The present study investigates the effectiveness of platelet-rich plasma (PRP) gel without adjunct to induce cartilage regeneration in large osteochondral defects in a rabbit model. A bilateral osteochondral defect was created in the femoral trochlear groove of 14 New Zealand white rabbits. The right knees were filled with PRP gel and the contralateral knees remained untreated and served as control sides. Some animals were killed at week 3 and others at week 12 postoperatively. The joints were harvested and assessed by Magnetic Resonance Observation of Cartilage Repair Tissue (MOCART) MRI scoring system, and examined using the International Cartilage Repair Society (ICRS) macroscopic and ICRS histological scoring systems. Methods
We present two cases of metastatic lung cancer which occurred at the site of a previously united tibial fracture. Both patients were treated with a locked intramedullary nail. The patients presented with metastases at the site of their initial fracture approximately 16 and 13 months after injury respectively. We retrospectively examined the prevalence and
natural history of asymptomatic lumbar canal stenosis in patients treated
surgically for cervical compressive myelopathy in order to assess
the influence of latent lumbar canal stenosis on the recovery after
surgery. Of 214 patients who had undergone cervical laminoplasty
for cervical myelopathy, we identified 69 (32%) with myelographically
documented lumbar canal stenosis. Of these, 28 (13%) patients with
symptomatic lumbar canal stenosis underwent simultaneous cervical
and lumbar decompression. Of the remaining 41 (19%) patients with
asymptomatic lumbar canal stenosis who underwent only cervical surgery,
39 were followed up for ≥ 1 year (mean 4.9 years (1 to 12)) and
were included in the analysis (study group). Patients without myelographic
evidence of lumbar canal stenosis, who had been followed up for ≥ 1
year after the cervical surgery, served as controls (135 patients;
mean follow-up period 6.5 years (1 to 17)). Among the 39 patients
with asymptomatic lumbar canal stenosis, seven had lumbar-related
leg symptoms after the cervical surgery. Kaplan–Meier analysis showed that 89.6% (95% confidence interval
(CI) 75.3 to 96.0) and 76.7% (95% CI 53.7 to 90.3) of the patients
with asymptomatic lumbar canal stenosis were free from leg symptoms
for three and five years, respectively. There were no significant

The aim of the present study was to answer the question whether curve morphology and location have an influence on rigid conservative treatment in patients with adolescent idiopathic scoliosis (AIS). We retrospectively analyzed AIS in 127 patients with single and double curves who had been treated with a Chêneau brace and physiotherapeutic specific exercises (B-PSE). The inclusion criteria were the presence of structural major curves ≥ 20° and < 50° (Risser stage 0 to 2) at the time when B-PSE was initiated. The patients were divided into two groups according to the outcome of treatment: failure (curve progression to ≥ 45° or surgery) and success (curve progression < 45° and no surgery). Techniques for fixation of fractures of the lateral
malleolus have remained essentially unchanged since the 1960s, but
are associated with complication rates of up to 30%. The fibular
nail is an alternative method of fixation requiring a minimal incision
and tissue dissection, and has the potential to reduce the incidence
of complications. We reviewed the results of 105 patients with unstable fractures
of the ankle that were fixed between 2002 and 2010 using the Acumed
fibular nail. The mean age of the patients was 64.8 years (22 to
95), and 80 (76%) had significant systemic medical comorbidities.
Various different configurations of locking screw were assessed
over the study period as experience was gained with the device.
Nailing without the use of locking screws gave satisfactory stability
in only 66% of cases (4 of 6). Initial locking screw constructs
rendered between 91% (10 of 11) and 96% (23 of 24) of ankles stable.
Overall, seven patients had loss of fixation of the fracture and
there were five post-operative wound infections related to the distal
fibula. This lead to the development of the current technique with
a screw across the syndesmosis in addition to a distal locking screw.
In 21 patients treated with this technique there have been no significant
complications and only one superficial wound infection. Good fracture

The high prevalence of osteoarthritis (OA), as well as the current lack of disease-modifying drugs for OA, has provided a rationale for regenerative medicine as a possible treatment modality for OA treatment. In this editorial, the current status of regenerative medicine in OA including stem cells, exosomes, and genes is summarized along with the author’s perspectives. Despite a tremendous interest, so far there is very little evidence proving the efficacy of this modality for clinical application. As symptomatic relief is not sufficient to justify the high cost associated with regenerative medicine, definitive structural improvement that would last for years or decades and obviate or delay the need for joint arthroplasty is essential for regenerative medicine to retain a place among OA treatment methods. We evaluated the safety and efficacy of total
knee replacement in patients receiving continuous warfarin therapy. . We identified 24 consecutive patients receiving long-term warfarin
therapy who underwent total knee replacement between 2006 and 2008
and compared them with a group of age- and gender-matched patients
not on long-term anticoagulation. Primary observations were changes
in haemoglobin, transfusion rates and complications. Secondary observations
were fluctuations in the international normalised ratio (INR) and
post-operative range of movement. . There was no significant difference between the two groups in
pre- or post-operative haemoglobin, incidence of transfusion or
incidence of post-operative complications. There were no surgical
delays due to a high INR level. The mean change in INR during the
peri-operative phase was minimal (mean 0.4; . sd. 0.7). There
was no significant difference in the range of movement between the

The management of symptomatic osteochondral lesions of the talus (OLTs) can be challenging. The number of ways of treating these lesions has increased considerably during the last decade, with published studies often providing conflicting, low-level evidence. This paper aims to present an up-to-date concise overview of the best evidence for the surgical treatment of OLTs. Management options are reviewed based on the size of the lesion and include bone marrow stimulation, bone grafting options, drilling techniques, biological preparations, and resurfacing. Although many of these techniques have shown promising results, there remains little high level evidence, and further large scale prospective studies and systematic reviews will be required to identify the optimal form of treatment for these lesions.
