Aims. This study uses prospective registry data to compare early patient outcomes following arthroscopic repair or debridement of the acetabular labrum. Methods. Data on adult patients who underwent arthroscopic labral debridement or repair between 1 January 2012 and 31 July 2019 were extracted from the UK Non-Arthroplasty Hip Registry. Patients who underwent microfracture, osteophyte excision, or a concurrent extra-articular procedure were excluded. The EuroQol five-dimension (EQ-5D) and International Hip Outcome Tool 12 (iHOT-12) questionnaires were collected preoperatively and at six and 12 months post-operatively. Due to concerns over differential questionnaire non-response between the two groups, a combination of random sampling, propensity score matching, and pooled multivariable linear regression models were employed to compare iHOT-12 improvement. Results. A total of 2,025 labral debridements (55%) and 1,659 labral repairs (45%) were identified. Both groups saw significant (p < 0.001) EQ-5D and iHOT-12 gain compared to preoperative scores at 12 months (iHOT-12 improvement: labral repair = +28.7 (95% confidence interval (CI) 26.4 to 30.9), labral debridement = +24.7 (95% CI 22.5 to 27.0)), however there was no significant difference between procedures after multivariable modelling. Overall, 66% of cases achieved the minimum clinically important difference (MCID) and 48% achieved substantial clinical benefit at 12 months. Conclusion. Both labral procedures were successful in significantly improving early functional outcome following hip arthroscopy, regardless of age or sex. Labral repair was associated with superior outcomes in univariable analysis, however there was no significant superiority demonstrated in the multivariable model. Level of evidence: III. Arthroscopic management of femoroacetabular impingement (FAI) has become the mainstay of treatment. However, chondral lesions are frequently encountered and have become a determinant of less favourable outcomes following arthroscopic intervention. The aim of this systematic review and meta-analysis was to assess the outcomes of hip arthroscopy (HA) in patients with FAI and concomitant chondral lesions classified as per Outerbridge. A systematic search was performed using the PRISMA guidelines on four databases including MEDLINE, EMBASE, Cochrane Library and Web of Science. Studies which included HA as the primary intervention for management of FAI and classified chondral lesions according to the Outerbridge classification were included. Patients treated with open procedures, for osteonecrosis, Legg-Calve-Perthes disease, and previous ipsilateral hip fractures were excluded. From a total of 863 articles, twenty-four were included for final analysis. Demographic data, PROMs, and radiological outcomes and rates of conversion to total hip arthroplasty (THA) were collected. Risk of bias was assessed using ROBINS-I. Improved post-operative PROMs included mHHS (mean difference:-2.42; 95%CI:-2.99 to −1.85; p<0.001), NAHS (mean difference:-1.73; 95%CI: −2.23 to −1.23; p<0.001), VAS (mean difference: 2.03; 95%CI: 0.93-3.13; p<0.001). Pooled rate of revision surgery was 10% (95%CI: 7%-14%). Most of this included conversion to THA, with a 7% pooled rate (95%CI: 4%-11%). Patients had worse PROMs if they underwent HA with labral debridement (p=0.015), had Outerbridge 3 and 4 lesions (p=0.012), concomitant lesions of the femoral head and acetabulum lesions (p=0.029). Reconstructive cartilage techniques were superior to microfracture (p=0.042). Even in concomitant lesions of the femoral head and acetabulum, employing either microfracture or cartilage repair/reconstruction provided a benefit in PROMs (p=0.027). Acceptable post-operative outcomes following HA with labral repair/reconstruction and cartilage repair in patients with FAI and concomitant moderate-to-severe chondral lesions, can be achieved. Patients suffering from Outerbridge 3 and 4 lesions, concomitant acetabular rim and femoral head chondral lesions that underwent HA with labral debridement, had worse PROMs. Reconstructive cartilage techniques were superior to microfracture. Even in concomitant acetabular and femoral head chondral lesions, employing either microfracture or cartilage repair/reconstruction was deemed to provide a benefit in PROMs

Objectives. The aim of this study was to use registry data to report and compare early patient outcomes following arthroscopic repair or debridement of the acetabular labrum. Methods. Patients who underwent microfracture, osteophyte excision or a concurrent extra-articular procedure were excluded. Outcomes comprised EuroQol-5 Dimensions (EQ-5D) index and the International Hip Outcome Tool 12 (iHOT-12), preoperatively and at 6 and 12 months. Results. A total of 3,553 arthroscopies comprising 1,986 labral debridement (56%) and 1,567 labral repairs (44%) were identified. Both groups saw significant (p <0.0001) EQ-5D and iHOT-12 gain compared to pre-operative baseline scores at 6 months (iHOT-12: repair = +27.4 (95%CI 25.2 to 29.5), debridement = +24.8 (95%CI 22.7 to 26.9)) which was maintained at 12 months (repair = +28.3 (95%CI 26.0 to 30.6), debridement = +24.3 (95%CI 22.0 to 26.5)) with improvement being significantly greater in the repair group by 12 months (p = 0.016) including after modelling for patient and surgical factors. Overall, 64% of cases achieved the minimum clinically important difference (MCID) and 47% achieved substantial clinical benefit (SCB: repair = 51%, debridement = 43%, p = 0.005) at 12 months. Significant functional improvement was seen for both genders and in younger and older (>40 years) cohorts. Conclusion. Both labral repair and debridement techniques were successful in significantly improving 12-month outcomes following hip arthroscopy regardless of age or gender. Labral repair was associated with superior outcomes in comparison to debridement. Whilst patient selection is crucial, our findings support the repair of an injured labrum where possible. A multi-centre randomised controlled trial would be the most appropriate next step to overcome bias inherent in the existing literature

Background. Labral tears are now recognised as a common pathology especially in young adults. With advancement of arthroscopic techniques, most recent published literature is focused on short- or mid-term results of labral repair or re-fixation. There is limited data regarding long-term results of labral debridement and effect of co-existing pathology on outcomes. We investigated long-term results after arthroscopic labral debridement, the predictors of outcomes and correlation with any co-existing hip pathology. Materials and Methods. Between 1996 and 2003, 50 patients who underwent hip arthroscopy and labral debridement with mean follow-up of 8.4±1.7 years (range 7-13.6 years) were included in our study. Patients' pre-operative Harris Hip Score and co-existing pathologies such as FAI, dysplasia or arthritis were recorded as variables. Further, patients' post-operative HHS and satisfaction at the time of follow-up were recorded as outcomes. Spearman's rho correlation coefficient and regression analysis were calculated between these variables and outcomes. Results. Good or excellent results were noted in 80% of patients. The mean pre-operative HHS was 79.3±8.2 (range 58-86) which was improved to 92.2±12.1 (range 46-100) post-operatively. Patients without co-existing pathology had significantly higher satisfaction and HSS. Arthritis had a significantly negative correlation with post-op HHS (rho= -0.27, P<0.05) and satisfaction (rho= -0.32, P<0.05). Eighty-four percent of patients were satisfied and 16% were dissatisfied. All dissatisfied patients (eight cases) had arthritis; two patients also had FAI that did not have decompression of the lesion since it was not recognised at the time. In regression analysis, arthritis was the strongest independent predictors of post-op HHS and satisfaction. Conclusion. Arthroscopic labral debridement of symptomatic tears without co-existing pathology can result in favorable long-term results. Arthritis is the strongest independent predictors of outcomes

Background. The acetabular labrum provides sealing function and a degree of hip joint stability. Previous early(16 month) and mid-term(mean 3.5 years) follow-up of this cohort reported better patient related outcome measures in the refixation group. Methods. We reported patients who underwent labral debridement/focal labral excision during a period before the development of labral repair techniques. Patients with labral tears thought to be repairable with our current arthroscopic technique were compared with patients who underwent labral refixation. In 46 hips, the labrum was focally excised/debrided (group 1); in 54 hips, the labrum was refixed (group 2). Outcomes were measured with modified-Harris-Hip-Score (mHHS), Short Form-12 (SF-12), and a visual-analog-scale(VAS) for pain preoperatively and postoperatively. Results. Mean age was 33 years in group 1 and 28 years in group 2 with mean follow-up of 7 years (range, 2–13.6 years). At mean follow-up, subjective outcomes were significantly improved (P<.01) for both groups compared with preoperative scores. The mHHS (P=.005), SF-12 (P=.025), and VAS pain scores (P<.001) were all significantly better for refixation group compared with debridement group. Although most recent outcomes for both groups fell at mean 7 years' follow-up in comparison to 16 month and 3.5 year follow-up, the disparity between groups was greater in favor of labral refixation. Good-to-excellent results were 47.7% in debridement and 86.3% in refixation (p<.001), and failure rates were 29.6% (debridement) and 13.7% (refixation group) (p =.059). There were 4 revisions in the debridement group and 2 revisions in the refixation group. Conclusion. Longer term follow-up comparing focal labral debridement/excision to refixation revealed a decrease in patient related outcome scoring and good/excellent results in both groups. Ultimately, there was a greater drop in outcome measures and good/excellent results in the debridement group and better maintenance of results in the refixation group at mean 7 years follow-up. Over half of postpartum women experience pelvic ring or hip pain, with multiple anatomic locations involved. The sacroiliac joints, pubic symphysis, lumbar spine and pelvic girdle are all well documented pain generators. However, despite the prevalence of postpartum hip pain, there is a paucity of literature regarding underlying soft tissue intra-articular etiologies. The purpose of this systematic review is to document and assess the available evidence regarding underlying intra-articular soft tissue etiologies of peri- and postpartum hip pain. Three online databases (Embase, PubMed and Ovid [MEDLINE]) were searched from database inception until April 11, 2021. The inclusion criteria were English language studies, human studies, and those regarding symptomatic labral pathology in the peri- or postpartum period. Exclusion criteria were animal studies, commentaries, book chapters, review articles and technical studies. All titles, relevant abstracts and full-text articles were screened by two reviewers independently. Descriptive characteristics including the study design, sample size, sex ratio, mean age, clinical and radiographic findings, pathology, subsequent management and outcomes were documented. The methodological quality of the included studies was assessed using the Methodological Index for Non-Randomized Studies (MINORS) instrument. The initial search identified 2472 studies. A systemic screening and assessment of eligibility identified 5 articles that satisfied the inclusion criteria. Twenty-two females were included. Twenty patients presented with labral pathology that necessitated hip arthroscopy with labral debridement or repair with or without acetabuloplasty and/or femoroplasty. One patient presented with an incidental labral tear in the context of osteitis condensans illi. One patient presented with post-traumatic osteoarthritis necessitating a hip replacement. The mean MINORS score of these 5 non-comparative studies was 2.8 (range 0-7) demonstrating a very low quality of evidence. The contribution of intra-articular soft tissue injury is a documented, albeit sparse, etiology contributing to peri- and postpartum hip pain. Further research to better delineate the prevalence, mechanism of injury, natural history and management options for women suffering from these pathologies at an already challenging time is necessary to advance the care of these patients

This study aimed to analyse the incidence, the operative findings and outcomes of hip arthroscopy after periacetabular osteotomy (PAO). We conducted a retrospective study of prospective collected data to report the operative findings and outcomes of hip arthroscopy for recurrent pain following PAO for acetabular dysplasia. Demographic data, radiographic and arthroscopic findings were analysed. Pre- and post-operative patient reported outcome measures (PROMs) were collected. Of 184 PAO patients, 15 hips in 15 patients (8.2%) underwent post-PAO hip arthroscopy with 2 males and 13 females at a mean time of 3.9 ±2.9(0.3–10) years. Hip arthroscopy findings included labral tears (15 hips, 100%), chondral damage (11 hips, 73%) with one Beck 1, two Beck 2, four Beck 4, and four Beck 5 damage. There were 8 (53%) isolated labral debridement, 7 (47%) labral repair, and 2 (13%) adhesiolysis. A femoral osteochondroplasty was performed in 4 (27%) hips. Four hips (27%) were finally converted to total hip replacement at a mean delay of 2.3 ±1.4 (0.7–3.2) years post hip arthroscopy and these 4 patients were significantly older than those who were not (p=0.02). There were no significant differences in PROMs pre and post hip scope. 8.2% of patients undergoing a PAO required a hip scope for persistent hip pain. Common post-PAO hip arthroscopy findings included labral tears, chondral changes and femoroacetabular impingement. 27% of patients finally underwent conversion to total hip replacement at a mean follow-up of 4.5 years post hip scope

The purpose of this study is to audit the early clinical outcomes of a consecutive series of hip arthroscopies performed by one surgeon. Pre-operative WOMAC and Non Arthritic Hip scores were performed on the patients. All patients had MRI arthrograms. These were correlated with the clinical findings at the time of surgery. Postoperatively all complications were recorded and at one year post surgery the pre-operative scores were repeated. There are 37 consecutive cases. 15 males and 22 females. Average age 39 years. 76% of cases were accident related. The average length of symptoms pre-operatively was 21 months. There were 2 false negative MRI arthrograms and one false positive. The surgical treatment included debridement of 18 labral tears and 10 labral tears associated with an articular cartilage defect. The average traction time was 39 minutes. The complications were minor and all have resolved. There are many indications for hip arthroscopy however the outcomes of labral debridement are not clearly defined. This paper attempts to measure the success of arthroscopic labral debridement in a consecutive series of patients. The clinical symptoms and MRI arthrograms seem to have a good correlation with the arthroscopic findings. The short term results of this treatment are encouraging as is the low morbidity associated with the surgery itself

Purpose. Surgical dislocation is useful for assessing and treating proximal femoral hip deformities. Legg-Calv Perthes disease (LCPD) causes proximal femoral growth deformity, resulting in reduced femoral head-neck offset and femoracetabular incongruity. The purpose of this study was to demonstrate the efficacy and report the short-term results of surgical hip dislocation for the treatment of adolescents with healed LCPD. Method. This retrospective review included 29 adolescents [19 males and 10 females, age 17 (range nine-35)] with LCPD, who underwent surgical hip dislocation between January 2001 and December 2009. All subjects had a clear diagnosis of LCPD, pre and postoperative WOMAC scores and at least one year of clinical and radiographic follow up. In addition to surgical dislocation, all patients underwent femoral head-neck osteoplasty, 21 underwent relative femoral neck lengthening and trochanteric transfer, 12 underwent intertrochanteric osteotomy and seven had labral debridement. The average follow-up was three years from the time of surgical intervention. Results. Postoperative WOMAC scores improved globally for pain, stiffness and function (p<0.0001, p<0.0004 and p<0.0009 respectively). Eight patients required additional surgical procedures after surgical dislocation (one periacetabular osteotomy, one flexion intertrochanteric osteotomy, one arthrotomy, five arthroscopies with labral/cartilage debridement. Three patients underwent total hip arthroplasties during the follow-up period and were considered failures. Two of the 29 patients experienced superficial wound complications. Conclusion. Surgical dislocation is an effective technique for the treatment of proximal femur deformity associated with LCPD. In the short-term, patients experience improved symptoms and function from this procedure

This study presents the results of 60 consecutive hip arthroscopic procedures for the treatment of Acetabulo-Femoral Impingement. The procedures were performed by a single surgeon over a period of 36 months. The learning curve and the evolution of the current technique along with the clinical outcomes are discussed Additionally two new clinical signs of AFI are described, along with the correlation of radiological and arthroscopic findings. Sixty patients underwent hip arthroscopies. The procedures included labral debridement, labral repair, femoral and/or acetabular osteectomies. All patients underwent MRI examination and three-dimensional CT imaging to identify the impingement lesion. Follow up CT scanning was performed to assess the accuracy of the bony resection. Patients were reviewed at three months and subsequently at twelve monthly intervals. All patients participated in completing questionnaires. Post-operatively Modified Harris Hip score improved from 54 to 70, Non-Arthritic hip score improved from 58 to 75, SF12 score improved from 35 to 40. Three patients required a second procedure for further bony resection. One patient underwent a THR within 12 months. Two female patients suffered minor vaginal abrasions. Hip arthroscopy is a demanding procedure. Good clinical results are achieved only when the cause of impingement has been identified and treated

Arthroscopic labral debridement has become and accepted mode of treatment for acetabular labral tears (ALTs) and yet results in literature are few and conflicting especially with regards presence of cartilage damage and its influence on outcome. Aim of our prospective cohort study was to look at outcome of ALTs post arthroscopic limbectomy using validated questionnaires at average of 2 years. We also looked at the sensitivity of MR arthrography in detecting ALTs and cartilage damage. Consecutive cohort of 82 patients who were suspected of having ALTs by senior author had MR arthrography followed by hip arthroscopy. All patients filled out a WOMAC, NAHS scores before and after the operation. All patients who were followed up also filled an especially designed five-point functional questionnaire. At time of arthroscopy the presence of labral tear, its position in the acetabulum and degree of cartilage damage (based on Outerbridge classification) were recorded and correlated with Mra and patient outcome. Seventy six patients were found to have a labral tear, with majority of these lesions in the anterior or antero-superior quadrant. 59% of patients were shown to have an associated degree of cartilage damage. Although overall all patients showed significant improvement in their functional scores, those with high grade cartilage damage had poorer results in comparison. We also found that patients with older age are at higher risk of requiring a THJR in the first 2 years following arthroscopy. MRa proved to a sensitive tool at detecting labral pathology but not so at identifying presence of cartilage damage. Arthroscopic debridement of ALTs in patients presenting with mechanical hip symptoms provides good functional outcome, however it should be used with caution in those with high grade articular cartilage damage and older age group where there is higher likelihood of failure

Femoroacetabular impingement is defined as abnormal abutment between the femoral head, or the femoral head-neck junction, and the acetabulum. It is now established that FAI is a major etiological factor in the development of osteoarthritis (OA) of the hip. FAI is increasingly recognized as a cause of hip pain in young active individuals. Clinical features of FAI include pain in the groin but this may also be felt in the gluteal region, trochanteric region, or in the thigh. Symptoms most commonly begin as an intermittent discomfort, often during or following periods of repetitive hip motion e.g. running, walking, progressing to more constant and intense pain. Stiffness is common, with reductions in the range of hip flexion, and internal rotation in particular. Patients may also complain of clicking, popping, or snapping sensations in the affected hip. Operative treatment of FAI is principally aimed at removing cam lesions and increasing femoral head-neck offset (osteochondroplasty), and treating associated soft tissue lesions such as labral tears. Acetabular recession with labral reattachment is increasingly used for pincers lesions but great care is needed to ensure that the overall coverage of the hip is not reduced. As proficiency with arthroscopic techniques has improved, arthroscopic osteochondroplasty and labral debridement/repair has become increasingly popular for the treatment for FAI, with promising functional results in a range of studies. 150 consecutive patients who underwent hip arthroscopy for CAM type FAI with minimum 1 year follow up were prospectively studied. Patient satisfaction, non-arthritic hip score, re-operations and conversion to arthroplasty were analysed. There were a total of 90 males and 60 females. The average age group was 32 years (27 to 46 years). All patients underwent osteoplasty of the CAM lesion and stabilisation/ repair of the labral tears. A comparison of pre procedure non arthritic hip scores with the scores at latest follow up (minimum 1 year, maximum 3 years) showed a significant improvement (56 vs 86; p <0.01). There were 2 re operations for unresolved symptoms and 1 conversion to arthroplasty for continued pain associated with significant loss of articular cartilage. Perineal numbness was noted in 9 patients all of which resolved. No other major complications were noted in this series. Hip impingement symptoms resolved in all cases and a good (20 %) to excellent (75%) outcome was reported. Arthroscopic management of CAM type FAI is associated with subjective and objective improvement in hip function at early follow up. Longer follow-up is necessary to analyse the ability of this procedure to arrest progression of degenerative hip disease

Labral tears are increasingly recognized as a source of hip pain. These rarely occur in normal hips, but in individuals with subtle femoral deformities. Anterior impingement of the femoral neck on the acetabulum due to loss of anterior offset results in a labral tear in active individuals. Future surgeries should be directed at this anterior offset deficiency in order to completely alleviate pain and prevent further recurrences. To determine the incidence of anterior hip impingement and femoral pathology in patients undergoing hip arthroscopy for labral tear. Anterior labral tears occur in the presence of abnormal femoral anatomy, which results in anterior hip impingement. Future treatment of labral tears should include addressing the hip impingement. Labral tears of the hip are increasingly being recognized as a source of hip pain in young, active individuals. Athroscopic labral debridement is commonly associated with poor results. As a result, it is likely that other hip pathology is present at the time of labral tear. The purpose of this study was to determine the incidence of anterior hip impingement secondary to a pistol grip deformity in patients undergoing hip arthroscopy for labral tears. The charts and radiographs of forty consecutive patients who underwent hip arthroscopy were evaluated. X-rays were evaluated for presence of acetabular or femoral pathology. Femoral pathology was then graded based on degree of slip and posterior slip angle. All patients who underwent hip arthroscopy for labral tears had abnormal femoral or acetabular anatomy. In those patients without acetabular dysplasia, a pistol grip deformity of the femoral head was present in all cases. Evidence of anterior hip impingement with osteophyte formation was present in ninety-two percent. In these patients, hip arthroscopy results in complete resolution of mechanical symptoms, but hip pain was still present. Labral tears of the hip appear to be secondary to anterior femoral impingement. This is analogous to a rotator cuff tear in the presence of shoulder impingement. Anterior impingement of the femoral neck on the acatabulum due to an anterior femoral offset deformity results in both pain and labral tears. Future surgeries should be directed at this anterior offset deficiency in order to prevent recurrent injuries or residual pain

Purpose: This purpose of this study was to report 2 year outcomes following hip arthroscopy for the treatment of femoroacetabular impingement and chondrolabral dysfunction and determine factors associated with outcomes. Method: Between 3/2005 and 10/2005, 122 patients underwent hip arthroscopy by a single surgeon. Patients were included if they underwent arthroscopic treatment for FAI and chondrolabral dysfunction. All patients were prospectively enrolled in an IRB approved follow-up study. Ten patients refused to participate following enrollment. Results: At arthroscopy, for treatment of impingement, 23 patients underwent only osteoplasty for CAM impingement, 3 patients underwent only rim reduction for pincer impingement and 86 patients underwent osteoplasty and rim reduction for mixed type impingement. Ten patients underwent total hip arthroplasty at an average of 16 months (range: 8–26) after arthroscopy. Average follow-up was obtained on 90%(92/102) at 2.3 years (range: 2.0–2.9). The modified Harris Hip score(MHHS) improved from 57 to 84. HOS ADL improved from 69 to 88. HOS Sport improved from 41 to 69. NAHS improved from 64 to 82. All scores significantly improved (p< 0.05). The average patient satisfaction was 8.4 (range: 1–10). All patients returned to work (15% returned within 1 week, 53% returned in 1–5 weeks, 18% returned in 6–8 weeks, 13% returned in 2 to 6 months). Ten patients underwent THA at an average of 16 months. These patients were significantly older at arthroscopy (58 vs. 39;p=0.0001), had significantly less joint space at all 3 weight bearing surfaces (p=0.001), and patients with microfracture on both femoral head and acetabulum were more likely to undergo THA (p=0.001). The predictors of higher post-operative MHHS were preoperative modified Harris Hip score (p=0.018), joint space of 2mm or greater(p=0.005), and repair of labral pathology instead of debridement (p=0. 032)(r2=0.32;p=0.001). Patients with labral repair had higher MHHS compared to patients with labral debridement (87vs81). Independent predictors of patient satisfaction were postoperative MHHS(p=0.001) and joint space of 2mm or greater(p=0.040) (r2=0.67; p=0.001). Conclusion: Hip arthroscopy for femoroacetabular impingement accompanied by the prescribed rehabilitation led to return to excellent function and high patient satisfaction. Factors associated with higher outcomes included joint space of greater than 2mm, and labral repair for treatment of labral pathology

Introduction: There have been several recent studies outlining the role of femoroacetabular impingement (FAI) as a cause of early osteoarthritis in the non-dysplastic hip. The lesions can either be on the femoral side “cam” or acetabular “pincer”. The aim of surgical treatment of FAI is to improve the femoral head neck offset thereby improving joint clearance and preventing abutment of the femoral neck against the acetabulum. The classic treatment for FAI pioneered by Ganz involves dislocation of the femoral head through a trochanteric flip osteotomy. The procedure is extensive, technically difficult and not without complications. Hip arthroscopic debridement of FAI lesions offers similar results to open procedures allowing for full inspection of the joint and the treatment of any chondral lesion but with a quicker recovery time. It nonetheless has a very long learning curve and even in the most experienced hands the treatment of impingement lesions is complicated and technically challenging. The purpose of this cadaveric study was to assess the degree of exposure obtained using two different limited anterior approaches to the hip which would allow effective surgical treatment of cam and pincer FAI. Methods: We investigated two mini anterior approaches to the hip joint based on the Heuter and direct anterior approach to compare the parts of the acetabulum and femoral head exposed for the treatment of FAI in a total of 20 hips in 10 (5 male, 5 female) cadaveric specimens. Neurovascular structures were recorded in relation to the two approaches. The area of femoral head and acetabular rim exposed via each approach was documented and quantified. Results: We found that the two approaches were easy and reproducible. Both allowed exposure to the anterolateral aspect of the femoral head. The mean length of acetabular rim accessible via the Heuter approach was 1.9cm (1.1–2.4) and 2.2cm (1.2–3) using the direct anterior approach The area of acetabular rim accessible varied according to the approach (p< 0.001). We also found that the position of the anterior inferior iliac spine in relation to the acetabular rim also affected the area of acetabular rim exposed (p< 0.001). The most proximal nerve branch to sartorious was found 7.3cm (6.5–8.7cm) distal to the anterior inferior iliac spine. The most proximal nerve branch to rectus femoris was located 8.6cm (7–10) distal to the anterior inferior iliac spine and was consistently found to be distal to the nerve to sartorious. Discussion: Treating impingement of the hip through a direct open approach is not a novel idea. A recent report of failed arthroscopic labral debridement, describes treatment of the underlying bony impingement in some cases by a combination of hip arthroscopy followed by anterior arthrotomy. In summary cam and pincer impingement of the hip can be treated by either the direct anterior or Heuter approach. The choice of approach would be dictated after careful consideration as to which portion of the anterior acetabular rim required surgery, with more lateral acetabular lesions being favoured by the Heuter approach and more medial impingement sites by the anterior approach we have described

Aims

The primary aim of this study was to determine the ten-year outcome following surgical treatment for femoroacetabular impingement (FAI). We assessed whether the evolution of practice from open to arthroscopic techniques influenced outcomes and tested whether any patient, radiological, or surgical factors were associated with outcome.

Methods

Prospectively collected data of a consecutive single-surgeon cohort, operated for FAI between January 2005 and January 2015, were retrospectively studied. The cohort comprised 393 hips (365 patients; 71% male (n = 278)), with a mean age of 34.5 years (SD 10.0). Over the study period, techniques evolved from open surgical dislocation (n = 94) to a combined arthroscopy-Hueter technique (HA + Hueter; n = 61) to a pure arthroscopic technique (HA; n = 238). Outcome measures of interest included modes of failures, complications, reoperation, and patient-reported outcome measures (PROMs). Demographic, radiological, and surgical factors were tested for possible association with outcome.

Aims

Periacetabular osteotomy (PAO) is an established treatment for acetabular dysplasia. It has also been proposed as a treatment for patients with acetabular retroversion. By reviewing a large cohort, we aimed to test whether outcome is equivalent for both types of morphology and identify factors that influenced outcome.

Methods

A single-centre, retrospective cohort study was performed on patients with acetabular retroversion treated with PAO (n = 62 hips). Acetabular retroversion was diagnosed clinically and radiologically (presence of a crossover sign, posterior wall sign, lateral centre-edge angle (LCEA) between 20° and 35°). Outcomes were compared with a control group of patients undergoing PAO for dysplasia (LCEA < 20°; n = 86 hips). Femoral version was recorded. Patient-reported outcome measures (PROMs), complications, and reoperation rates were measured.

Outcomes following different types of surgical intervention for femoroacetabular impingement (FAI) are well reported individually but comparative data are deficient. The purpose of this study was to conduct a systematic review (SR) and meta-analysis to analyze the outcomes following surgical management of FAI by hip arthroscopy (HA), anterior mini open approach (AMO), and surgical hip dislocation (SHD). This SR was registered with PROSPERO. An electronic database search of PubMed, Medline, and EMBASE for English and German language articles over the last 20 years was carried out according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. We specifically analyzed and compared changes in patient-reported outcome measures (PROMs), α-angle, rate of complications, rate of revision, and conversion to total hip arthroplasty (THA). A total of 48 articles were included for final analysis with a total of 4,384 hips in 4,094 patients. All subgroups showed a significant correction in mean α angle postoperatively with a mean change of 28.8° (95% confidence interval (CI) 21 to 36.5; p < 0.01) after AMO, 21.1° (95% CI 15.1 to 27; p < 0.01) after SHD, and 20.5° (95% CI 16.1 to 24.8; p < 0.01) after HA. The AMO group showed a significantly higher increase in PROMs (3.7; 95% CI 3.2 to 4.2; p < 0.01) versus arthroscopy (2.5; 95% CI 2.3 to 2.8; p < 0.01) and SHD (2.4; 95% CI 1.5 to 3.3; p < 0.01). However, the rate of complications following AMO was significantly higher than HA and SHD. All three surgical approaches offered significant improvements in PROMs and radiological correction of cam deformities. All three groups showed similar rates of revision procedures but SHD had the highest rate of conversion to a THA. Revision rates were similar for all three revision procedures.

Purpose: Hip arthroscopy has been used at an increasing frequency over the last few years. Majority of patients undergoing hip arthroscopy are young and active individuals who seek definitive therapy for a painful hip condition and wish to avoid undergoing a hip replacement. Although relatively successful, complications following hip arthroscopy occur. This multi-institutional study presents a worrisome and previously unrecognized complication of hip arthroscopy, namely chondrolysis that lead to accelerated development of end-stage arthritis. Method: Using the computerized database in each institution, patients undergoing total hip arthroplasty between 1999–2008 who had received hip arthroscopy prior to arthroplasty were identified. 39 patients were identified to have undergone at least one hip arthroscopy on the affected hip prior to arthroplasty. There were 21 female and 18 male patients. The mean age of patients at the time of hip arthroscopy was 45.9 years. Data regarding demographics, comorbidities, preoperative diagnosis, number of previous procedures, and the details of the surgical procedure were compiled. Radiographs were evaluated. Results: The median time from arthroscopy to arthroplasty was 14.8 months (range 2.2 months to 7 years). Fourteen patients (35%) underwent THA within 12 months of a previous hip arthroscopy and in nine of these patients the indication for hip arthroscopy was labral tear debridement. These patients despite having none to minimal arthritis at the time of arthroscopy developed accelerated arthritis within a year that necessitated hip replacement. Conclusion: Hip arthroscopy can cause accelerated degenerative arthritis of the hip in some patients. We believe the subset of our patients who developed arthritis so early after hip arthroscopy may have suffered chondrolysis and/or chondral injury during the arthroscopy that resulted in progressive and aggressive arthritis of the hip within 12 months. Hip arthroscopy should be reserved for a select group of patients. All measures to minimize the possibility of chondrolysis and/or chondral injury should be exercised

Objectives

The aim of this study was to investigate the structural integrity of torn and non-torn human acetabular labral tissue.

Methods

A total of 47 human labral specimens were obtained from a biobank. These included 22 torn specimens and 25 control specimens from patients undergoing total hip arthroplasty with macroscopically normal labra. The specimens underwent dynamic shear analysis using a rheometer to measure storage modulus, as an indicator of structural integrity.
