Introduction:. Recently, patient expectations regarding the outcomes of total knee arthroplasty (TKA) have increase due to advances in knee implant design, surgical techniques, and procedure safety. However, outcomes do not always meet preoperative expectations. The purpose of this study is to investigate what Korean patients expect from their TKA and relationship between Pre-TKA expectation and demographic factors. Methods:. We performed a prospective study of 228 patients undergoing primary TKA from April 2013 to May 2013. The number of men and women were 211 (92.5%) and 17 (7.5%) respectively, and mean age was 70.9. We used 19-item clinical knee assessment questionnaire including “crossed leg” item for unique sedentary culture in eastern countries. Each item was evaluated on a Likert scale from 0 to 4 (0: not relevant, 1: relevant, 2: little important, 3: somewhat important, 4: very important). We also used another questionnaire regarding to patient's demographics. Score of all expectation was calculated by sum of all item score. Results:. The five most expectations among 19 items before TKA were 1) Improvement in walking, 2) Reduction in pain, 3) Improvement in climbing upstairs and downstairs, 4) Improvement in changing position, 5) improvement in daily activity (average score: 3.98 ± 0.22, 3.96 ± 0.25, 3.95 ± 0.28, 3.94 ± 0.25 and 3.92 ± 0.34 respectively). Crossed leg was ranked 7. th. In univariate logistic regression test, no demographic factors (age, BMI, sex, use of the bed, urban dwelling, cohabitation, degree of daily exercise, income, degree of education, presence of spouse, religion, employed or not) statistically influenced on these five items. But in minor expectation items, some association was noted with demographic factors. Low income influenced on expectation of public transportation use (p = 0.001) and job activity (p = 0.023). Young age and absence of spouse influenced on expectation of leisure activity (p = 0.004 and 0.022). Large amount of exercise influenced on expectation of sexual activity after TKA (p = 0.00001). In score of all expectation, more expectation on TKA was reported in young age (p = 0.002), male sex (p = 0.015), urban dwelling (p = 0.037) and large amount of exercise (p = 0.002). Discussion and conclusion:. Most of patients wanted pain relief and improvement of walking and exercise activity regardless of demographic factors. But in minor expectation items, some association was noted with demographic factors. Young age, male, urban dweller and patients with more exercise demanded more expectation on TKA. We need more study evaluating whether this expectation is same after one or more years of TKA

Over the past decade, there has been an increase in the number of total knee arthropalsty (TKA). Demand of TKA for the young patients who often have high physical demands is also increasing. However, the revision rate in such young patients is much higher due to polyethylene (PE) wear and instability (Julin J, Acta Orthop 2010). Therefore, next generation total knee prostheses are expected to decrease PE wear and to provide stability.

Although in vitro study such as wear simulator test provides important information about PE wear, we have often encountered the discrepancy between the in vitro results and in vivo results. Thus we have performed in vivo PE wear particle analysis, and showed that in vivo PE wear was affected by the design of articulating surface and the materials of femoral component and PE insert (Minoda Y, JBJS Am 2009). Medial pivot design, ceramic femoral component, and highly cross-linked PE decreased in vivo PE wear particle generation.

Patients who underwent bilateral staged TKAs were more likely to prefer medial pivot prosthesis or ACL-PCL retaining prosthesis than the other types of prostheses, because they feels “more stable overall” (Pritchett JW, J Arthroplasty 2011). In vivo fluoroscopic 3D analysis showed that medical pivot and bi-cruciate substituting designs restored physiological knee motion and provided higher reproducibility (Mueller J. Komistek RD, Trans ORS 2009, Iwakiri K, Trans ORS 2007).

The excellent mid-term clinical results of those newly introduced total knee prosthesis, such as alumina medial pivot TKA (Iida T, ORS 2008), medial pivot TKA (Mannan K, JBJS Br 2009, Kakachalions T, Knee 2009), ACL-PCL retaining TKA (Clouter JM, JBJS Am 1999), and highly cross-linked PE (Hodrick JT, CORR 2008), have been reported.

From the point of view of in vivo PE wear, in vivo stability, and the mid-term clinical results, we suspect that medial pivot prosthesis is one of the prostheses which meet the demand in future especially for young active patients.

Introduction. Positive expectations can increase compliance with treatment and realistic expectations may reduce postoperative dissatisfaction. Recently there are articles regarding expectations of patients from their TKA in western literature and only few articles based on Korean populations which don't encompass the whole spectrum of expectations in Korean patients. In all those articles based on pre-operative expectation, results were applied to whole expectation category uniformly not differentially. We aimed to document the pre-operative expectations in Korean patients undergoing total knee replacement using an established survey form and to determine whether expectations were influenced by socio-demographic factors and socio-demographic factors influences expectation items in particular category uniformly or differentially. Methods. Expectations regarding 19 items in the Knee Replacement Expectation Survey form were investigated in 228 patients scheduled for total knee replacement. The levels and distribution patterns of individual and summated expectation of five expectation categories; relief from pain, baseline activity, high flexion activity, social activity and psychological wellbeing, constructed from the 19 items were assessed. Univariate analyses and Binary logistic regression were performed and analyzed to examine the association of expectations with the socio-demographic factors. Results. The top five expectations among individual items were: improvement in walking ability, relief from pain, ascending and descending stairs, improvement in changing position and improvement in daily activity, respectively. Among the five expectation categories, relief from pain was ranked the highest, followed by restoration of baseline activity, ability to perform high flexion activities, psychological well-being and ability to participate in social activities respectively. Conclusion. There was a high expectation of restoration of walking ability, relief from pain and high flexion activity, whereas the expectation was more variable for psychological well-being and social activities. An age of < 65years, being employed, male gender, previous participation in high level sports activity, less income were all found to be significantly associated with higher expectations in social activity category whereas in baseline category, pain relief category and psychological well being category there are no significantly associated socio-demographic factors found, which shows these are the expectation items/categories which don't depend on socio-demographic factors and are highly expected by all patients undergoing TKA in our study. We also found that Socio-demographic factors influence individual expectation items in particular category differentially not uniformly hence results of individual items don't reflect the whole category or vice versa

Introduction. Patients-reported outcome measures (PROMs) have been reported as the important methods to evaluate clinical outcomes in total knee arthroplasty (TKA). The patient satisfaction score in Knee Society Score (KSS-2011) has been used in the recent literatures. Patient satisfaction was subjective parameter, and would be affected by multiple factors including psychological factors and physical conditions at not only affected joint but also elsewhere in the body. The question was raised regarding the consistency of patient satisfaction score in KSS-2011 to other PROMs. The purpose of this study was to investigate the correlation of patient satisfaction in KSS-2011 to other categories in KSS-2011 and to other PROMs including Forgotten Joint Score (FJS-12), EuroQol-5 Dimensions (EQ-5D) and 25-questions in Geriatric Locomotive Function Scale (GLFS-25). Material & Method. 83 patients over 65 years old with osteoarthritic knees were involved in this study. All patients underwent CR-TKAs (Persona CR. R. ). The means and ranges of demographics were as follows: age; 74.5 years old (65–89), Hip-Knee-Ankle (HKA) angle; 12.4 (−6.2–22.5) in varus. We asked patients to fulfill the questionnaire including KSS-2011, FJS-12, EQ-5D and GLFS-25 at 1-year postoperative follow-up visit. KSS-2011 consisted of 4 categories of questions; patient satisfaction (PS), symptoms, patient expectations (PE), functional activities (FA). We evaluated the correlation of PS to other PROMs using simple linear regression analyses (p<0.001). Results. The means and standard deviations of 1-year postoperative scores were as follows: PS; 28.5 ± 7.0, symptoms; 19.1 ± 4.3, PE; 11.2 ± 2.9, FA; 71.5 ± 16.6, FJS-12; 51.5 ± 18.6, EQ5D; 0.69 ± 0.10, GLFS-25; 25.7 ± 16.9. PSs were moderately positively correlated to other categories in KSS-2011(correlation coefficient (r): symptoms; 0.69, PE; 0.73, FA; 0.69). PSs were positively correlated to both FJS-12 and EQ5D (r: FJS-12; 0.72, EQ-5D; 0.67) and negatively correlated to GLFS-25(r; −0.74). Discussions. Patient satisfaction score positively correlated to the symptoms, patient expectation and functional activities in KSS-2011 with moderately high correlation coefficient. This meant the better pain relief and functional outcome improved patient satisfaction. Although there had be reported preoperative higher expectation would lead to poor patient satisfaction postoperatively, we interestingly found positive correlation between patient satisfaction and expectation at 1 year after TKA. Patient with the higher satisfaction tended to expect more in the future, on the other hand, unsatisfied patient with residual pain and/or poor function would resign themselves to the present status and reduced their expectation in our patient population. We have found patient satisfaction score in KSS-2011 significantly correlated to FJS-12 and GLFS-25 with strong correlation coefficient. This meant patient satisfaction could be considered consist to other PROMS in relatively younger patient with better functional status in this study. Conclusion. The patient satisfaction score in KSS-2011 was found to be consistent with moderately high correlations coefficient to other categories in KSS-2011 and other PROMs including FJS-12, EQ-5D, GLFS-25 at 1 year after (CR)-TKA. Total hip arthroplasty (THA) has high rates of patient satisfaction; however patient expectations for recreational and sporting activities are not always met. Our study aimed to identify preoperative factors that predict whether patient expectations for sporting or recreational activity are met 12 months following THA. Patient reported outcome measures (PROMs) were collected prospectively from 2015-2018 at one private hospital in Sydney. Age, gender, postcode, weight, and height were recorded preoperatively. Included participants underwent primary THA by one of the investigating surgeons. Univariable and multivariable analyses were performed with an expectation fulfilment score used as the primary outcome variable. Preoperative predictor variables included: age, gender, BMI, Socio-economic Indexes for Areas (SEIFA), Oxford Hip Score, Hip Osteoarthritis Outcome Score, EQ-5D-5L and EQ Visual Analogue Scale (EQ VAS). 1019 participants were eligible and included. 13% reported that preoperative expectations of sport or recreation were not met at 12 months. Younger age, lower preoperative EQ VAS, and higher BMI were associated with failed expectations on multivariable analysis. Odds of failed expectations increased by 2% for every one year younger in age (OR= 0.98, 95% CI = 0.96 to 1.00, p=.048), by 2% for every one point lower on EQ VAS (OR=0.98, 95% CI = 0.98 to 0.99, p=.002), and by 4% for every one-point increase in BMI (OR = 1.04, 95% CI = 1.00 to 1.09, p=.042). Failure to have expectations met for sporting or recreational activity was associated with younger age, poorer general health, and high BMI. With a rise in younger patients who likely have higher physical demands, a tailored preoperative education is preferable to generic models to better manage patient expectations. Younger age, higher BMI, and poorer health may predict unmet expectations for sport and recreation after THA. Tailored education in these groups should be considered

Patient expectations and their fulfilment are an important factor in determining patient-reported outcome and satisfaction of hip (THR) and knee replacement (TKR). The aim of this prospective cohort study was to examine the expectations of patients undergoing THR and TKR, and to identify differences in expectations, predictors of high expectations and the relationship between the fulfilment of expectations and patient-reported outcome measures. During the study period, patients who underwent 346 THRs and 323 TKRs completed an expectation questionnaire, Oxford score and Short-Form 12 (SF-12) score pre-operatively. At one year post-operatively, the Oxford score, SF-12, patient satisfaction and expectation fulfilment were assessed. Univariable and multivariable analysis were performed. Improvements in mobility and daytime pain were the most important expectations in both groups. Expectation level did not differ between THR and TKR. Poor Oxford score, younger age and male gender significantly predicted high pre-operative expectations (p < 0.001). The level of pre-operative expectation was not significantly associated with the fulfilment of expectations or outcome. THR better met the expectations identified as important by patients. TKR failed to meet expectations of kneeling, squatting and stair climbing. High fulfilment of expectation in both THR and TKR was significantly predicted by young age, greater improvements in Oxford score and high pre-operative mental health scores. The fulfilment of expectations was highly correlated with satisfaction

Up to 20 percent of patients remain dissatisfied after primary total knee arthroplasty (TKA) surgery. Understanding the reasons for dissatisfaction post TKA may allow for better patient selection and optimized treatment for those who remain dissatisfied. The association between function, mobility and satisfaction are not well understood. The purpose of this study was to investigate the association between post-TKA satisfaction and i) pre-operative, ii) post-operative, and iii) change in knee joint function during gait. Thirty-one patients scheduled to receive primary TKA for knee osteoarthritis (OA) diagnosis were recruited and visited the Dynamics of Human Motion laboratory for instrumented walking gait analysis (using a synchronized NDI Optotrak motion capture system and AMTI force platforms in the walkway) at two time points, first within the week prior to their surgery, and second at approximately one year after surgery. At their post-operative visit, patients were asked to indicate their satisfaction with their knee prosthesis on a scale from zero to 100, with zero being totally unsatisfied and 100 being completely satisfied. Knee joint mechanics during gait at both time points were characterized by discriminant scores, the projection of their three-dimensional knee angles and moments during gait onto an existing discriminant model that was created to optimize separation of severe knee OA and healthy asymptomatic gait patterns. This discriminant model was created using data from 73 healthy participants and 73 with severe knee OA, and includes the magnitude and pattern features (captured with principal component analysis) of the knee adduction and flexion moment, and the magnitude of the knee flexion angle during gait. Larger discriminant scores indicate improved function toward healthy patterns, and smaller scores indicate more severe function. Associations between post-operative satisfaction and pre, post and change in discriminant scores were examined using Pearson correlation analyses. We also examined associations between satisfaction and pre-operative BMI, EQ5D and Oxford 12 scores, as well as changes in these scores from pre to post-TKA. Discriminant scores representing knee joint function during gait significantly improved on average after surgery (P =0.05). While overall knee joint function improved after primary TKA surgery, the amount of improvement in function was not reflected in post-operative patient satisfaction. However, the pre-operative function of the patient was negatively associated with satisfaction, indicating that patients with higher pre-operative function are overall less satisfied with their TKA surgery, regardless of any functional improvement due to the surgery. Interestingly, the only significant association with post-operative satisfaction was knee joint function, and the relationship between function and patient satisfaction following TKA appears to relate only to the baseline functional state of the patient, and not with functional improvement. This suggests that dissatisfaction post-surgery is more likely reflecting the unmet expectations of a higher functioning patient, and has implications for the need for improved understanding of pre-operative patient functional variability in TKA triage and expectation management

The discussion of outpatient unicompartmental knee arthroplasty (UKA) requires proof that it can be done safely and effectively, and also begs the question of whether it can be performed in an ambulatory surgery center (ASC) rather than a general hospital (which raises costs and is typically less efficient). Successful outpatient UKA requires carefully crafted algorithms/protocols, home support, preoperative planning and preparation, expectation management, risk stratification (not everyone is a candidate), perioperative pain management and buy-in from patients, support networks and the health care team. Relatively little data is available on the feasibility, safety and potential cost savings associated with this shift in care. We evaluated the costs and short term outcomes and complications of 150 consecutive UKAs performed in an ASC compared to those done in a general hospital both on an inpatient and outpatient basis. Determination of the setting of the outpatient surgery was made based on geographic preference by the patients; otherwise choice of inpatient or outpatient surgery in the hospital was left to the discretion of the surgeon and was primarily based on the patients' comorbidity profile and circumstances of home help. Total direct facility costs were calculated, including institutional supplies and services, anesthesia services, implants, additional PACU medications and services required, and costs associated with operating room use. Only total cost was evaluated, as it is the most consistent cost variable amongst the two institutions evaluated. The mean total direct cost of UKA in a general community hospital with an overnight stay was 1.24 and 1.65 times greater than the cost of UKA performed at the same hospital or an ASC on an outpatient basis, respectively. The mean total direct cost of outpatient UKA in a general hospital was 1.33 times greater than the mean total cost of UKA performed in an ASC. Semi-autonomous robotic technology has been introduced to optimise accuracy of implant positioning and soft tissue balance in UKA, with the expectation of resultant improvement in durability and implant survivorship. Currently, nearly 20% of UKA's in the U.S. are being performed with robotic assistance. It is anticipated that there will be substantial growth in market penetration over the next decade, projecting that nearly 37% of UKA's and 23% of TKA's will be performed with robotics in 10 years (Medical Device and Diagnostic Industry, March 5, 2015). First generation robotic technology improved substantially implant position compared to conventional methods; however, high capital costs, uncertainty regarding the value of advanced technologies, and the need for preoperative CT scans were barriers to broader adoption. Newer image-free robotic technology offers an alternative method for further optimizing implant positioning and soft tissue balance without the need for preoperative CT scans and with price points that make it suitable for use in an ASC. Currently, as a result of cost and other practical issues, <1% of first generation robotic technologies are being used in ASC's. Alternatively, more than 35% of second generation robotic systems are in use in ASC's for UKA, due to favorable pricing. In conclusion, UKA can be safely performed in the outpatient setting in select patients. Additionally, we demonstrated a substantial cost savings when UKA is performed in an outpatient setting and care is shifted from a general community hospital to an ASC. Finally, robotics can be utilised to optimise accuracy of implant placement and soft tissue balance in UKA, and newer image-free robotic technology is cost effective for outpatient UKA

A significant portion of ankle arthroplasty and ankle arthrodesis procedures performed in British Columbia are funded by the public medical services plan (MSP). However, some patients are treated privately through self-pay or by the workers compensation board (WCB), with the latter two groups being more likely to receive treatment sooner. The potential effect of payer on patient-reported outcomes and reoperation rates has not been previously explored. A retrospective chart review was performed using data from the Canadian Orthopaedic Foot and Ankle Society Prospective Ankle Reconstruction Database. N=443 patients (393 MSP, 26 self-pay, 24 WCB), treated with total ankle replacement or ankle arthrodesis by three subspecialty-trained surgeons in Vancouver from 1999–2003, were analyzed. Outcomes were compared, by payer, preoperatively and at long-term follow-up (6.3 years, range 2–14 years). Function was assessed using the Ankle Osteoarthritis Scale (AOS) Total score (primary outcome) and the AOS Pain and Difficulty subscores. Expectation and satisfaction with symptoms was assessed using the Musculoskeletal Outcomes Data Evaluation and Management Scale, and physical and emotional quality of life was assessed using the Short Form-36 (SF-36) Health Survey in terms of the mental component summary (MCS) and physical component summary (PCS). Swelling and reoperation rates were also compared. AOS Total score was not significantly different between payers. WCB patients had significantly worse preoperative AOS Difficulty scores (73, 95%CI 65–80) compared to MSP (65, CI 63–67) and self-pay patients (56, CI 49–63)(p < 0 .008). Their SF-36 MCS scores were also significantly worse pre- and postoperatively (WCB: 43, CI 38–49, 45, CI 40–50, MSP: 51, CI 50–52, 51, CI 50–52, self-pay: 51, CI 46–56, 54, CI 49–58)(p < 0 .03). AOS Pain scores and SF-36 PCS scores were not different. Pre- and postoperatively, MSP patients reported more satisfaction with symptoms (1.31, CI 1.24–1.38, 3.21, CI 3.07–3.35), compared to WCB (1.13, CI 0.84–1.41, 2.83, CI 2.26–3.41) and self-pay patients (1.19, CI 0.91–1.47, 2.88, CI 2.33–3.44). Preoperatively, WCB patients had the lowest expectations (76, CI 69–84), the worst AOS Total (64, CI 57–71) and SF-36 scores (MCS 43, CI 38–49, PCS 28, CI, 25–32), and the most swelling (3.5, CI 3.1–4). Conversely, self-pay patients had the highest preoperative expectations (88, CI 81–95), the best AOS Total (53, CI 46–60) and SF-36 scores (MCS 51, CI 46–56, PCS: 34, CI 30–37) and the least swelling (3, CI 2.6–3.4). Postoperatively, WCB and self-pay patients had lower expectations met (35, CI 23–47 and 40, CI 28–51) and worse AOS Total scores (36, CI 27–45 and 35, 26–43), compared to MSP patients (Expectations: 29, CI 26–32, AOS Total: 31, CI 29–33). Reoperation rates were similar among groups. WCB patients had significantly more difficulty with symptoms prior to surgery and worse SF-36 MCS scores pre- and postoperatively. The preoperative expectations of WCB patients were lowest, while those of self-pay patients were highest. Both groups had lower expectations met postoperatively

INTRODUCTION. Since the early 2000s, highly cross-linked (HXL) UHMWPE's have become a popular option with multiple experimental and clinical studies showing that gamma or electron radiation doses between 50–100kGY reduce wear and potentially extend the bearing life of UHMWPE. However, the increased wear resistance came at a compromise to mechanical properties due to the cross-linking process. Vitamin E has been added to some HXL UHMWPE materials to offer a solution to the compromise by increasing oxidation resistance and maintaining sufficient fatigue strength. However, limited data is available on the effect of the fabrication process, especially the method of irradiation, on the properties of the Vitamin E blended HXL UHMWPE. The purpose of this study was to evaluate the effects of adding the antioxidant vitamin E to highly crosslinked UHMWPE on wear rates. METHODS. Wear testing was performed on six highly crosslinked UHMWPE acetabular liners containing vitamin E (0.1% wt. alpha tocopherol) fabricated using the Cold Irradiation Mechanically Annealed (CIMA) process, initially cross-linked with approximately 100 kGy gamma irradiation, and terminally gamma sterilized. The liners were paired with three 40mm CoCr femoral heads and 40mm three ceramic femoral heads. Testing was completed per ASTM F1714 and ISO 14242 on an orbital hip joint wear simulator (Shore Western, California) and lubricated with 90% bovine calf serum, 20mM EDTA, 0.2% wt. NaN. 3. and DI water. A 1.1Hz Paul-type loading waveform with a peak of 2kN was used for a total of 5E6 wear cycles. Three loaded soak controls were used in parallel to adjust for fluid absorption. Samples were weighed every 5E5 wear cycles. RESULTS. The wear rates for the HXL blended vitamin-E liners were calculated using the slope of the linear regression over the steady state and resulted in a wear rate of 0.49mg/Mc. This is a decrease of approximately 95% compared to the 9.54 mg/Mc 28mm ID conventional UHMWPE wear rates as well as a notable difference for the other HXL UHMWPE liner wear rates discussed in the review. DISCUSSION. HXL blended vitamin-E 40mm liners demonstrated an approximate 95% reduction in wear rates compared to a 28mm ID conventional UHMWPE. The reduced wear rate confirmed the design expectation that a higher irradiation dose in the fabrication process resulted in an increased amount of polymer crosslinking. Additionally, the wear rate of the HXL blended vitamin-E liners studied was well below 20mg/Mc, which was shown by Dowd et al. to be the threshold of osteolysis in THA. SIGNIFICANCE. The HXL vitamin E blended UHMWPE liner tested in this study demonstrated reduced wear rates by approximately 95% compared to conventional polyethylene. Osteolysis-causing wear debris is reduced while maintaining other mechanical properties. This liner material and manufacturing process is a promising alternative to conventional polyethylene, but long-term clinical results are warranted

Introduction. Robotic systems have been used in TKA to add precision, although few studies have evaluated clinical outcomes. We report on early clinical results evaluating patient reported outcomes (PROs) on a series of robotic-assisted TKA (RAS-TKA) patients, and compare scores to those reported in the literature. Methods. We prospectively consented and enrolled 106 patients undergoing RAS-TKA by a single surgeon performing a measured-resection femur-first technique using a miniature bone-mounted robotic system. Patients completed a KOOS, New Knee Society Score (2011 KSS) and a Veterans RAND-12 (VR-12) pre-operatively and at 3, 6 and 12 months (M) post- operatively. At the time of publication 104, 101, and 78 patients had completed 3M, 6M, and 12M PROs, respectively. Changes in the five KOOS subscales (Pain, Symptoms, Activities of Daily Living (ADL), Sport and recreation function (Sport/Rec) and Knee-related Quality of Life (QOL)) were compared to available literature data from FORCE – TJR, a large, prospective, national cohort of TJR patients enrolled from diverse high-volume centers and community orthopaedic practices in the U.S, as well as to individual studies reporting on conventional (CON-TKA) and computer-assisted (CAS- TKA) at 3M, and on conventional TKA at 6M. The 2011 KSS is a validated method for quantifying patient's expectations and satisfaction with their TKA procedure. Improvements in the 2011 KSS were compared with literature data at 6M post-operatively. Results. RAS-TKA PRO's significantly improved at 3, 6, and 12M from pre-operative baseline values. When compared to the FORCE registry cohort data, the improvement in KOOS subscales were generally higher for RAS for pain at 6M, and for pain, ADL, and QOL at 1Y when compared with FORCE 2Y data. Higher improvements were also seen at 3M, except for Sports/Rec, and at 6M for symptoms and QOL when compared with smaller cohort studies. Improvements in 2011 KSS patient satisfaction and functional scores at 6M were 11 and 10 points greater than those reported for conventional TKA. A mean of 31 pts for the Patient Satisfaction score indicates that on average patients were ‘Satisfied’ with their knee function and pain level. Mean rates of dissatisfaction with knee pain level and function were 9.2%, 3.8% and 3.1% at 3, 6, and 12M postoperatively, respectively. A mean of 10pts for the Expectation score post-operatively indicates that on average patients felt their expectations for pain relief, ADL, and leisure/sports/rec activities were between “Just Right” or “Too Low”. Discussion. Early results of RAS-TKA demonstrated significant improvements in pain, function, and QOL from baseline pre-operative values. PROs for robotic TKA also compared favorably with results reported in the literature; however, additional randomized control studies are required to provide more meaningful comparisons with conventional techniques and with other advanced technologies

Patient perceptions regarding the functional outcomes and return to sports after shoulder replacement are often pessimistic, with many patients presenting for shoulder replacement surgery after months or years of avoiding the procedure so they could continue to live the current life they have, despite the increasing pain and dysfunction. Less common, but becoming more frequent, patients present with expectations that they will be able to return to all activities including heavy resistance training, cross-fit, rock climbing, and other strenuous overhead sports. In the past, little information has been available regarding the activities of shoulder arthroplasty patients after surgery. Typically, the boundaries have been set by the surgeon, with many patients cautioned or even prohibited from overhead sports, weight training, or heavy work responsibilities. A typical set of guidelines may include no repetitive overhead sports, except for recreational swimming, and no lifting over 20 pounds. Golf, jogging, hiking and other activities are allowed. The origin of these restricted guidelines and expectations is unknown, but many believe that since the results of shoulder replacements are less favorable in younger patients, it may be due to the overuse or abuse of the shoulder joint that is more typical at a younger age. Others have suggested that common sense prevails and that an artificial joint made of metal and plastic has a finite number of total movements and tolerance to resistance activities, and therefore keeping these activities at a minimum would extend the longevity of the artificial joint. None of these concepts are backed up by evidence-based literature, essentially reflecting the personal bias of the surgeons who care for patients with these problems. Despite all of the sophisticated research, scoring scales, outcome measures, and value-based metrics, the only outcome that really matters is whether the patient can return back to their normal way of life, at home, at work, during sport, or any activity that is important to them. Recent studies of patients who have had joint replacement surgery have revealed that our patients who participated in sports and work activities before surgery have a strong predilection to returning to those activities after successful shoulder replacement. The most common sports that shoulder arthroplasty patients enjoy including golf, swimming, tennis, but may also include many other choices including fitness activities, rowing, skiing, basketball, and softball. As expected, the return to these sports is less for reverse shoulder arthroplasty patients vs. anatomic shoulder arthroplasty patients. In a systematic review, more than 90% of anatomic shoulder replacement patients returned to sport, while 75% of reverse shoulder arthroplasty patients returned to some sporting activity. This may reflect the constraints of the reverse prosthesis, or, quite possibly reflect the typically older age and more sedentary lifestyle of patients who are indicated for reverse shoulder arthroplasty. In addition, if the patient had a preoperative expectation of return to recreational and sports activities as part of their normal way of life, their final results demonstrated better overall outcomes. Shoulder arthroplasty surgeons should be concerned about the outcomes desired by our patients, and the results that provide true value to their lives. We are now more aware of the activities that they are going to return to, whether we recommend restrictions or not

Total hip arthroplasty (THA) has been cited as one of the most successful surgical procedures performed today. However, as hip surgeons, we desire constantly improving outcomes for THA patients with more favorable complication rates. At the same time, patients desire hip pain relief and return to function with as little interruption of life as possible. The expectation of patients has changed; they have more physical demands for strength and flexibility, and aspire to achieve more in their recreational pursuits. Additionally, health care system constraints require the THA episode of care to become more efficient as the number of procedures increases with time. These factors, over the past fifteen years, have led to a search for improved surgical approaches and peri-operative pain and rehabilitation protocols for primary THA. The orthopaedic community has seen improved pain control, length of stay, and reduction in complications with changes in practice and protocols. However, the choice of surgical approach has provided significant controversy in the orthopaedic literature. In the 2000s, the mini-posterior approach (MPA) was demonstrated as the superior tissue sparing approach. More recently, there has been a suggestion that the direct anterior approach (DAA) leads to less muscle damage, and improved functional outcomes. A recent prospective randomised trial has shown a number of early deficits of the posterior approach when compared to the direct anterior approach. The posterior approach resulted in patients taking an additional 5 days to discontinue a walker, discontinue all gait aids, discontinue narcotics, ascend stairs with a gait aid, and to walk 6 blocks. Patients receiving the posterior approach required more morphine equivalents in the hospital, and had higher VAS pain scores in the hospital than the direct anterior approach. Interestingly, activity monitoring at two weeks post-operatively also favored DAA with posterior approach patients walking 1600 steps less per day than DAA patients. There has been little difference in the radiographic outcomes or complications between approaches in prospective randomised trials. A number of randomised clinical trials have demonstrated that both the direct anterior and posterior approach provided excellent early post-operative recovery with a low complication rate. DAA patients have objectively faster recovery with slightly shorter times to achieve milestones of function, with similar radiographic and clinical outcomes at longer-term outcomes, with a similar complication rate

Although many distal fit and fill design cementless stems have shown a very good long term stable fixation, short proximal coated stems are recently increasing in their use with an expectation of less stress shielding and an ease of removal at revision surgery. We introduced an anatomic short stem made from titanium alloy with proximal plasma-spray titanium and hydroxyapatite coating (CentPillar, Stryker, Mahwah) in 2002. To evaluate a minimum 10-year outcome of the system in terms of fixation and stress shielding, we reviewed initial 100 consecutive cases operated by a single surgeon. There were 91 hips with osteoarthritis and 9 hips with osteonecrosis. There were 94 females and 6 males. Average age at operation was 58 years. The patients were followed up for an average of 11 years. Average JOA hip score improved significantly from 46.9 preoperatively to 96.7 at the final examination. There were no dislocation, or revision, or radiographic loosening. When we looked at the level of bone atrophy, 80% of cases showed no stress shielding below the lessor trochanter. We conclude that the CentPillar stem showed mild stress shielding due to short proximal bone ongrowth coating while keeping a long term good clinical score and radiographic stability

Despite the successful, durable results, concern remains for using TKA in patients with isolated patello-femoral OA, as it requires an extensive surgical exposure and bone resection, a long recovery time, and a potentially more complex revision than that required for a patient with a failed patello-femoral arthroplasty (PFA). PFA was introduced in the late 1970s. While PFA was successful in providing pain relief, the procedure did not gain widespread use because of initial design limitations that predisposed to PF maltracking, catching, and subluxation. The mechanical complications associated with first-generation PFA offset the potential advantages of maintaining the knee's native soft tissues and spurred efforts to improve implant design, and to refine surgical techniques and patient selection. Over the past two decades, second generation PFAs incorporated changes in implant design and instrumentation and have shown promising results when used in the properly selected patient population. In addition, with improved instrumentation and robotics, adequate implant alignment and rotation can be achieved in the majority of patients, including those with severe patellofemoral dysplasia. Our meta-analysis of TKA and PFA for the treatment of isolated patello-femoral OA showed that the rate of complications of patients undergoing PFA was 30% after a median follow up of 5.3 years, which is significantly higher than the 7% rate of complications in patients who underwent TKA. The most frequent type of complication associated with PFA was mechanical (including loosening and instability), which is consistent with the malalignment and maltracking-related failures previously reported. The incidence of re-operation after PFA (21%) was significantly higher than that seen after TKA (2%). The most frequent indication for re-operation after PFA was mechanical failure (7%), followed by progression of OA (6%), and persistent pain or stiffness (5%). The most common re-operations after PFA were conversion to TKA, revision of PFA components, lateral releases, open or arthroscopic debridement, manipulations under anesthesia, and bony and/or soft tissue extensor mechanism re-alignment procedures. In our study, 11% of patients treated with PFA underwent a revision arthroplasty, with 4% undergoing revision PFA and 7% undergoing conversion to TKA. Our comparison of patients who were treated with second-generation PFA designs versus TKA showed no difference in the rate of complications, re-operation, or revision arthroplasty. Additionally, length of follow-up did not significantly influence any of these outcomes when comparing second-generation PFA and TKA. These observations provide support for the use of current PFA designs. The mechanical complications and subsequent re-operations that affected first-generation PFA designs appear to be of less concern with proper patient selection, meticulous surgical technique, current implant designs and peri-operative care. While it is difficult to predict the survivorship of current PFA designs, it is our expectation that patient selection will continue to be a critical component in determining long-term results. The potential benefit of providing pain relief while preserving the tibiofemoral articulations makes PFA a promising treatment option

Background: Metal sensitivity following total joint arthroplasty (TJA) has been of increased concern, but the impact of a patient-reported metal allergy on clinical outcomes has not been investigated. The purpose of this study was to report the incidence and impact of patient-reported metal allergy following total knee (TKA) and total hip arthroplasty (THA). Methods: This was a retrospective, IRB-approved investigation of patients undergoing a primary, elective TJA between 2009 and 2011. All patients completed a pre-operative questionnaire asking about drug and environmental allergies. In January of 2010, a specific question was added regarding the presence of a metal allergy. UCLA Activity, SF-12, Modified Harris Hip (MHHS), and Knee Society (KSS) scores were collected pre-operatively and at most recent follow-up. Overall cohorts of metal allergy and non-metal allergy patients were compared and a 1:2 matching analysis was also performed. Results: 906 primary THAs and 589 primary TKAs were included. The incidence of patient-reported metal allergy was 1.7% before January 2010 and 4.0% after (overall 2.3% of THAs and 4.1% of TKAs). 97.8% of metal allergy patients were female. Following TKA, post-operative KSS function, symptoms, satisfaction, and expectation scores were all decreased in the metal allergy cohort (p<0.001 to 0.002). Following THA, metal allergy patients had a decreased post-operative SF-12 MCS score and less incremental improvement in their SF-12 MCS score versus the non-metal allergy cohort (p<0.0001 and p<0.001). Conclusion: Patient-reported metal allergy is associated with decreased functional outcomes following TKA and decreased mental health scores following THA

This study was performed to investigate the concentration of silver ions release up to a time of 9 weeks as well as the antimicrobial activity of silver sulfate and Nano-silver mixed bone cement on Candida albicans, in expectation of a new way of therapy in manner of a time limited application – a silverions releasing bone cement spacer. Two different kinds of silver products were used and mixed with polymethylmetacrylate (PMMA, De Puy) bone cement:. Nano-silver with a particle size of 5–50 nm and active surface of 4 m2/ g. (Nanonet Styria, Austria). Silver sulfate in a finely powdered form (Fisher, GB). Concentrations of 0.1%, 0.5%, 1% and 5% of the Nano-silver and the silver-salt by weight were mixed with the dry powder portion of the cement. To test the silver-ions release from the silver-containing bone cement two models of elution, a static model and a dynamic model were created. To test the antifungal effectiveness of the various concentrations of Ag-PMMA the bone cement samples were tested by agar diffusion assay. With respect to minimal inhibition concentration (MIC) the sample containing 0.5 % silver sulfate showed required concentration at the dynamic elution model but none of the nano-silver samples did. In static elution model we measured the maximum concentration of 466.5 µg/l at the 0.5 % silver sulfate sample which is much below the toxic concentration. Agar diffusion assay showed no zone of inhibition from Nano-silver samples. In contrast, silver sulfate containing samples showed a zone of inhibition exactly growing, depending on the samples silver sulfate concentration. According to results, silver sulfate addition to PMMA might be another approach in treatment of candida associated periprosthetic joint infection

Purpose. Total knee arthroplasty (TKA) is one of the most successful surgeries with respect to relieving pain and restoring function of the knee. However, some studies have reported that patients are not always satisfied with their results after TKA. The aim of this study was to determine which factors contribute to patient's satisfaction after TKA. Methods. We evaluated 69 patients who had undergone 76 primary TKAs between March 2012 and June 2013, and assessed patient- and physician- reported scores using the 2011 Knee Society Scoring System and clinical variables before and after TKAs. We determined the correlation between patient satisfaction and clinical variables. Results. The mean (SD) pre-operative score was 8.2 (4.9) for symptoms, 11.5 (4.5) for patient's satisfaction, 13.1 (2.2) for patient's expectations, and 35.4 (18.2) for functional activities. The mean (SD) post-operative score was 16.3 (5.0) for symptoms, 20.7 (6.4) for patient's satisfaction, 9.1 (2.3) for patient's expectations, and 47.4 (19.6) for functional activities. We found that physician- reported scores were higher than patient- reported scores, and improvement in patient- reported scores was lower than that of physician- reported scores following TKA. We did not found a correlation between any pre-operative variables including expectation and post-operative satisfaction. Post-operative symptoms (r=0.51, p<0.01) and functions (r=0.39, p<0.01) correlated with post-operative satisfaction. The predictors of patient dissatisfaction after TKA were remaining symptoms and low postoperative activities. Conclusions. Our study demonstrates that to relieve pain and to restore activities is important for increasing patient satisfaction after TKA. The 2011 Knee Society Scoring System allows surgeons to appreciate differences in the priorities of patients with TKA

Reverse shoulder arthroplasty (RSA) was released into the United States market in 2004 for the indication of Rotator Cuff Tear Arthropathy. Since that time the indications have widened and now include massive rotator cuff tears that are not deemed to be surgically repairable. This diagnosis includes patients that are considered young in terms of shoulder replacement surgery. These patients do not have the radiographic changes of arthropathy, but most have undergone multiple surgical procedures in the past to attempt to repair the rotator cuff deficiency. These younger patients have a different post-op expectation than the patient with advanced arthropathy over the age of 70. The complication rate seen in this younger patient population is higher than seen in the older population undergoing RSA. The post-operative survey for satisfaction is also considerably lower in the younger population group. RSA for massive rotator cuff tears in a younger population under the age of 65 years gives a lower patient satisfaction result and a higher complication rate than age-matched patients older than 65 years. Although this is still the best procedure available for this difficult diagnosis, patients younger than 65 years should be consulted as to what their expectations are post-operatively. Pain relief is still the main reason to undergo an RSA

Recent reports observe that orthopaedic surgeons lack essential knowledge about ionising radiation. We aim to demonstrate perceived use of image-intensifiers by surgeons and awareness of radiation doses used during fractured neck of femur surgery. Surgeons at a regional trauma centre were sent an online questionnaire. They were shown two neck of femur fracture radiographs and asked the total number of images they would use to reduce and fix the fracture with a dynamic-hip-screw / inter-medullary nail respectively. They were asked the maximum safe radiation dose, and that of ‘hip pining’ compared to CXR as outlined by the Ionising Radiation Regulations 1999. For a DHS, consultants and registrars estimate their image use similarly. For IM nailing, consultants estimated higher image use than registrars, and double the number of X-rays taken for IM nailing compared to DHS. Knowledge levels regarding radiation doses during orthopaedic hip procedures are very low. There is an expectation that more images will be used in IM nailing procedures. We plan to educate orthopaedic surgeons about radiation dose and safety. Correlating our findings with actual use of image in theatre when performing hip fracture surgery would extend the use of this study
