Instability and aseptic loosening are the two main complications after revision total hip arthroplasty (rTHA). Dual-mobility (DM) cups were shown to counteract implant instability during rTHA. To our knowledge, no study evaluated the 10-year outcomes of rTHA using DM cups, cemented into a metal reinforcement ring, in cases of severe acetabular bone loss. We hypothesized that using a DM cup cemented into a metal ring is a reliable technique for rTHA at 10 years, with few revisions for acetabular loosening and/or instability. Uncemented component retention with polyethylene (PE) liner and femoral head exchange is commonly used to treat periprosthetic osteolysis. The purpose was to determine long-term implant survivorship, risk factors for aseptic failures, clinical outcomes, and complications following PE liner and head exchange. We identified 116 hips in 110 patients treated with PE liner and head exchange for osteolysis from 1993 to 2004. The mean age was 58, 64 were women, and mean follow-up was 11 years. THA in patients with acetabular bone defects is associated with a high risk of dislocation. Dual mobility (DM) cups are known to prevent and treat chronic instability. The aim of this study was to evaluate the dislocation rate and survival of jumbo DM cups. This was a retrospective, continuous, multicenter study of all the cases of jumbo DM cup implantation between 2010 and 2017 in patients with acetabular bone loss (Paprosky 2A: 46%, 2B: 32%, 2C: 15% and 3A: 6%). The indications for implantation were revisions for aseptic loosening of the cup (n=45), aseptic loosening of the femoral stem (n=3), bipolar loosening (n=11), septic loosening (n=10), periprosthetic fracture (n=5), chronic dislocation (n=4), intraprosthetic dislocation (n=2), cup impingement (n=1), primary posttraumatic arthroplasty (n=8), and acetabular dysplasia (n=4). The jumbo cups used were COPTOS TH (SERF), which combines press-fit fixation with supplemental fixation (acetabular hook, two superior flanges with one to four screws, two acetabular pegs). Chronic pelvic discontinuity (CPD) during revision hip arthroplasty is a challenging entity to address. The aim of this study was to evaluate the clinical and radiologic outcomes, and complications of the “acetabular distraction technique” for the management of CPD during revision hip arthroplasty. Patients with CPD, who underwent acetabular revision between 2014 and 2022 at two tertiary care centres, using an identical distraction technique, were evaluated. Demographic parameters, pre-operative acetabular bone loss, duration of follow-up, clinical and radiologic outcomes, and survivorship were evaluated. In all, 46 patients with a mean follow-up of 34.4 (SD=19.6, range: 24–120) months were available for evaluation. There were 25 (54.3%) male, and 21 (45.7%) female patients, with a mean age of 58.1 (SD=10.5, range: 40–81) years at the time of revision surgery. Based on the Paprosky classification of acetabular bone loss, 19 (41.3%), 12 (26.1%), and 15 (32.6%) patients had type 3b, 3a, and 2c defects. All patients were managed using the Trabecular Metal™ Acetabular Revision System; 16 patients required additional Trabecular Metal™ augments. The mean HHS improved from 50.1 (SD=7.6, range: 34.3 – 59.8) pre-operatively, to 86.6 (SD=4.2, range: 74.8 -91.8) at the last follow-up. Two patients (4.3 %) developed partial sciatic nerve palsy, two (4.3%) had posterior dislocation, and one (2.2%) required re-revision for aseptic loosening. Angular proximal femoral deformities increase the technical complexity of primary total hip arthroplasties (THAs). The goals were to determine the long-term implant survivorship, risk factors, complications, and clinical outcomes of contemporary primary THAs in this difficult cohort. Our institutional total joint registry was used to identify 119 primary THAs performed in 109 patients with an angular proximal femoral deformity between 1997 and 2017. The deformity was related to previous femoral osteotomy in 85%, and developmental or metabolic disorders in 15%. 53% had a predominantly varus angular deformity. The mean age was 44 years, mean BMI was 29 kg/m. 2. , and 59% were female. An uncemented metaphyseal fixation stem was used in 30%, an uncemented diaphyseal fixation stem in 28%, an uncemented modular body stem with metaphyseal fixation sleeve in 24%, and a cemented stem in 18%. Simultaneous corrective femoral osteotomy was performed in 18%. Kaplan-Meier survivorships and Harris hip scores were reported. Mean follow-up was 8 years. The decision to undertake total hip replacement (THA) in a child is complex and daunting. This is augmented by the paucity of data on potential quality of life (QoL) improvement and functional outcomes. Therefore, the aim of this study was to ascertain whether outcomes after surgery are influenced by the nature of the primary diagnosis. This was a prospective, consecutive cohort study of patients under the age of 18 years undergoing THA by a single arthroplasty surgeon in collaboration with colleagues at a regional paediatric hospital. Patient electronic notes, radiographs and PROMS (EQ5D-Y, Oxford Hip Score (OHS) and modified Harris Hip Score(mHHS)) were reviewed. Twenty-two THAs were performed in patients less than 18 years (median 15 (range 10.7–17.9), with 7 patients undergoing bilateral surgery. Mean follow-up was 2 years. Thirteen of the THAs were undertaken for systemic conditions effecting multiple joints (Mucopolysaccharidoses, Mucolipidosis and Scwachman-Diamond syndrome) with the hip the worst affected joint and 9 for single joint disease (AVN, Perthes, dysplasia and idiopathic chondrolysis). PROMS scores showed reliable improvements with no differences between the two groups. Background. Reduced dislocation rates using lipped polyethylene (PE) liners in modular uncemented acetabular components has been shown, yet there may be increased wear because of impingement, which may lead to aseptic loosening. We used New Zealand Joint Registry (NZJR) data to compare survival rates, revision rates for dislocation and aseptic loosening between lipped and neutral liners. Methods. 31,247 primary THAs using the four commonly used uncemented modular cups were identified (January 1, 1999 to December 31, 2018). The lipped liner group comprised 49% males vs 42% in the neutral group (p < 0.001); 96% had OA vs 95% in the neutral group (p < 0.001). There was no difference in other patient characteristics. Mean follow-up was 5.1 years (SD 3.9); longest follow-up 19.3 years. Kaplan Meier survival rates were compared (20,240 lipped and 11,007 neutral PE liners). Highly cross-linked PE was used in 99% of lipped liner cups vs 85% of neutral liner cups. Associated hazard ratios were calculated using a Cox regression analysis. Results. KM survival at 10 years for lipped PE liners was 96% for lipped (95%CI 95.4%-96.2%) and 95% for neutral liners (95%CI 94.7%-95.9%). Controlling for age, gender approach, head size, image guidance, the all-cause revision risk was greater for neutral liner (HR 1.17 [95% CI 1.06 to 1.36]; p = 0.032). Introduction. The Opera acetabular component (Smith & Nephew Ltd) shares many same design features with the Ogee (Depuy Int.). Differences are a malleable flange, instrumentation designed to improve cement pressurisation and the specifics of the UHMW-poly used. Methods. Data was collected prospectively on 419 consecutive replacements (390 patients), performed between March 2000 and February 2005. A posterior approach was used, and the acetabulum was prepared with multiple key holes. Palacos-R bone cement containing Gentamicin was used with a cement pressuriser. Results. There were 243 Operas (221 patients) and 176 Ogees (169 patients). There were 262 in female patients (62.5%) and age at surgery averaged 68.3yrs (25–92). With increasing burden of revision hip arthroplasty, one of the major challenge is the management of bone loss associated with previous multiple surgeries. Proximal femoral replacement (PFR) has already been popularised for tumour surgeries. The inherent advantages of PFR over allograft –prosthesis system, which is the other option for addressing severe bone loss include, early weight bearing and avoidance of non-union and disease transmission. Our study explores PFR as a possible solution for the management of complex hip revisions. Thirty consecutive hips (29 patients) that underwent PFR between January 2009 and December 2015 were reviewed retrospectively for their clinical and radiological outcomes. The Stanmore METS system was used in all these patients. Mean age at the index surgery (PFR) was 72.69 years (range 50–89) with number of previous hip arthroplasties ranging from 1–5. At mean follow up of 32.27 months, there were no peri-prosthetic fractures and no mechanical failure of the implants. Clearance of infection was achieved in 80% of cases. There was 1 early failure due to intra-operative perforation of femoral canal needing further revision and two were revised for deep infection. Instability was noted in 26.7% (8) of the hips, of which, 87.5% (7) needed further revision with constrained sockets. Out of these 8 hips with instability, 5 had pre-operative infection. Deep infection was noted in 20% (6) of the hips, of which, 5 were primarily revised with PFR for septic loosening. However, further surgeries were essential for only 3 patients. The treatment of severe acetabular bone loss is challenging, especially in the setting of an associated chronic pelvic discontinuity. There are several available treatment options for chronic pelvic discontinuity, each of which has its own disadvantages. One of the major difficulties with this entity, regardless of the reconstructive technique chosen, is the inability to obtain reproducible healing of the discontinuity. We evaluated the use of acetabular distraction, a technique which achieves peripheral or lateral distraction and central or medial compression across the discontinuity. We recommend acetabular distraction to allow for implantation of a stable construct, achieve biologic fixation and increase the likelihood of discontinuity healing. In this multi-center trial, 32 patients that underwent acetabular revision for a chronic pelvic discontinuity using acetabular distraction were radiographically evaluated at a minimum of 25 months (range, 25 to 160 months). The study cohort was categorized according to the Paprosky acetabular bone loss classification: seven (22%) type IIC, five (16%) type IIIA, and 20 (62%) type IIIB defects. Fourteen (70%) of the 20 patients with a type IIIB acetabular bone loss pattern required use of augments for acetabular reconstruction. Of the 32 patients, 1 (3%) patient required a revision for aseptic loosening, 2 (6%) patients had evidence of radiographic loosening but were not revised, and 3 (9%) patients had migration of the acetabular component into a more stable position. Introduction. Reverse hybrid total hip replacement (THR) offers significant theoretical benefits but is uncommonly used. Our primary objective was to evaluate implant survival with all cause revision and revision for aseptic loosening of either component as endpoints. Patients/Materials & Methods. Data was collected prospectively on 1, 088 (988 patients) consecutive reverse hybrid THRs. Mean patient age was 69.3 years (range, 21–94) and mean follow-up was 8.2 years (range, 5–11.3). No patients were lost to follow-up. Overall, 194 (17.8%) procedures were performed in patients under 60 years, 666 (61.1%) were performed in female patients and 349 (32.1%) were performed by a trainee. Acetabular components were ultra-high molecular weight polyethylene in 415 (38.1%) hips, highly cross-linked polyethylene in 669 (61.5%) hips and vitamin E stabilised polyethylene in 4 (0.4%) hips. Femoral stems were collared in 757 (69.7%) hips and collarless in 331 (30.3%) hips. Femoral head sizes were 28 mm in 957 (87.9%) hips and 32 mm in 131 (12.1%) hips. The aim of this study was to evaluate the survival of a collarless, straight, hydroxyapatite-coated femoral stem in total hip arthroplasty (THA) at a minimum follow-up of 20 years. We reviewed the results of 165 THAs using the Omnifit HA system in 138 patients, performed between August 1993 and December 1999. The mean age of the patients at the time of surgery was 46 years (20 to 77). Avascular necrosis was the most common indication for THA, followed by ankylosing spondylitis and primary osteoarthritis. The mean follow-up was 22 years (20 to 31). At 20 and 25 years, 113 THAs in 91 patients and 63 THAs in 55 patients were available for review, respectively, while others died or were lost to follow-up. In revision hip surgery, a solidly cemented femoral component may obstruct access to the acetabulum, may be poorly orientated, or may have inadequate offset and head diameter. These problems can be addressed by revising the femoral component. The object of this study was to determine the outcome of cementing a polished, tapered, modular implant into the retained cement mantle. Introduction: The aim of this study was to study the intermediate term (5-10 year) results of the ‘Metasul’ type of metal-on-metal hip arthroplasty. Method: A prospective analysis was performed of the clinical and radiological findings of the patients who underwent the ‘Metasul’ hip arthroplasty by the senior author (Mr. Benke). The clinical evaluation was performed with the Charnley’s modification of the Merle d’ Aubigne scoring system as well as the Oxford Hip Scoring Sheet. All the patients operated from February 1995 till July 2000, were included in the study. Of the total of 99 patients, 5 patients died prior to the final review and 4 were lost to follow-up. The final study group hence contained 90 Hip Arthroplasties in 77 patients. Results: The results were analysed using the Charnley categories, and as expected the patients of Category ‘A’ achieved the best final results with the d’Aubigne score rising from 8.6 to 17.2 and Oxford scores of 5.7/60. The patients of Category ‘B’ had good results (Final Oxford score of 7.3/60, and d’Aubigne score of 16.9). The Category ‘C’ patients had moderate results with the d’Aubigne score of 15.1 and Oxford score of 17.7/60. Three patients needed revision hip arthroplasty for the following reasons:

Purpose: Instability is a major cause of revision total hip arthroplasty (rTHA). It was the third leading cause (5%) after aseptic loosening (75.7%), and infection (7.2%) in the Swedish registry for 2000. The rate of dislocation is however variable depending on the reported series. We conducted a statistical analysis of nearly 3000 files of rTHA to study factors favouring failure. Material and methods: AVIO, a watch association for orthopaedic implants, was created in 1994 to evaluate factors favouring failure of THA in France, and to furnish statistical data on prosthesis revision. About one hundred orthopaedic surgeons throughout France completed an information card for each revision for a five year period (September 1994 to September 1999). A total of 2926 card were analysed. Patient data (gender, side, number of revisions, age at implantation, age at removal, reason for THA, reason for revision, duration of THA) were cross analysed. The statistical analysis was conducted with chi-square tests and non-parametric tests. Significant results were compared with data in the literature. We present here the results concerning dislocation as the reason for revision. Results: The rTHA for dislocation accounted for 9.2% of the revisions. Dislocation was the primary reason for revision in patients with repeated revisions and was also the main reason for revision in patients aged over 70 years (20.6%). In this group of older patients, 72.8% of the prostheses were implanted for degenerative disease. Discussion: The original finding in this study, not clearly reported in the literature, is that dislocation is the primary cause of revision after 70 years, coming before
