Purpose

We analyzed a consecutive series of 36 total hip arthroplasties using cementless conical stem with a shortening osteotomy combined with greater trochanter transfer in cases with a high dislocated hip.

Material and Methods

The causes of total hip arthroplasty were the sequelae of a septic hip in 20 cases and developmental dysplasia of the hip (DDH) in 16 cases. Mean patient age was 43.4 and the mean follow-up period was 3.3 years. We compared perioperative parameters, clinical, radiological results and complications between the two groups.

Purpose

Bilateral simultaneous and staged total hip arthroplasty has been issues in terms of safety and costs with development of surgical technique, postoperative medical care. The purpose of this prospective study is to compare the outcomes of simultaneous and staged bilateral operations, in terms of outcome, safety, and socioeconomic-effectiveness.

Patients and Methods

All patients(470 cases) that underwent simultaneous or staged bilateral THA using modified minimally invasive two-incision technique between January 2004 and November 2009 were registered, and after exclusion divided into two groups; simultaneous bilateral THA group (Group A: 171 patients) and staged bilateral THA group (Group B: 64 patients) by patient's condition and preference. Staged operations were performed at the time when patients want to get surgery due to undurable pain. For clinical evaluations, amounts of blood loss, blood transfusion, postoperative morbidity and mortality were compared. And for socioeconomic-effectiveness, costs for hospitalization and time for returning to previous job were investigated.

Purpose

The purpose of this study is to know the peri-operative morbidity, clinical and radiographical outcomes of conversion THA from failed transtrochanter rotational osteotomy (TRO).

Patients and methods

From 2003 January to 2009 January, there were 18 hips(18 patients) who underwent conversion THA from TRO for osteonecrosis of the femoral head (ONFH) (Group I). The mean duration from TRO to conversion THA was 2.6 years. We made a matched control group of 18 primary THA for ONFH (Group II) and we evaluated perioperative morbidity and complications in each group. For the clinical evaluation, we checked Harris hip score (HHS) and WOMAC score. For the radiographical evaluation, we evaluated implant position, stability and osteolysis.

Introduction

This study was performed to evaluate the minimum 5-year clinical and radiological results of liner cementation into a stable acetabular shell using a metal-inlay, polyethylene liner during revision total hip arthroplasty (THA).

Methods

Sixty-six hips (63 patients) that underwent revision THA using a metal-inlay polyethylene liner cementation were included. The causes of revision were; polyethylene wear in 37 cases, femoral stem loosening in 20 cases, ceramic head fracture in 4 cases, and recurrent dislocation in 5 cases. Clinical results were graded at final follow-up using Harris hip scores, and radiographs were evaluated to determine acetabular component inclination, the stabilities of acetabular and femoral components, correction of hip centers, and the progression of osteolysis.

Purpose

Hip arthroplasty is a good treatment option for displaced femoral neck fracture in elderly patients. However, neuromuscular disease such as cerebral infarction or hemorrhage can be a concerning problem for THA since dislocation after operation can frequently occur. The purpose of this prospective study was to evaluate the functional results of modified minimally invasive (MI) two-incision total hip arthroplasty (THA) with the use of large-diameter (>38mm) metal-on-metal articulation in patients with muscle weakness.

Patients and Methods

19 consecutive patients (19 hips) with displaced femoral neck fracture with muscle weakness were enrolled. There were 11 patients with cerebral infarction, 4 patients with cerebral hemorrhage and 4 patients with Parkinson's disease. In the lateral position, an anterolateral approach between the gluteus medius and tensor fascia lata and a posterior approach between the piriformis and gluteus medius were used.

Surgical morbidity, functional recovery, radiological implantation properties, range of motion (ROM) and complications were assessed.

Introduction: Alendronate is a well known inhibitor of osteoclastic activity and is used for the treatment of various metabolic bone diseases. Recent studies indicate that alendronate may have additional effect on osteoblastic activity. In this study, we evaluated the effect of alendronate on the osteogenic differentiation of mouse mesenchymal stem cells.

Materials and Methods: D1 cells, known as multipotent mouse mesenchymal stem cells, were cultured in the presence of osteogenic differentiation medium (ODM) for 7 days, then treated with alendronates, and 2 days later the cells were used for testing. The cell proliferation was analyzed using an MTT assay. Alizarin red staining was done for mineralization. Alkaline phosphatase activity was measured using a commercial ELISA kit. Calcification was analyzed using energy dispersive X-ray spectrophometric analysis (EDX). Osteogenic gene expression was analyzed using RT-PCR. The change of CD 44 expression was observed using confocal microscopy and FACS analysis.

Results: The D1 cells, in the presence of ODM, differentiated into osteoblasts. The evidence of osteogenic differentiation was confirmed by the positive stain by Alizarin red S, the increased activity of ALP, the increased mRNA expression of osteocalcin, a calcium peak in the EDX analysis, and by the positive immunofluorescence staining against CD 44 (an antigen detected on osteoblasts). With the addition of alendronate in the ODM medium, the osteogenic differentiation was enhanced. The enhancing effect was confirmed by the increased intensity of the Alizarin red S staining, higher activity of ALP, the higher mRNA expression of osteocalcin, a higher calcium peak in the EDX analysis, and by the increased immunofluorescence staining against CD 44 in the FACS analysis.

Conclusion: We have demonstrated that alendronate enhances osteogenic differentiation when given to mouse mesenchymal stem cells in association with the ODM. Alendronate may increase the bone density not only by inhibiting the osteoclastic activity but also by increasing the osteogenic differentiation of the mesenchymal stem cells.
