The use of cementless total knee arthroplasty (TKA) components has increased during the past decade. The initial design of cementless metal-backed patellar components had shown high failure rates due to many factors. The aim of this study was to evaluate the clinical results of a second-generation cementless, metal-backed patellar component of a modern design. This was a retrospective review of 707 primary TKAs in 590 patients from a single institution, using a cementless, metal-backed patellar component with a mean follow-up of 6.9 years (2 to 12). A total of 409 TKAs were performed in 338 females and 298 TKAs in 252 males. The mean age of the patients was 63 years (34 to 87) and their mean BMI was 34.3 kg/m2 (18.8 to 64.5). The patients were chosen to undergo a cementless procedure based on age and preoperative radiological and intraoperative bone quality. The purpose of this study was to compare the clinical outcomes, mortalities, implant survival rates, and complications of total knee arthroplasty (TKA) in patients with or without hepatitis B virus (HBV) infection over at least ten years of follow-up. From January 2008 to December 2010, 266 TKAs were performed in 169 patients with HBV (HBV group). A total of 169 propensity score–matched patients without HBV were chosen for the control group in a one-to-one ratio. Then, the clinical outcomes, mortalities, implant survival rates, and complications of TKA in the two groups were compared. All primary TKRs for osteoarthritis alone performed between 1 April 2003 and 31 December 2016 were eligible (n = 842,072). Patellar resurfacing during TKR was performed in 36% (n = 305,844). The primary outcome was all-cause revision surgery. Secondary outcomes were the number of excess all-cause revisions associated with using TKRs without (versus with) patellar resurfacing, and the risk of re-revision after secondary patellar resurfacing. The aim of this study was to investigate whether a preoperative Risk Stratification Tool (RST) devised in our institution could reduce unexpected intensive care unit (ICU) transfers and 90-day emergency department (ED) visits, readmissions, and reoperations after TKA in obese patients. Methods. We retrospectively reviewed 1,614 consecutive patients undergoing primary unilateral TKA. Their mean age was 65.1 years (17.9 to 87.7) and the mean BMI was 34.2 kg/m. 2. (SD 7.7). All patients underwent perioperative optimization and monitoring using the RST, which is a validated calculation tool that provides a recommendation for postoperative ICU care or increased nursing support. Patients were divided into three groups: non-obese (BMI < 30 kg/m. 2. , n = 512); obese (BMI 30 kg/m. 2. to 39.9 kg/m. 2. , n = 748); and morbidly obese (BMI > 40 kg/m. 2. , n = 354). Logistic regression analysis was used to evaluate the outcomes among the groups adjusted for age, sex, smoking, and diabetes. Results. Obese patients had a significantly increased rate of discharge to a rehabilitation facility compared with non-obese patients (38.7% (426/1,102) vs 26.0% (133/512), respectively; p < 0.001). When stratified by BMI, discharge to a rehabilitation facility remained significantly higher compared with non-obese (26.0% (133)) in both obese (34.2% (256), odds ratio (OR) 1.6) and morbidly obese (48.0% (170), OR 3.1) patients (p < 0.001). Background. There are limited data on the complication rates and risk of periprosthetic joint infection (PJI) in patients who have an acute wound dehiscence after total knee arthroplasty (TKA). Methods. From 2002 to 2018, 16,134 primary TKAs were performed at a single institution. Twenty-six patients (0.1%) had a traumatic wound dehiscence within the first 30 days. Mean age was 68, 38% were female, and mean BMI was 33 kg/m2. Median time to dehiscence was 13 days. The dehiscence resulted from a fall in 22 cases, including 4 in-hospital falls (3 with femoral nerve blocks), and sudden flexion after staple removal in 4 cases. The arthrotomy was disrupted in 58%, including a complete extensor mechanism disruption in 4 knees. A surgical debridement was performed within 48 hours in 19 of 26 knees. Two-thirds were discharged on antibiotic therapy. Results. Only two knees were complicated by PJI (cumulative incidence of 11% vs. 1% in all other primary TKAs, HR 6.5, p <0.01). One patient who developed a PJI was initially treated with a bedside closure in the ER rather than surgical debridement. The second patient had a complete extensor mechanism disruption at the time of dehiscence and was treated with surgical debridement, but did not receive a subsequent course of antibiotics. There were no PJIs in any of the cases that were treated with surgical debridement and a course of antibiotics. Three knees required reoperation (cumulative incidence of reoperation was 16% at 2 years, compared to 6% in all other primary TKAs, p=0.32) including 1 two-stage exchange for PJI, 1 irrigation and debridement with component retention for PJI, and 1 revision for tibial component aseptic loosening. Conclusion. Despite having a traumatic wound dehiscence, with nearly 60% resulting in arthrotomies that exposed implants, the risk of PJI was low but significantly higher than not having a traumatic arthrotomy. We recommend urgent surgical irrigation and debridement, inspection of arthrotomy integrity, and antibiotic treatment to decrease the risk of PJI. Summary. Introduction. Allogeneic blood transfusion (ABT) remains a widely used therapeutic intervention in patients undergoing total knee arthroplasty (TKA). There is mounting evidence that tranexamic acid (TXA), a powerful antifibinolytic, can significantly reduce perioperative blood loss with a concomitant lower ABT rate. In May 2012, TXA intravenous infusion was introduced as standard therapy in all patients undergoing major hip and knee arthroplasty. The TXA protocol included infusing 1 gm prior to incision and 1 gm after lowering the tourniquet.
