This study describes a safe endoscopic technique for decompression of trochanteric bursa and presents the results of this procedure. Fifteen patients who had failed non-operative treatment for trochanteric bursitis were treated by endoscopic lengthening of fascia lata (FL) and trochanteric bursectomy. Two patients had also failed open decompressions performed at another institution prior to their endoscopic surgery. All patients took part in questionnaires pre-operatively and at three months and twelve months. A two-portal endoscopic procedure was performed in all subjects. A Cruciate incision was made in the FL hence lengthening it in three dimensions. A trochanteric bursectomy was then performed using a mechanical shaver. No patients were lost to follow up. At last review 14 patients rated their result as excellent and one patient had a fair result. There were no complications. The modified Harris hip score improved from 45 to 60, Non-arthritic hip score improved from 45 to 64 and SF12 score improved from 31 to 34. Endoscopic lengthening of FL and trochanteric bursectomy is a safe and effective procedure in relieving the symptoms of persistent trochanteric bursitis

Introduction. Trochanteric bursitis is described as pain over the lateral side of the hip and thigh. The usual treatment is rest, administration of NSAIDs and injection of local anaesthetic and corticosteroid. Occasionally it is resistant to these measures. Background. Whilst performing an ablation of the trochanteric bursa we noticed that the fascia lata looked particularly tight. Therefore a z-plasty of the fascia lata was performed. Methods. Patients were identified from the senior author's logbook. Retrospective case note review and telephone interview study was performed. All patients who have undergone this procedure between October 2004 and September 2006 are included. They had all failed a regime of 8/52 physiotherapy and NSAID followed by x3 injections 8/52 apart. The visual analogue pain scoring system was used to assess all the patients post-operatively. Complete resolution of pain was seen as an excellent result. A score of 1-2 was regarded as good, and 3 or above as poor. All were followed up in the outpatients until they had returned to normal function. Results. We present 15 patients with a mean follow-up of 14 (4-27) months. Mean age 50 (18-76). 10 were women. All were unilateral. 13 excellent and 2 good results. All returned to normal function. There was 1 superficial infection. No patients had a snapping IT band. Discussion. Trochanteric bursitis is a common condition with an incidence in primary care of as 1.8 patients per 1000 per year. It is more common in females (80%). Trochanteric bursitis is commonly due to repetitive trauma with repetitive irritation of the bursa by the fascia lata during walking. Therefore by elongating the fascia lata slightly you can prevent this repetitive trauma. Conclusion. Z-plasty of the fascia lata is a procedure that can be used for the treatment of intractable trochanteric bursitis

Anterior surgical approaches for total hip arthroplasty (THA) have increased popularity due to expected faster recovery and less pain. However, the direct anterior approach (Heuter approach which has been popularised by Matta) has been associated with a higher rate of early revisions than other approaches due to femoral component loosening and fractures. It is also noted to have a long learning curve and other unique complications like anterior femoral cutaneous and femoral nerve injuries. Most surgeons performing this approach will require the use of an expensive special operating table. An alternative to the direct anterior approach is the anterior-based muscle-sparing approach. It is also known as the modified Watson-Jones approach, anterolateral muscle-sparing approach, minimally invasive anterolateral approach and the Röttinger approach. With this technique, the hip joint is approached through the muscle interval between the tensor fascia lata and the gluteal muscles, as opposed to the direct anterior approach which is between the sartorius and rectus femoris and the tensor fascia lata. This approach places the femoral nerve at less risk for injury. I perform this technique in the lateral decubitus position, but it can also be performed in the supine position. An inexpensive home-made laminated L-shaped board is clamped on end of table allowing the ipsilateral leg to extend, adduct, and externally rotate during the femoral preparation. This approach for THA has been reported to produce excellent results. One study reports a complication rate of 0.6% femoral fracture rate and 0.4% revision rate for femoral stem loosening. In a prospective randomised trial looking at the learning curve with new approach, the anterior-based muscle-sparing anterior approach had lower complications than a direct anterior approach. The complications and mean operative time with this approach are reported to be no different than a direct lateral approach. Since this surgical approach is not through an internervous interval, a concern is that this may result in a permanent functional defect as result of injury to the superior gluteal nerve. At a median follow-up of 9.3 months, a MRI study showed 42% of patients with this approach had fat replacement of the tensor fascia lata, which is thought to be irreversible. The clinical significance remains unclear, and inconsequential in my experience. A comparison MRI study showed that there was more damage and atrophy to the gluteus medius muscle with a direct lateral approach at 3 and 12 months. My anecdotal experience is that there is faster recovery and less early pain with this approach. A study of the first 57 patients I performed showed significantly less pain and faster recovery in the first six weeks in patients performed with the anterior-based muscle-sparing approach when compared to a matched cohort of THA patients performed with a direct lateral approach. From 2004 to 2017, I have performed 1308 total hip replacements with the anterior-based muscle sparing approach. Alternatively, I will use the direct lateral approach for patients with stiff hips with significant flexion and/or external rotation contractures where I anticipate difficulty with femoral exposure, osteoporotic femurs due to increased risk of intraoperative trochanteric fractures, previously operated hips with scarring or retained hardware, and Crowe III-IV dysplastic hips when there may be a need for a femoral shortening or derotational osteotomy. Complications have been very infrequent. This approach is a viable alternative to the direct anterior approach for patients desiring a fast recovery. The anterior-based muscle-sparing approach is the approach that I currently use for all outpatient total hip surgeries

Introduction:. Purpose of mini-invasive hip arthroplasty, is least damage to skin and muscles. Unlike Roettinger modification to Watson-Jones, our approach requires no special table or instruments. Besides, direction of skin incision is perpendicular to interval between glutei and tensor muscles, thus called a Crisscross Approach. Incision is at direction of retractors causing less skin damage; and parallel to femur allowing expansion. No tendon or muscles are severed achieving a true inter-muscular non invasive approach. Unlike anterior approach, femoral circumflex vessels and lateral femoral cutaneous nerve are spared. Material and Methods:. 240 prospective patients underwent mini-invasive crisscross technique since December 2006. A standard non-cemented hip was implanted. Previously disrupted hip muscles patients were excluded. In the operating room, patients were secured in a lateral decubitus position with the pelvis flexed at 20°–30° to allow operated leg to extend beyond the table to be placed in a standard plastic bag. The anterior superior iliac spine (ASIS), the greater trochanter (GT) and its tubercle are identified and marked. A line is drawn between ASIS and GT tubercle representing the interval between the glutei and the tensor fascia lata muscles. Another line representing the skin incision is drawn perpendicular. It may be curved a little toward the femur starting two inches inferior and posterior to ipsilateral ASIS extending distally for 3 inches or more for obese or muscular patients. The Crisscross Approach starts with a skin incision being made as above and through the subcutaneous fat identifying the inter-muscular interval between the glutei and the tensor fascia lata. Sharp dissection is made in the connecting fascia only and blunt dissection is needed to separate the two muscles. A branch of the superior gluteal nerve proximally crossing from the glutei to the tensor fascia lata may be encountered but it should not be disturbed as long as blunt dissection is maintained. Curved retractors are placed one above and the other below the femoral neck exposing the anterior capsule. Incision is made in the capsule and the retractors are re-placed to better expose the femoral neck. The appropriate level of neck is osteotomized and the head is extracted as routine. Acetabulum is further exposed by placing the curved retractors at about mid anterior and mid posterior. The final appropriate cup size and orientation is implanted routinely. Before exposing the femoral canal the deep fascia at the junction of the glutei and the vastus lateralis should be incised (about 2–3 inches). This will tremendously help femoral canal exposure. Then the surgeon is positioned anterior, the patient is made fully paralyzed and the table is tilted 20°–30° posteriorly (away from the surgeon). Hip extended 20°–30°, externally rotated to 80°–90° and adducted with a retractor underneath femoral neck and a curved one on greater trochanter to protect the glutei. Leg is allowed to drop in a bag (posteriorly). Canal finder is helpful to avoid going through the cortex Broaching or reaming and final implant insertion as routine. Posterior capsule need not be disturbed; however, the superior and inferior capsule should be detached from the neck to allow better exposure of the femoral canal. Closure starts with one or two stitches in the remainder of the capsule. Then suture deep fascia at the junction of glutei and vastus lateralis with absorbable suture. Finally, subcutaneous fat and skin are closed as routine. Results:. There was no major neurovascular damage or complications related to this exposure. Follow up to a maximum of 75 months revealed no deep infection and no dislocation or fracture. We undersized three stems at the beginning but that did not require re-operation. Surgery time averaged 15 minutes longer but that was reduced as we gained experience. Cup position was navigated and postoperative CT scan confirmed satisfactory cup and stem position. One case that had an undersized prosthesis settling to about half an inch short which required a 3/8 inch insole for the patient to wear. Rehab goals were met after 4–6 sessions in all patients. Patients were allowed to go home on two crutches in 2–3 days. Full weight bearing was allowed in 2–4 weeks. No limping noted at 3 months follow up. Discussion:. Crisscross approach differs by transecting no tendon or muscles, requiring no special table or instruments with incision parallel to femur facilitating expansion and reducing skin damage resulting in true non-invasive approach

Aim. Reconstruction of composite soft-tissue defects with extensor apparatus deficiency in patients with periprosthetic joint infection (PJI) of the knee is challenging. We present a single-centre multidisciplinary orthoplastic treatment concept based on a retrospective outcome analysis over 20 years. Method. One-hundred sixty-seven patients had PJI after total knee arthroplasty. Plastic surgical reconstruction of a concomitant perigenicular soft-tissue defect was indicated in 49 patients. Of these, seven presented with extensor apparatus deficiency. Results. One patient underwent primary arthrodesis and six patients underwent autologous reconstruction of the extensor apparatus. The principle to reconstruct missing tissue ‘like with like’ was thereby favoured: Two patients with a wide soft-tissue defect received a free anterolateral thigh flap with fascia lata; one patient with a smaller soft-tissue defect received a free sensate, extended lateral arm flap with triceps tendon; and three patients received a pedicled medial sural artery perforator gastrocnemius flap, of which one with Achilles tendon. Despite good functional results 1 year later, long-term follow-up revealed that two patients had to undergo knee arthrodesis because of recurrent infection and one patient was lost to follow-up. In parts, results have been published under doi: 10.7150/jbji.47018. Conclusions. A treatment concept and its rationale, based on a single-centre experience, is presented. It differentiates between various types of soft-tissue defects and shows reconstructive options following the concept to reconstruct ‘like with like’. Despite good results 1 year postoperatively, PJI of the knee with extensor apparatus deficiency remains a dreaded combination with a poor long-term outcome

Introduction. Fast track arthroplasty regimens require preservation of motor power to perform early rehabilitation and ensure early discharge (1). Commonly performed nerve blocks like femoral and Sciatic nerve blocks results in motor weakness thereby interfering with early rehabilitation and may also predispose to patient falls (2, 3). Hence, targeting the terminal branches of the femoral and sciatic nerves around the knee joint under ultrasound is an attractive strategy. The nerve supply of interest for knee analgesia are the terminal branches of the femoral nerve, the genicular branches of the lateral cutaneous nerve of thigh, obturator and sciatic nerves (4). Methods. We modified the performance of the adductor canal block and combined it with US guided posterior pericapsular injection and lateral femoral cutaneous nerve block to provide analgesia around the knee joint. The femoral artery is first traced under the sartorius muscle until the origin of descending geniculate artery and the block is performed proximal to its origin. A needle is inserted in-plane between the Sartorius and rectus femoris above the fascia lata and 5 ml of 0.5% ropivacaine (LA) is injected to block the intermediate cutaneous nerve of thigh. The needle is then redirected to enter the fascia of Sartorius to deliver an additional 5ml of LA to cover the medial cutaneous nerve of thigh following which it is further advanced till the needle tip is seen to lie adjacent to the femoral artery under the Sartorius to perform the adductor canal block with an additional 15–20 ml of LA to cover nerve to vastus medialis, saphenous nerve and posterior division of the obturator nerve (Fig 1). The lateral cutaneous nerve of thigh is optionally blocked with 10 ml of LA near the anterior superior iliac spine between the origin of Sartorius and tensor fascia lata (Fig 2). The terminal branches of sciatic nerve to the knee joint is blocked by depositing 25 ml of local anesthetic solution between the popliteal artery and femur bone at the level of femoral epicondyles (Fig 3). Results. The initial experience of the block performed on 10 patients reveal the median (IQR) block duration is noted to be around 20 (±6.5) hours. The median (IQR) pain scores in the first 24 postoperative hours ranged from 0 (±0.5) to 3 (±2.5) at rest and 1.5 (±3.5) to 5.5 (±1) on movement. All patients were successfully mobilized on the morning of the first postoperative day. Conclusion. Motor sparing from the blocks while providing adequate analgesia can be achieved by selectively targeting the sensory innervation of the knee joint. Future comparative studies are needed to evaluate the performance of the block against other modes of analgesia for knee arthroplasty

We perform the direct approach using a standard radiolucent operative table with extender at the foot, and the assistance of fluoroscopy. The patient is positioned supine with the pubic symphysis aligned at the table break. The anterior superior iliac spine (ASIS) and center of the knee are marked, and a line drawn between. The incision commences proximally from two finger breadths distal and two finger breadths lateral to the ASIS, and extends distally 8–10 cm. Using fluoroscopy, the anterior aspect femoral neck is located. The incision is placed over the lateral aspect of the greater trochanter, which avoids the lateral femoral cutaneous nerve. The tensor fascia lata is identified, which has a distinctive purple hue, and dissected free from the intermuscular septum lateral to the sartorius and the rectus muscles. The deep, investing aponeurosis of the tensor fascia lata is split using a tonsil. Just below lie the lateral circumflex vessels, two veins and one artery, which must be either ligated or cauterised. A retractor is placed superior to the femoral neck over top of the superior hip capsule. A blunt, cobra-type retractor is then placed along the inferior femoral neck, deep to the rectus muscle and the rectus tendon. A sharp retractor is then used to peel the rectus off from the anterior capsule and placed over the anterior rim of the acetabulum. An anterior capsulectomy is performed. A saw blade is positioned for femoral neck resection and confirmed with fluoroscopy. After resection, acetabular retractors are placed, the socket is reamed, the cup is placed, and position confirmed with fluoroscopy. Turning to the femoral side, the surgeon palpates underneath and around the tensor, around the lateral aspect of the femur, proximal to the gluteus maximus tendon, and places a bone hook around the proximal femur. Femoral preparation and stem insertion require maneuvering the table and adjusting the patient position. The table is “jack-knifed” by lowering the foot of the table to approximately 45 degrees and placing the bed into approximately 15 degrees of Trendelenburg. The contralateral well leg is placed on the padded Mayo stand. A table-mounted femur elevator is attached to the bed, requiring a change in surgical gloves, and attached to the traction hook around the proximal femur. Gentle retraction is placed on the femur to tension the capsule. As the capsule is released the femur will begin to come up/out of the wound and into view. With increasing gentle retraction via the table-mounted hook, the femur is elevated. Simultaneously, the operative limb is externally rotated and adducted underneath the non-operative leg in a lazy “figure of 4” position by the assistant. The use of a “broach-only” stem design is preferred as direct straight reaming of the femur is difficult in most cases. Fluoroscopic images are obtained to confirm femoral implant positioning, offset, neck and leg length. A standardised rapid recovery hospitalization and rehabilitation protocol is used in all cases

Introduction. A deficient abductor mechanism leads to significant morbidity and few studies have been published describing methods for reconstruction or repair. This study reports the reconstruction of hip abductor deficiency using human allograft. Methods. All patients were identified as having deficient abductor mechanisms following total hip arthroplasty through radiographic assessment, MRI, clinical examination and intra-operative exploration. All patients underwent hip abductor reconstruction using a variety of human allografts including proximal humeral, tensor fascia lata, quadriceps and patellar tendon. The type of allograft reconstruction used was customized to each patient, all being attached to proximal femur, allograft bone adjacent to host bone, with cerclage wires. If a mid-substance muscle rupture was identified an allograft tendon to host tendon reconstruction was performed. Results. Allograft reconstruction was performed in 15 patients over 18 months. One patient had an abductor deficiency after a primary total hip. All patients had an abductor lurch gait and positive Trendelenburg test preoperatively. Manual muscle strength testing showed significant weakness with a mean MRC grade of 3+/5. Peri-trochanteric pain was cited as a significant complaint in > 80 % of patients. Proximal humeral allografts, with rotator cuff, were used in 8 patients, 5 had tensor fascia lata and the remainder had patella with attached tendon allograft. The majority of patients had a reduction in pain and 8/15 (53%) increased their abductor strength by almost a full grade. A reduced lurch was observed in 10 (66%) patients and one patient re-dislocated after a failed revision for instability. Conclusion. To our knowledge, this is the largest reported series of allograft reconstruction for a deficient abductor mechanism following hip arthroplasty. A viable solution is demonstrated, with promising early results for a difficult problem, utilizing a straightforward technique with low morbidity

Purpose. Minimally invasive anterolateral approach (ALA) for total hip arthroplasty (THA) has gained popularity in recent years as better postoperative functional recovery and lower risk of postoperative dislocation are claimed. However, difficulties for femur exposure and intraoperative complications during femoral canal preparation and component placement have been reported. This study analyzes the anatomical factors likely to be related with intraoperative complications and the difficulties of access noted by the surgeons through a modified minimally invasive ALA. The aim is to define the profile for patient at risk of intraoperative complications during minimally invasive ALA. Methods. We retrospectively included 310 consecutive patients (100 males, 210 females) who had primary unilateral THA using the same technique in all cases. The approach was performed between the tensor fascia lata and the gluteus medius and minimus, without incising or detaching muscles and tendons. Posterior translation was combined to external rotation for proximal femur exposure (Fig. 1). All patients were reviewed clinically and radiologically. For the radiological evaluation, all patients underwent pre- and postoperative standing and sitting full-body EOS acquisitions. Pelvic [Sacral slope, Pelvic incidence (PI), Anterior pelvic plane angle] and femoral parameters were measured preoperatively. We assessed all intraoperative and postoperative complications for femoral preparation and implantation. Intraoperative complications included the femoral fractures and difficulties for femoral exposure (limitations for exposure and lateralization of the proximal femur). The patients were divided into two groups: patients with or without intraoperative complications. Results. Ten patients (3.2%) had intraoperative femoral fractures (greater trochanter: 2 cases, calcar: 8 cases). Five fractures required additional wiring. Difficult access to proximal femur was reported in the operative records for 10 other patients (3.2%). There was no difference in diagnosis, age, sex, BMI between the patients with or without intraoperative complications. No significant group differences were found for surgical time, and blood loss. Patients with intraoperative complication, presented a significantly lower pelvic incidence than patients without intraoperative complications (mean PI: 39.4° vs 56.9°, p<0.001). The relative risk of intraoperative complications in patients with low PI (PI<45°) was more than thirteen times (relative risk; 13.3, 95% CI= 8.2 to 21.5. p<0.001) the risk for patients with normal and high PI (PI>45°). Conclusions. Anterolateral approach for THA implantation in lateral decubitus is reported to have anatomical and functional advantages. Nevertheless, the exposure of the femur remains a limitation. This study highlights a significant increased risk in case of low PI less than 45°. This specific anatomical pattern reduces the local working space as the possibility for posterior translation and elevation of the proximal femur is less on a narrow pelvis. This limitation is due to the length and the orientation of the hip abductor and short external rotator muscles related to the relative positions of iliac wing and greater trochanter (Fig. 2). This study points out the importance of pelvic incidence for the detection of anatomically less favourable patients for THA implantation using ALA. Shoulder arthritis in the young adult is a deceptive title. The literature is filled with articles that separate outcomes based on an arbitrary age threshold and attempt to provide recommendations for management and even potential criteria for implanting one strategy over another using age as the primary determinant. However, under the age of 50, as few as one out of five patients will have arthritis that can be accurately classified as osteoarthritis. Other conditions such as post-traumatic arthritis, post-surgical arthritis including capsulorrhaphy arthropathy, and rheumatoid arthritis create a mosaic of pathologic bone and soft tissue changes in our younger patients that distort the conclusions regarding “shoulder arthritis” in the young adult. In addition, we are now seeing more patients with unique conditions that are still poorly understood, including arthritis of the pharmacologically performance-enhanced shoulder. Early arthritis in the young adult is often recognised at the time of arthroscopic surgery performed for other preoperative indications. Palliative treatment is the first option, which equals “debridement.” If the procedure fails to resolve the symptoms, and the symptoms can be localised to an intra-articular source, then additional treatment options may include a variety of cartilage restoration procedures that have been developed primarily for the knee and then subsequently used in the shoulder, including microfracture, and osteochondral grafting. The results of these treatments have been rarely reported with only case series and expert opinion to support their use. When arthritis is moderate or severe in young adults, non-arthroplasty interventions have included arthroscopic capsular release, debridement, acromioplasty, distal clavicle resection, microfracture, osteophyte debridement, axillary nerve neurolysis, and bicep tenotomy or tenodesis, or some combination of these techniques. Again, the literature is very limited, with most case series less than 5 years of follow-up. The results are typically acceptable for pain relief, some functional improvement, but not restoration to completely normal function from the patient's perspective. Attempts to resurface the arthritic joint have resulted in limited benefits over a short period of time in most studies. While a few remarkable procedures have provided reasonable outcomes, they are typically in the hands of the developer of the procedure and subsequently, other surgeons fail to achieve the same results. This has been the case with fascia lata grafting of the glenoid, dermal allografts, meniscal allografts, and even biologic resurfacing with large osteochondral grafts for osteoarthritis. Most surgical interventions that show high value in terms of improvement in quality of life require 10-year follow-up. It is unlikely that any of these arthroscopic procedures or resurfacing procedures will provide outcomes that would be valuable in terms of population healthcare; they are currently used on an individual basis to try to delay progression to arthroplasty, with surgeon bias based on personal experience, training, or expert opinion. Arthroplasty in the young adult remains controversial. Without question, study after study supports total shoulder arthroplasty over hemiarthroplasty once the decision has been made that joint replacement is the only remaining option

Avulsion of the abductor muscles of the hip may cause severe limp and pain. Limited literature is available on treatment approaches for this problem, and each has shortcomings. This study describes a muscle transfer technique to treat complete irreparable avulsion of the hip abductor muscles and tendons. Ten adult cadaver specimens were dissected to determine nerve and blood supply point of entry in the gluteus maximus and tensor fascia lata (TFL) and evaluate the feasibility and safety of transferring these muscles to substitute for the gluteus medius and minimus. In this technique, the anterior portion of the gluteus maximus and the entire TFL are mobilised and transferred to the greater trochanter such that the muscle fiber direction of the transferred muscles closely matches that of the gluteus medius and minimus. Five patients (five hips) were treated for primary irreparable disruption of the hip abductor muscles using this technique between January 2008 and April 2011. All patients had severe or moderate pain, severe abductor limp, and positive Trendelenburg sign. Patients were evaluated for pain and function at a mean of 28 months (range, 18–60 months) after surgery. All patients could actively abduct 3 months post-operatively. At 1 year post-operatively, three patients had no hip pain, two had mild pain that did not limit their activity, three had no limp, and one had mild limp. One patient fell, fractured his greater trochanter, and has persistent limp and abduction weakness. The anterior portion of the gluteus maximus and the TFL can be transferred to the greater trochanter to substitute for abductor deficiency. In this small series, the surgical procedure was reproducible and effective; further studies with more patients and longer follow-up are needed to confirm this

The Achilles tendon is the strongest and largest tendon in the body. Rupture of this tendon usually occurs in the third and fourth decade and can be significantly debilitating. Repair of neglected ruptures of Achilles tendon pose a challenge to the orthopaedic surgeon due to the retraction and atrophy of the ends of the tendon. Various surgical procedures have been described which include VY plasty, fascia lata, peroneus brevis, plantaris tendon, flexor digitorum longus, flexor hallucis longus, allograft, and synthetic materials. We carried out a prospective study to compare the results of peroneus brevis transfer with flexor hallucis longus transfer in the management of neglected ruptures of Achilles tendon. Forty-seven patients who had neglected ruptures of Achilles tendon were included in this study. They were randomly divided into two groups; the first group underwent peroneus brevis transfer (24 patients) and the second group had flexor hallucis longus transfer (23 patients). Patients were assessed using the Quigley's scoring system. We conclude that Flexor hallucis longus transfer is better than peroneus brevis transfer as it is a long, durable tendon which is much stronger when compared to other tendon transfers. Flexor hallucis longus acts in the same axis as the Achilles tendon, is in the same gait phase and is in close proximity, making harvesting of the tendon easy

Purpose. Chronic massive rotator cuff tears are challenging to repair completely because of the development of tendon retraction with inelasticity, muscle atrophy and fatty infiltration. The objective of this study was to investigate the clinical outcome and MRI findings after arthroscopic superior capsule reconstruction (ASCR) for symptomatic irreparable rotator cuff tears. Methods. From 2011 to 2013, 12 shoulders in 12 consecutive patients (mean, 70.8 years) with irreparable massive rotator cuff tears underwent ASCR using fascia lata. We used suture anchors to attach the graft medially to the glenoid superior tubercle and laterally to the greater tuberosity as same technique by Mihata et al. We added side-to-side sutures between the graft and infraspinatus tendon and between the graft and residual anterior supraspinatus/subscapularis tendon to improve force coupling. Physical examination, clinical rating system, and magnetic resonance imaging (MRI) were performed before surgery; at 6 and 12 months after surgery; and 6 months thereafter. Average follow-up was 19.1 months (12 to 28 months) after surgery. We assessed patients preoperatively by using the scoring systems of the shoulder index of the American Shoulder and Elbow Surgeons (ASES), the Japanese Orthopaedic Association, and the University of California, Los Angeles. Results. Mean active elevation increased from 59° to 111° significantly and external rotation increased from 28 to 33. The average preoperative scores were 16.0 points by ASES, 48.0 points by JOA, and 10.8 points by UCLA. Average clinical outcome scores all improved significantly after ASCR at the final follow-up (ASES, 73.3 points; JOA, 73.7 points; UCLA, 22.7 points). There was surgical complication as infection. Although infection was controlled by arthroscopic irrigation, the transplanted tendon was absorbed, and preoperative conditions did not functionally improve. Five patients had graft tear during follow-up. Postoperative active range of motion in the healed patients was significantly greater than in the unhealed patients who had graft tears. Conclusions. ASCR restored superior glenohumeral stability and function of the shoulder joint with irreparable rotator cuff tears compared to preoperative function in the cases without postoperative complications. However ratio of graft tears were 50% and clinical results of these cases were not improved sufficiently

The authors modified the original minimally invasive (MI) two-incision total hip arthroplasty (THA) technique and used large-diameter (32mm, 36mm) ceramic-on-ceramic articulation. One hundred and seventy patients that underwent unilateral MI two-incision THA retrospectively reviewed, and surgical morbidities, functional recoveries, radiological implantation properties, and complications were assessed. The mean follow-up was 30.2 months (range, 24-42 months) and mean patient age was 50.4 years (range, 22-83 years). In the lateral position, an anterolateral approach between the gluteus medius and tensor fascia lata and a posterior approach between the piriformis and gluteus medius was used. Mean operation time was 71.2 minutes (range, 48-91 minutes). Mean Harris hip score improved from 41.8 (range, 10-59) to 96.1 (range, 73-100) at last follow-up, and mean WOMAC score from 66.2 (range, 49-96) to 26.9 (range, 24-39). The mean lateral opening angle of the acetabular component was 38.2° (range, 32.1°-47.7°) and the mean stem position was valgus 1.9° (range, varus 2.3° to valgus 4.8°). One patient suffered an intraoperative femur fracture and another underwent revision surgery due to stem subsidence. No patient experienced dislocation. Our data suggest the this modified technique is safe and reproducible in terms of achieving proper implant positioning and early functional recovery. In particular, the complication rates encountered, especially the dislocation rate, were low

This study aims to investigate femoral blood flow during Metal-on-Metal Hip Resurfacing (MMHR) by monitoring oxygen concentration during the operative procedure. Patients undergoing MMHR using the posterior approach were evaluated. Following division of fascia lata, a calibrated gas-measuring electrode was inserted into the femoral neck, aiming for the supero-lateral quadrant of the head. Baseline oxygen concentration levels were detected after electrode insertion 2-3cm below the femoral head surface and all intra-operative measures were referenced against these. Oxygen levels were continuously monitored throughout the operation. Data from ten patients are presented. Oxygen concentration dropped most noticeably during the surgical approach and was reduced by 62% (Std.dev +/-26%) following dislocation and capsulectomy. Insertion of implants resulted in a further oxygenation decrease by 18% (Std.dev +/-28%). The last obtained measure before wound closure detected 22% (Std.dev +/-31%) of initial baseline oxygen levels. Variation between subjects was observed and three patients demonstrated a limited recovery of oxygen levels during implant insertion and hip relocation. Intra-operative measurement of oxygen concentration in blood perfusing the femoral head is feasible. Results in ten patients undergoing MMHR showed a dramatic effect on the oxygenation in the femoral head during surgical approach and implant fixation. This may increase the risk of avascular necrosis and subsequent femoral neck fracture. Future experiments will determine if less invasive procedures or specific positioning of the limb can protect the blood supply to femoral neck and head

Purpose. Hip arthroplasty is a good treatment option for displaced femoral neck fracture in elderly patients. However, neuromuscular disease such as cerebral infarction or hemorrhage can be a concerning problem for THA since dislocation after operation can frequently occur. The purpose of this prospective study was to evaluate the functional results of modified minimally invasive (MI) two-incision total hip arthroplasty (THA) with the use of large-diameter (>38mm) metal-on-metal articulation in patients with muscle weakness. Patients and Methods. 19 consecutive patients (19 hips) with displaced femoral neck fracture with muscle weakness were enrolled. There were 11 patients with cerebral infarction, 4 patients with cerebral hemorrhage and 4 patients with Parkinson's disease. In the lateral position, an anterolateral approach between the gluteus medius and tensor fascia lata and a posterior approach between the piriformis and gluteus medius were used. Surgical morbidity, functional recovery, radiological implantation properties, range of motion (ROM) and complications were assessed. Results. The mean operation time was 73.5 minutes and the average perioperative blood loss was 725.9cc. The mean head diameter used was 44 mm (38–50). The mean lateral opening angle of the acetabular component was 38.4°, the mean anteversion of the acetabular component was 16.4°, and the mean stem position was 0.3° valgus. The average postoperative ambulation time was 2.4 days. The mean Harris hip score was 81.0 at final follow-up, and the mean WOMAC score was 42.9. At final follow-up, there was no case of dislocation. There was no hypersensitivity, no osteolysis, and no revision. Conclusions. Our study suggests that the functional results of modified MI two-incision THA with the use of large-diameter metal on metal articulation in patients with muscle weakness can produce satisfactory early functional recovery and can reduce the dislocation rate

We used a laser Doppler flow-meter with high energy (20 m W) laser (Moor Instruments Ltd. Milwey, UK) to measure the blood flow to the femoral head during resurfacing arthroplasty. Twenty-four hips were studied; 12 underwent a posterior approach and twelve a Ganz's trochanteric flip osteotomy. The approach was determined according to surgeon preference. Three patients were excluded, The exclusion criteria were previous hip surgery, history of hip fracture and avascular necrosis (AVN). All patients had the hybrid implant with cemented femoral component. During surgery a 2.0mm drill bit was passed via the lateral femoral cortex to the superior part of the head neck junction. The position was confirmed using fluoroscopy. The measurements were taken during five stages of the operation: when the fascia lata was opened (baseline), at the end of soft tissue dissection, following dislocation of the hip, after relocation back into the socket, after inserting the implants prior to closing the soft tissues and, finally, at the end of soft tissue closure. The results were analysed and the values were normalised to a percentage of the baseline value. We found a mean drop of 38.6 % in the blood flow during the posterior approach and a drop of 10.34% with the trochanteric flip approach. The significant drop occured between the baseline (1st stage) and the end of the soft-tissue dissection (2nd stage). In both groups the blood flow remained relatively constant afterwards. Our study shows that there is a highly significant drop in blood flow (p<0.001) during the posterior approach compared with the trochanteric flip approach

We present the results of a new non-invasive lengthening nail enabling accurate control of the lengthening process and joint rehabilitation. Introduction. The use of intramedullary lengthening nails have gained popularity as they reduce common complications associated with external fixators, including infection, joint stiffness, bone regenerate deformity, late fracture and patient implant acceptance. Current nails however are associated with complications including implant breakage, mechanical failure, runaway nail and requiring MUA to restart or obtain segment lengthening. The Precice nail incorporates magnet technology with a hand held device allowing non-invasive lengthening. The nail is also reversible allowing shortening if required. Physiotherapy can continue throughout treatment to maintain joint range of motion without concern of uncontrolled nail runaway. The lengthening is axial reducing shear/torsional forces on the regenerate. Method. The lengthening of 4 femora was undertaken in 3 patients, mean age 34 yrs for post-traumatic shortening and short stature. A standard technique included an Ilizarov corticotomy followed by a 6 day latent period. Patients were mobilised partial weight bearing and knee range of motion maintained. The femora were lengthened one third of a millimetre three times per day. Radiographic and clinical review was performed every 2 weeks. Results. The desired femoral length was obtained in all patients, mean 5.6 cms (4.5–6.5). There were no complications of infection, poor bone regenerate or premature consolidation. One patient undergoing bilateral femoral lengthening underwent surgical release of a tight Tensor Fascia Lata following completion of lengthening due to hip flexion contracture. One patient required slowing of the lengthening rate due to knee flexion contracture. Physiotherapy corrected the deformity and the normal lengthening rate could be resumed. Conclusion. The Precice nail is a new device that offers accurate control of the lengthening process and rehabilitation. This should reduce complications of poor regenerate formation and soft tissue contractures

INTRODUCTION. Cadaveric studies have reported damage to the direct head of rectus femoris and tensor fascia lata muscles with direct anterior approach(DAA) and to the abductors, external rotators with posterior approach(PA). The aim of this prospective study was to evaluate differences in hip muscle strength recovery between DAA and posterior approach (PA) THA. METHODS. Patients with unilateral hip osteoarthritis undergoing THA at a single institution from January 2011 to October 2011 were enrolled. All DAA THA's were performed by one surgeon, and all PA THA's were performed by another surgeon with similar design of components, pain management and rehabilitation protocols. Hip muscle strength was measured with a handheld dynamometer in all planes by a single observer preoperatively, at 6 weeks, 3 months and 6 months. Functional recovery was assessed with the motor component of Functional Independence Measure, UCLA activity score, Harris hip score, SF-12 score. RESULTS. There were 30 patients (15 per group) with similar age, sex, BMI and preoperative functional scores. There was a significant difference between groups in ER strength recovery pattern(p=0.04) with greater mean deficit in PA group from preoperative to 6 weeks(37%), 3 months (28%) and 6 months (25%); whereas DAA group demonstrated 3% mean deficit at 6 weeks, 2% mean improvement at 3 months and 10% mean improvement at 6 months from preoperative values. Flexion strength decreased in DAA group by a mean of 10% at 6 weeks(p=0.07) but improved at 3 months by a mean of 12% as compared to preoperative values. There were no differences in recovery pattern of other hip movements between groups. Functional recovery scores between groups were similar. DISCUSSION. Both DAA and PA THA offer similar recovery in hip muscle strength up to 6 months with exceptions of ER strength deficit in PA group and a trend to flexion strength deficit at 6 weeks in DAA group

Introduction. Rottinger published a description of an anterior muscle sparing approach to the hip. It utilizes the same muscle interval as the classic WatsonJones approach between the gluteus medius laterally and tensor fascia lata medially. However, this technique has the disadvantage of needing asplit table and a sterile bag to mobilize the operative leg as extension, adduction and external rotation are the key points for femoral preparation. This study describes our experience for an equivalent of the Watson Jones approach with a simplified technique for the femoral preparation. Material and Methods. Incision starts 1cm distal and 3cm posterior to the ASIS and continues distally for about 8–10 cm along the straightline joining the lateral edge of the patella. It can be extended proximally or distally if necessary. The surgeon is placed posteriorly and the assistant anteriorly. The hip is dislocated with extension and external rotation to osteotomize the femoral neck. During the preparation of the acetabulum the femur is pushed posteriorly with internal rotation. Steinman pins are placed around the acetabulum to improve visualization for reaming and implanting theacetabular components. The femur is then exposed in a simplified way. The operated limb remains on the table. It is adducted above the contralateral limb and rotated outward to allow the femoral metaphysis to protrude. The foot is placed on the edge of the table beside the assistant, the knee is maintained with 45° flexion. The hip capsule is released postero-laterally to improve the femur exposure using Hohman retractors without cutting the short external rotator muscles. Femoral preparation is performed in this position. Once the appropriate implant is selected, the desired head trials are placed. The hip is reduced and the length and stability can be checked with the leg free. In case of isolated cup revision, the femoral head can be conserved. In case of femoral revision, a femorotomy can be easily performed due to the possibility of extended and stable exposure of the femur. Table 1 summarizes the main data of the series. Results. Mean operative duration was 57mn for primary THP (SD 10mn) and 124mn for revisions (SD 28mn). Table 2 summarizes the main complications according to the time line after the first implantation. High BMI patients and exposure of the acetabulum were never a problem; moreover this technique is very attractive for isolated cup revisions. Complications were mainly focused on femoral preparation due to a suboptimal use of the retractors for the exposure (4/13 fracture cases). Poor bone quality (old patients) was responsible for the 9 additional fractures. False route were observed in 4 curved femurs. 3 nerve compressions were due to excessive pressure on the knee by the assistant. Using short stems facilitates femoral exposure and reduces operative time. Conclusions. Hip approaches have been modified throughout the years in attempts to improve patient outcomes. This simplified technique did not induceunusual morbidity or mechanical problems. The benefit in terms of operative time and complications is significant in comparison with our previous experience using a fracture table. To view tables/figures, please contact authors directly
