Introduction. Traditionally correction of idiopathic paediatric scoliosis is done by hybrid fixation. This involves a judicious combination of mono-axial and poly-axial screw constructs. This has inherent perceived advantages with better deformity correction and maintaining alignment without loss of correction over time. Study design. Single centre retrospective review of prospective collected data on the radiological analysis of idiopathic paediatric scoliosis corrections. The study compared hybrid screw constructs (poly-axial & mono-axial) to all poly-axial screw constructs over 28 months. Objective. Compare loss of correction between hybrid screw construct group (HSG) and all poly-axial screw construct group (PSG). Method. Retrospective review of preoperative, post-operative and latest follow-up radiographs on the cohort of 42 consecutive patients over a period of 28 months from a single surgeon series. Results. There were 19 patients (16 females, 3 male) in HSG and 23 (18 females, 5 male) in PSG. Average age at surgery was 14 years for HSG and 15.8 years for PSG. The average baseline Cobbs angle for HSG was 64.57°and 60.79° for PSG. In the HSG, on average 11.6 levels were fused and, in the PSG, it was 11.3 level. Mean screw density for HSG was 1.54 and PSG was 1.6. Mean correction from pre-op to immediate post-op was 46.06° (70.10%) in the HS group and 41.24 degrees (67.78%) in the PS group. At the last follow-up, mean correction was 45.12° (68.0%) for the HSG and 42.43° (70.39%) for PSG. Loss of correction from post-operative radiographs to latest follow up averaged 10.05% in HSG and 3.86% for PSG. Discussion. All poly-axial screw constructs has the advantage of minimal tray inventory, simple logistics, decreased surgical time and overall better efficiency. Rod application and derotation over poly-axial screw constructs is well controlled and we found no difference in the performance of these screws during and after the procedure. Conclusion. There was no statistically significant difference in the degree or loss of correction in HSG or PSG. No difference in radiological outcomes. In poly-axial pedicle screw construct, threading the rod and correction manoeuvres are easier and thereby reducing surgical times. There was no compromise on the derotation manoeuvre and correction of the apical rotation deformity. Our findings show that all poly-axial screw constructs in the management of idiopathic scoliosis corrections is emerging as a tangible alternative for the future

We compared the clinical, radiological and quality-of-life outcomes between hybrid and total pedicle screw instrumentation in patients undergoing surgery for neuromuscular scoliosis. A matched comparison using prospectively collected data was undertaken. A total of 66 patients underwent posterior or anteroposterior correction and fusion with hybrid (n = 33, mean age at surgery 15.8 years (9.10 to 19.6)) or total pedicle screw instrumentation (n = 33, mean age 14.7 years (7.0 to 20.7)) with a minimum follow-up of two years. The major curve pre-operatively was a mean of 87° (. sd. 29, 25° to 141°) and 81° (. sd. 18, 47° to 116°) in the hybrid and total pedicle screw groups, respectively (p = 0.29) and at a minimum of two years it was 33° (. sd. 20; 2° to 87°) and 20° (. sd. 12; 1° to 55°), respectively (p = 0.0016). The mean correction of the major curve was 59% (41% to 88%) in the hybrid and 75% (43% to 99%) in the total pedicle screw groups at two-year follow-up (p = 0.0011). The mean operating time was 7.45 hours (. sd. 2.18) and 6.04 hours (. sd. 1.71) in the hybrid and total pedicle screw groups, respectively (p = 0.001), and the mean intra-operative blood loss was 3760 ml (. sd.  2790) and 1785 ml (. sd. 1110), respectively (p = 0.001). Total pedicle screw instrumentation provided shorter operating times, less blood loss and better correction of the major curve compared with hybrid constructs in patients undergoing surgery for neuromuscular scoliosis


Background. Magnetic resonance imaging (MRI) algorithm identifies end stage severely degenerated disc as ‘black’, and a moderately degenerate to non-degenerated disc as ‘white’. MRI is based on signal intensity changes that identifies loss of proteoglycans, water, and general radial bulging but lacks association with microscopic features such as fissure, endplate damage, persistent inflammatory catabolism that facilitates proteoglycan loss leading to ultimate collapse of annulus with neo-innervation and vascularization, as an indicator of pain. Thus, we propose a novel machine learning based imaging tool that combines quantifiable microscopic histopathological features with macroscopic signal intensities changes for hybrid assessment of disc degeneration. Methods. 100-disc tissue were collected from patients undergoing surgeries and cadaveric controls, age range of 35–75 years. MRI Pfirrmann grades were collected in each case, and each disc specimen were processed to identify the 1) region of interest 2) analytical imaging vector 3) data assimilation, grading and scoring pattern 4) identification of machine learning algorithm 5) predictive learning parameters to form an interface between hardware and software operating system. Results. Kernel algorithm defines non-linear data in xy histogram. X,Y values are scored histological spatial variables that signifies loss of proteoglycans, blood vessels ingrowth, and occurrence of tears or fissures in the inner and outer annulus regions mapped with the dampening and graded series of signal intensity changes. Conclusion. To our knowledge this study is the first to propose a machine learning method between microscopic spatial tissue changes and macroscopic signal intensity grades in the intervertebral disc. Introduction and Objective

Almost 60% of the population can expect to experience low back pain (LBP) during their life. Several radiological tools are used to investigate LBP. However, adequate evidence is unavailable to support the use of single photon emission computer tomography (SPECT) in patients with LBP. The objective of this study is to assess the role and efficiency of SPECT in evaluation and management of patients with LBP.

Method

Ninety-two patients with LBP were examined and assessed. All the patients received a magnetic resonance imaging (MRI) scan and were referred for a SPECT. We interpreted the modic and degenerative changes found on the MRI and compared it with SPECT tracer uptake. SPECT was used to identify the pain generator and then a surgical plan was made. Data was analyzed for pain improvement in those who underwent surgical treatment to establish the accuracy of CT SPECT in identification of primary pain generator.

Aims. To report the surgical outcome of patients with severe Scheuermann’s kyphosis treated using a consistent technique and perioperative management. Methods. We reviewed 88 consecutive patients with a severe Scheuermann's kyphosis who had undergone posterior spinal fusion with closing wedge osteotomies and hybrid instrumentation. There were 55 males and 33 females with a mean age of 15.9 years (12.0 to 24.7) at the time of surgery. We recorded their demographics, spinopelvic parameters, surgical correction, and perioperative data, and assessed the impact of surgical complications on outcome using the Scoliosis Research Society (SRS)-22 questionnaire. Results. The mean follow-up was 8.4 years (2 to 14.9). There were 85 patients (96.6%) with a thoracic deformity. Posterior spinal fusion with closing-wedge osteotomies and hybrid instrumentation was used in 86 patients; two patients underwent combined anterior and posterior spinal fusion. The mean kyphosis was corrected from 94.5° to 47.5° (p < 0.001). Coronal and sagittal balance returned to normal. The rate of complications was 12.5%: there were no neurological deficits, implant failure, or revision surgery. SRS-22 scores improved from a mean 3.6 (1.3 to 4.1) to 4.6 (4.2 to 5.0) at two years (p < 0.001) with a high rate of patient satisfaction. Non-smokers and patients with lower preoperative SRS-22 scores showed greater improvement in their quality of life. Sagittal pelvic balance did not change after correction of the kyphosis and correlated with lumbar lordosis but not with thoracic or thoracolumbar kyphosis. Conclusion. Posterior spinal fusion using hybrid instrumentation, closing-wedge osteotomies, and iliac bone grafting achieves satisfactory correction of a severe kyphosis resulting in improvements in physical and mental health and a high degree of patient-reported satisfaction. Introduction. Treatment of spinal metastatic disease has evolved with the advent of advanced interventional, surgical and radiation techniques. Spinal Oligometastatic disease is a low volume disease state where en bloc resection of the tumour, based on oncological principles, can achieve maximum local control (MLC). Hybrid therapy incorporating Separation surgery (>2mm clearance of the thecal sac) and Stereotactic Ablative Radiotherapy (SABR) offer an alternative approach to achieving MLC. Hybrid therapy is also a viable option in patients eligible for SBRT who have failed conventional radiation therapy. En-bloc surgery may be a suitable option for those patients who are ineligible for or have failed SBRT. A multidisciplinary approach is particularly important in the decision-making process for these patients. Metal free instrumentation is aiding the optimization of these surgeries. The authors present a supra-regional centre's experience in managing spinal oligometastases. Methods. Retrospective review of oligometastatic spinal disease at a supra-regional centre between 2017 and 2021. Demographics, operative course, complications and Instrument type are examined. Results. Demographics: 24 patients with mean age 53.8y (range 12–77), 44% (40y–59y), 40% (60y–69y); 51% Male. Histology: Breast, Renal and Sarcoma accounted for 16.7% each; Thyroid, Prostate and Chordoma accounted for 8.3% each. Primary disease 7%, Synchronous 15%, Metachronous 78%. Instrumentation: Carbon-fibre (85%), TiAl (11%), Non-Instrumented (3%). Separation Surgery (70%), En-bloc resection/Tomita surgery (30%); SABR/Proton Beam Planned: 70%. Average length of hospital stays 19.1 days; twenty patients required intensive care admission for an average 2.7 days. 30 Day Mortality 8.3% (n=2: COVID-19 during admission and ventriculitis post discharge), 1y Mortality – 16.7%, 3y Mortality – 25%; Synchronous Mortality 75% (n=3) at 3 years. 30 Day infection rate 3%; 1y infection rate 7%. 1 Non-instrumented case developed proximal junctional failure post proton beam therapy and required a vascularised fibular strut graft. 2-year Revision for Local Recurrence 5% (Revision at 23 months). Conclusion. There are very few case series of oligometastatic spinal disease due to the relatively new concept of adjuvant SABR and its limited availability. Solid tumours pre-dominated the histology in our series with metachronous disease being the most commonly operated disease state. 92% of cases were eligible for SABR. The majority (85%) of cases were performed with Carbon-fibre instrumentation and has been shown to be safe with no mechanical failures in this series. Infection rates are in keeping with patients requiring radical radiotherapy with 3% early and 7% late. 30-day mortality was 8.3%, 1y=16.7% and 25% at last follow up. Mortality, as expected, is highest within the synchronous disease group and should be operated on sparingly. With the current management strategy, there was no local recurrence at 1 year and excellent local recurrence rate at 2 years (5%). Although radical en bloc surgery carries significant morbidity, it should be considered in selective cases to achieve MLC. All Oligometastatic cases deserve extra consideration and specialist MDT as not all are suitable for SABR. Multimodal Hybrid therapy, incorporating less invasive surgical techniques and SABR, represents a paradigm shift in achieving MLC in oligometastatic spinal disease

Background. A local authority approached us, for a cost-beneficial solution to their increasing low back pain referrals. We proposed developing a student-led clinic – an intervention delivered by students but supervised by clinicians. We then conducted scoping reviews on student-led clinics in the management of health conditions and on the self-management of back pain. The findings suggested that student-led self-management interventions for low back health should be feasible. The next step was to co-construct the intervention with key stakeholders. Co-Construction. A hybrid of Action Research and Design Science methodology was used to co-construct the intervention with five key stakeholder groups (council staff, managers and human resources, employee healthcare, students, and lecturers). Three rounds of focus groups explored the ‘problem’, the possible solutions, the process, and the content. Themes were taken from each of these focus groups and the similarities and differences were analysed. This analysis and subsequent synthesis with the evidence base created potential intervention models, which were discussed and refined with the stakeholder groups. Intervention. The proposed intervention is focused on providing evidence informed biopsychosocial support for work-relevant back pain, based on identifying obstacles and solutions to improve coping with back health at work. An onboarding workshop challenges positively their thinking around back pain and work. This is followed by up to three 1:1 sessions that support the individual to identify work-relevant back health goals and agree a plan to achieve them using techniques to facilitate behaviour change. Conclusion. The intervention is evidence informed and aims to address the prioritised needs of the stakeholders. Conflicts of interest. No conflicts of interest. Sources of funding. Aims. To compare the rates of sagittal and coronal correction for all-pedicle screw instrumentation and hybrid instrumentation using sublaminar bands in the treatment of thoracic adolescent idiopathic scoliosis (AIS). Methods. We retrospectively reviewed the medical records of 124 patients who had undergone surgery in two centres for the correction of Lenke 1 or 2 AIS. Radiological evaluation was carried out preoperatively, in the early postoperative phase, and at two-year follow-up. Parameters measured included coronal Cobb angles and thoracic kyphosis. Postoperative alignment was compared after matching the cohorts by preoperative coronal Cobb angle, thoracic kyphosis, lumbar lordosis, and pelvic incidence. Results. A total of 179 patients were available for analysis. After matching, 124 patients remained (62 in each cohort). Restoration of thoracic kyphosis was significantly better in the sublaminar band group than in the pedicle screw group (from 23.7° to 27.5° to 34.0° versus 23.9° to 18.7° to 21.5°; all p < 0.001). When the preoperative thoracic kyphosis was less than 20°, sublaminar bands achieved a normal postoperative thoracic kyphosis, whereas pedicle screws did not. In the coronal plane, pedicle screws resulted in a significantly better correction than sublaminar bands at final follow-up (73.0% versus 59.7%; p < 0.001). Conclusion. This is the first study to compare sublaminar bands and pedicle screws for the correction of a thoracic AIS. We have shown that pedicle screws give a good coronal correction which is maintained at two-year follow-up. Conversely, sublaminar bands restore the thoracic kyphosis better while pedicle screws are associated with a flattening of the thoracic spine. In patients with preoperative hypokyphosis, sublaminar bands should be used to restore a proper sagittal profile. Cite this article: Bone Joint J 2020;102-B(3):376–382

Introduction. We aimed to determine the midterm effect of pedicle screw instrumentation on sagittal plane alignment, compared with a hybrid alignment, in the treatment of thoracic adolescent idiopathic scoliosis (AIS). Methods. 88 consecutive patients with AIS with a Lenke type 1 curve treated between 1998 and 2003 were analysed. Thoracic hooks were used in 45 patients (group Hy) and thoracic screws alone in 43 patients (group TPS). Preoperative average age (Hy 15·3 years vs TPS 16 years), sex (38 female and seven male vs 37 female and six male), Risser sign (2·9 vs 2·9), main thoracic curve (64° vs 65·5°), and thoracic kyphosis (22·6° vs 21·4°) were similar in both groups. Pearson correlation coefficient and univariate ANOVA were used. Results. At a mean follow-up of 7·3 years (range 5–10), the TPS group had a greater final main thoracic curve correction (Hy 46·4% vs TPS 58·4%; p<0·001), with inferior loss of initial correction (–11·1° vs TPS –1·3°; p<0·0005). Absolute final thoracic kyphosis was similar in both groups (31·4° vs 25·4°; r=0·002; p>0·05), with both groups showing an equally statistically significant amelioration of the sagittal contour (Hy 49·2%; p<0·001 vs TPS 43·4%; p<0·001) (difference between groups p>0·05). We recorded a significant correlation between absolute kyphosis correction (final preoperative) and percentage main thoracic curve initial correction loss in the Hy group (r=0·35, p<0·001). The SRS–30 assessment showed an improvement in self-image and satisfaction, with no significant differences between groups. Conclusions. The previously reported loss of kyphosis after a pedicle screw instrumentation in AIS, when compared with hybrid or hook only implants, should be questioned. At a minimum 5 years of follow-up, sagittal contour in the thoracic spine was not less kyphotic when pedicle screws were compared with hybrid constructs. The higher the loss of initial correction of main thoracic curve, the greater the postoperative absolute kyphosis at T5–T12 level. The clinical relevance of such radiographic differences is still undetectable with present self-assessment methods

Background Context. SPECT-CT is a hybrid imaging modality, which has become very well established in the diagnosis of inflammatory, vascular and malignant processes affecting the spine. However, little evidence exists on its application with degenerative pathologies. Purpose & Study Design. Systematic review on the use of SPECT-CT in the diagnosis of degenerative facet joint arthropathy. Patient Sample. 824 patients across 10 studies conducted over the last 6 years (2012 – 2018), with a mean age of 51.8 years old. All studies excluded patients with malignancy, infection or inflammatory aetiologies or those related to acute trauma. Only degenerative pathologies of the lumbar spine in an adult population were included. Methods. A systematic review of the literature available on the topic was conducted using the PubMed, Cochrane, Medline and Embase online databases of journal articles. Results. Five studies focused on SPECT-CT alone / versus CT. Two focused on SPECT-CT vs MRI, and three focussed on SPECT-CT guided facet joint injections. Of these 10 studies, seven concluded with results supporting SPECT CT's use, two reported discordance between SPECT-CT findings with those diagnosed from clinical examination / MRI scans, and one study's conclusions were equivocal. Conclusion. SPECT-CT shows promise as an imaging adjunct in assisting with the diagnosis and subsequent treatment of degenerative facet joint arthropathies. In patients whose MRI scans reveal multiple potential pain generators, SPECT-CT could help differentiate between clinically significant sources of pain and may even serve a purpose in guiding facet joint injections in patients with inconclusive MRI / CT scans. This review of the literature presents the current understanding of Scheuermann’s kyphosis and investigates the controversies concerning conservative and surgical treatment. There is considerable debate regarding the pathogenesis, natural history and treatment of this condition. A benign prognosis with settling of symptoms and stabilisation of the deformity at skeletal maturity is expected in most patients. Observation and programmes of exercise are appropriate for mild, flexible, non-progressive deformities. Bracing is indicated for a moderate deformity which spans several levels and retains flexibility in motivated patients who have significant remaining spinal growth. The loss of some correction after the completion of bracing with recurrent anterior vertebral wedging has been reported in approximately one-third of patients. Surgical correction with instrumented spinal fusion is indicated for a severe kyphosis which carries a risk of progression beyond the end of growth causing cosmetic deformity, back pain and neurological complications. There is no consensus on the effectiveness of different techniques and types of instrumentation. Techniques include posterior-only and combined anteroposterior spinal fusion with or without posterior osteotomies across the apex of the deformity. Current instrumented techniques include hybrid and all-pedicle screw constructs

Introduction. Recently published results suggest insertion of shorter screws in L5/S1 stand-alone anterior interbody fusion, fearing S1 nerve root violation. However, insertion of shorter screws led to screw fixation failure and new onset of S1 body fractures. Material and Methods. Retrospective review of patients with L5/S1 stand-alone anterior interbody fusion, focussing on screw length, radiological outcomes (especially metal work failure, screw fixation and S1 body fractures) and new onset of S1 nerve root irritation. Results. 38 patients were included (mean age 46.2±13.3 years, 21 females, 17 males). Fusion of the L5/S1 segment was performed in between 2003-2010; postoperative follow-up ranged from 2-24 months. 15 patients had multilevel surgery (7 multiple segmental fusion, 8 hybrid procedures). Screw length ranged from 20-30 mm. No patient had new postoperative S1 nerve root irritation. Interestingly, 2 patients out of the hybrid group had a new onset of L5 radiculopathy, concordant to the level of disc-replacement. Follow-up x-ray review showed no fracture of S1 body fractures in all patients. No evidence of screw loosening, migration or metal work failure was reported. Conclusion. In our opinion, this review showed that insertion of longer screws for stand-alone anterior interbody fusion in L5/S1 is safe. Longer screws offer better stabilization and seem to minimize risks like S1 body fractures. Short and long-term follow-ups were satisfactory regarding screw placement, migration and positioning of implants in all patients

Introduction:. Scheuermann's kyphosis is a fixed round back deformity characterised by wedged vertebrae seen on radiograph. It is known patients presented with a negative sagittal balance before operation. Few studies investigated the outcome after operation, especially the change in the lumbar hyperlordosis. Aim:. To investigate the change in sagittal profile after correction surgery. Method:. This is a retrospective review of cases from 2001 to 2012. Our centre uses a posterior, four rod cantilever reduction technique for all Scheuermann's Kyphosis correction. 36 cases are identified. They include 24 males and 12 females with an average age of 20 and follow up of 27 months. First 8 cases used the stainless steel hybrid implants. The remaining 28 had titanium all pedicle screw system. All had intra-operative spinal cord monitoring. Results:. The target of thoracic kyphosis correction is around the accepted upper end of normal limit (40°). The average thoracic kyphosis Cobb angle was 78.5°. The immediate post-op angle was 43.2° and at final follow up, 43.6°. The average lumbar lordosis changed from 65.7° pre-op to 48.8° post-op, which is now bigger than the thoracic kyphosis. The result is the transfer of average sagittal balance (C7 plumb line) from −2.2 cm to −3.5 cm, which remains posterior to the posterior corner of S1 after the surgery. Discussion:. Surgery can improve the roundback deformity but not the overall sagittal profile. We have no explanation to this phenomenon. This could imply the pathology of Scheuermann's Kyphosis involves the whole spine, not just the wedging thoracic segment. Conflict Of Interest Statement: No conflict of interest

Aim:. To compare the ability of fulcrum bend and traction radiographs to predict correction of AIS using pedicle screw only constructs and to compare the fulcrum bending correction index (FBCI) with a new measurement: the traction correction index (TCI). Method:. Retrospective radiographic analysis of eighty patients, average age 14 yrs, who underwent posterior correction of scoliosis using pedicle screw only construct. Analysis was carried out on the pre-op and immediate post-op PA radiographs and the pre-op fulcrum bend and traction radiographs. Correction rate, fulcrum flexibility, traction flexibility, FBCI and TCI was calculated. Results:. Preoperative mean Cobb angle of 63.9 degrees was corrected to 25.8 degrees postoperatively. The mean fulcrum bending Cobb angle was 37.6 degrees and traction Cobb angle 26.6 degrees. The mean fulcrum flexibility was 41.1%, traction flexibility 58.4% and correction rate 59.6%. The mean FBCI was 144.9% and TCI was 102.1%. Discussion:. Radiographic correction of scoliosis based on correction rate does not take into consideration the curve flexibility. It has been suggested by Cheung et al, that fulcrum bending radiographs predict curve correction in AIS. Ibrahim et al, have questioned this, and they suggested that traction radiographs are more predictive in a mixed group of patients with hybrid and screw only constructs. Conclusions:. When comparing fulcrum bend and traction radiographs, we found the latter to be more predictive of curve correction in AIS using pedicle screw constructs. Aim:. To compare the degree of deformity correction achieved using cobalt chromium versus titanium alloy rods in patients with Adolescent Idiopathic Scoliosis. Method:. A retrospective comparison of two cohorts of patients with Adolescent Idiopathic Scoliosis treated with posterior segmental pedicle screw fixation using either Titanium or Cobalt Chromium rods. The radiographs of 50 patients treated before 2009 (Ti group) and 50 patients after 2009 (CoCr group) were reviewed for changes in: Main Coronal Curvature Sagittal Balance (C7 Plumb Line) Kyphosis (T5-12). Results:. Thirteen were excluded because of incomplete radiographs. 38 patients received CoCr, 45 Ti and 4 patients received hybrid constructs. Correction rate of curves measuring >50 was significantly improved with CoCr (81% vs 69%, p=0.02). Sagittal balance was improved in both groups (CoCr 27.8, Ti 28.0 mm) but no significant difference was seen (p=0.84). Within the Ti group 12 patients moved for normal kyphosis (20–40) to abnormal (<20, >40) while 9 patients moved from abnormal to normal (p=0.66). Within the CoCr Group 10 patients were normalised while only 2 patients moved from normal to abnormal (p=0.04). Mean change in kyphosis showed a trend towards improved correction with CoCr (4.2 vs 2.9) but failed to reach significance (p=0.62). Discussion:. We have demonstrated that CoCr rods significantly improve coronal correction in patient with >50 curves. No difference in overall sagittal balance was seen between metal alloys. There is a trend towards better restoration of T5-12 kyphosis with CoCr however it is unclear if this small difference is clinically relevant. Aims

To evaluate the perioperative complications associated with total en bloc spondylectomy (TES) in patients with spinal tumours, based on the extent and level of tumour resection.

Methods

In total, 307 patients who underwent TES in a single centre were reviewed retrospectively. There were 164 male and 143 female patients with a mean age at the time of surgery of 52.9 years (SD 13.3). A total of 225 patients were operated on for spinal metastases, 34 for a malignant primary tumour, 41 for an aggressive benign tumour, and seven with a primary of unknown origin. The main lesion was located in the thoracic spine in 213, and in the lumbar spine in 94 patients. There were 97 patients who underwent TES for more than two consecutive vertebrae.

To compare the ability of fulcrum bend and traction radiographs to predict correction of AIS using screw only implants and to assess the fulcrum bending correction index (FBCI) with a new measurement: the traction correction index (TCI). Retrospective radiographic analysis of case series (Level IV). Radiographic correction of scoliosis based on correction rate does not take into consideration the curve flexibility. It has been suggested that fulcrum bending radiographs predict curve correction in AIS [1]. This has been questioned [2] and has been suggested that traction radiographs are more predictive in a mixed group of patients with hybrid and screw only constructs. Twenty three patients average age 15, who underwent posterior correction of scoliosis using pedicle screw only construct. Analysis was carried out on the pre-op and immediate post-op AP radiographs and the pre-op fulcrum bend and traction radiographs. Correction rate, fulcrum flexibility, traction flexibility, FBCI and TCI was calculated. Preoperative mean Cobb angle of 66 degrees was corrected to 25 degrees postoperatively. The mean fulcrum bending Cobb angle was 38 degrees and traction Cobb angle 28 degrees. The mean fulcrum flexibility was 45%, traction flexibility 59% and correction rate 63%. The mean FBCI was 182% and TCI was112%. When comparing fulcrum bend and traction radiographs, we found the latter to be more predictive of curve correction in AIS using pedicle screw constructs. The TCI better takes into account the curve flexibility than the FBCI

To describe a modification of the existing technique for C2 translaminar screw fixation that can be used for salvage in difficult cases. Bilateral crossing C2 laminar screws have recently become popular as an alternative technique for C2 fixation. This technique is particularly useful in patients with anomalous anatomy, as a salvage technique where other modes of fixation have failed or as a primary procedure. However, reported disadvantages of this technique include breach of the dorsal lamina and spinal canal, early hardware failure and difficulty in bone graft placement due to the position of the polyaxial screw heads. To address some of these issues, a modified technique is described. In this technique, the upper part of the spinous process of C2 is removed and the entry point of the screw is in the base of this removed spinous process. From October 2008 to March 2009, 6 patients underwent insertion of unilateral translaminar screws using our technique. The indications were: basilar invagination(three cases), C1/C2 fracture (two cases), tumour (one case). Age varied from 22 to 81 years (mean 48 years). All patients had post-operative x-ray and CT scan to assess position of the screws. Mean follow-up was 6 months. The screw position was satisfactory in all patients. There were no intraoperative or early postoperative complications. Our modification enables placement of bone graft on the C2 lamina and is also less likely to cause inadvertent cortical breach. Because of these advantages, it is especially suitable for patients with advanced rheumatoid arthritis with destruction of the lateral masses of C2 or as part of a hybrid construct in patients with unilateral high riding vertebral artery. This technique is not suitable for bilateral translaminar screw placement

To determine the current practice of scoliosis surgery in the UK. A 10 point questionnaire was constructed to identify the philosophy of surgeons on various aspects of scoliosis surgery such as choice of implant, bone graft, autologous blood transfusion (ABT), cord monitoring and computer assisted surgery. Results are compared with the current best evidence. Consultants and Fellows attending the 2009 British Scoliosis Society meeting. 50 questionnaires were completed: 45 Consultants and 5 Fellows. All pedicle screw construct favored by 25/50, hybrid 24/50 (one undecided). Posterior construct of less than 10 levels, 20/50 would not cross-link, 11/50 used one and 19/20 used two or more. More than ten levels 17/50 considered cross-links unnecessary, 4/50 used one and 29/50 used two or more. 88% preferred titanium alloy implants, while a mixture of stainless steel and cobalt chrome was used by others. For bone graft, substitutes (24), iliac crest (14), allograft (12) and demineralised bone matrix (9) in addition to local bone. 10/50 would use recombinant bone morphogenetic protein (3 for revision cases only). 39/50 routinely used intra-operative cell salvage or ABT drains and 4/50 never used autologous blood. All used cord monitoring, Sensory (19/50), Motor (2/50) and combined (29/50). None used computer-aided surgery. 26 operated alone 12 operated in pairs and 12 varied depending on type of case. This survey has brought to light interesting variations in scoliosis surgery in UK. It may reflect the conflicting evidence in the literature

Aims

Whether a combined anteroposterior fusion or a posterior-only fusion is more effective in the management of patients with Scheuermann’s kyphosis remains controversial. The aim of this study was to compare the radiological and clinical outcomes of these surgical approaches, and to evaluate the postoperative complications with the hypothesis that proximal junctional kyphosis would be more common in one-stage posterior-only fusion.

Methods

A retrospective review of patients treated surgically for Scheuermann’s kyphosis between 2006 and 2014 was performed. A total of 62 patients were identified, with 31 in each group. Parameters were compared to evaluate postoperative outcomes using chi-squared tests, independent-samples t-tests, and z-tests of proportions analyses where applicable.
