Purpose/Background. Health inequalities are defined as unfair and avoidable differences in health across the population. People belonging to inclusion health groups are most at risk of experiencing these inequalities and tackling them is an NHS priority. Inclusion health groups include ‘people in contact with the justice system’ and BCHC NHS FT is commissioned to provide physiotherapy to prisoners at HMP Birmingham. The purpose of this study was to better understand the MSK health-status and MSK conditions affecting this minoritised patient population. Methods/Results. The MSK service at HMP Birmingham collects the MSK-HQ PROM for all prisoners attending physiotherapy. A retrospective analysis of the electronic patient record was performed using convenience sampling, with baseline MSK-HQ scores of prisoners seen between 01/04/2023 and 01/02/2024 captured alongside the primary presenting MSK condition. Simple descriptive statistics were used to evaluate the data. A total of 129 patients attended initial physiotherapy appointments within the time-period. Baseline MSK-HQ was collected for 62 patients (48%) with a mean (SD) score of 23.08(11.38). The most common MSK condition was Back pain with a count of 12(19.35%). The mean (SD) MSK-HQ score for prisoners with back pain was 22.08(9.98). Conclusion. This is the first known evaluation in the published literature of the baseline MSK health-status and primary MSK conditions affecting prisoners. A published evaluation of baseline MSK health-status for patients attending a community MSK service reported a mean (SD) MSK-HQ of 28.59(10.80) indicating that, a poorer MSK health-status is experienced by this minoritised group. This study investigated the effects on friction of changing the dimensions of a ball-and-socket Total Disc Arthroplasty (TDA). A generic ball-and-socket model was designed and manufactured based on the dimensions and geometry of a metal-on-metal Maverick (Medtronic, Minneapolis, USA) device. Keeping the radial clearance similar to the Maverick, the ball and socket dimensions varied between 10 to 16 mm and 10.015 to 16.015 mm, respectively, in order to enable the comparison between different dimensions. The implants were made out of Cobalt Chrome Molybdenum alloy, with a surface roughness of 0.05 μm. A Bose spine simulator (Bose Corporation, ElectroForce Systems Group, Minnesota, USA) was used to apply an axial compressive force to the TDA. Axial rotation of ±2° was then applied at various frequencies and the resulting frictional torque measured. The tests were performed under an axial load of 50, 600 and 1200 N and frequencies of 0.5, 1.0, 1.5 and 2.0 Hz, for four different samples of radii 10, 12, 14 and 16 mm (48 combinations in total). The results showed variation of frictional torque in different frequencies for all four samples under constant axial load. It was observed that the frictional torque had the lowest value for the implant with ball radius of 16mm. It might be concluded that the implant with larger ball radius may create less friction and hence offer a longer life

Aims

Patients with cauda equina syndrome (CES) require emergency imaging and surgical decompression. The severity and type of symptoms may influence the timing of imaging and surgery, and help predict the patient’s prognosis. Categories of CES attempt to group patients for management and prognostication purposes. We aimed in this study to assess the inter-rater reliability of dividing patients with CES into categories to assess whether they can be reliably applied in clinical practice and in research.

Methods

A literature review was undertaken to identify published descriptions of categories of CES. A total of 100 real anonymized clinical vignettes of patients diagnosed with CES from the Understanding Cauda Equina Syndrome (UCES) study were reviewed by consultant spinal surgeons, neurosurgical registrars, and medical students. All were provided with published category definitions and asked to decide whether each patient had ‘suspected CES’; ‘early CES’; ‘incomplete CES’; or ‘CES with urinary retention’. Inter-rater agreement was assessed for all categories, for all raters, and for each group of raters using Fleiss’s kappa.

Background. Chronic pain is a complex condition that demonstrates better outcomes in multidisciplinary rehabilitation, typically delivered to groups of patients by tertiary healthcare teams. An inter-disciplinary pain management course for individual patients was developed to increase the scope of physical therapists working in primary care by integrating osteopathic manual therapy with psychological interventions from Acceptance and Commitment Therapy (ACT), a form of ‘3rd wave’ Cognitive Behaviour Therapy. Method and Results. A single cohort study with pre-course (n=180) and post-course (n=79) self-report measures (44% response rate) evaluated six week interventions which combined individual manual therapy with self-management, delivered by teams of qualified and student osteopaths. Data included: quality of life (European Quality of Life Questionnaire); pain, mood and coping (Bournemouth Questionnaire); psychological flexibility (Revised Acceptance and Action Questionnaire); and mindfulness (Freiburg Mindfulness Inventory). Participants were predominantly female (68%), unemployed (59%), with an average age of 49 and pain duration of more than 12 months (86%). Commonly reported symptoms were low back pain (82%), neck pain (60%) and multiple sites (86%). At six months, there were statistically significant improvements in all four outcome measures (p<0.0005), with promising effect sizes in quality of life and pain coping (r=0.52) which appeared to be mediated by changes in psychological flexibility. Conclusions. This innovative, integrated, patient-centred chronic pain management course demonstrated promising outcomes when delivered by osteopaths with varying experience. Randomised clinical trials are now needed to assess outcomes in comparison with standard care, and optimal ways of training physical therapists to deliver effective psychological interventions. Spinal Deformity Service, Royal Orthopaedic Hospital, Birmingham, UK. To describe the technique of nonfusion annulotomy and nuclectomy with posterior growing rod instrumentation for the treatment of non-congenital early onset scoliosis. To present our results of the application of this technique in a consecutive series of nine patients with mean follow up of 76 months (range 16 to 123 months). We undertook retrospective observational casenote and radiograph study of patients with noncongenital early onset scoliosis having annulotomy and nuclectomy at the apex of their respective curves with standard posterior growing rod instrumentation between 1998 and 2009. 10 patients were identified with one excluded due to short follow up period. Of the nine patients included mean follow up was 76 months. Mean age at primary surgery was 71.7 months (range 29- 97 months). We measured pre and post operative Cobb angle, T1-S1 height change and change in height over the apical segments. Mean pre op cobb angle was 74° (range 62- 81°). Mean post op cobb angle was 38°. Three patients have completed their treatment programme with a mean number of 9 lengthenings achieved. Six patients are still undergoing lengthenings. CT demonstrates that the annulotmized segment does not fuse at a mean 3.5 years postoperatively. All patients demonstrated growth over total spinal height and also over apical segments which had undergone annultomy/ nuclectomy. We also report complications. We have demonstrated that anterior annulotomy and nuclectomy with posterior growing rod construct does allow for spinal growth over released segments without autofusion

Aims

The primary aim of this study was to evaluate the performance and safety of magnetically controlled growth rods in the treatment of early onset scoliosis. Secondary aims were to evaluate the clinical outcome, the rate of further surgery, the rate of complications, and the durability of correction.

Patients and Methods

We undertook an observational prospective cohort study of children with early onset scoliosis, who were recruited over a one-year period and followed up for a minimum of two years. Magnetically controlled rods were introduced in a standardized manner with distractions performed three-monthly thereafter. Adverse events which were both related and unrelated to the device were recorded. Ten children, for whom relevant key data points (such as demographic information, growth parameters, Cobb angles, and functional outcomes) were available, were recruited and followed up over the period of the study. There were five boys and five girls. Their mean age was 6.2 years (2.5 to 10).

Objectives

To evaluate the neck strength of school-aged rugby players, and to define the relationship with proxy physical measures with a view to predicting neck strength.

Methods

Cross-sectional cohort study involving 382 rugby playing schoolchildren at three Scottish schools (all male, aged between 12 and 18 years). Outcome measures included maximal isometric neck extension, weight, height, grip strength, cervical range of movement and neck circumference.
