Abstract

Introduction

Minimising postoperative complications and mortality in COVID-19 patients who were undergoing trauma and orthopaedic surgeries is an international priority. Aim was to develop a predictive nomogram for 30-day morbidity/mortality of COVID-19 infection in patients who underwent orthopaedic and trauma surgery during the coronavirus pandemic in the UK in 2020 compared to a similar period in 2019. Secondary objective was to compare between patients with positive PCR test and those with negative test.

Methods

Retrospective multi-center study including 50 hospitals. Patients with suspicion of SARS-CoV-2 infection who had underwent orthopaedic or trauma surgery for any indication during the 2020 pandemic were enrolled in the study (2525 patients). We analysed cases performed on orthopaedic and trauma operative lists in 2019 for comparison (4417). Multivariable Logistic Regression analysis was performed to assess the possible predictors of a fatal outcome. A nomogram was developed with the possible predictors and total point were calculated.

Abstract

Background

Healthcare workers have had to make rapid and drastic adjustments to their practice in response to the COVID-19 pandemic. This work describes the effect on their physical, mental, financial and family well-being and assesses the support provided by their institutions.

Methods

An online survey was distributed through medical organisations, social media platforms and collaborators to staff based in an operating theatre environment.

We have investigated whether the pattern of subchondral acetabular cyst formation reflects hip pathology and may provide a prognostic indicator for treatment.

A single surgeon series of sequential hip arthroscopies was reviewed to identify the most recent 200 cases undertaken on a previously un-operated joint with pre-operative plain radiographs and computed tomography or magnetic resonance scan available for review. Also, serial “non-arthritic hip scores” (NAHS) recorded pre-operatively, at 6 weeks and 3 months post-surgery. The acetabular Lateral Centre Edge Angle, the Acetabular Index, the FEAR index and the Kallgren and Lawrence grade were determined. All images were reviewed by two independent assessors and divided into four groups according to acetabular subchondral cyst distribution. NC - No cysts - 74 cases (mean: 40.1 years), SPC - a Single Peripheral Cyst – 58 cases (mean: 43.1 years), SDC - a Solitary Dome Cyst – 32 cases (mean: 33 years) and MC - Multiple Cysts – 36 cases (mean: 42.7 years).

No association was identified between gender and patient reported outcomes.

SDC patients were significantly younger than the other three groups (p <0.001). At three months after surgery, the average increase of the NAHS in the four groups was 25.3, 23.5, 4.2 and 4.9 respectively. Acetabular dysplasia was identified in 72% of the SDC group compared to 18%, 16% and 33% in NC, SPC and MC groups. Degenerative change was identified in 86% of the MC group compared to 18%, 40% and 41% of the NC, SPC and SDC groups.

The early patient reported outcome following hip arthroscopy was significantly affected by the location and number of acetabular subchondral cysts. A Solitary Dome Cyst is indicative of underling hip dysplasia. Multiple Cysts are indicative of degenerative disease. The majority of patients in these groups have poor three-month outcomes after hip arthroscopy.

Introduction

Total knee arthroplasty (TKA) designs evolve as evidence accumulates on natural and prosthetic knee function. TKA designs based upon a medially conforming tibiofemoral articulation seek to reproduce essential aspects of normal knee stability and have enjoyed good clinical success and high patient satisfaction for over two decades. Fluoroscopic kinematic studies on several medially conforming knee designs show extremely stable knee function, but very small ranges of tibial axial rotation compared to healthy knees. The GMK Sphere TKA is a recent evolution in medially-conforming TKA designs that adopts a sagittally unconstrained lateral tibiofemoral articulation to allow more natural tibial rotation. This study was conducted to quantify motions in knees with this prosthesis to address two questions:

Does the medially conforming GMK Sphere design provide an AP-stable articulation that provides for tibiofemoral translations that are comparable to, but not larger than, translations measured in natural knees?

Does the medially conforming GMK Sphere design provide sufficient rotatory laxity to allow tibiofemoral rotations comparable to, but not larger than, rotations measured in natural knees?

Materials and Methods

Fifteen patients (9 females), mean age 65 years and mean BMI of 30 ±3, consented to participate. Sixteen knees received the GMK Sphere TKA. Mean Oxford Knee Score (OKS) improved significantly from 19±7 to 40±3 six months post surgery (P< 0.0001). On the day of the study, the mean OKS, Knee Society Score, EQ5D and Heath status scores were 40, 87, 0.83 and 85 respectively. Mean ROM from active maximum extension till maximum supine flexion was 108°±8°.

Motions in 16 knees were observed using pulsed-fluoroscopy during a range of activities. Subjects were observed in maximum flexion kneeling and lunging positions, and in stepping up/down on a 22cm step. Model-image registration methods were used to quantify three-dimensional knee motions from digitized fluoroscopic images.

Introduction

The Metasul articulation was introduced in the early 1990's. It comprises a 28mm forged, cobalt-chrome head and a forged metal inlay, set into the polyethylene element of the acetabular component. During the 1990's and early 2000's anatomic sized MoM articulations were re-introduced for hip resurfacing and stemmed implant designs. Adverse tissue reactions to metal debris and corrosion products subsequently led to a decline in MoM usage.

Materials & Methods

Between January 1995 and December 1998, 133 hips underwent primary THR using the Metasul 28mm bearing. Patients were routinely sent an annual postal Oxford Hip Score (OHS) and radiographs were obtained from the second post-operative year. We have retrospectively reviewed the clinical and radiographic dataset following the 15 year follow-up time point.
