Traditional radiographic criteria might underestimate or fail to detect subtle types of acetabular dysplasia. Acetabular sector angles (ASA) can measure the degree of anterior and posterior coverage of the femoral head on computed tomography (CT). This study aims to determine ASA values at different axial levels in a cohort of (1) asymptomatic, high-functioning hips without underlying hip pathology (controls); and (2) symptomatic, dysplastic hips that underwent periacetabular osteotomy (PAO). Thereby, we aimed to define CT-based thresholds for hip dysplasia and its subtypes.

This is an IRB approved cross-sectional study of 51 high functioning, asymptomatic patients (102 hips) (Oxford Hip Score >43), without signs of osteoarthritis (Tönnis grade≤1), who underwent a CT scan of the pelvis (mean age: 52.1±5.5 years; 52.9% females); and 66 patients (72 hips) with symptomatic hip dysplasia treated with peri-acetabular osteotomy (PAO) (mean age: 29.3±7.3 years; 85.9% females). Anterior and posterior acetabular sector angles (AASA & PASA) were measured by two observers at three CT axial levels to determine equatorial, intermediate, and proximal ASA. Inter- and intra-observer reliability coefficient was high (between 0.882–0.992). Cut-off values for acetabular deficiency were determined based on Receiver Operating Characteristic (ROC) curve analysis, area under the curve (AUC) was calculated.

The dysplastic group had significantly smaller ASAs compared to the Control Group, AUC was the highest at the proximal and intermediate PASA. Controls had a mean proximal PASA of 162°±17°, with a cut-off value for dysplasia of 137° (AUC: 0.908). At the intermediate level, the mean PASA of controls was 117°±11°, with a cut-off value of 107° (AUC 0.904). Cut-off for anterior dysplasia was 133° for proximal AASA (AUC 0.859) and 57° for equatorial AASA (AUC 0.868). Cut-off for posterior dysplasia was 102° for intermediate PASA (AUC 0.933).

Measurement of ASA on CT is a reliable tool to identify dysplastic hips with high diagnostic accuracy. Posterior ASA less than 137° at the proximal level, and 107° at the intermediate level should alert clinicians of the presence of dysplasia.

Femoroacetabular impingement (FAI) results from a morphological deformity of the hip and is associated with osteoarthritis (OA). Increased bone mineral density (BMD) is observed in the antero-superior acetabulum rim where impingement occurs. It is hypothesized that the repeated abnormal contact leads to damage of the cartilage layer, but could also cause a bone remodelling response according to Wolff's Law. Thus the goal of this study was to assess the relationship between bone metabolic activity measured by PET and BMD measured in CT scans.

Five participants with asymptomatic cam deformity, three patients with uni-lateral symptomatic cam FAI and three healthy controls were scanned in a 3T PET-MRI scanner following injection with [18F]NaF. Bone remodelling activity was quantified with Standard Uptake Values (SUVs). SUVmax was analyzed in the antero-superior acetabular rim, femoral head and head-neck junction. In these same regions, BMD was calculated from CT scans using the calibration phantom included in the scan. The relationship between SUVmax and BMD from corresponding regions was assessed using the coefficient of determination (R2) from linear regression.

High bone activity was seen in the cam deformity and acetabular rim. SUVmax was negatively correlated with BMD in the antero-superior region of the acetabulum (R2=0.30, p=0.08). SUVmax was positively correlated with BMD in the antero-superior head-neck junction of the femur (R2=0.359, p=0.067). Correlations were weak in other regions.

Elevated bone turnover was seen in patients with a cam deformity but the relationship to BMD was moderate. This study demonstrates a pathomechanism of hip degeneration associated with FAI deformities, consistent with Wolff's law and the proposed mechanical cause of hip degeneration in FAI. [18F]-NaF PET SUV may be a biomarker of degeneration, especially in early stages of degeneration, when joint preservation surgery is likely to be the most successful.

It has been reported that 60-85% of patients who undergo PAO have concomitant intraarticular pathology that cannot be addressed with PAO alone. Currently, there are limited diagnostic tools to determine which patients would benefit from hip arthroscopy at the time of PAO to address intra-articular pathology. This study aims to see if preoperative PROMs scores measured by IHOT-33 scores have predictive value in whether intra-articular pathology is addressed during PAO + scope. The secondary aim is to see how often surgeons at high-volume hip preservation centers address intra-articular pathology if a scope is performed during the same anesthesia event.

A randomized, prospective Multicenter trial was performed on patients who underwent PAO and hip arthroscopy to treat hip dysplasia from 2019 to 2020. Preoperative PROMs and intraoperative findings and procedures were recorded and analyzed. A total of 75 patients, 84% Female, and 16% male, with an average age of 27 years old, were included in the study. Patients were randomized to have PAO alone 34 patients vs. PAO + arthroscopy 41 patients during the same anesthesia event. The procedures performed, including types of labral procedures and chondroplasty procedures, were recorded. Additionally, a two-sided student T-test was used to evaluate the difference in means of preoperative IHOT score among patients for whom a labral procedure was performed versus no labral procedure.

A total of 82% of patients had an intra-articular procedure performed at the time of hip arthroscopy. 68% of patients who had PAO + arthroscopy had a labral procedure performed. The most common labral procedure was a labral refixation which was performed in 78% of patients who had a labral procedure performed. Femoral head-neck junction chondroplasty was performed in 51% of patients who had an intra-articular procedure performed. The mean IHOT score was 29.3 in patients who had a labral procedure performed and 33.63 in those who did not have a labral procedure performed P- value=0.24.

Our findings demonstrate preoperative IHOT-33 scores were not predictive in determining whether intra-articular labral pathology was addressed at the time of surgery. Additionally, we found that if labral pathology was addressed, labral refixation was the most common repair performed. This study also provides valuable information on what procedures high-volume hip preservation centers are performing when performing PAO + arthroscopy.

Developmental dysplasia of the hip (DDH) is a common risk factor of early osteoarthritis (OA), with insufficient coverage of the femoral head by the acetabulum which leads to excessive cartilage stresses in the hip joint. Knowledge of the molecular health of cartilage using MRI may diagnose and stage chondral disease, but more importantly allows for treatment stratification and prognostication. Delayed gadolinium-enhanced magnetic resonance imaging of cartilage (dGEMRIC) is a validated MRI technique for detecting early loss of proteoglycan (PG). However, it requires an injection of contrast agent and exercise prior to the scan. MRI techniques such as T1ρ and T2 mapping have also been shown to be sensitive to early biochemical changes in cartilage but can be performed without any contrast injection. In this study we evaluate three quantitative MR techniques (dGEMRIC, T1ρ and T2 mapping) in patients with DDH. Our hypothesis is that both T1ρ and T2 correlate with dGEMRIC, and thus may be effective non-contrast based techniques for biochemical cartilage mapping in DDH hips.

Seven informed and consented patients (mean age: 31.1 years) with DDH were enrolled in this IRB approved MRI study before surgery. DDH was defined as a lateral center-edge angle under 25º and acetabular index >13º on the plain x-ray. All subjects underwent two successive MRI sessions at 3T: In the first cartilage T1ρ and T2 mapping were performed. After leaving the scanner the subjects were injected with 0.4ml/kg Dotarem (i.v.), walked for 15min and rested for 25min before returning into the MRI. dGEMRIC (T1post) mapping was initiated approximately 45min after the injection. Image post-processing, registration and cartilage segmentation was performed with Matlab. The joint was subdivided into anterior and posterior regions in the sagittal plane and into lateral, intermediate and medial zones in the transverse plane, resulting in six region of interest (ROIs): antero-lateral, antero-intermediate, antero-medial, postero-lateral, postero-intermediate and postero-medial. The correlation between the dGEMRIC and T1ρ and dGEMRIC and T2 were evaluated using Spearman's Rho and tested for significance.

The analysis of all six cartilage ROIs for all subjects resulted in a significant (p < 0 .001) negative correlation (Rho = −0.50) between the dGEMRIC index (T1post) and the T1ρ relaxation time. The dGEMRIC index and T2 correlated positive (Rho = 0.55) and significant (p < 0 .001).

Although this pilot study has a small sample size a negative correlation between dGEMRIC and T1ρ was found in patients with DDH. Both methods are known to probe the PG content of cartilage, where a decreased PG content leads to lower dGEMRIC index and an increased T1ρ value. The correlation coefficient was moderate, but significant, which shows that T1ρ mapping as an effective tool to probe the cartilage PG content similar to dGEMRIC. A comparable, but positive correlation was found between dGEMRIC and T2. T2 is sensitive to the cartilage collagen content with a decreased T2 value in degenerated cartilage. In symptomatic DDH, where an onset of OA is assumed, both PG depletion and collagen decay are in progress and can be evaluated using these mapping techniques.

Introduction

Acetabular and spino-pelvic (SP) morphological parameters are important determinants of hip joint dynamics. This study aims to determine whether acetabular and SP morphological differences exist between hips with and without cam morphology and between symptomatic and asymptomatic hips with cam morphology.

Patients/Materials & Methods

A prospective cohort of 67 patients/hips was studied. Hips were either asymptomatic with no cam (Controls, n=18), symptomatic with cam (n=26) or asymptomatic with cam (n=23). CT-based quantitative assessments of femoral, acetabular, pelvic and spino-pelvic parameters were performed. Measurements were compared between controls and those with a cam deformity, as well as between the 3 groups. Morphological parameters that were independent predictors of a symptomatic Cam were determined using a regression analysis.

Introduction

This study aims to determine how the acetabular version changes during the key developmental stage of adolescence, and what contributes to this change. In addition, we examined whether patient factors (BMI, activity levels) or the femoral-sided anatomy contribute to any observed changes.

Patients/Materials & Methods

This prospective longitudinal cohort study included 19 volunteers (38 healthy hips). The participants underwent clinical examination (BMI, range of movement assessment), MRIs of both hips at recruitment and at follow-up (6 ± 2 years) and HSS Paediatric Functional Activity Brief Scale (Pedi-FABS) questionnaire. MRI scans were assessed at both time points to determine change of the tri-radiate cartilage complex (TCC), the acetabular anteversion, the degree of anterior, posterior, and superior femoral head coverage by the acetabulum, and anterior and antero-superior alpha angles. We investigated if the change in anteversion and sector angles was influenced by the BMI, range of movement measurements, the Pedi-FABS or the alpha angle measurements.

Cam-type femoral acetabular impingement (FAI), is a common structural hip deformity and thought to be a leading cause of early hip osteoarthritis. Although patients who undergo surgical correction notice improved clinical function it is unclear what impact this has on the overall health of the cartilage. T1rho MRI cartilage mapping has been shown to be a reliable imaging technique to assess the proteoglycan (PG) content potentially serving as a biomarker. This study analyses post surgical changes in T1rho levels in hip joints treated with cam FAI.

Eleven patients with a mean age of 38 (all males) underwent pre and post T1Rho Cartilage mapping of their hips at a mean time of 20 months post surgical intervention. The acetabulum was spatially divided into 4 main regions of interest (ROI), with levels of T1Rho in cartilage quantified as a whole and in each spatial segment. T1Rho signal is inversely correlated with level of PG content.

All patients demonstrated loss of PG content on pre-op imaging with a T1Rho of 33.5ms+2.6ms. Preop T1rho levels were found to significantly correlated with the difference between pre-op and post-op T1rho in entire hip cartilage (R: 0.73; p=0.016). This correlation was reflected both in the anterolateral quadrant (R: 0.86; p=0.002), and in the posteriosuperior quadrant (R:0.70; p=0.035). Additionally, significant correlation was found between improvement of WOMAC pain score over time, and difference of T1rho values over time in the most lateral 3mm slice of the anterolateral quadrant (R: 0.81; p=0.045). Significant correlation was found between pre-op alpha angle at 1:30 and difference between pre-op and post-op total cartilage T1rho content (R: −065;p=0.038).

T1Rho Cartilage mapping of the hip is a useful biomarker in the assessment of the surgical management of Cam type FAI. This preliminary data provides some evidence that surgical correction of the deformity can help minimise disease progression.

Objectives

Hips with metal-on-metal total hip arthroplasty (MoM THA) have a high rate of adverse local tissue reactions (ALTR), often associated with hypersensitivity reactions. Dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) measures tissue perfusion with the parameter Ktrans (volume transfer constant of contrast agent). Our purpose was 1) to evaluate the feasibility of DCE-MRI in patients with THA and 2) to compare DCE-MRI in patients with MoM bearings with metal-on-polyethylene (MoP) bearings, hypothesising that the perfusion index Ktrans in hips with MoM THA is higher than in hips with MoP THA.

Methods

In this pilot study, 16 patients with primary THA were recruited (eight MoM, eight MoP). DCE-MRI of the hip was performed at 1.5 Tesla (T). For each patient, Ktrans was computed voxel-by-voxel in all tissue lateral to the bladder. The mean Ktrans for all voxels was then calculated. These values were compared with respect to implant type and gender, and further correlated with clinical parameters.

Purpose

The importance of femoral head-neck morphology in the development of early hip osteoarthritis is recognized in femeroacetabular impingement (FAI), however no studies have examined FAI morphology in the developing hip, i.e. pre-closure of the proximal femoral physis. We developed a pilot project to study prevalence of CAM-type FAI hip morphology in both the pre- and post-closure proximal femoral physes of asymptomatic children using MR-imaging. We also examined biologic markers possibly related to FAI etiology, including Vitamin D metabolites, BMI, family history, and activity levels.

Method

Recruitment included volunteers with asymptomatic lower extremities, and either pre- or post-closure of the proximal femoral physis. Males were 10–12 years (pre-closure) or 15–18 years (post-closure); females were 8–10 years or 14 18 years. Phlebotomy and urine sampling were used to assess metabolic markers. MRI of bilateral hips and a clinical exam including hip impingement tests were conducted. MR imaging assessment was independent and blinded and recorded using established parameters including alpha angles measured at both the 3:00 (anterior head-neck junction) and 1:30 (antero-superior head-neck junction) radial image positions.

Controversy exists regarding the optimal technique for arthroscopic rotator cuff repair. The purpose of this multicentre randomized double-blind controlled study was to compare the functional outcomes and healing rates of double-row suture techniques with single row repair.

Method

Patients undergoing arthroscopic rotator cuff repair were randomized to receive either a double row (DR) or single row (SR) repair. The primary objective was to compare the WORC score at 24 months. Secondary objectives included anatomical outcomes by MRI or ultrasound, the Constant, and ASES scores. A sample size calculation determined that 84 patients provided 80% power with a 50% effect size to detect a statistical difference between groups.

Controversy exists regarding the optimal technique of subscapularis mobilization during shoulder arthroplasty. The purpose of this multicentre randomized double-blind study was to compare the functional outcomes and healing rates of the lesser tuberosity osteotomy (LTO) to the subscapularis tenotomy (ST).

Method

Patients undergoing shoulder arthroplasty were randomized to receive either a LTO or ST. The primary outcome was to compare subscapularis strength, as measured by an electronic hand-held dynamometer at 12 months. Secondary outcomes included range of motion, WOOS, Constant and ASES scores. A sample size calculation determined that 80 patients provided 80% power with a 50% effect size to detect a significant difference between groups.

Purpose: A growing body of literature confirms that idiopathic OA is frequently caused by subtle, and often radiographically occult, abnormalities at the femoral head-neck junction or acetabulum that result in abnormal contact between the femur and acetabulum. This condition, known as femoroacetabular impingement, is a widely accepted cause of early OA of the hip. MRI is the imaging modality that is most sensitive in detecting cam morphology. There is currently little published data regarding the prevalence of abnormalities of the femoral head-neck junction in patients without hip pain or previous hip pathology. The primary aim of this project is to examine the incidence of cam morphology in a population without hip pain or pre-existing hip disease using non-contrast MRI.

Method: Two hundred asymptomatic volunteers underwent magnetic resonance imaging targeted to both hips. Subjects were examined at the time of MRI to document internal rotation of the hips at 90 degrees flexion and to assess for a positive impingement sign. The mean age was 29.4 years (range 21.4–50.6); 77.5% were Caucasian and 55.5% female. The Nötzli alpha angle was measured on oblique axial images through the middle of the femoral neck for each hip. A value greater than 50 degrees was considered consistent with cam morphology. Measurements were performed independently by two musculoskeletal radiologists.

Results: Twenty-six percent of volunteers had at least one hip with cam morphology: 20% had an elevated alpha angle on either the right or the left side, and 6% had bilateral deformity. The average alpha angle was 42.6 degrees on the right (SD=7.9) and 42.4 degrees on the left (SD=7.7). Internal rotation was negatively correlated with alpha angle (p< .05). Patients with an elevated alpha angle on at least one side tended to be male (p< .01).

Conclusion: The high prevalence of cam morphology in asymptomatic individuals is critical information in determining the natural history of FAI as well as establishing treatment strategies in patients presenting with pre-arthritic hip pain.

The traditionally accepted etiology of Scapholunate Advanced Collapse (SLAC) requires traumatic rupture of the scapholunate (SL) ligament which leads to abnormal wrist kinematics and thereafter severe localised degenerative arthritis of the wrist. The purpose of this prospective blinded kinematic analysis was to demonstrate that SLAC wrist also exists in the absence of trauma, and that abnormal carpal bone kinematics (specifically, decreased lunate flexion) is the initiating factor.

Patients with SLAC and no history of upper extremity trauma were compared with an age matched control group. All patients completed a questionnaire, personal interview, and a physical examination. A specialised flexion / extension radiographic jig was designed to control for the magnitude of force and position of the wrist in all planes.

A total of thirty-five subjects (sixty-nine wrists) were retained for the study, including thirty-three non-traumatic SLAC wrists and thirty-six control wrists. The non-traumatic SLAC group had significantly different radiographic kinematic analysis compared to the control group: increased Watson Stage (2 v 0), SL gap (3.4 v 1.8mm), revised carpal height ratio (rCHR) (77 v 68), SL angle in flexion (forty-one v twenty-eight degrees), and decreased radiolunate (RL) joint flexion (nine v twenty-seven degrees). Most importantly flexion of the asymptomatic non-degenerative wrist of the non-traumatic SLAC group was distributed 70% through the lunocapitate (LC) joint and only 30% through the RL joint (p< 0.05). Conversely, flexion was more evenly distributed in the control group (48% LC and 52% RL). Non-traumatic or developmental SLAC does exist. SLAC can thus be classified into non-traumatic (developmental) and traumatic types.

Non-traumatic SLAC begins with abnormal wrist kinematics. Over time restricted lunate flexion and normal scaphoid flexion leads to increased SL angles and eventual attrition of the SL ligament and predisposes patients to SLAC despite having no history of trauma.
