Objectives. There are few reports on total hip arthroplasty (THA) for hip osteoarthritis associated with so-called Perthes-like change including high great trochanter, short neck hip or flattened femoral head (hereinafter called “Perthes-like change”) as the operative procedures are difficult. We studied THA for “Perthes-like change” carried out in our department. Methods. We covered 14 cases (15 hips), which underwent THA for “Perthes-like change” (hereinafter called “Perthes-like change group,” operated from 2008 to September 2011. The average age at the operation was 62 (53 to 83 years old), 7 males and 7 females, and the average follow-up period was 21.8 months (6 to 48 months). For these cases we studied the clinical items and further made a comparative review of the 258 hips as a control group (Group C), which underwent THA during the same period for osteoarthritis (OA) originating in DDH (developmental dysplasia of the hip) (Crowe type 1 and 2), excluding the “Perthes-like change group.” The items reviewed include the age at the operation, operation time and intraoperative blood loss. Results. The average JOA score of the “Perthes-like change group” at the time of the study was 89 and favorable. The average operation time of the “Perthes-like change group” was 113 minutes (69 to 202 minutes) and its average intraoperative blood loss was 1066 g (490 to 3314 g). The operation time of the “Perthes-like change group” was significantly longer compared to that of the Group C (p=0.004), and its intraoperative blood loss was also significantly larger than that of the Group C (p=0.018). We carried out the muscle release operation for 8 hips (53.3%) of the “Perthes-like change group” and we combined the retachment of the great trochanter for the 1 case of them. There was no dislocation, infection, neuroparalysis and pulmonary embolism. Consideration. The “Perthes-like change group” had a longer operation time and a larger intraoperative blood loss than those of the Group C. Also there were quite a few cases that needed muscle release operation. Therefore, care should be taken in THA for hip osteoarthritis associated with “Perthes-like change” although the clinical results were favorable

Introduction. Hip osteoarthritis (OA) results in reduced hip range of motion and contracture, affecting sitting and standing posture. Spinal pathology such as fusion or deformity may alter the ability to compensate for reduced joint mobility in sitting and standing postures. The effects of postural spinal alignment change between sitting and standing is not well understood. Methods. A retrospective radiographic review was performed at a single academic institution of patients with sitting and standing full-body radiographs between 2012 and 2017. Patients were excluded if they had transitional lumbosacral anatomy, prior spinal fusion or hip prosthesis. Hip OA severity was graded by the Kellgren-Lawrence grades and divided into two groups: low-grade OA (LOA; grade 0–2) and severe OA (SOA; grade 3–4). Spinopelvic parameters (Pelvic Incidence (PI), Pelvic Tilt (PT), Lumbar Lordosis (LL), and PI-LL), Thoracic Kyphosis (TK; T4-T12), Global spinal alignment (SVA and T1-Pelvic Angle; TPA; T10-L2) as well as proximal femoral shaft angle (PFSA: as measured from the vertical), and hip flexion (difference between change in PT and change in PFSA) were also measured. Changes in sit-stand radiographic parameters were compared between the LOA and SOA groups with unpaired t-test. Results. 548 patients were identified with sit-stand radiographs, of which there were 311 patients with LOA & 237 with SOA. After propensity score matching for Age, BMI, and PI, 183 LOA & 183 SOA patients were analyzed. Standing alignment analysis demonstrated that SOA patients had greater SVA (31.1 ± 36.68 vs 21.7 ± 38.83, p=0.02), and lower TK (−36.21 ± 11.98 vs −41.09 ± 11.47, p<0.001). SOA patients had lower PT, greater PI-LL, lower LL, lower T10-L2, and lower TPA (p>0.05). PFSA (9.09 5.19 vs 7.41 4.48, p<0.001) was significantly different compared to LOA while SOA KA was not significantly different compared to LOA. Sitting alignment analysis demonstrated that SOA patients had higher PT (29.69 ± 15.65 vs 23.32 ± 12.12, p<0.001), higher PI-LL (21.64 ±17.86 vs 12.44 ±14.84 p<0.001), lower LL (31.67 ± 16.40 vs 41.58 ± 14.73, p<0.001), lower TK (−33.22 ± 15.76 vs −38.57 ± 13.01, p=0.01), greater TPA (27.91 ± 14.7 vs 22.55 ± 11.38 p=0.01). TK, SVA, and PFSA were not significantly different compared to LOA. SOA and LOA groups demonstrated differences in standing and sitting spinopelvic alignment for all global and regional parameters except PI. When examining the postural changes from standing to sitting, there was less hip ROM in SOA than LOA (71.45 ± 18.55 vs 81.64 ± 12.57, p<0.001). As a result, SOA patients had more change in PT (15.24 ± 16.32 vs 7.28 ± 10.19, p<0.001), PI-LL (20.62 ± 17.25 vs 13.74 ± 11.16, p<0.001), LL (−21.37 ± 15.55 vs −13.09 ± 12.34, p<0.001), and T10-L2 (−4.94 ± 7.45 vs −1.08 ± 5.19, p<0.001) to compensate. SOA had a greater improvement in TPA (15.06 vs 9.59, p<0.001), and less change in PFSA (86.65 vs 88.81, p<0.001) compared to LOA. Conclusions. Spinopelvic compensatory mechanisms are adapted for reduced joint mobility associated with hip OA in standing and sitting

Introduction. The advance of surgical technique and implant design have led to improvement in total hip arthroplasty (THA), and short stem THA is now gaining number as a treatment option for younger patients to preserve bone in the proximal femur for a future revision. The SMF stem is shorter stem, and requires a slightly higher neck resection and implanted in slight varus to contact at lateral cortex. Developmental dysplasia of the hip (DDH) is the most common cause to hip osteoarthritis (OA) in Japan, and the morphology of the dysplastic femur is narrow canal and increased anteversion. Thus, the purpose of this study is to evaluate the SMF stem design can fit for Japanese patients, using CT based 3-D template planning. Methods. We evaluated 30 patients who required THA in our institution. Inclusion criteria are hip OA, but cases with post-trauma, post-osteotomy, and any other hip disease in childhood are excluded. Patients were selected with their femoral anteversion, based on the dispersion of anteversion in dysplastic hip population, which was reported by Noble and collegues in 2003. Preoperative planning with ZedHip software (Lexi, Japan) was performed by established protocol. The center of socket was placed at 15mm proximal from teardrop and medialized to primary acetabulum, with 40° of radiographic inclination and 20° of anteversion. Neck resection of femur was 20mm proximal from the top of smaller trochanter, and stem was placed with lateral fit at distal and medial fit at calcar with appropriate size. Stem offset was selected by leg extension and balanced shenton line. Finally, ROM simulation was performed and the socket anteversion was arranged to achieve the optimal ROM. And then, SMF stem alignment and appropriate fitting was evaluated in Japanese OA cases. Results. Within 30 cases, 28 were female and 2 were male. Mean age was 66.0±9.6, and 8 (26.7%) were primary OA and 22 (73.3%) were secondary OA, from dysplastic hip. SMF stem was placed at average 2.7° varus and 7.2° flexion against femoral axis [Fig. 1]. The average degree of femoral anteversion was 34.0°±10.9 at pre-implantation and 22.7°±9.9 after implantation, which means average 11.4° retroverted [Fig. 2]. Stem offset and neck length variation was acceptable in 28/30 cases (93.3%). Finally, the mean flexion was 130°±7.1 and abduction was 56.0°±5.5, and optimal ROM was achieved 19/30 cases (63.3%). We cannot find any correlation between femoral anteversion pre-implantion and ROM post-implantation, however positive correlation was found between flexion angle and offset length after implantation (r2=0.48). Conclusion. The stem design of SMF is unique to preserve bone stock, but impossible to control anteversion after implantation, so it should be evaluated if it could fit to dysplastic hip. In this 3-D simulation study, SMF stem can be appropriately fitted in most of Japanese cases. Neck length and offset is important to achieve optimal ROM. Large diameter head (≥32mm) and high-offset stem would be suggested, and it may spread the indication for most of the cases regardless of the anteversion of femur. Purpose. Femoroacetabular impingement (FAI) may contribute to the development of early onset hip osteoarthritis (OA). A cam lesion (or pistol grip deformity) of the proximal femur reduces head-neck offset resulting in cam type FAI. The alpha angle is a radiographic measurement recommended for diagnosis of cam type FAI. The purpose of this study was to determine if patients that develop end stage hip OA prior to 55 years of age have radiographic evidence of cam type FAI. Method. The anteroposterior (AP) pelvis and lateral hip radiographs of 244 patients (261 hips) who presented to our institution for hip arthroplasty or hip fracture fixation between 2006 and 2008 were retrospectively reviewed. Three cohorts were compared: 1) patients with end stage hip OA < 55 years old (N=76); 2) patients with end stage hip OA > 55 years old (N=84); 3) hip fracture patients > 65 years old without radiographic evidence of hip arthritis were used as controls (N=101). Patients with inflammatory arthritis, avascular necrosis and post-traumatic hip OA were excluded. Alpha angles were measured on the AP pelvis and lateral radiographs by three coauthors using ImageJ 1.43 software (National Institutes of Health, USA). For patients with end stage hip OA, AP alpha angles were measured on both the hip with OA and the contralateral hip. Lateral alpha angles were measured only on the hip with OA. For patients with hip fracture, AP alpha angles were measured on the non-fractured hip and lateral alpha angles were measured on the fractured hip. A one-way ANOVA with post hoc Tukeys HSD test was used to compare the AP and lateral alpha angles for the three cohorts. Results. The intraclass correlation coefficient (ICC) for the three coauthors measuring AP and lateral alpha angles was 0.85 and 0.86 respectively, indicating excellent inter-rater agreement. Patients < 55 years old with end stage hip OA had the largest AP and lateral alpha angles (82.711.6 degrees AP and 63.918.5 degrees lateral). These angles were significantly larger (p<0.01 for both comparisons) than patients > 55 years old with end stage hip OA (71.717.8 degrees AP and 55.518.0 degrees lateral) and hip fracture patients without hip OA (52.710.9 degrees AP and 44.411.4 degrees lateral). Comparing AP alpha angles of the contralateral hips, the mean AP alpha angle for patients < 55 years old with hip OA (70.813.2 degrees) was significantly larger (p=0.04) than patients > 55 years old with hip OA (64.516.2 degrees) which in turn was significantly larger (p<0.01) than the hip fracture patients (52.710.9 degrees). Conclusion. Patients < 55 years old with hip OA had the largest mean AP and lateral alpha angles, significantly larger than patients > 55 years old with hip OA and hip fracture patients without hip OA. Thus young patients with end stage hip OA do have radiographic evidence of cam type FAI. Furthermore, this case-controlled study suggests that cam type FAI may contribute to the development of early onset hip OA

Introduction

Wearable sensors are promising tools for fast clinical gait evaluations in individuals with osteoarthritis (OA) of the knee and hip. However, gait assessments with wearable sensor are often limited to relatively simple straight-ahead walking paradigms. Parameters reflecting more complex and relevant aspects of gait, including dual-tasking, turning, and compensatory upper body motion are often overlooked in literature. The aim of this study was to investigate turning, dual-task performance, and upper body motion in individuals with knee or hip OA in addition to spatiotemporal gait parameters, taking shared covariance between gait parameters into account.

Methods

Gait was compared between individuals with unilateral knee (n=25) or hip (n=26) OA scheduled for joint replacement, and healthy controls (n=27). For 2 minutes, subjects walked back-and-forth a 6 meter trajectory making 180 degree turns, with and without a secondary cognitive task. Gait parameters were collected using four inertial measurement units on feet, waist, and trunk. To test if turning, dual-tasking, and upper body motion had added value above common spatiotemporal parameters, a factor analysis was conducted. Standardized mean differences were computed for the comparison between knee or hip OA and healthy controls. One gait parameter was selected per gait domain based on factor loading and effect size for the comparison between OA groups and healthy controls.

Spinopelvic mobility describes the change in lumbar lordosis and pelvic tilt from standing to sitting position. For 1° of posterior pelvic tilt, functional cup anteversion increases by 0.75° after total hip arthroplasty (THA). Thus, spinopelvic mobility is of high clinical relevance regarding the risk of implant impingement and dislocation.

Our study aimed to 1) determine the proportion of OA-patients with stiff, normal or hypermobile spino-pelvic mobility and 2) to identify clinical or static standing radiographic parameters predicting spinopelvic mobility.

This prospective diagnostic cohort study followed 122 consecutive patients with end-stage osteoarthritis awaiting THA. Preoperatively, the Oxford Hip Score, Oswestry Disability Index and Schober's test were assessed in a standardized clinical examination. Lateral view radiographs were taken of the lumbar spine, pelvis and proximal femur using EOS© in standing position and with femurs parallel to the floor in order to achieve a 90°-seated position. Radiographic measurements were performed for the lumbar lordosis angle (LL), sacral slope (SS), pelvic tilt (PT), pelvic incidence (PI) and pelvic-femoral-angle (PFA). The difference in PT between standing and seated allowed for patient classification based on spino-pelvic mobility into stiff (±30°).

From the standing to the sitting position, the pelvis tilted backwards by a mean of 19.6° (SD 11.6) and the hip was flexed by a mean of 57° (SD 17). Change in pelvic tilt correlated inversely with change in hip flexion.

Spinopelvic mobility is highly variable in patients awaiting THA and we could not identify any clinical or static standing radiographic parameter predicting the change in pelvic tilt from standing to sitting position. In order to identify patients with stiff or hypermobile spinopelvic mobility, we recommend performing lateral view radiographs of the lumbar spine, pelvis and proximal femur in all patients awaiting THA. Thereafter, implants and combined cup inclination/anteversion can be individually chosen to minimize the risk of dislocation.

No predictors could be identified. We recommend performing sitting and standing lateral view radiographs of the lumbar spine and pelvis to determine spinopelvic mobility in patients awaiting THA.

Aims

The aetiologies of common degenerative spine, hip, and knee pathologies are still not completely understood. Mechanical theories have suggested that those diseases are related to sagittal pelvic morphology and spinopelvic-femoral dynamics. The link between the most widely used parameter for sagittal pelvic morphology, pelvic incidence (PI), and the onset of degenerative lumbar, hip, and knee pathologies has not been studied in a large-scale setting.

Methods

A total of 421 patients from the Cohort Hip and Cohort Knee (CHECK) database, a population-based observational cohort, with hip and knee complaints < 6 months, aged between 45 and 65 years old, and with lateral lumbar, hip, and knee radiographs available, were included. Sagittal spinopelvic parameters and pathologies (spondylolisthesis and degenerative disc disease (DDD)) were measured at eight-year follow-up and characteristics of hip and knee osteoarthritis (OA) at baseline and eight-year follow-up. Epidemiology of the degenerative disorders and clinical outcome scores (hip and knee pain and Western Ontario and McMaster Universities Osteoarthritis Index) were compared between low PI (< 50°), normal PI (50° to 60°), and high PI (> 60°) using generalized estimating equations.

Melorheostosis is a very rare mesenchymal dysplasia of bone, characterized by sclerosing hyper-pigmentation appearances on the bone, may involve the adjacent soft tissues and lead to joint pain, limitation of joint motion and stiffness as a result of abnormal ossifications and soft tissue contractures, due to periarticular fibrosis. It is well known to tend to affect only one limb, but multifocal involvement, such as multiple limbs, spine and rib, has been extremely rarely reported. A variety of treatment options have been tried so far, none being specific surgical treatments.

Here we present a case of a 43-year-old man who sustained melorheostosis with multifocal involvement including the axial skeleton and a whole entire lower limb. He had painful swelling of his left lower limb and mainly complained of difficulty walking due to severe hip pain and knee stiffness, which persisted for 20 years and was aggravated during the last 5 years. Total hip arthropasty [Fig. 1] was done first, and then total knee arthroplasty [Fig. 2, 3] was performed. During operation, there were difficulties in bone cutting and implant insertion due to mixed pattern of hard sclerotic portion and osteoporotic portion despite complete synovectomy and sufficient soft tissue release. He was eventually free of pain during walking and able to walk without a crutch and joint motion of hip and knee was substantially improved after surgery.

We found that hip pain and contracture due to osteoarthritis and knee contracture secondary to multifocal melorheostosis could be successfully treated by total hip and knee arthroplasty. To the best of our knowledge, this is the first reporting the total joint arthroplasty performed in the patients with multifocal melorheostosis.

Psoas tendinopathy is a potential cause of groin pain after primary total hip arthroplasty (THA). The direct anterior approach (DAA) is becoming increasingly popular as the standard approach for primary THA due to being a muscle preserving technique. It is unclear what the prevalence is for the development of psoas-related pain after DAA THA, how this can influence patient reported outcome, and which risk factors can be identified. This retrospective case control study of prospectively recorded data evaluated 1784 patients who underwent 2087 primary DAA THA procedures between January 2017 and September 2019. Psoas tendinopathy was defined as (1) persistence of groin pain after DAA THA and was triggered by active hip flexion, (2) exclusion of other causes such as dislocation, infection, implant loosening or (occult) fractures, and (3) a positive response to an image-guided injection with xylocaine and steroid into the psoas tendon sheath. Complication-, re-operation rates, and patient-reported outcome measures (PROMs) were measured. Forty-three patients (45 hips; 2.2%) were diagnosed with psoas tendinopathy according to the above-described criteria. The mean age of patients who developed psoas tendinopathy was 50.8±11.7 years, which was significantly lower than the mean age of patients without psoas pain (62.4±12.7y; p<0.001). Patients with primary hip osteoarthritis were significantly less likely to develop psoas tendinopathy (14/1207; 1.2%) in comparison to patients with secondary hip osteoarthritis to dysplasia (18/501; 3.6%) (p<0.001) or FAI (12/305; 3.9%) (p<0.001). Patients with psoas tendinopathy had significantly lower PROM scores at 6 weeks and 1 year follow-up. Psoas tendinopathy was present in 2.2% after DAA THA. Younger age and secondary osteoarthritis due to dysplasia or FAI were risk factors for the development of psoas tendinopathy. Post-operatively, patients with psoas tendinopathy often also presented with low back pain and lateral trochanteric pain. Psoas tendinopathy had an important influence on the evolution of PROM scores

Simultaneous bilateral total hip arthroplasty (THA) in patients with bilateral hip osteoarthritis is gradually becoming attractive, as it requires a single anesthesia and hospitalization. However, there are concerns about the potential complications following this surgical option. The purpose of this study is to compare the short-term major and minor complications and assess the readmission rate, between patients treated with same-day bilateral THA and those with staged procedures within a year. We retrospectively reviewed the charts of all patients with bilateral hip osteoarthritis that underwent simultaneous or staged (within a year) bilateral total THA in our institution, between 2016-2020. Preoperative patient variables between the two groups were compared using the 2-sample t-test for continuous variables, the Fisher's exact test for binary variables, or the chi-square test for multiple categorical variables. Similarly, differences in the 30-day major and minor complications and readmission rates were assessed. A logistic regression model was also developed to identify potential risk factors. A total of 160 patients (mean age: 64.3 years, SD: ±11.7) that underwent bilateral THA was identified. Seventy-nine patients were treated with simultaneous and eighty-one patients with staged procedures. There were no differences in terms of preoperative laboratory values, gender, age, Body Mass Index (BMI), or American Society of Anesthesiologists Scores (ASA) (p>0.05) between the two groups. Patients in the simultaneous group were more likely to receive general anesthesia (43% vs 9.9%, p0.05). After controlling for potential confounders, the multivariable logistic regression analysis showed similar odds of having a major (odds ratio 0.29, 95% confidence interval [0.30-2.88], p=0.29) or minor (odds ratio 1.714, 95% confidence interval [0.66-4.46], p=0.27) complication after simultaneous compared to staged bilateral THA. No differences in emergency department visits or readmission for reasons related to the procedure were recorded (p>0.05). This study shows that similar complication and readmission rates are expected after simultaneous and staged THAs. Simultaneous bilateral THA is a safe and effective procedure, that should be sought actively and counselled by surgeons, for patients that present with radiologic and clinical bilateral hip disease

Obesity is a known risk factor for hip osteoarthritis. The aim of this study was to compare the incidence of obesity in Australians undergoing hip replacements (HR) for osteoarthritis to the general population. A cohort study was conducted comparing data from the Australian Bureau of Statistics and the Australian Orthopaedic Association National Joint Replacement Registry (AOANJRR) from 2017-18. Body mass index (BMI) data for patients undergoing primary total hip replacement and resurfacing for osteoarthritis were obtained from the AOANJRR. The distribution of HR patients by BMI category was compared to the general population, in age and sex sub-groups. During the study period, 32,495 primary HR were performed for osteoarthritis in Australia. Compared to the general population, there was a higher incidence of Class I, II, and III obesity in patients undergoing HR in both sexes aged 35 to 74 years old. Class III obese females and males undergoing HR were on average 6 to 7 years younger than their normal weight counterparts. Class III obese females and males aged 55-64 years old were 2.9 and 1.7 times more likely to undergo HR, respectively (p<0.001). There is a strong association between increased BMI and relative risk of undergoing HR. Similar findings have been noted in the United States of America, Canada, United Kingdom, Sweden and Spain. A New Zealand Registry study and recent meta-analysis have also found a concerning trend of Class III obese patients undergoing HR at a younger age. Obese Australians are at increased risk of undergoing HR at a younger age. A national approach to address the prevalence of obesity is needed

The presence of hip osteoarthritis is associated with abnormal spinopelvic characteristics. This study aims to determine whether the pre-operative, pathological spinopelvic characteristics “normalize” at 1-year post-THA. This is a prospective, longitudinal, case-control matched cohort study. Forty-seven patients underwent pre- and post- (at one-year) THA assessments. This group was matched (age, sex, BMI) with 47 controls/volunteers with well-functioning hips. All participants underwent clinical and radiographic assessments including lateral radiographs in standing, upright-seated and deep-flexed-seated positions. Spinopelvic characteristics included change in lumbar lordosis (ΔLL), pelvic tilt (ΔPT) and hip flexion (pelvic-femoral angle, ΔPFA) when moving from the standing to each of the seated positions. Spinopelvic hypermobility was defined as ΔPT>30° between standing and upright-seated positions. Pre-THA, patients illustrated less hip flexion (ΔPFA −54.8°±17.1° vs. −68.5°± 9.5°, p<0.001), greater pelvic tilt (ΔPT 22.0°±13.5° vs. 12.7°±8.1°, p<0.001) and greater lumbar movements (ΔLL −22.7°±15.5° vs. −15.4°±10.9°, p=0.015) transitioning from standing to upright-seated. Post-THA, these differences were no longer present (ΔPFApost −65.8°±12.5°, p=0.256; ΔPTpost 14.3°±9.5°, p=0.429; ΔLLpost −15.3°±10.6°, p=0.966). The higher prevalence of pre-operative spinopelvic hypermobility in patients compared to controls (21.3% vs. 0.0%; p=0.009), was not longer present post-THA (6.4% vs. 0.0%; p=0.194). Similar results were found moving from standing to deep-seated position post-THA. Pre-operative, spinopelvic characteristics that contribute to abnormal mechanics can normalize post-THA following improvement in hip flexion. This leads to patients having the expected hip-, pelvic- and spinal flexion as per demographically-matched controls, thus potentially eliminating abnormal mechanics that contribute to the development/exacerbation of hip-spine syndrome

Total hip arthroplasty (THA) is an effective treatment for symptomatic hip osteoarthritis (OA). While computer-navigation technologies in total knee arthroplasty show survivorship advantages and are widely used, comparable applications within THA show far lower utilisation. Using national registry data, this study compared patient reported outcome measures (PROMs) in patients who underwent THA with and without computer navigation. Data from Australian Orthopaedic Association National Joint Replacement Registry (AOANJRR) PROMs program included all primary THA procedures performed for OA up to 31 December 2020. Procedures using the Intellijoint HIP® navigation system were identified and compared to procedures using other computer navigation systems or conventional instrumentation only. Changes in PROM scores between pre-operative and 6-month post-operative time points were analysed using multiple regression model, adjusting for pre-operative score, patient age, gender, ASA score, BMI, surgical approach, and hospital type. There were 65 primary THA procedures that used the Intellijoint HIP® system, 90 procedures used other types of computer navigation, and the remaining 5,284 primary THA procedures used conventional instrumentation. The estimated mean changes in the EuroQol visual analogue scale (EQ VAS) score and Oxford Hip score did not differ significantly when Intellijoint® was compared to conventional instruments (estimated differences of 2.4, 95% CI [-1.7, 6.5], p = 0.245, and −0.5, 95% CI [-2.5, 1.4], p = 0.592, respectively). The proportion of patients who were satisfied with their procedure was also similar when Intellijoint® was compared to conventional instruments (rate ratio 1.06, 95% CI [0.97, 1.16], p = 0.227). The preliminary data demonstrate no significant difference in PROMs when comparing the Intellijoint HIP® THA navigation system with both other navigation systems and conventional instrumentation for primary THAs performed for OA. Total hip arthroplasty (THA) has high rates of patient satisfaction; however patient expectations for recreational and sporting activities are not always met. Our study aimed to identify preoperative factors that predict whether patient expectations for sporting or recreational activity are met 12 months following THA. Patient reported outcome measures (PROMs) were collected prospectively from 2015-2018 at one private hospital in Sydney. Age, gender, postcode, weight, and height were recorded preoperatively. Included participants underwent primary THA by one of the investigating surgeons. Univariable and multivariable analyses were performed with an expectation fulfilment score used as the primary outcome variable. Preoperative predictor variables included: age, gender, BMI, Socio-economic Indexes for Areas (SEIFA), Oxford Hip Score, Hip Osteoarthritis Outcome Score, EQ-5D-5L and EQ Visual Analogue Scale (EQ VAS). 1019 participants were eligible and included. 13% reported that preoperative expectations of sport or recreation were not met at 12 months. Younger age, lower preoperative EQ VAS, and higher BMI were associated with failed expectations on multivariable analysis. Odds of failed expectations increased by 2% for every one year younger in age (OR= 0.98, 95% CI = 0.96 to 1.00, p=.048), by 2% for every one point lower on EQ VAS (OR=0.98, 95% CI = 0.98 to 0.99, p=.002), and by 4% for every one-point increase in BMI (OR = 1.04, 95% CI = 1.00 to 1.09, p=.042). Failure to have expectations met for sporting or recreational activity was associated with younger age, poorer general health, and high BMI. With a rise in younger patients who likely have higher physical demands, a tailored preoperative education is preferable to generic models to better manage patient expectations. Younger age, higher BMI, and poorer health may predict unmet expectations for sport and recreation after THA. Tailored education in these groups should be considered

Total hip arthroplasty (THA) is an effective treatment for symptomatic hip osteoarthritis (OA). Computer-navigation technologies in total knee arthroplasty show evidence-supported survivorship advantages and are used widely. The aim of this study was to determine the revision outcome of hip commercially available navigation technologies. Data from the Australian Orthopaedic Association National Joint Replacement Registry from January 2016 to December 2020 included all primary THA procedures performed for osteoarthritis (OA). Procedures using the Intellijoint HIP® navigation were identified and compared to procedures inserted using ‘other’ computer navigation systems and to all non-navigated procedures. The cumulative percent revision (CPR) was compared between the three groups using Kaplan-Meier estimates of survivorship and hazard ratios (HR) from Cox proportional hazards models, adjusted for age and gender. A prosthesis specific analysis was also performed. There were 1911 procedures that used the Intellijoint® system, 4081 used ‘other’ computer navigation, and 160,661 were non-navigated. The all-cause 2-year CPR rate for the Intellijoint HIP® system was 1.8% (95% CI 1.2, 2.6), compared to 2.2% (95% CI 1.8, 2.8) for other navigated and 2.2% (95% CI 2.1, 2.3) for non-navigated cases. A prosthesis specific analysis identified the Paragon/Acetabular Shell THAs combined with the Intellijoint HIP® system as having a higher (3.4%) rate of revision than non-navigated THAs (HR = 2.00 (1.01, 4.00), p=0.048). When this outlier combination was excluded, the Intellijoint® system group demonstrated a two-year CPR of 1.3%. There was no statistical difference in the CPR between the three groups before or after excluding Paragon/Acetabular Shell system. The preliminary data presented demonstrate no statistical difference in all cause revision rates when comparing the Intellijoint HIP® THA navigation system with ‘other’ navigation systems and ‘non-navigated’ approaches for primary THAs performed for OA. The current sample size remains too small to permit meaningful subgroup statistical comparisons

This study used model-based radiostereometric analysis (MBRSA) to compare migration of a recently introduced cementless hip stem to an established hip stem of similar design. Novel design features of the newer hip stem included a greater thickness of hydroxyapatite coating and a blended compaction extraction femoral broach. Fifty-seven patients requiring primary total hip arthroplasty (THA) were enrolled at a single centre. Patients were randomized to receive either an Avenir collarless stem and Trilogy IT cup (ZimmerBiomet) or a Corail collarless stem and Pinnacle cup (DePuy Synthes) via a posterior or lateral approach. Both stems are broach-only femoral bone preparation. RSA beads (Halifax Biomedical) were inserted into the proximal femur during surgery. Patients underwent supine RSA imaging a 6 weeks (baseline), 6, 12, and 24 months following surgery. The primary study outcome was total subsidence of the hip stem from baseline to 24 months as well as progression of subsidence between 12 and 24 months. These values were compared against published migration thresholds for well-performing hip stems (0.5mm). The detection limit, or precision, of MBRSA was calculated based on duplicate examinations taken at baseline. Patient reported outcome measures were collected throughout the study and included the Oxford-12 Hip Score (OHS), EuroQoL EQ-5D-5L, Hip Osteoarthritis Score (HOOS) as well as visual analogue scales (VAS) for thigh pain and satisfaction. Analysis comprised of paired and unpaired t-tests with significance set at p≤0.05. Forty-eight patients (30 males) were included for analysis; 7 patients received a non-study hip stem intra-operatively, 1 patient suffered a traumatic dislocation within three weeks of surgery, and 1 patient died within 12 months post-surgery. RSA data was obtained for 45 patients as three patients did not receive RSA beads intra-operatively. Our patient cohort had a mean age of 65.9 years (±;7.2) at the time of surgery and body mass index of 30.5 kg/m2 (±;5.2). No statistical difference in total stem migration was found between the Avenir and Corail stems at 12 months (p=0.045, 95%CI: −0.046 to 0.088) and 24 months (p=0.936, 95% CI: −0.098 to 0.090). Progression of subsidence from 12-24 months was 0.011mm and 0.034mm for the Avenir and Corail groups which were not statistically different (p=0.163, 95%CI: −0.100 to 0.008) between groups and significantly less than the 0.5mm threshold (pNo statistically significant differences existed between study groups for any pre-operative function scores (p>0.05). All patients showed significant functional improvement from pre- to post-surgery and no outcome measures were different between study groups with exception of EQ-5D-5L health visual analogue scale at 12 months which showed marginally superior (p=0.036) scores in the Avenir group. This study was not powered to detect differences in clinical outcomes. This study has demonstrated no statistical difference in subsidence or patient-reported outcomes between the Corail hip stem and the more recently introduced Avenir hip stem. This result is predictable as both stems are of a triple-tapered design, are coated with hydroxyapatite, and utilize a broach-only bone preparation technique. Both stem designs demonstrate migration below 0.5mm suggesting both are low-risk for aseptic loosening in the long-term

Femoroacetabular impingement (FAI) – enlarged, aspherical femoral head deformity (cam-type) or retroversion/overcoverage of the acetabulum (pincer-type) – is a leading cause for early hip osteoarthritis. Although anteverting/reverse periacetabular osteotomy (PAO) to address FAI aims to preserve the native hip and restore joint function, it is still unclear how it affects joint mobility and stability. This in vitro cadaveric study examined the effects of surgical anteverting PAO on range of motion and capsular mechanics in hips with acetabular retroversion. Twelve cadaveric hips (n = 12, m:f = 9:3; age = 41 ± 9 years; BMI = 23 ± 4 kg/m2) were included in this study. Each hip was CT imaged and indicated acetabular retroversion (i.e., crossover sign, posterior wall sign, ischial wall sign, retroversion index > 20%, axial plane acetabular version < 15°); and showed no other abnormalities on CT data. Each hip was denuded to the bone-and-capsule and mounted onto a 6-DOF robot tester (TX90, Stäubli), equipped with a universal force-torque sensor (Omega85, ATI). The robot positioned each hip in five sagittal angles: Extension, Neutral 0°, Flexion 30°, Flexion 60°, Flexion 90°; and performed hip internal-external rotations and abduction-adduction motions to 5 Nm in each position. After the intact stage was tested, each hip underwent an anteverting PAO, anteverting the acetabulum and securing the fragment with long bone screws. The capsular ligaments were preserved during the surgery and each hip was retested postoperatively in the robot. Postoperative CT imaging confirmed that the acetabular fragment was properly positioned with adequate version and head coverage. Paired sample t-tests compared the differences in range of motion before and after PAO (CI = 95%; SPSS v.24, IBM). Preoperatively, the intact hips with acetabular retroversion demonstrated constrained internal-external rotations and abduction-adduction motions. The PAO reoriented the acetabular fragment and medialized the hip joint centre, which tightened the iliofemoral ligament and slackenend the pubofemoral ligament. Postoperatively, internal rotation increased in the deep hip flexion positions of Flexion 60° (∆IR = +7°, p = 0.001) and Flexion 90° (∆IR = +8°, p = 0.001); while also demonstrating marginal decreases in external rotation in all positions. In addition, adduction increased in the deep flexion positions of Flexion 60° (∆ADD = +11°, p = 0.002) and Flexion 90° (∆ADD = +12°, p = 0.001); but also showed marginal increases in abduction in all positions. The anteverting PAO restored anterosuperior acetabular clearance and increased internal rotation (28–33%) and adduction motions (29–31%) in deep hip flexion. Restricted movements and positive impingement tests typically experienced in these positions with acetabular retroversion are associated with clinical symptoms of FAI (i.e., FADIR). However, PAO altered capsular tensions by further tightening the anterolateral hip capsule which resulted in a limited external rotation and a stiffer and tighter hip. Capsular tightness may still be secondary to acetabular retroversion, thus capsular management may be warranted for larger corrections or rotational osteotomies. In efforts to optimize surgical management and clinical outcomes, anteverting PAO is a viable option to address FAI due to acetabular retroversion or overcoverage

Background. The aim of this study is to analysis the ability of these patients, treated with MOMHR, to resume sport activities by gait analysis and clinical evaluations. Metal on metal hip resurfacing (MOMHR) is indicated to treat symptomatic hip osteoarthritis in young active patients. These patients require a high level of function and desire to resume sport activities after surgery. Study Design & Methods. 30 consecutive male patients playing high impact sports with unilateral hip osteoarthritis and normal contralateral hip were included in the study, they were treated with MOMHR by the same surgeon. No patients were lost to follow. The mean age at operation was 39.1 years (range 31 to 46). Primary diagnosis was osteoarthritis. OHS, HHS, UCLA activity score were completed at pre-operative time, six months and one year after surgery. Functionally, gait analysis was performed in all patients 6 months and one year after surgery. A stereophotogrammetric system (Smart-DX, BTS, Milano, Italy, 10 cameras, 250Hz) and two platforms (9286BA Kistler Instrumente AG, Switzerland) were used. Cluster of 4 markers were attached on the skin of each bone segment, a number of anatomical landmarks were calibrated and segment anatomical frames defined, markers were positioned by the same operator. Walking, running and squat jump were analyzed and strength and range of movement of the hips and knees were calculated. Results. At follow-up times the survival rate for the whole cohort was 100%. The mean pre-op OHS was 28.1 points (range 15.0 to 38.0), at 6 months after surgery was 44.5 points (range 44 to 48), at one year after surgery was 47.9 points (range 45 to 48). The mean pre-op HHS was 54.7 points (range 33.1 to 73.4), at 6 months after surgery was 96.7 points (range 93.4 to 100), at one year after surgery was 99.7 points (range 95.7 to 100). The mean pre-op UCLA activity score was 2.7 (range 2 to 4), at 6 months after surgery was 7.4 (range 5 to 10), at one year after surgery was 8.6 (range 7 to 10). At 6 months after surgery, patients showed a reduction of the differences between the operated and the contralateral side during walking, running and squat jumping. (p<0.01). One year after the operation there were no differences. At 3 months after surgery the mean hip flexion extension range of motion was in the normal hips 41±1.7 and in the operated hips 37.3±2.1; at 6 months after surgery the mean hip flexion extension range of motion was in the normal hips 45.4±1.8 and in the operated hips 42.0±1.7; At 1 year after surgery the hip flexion extension range of motion was in the normal hips 42.9±1.7 and in the operated hips 45.5±1.4. (p=0.001). Conclusions. Our gait analysis study shows that the biomechanical function of the operated hip is completely recovered 1 year after MOMHR operation. As a consequence sport activities can be successfully resumed. MOMHR is a good choice for young and active patients affected by hip osteoarthritis requiring a high level of activity

Femoroacetabular impingement (FAI) deformities are a potential precursor to hip osteoarthritis and an important contributor to non-arthritic hip pain. Some hips with FAI deformities develop symptoms of pain in the hip and groin that are primarily position related. The reason for pain generation in these hips is unclear. Understanding potential impingement mechanisms in FAI hips will help us understand pain generation. Impingement between the femoral head-neck contour and acetabular rim has been proposed as a pathomechanism in FAI hips. This proposed pathomechanism has not been quantified with direct measurements in physiological postures. Research question: Is femoroacetabular clearance different in symptomatic FAI hips compared to asymptomatic FAI and control hips in sitting flexion, adduction, and internal rotation (FADIR) and squatting postures?. We recruited 33 participants: 9 with symptomatic FAI, 13 with asymptomatic FAI, and 11 controls from the Investigation of Mobility, Physical Activity, and Knowledge Translation in Hip Pain (IMAKT-HIP) cohort. We scanned each participant's study hip in sitting FADIR and squatting postures using an upright open MRI scanner (MROpen, Paramed, Genoa, Italy). We quantified femoroacetabular clearance in sitting FADIR and squatting using beta angle measurements which have been shown to be a reliable surrogate for acetabular rim pressures. We chose sitting FADIR and squatting because they represent, respectively, passive and active maneuvers that involve high flexion combined with internal/external rotation and adduction/abduction, which are thought to provoke impingement. In the squatting posture, the symptomatic FAI group had a significantly smaller minimum beta angle (−4.6º±15.2º) than the asymptomatic FAI (12.5º ±13.2º) (P= 0.018) and control groups (19.8º ±8.6º) (P=0.001). In the sitting FADIR posture, both symptomatic and asymptomatic FAI groups had significantly smaller beta angles (−9.3º ±14º [P=0.010] and −3.9º ±9.7º [P=0.028], respectively) than the control group (5.7º ±5.7º). Our results show loss of clearance between the femoral head-neck contour and acetabular rim (negative beta angle) occurred in symptomatic FAI hips in sitting FADIR and squatting. We did not observe loss of clearance in the asymptomatic FAI group for squatting, while we did observe loss of clearance for this group in sitting FADIR. These differences may be due to accommodation mechanisms in the active, squatting posture that are not present in the passive, sitting FADIR posture. Our results support the hypothesis that impingement between the femoral head-neck contour and acetabular rim is a pathomechanism in FAI hips leading to pain generation

To evaluate the short-term clinical outcomes of patients treated arthroscopically with chitin-based scaffolding for acetabular chondral defects in conjunction with microfracture compared to microfracture alone. This study is a retrospective analysis of prospectively collected data. A review of charts was performed (2014–2016) on all patients who underwent hip arthroscopy and had microfracture +/− scaffolding for acetabular chondral defects, intraoperative details (lesion size, grade, labral repair/reconstruction) and postoperative complications were recorded with a minimum follow-up of 2 years. Clinical outcomes were assessed by analysing iHOT and HOS scores which were obtained pre-operatively, at six months, one year and two years post-surgery. Plain radiographs were assessed for hip osteoarthritis by Kellgren & Lawrence grading. A total of 60 patients (microfracture=25, scaffolding=35) were included. Patients had a mean age of 36.2 years at the time of the index operation. There were no major adverse events of deep vein 36.2 years at the time of the index operation. There were no major adverse events of deep vein thrombosis, blood vessel or nerve damage, hemarthrosis or device related adverse events in both groups. Two patients were readmitted due to pain as a result of an inflammatory reaction in the scaffolding group. Both treatments of microfracture and scaffolding showed significant improvement in outcome score (iHOT) (p < 0 .001) when compared postoperative to preoperative. Both the arthroscopic treatment of chondral acetabular defects with chitin based scaffolding and microfracture demonstrated significant improvement from their pre-operative outcomes
