Aims. While residual fixed flexion deformity (FFD) in unicompartmental knee arthroplasty (UKA) has been associated with worse functional outcomes, limited evidence exists regarding FFD changes. The objective of this study was to quantify FFD changes in patients with medial unicompartmental knee arthritis undergoing UKA, and investigate any correlation with clinical outcomes. Methods. This study included 136 patients undergoing robotic arm-assisted medial UKA between January 2018 and December 2022. The study included 75 males (55.1%) and 61 (44.9%) females, with a mean age of 67.1 years (45 to 90). Patients were divided into three study groups based on the degree of preoperative FFD: ≤ 5°, 5° to ≤ 10°, and > 10°. Intraoperative optical motion capture technology was used to assess pre- and postoperative FFD. Clinical FFD was measured pre- and postoperatively at six weeks and one year following surgery. Preoperative and one-year postoperative Oxford Knee Scores (OKS) were collected. Results. Overall, the median preoperative navigated (NAV) FFD measured 6.0° (IQR 3.1 to 8), while the median postoperative NAV FFD was 3.0° (IQR 1° to 4.4°), representing a mean correction of 49.2%. The median preoperative clinical FFD was 5° (IQR 0° to 9.75°) for the entire cohort, which decreased to 3.0° (IQR 0° to 5°) and 2° (IQR 0° to 3°) at six weeks and one year postoperatively, respectively. A statistically significant improvement in PROMs compared with baseline was evident in all groups (p < 0.001). Regression analyses showed that participants who experienced a larger FFD correction, showed greater improvement in PROMs (β = 0.609, p = 0.049; 95% CI 0.002 to 1.216). Conclusion. This study found that UKA was associated with an approximately 50% improvement in preoperative FFD across all three examined groups. Participants with greater correction of FFD also demonstrated larger OKS gains. These findings could prove a useful augment to clinical decision-making regarding candidacy for UKA and anticipated improvements in FFD

Aims. Hip displacement, common in patients with cerebral palsy (CP), causes pain and hinders adequate care. Hip reconstructive surgery (HRS) is performed to treat hip displacement; however, only a few studies have quantitatively assessed femoral head sphericity after HRS. The aim of this study was to quantitatively assess improvement in hip sphericity after HRS in patients with CP. Methods. We retrospectively analyzed hip radiographs of patients who had undergone HRS because of CP-associated hip displacement. The pre- and postoperative migration percentage (MP), femoral neck-shaft angle (NSA), and sphericity, as determined by the Mose hip ratio (MHR), age at surgery, Gross Motor Function Classification System level, surgical history including Dega pelvic osteotomy, and triradiate cartilage status were studied. Regression analyses using linear mixed model were performed to identify factors affecting hip sphericity improvement. Results. A total of 108 patients were enrolled. The mean preoperative MP was 58.3% (SD 31.7%), which improved to 9.1% (SD 15.6%) at the last follow-up. NSA and MHR improved from 156.5° (SD 11.5°) and 82.3% (SD 8.6%) to 126.0° (SD 18.5°) and 89.1% (SD 9.0%), respectively. Factors affecting the postoperative MHR were preoperative MP (p = 0.005), immediate postoperative MP (p = 0.032), and history of Dega osteotomy (p = 0.046). Conclusion. We found that hip sphericity improves with HRS. Preoperative MP, reduction quality, and acetabular coverage influence femoral head remodelling. We recommend that surgeons should consider intervention early before hip displacement progresses and that during HRS, definite reduction and coverage of the femoral head should be obtained. Introduction. The role for hip arthroscopy (HS) in combination with periacetabular osteotomy (PAO) for the treatment of symptomatic acetabular dysplasia has not been clearly defined. In this study, we assessed midterm outcomes of concurrent hip arthroscopy and PAO (HS/PAO) for the treatment of symptomatic acetabular dysplasia with associated intra-articular pathology. Methods. We performed a retrospective review of concurrent HS/PAO cases in a prospectively collected institutional hip preservation database. From November 2005 to December 2012, 85 hips in 80 patients underwent combined HS/PAO. Two hips had a diagnosis other than acetabular dysplasia and 12 hips were lost to follow up, leaving 71 hips for analysis at mean 6.6-year follow-up (range 4–11 years). Hips were evaluated using the University of California, Los Angeles (UCLA) Activity Score, modified Harris Hip Score (mHHS), and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) pain subscore. Hips were stratified into preserved or failed groups, with failure defined as conversion to THA or a symptomatic hip (mHHS <70 or WOMAC pain subscore > 10). Complications were recorded and graded by the Clavien-Dindo classification. Regression analyses were performed to identify the interaction between patient factors, radiographic measures, and surgical details with outcome. Results. At final follow-up, two hips had undergone THA and an additional 15 hips were deemed symptomatic. 54 hips (76.1%) did not undergo conversion to THA or meet symptom thresholds, and this group had mean mHHS 92.1 ± 8.8, UCLA activity score 7.8 ± 2.1, and WOMAC pain subscore 1.8 ± 2.4. In univariate analysis, no patient factors, pre- or post-operative radiographic metrics, or intra-operative findings or procedures were associated with failure. There were 3 (4.2%) major complications (Clavien-Dindo Grade III or IV), and 5 patients underwent repeat surgery for recurrent symptoms (4 hip arthroscopy, 1 open psoas release with ramus osteoplasty) during the follow up period with resolution of symptoms following repeat surgery. Conclusion. This study demonstrates that concurrent hip arthroscopy and periacetabular osteotomy is associated with good clinical outcomes and an acceptable complication profile at midterm follow up

Introduction. Biomechanists have been trying to obtain integrated and accurate human motion data. However, it is not so easy, because some have limitation of accuracy, some have limitation of the observation area, and some are expensive. For example, motion capturing can obtain whole body motion data, but needs space, is expensive, but only surface motion could be obtained. So is not so sensitive for the bone rotation. Sensors of pressure, acceleration, and so forth are less expensive and less hard to use, but the data are limited. 2D–3D shape matching such as Jointtrack can describe bone motion including rotation, but the detectable area is limited by the size of flat panel fluoroscopy. In this study, we have combined multiple joint motion analysis by Jointtrack and reconstructed full lower extremities' motion. Method. Pelvis, bilateral femurs, and bilateral tibiae geometries were obtained from CAT scan using Mimics®. Gait motion fluoroscopy was done on a treadmill around hip joints and knee joints (Fig.1). On each heal thin film switch was attached and connected to electrically driven metal flag which can be recorded in fluoroscopic images on heal strikes. Images of five gait cycles were taken with 15Hz and every image was sorted by the percentage of gait cycle, and then processed by Jointtrack. Centre of femoral head of observing side was defined as our origin. Using treadmill, the walking direction could be uniquely defined. From the femoral 3D displacement and rotation, knee position can be calculated. The same procedures were done for the knee assessment, mutual coordinate of hip centre from the knee can be calculated. All of them are sorted by the percentage of the gait cycle too. Combining data from hip and knee, complete lower extremities' motion could be described. Regression analyses of x, y, z coordinates of femurs from hip and from knee were done to evaluate the accuracy. Motion capture of floor gait and treadmill gait were done to evaluate the difference. (Fig.2). Materials. Five arthritic hip patients and one normal control were assessed in hip and knee by Jointtrack. Normal Control was assessed by motion capture too. Results. The motions of lower extremities were descrobbed as the translations of coordinates of hip, knee, and ankle joints (Fig.3). The coefficients of determination were calculated for each patient. Except for one patient, good regression was obtained between hip assessment and knee assessment. In motion capture, the difference of the gait speed on the floor and on the treadmill effected to the gait pattern. Discussion. Motion capture can assess multiple joint motions in natural state, but they are only skin motion. Jointtrack can deprive 3D translation and rotation data of the bones, but the object is limited to single joint and is unnatural in traveling direction. Our method can enlarge its object to multiple joints. Getting more data, we hope we can analyze bone motions including rotations and resultant force etc. more accurately and more practically

The two most common surgical approaches used in primary hip arthroplasty are the anterolateral and posterolateral approach. While both approaches have potential advantages and disadvantages in comparison to the other, there are no reports with sufficient power to compare their effect on post-operative function and quality of life. This prospective cohort study evaluates and compares the independent effect of surgical approach on function and quality of life among patients who have received their total hip arthroplasty through one of these techniques. Results suggest that there is no significant difference in function or quality of life among these respective cohorts. The purpose of this study was to evaluate and compare the effect of anterolateral versus posterolateral approach on function and quality of life following primary total hip arthroplasty. There is no significant difference in outcome as a result of surgical approach. Significance: This is the first report with sufficient power to evaluate and compare outcomes of these two approaches among high volume hip surgeons. This study suggests that surgeons need not worry that the anterolateral or posterolateral approach may compromise patient function or generic quality of life more-so than the alternative approach. Univariate and regression analyses demonstrate similar outcomes on WOMAC, Oxford-12, SF-12 and satisfaction among cohorts of patients receiving the anterolateral or posterolateral approach for primary total hip arthroplasty. A prospective evaluation of function and quality of life outcomes was performed on patients receiving the anterolateral or posterolateral approach for primary hip arthroplasty. All participating surgeons were considered to be high volume arthroplasty surgeons. One hundred and sixty procedures were followed and provided complete outcomes data including WOMAC, Oxford, SF-12, and satisfaction scores. Univariate analyses demonstrated no differences among the two cohorts with regards to baseline variables. Regression analyses were performed for the outcome measures with surgical approach treated as an independent covariate while controlling for other potential confounders. The results suggest that there is no independent effect of surgical approach on patient reported outcomes. This suggests that the surgeon should continue to employ the approach which is the most familiar and most likely to result in consistent results in their clinical environment

Aims

Several studies have reported that patients presenting during the evening or weekend have poorer quality healthcare. Our objective was to examine how timely surgery for patients with severe open tibial fracture varies by day and time of presentation and by type of hospital. This cohort study included patients with severe open tibial fractures from the Trauma Audit and Research Network (TARN).

Methods

Provision of prompt surgery (debridement within 12 hours and soft-tissue coverage in 72 hours) was examined, using multivariate logistic regression to derive adjusted risk ratios (RRs). Time was categorized into three eight-hour intervals for each day of the week. The models were adjusted for treatment in a major trauma centre (MTC), sex, age, year of presentation, injury severity score, injury mechanism, and number of operations each patient received.

Aims

Complex joint fractures of the lower extremity are often accompanied by soft-tissue swelling and are associated with prolonged hospitalization and soft-tissue complications. The aim of the study was to evaluate the effect of vascular impulse technology (VIT) on soft-tissue conditioning in comparison with conventional elevation.

Methods

A total of 100 patients were included in this prospective, randomized, controlled monocentre study allocated to the three subgroups of dislocated ankle fracture (n = 40), pilon fracture (n = 20), and intra-articular calcaneal fracture (n = 40). Patients were randomized to the two study groups in a 1:1 ratio. The effectiveness of VIT (intervention) compared with elevation (control) was analyzed separately for the whole study population and for the three subgroups. The primary endpoint was the time from admission until operability (in days).

Objectives

The use of ceramic femoral heads in total hip arthroplasty (THA) has increased due to their proven low bearing wear characteristics. Ceramic femoral heads are also thought to reduce wear and corrosion at the head-stem junction with titanium (Ti) stems when compared with metal heads. We sought to evaluate taper damage of ceramic compared with metal heads when paired with cobalt chromium (CoCr) alloy stems in a single stem design.

Methods

This retrieval study involved 48 total hip arthroplasties (THAs) with CoCr V40 trunnions paired with either CoCr (n = 21) or ceramic (n = 27) heads. The taper junction of all hips was evaluated for fretting/corrosion damage and volumetric material loss using a roundness-measuring machine. We used linear regression analysis to investigate taper damage differences after adjusting for potential confounding variables.

Objectives

The purpose of this study was to develop an accurate, reliable and easily applicable method for determining the anatomical location of the joint line during revision knee arthroplasty.

Methods

The transepicondylar width (TEW), the perpendicular distance between the medial and lateral epicondyles and the distal articular surfaces (DMAD, DLAD) and the distance between the medial and lateral epicondyles and the posterior articular surfaces (PMAD, DLAD) were measured in 40 knees from 20 formalin-fixed adult cadavers (11 male and nine female; mean age at death 56.9 years, sd 9.4; 34 to 69). The ratios of the DMAD, PMAD, DLAD and PLAD to TEW were calculated.
