Aims

The prevalence of obesity is increasing substantially around the world. Elevated BMI increases the risk of complications following total hip arthroplasty (THA). We sought to evaluate trends in BMI and complication rates of obese patients undergoing primary THA over the last 30 years.

Methods

Through our institutional total joint registry, we identified 15,455 primary THAs performed for osteoarthritis from 1990 to 2019. Patients were categorized according to the World Health Organization (WHO) obesity classification and groups were trended over time. Cox proportional hazards regression analysis controlling for confounders was used to investigate the association between year of surgery and two-year risk of any reoperation, any revision, dislocation, periprosthetic joint infection (PJI), venous thromboembolism (VTE), and periprosthetic fracture. Regression was stratified by three separate groups: non-obese; WHO Class I and Class II (BMI 30 to 39 kg/m2); and WHO Class III patients (BMI ≥ 40 kg/m2).

Aims

This study compared multiple sclerosis (MS) patients who underwent primary total hip arthroplasty (THA) with a matched cohort. Specifically, we evaluated: 1) implant survivorship; 2) functional outcomes (modified Harris Hip Scores (mHHS), Hip Disability and Osteoarthritis Outcome Score, Joint Replacement (HOOS JR), and modified Multiple Sclerosis Impact Scale (mMSIS) scores (with the MS cohort also evaluated based on the disease phenotype)); 3) physical therapy duration and return to function; 4) radiographic outcomes; and 5) complications.

Patients and Methods

We reviewed our institution’s database to identify MS patients who underwent THA between January 2008 and June 2016. A total of 34 MS patients (41 hips) were matched in a 1:2 ratio to a cohort of THA patients who did not have MS, based on age, body mass index (BMI), and Charlson/Deyo score. Patient records were reviewed for complications, and their functional outcomes and radiographs were reviewed at their most recent follow-up.

Total hip replacement (THR) has been shown to be a cost-effective procedure. However, it is not risk-free. Certain conditions, such as diabetes mellitus, are thought to increase the risk of complications. In this study we have evaluated the prevalence of diabetes mellitus in patients undergoing THR and the associated risk of adverse operative outcomes. A meta-analysis and systematic review were conducted according to the guidelines of the meta-analysis of observational studies in epidemiology. Inclusion criteria were observational studies reporting the prevalence of diabetes in the study population, accompanied by reports of at least one of the following outcomes: venous thromboembolic events; acute coronary events; infections of the urinary tract, lower respiratory tract or surgical site; or requirement for revision arthroplasty. Altman and Bland’s methods were used to calculate differences in relative risks. The prevalence of diabetes mellitus was found to be 5.0% among patients undergoing THR, and was associated with an increased risk of established surgical site infection (odds ratio (OR) 2.04 (95% confidence interval (CI) 1.52 to 2.76)), urinary infection (OR 1.43 (95% CI 1.33 to 1.55)) and lower respiratory tract infections (OR 1.95 (95% CI 1.61 to 2.26)). Diabetes mellitus is a relatively common comorbidity encountered in THR. Diabetic patients have a higher rate of developing both surgical site and non-surgical site infections following THR.

Aims. Delirium is associated with adverse outcomes following hip fracture, but the prevalence and significance of delirium for the prognosis and ongoing rehabilitation needs of patients admitted from home is less well studied. Here, we analyzed relationships between delirium in patients admitted from home with 1) mortality; 2) total length of hospital stay; 3) need for post-acute inpatient rehabilitation; and 4) hospital readmission within 180 days. Methods. This observational study used routine clinical data in a consecutive sample of hip fracture patients aged ≥ 50 years admitted to a single large trauma centre during the COVID-19 pandemic between 1 March 2020 and 30 November 2021. Delirium was prospectively assessed as part of routine care by the 4 A’s Test (4AT), with most assessments performed in the emergency department. Associations were determined using logistic regression adjusted for age, sex, Scottish Index of Multiple Deprivation quintile, COVID-19 infection within 30 days, and American Society of Anesthesiologists grade. Results. A total of 1,821 patients were admitted, with 1,383 (mean age 79.5 years; 72.1% female) directly from home. Overall, 87 patients (4.8%) were excluded due to missing 4AT scores. Delirium prevalence in the whole cohort was 26.5% (460/1,734): 14.1% (189/1,340) in the subgroup of patients admitted from home, and 68.8% (271/394) in the remaining patients (comprising care home residents and inpatients when fracture occurred). In patients admitted from home, delirium was associated with a 20-day longer total length of stay (p < 0.001). In multivariable analyses, delirium was associated with higher mortality at 180 days (odds ratio (OR) 1.69 (95% confidence interval (CI) 1.13 to 2.54); p = 0.013), requirement for post-acute inpatient rehabilitation (OR 2.80 (95% CI 1.97 to 3.96); p < 0.001), and readmission to hospital within 180 days (OR 1.79 (95% CI 1.02 to 3.15); p = 0.041). Conclusion. Delirium affects one in seven patients with a hip fracture admitted directly from home, and is associated with adverse outcomes in these patients. Aims. This work aimed at answering the following research questions: 1) What is the rate of mechanical complications, nonunion and infection for head/neck femoral fractures, intertrochanteric fractures, and subtrochanteric fractures in the elderly USA population? and 2) Which factors influence adverse outcomes?. Methods. Proximal femoral fractures occurred between 1 January 2009 and 31 December 2019 were identified from the Medicare Physician Service Records Data Base. The Kaplan-Meier method with Fine and Gray sub-distribution adaptation was used to determine rates for nonunion, infection, and mechanical complications. Semiparametric Cox regression model was applied incorporating 23 measures as covariates to identify risk factors. Results. Union failure occured in 0.89% (95% confidence interval (CI) 0.83 to 0.95) after head/neck fracturs, in 0.92% (95% CI 0.84 to 1.01) after intertrochanteric fracture and in 1.99% (95% CI 1.69 to 2.33) after subtrochanteric fractures within 24 months. A fracture-related infection was more likely to occur after subtrochanteric fractures than after head/neck fractures (1.64% vs 1.59%, hazard ratio (HR) 1.01 (95% CI 0.87 to 1.17); p < 0.001) as well as after intertrochanteric fractures (1.64% vs 1.13%, HR 1.31 (95% CI 1.12 to 1.52); p < 0.001). Anticoagulant use, cerebrovascular disease, a concomitant fracture, diabetes mellitus, hypertension, obesity, open fracture, and rheumatoid disease was identified as risk factors. Mechanical complications after 24 months were most common after head/neck fractures with 3.52% (95% CI 3.41 to 3.64; currently at risk: 48,282). Conclusion. The determination of complication rates for each fracture type can be useful for informed patient-clinician communication. Risk factors for complications could be identified for distinct proximal femur fractures in elderly patients, which are accessible for therapeutical treatment in the management. Introduction. The association between preoperative opioid use and adverse outcomes after total hip arthroplasty (THA) has been reported. However, a quantitative assessment used to evaluate the combined effect of controlled substance use and define a threshold for adverse outcomes after THA has not been established. The current study aimed to identify the association between the NarxCare Score (NCS) (indicative of controlled substance use) and length of stay (LOS), discharge disposition, 90-day emergency department (ED) visits, readmission, and reoperation after primary THA, necessary to identify a preoperative NCS threshold for adverse outcomes. Methods. A total of 2,787 THAs (57.3% female; mean age: 64.3±11.14 years; NCS range: 0–800) were performed between November 2018-January 2020 at an integrated healthcare system with preoperative NCSs. Outcomes assessed included LOS, discharge disposition, 90-day ED visits, readmission (all-cause, procedure and non-procedure-related) and reoperation. The association between NCS category (in 100-point increments) and was analyzed through multivariate logistic regression accounting for risk factors. Results. NCS of zero, 1–99 and 100–199 were similar regarding all investigated outcomes (p>0.05 for all). Compared to a NCS of zero, NCS of 300–399 was independently associated with higher odds of a LOS >2 days (odds ratio [OR]:2.27; 95% confidence interval [CI][1.59–3.24]; p<0.001), non-home discharge (OR: 2.0; 95% CI [1.34–2.99]; p=0.001), 90-day all-cause readmission (OR: 2.0; 95% CI [1.16–3.36]; p=0.012). Furthermore, NCS of 300–399 exhibited higher odds of a procedure-related readmission (OR: 3.74; 95% CI [1.48–8.13]; p=0.004). There were no differences between the NCS strata in 90-day reoperation and non-procedure related readmission rates (p>0.05 for each). Conclusion. Higher NCS was associated with greater LOS and 90-day readmission, as well as lower odds of discharge to home. A NCS threshold of 300 can potentially designate high-risk patients for adverse outcomes after THA. This score may help stratify patients in the current alternative payment environment

Ceramic bearing fractures are rare events, but mandate revision and implantation of new bearings. Revisions using metal heads have been reported to lead to gross volumetric head wear (due to abrasive retained ceramic micro-debris), cobalt toxicity, multi-organ failure and death. Such complications are widely published (50+ reports), yet we know that patients continue to be put at risk. Using data from the NJR and AOANJRR, this study seeks to compare the risk of re-revision and death by revision bearing combination following a ceramic bearing fracture. Data were extracted from the NJR and AOANJRR, identifying revisions for ceramic bearing fracture. Subsequent outcomes of survival, re-revision and death were compared between revision bearing combinations (ceramic-on-ceramic, ceramic-on-polyethylene, and metal-on-polyethylene). 366 cases were available for analysis from the NJR dataset (MoP=34, CoP=112, CoC=221) and 174 from the AOANJRR dataset (MoP=17, CoP=44, CoC=113). The overall incidence rate of adverse outcome (revision or death) was 0.65 for metal heads and 0.23 for ceramic head articulations (p=0.0012) across the whole time period (NJR). Kaplan-Meir survival estimates demonstrate an increased risk of both re-revision and death where a metal head has been used vs a ceramic head following revision for ceramic fracture. There are few decisions in arthroplasty surgery that can lead to serious harm or death for our patients, but revision using a metal head following ceramic bearing fracture is one of them. This study enhances the signal of what is already known but previously only reported as inherently low-level evidence (case reports and small series) due to event rarity. Use of a metal head in revision for ceramic fracture represents an avoidable patient safety issue, which revision guidelines should seek to address

Introduction. Quality measures play a substantial role in the Centers for Medicare and Medicaid Services' hospital payment and public reporting programs. The purpose of this study was to assess whether public measurement of total hip and knee arthroplasty risk-standardized readmission (RSRRs) and complication rates (RSCRs) was temporally associated with decreasing rates of adverse outcomes among Medicare beneficiaries. Methods. We evaluated annual trends in hospital-level risk-standardized complication and readmission rates in the fiscal years 2010–11 and 2016–17 for patients undergoing hospital-based inpatient hip/knee replacement procedures. We calculated hospital-level rates using the same measures and methodology used in public reporting. We examined annual trends in the distribution of hospital-level outcomes through density plots (similar to histograms). Results. Both rates of and variation in the complication and readmission outcomes declined steadily between 2010–11 and 2016–17. We noted a 33% relative reduction in hospital-level risk-standardized complication rates (RSCRs) and 25% relative reduction in hospital-level risk-standardized readmission rates (RSRRs). The interquartile range of RSCRs decreased by 18% (relative reduction) and of RSRRs decreased by 34% (relative reduction). Risk variable frequency in complication and readmission models did not systematically change over time, suggesting no evidence of widespread bias or up-coding. Conclusion. This study demonstrates that hospital-level complication and readmission rates after hip/knee replacements and variation in hospital-level performance declined following the start of public reporting and financial incentives associated with measurement. The consistent decreasing trend in rates of and variation outcomes suggests steady improvements and greater consistency among hospitals in clinical outcomes for hip/knee replacement patients in the years 2016–17 compared to 2010–11. While improvements in surgical practices and care coordination may partially account for these findings, the precise influence of measure public reporting on clinical outcomes is difficult to ascertain. Interactions between public reporting, payment, and hospital coding practices are complex and require continued study


Outcomes following metal-on-metal hip replacement (MoMHR) revision surgery for adverse reactions to metal debris (ARMD) have been poor, and inferior compared with non-ARMD revisions. Subsequently, surgeons and worldwide authorities widely recommended early revision for ARMD, with a lower surgical threshold adopted. However, the impact of early surgery for ARMD is unknown. We compared the rates of adverse outcomes following MoMHR revision surgery in matched ARMD and non-ARMD patients. We performed a retrospective observational study using data from the National Joint Registry for England, Wales, Northern Ireland and the Isle of Man. All MoMHR patients subsequently undergoing revision surgery for any indication between August 2008 and August 2014 were eligible. ARMD and non-ARMD revisions were matched one-to-one for multiple potential confounding factors using propensity scores. Adverse outcomes following revision surgery (intra-operative complications, mortality, re-revision surgery) were compared between matched groups using regression analysis. In 2,576 matched MoMHR revisions (ARMD=1,288 and non-ARMD=1,288), intra-operative complications were similar between ARMD (2.4%) and non-ARMD (2.5%) revisions (odds ratio=0.97, 95% CI=0.59–1.60; p=0.899). All-cause mortality rates were lower following ARMD revision compared with non-ARMD revision (hazard ratio (HR)=0.43, 95% CI=0.22–0.86; p=0.018). All-cause re-revision rates were lower following ARMD revision compared with non-ARMD revision (HR=0.52, 95% CI=0.36–0.75; p<0.001). Compared with ARMD revision (5-years=94.3%), MoMHR revisions for infection (5-years=81.2%) and dislocation/subluxation (5-years=81.9%) had the lowest implant survival rates. Contrary to previous observations, MoMHRs revised for ARMD have approximately half the risk of re-revision and death compared to non-ARMD revisions. We suspect worldwide regulatory authorities have positively influenced outcomes following ARMD revision by widely recommending that surgeons exercise a lower revision threshold. Our findings suggest the threshold for ARMD revision surgery need not be lowered further. The high risk of failure following MoMHR revision for infection and dislocation is concerning

Background. Postoperative recovery after routine total hip arthroplasty (THA) can lead to the development of prolonged opioid use but there are few tools for predicting this adverse outcome. The purpose of this study was to develop machine learning algorithms for preoperative prediction of prolonged post-operative opioid use after THA. Methods. A retrospective review of electronic health records was conducted at two academic medical centers and three community hospitals to identify adult patients who underwent THA for osteoarthritis between January 1. st. , 2000 and August 1. st. , 2018. Prolonged postoperative opioid prescriptions were defined as continuous opioid prescriptions after surgery to at least 90 days after surgery. Five machine learning algorithms were developed to predict this outcome and were assessed by discrimination, calibration, and decision curve analysis. Results. Overall, 5507 patients underwent THA, of which 345 (6.3%) had prolonged postoperative opioid prescriptions. The factors determined for prediction of prolonged postoperative opioid prescriptions were: age, duration of pre-operative opioid exposure, preoperative hemoglobin, and certain preoperative medications (anti-depressants, benzodiazepines, non-steroidal anti-inflammatory drugs, and beta-2-agonists). The elastic-net penalized logistic regression model achieved the best performance across discrimination (c-statistic = 0.77), calibration, and decision curve analysis. If externally validated in independent populations, the algorithms developed in this study could improve preoperative screening and support for THA patients at high-risk for prolonged postoperative opioid use. Aims

Hip arthroscopy has gained prominence as a primary surgical intervention for symptomatic femoroacetabular impingement (FAI). This study aimed to identify radiological features, and their combinations, that predict the outcome of hip arthroscopy for FAI.

Methods

A prognostic cross-sectional cohort study was conducted involving patients from a single centre who underwent hip arthroscopy between January 2013 and April 2021. Radiological metrics measured on conventional radiographs and magnetic resonance arthrography were systematically assessed. The study analyzed the relationship between these metrics and complication rates, revision rates, and patient-reported outcomes.

Introduction. Smoking, a modifiable factor, may adversely affect post-operative outcomes. Healthcare providers are increasingly denying smokers access to total hip arthroplasty (THA) until they stop smoking. Evidence supporting this is unclear. We assessed the effect of smoking on outcomes following THA. Patients and Methods. We performed a retrospective observational study involving 60,812 THAs (12.4%=smokers, 31.2%=ex-smokers, 56.4%=non-smokers) from the Clinical Practice Research Datalink. Data were linked with Hospital Episode Statistics and the Office for National Statistics to identify outcomes. The effect of smoking on post-operative outcomes (complications, medications, revision, mortality, PROMs) was assessed using adjusted regression. Results. Following THA, smokers had a significantly increased risk of lower respiratory tract infection (odds ratio (OR)=0.53; 95% CI=0.43–0.64), myocardial infarction (OR=0.41; CI=0.24–0.71), cerebrovascular disease (OR=0.54; CI=0.32–0.93), and ischaemic heart disease (OR=0.62; CI=0.43–0.91) compared with non-smokers. The risk of these complications in smokers was also significantly higher compared with ex-smokers. The risk of other complications, including DVT and wound infection, was similar between smoking groups. Compared with non-smokers (OR=0.55; CI=0.51–0.60) and ex-smokers (OR=0.85; CI=0.78–0.92), smokers had increased opioid usage at one-year post-surgery. Similar patterns were observed for weak opioids and paracetamol. One-year mortality rates were higher in smokers compared with non-smokers (hazard ratio (HR)=0.39, CI=0.30–0.50) and ex-smokers (HR=0.50, CI=0.39–0.65). Long-term revision rates were not increased in smokers. Smokers had significant improvement in PROMs compared with pre-operatively, with no clinically important difference in postoperative PROMs between smokers, non-smokers and ex-smokers. Discussion. Smoking was associated with more medical complications (namely vascular), higher analgesia usage, and increased mortality following THA. Most adverse outcomes were reduced in ex-smokers, therefore smoking cessation should be encouraged before arthroplasty. However, THA is clinically effective in smokers, who gain meaningful PROM improvement with no increased revision risk. Conclusion. Smokers should not be universally denied access to primary THA

Aims

Periprosthetic fracture and implant loosening are two of the major reasons for revision surgery of cementless implants. Optimal implant fixation with minimal bone damage is challenging in this procedure. This pilot study investigates whether vibratory implant insertion is gentler compared to consecutive single blows for acetabular component implantation in a surrogate polyurethane (PU) model.

Methods

Acetabular components (cups) were implanted into 1 mm nominal under-sized cavities in PU foams (15 and 30 per cubic foot (PCF)) using a vibratory implant insertion device and an automated impaction device for single blows. The impaction force, remaining polar gap, and lever-out moment were measured and compared between the impaction methods.

Aims

Hip fractures are some of the most common fractures encountered in orthopaedic practice. We aimed to identify whether perioperative hypotension is a predictor of 30-day mortality, and to stratify patient groups that would benefit from closer monitoring and early intervention. While there is literature on intraoperative blood pressure, there are limited studies examining pre- and postoperative blood pressure.

Methods

We conducted a prospective observational cohort study over a one-year period from December 2021 to December 2022. Patient demographic details, biochemical results, and haemodynamic observations were taken from electronic medical records. Statistical analysis was conducted with the Cox proportional hazards model, and the effects of independent variables estimated with the Wald statistic. Kaplan-Meier survival curves were estimated with the log-rank test.

Aims

The aim of this study was to determine the clinical outcomes and factors contributing to failure of transposition osteotomy of the acetabulum (TOA), a type of spherical periacetabular osteotomy, for advanced osteoarthritis secondary to hip dysplasia.

Methods

We reviewed patients with Tönnis grade 2 osteoarthritis secondary to hip dysplasia who underwent TOA between November 1998 and December 2019. Patient demographic details, osteotomy-related complications, and the modified Harris Hip Score (mHHS) were obtained via medical notes review. Radiological indicators of hip dysplasia were assessed using preoperative and postoperative radiographs. The cumulative probability of TOA failure (progression to Tönnis grade 3 or conversion to total hip arthroplasty) was estimated using the Kaplan-Meier product-limited method. A multivariate Cox proportional hazards model was used to identify predictors of failure.

Aims

This study describes the variation in the annual volumes of revision hip arthroplasty (RHA) undertaken by consultant surgeons nationally, and the rate of accrual of RHA and corresponding primary hip arthroplasty (PHA) volume for new consultants entering practice.

Methods

National Joint Registry (NJR) data for England, Wales, Northern Ireland, and the Isle of Man were received for 84,816 RHAs and 818,979 PHAs recorded between April 2011 and December 2019. RHA data comprised all revision procedures, including first-time revisions of PHA and any subsequent re-revisions recorded in public and private healthcare organizations. Annual procedure volumes undertaken by the responsible consultant surgeon in the 12 months prior to every index procedure were determined. We identified a cohort of ‘new’ HA consultants who commenced practice from 2012 and describe their rate of accrual of PHA and RHA experience.

Introduction. Despite evidence-based recommendations opioids and benzodiazepines are commonly prescribed to treat osteoarthritis. This study aims to quantify the prevalence of opioid and benzodiazepine prescriptions given for the primary diagnosis of osteoarthritis across a large healthcare system. Additionally, we aim to characterize risk factors for drug misuse, abuse, and diversion. Methods. We conducted a descriptive analysis of adult patient encounters with a primary diagnosis of osteoarthritis during a one-year time period from Jan –Dec 2016 at a large, healthcare system. Demographic data, prescription data, patient-specific prescription drug use risk criteria were collected. Descriptive analysis was conducted to characterize the population of arthritis patients who received prescription opioids. The rate of controlled substance prescribing was calculated for the population as a whole and among subgroups (age, facility type, and patient risk profile). Results. During the one-year time frame, our system had 45,341 outpatient encounters with a primary diagnosis of osteoarthritis. Almost a third of these encounters resulted in a prescription for an opioid or benzodiazepine (n = 7,375; 31.9%). The majority of patients were between ages 18 and 64 (57.1%), with 42.9% patients >65. While 38.8% were prescribed in an acute care setting (ED or Urgent Care), the majority were in outpatient clinics (58.8%) with 36.2 % of outpatient encounters originating from orthopaedic clinics. 14.3% of those who received a prescription had a risk factor for prescription misuse, the most prevalent being “early refill” (7.0%). Conclusion. Opioids remain a prevalent treatment for osteoarthritis, given in over a third of encounters. These prescriptions pose a risk for adverse outcomes, since over a third of patients had a risk factor for misuse. Continued efforts to improve compliance with evidence-based guidelines as well as alternative pain management pathways are critical to help curb the use of opioids for management of osteoarthritis

Introduction. Malposition of the acetabular component in total hip arthroplasty (THA) is linked to multiple adverse outcomes. Changes in the sagittal plane position of the pelvis, owing both to patient positioning in the operating room and to altered spinopelvic alignment following surgery, potentially contribute to variation in component position. The dynamics of sagittal plane pelvic position before, during, and after THA have not been defined. We measured the differences in pelvic ratio, a measure of sagittal plane pelvic position, between preoperative, intraoperative, and postoperative anteroposterior (AP) radiographs of patients undergoing THA in the lateral decubitus position. Methods. We retrospectively compared the radiographic pelvic ratio among 90 patients undergoing THA. AP radiographs were obtained in the standing position preoperatively and at 6 weeks after surgery; in the lateral decubitus position after trial reduction intraoperatively; and in the supine position in the post anesthesia care unit (PACU). Pelvic ratio was defined as the ratio between the vertical distance from the inferior sacroiliac (SI) joints to the superior pubic symphysis and the horizontal distance between the inferior SI joints. Radlink software was used to determine the pelvic ratio on each radiograph. Changes in apparent cup position based on changes in pelvic ratio were calculated using data from the literature, and a change of at least 10 degrees in acetabular component position was defined as clinically meaningful. Analyses were performed using paired t-tests, with p<0.05 defined as significant. Results. 54% of patients had a change in pelvic ratio large enough to alter the apparent acetabular component anteversion by 10 degrees (49% increased and 6% decreased), and 12% had a change large enough to alter the apparent acetabular component inclination by 10 degrees (12% increased and 0% decreased) when the intraoperative AP radiograph was compared to the preoperative AP radiograph. 36% of patients had a change in pelvic ratio from the preoperative radiograph to the 6 week preoperative radiograph large enough to alter the apparent acetabular component anteversion by 10 degrees (5% increased and 31% decreased), and 8% had a change large enough to alter the apparent acetabular component inclination by 10 degrees (6% increased and 1% decreased). Discussion. Changes in the sagittal plane pelvic position between preoperative, intraoperative, and postoperative radiographs occur in a substantial number of patients. These changes correspond to altered functional position of the acetabular component in over half of patients on the intraoperative radiograph and over one third of patients on postoperative radiographs. This variability suggests that intraoperative imaging may be useful for avoiding outliers of component position, and calls into question the feasibility of achieving targeted component positions based on preoperative imaging alone

Aims

Leg length discrepancy (LLD) is a common pre- and postoperative issue in total hip arthroplasty (THA) patients. The conventional technique for measuring LLD has historically been on a non-weightbearing anteroposterior pelvic radiograph; however, this does not capture many potential sources of LLD. The aim of this study was to determine if long-limb EOS radiology can provide a more reproducible and holistic measurement of LLD.

Methods

In all, 93 patients who underwent a THA received a standardized preoperative EOS scan, anteroposterior (AP) radiograph, and clinical LLD assessment. Overall, 13 measurements were taken along both anatomical and functional axes and measured twice by an orthopaedic fellow and surgical planning engineer to calculate intraoperator reproducibility and correlations between measurements.

Outcomes for patients with acute illnesses may be affected by the day of the week they present to hospital. Policy makers state this ‘weekend effect’ to be the main reason for pursuing a change in consultant weekend working patterns. However, it is uncertain whether such a phenomenon exists for elective orthopaedic surgery. This study investigated whether a ‘weekend effect’ contributed to adverse outcomes in patients undergoing elective hip and knee replacements. Retrospectively collected data was obtained from our institutions electronic patient records. Using univariate analysis, we examined potential risk factors including; Age, Sex, ASA Grade, Comorbidities, as well as the day of the week, hospital admission and surgery occurred. Subsequent multivariate analyses identified covariate- adjusted risk factors, associated with prolonged hospital stays. 30-day mortality data was assessed according to the day of the week surgery was performed. 892 patients underwent arthroplasty surgery from 01/09/2014 till the 31/08/2015. 457 patients had a total hip and 435 had a total knee replacement. 814 patients (91.3%) underwent surgery during the week, while 78 (8.7%) had surgery on a Saturday. There was no difference in the average Length of Stay (LOS) between groups (5.0, 2.6 versus 5.0, 3.4, p=0.95), and weekend surgery was not associated with a LOS greater than 4 days. The two variables found to be associated with a prolonged LOS were; increasing age (RR) 1.02 (95% CI: 1.01–1.03, p<0.001) and an ASA score of 2, (RR) 1.6 (95% CI: 1.15 − 2.20, p=0.005). There was one death in a patient who was ASA III, and who underwent surgery on a Monday. There is no ‘weekend effect’ for elective orthopaedic surgery. Changes in consultant weekend working patterns are unlikely to have any effect on mortality or LOS for elective orthopaedic patients
