The Pavlik harness (PH) is commonly used to treat infantile dislocated hips. Variability exists in the duration of brace treatment after successful reduction of the dislocated hip. In this study we evaluate the effect of prescribed time in brace on acetabular index (AI) at two years of age using a prospective, international, multicenter database.

We retrospectively studied prospectively enrolled infants with at least one dislocated hip that were initially treated with a PH and had a recorded AI at two-year follow-up. Subjects were treated at one of two institutions. Institution 1 used the PH until they observed normal radiographic acetabular development. Institution 2 followed a structured 12-week brace treatment protocol. Hip dislocation was defined as less than 30% femoral head coverage at rest on the pre-treatment ultrasound or IHDI grade III or IV on the pre-treatment radiograph.

Fifty-three hips met our inclusion criteria. Hips from Institution 1 were treated with a brace 3x longer than hips from institution 2 (adjusted mean 8.9±1.3 months vs 2.6±0.2 months)(p < 0 .001). Institution 1 had an 88% success rate and institution 2 had an 85% success rate at achieving hip reduction (p=0.735). At 2-year follow-up, we observed no significant difference in AI between Institution 1 (adjusted mean 25.6±0.9˚) compared to Institution 2 (adjusted mean 23.5±0.8˚) (p=0.1). However, 19% of patients from Institution 1 and 44% of patients from Institution 2 were at or below the 50th percentile of previously published age- and sex- matched AI normal data (p=0.049). Also, 27% (7/26) of hips from Institution 1 had significant acetabular dysplasia, compared to a 22% (6/27) from Institution 2 (p=0.691). We found no correlation between age at initiation of bracing and AI at 2-year follow-up (p=0.071).

Our findings suggest that prolonged brace treatment does not result in improved acetabular index at age two years. Hips treated at Institution 1 had the same AI at age two years as hips treated at Institution 2, while spending about 1/3 the amount of time in a brace. We recommend close follow-up for all children treated for dislocated hips, as ~1/4 of infants had acetabular index measurements at or above the 90th percentile of normal. Continued follow-up of this prospective cohort will be critical to determine how many children require acetabular procedures during childhood. The PH brace can successfully treat dislocated infant hips, however, prolonged brace treatment was not found to result in improved acetabular development at two-year follow-up.

Background

The patients with high hip dislocation due to the sequelae of septic hip or neglected Developmental Dysplasia of the Hip (DDH) show severely impaired gait pattern. Total hip arthroplasty (THA) for these patients are expected to restore gait pattern by establishing better joint stability and biomechanics. To our knowledge, no study have investigated about objective change in gait parameters after total hip arthroplasty (THA) for these patients. So, we are to prospectively evaluate change in gait patterns after THA.

Methods

Between 2012 and 2013, 11 patients with highly dislocated hip underwent unilateral THA with subtrochanteric osteotomy. There was 6 patients with DDH sequelae and 5 patients with septic hip sequelae. Spatio-temporal gait analysis was performed preoperatively and at 12 months after THA. We followed the patient 3, 6, 12 months and then annually postoperatively. The gait patterns were analyzed by several parameters such as cadence, speed, stride length, step length, step time, initial double support (IDS), terminal double support (TDS), stance phase and swing phase by a three-dimensional (3D) high-speed motion-capturing system (eight Eagle® cameras; Motion Analysis, Santa Rosa, CA, USA). Also dynamic range of motion (ROM) of hip joint and ground-reaction forces (GFR) were recorded. Clinical outcome was evaluated by using the Harris Hip Score (HHS). Radiographic assessments were evaluated for the changes in leg length discrepancy (LLD).

Abduction braces are commonly prescribed following the closed reduction of a dislocated prosthetic hip joint. Their use is controversial with limited evidence to support their use. We have conducted a retrospective review of dislocations in primary total hip replacements over a nine year period and report redislocation rates in patients braced, compared to those who were not. 67 patients were identified. 69% of those patients who were braced had a subsequent dislocation. Likewise 69% of those who did not receive a brace re-dislocated. 33% of patients that were braced dislocated whilst wearing the brace. Bracing was associated with patient discomfort, sleep disturbance, skin irritation and breakdown. Small femoral head size, monoblock femoral components and poor biomechanical reconstruction was prevalent amongst dislocators. Abduction bracing following closed reduction of a total hip replacement is costly(e950), does not prevent redislocation and may be the cause of considerable morbidity to the patient.

Introduction

Total hip arthroplasty (THA) for a highly dislocated hip can be problematic and technically challenging. Our previous study on cemented THA with subtrochanteric femoral shortening osteotomy revealed a high incidence (20%) of non-union. Therefore, in 2008, we introduced reverse hybrid THA using S-ROM stem for the treatment of a highly dislocated hip. The purpose of this study was to assess the short-term clinical outcomes of this new method.

Patients and methods

Between 2008 and 2014, 13 consecutive reverse hybrid THAs were performed on nine female patients with highly dislocated hips. The average age at the time of operation was 66 years (range, 55–85 years). The acetabular component was fixed in the true acetabulum with bone cement. Transverse osteotomy was performed below the lesser trochanter to shorten the femur and to prevent over-lengthening. The proximal sleeve of the S-ROM stem was then fixed within the proximal fragment, and the distal fin provided rotational stability of the distal fragment. Thus, the two fragments were fixed to each other with the S-ROM stem, and the resected segment was longitudinally cut for grafting at the junction. The postoperative follow-up period was an average of 4 years (range, 1–7 years), and no patients were lost. Preoperative and final Japanese Orthopaedic Association (JOA) hip score, operation time, bleeding amount, intraoperative and postoperative complications, bone healing at the osteotomy site, implant loosening, and revision surgery were retrospectively investigated.

Background. Postoperative dislocation is one of the main surgical complications and the primary cause for revision surgery after 2-stage implant exchange due to periprosthetic infection of a total hip arthroplasty. Objective. The aims of our study were (1) to determine the incidence of dislocation after two-stage THA reimplantation without spacer placement, (2) to evaluate relevant risk factors for dislocation and (3) to assess the final functional outcome of those patients. Method. We prospectively analyzed 187 patients who underwent a two-stage total hip arthroplasty (THA) revision after being diagnosed with periprosthetic joint infection (PJI) from 2013 to 2019. The mean duration of follow-up was 54.2 ± 24.9 months (>36 months). The incidence of postoperative dislocation and subsequent revision was estimated through Kaplan-Meier curves and potential risk factors were identified using Cox hazard regression. The functional outcome of the patients was assessed using the modified Harris Hip Score (mHHS). Results. The estimated cumulative dislocation-free survival was 87.2% (95% CI: 81.2%-91.3%) with an estimated 10% and 12% risk for dislocation within the first 6 and 12 months, respectively. The use of a dual-mobility construct had no significant impact on the dislocation rate. Increasing body mass index (BMI) (HR=1.11, 95% CI: 1.02-1.19, p=0.011), abductor mechanism impairment (HR=2.85, 95% CI: 1.01-8.01, p=0.047), the extent of elongation of the affected extremity between stages (HR=1.04, 95% CI: 1.01-1.07, p=0.017), the final leg length discrepancy (HR=1.04, 95% CI: 1.01-1.08, p=0.018) and PJI recurrence (HR=2.76, 95% CI: 1.00-7.62, p=0.049) were found to be significant risk factors for dislocation. Overall revision rates were 17% after THA reimplantation. Dislocated hips were 62% more likely to undergo re-revision surgery (p<0.001, Log-rank= 78.05). A significant average increase of 30 points in mHHS scores after second-stage reimplantation (p=0.001, Wilcoxon-rank) was recorded, but no difference was noted in the final HHS measurements between stable and dislocated hips. Conclusion. Dislocation rates after 2-stage THA reimplantation for PJI remain high, especially regarding overweight or re-infected patients. Careful leg length restoration and an intact abductor mechanism seem critical to ensure stability in these complex patients

Some DEFINITIONS are necessary: “STEMS” refers to “intramedullary stem extensions”, which may be of a variety of lengths and diameters, fixed with cement, porous coating or press fit alone and which may be modular or an inherent part of the prosthesis. The standard extension keel on the tibia does not qualify as a “stem (extension)”. COMPLEX implies multiple variables acting on the end result of the arthroplasty with the capability of inducing failure, as well as necessary variations to the standard surgical technique. A lesser degree of predictability is implied. More specifically, the elements usually found in an arthritic knee and used for the arthroplasty are missing, so that cases of COMPLEX primary TKA include: Soft tissue coverage-(not relevant here), Extensor mechanism deficiency-patellectomy, Severe deformity, Extra-articular deformity, Instability: Varus valgus, Instability: Plane of motion, Instability: Old PCL rupture, Dislocated patella, Stiffness, Medical conditions: Neuromuscular disorder, Ipsilateral arthroplasty, Prior incisions, Fixation hardware, Osteopenia, Ipsilateral hip arthrodesis, Ipsilateral below knee amputation, etc. Complexity includes MORE than large deformity, i.e., success with large deformity does NOT mean success with constrained implants regardless of indication. In addition, the degree of constraint must be specified to be meaningful. NECESSARY presumably this means: “necessary to ensure durable fixation in the face of poor bone quality or more mechanically constrained” and SUFFICIENT suggests that stems, by themselves or in some shape of form, by themselves “will ensure success (specifically here) of fixation”. If we can start with the second proposal, that STEMS are SUFFICIENT for success the answer is: “NO”, many more aspects of surgical technique and implant design are required. Even if all other aspects of the technique are exemplary, some types of stems or techniques are inadequate, e.g., completely uncemented, short stem extensions. The answer to the first proposal is: “YES, in many cases”. The problem will be to determine which cases. There are philosophical analogies to this question that we already know the answer to. ANALOGY: Is a life-raft necessary on a boat? Yes, you may not use it, but it is considered necessary. Is a life-raft “sufficient” on a boat? No, other problems may occur. Are seat belts necessary? Are child seats necessary? The AAOS already has a position on child restraints, an analogous situation, where a party who cannot control their situation (anesthetised patient/ child) functions in the care of a responsible party. The objection may be argued in terms of cost saving by NOT using increased fixation. A useful analogy, (that would of course require specific analysis), is that of patellar resurfacing: universal resurfacing is cost-effective when considering the expense of even a small number of secondary resurfacings. Of course a complex arthroplasty that requires a revision procedure is far more expensive than secondary patellar resurfacing and so universal use of the enhanced fixation in the face of increased constraint makes sense. The human cost of revision surgery tips the balance irrefutably. DANGER-We must avoid the glib conclusion, often based on poor quality data, that constrained implants do not need additional intramedullary fixation (with stem extensions). When “complexity” is involved, complex analysis is appropriate to select the best course

Background. The distal part of the radius is the most common localisation of fractures of the human body. Dislocated intraarticular fractures of the distal radius (FDR) are frequently treated by open reduction and internal fixation with a volar locking plate (VLP) under fluoroscopic guidance. Typically the locking screws are placed subchondral near the joint line to achieve maximum stability of the osteosynthesis. To avoid intraarticular screw placement an intraoperative virtual implant planning system (VIPS) as an application for mobile C-arms was established. The aim of the study was the validation of the implemented VIPS comparing the intraoperative planning with the actual placement of the screws. The study was conducted as a single-centre randomised controlled trial in a primary care institution. The hypothesis of the study was that there is conformity between the virtual implant position and the real implant placement. Patients/Material and Methods. 30 patients with FDR type A3, C1 and C2 according to the AO-classification were randomised in two treatment groups and allocated either in the conventional or in the VIPS group in which the patients underwent an intraoperative planning before screw placement. The randomisation was performed on the basis of a computer-generated code. After fracture reduction an initial diaphyseal fixation of the plate was done. Then the matching of the three-dimensional virtual plate with the image of the real plate in the fluoroscopy shots in two planes was performed automatically. The implant placement was planned intraoperatively in terms of orientation, angulation and length of the screws. After the placement of four or five locking screws the implant position was verified with an intraoperative three-dimensional mobile C-arm scan. The locking screws near the joint line were examined and compared in relation to the actual and the planned inclination angle, the azimuth angle which is determined analogue to a compass rose and the screw-tip distance. The planned and actual parameters of the locking screws were then statistically analysed applying the Shapiro-Wilk - and the Students t-test. Results. 15 patients with FDR were treated in the VIPS arm. In the VIPS group six fractures type A3 one type C1 and eight type C2 were included. The control group showed a similar fracture distribution with six type A3 and nine type C2 fractures. The discrepancy between the actual and the planned screw-tip distance was 2,24 ± 0,97 mm and did not differ significantly (p>0,05). The angle of the planned and actual screw placement also did not vary significantly (p>0,05). The difference of the actual to the planned azimut angle accounted for 18,69°± 29,84. The planned and real inclination of the screws differed by 1,66° ± 4,46. Conclusion. The analysis shows that the screws were almost placed as planned. Differences between actual and planned placement of the screws were observed but were not statistically significant. Therefore the hypothesis of the study can be accepted. We assume, that the precise planning of the screw placement in FDR with VIPS can be transported into the surgical treatment

Introduction

The aim of this study is to report the 30 day COVID-19 related morbidity and mortality of patients assessed as SARS-CoV-2 negative who underwent emergency or urgent orthopaedic surgery in the NHS during the peak of the COVID-19 pandemic.

Method

A retrospective, single centre, observational cohort study of all patients undergoing surgery between 17 March 2020 and 3May 2020 was performed. Outcomes were stratified by British Orthopaedic Association COVID-19 Patient Risk Assessment Tool. Patients who were SARS-CoV-2 positive at the time of surgery were excluded.
