Introduction. There is less literature reporting outcomes following total ankle replacement (TAR) in patients presenting with a coronal plane deformity preoperatively. This study compares clinical and patient reported outcomes at 1 year between TAR patients with and without coronal plane deformity. Methods. Patients from single centre prospective cohort (132) who underwent TAR between 2006 and 2010 were included. They were divided into 2 groups based on preoperative coronal plane deformity. Groups 1 and 2 had a coronal plane deformity of <10 and >10 respectively. Assessments included American Orthopaedic Foot and Ankle Score (AOFAS), Foot and Ankle Outcome Score (FAOS), SF-36 (Generic Health Measure) and complications recorded preoperatively and 3, 6 and 12 months postoperatively. Results. There were 102 ankles (77.2%) in group 1 and 30 ankles (22.7%) in group 2. There was no difference in age and BMI between groups (p > 0.05). AOFAS, FAOS and SF-36 scores showed improvement for both groups from pre-op to 1 year (p < 0.01) with no difference between groups (p > 0.05) at all assessment times; except for mental health scores (SF-36 component) which showed no significant improvement (p > 0.05) and no difference between groups (p > 0.05). There was no difference in patient satisfaction for overall outcomes, pain relief, improvement in ADL and recreational activities between groups at 1 year (p > 0.05). There was no incidence of DVT/PE in both groups. The incidence of peri-prosthetic fractures was 8 (7.8%) for group 1 and 2 (6.6%) in group 2. Discussion and Conclusion. These early results of total ankle replacement demonstrate that patients with >10 degrees coronal plane deformity had equivalent outcomes to patients with <10 degrees of coronal plane deformity. Our surgical techniques for correction of deformity are reliable to reduce the requirement of ankle fusion

We aim to assess the outcome of ankle arthrodesis performed for painful osteoarthritis in the presence of a coronal plane deformity of 20 degrees or more. To our knowledge this is the first reported series of such a cohort of patients. We have a consecutive and complete series of 24 patients with 26 ankle arthrodeses which were all performed for painful osteoarthritis in the presence of large coronal plane deformity. These patients have a minimum of twelve months clinical follow-up. The results showed a low non-union rate of 8% (2 ankles). These have subsequently been re-fused satisfactorily, and are excluded from further analysis. The results of the remaining 24 ankles, which united primarily, show that patients were very pleased with the outcome of their surgery. AOFAS scores were used to measure pain and function, both pre-operatively and post-operatively. These scores showed large improvements for both pain and function, and had a high statistical significance (p<0.0001). All patients improved in their walking distance and many patients reduced their need for walking aids. Stair climbing ability was also improved in some patients. It is recognised that an ankle arthrodesis usually relieves pain but does not result in a normal gait and full function. We feel that the high level of patient satisfaction in this series was due to the combination of deformity correction, restoring a functional foot position, and achieving a painless ankle. Arthroplasty of the ankle is a good procedure for relief of pain and restoration of function. However, in the presence of a large coronal plane deformity, ankle arthroplasty is known to fare badly with early failure. Therefore for patients with painful osteoarthritis and a coronal plane deformity of 20 degrees or more, we recommend ankle arthrodesis as the procedure of choice

Introduction and Aims: Many patients having total ankle replacement require correction of a coronal plane deformity. This study examines the pre-operative characteristics and short-term results of patients with a coronal plane deformity having total ankle replacement. It tests the hypotheses that the pre-operative deformity will be corrected and maintained at two years. Method: Eighteen of 86 patients who underwent Scandinavian Total Ankle Replacement between February 1998 and April 2001 had a pre-operative coronal plane deformity ≥ 10° and at least two-year follow-up. The goal of intraoperative alignment was to place all components perpendicular to the plumb line of the body, and to have this position maintained by appropriate ligament balancing. The mean patient age was 62.2±13.2 years. The etiology of arthrosis included eight post-traumatic, five idiopathic, one rheumatoid, and four other. There were 10 men and eight women. Valgus measurements are > 90°. Congruent ankles have pre-operative talar and tibial alignment within 10 degrees. Results: Ligament balancing consisting of lateral ligament reconstruction was performed in six patients and superficial deltoid release was performed in three patients. The eight ankles with pre-operative varus-congruent alignment improved both the talar and tibial alignment immediately post-operatively and at two-year follow-up (p< 0.05). There was only one ankle with valgus-congruent alignment precluding statistical testing. The six ankles with varus-incongruent alignment improved the talar alignment immediately post-operatively and at two-year follow-up (p< 0.05). The three ankles with valgus-incongruent alignment improved the talar alignment immediately post-operatively and at two-year follow-up (p< 0.05). Ankles with an incongruent pre-operative deformity had a greater loss of correction of the talus between the immediate pre-operative period and two-year follow-up than patients with a congruent pre-operative deformity (3.9±2.8 degrees vs. 1.3±1.0 degrees loss of correction, p< 0.05). The valgus-incongruent group lost 2.3 degrees of correction from the immediate post-operative period to the two-year follow-up (p< 0.05). The varus-incongruent showed a trend to lose 4.0 degrees of correction from the immediate post-operative period to the two-year follow-up (p=0.065). Conclusion: In patients with a pre-operative coronal plane deformity ≥ 10 degrees, alignment after ankle replacement improves toward a neutral axis in the postoperative period and at two-year follow-up. Ankles with incongruent pre-operative deformities have a greater loss of correction over the first two years than ankles with congruent pre-operative deformities

Introduction. Current techniques in total knee arthroplasty aim to restore the coronal mechanical axis to neutral. Preoperative planning has historically been based on long-leg standing films (LLSF) which allow surgeons to plan bony resection and soft tissue releases. However, LSSF can be prone to error if malrotated. Recently, patient-specific guides (PSG) utilizing supine magnetic resonance imaging (sMRI) have become an accepted technique for preoperative planning. In this study we sought to compare the degree of coronal deformity using LLSF and sMRI. Methods. Two hundred thirty knees underwent planning for total knee arthroplasty with sMRI and LLSF. Coronal plane deformity was determined based on the femoral-tibial angle (FTA) as defined by the angle formed between a line from the center of the femoral head to the intercondylar notch and a line from the middle of the tibial spines to the middle of the ankle joint. Mechanical axis values from the sMRI were compared with values obtained from LLSF. Results. There were 172 varus knees and 58 valgus knees. There was significant correlation (r=0.9215) between LLSF and sMRI for the measurement of coronal plane deformity for all knees. sMRI underestimated the severity of deformity by 2.19 degrees of varus (p<0.001). Additionally, as the severity of the deformity increased, there was also an increase in the discrepancy between sMRI and LLSF. There was a smaller discrepancy for valgus knees (−0.66 degrees) than varus knees (3.15 degrees, p<0.001). The discrepancy between the two modalities was not affected by gender (p=0.386). Conclusion. sMRI based imaging can help approximate coronal plane deformity in the preoperative planning of TKA but it has limitations. This MRI-based technique tended to underestimate deformity in varus knees and patients with extreme deformity. Surgeons may use sMRI for pre-operative planning but must understand that they tend to underestimate the severity of deformity

Preoperative talar valgus deformity increases the technical difficulty of total ankle replacement (TAR) and is associated with an increased failure rate. Deformity of ≥15° has been reported to be a contraindication to arthroplasty. The goal of the present study was to determine whether the operative procedures and clinical outcomes of TAR for treatment of end-stage ankle arthritis were comparable for patients with preoperative talar valgus deformity of ≥15° as compared to those with <15°. We will describe the evolving surgical technique being utilized to tackle these challenging cases.

Fifty ankles with preoperative coronal-plane tibiotalar valgus deformity of ≥15° “valgus” group) and 50 ankles with valgus deformity of <15° (“control” group) underwent TAR. The cohorts were similar with respect to demographics and components used. All TARs were performed by a single surgeon. The mean duration of clinical follow-up was 5.5 years (minimum two years). Preoperative and postoperative radiographic measurements of coronal-plane deformity, Ankle Osteoarthritis Scale (AOS) scores and Short Form (SF)-36 scores were prospectively recorded. All ancillary (intraoperative) and secondary procedures, complications and measurements were collected.

The AOS pain and disability subscale scores decreased significantly in both groups. The improvement in AOS and SF-36 scores did not differ significantly between the groups at the time of the final follow-up. The valgus group underwent more ancillary procedures during the index surgery (80% vs 26%). Tibio-talar deformity improved significantly toward a normal weight-bearing axis in the valgus group. Secondary postoperative procedures were more common in the valgus group (36%) than the controls (20%). Overall, re-operation was not associated with poorer patient outcome scores. Metal component revision surgery occurred in seven patients (three valgus and four controls). These revisions included two deep infections (2%), one in each group, which were converted to hindfoot fusions. Therefore, 94% of the valgus group retained their original components at final follow-up

Thus far, this is the largest reported study that specifically evaluates TAR with significant preoperative valgus alignment, in addition to having the longest follow-up. Satisfactory midterm results were achieved in patients with valgus mal-alignment of ≥15°. The valgus cohort required more procedures during and after their TAR, as well as receiving more novel techniques to balance their TAR. Whilst longer term studies are needed, valgus coronal-plane alignment of ≥15° should not be considered an absolute contraindication to TAR if the associated deformities are addressed.

Aim. To determine the rate of recurrence of coronal plane deformity in children treated with ‘guided growth’ using 8-plates, from the time of implant removal to skeletal maturity. Methods. Over a consecutive 5 year period between April 2008 and April 2013 we analysed our results of guided growth treatment using 8-plates to correct coronal plane lower limb deformity. Patients with neuromuscular disorders such as cerebral palsy were excluded. Deformity planning was performed using standardised techniques. Our standard practice is to remove the 8-plate and screws once deformity is corrected both clinically and radiologically. Patients were followed up until either skeletal maturity or recurrence, which necessitated reapplication of the 8-plate. We are aware of no study in which children treated with guided growth using 8-plates are followed up to skeletal maturity. Results. 267 patients were treated with 8-plates in our unit over this 5 year period. Of the patients in whom deformity was corrected and had subsequent plates removed, we identified 41 patients who have either reached skeletal maturity or had recurrence of deformity. Six patients required reapplication of the 8-plates implant. These were young and had skeletal dysplasia. Deformity parameters were analysed both clinically and radiologically in patients who have reached skeletal maturity and showed no recurrence, which necessitated further intervention. Conclusion. A higher proportion of younger patients, especially a sub-group with skeletal dysplasia had recurrence of deformity necessitating reapplication of the 8-plate device. In this group we recommend removal of only the metaphyseal screw once deformity is corrected. This would allow ease of reapplication if recurrence were to reoccur. Level of evidence: III

Introduction:

Patient specific instrumentation (PSI) is an innovative technology in total knee arthroplasty. With the use of a preoperative MRI or CT scan, custom guide blocks are individually manufactured for each patient. Contrary to other TKA technologies such as computer-assisted surgery, PSI utilizes measured resection technique rather than a primarily ligament balancing technique. This has the potential to negatively affect the operating surgeon's ability to achieve optimal soft tissue balancing, which is especially critical in patients with severe lower extremity malalignment. Despite early research suggesting that PSI is accurate, has a low learning curve, and can reduce OR time, it remains unclear whether a surgeon using PSI can achieve optimal soft tissue balancing using a measured resection technique. The purpose of this study is to evaluate the efficacy of PSI in patients with severe preoperative limb alignment deformities.

Methods:

Fifty PSI total knee arthroplasties were performed on 46 patients (21 male, 25 female) using the Zimmer NexGen Patient Specific Instrumentation system. Each patient included in the study had a minimum preoperative deformity of at least 10° varus or valgus measured on preoperative long leg standing radiographs, Zimmer preoperative software or both. Forty-three of the included knees had a varus deformity and 7 had a valgus deformity. Preoperative mechanical axis alignment measurements were obtained using the PSI preoperative planning software and were manually calculated using pre-operative long leg standing radiographs. Postoperative mechanical axis alignment measurements were calculated using plain long leg standing radiographs. The Knee Society Scoring System was used to evaluate clinical and functional outcomes at 1 to 6 months postoperatively.

We determined the frequency, rate and extent of development of scoliosis (coronal plane deformity) in wheelchair-dependent patients with Duchenne muscular dystrophy (DMD) who were not receiving steroid treatment. We also assessed kyphosis and lordosis (sagittal plane deformity). The extent of scoliosis was assessed on sitting anteroposterior (AP) spinal radiographs in 88 consecutive non-ambulatory patients with DMD. Radiographs were studied from the time the patients became wheelchair-dependent until the time of spinal fusion, or the latest assessment if surgery was not undertaken. Progression was estimated using a longitudinal mixed-model regression analysis to handle repeated measurements.

Scoliosis ≥ 10° occurred in 85 of 88 patients (97%), ≥ 20° in 78 of 88 (89%) and ≥ 30° in 66 of 88 patients (75%). The fitted longitudinal model revealed that time in a wheelchair was a highly significant predictor of the magnitude of the curve, independent of the age of the patient (p <  0.001). Scoliosis developed in virtually all DMD patients not receiving steroids once they became wheelchair-dependent, and the degree of deformity deteriorated over time.

In general, scoliosis increased at a constant rate, beginning at the time of wheelchair-dependency (p < 0.001). In some there was no scoliosis for as long as three years after dependency, but scoliosis then developed and increased at a constant rate. Some patients showed a rapid increase in the rate of progression of the curve after a few years – the clinical phenomenon of a rapidly collapsing curve over a few months.

A sagittal plane kyphotic deformity was seen in 37 of 60 patients (62%) with appropriate radiographs, with 23 (38%) showing lumbar lordosis (16 (27%) abnormal and seven (11%) normal).

This study provides a baseline to assess the effects of steroids and other forms of treatment on the natural history of scoliosis in patients with DMD, and an approach to assessing spinal deformity in the coronal and sagittal planes in wheelchair-dependent patients with other neuromuscular disorders.

Introduction. Most surgeons utilize one of three axis options in conventional total knee arthroplasty (TKA), the transepicondylar axis (TEA), Whiteside's line (WSL) or the posterior condylar axis (PCA) with an external rotation correction factor. Each option has limitations and no clear algorithm has been determined for which option to use and when. Many surgeons believe the TEA to be the gold standard for determining rotation however it can be difficult to access intraoperatively. WSL and PCA have been used as surrogates for determining axial rotation in conventional TKA but may also be prone to error. MRI based preoperative planning systems overcome intraoperative limitations while accounting for the individual anatomy of each patient, thus helping optimize femoral component rotation. The goal of this study was to examine if coronal plane deformity had any effect on the relationship of conventional referencing options such as WSL and PCA to the TEA. Methods. Utilizing a preoperative planning software based on MRI, we compared the preoperative posterior femoral condyle resections for three different axis options in 176 TKA. The difference in bone resection amount was used to determine the rotational differences between the axis options in all knees. Assuming that the TEA was the ideal rotational axis, we compared the TEA to both WSL and PCA. A 1-sample t-test and paired t-test were then used to determine if there was a significant rotational difference between the various axis options when accounting for degree and direction of preoperative deformity in the coronal plane. Results. In the overall population of 176 knees (42 valgus, 134 varus), neither WSL or PCA approximated the TEA accurately (p=0.016 and 0.001). In valgus deformity, WSL was found to approximate the TEA (p=0.68) better than the PCA (p=0.21). Minor varus deformity (< 3 degrees) favored the use of PCA (0.53) while moderate varus deformity (3–6 degrees) favored use of WSL (p=0.76). Severe varus (>6 degrees) deformity favored use of PCA due to lower variability. For complete results see Figure 2. Conclusion. Based on MRI data, our study indicates that preoperative coronal plane deformity should help determine the specific referencing option utilized for femoral component rotation in TKA. Broad application of either WSL or the PCA to all patients regardless of preoperative deformity did not accurately approximate TEA in femoral component rotation. Rather, analysis of the degree and direction of preoperative coronal plane deformity indicates that WSL and PCA should be used in specific scenarios to approximate the TEA. When WSL or PCA either both approximate or do not approximate the TEA, we recommend using the option with a lower standard deviation, and thus less variability. Although this MRI based technology is not in widespread use, we believe our findings (Figure 1) can assist the majority of surgeons determine when to use WSL or the PCA based on preoperative coronal plane deformity

Abstract. Aims. Growth disturbances after transphyseal paediatric ACL reconstruction have led to the development of physeal-sparing techniques. However, evidence in their favour remains weak. This study reviews the literature to identify factors associated with growth disturbances in paediatric ACL reconstructions. Materials and Methods. Web of Science, Scopus and Pubmed were searched for case series studying paediatric ACL reconstructions. Titles, abstracts, text, results and references were examined for documentation of growth disturbances. Incidences of graft failures were also studied in these selected studies. Results. 78 studies with 2693 paediatric ACL reconstructions had 70 growth disturbances (2.6%). Of these 17 were varus, 26 valgus, 13 shortening, 14 lengthening and 5 patients had reduced tibial slope. Coronal plane deformities were seen more frequently with eccentric physeal arrest and lengthening with intraepiphyseal tunnelling. Shortening and reduced tibial slope were related to large central physeal arrest and anterior tibial physeal arrest respectively. Extraphyseal technique were least likely to have growth disturbances. 62 studies documented 166 graft failures in 2120 patients (7.83%). Conclusion. Growth disturbances resulting from transphyseal ACL reconstruction can be minimised by keeping drill size small, drilling steep and away from the physeal periphery. Insertion of bone plug, hardware or synthetic material through the drilled physis should be avoided. The evidence to accurately quantify such growth disturbances till skeletal maturity remains weak. Robust long term studies such as national ligament registries may standardise preoperative and postoperative outcome assessment to further characterise the risk of growth disturbance and re-ruptures

Anterior vertebral body tethering (AVBT) is a growth modulating procedure used to manage idiopathic scoliosis by applying a flexible tether to the convex surface of the spine in skeletally immature patients. The purpose of this study is to determine the preliminary clinical outcomes for an adolescent patient cohort. 18 patients with scoliosis were selected using a narrow selection criteria to undergo AVBT. Of this cohort, 11 had reached a minimum follow up of 2 years, 4 had reached 18 months, and 3 had reached 6 months. These patients all demonstrated a primary thoracic deformity that was too severe for bracing, were skeletally immature, and were analysed in this preliminary study of coronal plane deformity correction. Using open-source image analysis software (ImageJ, NIH) PA radiographs taken pre-operatively and at regular follow-up visits post-operatively were used to measure the coronal plane deformity of the major and compensatory curves. Pre-operatively, the mean age was 12.0 years (S.D. 10.7 – 13.3), mean Sanders score 2.6 (S.D. 1.8-3.4), all Risser 0 and pre-menarchal, with mean main thoracic Cobb angle of 52° (S.D. 44.2-59.8°). Post-operatively the mean angle decreased to 26.4° (S.D. 18.4-32°) at 1 week, 30.4° (S.D. 21.3-39.6°) at 2 months, 25.7° (S.D. 18.7-32.8°) at 6 months, 27.9° (S.D. 16.2-39.6°) at 12 months, and 36.8° (S.D. 22.6– 51.0°) at 18 months and 38.2° (S.D. 27.6-48.7°) at 2 years. The change in curve at 2 years post-operative was statistically significant (P=0.004). There were 4 tether breakages identified that did not require return to theatre as yet, one patient underwent a posterior spinal instrumented fusion due to curve progression. AVBT is a promising new growth modulation technique for skeletally immature patients with progressive idiopathic scoliosis. This study has demonstrated a reduction in scoliosis severity

Collateral ligament release is advocated in total knee arthroplasty (TKA) to deal with significant coronal plane deformities, but is also associated with significant disadvantages. We describe steps to avoid release of the collateral (superficial medial and lateral collateral) ligaments during TKA in severely deformed knees, while correcting deformity and balancing the knee. . Introduction. Coronal plane deformity can pose difficulties with balancing in Total Ankle Replacement (TAR). Current reports outline improved outcomes in the presence of varus deformity. Soft tissue balancing techniques are well described, but are limited by no link to eitiology and pathoanatomy of the deformity. Method. A prospective review of all the TAR by the senior author was performed to identify cases of pre-operative varus greater than 10°. A chart review was performed to identify aetiology, intraoperative findings, and operative techniques to achieve a balanced TAR. X-rays were examined to measure deformity and amount of correction. Volumetric rendering and segmentalisation was performed on pre- and post-CTs to identify anatomical defects, osteophyte formation, rotational and translational changes of the hindfoot joints. Results. Between January 2002 and January 2009 there were thirty-five cases from two hundred and thirty cases with varus deformity greater than 10°, with an average 17° varus angle (range 10° to 30°). Multiple sprains and instability over several years was seen in 62% of patients. Clinically, increasing varus was associated with cavovarus foot position. Incongruent deformities had intact tibial plafond. Congruent deformities had tibial defects in the anteromedial tibial plafond and associated anterolateral tibial ostephyte. Increasing deformity often had lateral fibula osteophytes and ossicles between fibula osteophyte and anterolateral talar body. In more severe cases, 3D analysis showed the talus was anteriorly displaced and internally rotated. Post operative alignment improved from 17° to 1.5°. Conclusion. Understanding the pathoanatomy of the arthritic ankle with coronal plane deformity can help plan the surgical techniques required to correct this often challenging surgical reconstruction

Purpose. Coronal plane malalignment at the level of the tibiotalar joint is not uncommon in advanced ankle joint arthritis. It has been stated that preoperative varus or valgus deformity beyond 15 degrees is a relative contraindication and deformity beyond 20 degrees is an absolute contraindication to ankle joint replacement. There is limited evidence in the current literature to support these figures. The current study is a prospective clinical and radiographic comparative study between patients who underwent total ankle arthroplasty with coronal plane varus tibiotalar deformities greater than 10 degrees and patients with neutral alignment, less than 10 degrees of deformity. Method. Thirty-six ankles with greater than 10 degrees of varus alignment were compared to thirty-six ankles which were matched for implant type, age, gender, and year of surgery. Patients completed preoperative and yearly postoperative functional outcome scores including the American Orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot scores, the Ankle Osteoarthritis Scale (AOS) and the Short Form-36 Standard Version 2.0 Health Survey. Weightbearing preoperative and postoperative radiographs were obtained and reviewed by four examiners (AC, AQ, TD, TT) and measurements were taken of the degree of coronal plane deformity. Results. After a mean follow-up of 27 months (9–54), the varus ankles improved significantly on the AOFAS (P<0.0001), AOS-Pain Score (P<0.0001), AOS-Disability Score (P<0.0001), and SF-36 Physical Component Score (P<0.0001). There was no improvement in SF-36-Mental Component Score. (P=0.722). There was no statistically significant differences between the two groups when comparing AOFAS (P=0.155), AOS-Pain Score (P=0.854), AOSDisability Score (P=0.593), SF-36-Physical Component Score (P=0.433), SF-36 Mental Component Score (P=0.633). Sixteen of Thirty-Six ankles in the varus group needed a secondary procedure (implant failure, infection, malalignment) which was approaching significance in comparison to eight ankles in the neutral group. (P = 0.079). Secondary procedures in the varus group included: tendon transfers, calcaneal / metatarsal / malleolar ostoetomies and ligament reconstructions. Radiographically, the pre-operative coronal plane varus tibiotalar deformity averaged 19.4 6.4 and postoperatively 1.44 2.6 (P< 0.0001). There was no statistical difference in post operative tibiotalar alignment between the varus and neutral groups (P<0.05). Conclusion. The clinical outcome of TAR performed in ankles with pre-operative varus alignment >10 degrees is comparable with that of neutrally aligned ankles. The increased number of secondary procedures in the varus group was attributed to the complexity of the deformity and the steep learning curve. Outcomes as measured radiographically and through validate scores were similar to patients without deformity suggesting that varus coronal plane deformity of the talus is not a contraindication to total ankle replacement

Aims. To assess if older symptomatic children with club foot deformity differ in perceived disability and foot function during gait, depending on initial treatment with Ponseti or surgery, compared to a control group. Second aim was to investigate correlations between foot function during gait and perceived disability in this population. Methods. In all, 73 children with idiopathic club foot were included: 31 children treated with the Ponseti method (mean age 8.3 years; 24 male; 20 bilaterally affected, 13 left and 18 right sides analyzed), and 42 treated with primary surgical correction (mean age 11.6 years; 28 male; 23 bilaterally affected, 18 left and 24 right sides analyzed). Foot function data was collected during walking gait and included Oxford Foot Model kinematics (Foot Profile Score and the range of movement and average position of each part of the foot) and plantar pressure (peak pressure in five areas of the foot). Oxford Ankle Foot Questionnaire, Disease Specific Index for club foot, Paediatric Quality of Life Inventory 4.0 were also collected. The gait data were compared between the two club foot groups and compared to control data. The gait data were also correlated with the data extracted from the questionnaires. Results. Our findings suggest that symptomatic children with club foot deformity present with similar degrees of gait deviations and perceived disability regardless of whether they had previously been treated with the Ponseti Method or surgery. The presence of sagittal and coronal plane hindfoot deformity and coronal plane forefoot deformity were associated with higher levels of perceived disability, regardless of their initial treatment. Conclusion. This is the first paper to compare outcomes between Ponseti and surgery in a symptomatic older club foot population seeking further treatment. It is also the first paper to correlate foot function during gait and perceived disability to establish a link between deformity and subjective outcomes. The purpose of this study was to review the total ankle arthroplasties performed in consecutive series of 78 ankles and to determine the short-term results in cases with over 12 months follow-up. Preoperative diagnoses were post-traumatic osteoarthritis in 40 ankles (51.3%), primary osteoarthritis in 32 ankles (41.0%), and systemic arthritis in six ankles (7.7%). HINTEGRA. ®. (Newdeal SA, Lyon, France) total ankle system was used in all cases. Fifty-five total ankle arthroplasties including four revision cases, followed up for over 12 months (range, 13~49 months) were included in this study. Ankles were divided into three groups according to the coronal plane deformity in preoperative standing ankle AP radiograph; Varus (≥10°; 20 ankles (39.2%)), neutral (< 10° varus or valgus; 25 ankles (49%)), and valgus (≥10° valgus; 6 ankles (11.8%)). Various additional surgeries were performed simultaneously with the arthroplasty to correct the deformities; deltoid ligament release (25 cases), posterior tibialis tendon lengthening (2 cases), peroneus longus tendon transfer to brevis (5 cases), lateral ankle reconstruction with modified Broström procedure (4 cases), lateral closed-wedge calcaneal osteotomy (3 cases), percutaneous heel cord lengthening (19 cases), and gastrocnemius recession (1 case). In one patient with severe valgus deformity, staged total ankle arthroplasty was conducted after primary triple arthrodesis. Preoperative and postoperative visual analogue scale (VAS), American Orthopaedic Foot and Ankle Society (AOFAS) score, range of motion (ROM), as well as patient’s satisfaction and willingness to receive the operation again were evaluated The results were compared among the three groups. Serial radiographs were reviewed for any radiological changes. AOFAS score has improved from 54.3 ± 11.4 pre-operatively to 79.2 ± 11.4 at last follow-up. VAS has decreased from 6.8 ± 1.6 to 3.2 ± 1.6. Mean improvement in ROM was 15.6 ± 16.2 degrees. Forty-eight cases (873%) were satisfied with excellent or good results and 49 cases (89.1 %) were willing to receive the operation again. No significant differences in the postoperative VAS (p=0.14), AOFAS score (p=0.79), and ROM (p=0.06) were found among the three groups. Hetero-topic ossifications were observed in 12 cases (23.5%) and periosteal reactions proximal to medial malleolus occurred in four cases (7.8%). Perioperative complications include one intraoperative medial malleolus fracture which was successfully managed with two cannulated-screws, and one medial malleolar stress fracture at six weeks after surgery which has healed spontaneously. One case with osteolysis around tibial screws was managed with bone graft. One case with deep fungal infection was converted to arthrodesis after infection control. Four ankles had to be revised including three cases of polyethylene bearing change due to dislocation, and one case of tibial component and bearing change due to loosening. The patient with revised tibial component was converted to arthrodesis due to recurred loosening. The Kaplan-Meier cumulative survival rate was 90.9% at 12 months and 87.8% at 49 months postoperatively. The short term clinical results of HINTEGRA ankles showed favorable results. No significant differences were observed among different groups of coronal plane deformities when adequate additional surgeries were performed simultaneously. Long term follow-up study is required

Purpose. The correction obtained in the coronal plane knee deformity using guided growth was assessed in children with sick physes (Height<1SD of the normal children) to define the limits of this technique in sick physis. Methods. We retrospectively assessed deformity correction following guided growth using 8 plates in children with coronal plane deformity and metabolically abnormal physis- renal rickets in 6 and bone dysplasias (multiple epiphyseal dyspasia, spondyloepiphyseal dysplasia, metaphyseal dysplasia, mesomelic dysplasia, chondrodysplasia punctata) in 10 children aged 2 to 14 years. Lateral distal femoral angle(LDFA) and medial proximal tibia(MPTA) angles were serially assessed after eight plate application as a day care procedure. We noted correction achieved rate of correction, complications and additional surgical procedures. Fischer's exact test and multiple regression analysis was done to assess the effect of modifiers. Results. Sixteen children mean aged 7.8 years (2–14 years) with 9 boys and seven girls were followed for a mean of 16.125 months. Deviations from normal MPTA(n=6) were a mean of 11.82(range 8.4–16.9) and from normal LDFA(n=15) a mean of 13.96 (range 5.7–35.1). Mean rate of correction was 0.8°(range 0.1–2.65) per month in dysplasia group and 1.09°(range 0.5–1.6)per month. Complete correction was achieved in 2 out of 6 in renal and 4 out of 10 in dysplasia group. Complication included backing out of screw (1), suboptimal screw placement (2) in dysplasia group and stiffness of the knee (1). Multiple regression analysis found sex, severity of deformity >15° and duration of follow up to be not significant, however, age less than 6 years had a significantly higher chance of deformity correction(P=0.001). Conclusion. The rate of deformity correction with eight plates for guided growth is very variable even within the same patient in dysplasia group, chondrodyspalsia and SED having very slow correction rates. The deformities however correct completely before the age of 6 years irrespective of etiology. Bone dysplasias and metabolic causes require further study to understand their growth pattern and possibilities of relapse
