedly higher prevalence of scoliosis (33.1% (681/2,053) vs 8.52% (350/4,106)) in children with idiopathic short stature compared with controls (odds ratio 3.722; p < 0.001), although most cases were mild. In the longitudinal study, children with idiopathic short stature had a higher risk of the development and progression of scoliosis than the controls. Among children with idiopathic short stature without scoliosis at baseline, treatment with growth hormone significantly increased the risk of developing scoliosis (p = 0.015) and the need for bracing (p < 0.001). Among those with idiopathic short stature and scoliosis at baseline, treatment with growth hormone did not increase the risk of progression of the scoliosis, the need for bracing, or surgery. Conclusion. The impact of treatment with growth hormone on scoliosis in children with idiopathic short stature was considered controllable. However, physicians should pay close attention to the assessment of spinal curves in these children. Cite this article: Bone Joint J 2023;105-B(4):439–448

The aim of this study is to clarify the implication of ciliary pathway on the onset of the spinal curvature that occurs in Adolescent Idiopathic Scoliosis (AIS) patients through functional studies of two genes: POC5 and TTLL11. Since the genetic implication for AIS is accepted, many association and candidate gene analysis revealed the implication of ciliary genes. The characterisation of these two proteins was assessed by qPCR, WB and immunofluorescence in vitro using control cells and cells derived from AIS patients. The impact of genetic modification of these genes on the functionality of the proteins in vitro and in vivo was analysed in zebrafish model created by CRISPR/Cas9 using microCT and histologic analysis. Our study revealed that mutant cells, for both gene, were less ciliated and the primary cilia was significantly shorter compared to control cells. We also observed a default in cilia glutamylation by immunofluorescence and Western Blot. Moreover, we observed in both zebrafish model, a 3D spine curvature similar to the spinal deformation in AIS. Interestingly, our preliminary results of immunohistology showed a retinal defect, especially at the cone cell layer level. This study strongly supports the implication of the ciliary pathway in the onset of AIS and this is the first time that a mechanism is described for AIS. Indeed, we show that shorter cilia could be less sensitive to environmental factors due to lower glutamylation and result in altered signalling pathway. Identifying the biological mechanism involved is crucial for elucidating AIS pathogenesis

There is a need for non-radiographic, objective outcome measures for children with Adolescent Idiopathic Scoliosis (AIS). Standing balance and stability is altered in children with AIS. The Margin of Stability (MoS) has been used to compare gait stability in clinical populations. Our objective was to compare the MoS in anterior-posterior (MoS. AP. ) and mediolateral (MoS. ML. ) directions in girls with AIS to Controls. Girls with AIS and healthy girls walked at three speeds on an instrumented treadmill wearing retroreflective markers, surrounded by motion capture cameras. The MoS. AP/ML. was calculated at left and right heel strike. Data was processed in Visual 3D. A two-way ANOVA was used to compare MoS. AP/ML. between group, speed and the interaction between group and speed. Pearson's correlation coefficient was used to compare the MoS to Cobb angle. Statistical significance was accepted when p > 0.05. A priori power analysis suggested 12 participants per group. Three Cases and four Controls were recruited. Girls with AIS all had right-sided main thoracic curves (Lenke type 1a, 61.3° ± 10.0°). MoS. AP. was significantly bigger for Cases compared to Controls on the left (p=0.038) and right foot (p=0.041). There was no significant difference between Cases and Controls for MoS. ML. , but there was a visual trend for a smaller MoS. ML. in Cases. There was no significant difference for speed or the interaction between group and speed for MoS. AP. or MoS. ML. In Cases, MoS. AP. increased with increasing Cobb angle on the left (r. 2. =0.687, p=0.054) and right (r. 2. =0.634, p=0.067) and MoS. ML. decreased with increasing Cobb angle on the left (r. 2. =-0.912, p=0.002). Further subjects are being recruited. Girls with Lenke type 1a AIS are more stable in the AP direction and less stable in the ML direction than Controls during treadmill walking. AP stability increases and ML stability decreases with increasing Cobb angle. This research suggests that the MoS could be used as an outcome measure for children with AIS. Continued work is required to increase the power of this study. Further work could consider these changes during walking overground, measuring an MoS or MoS-like measure using a wearable device, and in different curve types

Introduction. Traditionally correction of idiopathic paediatric scoliosis is done by hybrid fixation. This involves a judicious combination of mono-axial and poly-axial screw constructs. This has inherent perceived advantages with better deformity correction and maintaining alignment without loss of correction over time. Study design. Single centre retrospective review of prospective collected data on the radiological analysis of idiopathic paediatric scoliosis corrections. The study compared hybrid screw constructs (poly-axial & mono-axial) to all poly-axial screw constructs over 28 months. Objective. Compare loss of correction between hybrid screw construct group (HSG) and all poly-axial screw construct group (PSG). Method. Retrospective review of preoperative, post-operative and latest follow-up radiographs on the cohort of 42 consecutive patients over a period of 28 months from a single surgeon series. Results. There were 19 patients (16 females, 3 male) in HSG and 23 (18 females, 5 male) in PSG. Average age at surgery was 14 years for HSG and 15.8 years for PSG. The average baseline Cobbs angle for HSG was 64.57°and 60.79° for PSG. In the HSG, on average 11.6 levels were fused and, in the PSG, it was 11.3 level. Mean screw density for HSG was 1.54 and PSG was 1.6. Mean correction from pre-op to immediate post-op was 46.06° (70.10%) in the HS group and 41.24 degrees (67.78%) in the PS group. At the last follow-up, mean correction was 45.12° (68.0%) for the HSG and 42.43° (70.39%) for PSG. Loss of correction from post-operative radiographs to latest follow up averaged 10.05% in HSG and 3.86% for PSG. Discussion. All poly-axial screw constructs has the advantage of minimal tray inventory, simple logistics, decreased surgical time and overall better efficiency. Rod application and derotation over poly-axial screw constructs is well controlled and we found no difference in the performance of these screws during and after the procedure. Conclusion. There was no statistically significant difference in the degree or loss of correction in HSG or PSG. No difference in radiological outcomes. In poly-axial pedicle screw construct, threading the rod and correction manoeuvres are easier and thereby reducing surgical times. There was no compromise on the derotation manoeuvre and correction of the apical rotation deformity. Our findings show that all poly-axial screw constructs in the management of idiopathic scoliosis corrections is emerging as a tangible alternative for the future


The aim of this study was to compare the treatment ouctomes of severe idiopathic scoliosis (IS) (>90 degrees) using the staged surgery with initial limited internal distraction and typical IS treated using segmental pedicle screw instrumentation. We hypothesized that staged surgical treatment of severe scoliosis would improve more HRQoL and pulmonary function (PF) as compared with posterior spinal fusion (PSF) for typical IS curves.

It was a retrospective review of a consecutive series of 60 IS, severe group (SG) vs. moderate group (MG) with min. 2 years of follow up (FU).

The mean preoperative major curve (MC) was 120° and thoracic kyphosis (TK) was 80° for the SG and 54° and 17° for the MG, respectively (p<0.001). The MC was corrected to 58° and TK to 32° for the SG; the MC to 26° and TK to 14°, for the MG, respectively (p<0.001). The mean preoperative AVT was 8.9 cm and improved to 2.8 cm at the final FU for the SG and from 6.5 cm to 2.2 cm at the final FU for the MG (p<0.001). At baseline, the FVC% & FEV1% values were significant different between the two groups (41.5% vs. 83%, p <0.001) & (41.6% vs. 77%, p <0.001). Compared the baseline for SG vs. the values at 2-year FU the FVC % values were (41.5% vs. 66.5%, p <0.001), and the baseline for MG vs. the values at 2-year FU, the FEV1 values were (77% vs. 81%, NS). At last FU, no complications were reported.

Gradual traction with complicity of multilevel Ponte's osteotomies and neuromonitoring followed by staged pedicle screws instrumentation in severe IS proved to be a safe and effective method improving spinal deformity (52% correction), PF (improved percentage of predicted forced vital capacity by 49%), and health-related quality and allows to achieve progressive curve correction with no neurologic complications associated to more aggressive one-stage surgeries.

Adolescent Idiopathic Scoliosis (AIS) patients were subjected to four weeks of Physiotherapy Scoliosis Specific Exercise (PSSE). 124 (Exclusion = 3) Patients were enrolled and assigned to either complete their treatment in one 4-week bout (4WC) (63 patients, M. age. = 14.52), or to complete their treatment in two separate fortnightly bouts (2X2WC) (63 patients, M. age. = 14.26). Clinical exam, surface topography and Scoliometer readings were compared. The SRS-30 questionnaire before and after treatment was conducted at 6, 12, 18 and 24 months. Group 1 (4WC) showed significant improvements from baseline (Pre-3.73 – Post 3.9; p=0.026) after the course of treatment, and showed significant improvements at 12 months follow up in Mental Health (p=0.006), Aggregate score (p= 0.005) and Satisfaction score (p=0.011). Satisfaction score remained statistically significant at 18 months follow up (p=0.016). Group 2 (2X2WC) did not record a significant improvement from baseline (p=0.058); however, showed significant improvements in self-image (p=0.013). There was no statically significant difference in SRS scores with respect to follow up time. We conclude that Physiotherapy Scoliosis Specific Exercise (PSSE) is a successful non-invasive therapy for AIS. The modified Schroth technique (ScolioGold) shows significant improvement in SRS30 scores with the 4-week intensive course that are sustained at a 2 year follow up

Aim. The aim of this study is to identify the incidence of the presence of an Inverted Radial Reflex (IRR) in asymptomatic subjects with Adolescent Idiopathic Scoliosis and determine its significance. Methods. Our study group consists of Adolescent Idiopathic Scoliosis patients who presented consecutively in our institution from June to September 2010. They were either seen in outpatient clinics or as elective admissions prior to operative correction of their spinal deformity. The presence of an idiopathic scoliosis deformity and the absence of any abnormal neurological symptoms were our inclusion criteria. They were examined by two clinicians for the presence of IRR using a tendon hammer. As part of their management all such patients routinely undergo an MRI scan in our Trust to investigate for the presence of intraspinal pathology. When the IRR was present we looked at their MRI scans to identify any relevant abnormalities. Results: We identified 100 subjects. There were 72 females and 28 males with an average age of 15 years. The IRR was present in 12 cases and in 6 of them the sign was present bilaterally. There were no further associated signs or symptoms. All cases had recently undergone MRI of their whole spine and their investigations did not demonstrate any abnormalities in the lower cervical spine. Conclusion. We have found an incidence of 12% for the presence of IRR in our study group. In all cases there were no abnormal cord signal changes in MRI scans and we can therefore conclude that in asymptomatic Adolescent Idiopathic Scoliosis patients the presence of IRR is of no clinical significance

Adolescent idiopathic scoliosis affects about 3% of children. Non-operative measures are aimed at altering the natural history to maintain the size of the curve below 40° at skeletal maturity. The application of braces to treat spinal deformity pre-dates the era of evidence-based medicine, and there is a paucity of irrefutable prospective evidence in the literature to support their use and their effectiveness has been questioned.

This review considers this evidence. The weight of the evidence is in favour of bracing over observation. The most recent literature has moved away from addressing this question, and instead focuses on developments in the design of braces and ways to improve compliance.

Aim:. To Determine The Effect Of Posterior Instrumented Fusion On Lung Function In Patients With Idiopathic Scoliosis Aged 8–11. Method:. Lung Function (Fvc And Fev1) Was Measured Before Surgery In 13 Patients (Aged 8 To 11) With Idiopathic Scoliosis. All Patients Had Curves Greater Than 50 And Had Undergone Posterior Instrumented Scoliosis Correction And Fusion With (3 Patients) Or Without (10 Patients) Same Day Anterior Convex Growth Arrest. Lung Function Tests Were Repeated 1–8 Years (Mean 5.3 Years) After Surgery. The Data Was Normalised To Take Into Account Standing Height And Loss Of Stature Due To Lateral Curvature, Allowing A Direct Comparison Of Percent Predicted Fev1 And Fvc Before And After Surgery. Results:. Mean Fvc Was 75% Before Surgery And 79% At Longest Follow-Up (1.9l To 2.7l). Mean Fev1 Was 73% Before Surgery And 76% At Longest Follow-Up (1.7 To 2.4). The Change In Lung Function After Surgery Did Not Correlate With Length Of Time To Longest Follow-Up. The Ratio Of Fev1 To Fvc Was 0.86 Before And After Surgery. Discussion:. In Idiopathic Scoliosis Below The Age Of 8 Years, Most Would Accept That Instrumentation Should Allow Spinal Growth To Try And Maintain Lung Function. Above The Age Of 11, Lung Function Is Probably Sufficient To Allow Instrumented Fusion With Its Reduced Complication Rate And Reduced Need For Further Surgery. Between The Ages Of 8 And 11 Controversy Exists. This Pilot Study Suggests That A Larger (Possibly Rct) Study Should Be Performed To Evaluate This Group Of Patients As Fusion May Be Appropriate As Percent Lung Function Corrected For Loss Of Height Due To Curvature Appears To Be Maintained. Background

Scoliosis is described as a lateral spinal curvature exceeding ten degrees on radiograph with vertebral rotation. Approximately 80% of scoliosis presentations are adolescent idiopathic scoliosis (AIS). Current management for AIS in the UK occurs in Surgeon or Paediatrician-led clinics and can be conservative or surgical. The musculoskeletal assessment and triage of AIS appears well-suited to an advanced physiotherapist practitioner (APP) skill set. The aim of this service evaluation was to scope, develop, implement and evaluate a four-month pilot of an APP-led AIS triage pathway.

Method and Results

Spinal Consultant deformity and scoliosis clinics were scoped and observed. Clinic inclusion criteria and a patient assessment form was developed. An APP AIS clinic was set up beside a consultant led clinic. All patients assessed were discussed with a spinal surgeon. Consultant and APP agreement (% of total), waiting times, surgical conversion, and patient satisfaction were reviewed. A clinical competency package was developed for training and development of APPs. A total of 49 patients were seen (20 sessions). Waiting list reduced from 10 weeks to 6 weeks. 45%(n=22) of new patients seen were diagnosed with AIS, 27% (n=6) were directly listed for surgery. Consultant/ APP percentage agreement was high for Cobb angle measurement (82%), management plans (90%), and further diagnostic requests (94%). There were no adverse events and high patient satisfaction levels (n=20), (100% Very satisfied or satisfied) were reported.

Introduction. The potential structural effect of the sacral tilt on the development of disc wedging adjacent to lowest instrumented vertebra (LEV), and consequent postoperative coronal trunk decompensation in the surgical treatment of idiopathic scoliosis are well known. The aim of this study is firstly to establish the possible role of sacral tilt, limb length inequality, and associated pelvic girdle anomalies in the cause of idiopathic scolisosis, and secondly to determine the effect of sacral tilt on adjacent disc wedging below LEV in some idiopathic curves such as thoracolumbar/lumbar curves, and double major curves postoperatively. Methods. Between 2006 and 2008, 159 patients with idiopathic scoliosis from outpatient clinics were included in analyses. In all patients, standing posteroanterior and lateral spinal column and Ferguson radiographs were routinely taken. If direct radiographs suggested iliac bone asymmetry, hemi-pelvis volume measurements were done with CT scan. Curve type, sacral tilt, L5 tilt, iliac asymmetry, and limb-length differences were measured. As a second part of study, 87 surgically treated patients with double major curves were examined retrospectively in terms of adjacent subjacent disc wedging below LEV and sacral tilt. Results. In the first part of the study, the mean age of patients was 14·21 years (range 2–25) and all but 14 were female. The curve types were lumbar in 31 patients, thoracolumbar in 42, double major in 55, thoracic in 27, and triple in four. Sacral tilt towards the convex side of TL/L curve was present in 117 (74%) patients with an average of 8° measured from Ferguson graphs. L5 tilt towards the convex side of TL/L curve, and limb length inequality of less than 5 mm on the convex side of TL/L curve were more prevalent in patients with sacral tilt (57%) than in those without sacral tilt (p=0·021). On hemi-pelvis volume measurement, patients with no sacral tilt (n=42) had no difference in both sides whereas patients with sacral tilt (n=117) showed significant difference, with the convex side being smaller. We noted a correlation between L5 tilt and sacral tilt (p=0·048) and between sacral tilt and hemi-pelvis volume (p=0·024). There was no correlation between sacral tilt and pelvic asymmetry, age, sex, curve type, or amount of limb length inequality. In the second part of the study, disc wedging adjacent to LEV was present in 75 of 87 patients and sacral tilt was present in 72 of 75 (96%) (p=0·008). Six patients had limb length inequality of more than 1 cm, showing the postoperative trunk decompensation, which was corrected by elevating the shorter leg by shoe-lift. Conclusions. This study revealed significant association between sacral tilt and hemi-pelvis hypoplasia independent from age. This association may have a primary role in the cause of this disease. Although LEV is paralel to pelvis, intervertebral disc adjacent to LEV is often wedged and there is lumbosacral hemicurve distally below the instrumentation in patients treated by either anterior or posterior surgery. We believe that lumbosacral hemicurve due to sacral tilt behaves like hemivertebra, which is impossible to detect with preoperative standing anteroposterior radiographs. Ferguson view is necessary to detect the sacral tilt. The postoperative trunk decompensation is greater with presence of sacral tilt and limb length inequality

Introduction. One of the most complicated problems of orthopaedics is the treatment of scoliosis. More than 90% of cases are attributable to idiopathic deformation, the cause of which is unknown. We investigated the cause and pathogenesis of this disorder. Methods. At our institution, more than 6900 patients aged 1–89 years have undergone inpatient and outpatient treatment in connection with spinal pain syndrome and different neurological disorders associated with idiopathic scoliosis. This study was undertaken between February, 1996, and February, 2010. All patients had had clinical, radiographic, and laboratory examinations. Results. 29·6% of patients were aged 31–50 years; 60% were men and 40% women. While examining patients with scoliosis deformation we noted symptoms of body asymmetry–ie, different volumes of right and left halves of face, body, and limbs. These features were typical for all patients irrespective of sex, age, and ethnic origin. 83·2% of patients had underdevelopment of left part of the body, and only 16·8% the right side. Analysis of published work in anatomy, physiology, neurophysiology, and vertebrology, done simultaneously with analysis of clinical material, allowed us to make some conclusions. Conclusions. First, asymmetrical structure of human body is based on laws of nature and is linked with difference of sizes and activity of brain's hemispheres, particularly of right or left gyrus centralis anterior, which control the muscle's function and our movements. Second, asymmetrical tension of Erector spinae muscles leads to inclination of the pelvis on a side of weak muscles; thus initiating development of lateral spine curves. Since such a situation is typical for all people, this deformation is known as functional scoliosis. Third, further development of bodies of vertebrae, their arches, processes, intervertebral discs, ligaments, and other anatomical elements in position of deviation leads to one-sided underdevelopment of these structures. As a result, areas of instability appear in each segment of spine (neck, chest, lumbar, and sacral areas). Fourth, the muscles in a growing body misbalance and on the ground of rotating movement start rotatory dislocation of vertebrae in zones of instability in all parts of the spine. As a result, torsion of deformed wedge-shaped vertebrae leads to formation of structural scoliosis. Rotation of vertebrae, described above, does not depend on sex, age, and ethnic origin of a patient and has a character of natural development. Thus, from our point of view, the term idiopathic scoliosis must be changed to spinal muscle asymmetrical deformation of a reflex origin. Understanding of this rotation allowed us to establish an effective non-surgical method of treatment of scoliosis and spinal pain syndrome in patients of all ages

Use of ultrasonic bone scalpel (UBS) is becoming popular in spinal surgery. This study presents the safety profile of UBS for posterior release in surgical correction of adolescent idiopathic scoliosis (AIS).

From 2016 to 2018, UBS (Misonix) was used by the senior author in a variety of spinal operations. Data for intraoperative complications when this devise was used for posterior correction of AIS were collected. Revision cases were excluded.

UBS was used for posterior release of AIS in 65 patients (58 female, seven male) with an average age of 15.6 years (range 11–23). Average length of posterior instrumentation was 12 levels (range 6–14). Instrumentation was exclusively from T2 to L4. To achieve adequate release for correction, UBS was used to perform a total of 644 modified in-situ chevron osteotomies (average ten, range six to 12) and 31 rib osteotomies. Overall, three complications (4.6 %) were directly related to the use of UBS: one haemopneumothorax, which was successfully treated with a chest drain; and two loss of motor-evoked potentials during monitoring, which led to the postponement of the final correction. These two patients did not have any neurological problems and their surgery was completed successfully within 1 week of the initial surgery. One late deep infection was reported. This was not thought to be directly related to the use of UBS.

Use of UBS in the posterior surgical treatment of AIS appears to be relatively safe with a low level of acceptable complications. However, appropriate training is required for the use of UBS.

To determine whether spinal facet osteoblasts at the curve apex display a different phenotype to osteoblasts from outside the curve in patients with adolescent idiopathic scoliosis (AIS). Intrinsic differences in the phenotype of spinal facet bone tissue and in spinal osteoblasts have been implicated in the pathogenesis of AIS. However, no study has compared the phenotype of facet osteoblasts at the curve apex with the facet osteoblasts from outside the curve in patients with AIS.

Facet bone tissue was collected from three sites, the concave and convex side at the curve apex and from outside the curve from three female patients with AIS (aged 13–16 years). Micro-CT analysis was used to determine the density and trabecular structure. Osteoblasts were then cultured from the sampled bone. Osteoblast phenotype was investigated by assessing cellular proliferation (MTS assay), cellular metabolism (alkaline phosphatase and Seahorse Analyser), bone nodule mineralisation (Alizarin red assay), and the mRNA expression of Wnt signalling genes (quantitative RT-PCR).

Convex bone showed greater bone mineral density and trabecular thickness than did concave bone. The convex side of the curve apex exhibited a significantly higher proliferative and metabolic phenotype and a greater capacity to form mineralised bone nodules than did concave osteoblasts. mRNA expression of SKP2 was significantly greater in both concave and convex osteoblasts than in non-curve osteoblasts. The expression of SFRP1 was significantly downregulated in convex osteoblasts compared with either concave or non-curve.

Intrinsic differences that affect osteoblast function are exhibited by spinal facet osteoblasts at the curve apex in patients with AIS.

Aim:. To Determine Whether Maximal Rib Prominence Measured On Lateral Radiographs Can Be Used As A Surrogate To Rib Rotation Determined By Surface Tomography (Quantecscanning) In Assessment Of Spinal Rotation. Method:. Patients With Adolescent Idiopathic Scoliosis Underwent Plain Lateral Radiographs And Quantec Scans. Maximal Rib Prominence On The Lateral Radiograph Was Defined Pre- And Post-Operatively By Distance From Most Posterior Aspect Of The Rib To The Facet Joint And Instrumented Rod, Respectively. Rib Rotation Was Measured By Surface Tomography Quantec Scan Using The Suzuki Method. This Was Then Repeated At A Later Time And By An Additional Investigator To Assess Intra- Observer And Inter-Observer Variability. The Correlation Between Maximal Rib Prominence And The Suzuki Ratio Was Determined. Results:. 20 Patients, Median Age 15 Years, Were Evaluated. The Correlation Coefficient Between Radiograph Determined Maximal Rib Prominence And Quantec Derived Suzuki Ratio Was 0.68, R2=0.46 (See Figure). Discussion:. There Was A Weak Positive Correlation Between Radiograph Determined Maximal Rib Prominence And Quantec Derived Suzuki Ratio. Further Investigation In A Larger Cohort Is Warranted. A Core Outcome Set (COS) for treatment of adolescent idiopathic scoliosis (AIS) is essential to ensure that the most meaningful outcomes are evaluated and used consistently. Measuring the same outcomes ensures evidence from clinical trials and routine clinical practice of different treatments can be more easily compared and combined, therefore increasing the quality of the evidence base. The SPINE-COS-AYA project aims to develop a gold standard COS which can be used internationally in research and routine clinical practice to evaluate the treatment (surgical and bracing) of AIS.

In this qualitative study, the views of adolescents and young adults with AIS (10-25 years of age), their family members and healthcare professionals in a UK region were sought, via interviews, on treatment outcomes. Participants were purposively recruited from a variety of sources including NHS outpatient clinics and social media. Semi-structured interviews were analysed using thematic analysis.

Key findings will be presented, to include potential core outcome domains identified by the different subgroups.

The core outcome domains identified in this research programme will subsequently form part of an international consensus survey to agree a COS. In future, if the COS is used by healthcare staff and researchers, it will be easier for everyone, including patients and their families, to assess which treatment works best.

Introduction. Autonomic nerve system (ANS) regulates intercostal vascular nutrition (internal mammary artery), and its pathological status leads to developmental asymmetry of the trunk and rib cage, and consequently producing scoliotic deformity of the spine. The aim of this study is to investigate the possible causation of idiopathic scoliosis in development abnormalities of ANS. Methods. We evaluated samples taken from 12 patients with idiopathic scoliotic deformities and a control set of three patients without scoliotic deformity. We examined the samples of autonomic nerves taken from convexity and concavity of the scoliotic deformity during the patients' surgical correction by the transthoracic approach. We used the electronmicroscopic method to analyse samples, and the morphometric method for statistical evaluation. Results. Evaluation of the samples taken from scoliotic convexity and the control samples of non-scoliotic patients showed normal findings in autonomic nerve structures. We detected significant morphological changes in all scoliotic samples taken from concavity. These changes were mostly in myelin vaginas with abnormalities and compression of the axon fibre, massive lesion and separation of the myelin sheath, vacualisation of cytoplasma of the Schwann cells, and condensation of the cytoblast. By morphometric measurements we found 23·71% of myelinised nerve fibres (MNF), 12·21% of unmyelinised nerve fibres (UNF), and 5·0% of Schwann cells (SC) in samples taken from scoliotic convexity, and 29·9% of MNF, 19·9% of UNF, and 16·7% of SC in control non-scoliotic samples. We recorded 17·36% of MNF, 5·82% of UNF, and 5·27% of SC in samples taken from concavity. Conclusions. We noted abnormalities in structure of ANS in concave side of scoliotic curves, and statistically significant differences between both sides of scoliotic deformity (convexity and concavity). Furthermore, we recorded discrepancies between scoliotic samples and non-scoliotic control samples. The abnormalities, mostly in the myelinated fibres, might be originated by the primary genetic lesion and thus could affect the development of scoliosis. The abnormalities of ANS can produce changes in internal mammary artery, and consequently can lead to the abnormal blood supply of vertebrae as well as anterior wall chest. These abnormalities of ANS could lead to the scoliotic origin in growing spine

Introduction. Postural and motor activities are the results of interactions of smaller inhibitory and larger facilitating structures of the central nervous system (CNS). In the case of dysbalance of inhibitory and facilitating structures during CNS evolution, the asymmetry of postural activities can appear. This asymmetry remains hidden in the early periods of evolution and becomes apparent in the periods of quick growth and increased hormonal and metabolic activities. Genetic and neural factors have proven to be significant in the cause of idiopathic scoliosis (IS), so we propose a neural developmental hypothesis of this disease. Methods. We evaluated a cohort of 19 patients, all of whom were girls with a mean age of 14·7 years (range 13–18) with right idiopathic thoracic curve (mean Cobb angle 53·5°, range 37–72°; of the apical vertebra from T7 to T9). Heart and pulmonary functions were evaluated by heart ECHO and spiroergometry. Results. Normal heart and pulmonary functions support the opinion that heart and pulmonary changes are not the causative factors in IS. The CNS is specialised computer, so we can found hardware and software damages and faults, resulting in asymmetric function of postural motor muscles. Hardware damage–structural changes from brain cortex to spinal cord–can be of genetic, perinatal, or aquired origin (possibly up to 2 years of age). Software damage–repeated asymmetric positions–can cause an increased plasticity period (quick evolution) of CNS pathological programmes of postural reactions in children at risk of IS–uprighting. Conclusions. Children at risk of IS have postural reaction and postural evolution (quick uprighting) pathology, which can be diagnosed during the first year of age by Vojta diagnostic system. The treatment can be effective only in the period of increased plasticity and development of the CNS, mainly in the first 6 months of age. The symmetric positions and symmetric sensory, and proprioceptive input and motor activities can affect CNS evolution. By slowing down the uprighting the CNS has time to repair the damaged software or replace the damaged connections/programmes. In this way we could minimise structural damage. Developmental hip dysplasia treated by Frejka pillow and Pavlik harness do not affect future motor activities (IS was not detected in these patients). So these controlled motion exercises, which enable symmetric evolution and slow down uprighting of the child, could be a good treatment option in children at risk of IS. Acknowledgments. 1312 patients with idiopathic scoliosis completed the ScolioGold Body Image Questionnaire (SBIQ) before and after a course of intensive PSSE treatment. A paired T-test compared the patient's perception of Body image. Mean age was 24.7 with a range of 6 to 83 years old (<18 = 687; >18 = 625), female = 1061, male = 251).

Pre-treatment average was 5.3 (SD=2.07), which improved to 2.8 after treatment (SD=1.8). The difference of 2.6 was statistically significant (p<0.005). Pre-treatment, the patients in the children group scored 4.67, which was significantly lower than the adult group score of 6.32. On average, the children improved their scores by 2.48 to 2.19 which was statistically significant (p=0.000). The adults improved their scores by 2.81 to 3.51 which was statistically significant (p=0.000). On average patients with mild (11° to 25° Cobb angles) and moderate (26° to 50° Cobb angles) curvatures achieved a larger improvement compared to patients with severe curvatures (>50° Cobb angles). This difference of 0.35 was statistically significant (n=641, p<0.05).

There were significant improvements in the total SBIQ scores before and after treatment in both the child and adult groups with mild to severe curvatures after an intensive course of PSSE treatment. This shows that intensive PSSE treatment can improve scoliotic patients' self-image, therefore addressing aspects of their psychosocial well-being.
