Aims. Prophylactic antibiotic regimens for elective primary total hip and knee arthroplasty vary widely across hospitals and trusts in the UK. This study aimed to identify antibiotic prophylaxis regimens currently in use for elective primary arthroplasty across the UK, establish variations in antibiotic prophylaxis regimens and their impact on the risk of periprosthetic joint infection (PJI) in the first-year post-index procedure, and evaluate adherence to current international consensus guidance. Methods. The guidelines for the primary and alternative recommended prophylactic antibiotic regimens in clean orthopaedic surgery (primary arthroplasty) for 109 hospitals and trusts across the UK were sought by searching each trust and hospital’s website (intranet webpages), and by using the MicroGuide app. The mean cost of each antibiotic regimen was calculated using price data from the British National Formulary (BNF). Regimens were then compared to the 2018 Philadelphia Consensus Guidance, to evaluate adherence to international guidance. Results. The primary choice and dosing of the prophylactic antimicrobial regimens varied widely. The two most used regimens were combined teicoplanin and gentamicin, and cefuroxime followed by two or three doses of cefuroxime eight-hourly, recommended by 24 centres (22.02%) each. The alternative choice and dosing of the prophylactic antimicrobial regimen also varied widely across the 83 centres with data available. Prophylaxis regimens across some centres fail to cover the likeliest causes of surgical site infection (SSI). Five centres (4.59%) recommend co-amoxiclav, which confers no Staphylococcus coverage, while 33 centres (30.28%) recommend cefuroxime, which confers no Enterococcus coverage. Limited adherence to 2018 Philadelphia Consensus Guidance was observed, with 67 centres (61.50%) not including a cephalosporin in their guidance. Conclusion. This analysis of guidance on antimicrobial prophylaxis in primary arthroplasty across 109 hospitals and trusts in the UK has identified widespread variation in primary and alternative antimicrobial regimens currently recommended. Aims. When performing revision total hip arthroplasty using diaphyseal-engaging titanium tapered stems (TTS), the recommended 3 to 4 cm of stem-cortical diaphyseal contact may not be available. In challenging cases such as these with only 2 cm of contact, can sufficient axial stability be achieved and what is the benefit of a prophylactic cable? This study sought to determine, first, whether a prophylactic cable allows for sufficient axial stability when the contact length is 2 cm, and second, if differing TTS taper angles (2° vs 3.5°) impact these results. Methods. A biomechanical matched-pair cadaveric study was designed using six matched pairs of human fresh cadaveric femora prepared so that 2 cm of diaphyseal bone engaged with 2° (right femora) or 3.5° (left femora) TTS. Before impaction, three matched pairs received a single 100 lb-tensioned prophylactic beaded cable; the remaining three matched pairs received no cable adjuncts. Specimens underwent stepwise axial loading to 2600 N or until failure, defined as stem subsidence > 5 mm. Results. All specimens without cable adjuncts (6/6 femora) failed during axial testing, while all specimens with a prophylactic cable (6/6) successfully resisted axial load, regardless of taper angle. In total, four of the failed specimens experienced proximal longitudinal fractures, three of which occurred with the higher 3.5° TTS. One fracture occurred in a 3.5° TTS with a prophylactic cable yet passed axial testing, subsiding < 5 mm. Among specimens with a prophylactic cable, the 3.5° TTS resulted in lower mean subsidence (0.5 mm (SD 0.8)) compared with the 2° TTS (2.4 mm (SD 1.8)). Conclusion. A single prophylactic beaded cable dramatically improved initial axial stability when stem-cortex contact length was 2 cm. All implants failed secondary to fracture or subsidence > 5 mm when a prophylactic cable was not used. A higher taper angle appears to decrease the magnitude of subsidence but increased the fracture risk. The fracture risk was mitigated by the use of a prophylactic cable. Our study sought to establish the necessity of prolonged pre-operative antibiotic prophylaxis in patients presenting with zone II and zone V acute flexor tendon injuries (FTI). We hypothesized that a single dose of prophylactic antibiotic was adequate in prevention of post-operative wound infection in acute zone II and V FTI. This was a prospective study of 116 patients who presented with zone II and zone V acute FTI. The study included patients who were 18 years and older. Those with macroscopic contamination, immunocompromised, open fractures, bite injuries, and crush injuries were excluded. Patients were randomised into a group receiving a single dose of prophylactic antibiotic and another group receiving a continuous 8 hourly antibiotic doses until the day of surgery. Each group was subdivided into occupational and non-occupational injuries. Their post-operative wound outcomes were documented 10 – 14 days after surgery. The wound outcome was reported as no infection, superficial infection (treated with wound dressings), and deep infection (requiring surgical debridement). There was 0.9% rate of deep post-operative wound infections, which was a single zone V acute FTI case in a single dose prophylactic antibiotic group. There was a 7.8% superficial post-operative wound infection rate, which was mainly zone II acute FTI in both antibiotic groups. There was a strong association between zone II acute FTI and post-operative wound infection (p < 0.05). There was no association between (antibiotic dosage or place of injury) with post-operative wound infection (p > 0.05). There is no benefit in prescribing prolonged pre-operative antibiotic in patients with acute, simple lacerations to zone II and zone V FTI if there is no macroscopic wound contamination

Prophylactic pinning of the contralateral hip in unilateral Slipped Upper Femoral Epiphysis (SUFE) persists as a source of debate with the majority of surgeons selecting this option in a proportion of patients whom they regard as at increased risk of a subsequent slip. Universal prophylactic pinning was introduced in our region in 2005 after an audit of ten years local practice identified 25% of unilateral cases presented with a subsequent slip. This study reports our experience between 2005 and 2020. In this prospective study, 44 patients presented with 55 affected hips compared with 60 patients with 67 affected hips in the original study. Two patients were excluded as their initial slip had not been treated in our unit. Of the 42 hips seven were bilateral, 34 of the 35 unilateral hips underwent prophylactic pinning. The one exception subsequently underwent prophylactic pinning due to developing pain. Consistent with our original series, at a minimum follow up of 13 months there have been no complications of infection, fracture, chondrolysis or avascular necrosis subsequent to prophylactic pinning. Over 25 years 70 patients have undergone prophylactic pinning without complication. On the premise that 25% of our unpinned hips presented with subsequent slips before instituting our policy we estimate that we have prevented 17 subsequent slips over 25 years including the consequences which can be significant. We continue to advocate universal prophylactic pinning as an effective and safe practice in the management of SUFE

C. Difficile infections in elderly patients with hip fractures is associated with high morbidity and mortality. Antibiotic regimens with penicillin and its derivatives is a leading cause. Antibiotic prophylactic preferences vary across different hospitals within NHS. We compared two antibiotic prophylactic regimens - Cefuroxime only prophylaxis and Teicoplanin with Gentamicin prophylaxis in fracture neck of femur surgery, and evaluated the incidence of C. Difficile diarrhea and Surgical Site Infection (SSI). To assess the Surgical Site Infection and C. Difficile infection rate associated with different regimens of antibiotics prophylaxis in fracture neck of femur surgery. Data was analyzed retrospectively. Neck of femur fracture patients treated surgically from 2009 in our unit were included. Age, gender, co morbidities, type of fracture, operation, ASA grade was collected. 1242 patients received Cefuroxime only prophylaxis between January 2009 and December 2012 (Group 1) and 486 patients received Teicoplanin with Gentamicin between October 2015 and March 2017 (Group 2). There were 353 males and 889 female patients in Group 1 and 138 males and 348 female patients in Group 2. The co morbidities in both groups were comparable. Incidence of C. Difficile diarrhea and Surgical Site Infection (SSI) was noted. Statistical analysis with chi square test was performed to determine the ‘p’ value. C. Diff diarrhea rate in Group 2 was 0.41 % as compared to 1.29 % in Group 1. The Surgical Site Infection (SSI) rate in Group 2 was 0.41 % as compared to 3.06 % in Group 1. The comparative results were statistically significant (p = 0.0009). Prophylactic antibiotic regimen of Teicoplanin with Gentamicin showed significant reduction in C. Difficile diarrhea & Surgical Site Infection in fracture neck of femur patients undergoing surgery

The femur is a common site for skeletal bony metastases. The aim of this study is to evaluate the outcomes of femoral intramedullary nailing in prophylactic versus therapeutic treatment in femoral metastases. All femoral nails between April 2011 and November 2015 at a district general hospital were assessed. Intramedullary nailing performed for prophylactic or therapeutic management were included. Outcomes include mortality, survival time and length of stay in hospital. A total of 40 cases were included. In the prophylactic group there were 25 patients and in the therapeutic group there were 15 patients. In the prophylactic group, mean age was 70 years (range 41–91); male to female ratio is 23:17 and 26 patients of this group was deceased. In the therapeutic group, mean age was 76 years (range 56–92); male to female ratio 15:10 and 10 patients were deceased in this group. The most common primary was prostate carcinoma followed by breast carcinoma. In the prophylactic group, mean survival was 25 weeks (range 2–147) and in the therapeutic group mean survival was 20 weeks (range 2–39). The length of stay was 21 days (range 3–80) in the prophylactic group and 28 days (range 7–63) in the therapeutic group. Femoral nailing for metastases helps improve quality of life and we observed a mean survival time of 20–25 weeks postoperatively in both therapeutic and prophylactic nailing

Mirels’ score predicts the likelihood of sustaining pathological fractures using pain, lesion site, size and morphology. The aim is to investigate its reproducibility, reliability and accuracy in upper limb bony metastases and validate its use in pathological fracture prediction. A retrospective cohort study of patients with upper limb metastases, referred to an Orthopaedic Trauma Centre (2013–18). Mirels’ was calculated in 32 patients; plain radiographs at presentation scored by 6 raters. Radiological aspects were scored twice by each rater, 2-weeks apart. Inter- and intra-observer reliability were calculated (Fleiss’ kappa test). Bland-Altman plots compared variances of individual score components &total Mirels’ score. Mirels’ score of ≥9 did not accurately predict lesions that would fracture (11% 5/46 vs 65.2% Mirels’ score ≤8, p<0.0001). Sensitivity was 14.3% &specificity was 72.7%. When Mirels’ cut-off was lowered to ≥7, patients were more likely to fracture (48% 22/46 versus 28% 13/46, p=0.045). Sensitivity rose to 62.9%, specificity fell to 54.6%. Kappa values for interobserver variability were 0.358 (fair, 0.288–0.429) for lesion size, 0.107 (poor, 0.02–0.193) for radiological appearance and 0.274 (fair, 0.229–0.318) for total Mirels’ score. Values for intraobserver variability were 0.716 (good, 95% CI 0.432–0.999) for lesion size, 0.427 (moderate, 95% CI 0.195–0.768) for radiological appearance and 0.580 (moderate, 0.395–0.765) for total Mirels’ score. We showed moderate to substantial agreement between &within raters using Mirels’ score on upper limb radiographs. Mirels’ has poor sensitivity &specificity predicting upper limb fractures - we recommend the cut-off score for prophylactic surgery should be lower than for lower limb lesions

From 1992 to 1999, 713 total joint arthroplasties were performed in The Department of Orthopaedic Surgery, The University of Hong Kong. Since January 1993, a uniform prophylactic antibiotic regime was employed: one dose of first generation cephalosporin (one gram cephazonlin) on induction and every 4-hourly. In case of sequential bilateral total knee arthroplasty, one gram of cephazolin will be given on induction for the first knee and one hour before the operation on the opposite knee. Antibiotic will be discontinued post-operatively. No significant difference was identified between the infection rate before (1.4%) and after (1.2%) the adoption of the prophylactic antibiotic guidelines (p > 0.4). The study had shown that one dose of first generation cephalosporin is as effective as multiple dose of prophylactic antibiotic, either first or second generation cephalosporin, in preventing infection in total joint arthroplasty

Prophylactic fixation of the contralateral hip in cases of unilateral slipped capital femoral epiphysis (SCFE) remains contentious. Our senior author reported a 10 year series in 2006 that identified a rate of subsequent contralateral slip of 25percnt; when prophylactic fixation was not performed. This led to a change in local practice and employment of prophylactic fixation as standard. We report the 10 year outcomes following this change in practice. A prospective study of all patients who presented with diagnosis of SCFE between 2004 and 2014 in our region. Intra-operative complication and post-operative complication were the primary outcomes. 31 patients presented during the study period: 16 male patients and 15 female patients. The mean age was 12.16 (8–16, SD 2.07). 25 patients had stable SCFE and 5 had unstable SCFE. Stability was uncertain in 1 patient. 25 patients had unilateral SCFE and 6 had bilateral SCFE. 24 patients who had unilateral SUFE had contralateral pinning performed. 1 unilateral SCFE did not have contralateral pinning performed as there was partial fusion of physis on contralateral side. In the hips fixed prophylactically there was 1 cases of transient intraoperative screw penetration into the joint and 1 case of minor wound dehiscence. There were no cases or chondrolysis or AVN. There were no further contralateral slips. This change in practice has been adopted with minimal complication. The fixation of the contralateral side is not without risk but by adopting this model the risk of subsequent slip has been reduced from 25percnt; to 0percnt;

Introduction: The role of prophylactic antibiotics in reducing the incidence of infection following hip and knee arthroplasty is well established. The British Orthopaedic Association (BOA) published best practice guidance on the use of prophylactic antibiotics in hip and knee arthroplasty. The guidance stated that all patients should receive prophylactic antibiotics at induction of anaesthesia and that each unit should have a locally agreed policy with advice from microbiologist. The aim of this audit was to compare the practice in our unit with the BOA guidance and implement necessary changes. Patients and Methods: A prospective audit was conducted over a one month period in 2007 and included all patients undergoing elective primary hip and knee replacements. A similar re-audit was conducted over one month period in 2008 after the initial audit recommendations were implemented. Results: Forty patients (40) were included in the initial audit. All patients received prophylactic antibiotics at induction but the choice, dose and duration of administration of antibiotics varied widely among surgeons in the unit. After discussion with the local microbiologist, we recommended a departmental policy for prophylactic antibiotics. The policy recommended a single dose of Cefuroxime and Gentamycin for standard cases and a single dose of Teicoplanin and Gentamycin for high-MRSA risk cases. A re-audit was conducted after the new policy was agreed. The re-audit included 33 patients. All patients received prophylactic antibiotics at induction. The choice of antibiotics was concordant with the policy in 79% of cases and duration of administration was appropriate in 85% of cases. Overall, the policy was adhered to in 22 cases (67 %). Discussion & Conclusions: The closed audit cycle resulted in improvement of our practice but the compliance rate with the new policy was lower than expected. Although it is the primary responsibility of the operating surgeon to ensure the appropriate prophylactic antibiotics are administered, more awareness of other team members is necessary to improve the compliance rate with the new policy

Objectives. We wanted to investigate regional variations in the organisms reported to be causing peri-prosthetic infections and to report on prophylaxis regimens currently in use across England. Methods. Analysis of data routinely collected by Public Health England’s (PHE) national surgical site infection database on elective primary hip and knee arthroplasty procedures between April 2010 and March 2013 to investigate regional variations in causative organisms. A separate national survey of 145 hospital Trusts (groups of hospitals under local management) in England routinely performing primary hip and/or knee arthroplasty was carried out by standard email questionnaire. Results. Analysis of 189 858 elective primary hip and knee arthroplasty procedures and 1116 surgical site infections found statistically significant variations for some causative organism between regions. There was a 100% response rate to the prophylaxis questionnaire that showed substantial variation between individual trust guidelines. A number of regimens currently in use are inconsistent with the best available evidence. Conclusions. The approach towards antibiotic prophylaxis in elective arthroplasty nationwide reveals substantial variation without clear justification. Only seven causative organisms are responsible for 89% of infections affecting primary hip and knee arthroplasty, which cannot justify such widespread variation between prophylactic antibiotic policies. This purpose of this study was to investigate whether prophylactic pinning of the contralateral hip in unilateral slipped upper femoral epiphysis affects subsequent femoral morphology. To determine the effect of prophylactic pinning on growth we compared contralateral hip radiographs of 24 proximal femora prophylactically pinned with 26 cases observed, in a cohort of patients with unilateral SUFE. Validated measurements were used to determine hip morphology; the articulo-trochanteric distance (ATD) and the ratio of the trochanteric-trochanteric distance (TTD) to articulo-trochanteric distance (TTD:ATD) in addition to direct measurement of the femoral neck length. Post-operative radiographs were compared to radiographs taken at a 12–84 months follow-up. Comparing pinned and unpinned hips the neck length was shorter (mean 5.1 mm vs 11.1 mm) and the ATD was lower (p=0.048). The difference between initial and final radiograph TTD:ATD ratio for each case was calculated. The average was 0.63 in the prophylactically pinned group and 0.25 in the unpinned group (p=0.07). When hips of the same patient were compared on final radiographs, there was a smaller difference in TTD:ATD between the two sides when the patient had been prophylactically pinned (0.7) as opposed to observed (1.47). This was not statistically significant (p=0.14). Universal prophylactic pinning of the contralateral hip in slipped upper femoral epiphysis is controversial and alteration of the proximal femoral morphology is one reason for this. Our results show that prophylactic pinning does not stop growth but does alter subsequent proximal femoral morphology by causing a degree of coxa vara and breva. Some loss of growth in the prophylactically pinned hip contributes to reduction in leg length inequality at skeletal maturity which is advantageous. No iatrogenic complications were observed with single cannulated screw fixation. Prophylactic pinning prevents the potential catastrophe of a subsequent slip, is safe and the effect on growth is, if anything, beneficial. Level of evidence: III

Despite increasing use of vena cava filters (VCFs) for pulmonary embolism (PE) prophylaxis after major trauma, there is continued debate regarding their safety and efficacy. We aimed to evaluate the impact of prophylactic VCFs on the incidence of PE after major trauma and to describe VCF related complications. Prospectively collected data from The Alfred Hospital Trauma Registry were used to identify all major trauma patients admitted between 1 July 2001 and 1 July 2008. Data for each patient was collated from the registry. This included patient demographics, injury specific data, management details (including prophylactic VCF use) and clinical outcomes (including the occurrence of PE). Medical record and radiology chart review was used to verify all PEs. Potential PE risk factors were assessed as covariates in a univariate analysis, with PE as the dependent variable. A multivariate analysis was then performed using multiple logistic regression adjusting for baseline imbalances and known covariates. During this period, 6,344 major trauma patients were treated, with 73.2% male, mean age of 44.2 +/− 21.0 SD, 90.2% with a blunt mechanism of injury and mean ISS of 24.3 +/− 12.0 SD. Of these patients, 511 (8.1%) received prophylactic VCFs, (inserted in absence of PE) at the discretion of the treating clinician. There were 45 PE (incidence of 0.71%), of which two were fatal. Three variables were independently associated with the occurrence of PE in the multivariate analysis: (i) presence of prophylactic VCF (OR 0.28; 95%CI 0.09 - 0.89); (ii) number of injuries to the AIS body region lower limb (OR 1.31; 95%CI 1.17 - 1.47) and (iii) central venous catheterisation (OR 1.87; 95%CI 1.88 - 6.17). Data was available on the VCF database for 429 of the 511 patients with VCFs (84.0%). The mean time to prophylactic VCF insertion was 3.6 +/− 0.2 SEM days after admission. The VCF major complication rate was 2.6% (n=11), including four non-fatal PE. The technical success rate for retrieval was 92.4% (279 retrievals from 302 attempts) and the overall retrieval rate was 65.0% (279 retrievals from 429 placements). Prophylactic VCFs are associated with a reduced rate of PE when used in selected major trauma patients. In addition, prophylactic VCFs have a low major complication rate and high rate of technical success for retrieval

Introduction. Perioperative antibiotic prophylaxis use in modern orthopaedic procedures is well established. Studies have shown significant reduction in risk of post-operative infections. However, as effectiveness of these antibiotics is dependent on achieving high serum and tissue concentrations that exceed the minimum inhibitory concentrations of infective organisms for operation duration, the timing of prophylaxis is crucial. Although, optimal timing for administering prophylaxis varies in the literature, 30 to 60 minutes prior to skin incision or inflation of tourniquet is considered best standards. Objectives. To audit our practice of timing of administration of perioperative prophylactic antibiotics in trauma and orthopaedic procedures. Methods. A double-centre prospective analysis was conducted over a period of 2 week (trauma) and 2 months (elective). Centre A involved trauma cases where centre B involved elective Total Knee Arthroplasty cases. Patients were excluded if they were already on regular antibiotics or when prophylaxis was not indicated. Prescription charts and anaesthetic charts were reviewed along with electronic theatre database. Results. A total of 109 patients were included (52 elective and 57 trauma). Only 21% of patients received prophylactic antibiotics within 30 to 60 minutes of knife to skin time or inflation of tourniquet. Two third of patients received prophylaxis earlier than recommended with 28% receiving prophylaxis between 5 to 10 minutes and 33% between 15 to 20 minutes prior to knife to skin time or inflation of tourniquet. Conclusions. Our audit highlights an important deviation from standard best practice. To address this problem we recommend a multidisciplinary team approach including anaesthetic, ward and theatre teams. Level of Evidence. Introduction and Aims: Practice standards vary considerably for prophylactic pinning the contralateral hip opposite a scfe. This work provides a data-driven framework with which to analyse the risks, benefits, and costs of two modes of treatment: prophylactic pinning the normal side contralateral to a scfe versus observation; and pinning of subsequent slips. Method: A decision analysis model was constructed using the English language literature to estimate SCFE incidence and severity. The model framework assumes that if a hip is pinned prophylactically it will not slip. The benefits of prophylactic pinning are therefore determined by identifying the percentage of contralateral hips that will subsequently slip and then develop early osteoarthritis requiring total hip arthroplasty at a young age. In our cost analysis model, the cost of diagnosis, treatment, and follow-up was developed for the two treatment modes using actual hospital costs and standardised medicare reimbursement schedules for professional fees. Results: If a hip is not pinned prophylactically, there is a 7% risk of requiring a total hip arthroplasty in the contralateral hip at an early age due to osteoarthritis. This 7% is a combination of patients whose contralateral femoral epiphysis slipped moderately or severely and was pinned in a non-anatomic position (1.5% of the initial population) and patients whose substantial slip was not detected (5.5%). The risk of prophylactic pinning appears to be associated with a 0.3% chance of developing avascular necrosis. Other risks would include chances of infection or chondrolysis, although these have not been reported to date. If every patient is managed by the prophylactic pinning protocol, the total cost per patient, not including lost time at work or school, for pinning a slipped capital femoral epiphysis, prophylactically pinning the contra lateral side, and post-operative follow-up is $6266. Conversely, the total cost per patient for the second mode, pinning a slipped capital femoral epiphysis, post-operative follow-up and pinning of subsequent contralateral slips that are detected is $6864. Costs of later hip arthroplasty were not included. This analysis suggests that costs to a national health care system for treatment of SCFE would not increase by prophylactic contralateral pinning. Conclusions: Prophylactic pinning of the contralateral side is merited on the basis of both risk/benefit and cost-effectiveness analyses. Ultimately, the decision must incorporate the physician’s assessment of patient risk factors including: age, weight, co-morbidities, sports activities, the likelihood and feasibility of regular follow-up, and the patient’s and parents’ acceptance of prophylactic surgery

Introduction. Prophylactic pinning of the contralateral hip in the treatment of slipped upper femoral epiphysis (SUFE) has been shown to be safer than continued observation of the contralateral hip. This treatment remains controversial due to the potential for harm caused to an apparently unaffected hip. There is evidence that pinning of an already slipped epiphysis causes growth disturbance of the proximal femur, however this has been questioned in that the slip occurs at the hypertrophic layer of the growth plate with no damage to the germative layer. Aim. To determine whether prophylactic pinning affects subsequent growth of the unaffected hip in cases of unilateral SUFE. Method. In order to determine the effect of prophylactic pinning we compared radiographs of skeletally mature patients who had either undergone prophylactic pinning (group 1), pinning of the affected side only (group 2), and adults with no history of SUFE (group 3). We measured the articulo-trochanteric distance (ATD) and calculated the ratio of the trochanteric-trochanteric distance to articulo-trochanteric distance. These measures have been used in previous studies and have been shown to be reliable indicators of disturbed proximal femoral growth. As this was a pilot study we recruited 8 to each group. Results. The absolute sum of the ATDs were 219mm (average 27.3mm) Group 1, 213mm (average 26.6mm) Group2 and 258mm (average 32.5mm). The average trochanter-trochanter: ATD ratio in group 1 was 2.7 (1.9–3.8) compared to 2.7 (2.3–3.2) and 2.3 (1.9–2.7) in groups 2 and 3 respectively. Conclusion. Our results suggest no difference in subsequent growth between hips that are prophylactically pinned and those that are not. Abnormal growth was observed in unpinned hips suggesting undiagnosed SUFE in some cases. In this series pinning had no effect on proximal femoral growth in patients with SUFE

Purpose: After vertebroplasty and kyphoplasty the recurrent fracture risk after kyphoplasty is inferior to vertebroplasty, but the risk is still eminent. The reduction of kyphosis is strongly related to the age of the fracture, therefore the reduction and the correction of the kyphosis varies. We investigated the indication of a prophylactic kyphoplasty of adjacent levels to the fracture site in order to decrease the postoperative refracture risk. Methods: Prospective randomized clinical study, 2 groups: monosegmental kyphoplasty versus bi- or mul-tisegmental kyphoplasty with prophylactic level superior to the fracture. F/U 6 months with X-rays. Results: Group 1: monosegmental stabilization: (23 pat. 4 male, 19 female, 23 levels). 5 refractures. 3 refractures adjacent to the kyphoplasty, 2 cases not related to the primary stabilization. In 3 cases cement leakage was seen as direct cause of the refracture. |Group 2: pro-phylactic stabilization (27 pat. - 4 male, 23 female, 27 prophylactic levels). 7 refractures, all adjacent to kypho-plasty. In 3 cases cement leakage as cause of recurrent fracture. No statistical difference between both groups was found, however in 6 out of 12 recurrent fractures we found intradiscal cement leakage as the direct cause of the fracture.|. Conclusions: No statistical difference between both groups (group 1: 22% refracture risk, group 2: 26% refracture risk, p=1). Therefore we do not see the need for a generalized prophylactic stabilization of adjacent levels with kyphoplasty. However, we conclude that it is crucial to avoid any leakage, which has a direct impact on the fracture rate, only in those cases we advise a prophylactic stabilization at the leakage site. In general kyphoplasty is preferable to vertebroplasty due to generally decreased leakage and embolism rate

Introduction: Percutaneous vertebroplasty (PVP) is a treatment option for osteoporotic vertebral compression fractures (VCFs). Short-term results are promising but longer-term studies have demonstrated an accelerated failure rate in the adjacent vertebral body (VB). Limited research has been conducted into the effects of prophylactic PVP in osteoporotic vertebrae. The objective of this study was to investigate the biomechanical characteristics of prophylactic vertebral reinforcement and post-fracture augmentation. Methods: Human vertebrae were assigned to two scenarios: Scenario 1 used an experimental model for simulating VCFs followed by cement augmentation; Scenario 2 involved prophylactic augmentation using vertebroplasty. μCT imaging was performed to assess the bone mineral density (BMD), vertebral dimensions, fracture pattern and cement volume. All augmented VBs were then axially compressed to failure. Results: Product of BMD value and endplate surface area gave the best prediction of failure strength when compared to actual failure strength of specimens in scenario 1. Augmented VBs showed an average cement fill of 23.9%±8.07% S.D.. In scenario 1, there was a significant post-vertebroplasty factorial increase of 1.72 and in scenario 2 a 1.38 increase in failure strength. There was a significant reduction in stiffness following augmentation for scenario 1 (t=3.5, P=0.005). Stiffness of the VB in scenario 2 was significantly greater than observed in scenario 1 (t=4.4, P=0.0002). Discussion: Results suggest that augmentation of the VB post-fracture significantly increases failure load, whilst stiffness is not restored. Prophylactic augmentation was seen to increase failure strength in comparison to the predicted failure load. Stiffness appears to be maintained suggesting that prophylactic PVP maintains stiffness better than PVP post-fracture

Prophylactic stabilisation with internal fixation of the asymptomatic hip in unilateral slipped capital femoral epiphysis is controversial. The incidence of bilaterality varies from 20–80% depending on the length of follow-up. The opposite hip has 2335 times higher incidence of developing a slip in cases of a unilateral slip at presentation and there is no chemical, anatomic or radiological feature which can predict a slip. The arguments regarding prophylactic fixation are based on risks of AVN, chondrolysis, and problems with implant removal and joint penetration. We present a retrospective analysis of sixty-five patients who had prophylactic fixation of the uninvolved hip at the same time as their opposite slipped femoral physis. None had an underlying systemic or endocrine disorder and the average age was 12.5 years (range 11–15 years).A single 7.0 mm cannulated screw was used in all cases. The average time to fusion was 18 months (range 6 to 36 months) and duration of follow up ranged from 3–8 years (mean 4.5 years). None of the patients had implant removal and at latest review did not show any evidence of chondrolysis, avascular necrosis, premature physeal arrest or secondary arthrosis in the prophylactically fixed hip. There were a couple of cases of inadvertent wire penetration into joint, which were recognised and rectified immediately, and a correct length screw inserted. Both these patients had an uneventful post-operative course with no problems of chondrolysis etc at latest follow-up (5 years). One patient (1.5%) developed a superficial wound infection, which cleared up with antibiotics. Conclusion: This study demonstrates the safety of prophylactic fixation using a single cannulated cancellous screw and is recommended for prevention of delayed slip and hence secondary osteoarthrosis

The consequences of the complications associated with the management of slipped upper femoral epiphysis are a major source of disability in young adults. Whilst the management of chondrolysis, avascular necrosis or malunion of the femoral neck is usually undertaken by paediatric orthopaedic surgeons the initial management of SUFE in many regions is as part of an adult trauma service. This retrospective audit assessed the outcome of the management of SUFE in one such health region in which treatment occurred at three sites by a number of surgeons of varying experience, during the period July 1994 to June 2004. The aim was to compare our outcomes with those published and to identify whether our service should be altered as a consequence. The case notes and x-rays as recorded in theatre records were retrieved. Of the 64 cases that were treated during this period adequate records for 60 patients were available. Of these 60 patients there were 7 bilateral cases. Fixation in all 67 cases was by a single cannulated screw. In the 53 unilateral cases 17 underwent prophylactic pinning, the remaining 36 remained under observation. Of these nine patients presented with subsequent slips, eight of which were unstable and two had slip angles greater than 60° in which one developed avascular necrosis. Four other cases of avascular necrosis were observed (incidence 6%). Chondrolysis occurred in one patient with persistent pin penetration. In the remaining 73 cannulated screws used for stabilisation and 17 for prophylactic fixation no complications were observed. The complication rates observed in this series are within those accepted in the literature. The high incidence of subsequent slips and the attendant severity of these when compared with the relative safety of contemporary cannulated screw fixation has led us to recommend prophylactic pinning in our region
