Based on 3D CT animations, we propose repetitive impact between the main supplying vessels on the posterolateral femoral neck and the posterior acetabular wall in hyperextension and external rotation as a possible cause of direct vascular damage, and subsequent femoral head necrosis in three adolescent female gymnasts we are reporting on. In order to release the contracture band completely without damaging normal tissues (such as the sciatic nerve) in the surgical treatment of gluteal muscle contracture (GMC), we tried to display the relationship between normal tissue and contracture bands by magnetic resonance neurography (MRN) images, and to predesign a minimally invasive surgery based on the MRN images in advance. A total of 30 patients (60 hips) were included in this study. MRN scans of the pelvis were performed before surgery. The contracture band shape and external rotation angle (ERA) of the proximal femur were also analyzed. Then, the minimally invasive GMC releasing surgery was performed based on the images and measurements, and during the operation, incision lengths, surgery duration, intraoperative bleeding, and complications were recorded; the time of the first postoperative off-bed activity was also recorded. Furthermore, the patients’ clinical functions were evaluated by means of Hip Outcome Score (HOS) and Ye et al’s objective assessments, respectively.Aims

Total hip arthroplasty (THA) is a common procedure to address pain and enhance function in hip disorders such as osteoarthritis. Despite its success, postoperative patient recovery exhibits considerable heterogeneity. This study aimed to investigate whether patients follow distinct pain trajectories following THA and identify the patient characteristics linked to suboptimal trajectories. This retrospective cohort study analyzed THA patients at a large academic centre (NYU Langone Orthopedic Hospital, New York, USA) from January 2018 to January 2023, who completed the Patient-Reported Outcomes Measurement Information System (PROMIS) pain intensity questionnaires, collected preoperatively at one-, three-, six-, 12-, and 24-month follow-up times. Growth mixture modelling (GMM) was used to model the trajectories. Optimal model fit was determined by Bayesian information criterion (BIC), Vuong-Lo-Mendell-Rubin likelihood ratio test (VLMR-LRT), posterior probabilities, and entropy values. Methods
We compared the clinical outcomes of curved intertrochanteric varus osteotomy (CVO) with bone impaction grafting (BIG) with CVO alone for the treatment of osteonecrosis of the femoral head (ONFH). This retrospective comparative study included 81 patients with ONFH; 37 patients (40 hips) underwent CVO with BIG (BIG group) and 44 patients (47 hips) underwent CVO alone (CVO group). Patients in the BIG group were followed-up for a mean of 12.2 years (10.0 to 16.5). Patients in the CVO group were followed-up for a mean of 14.5 years (10.0 to 21.0). Assessment parameters included the Harris Hip Score (HHS), Oxford Hip Score (OHS), Japanese Orthopaedic Association Hip-Disease Evaluation Questionnaire (JHEQ), complication rates, and survival rates, with conversion to total hip arthroplasty (THA) and radiological failure as the endpoints.Aims

In this in vitro study of the
hip joint we examined which soft tissues act as primary and secondary
passive rotational restraints when the hip joint is functionally
loaded. A total of nine cadaveric left hips were mounted in a testing
rig that allowed the application of forces, torques and rotations
in all six degrees of freedom. The hip was rotated throughout a
complete range of movement (ROM) and the contributions of the iliofemoral
(medial and lateral arms), pubofemoral and ischiofemoral ligaments
and the ligamentum teres to rotational restraint was determined
by resecting a ligament and measuring the reduced torque required
to achieve the same angular position as before resection. The contribution
from the acetabular labrum was also measured. Each of the capsular
ligaments acted as the primary hip rotation restraint somewhere
within the complete ROM, and the ligamentum teres acted as a secondary
restraint in high flexion, adduction and external rotation. The
iliofemoral lateral arm and the ischiofemoral ligaments were primary
restraints in two-thirds of the positions tested. Appreciation of
the importance of these structures in preventing excessive hip rotation
and subsequent impingement/instability may be relevant for surgeons

To investigate psychosocial and biomedical outcomes following
total hip replacement (THR) and to identify predictors of recovery
from THR. Patients with osteoarthritis (OA) on the waiting list for primary
THR in North West England were assessed pre-operatively and at six
and 12 months post-operatively to investigate psychosocial and biomedical
outcomes. Psychosocial outcomes were anxiety and depression, social
support and health-related quality of life (HRQoL). Biomedical outcomes
were pain, physical function and stiffness. The primary outcome
was the Short-Form 36 (SF-36) Health Survey Total Physical Function.
Potential predictors of outcome were age, sex, body mass index,
previous joint replacement, involvement in the decision for THR,
any comorbidities, any complications, type of medication, and pre-operative
ENRICHD Social Support Instrument score, Hospital Anxiety and Depression
scores and Western Ontario and McMaster Universities osteoarthritis index
