This study evaluated whether obese patients who
lost weight before their total joint replacement and kept it off post-operatively
were at lower risk of surgical site infection (SSI) and re-admission
compared with those who remained the same weight. We reviewed 444 patients who underwent a total hip replacement
and 937 with a total knee replacement who lost weight pre-operatively
and sustained their weight loss after surgery. After adjustments,
patients who lost weight before a total hip replacement and kept
it off post-operatively had a 3.77 (95% confidence interval (CI)
1.59 to 8.95) greater likelihood of deep SSIs and those who lost
weight before a total knee replacement had a 1.63 (95% CI 1.16 to
2.28) greater likelihood of re-admission compared with the reference
group. These findings raise questions about the safety of weight management

Systemic emboli released during total knee replacement have been implicated as a cause of peri-operative morbidity and neurological dysfunction. We undertook a prospective, double-blind, randomised study to compare the cardiac embolic load sustained during computer-assisted and conventional, intramedullary-aligned, total knee replacement, as measured by transoesophageal echocardiography. There were 26 consecutive procedures performed by a single surgeon at a single hospital. The embolic load was scored using the modified Mayo grading system for echogenic emboli. Fourteen patients undergoing computer-assisted total knee replacement had a mean embolic score of 4.89 (3 to 7) and 12 undergoing conventional total knee replacement had a mean embolic score of 6.15 (4 to 8) on release of the tourniquet. We examined the risk of thrombotic and major
bleeding events in patients undergoing total hip and knee replacement
(THR and TKR) treated with thromboprophylaxis, using nationwide
population-based databases. We identified 83 756 primary procedures
performed between 1997 and 2011. The outcomes were symptomatic venous thromboembolism
(VTE), myocardial infarction (MI), stroke, death and major bleeding
requiring hospitalisation within 90 days of surgery. A total of 1114 (1.3%) and 483 (0.6%) patients experienced VTE
and bleeding, respectively. The annual risk of VTE varied between
0.9% and 1.6%, and of bleeding between 0.4% and 0.8%. The risk of
VTE and bleeding was unchanged over a 15-year period. A total of
0.7% of patients died within 90 days, with a decrease from 1% in
1997 to 0.6% in 2011 (p <
0.001). A high level of comorbidity
and general anaesthesia were strong risk factors for both VTE and
bleeding, with no difference between THR and TKR patients. The risk
of both MI and stroke was 0.5%, which remained unchanged during
the study period. In this cohort study of patients undergoing THR and TKR patients
in routine clinical practice, approximately 3% experienced VTE,
MI, stroke or bleeding. These risks did not decline during the 15-year

In an initial randomised controlled trial (RCT)
we segregated 180 patients to one of two knee positions following total
knee replacement (TKR): six hours of knee flexion using either a
jig or knee extension. Outcome measures included post-operative
blood loss, fall in haemoglobin, blood transfusion requirements,
knee range of movement, limb swelling and functional scores. A second
RCT consisted of 420 TKR patients randomised to one of three post-operative
knee positions: flexion for three or six hours post-operatively,
or knee extension. Positioning of the knee in flexion for six hours immediately
after surgery significantly reduced blood loss (p = 0.002). There
were no significant differences in post-operative range of movement,
swelling, pain or outcome scores between the various knee positions
in either study. Post-operative knee flexion may offer a simple
and cost-effective way to reduce blood loss and transfusion requirements
following TKR. We also report a cautionary note regarding the potential risks
of prolonged knee flexion for more than six hours observed during
clinical practice in the intervening period between the two trials,
with 14 of 289 patients (4.7%) reporting lower limb sensory neuropathy

The outcome of high tibial osteotomy (HTO) deteriorates
with time, and additional procedures may be required. The aim of
this study was to compare the clinical and radiological outcomes
between unicompartmental knee replacement (UKR) and total knee replacement
(TKR) after HTO as well as after primary UKR. A total of 63 patients (63
knees) were studied retrospectively and divided into three groups:
UKR after HTO (group A; n = 22), TKR after HTO (group B; n = 18)
and primary UKR (group C; n = 22). The Oxford knee score (OKS),
Knee Society score (KSS), hip–knee–ankle angles, mechanical axis
and patellar height were evaluated pre- and post-operatively. At
a mean of 64 months (19 to 180) post-operatively the mean OKS was
43.8 (33 to 49), 43.3 (30 to 48) and 42.5 (29 to 48) for groups
A, B and C, respectively (p = 0.73). The mean KSS knee score was
88.8 (54 to 100), 88.11 (51 to 100) and 85.3 (45 to 100) for groups
A, B and C, respectively (p = 0.65), and the mean KSS function score
was 85.0 (50 to 100) in group A, 85.8 (20 to 100) in group B and
79.3 (50 to 100) in group C (p = 0.48). Radiologically the results
were comparable for all groups except for patellar height, with
a higher incidence of patella infra following a previous HTO (p

This is the second of a series of reviews of registries. This review looks specifically at worldwide registry data that have been collected on knee arthroplasty, what we have learned from their reports, and what the limitations are as to what we currently know.
The aim of this study was to review the early
outcome of the Femoro-Patella Vialla (FPV) joint replacement. A
total of 48 consecutive FPVs were implanted between December 2007
and June 2011. Case-note analysis was performed to evaluate the
indications, operative histology, operative findings, post-operative
complications and reasons for revision. The mean age of the patients
was 63.3 years (48.2 to 81.0) and the mean follow-up was
25.0 months (6.1 to 48.9). Revision was performed in seven (14.6%)
at a mean of 21.7 months, and there was one re-revision. Persistent
pain was observed in three further patients who remain unrevised.
The reasons for revision were pain due to progressive tibiofemoral
disease in five, inflammatory arthritis in one, and patellar fracture following
trauma in one. No failures were related to the implant or the technique.
Trochlear dysplasia was associated with a significantly lower rate
of revision (5.9% Focal patellofemoral osteoarthritis secondary to trochlear dysplasia
should be considered the best indication for patellofemoral replacement.
Standardised radiological imaging, with MRI to exclude overt tibiofemoral
disease should be part of the pre-operative assessment, especially
for the non-dysplastic knee. Cite this article:
Smart trials are total knee tibial trial liners
with load bearing and alignment sensors that will graphically show quantitative
compartment load-bearing forces and component track patterns. These
values will demonstrate asymmetrical ligament balancing and misalignments
with the medial retinaculum temporarily closed. Currently surgeons
use feel and visual estimation of imbalance to assess soft-tissue
balancing and tracking with the medial retinaculum open, which results
in lower medial compartment loads and a wider anteroposterior tibial
tracking pattern. The sensor trial will aid the total knee replacement
surgeon in performing soft-tissue balancing by providing quantitative
visual feedback of changes in forces while performing the releases
incrementally. Initial experience using a smart tibial trial is
presented.
The outcome after total hip replacement has improved
with the development of surgical techniques, better pain management
and the introduction of enhanced recovery pathways. These pathways
require a multidisciplinary team to manage pre-operative education,
multimodal pain control and accelerated rehabilitation. The current economic
climate and restricted budgets favour brief hospitalisation while
minimising costs. This has put considerable pressure on hospitals
to combine excellent results, early functional recovery and shorter
admissions. In this review we present an evidence-based summary of some common
interventions and methods, including pre-operative patient education,
pre-emptive analgesia, local infiltration analgesia, pre-operative
nutrition, the use of pulsed electromagnetic fields, peri-operative
rehabilitation, wound dressings, different surgical techniques, minimally
invasive surgery and fast-track joint replacement units. Cite this article:
Heterotopic ossification following joint replacement in the lower limb occurs in 3% to 90% of cases. Higher grades of heterotopic ossification can result in significant limitation of function and can negate the benefits of joint replacement. The understanding of the pathophysiology of this condition has improved in recent years. It would appear to be related to a combination of systemic and local factors, including over-expression of bone morphogenetic protein-4. There is currently little evidence to support the routine use of prophylaxis for heterotopic ossification in arthroplasty patients, but prophylaxis is recommended by some for high-risk patients. Radiotherapy given as one dose of 7 Gy to 8 Gy, either pre-operatively (<
four hours before) or post-operatively (within 72 hours of surgery), appears to be more effective than indometacin therapy (75 mg daily for six weeks). In cases of prophylaxis against recurrent heterotopic ossification following excision, recent work has suggested that a combination of radiotherapy and indometacin is effective. Advances in our understanding of this condition may permit the development of newer, safer treatment modalities.
Hip arthrodesis remains a viable surgical technique
in well selected patients, typically the young manual labourer with
isolated unilateral hip disease. Despite this, its popularity with
patients and surgeons has decreased due to the evolution of hip
replacement, and is seldom chosen by young adult patients today.
The surgeon is more likely to encounter a patient who requests conversion
to total hip replacement (THR). The most common indications are
a painful pseudarthrosis, back pain, ipsilateral knee pain or contralateral
hip pain. Occasionally the patient will request conversion because
of difficulty with activities of daily living, body image and perceived
cosmesis. The technique of conversion and a discussion of the results

The primary aim of this study was to develop
a patient-reported Activity &
Participation Questionnaire (the
OKS-APQ) to supplement the Oxford knee score, in order to assess
higher levels of activity and participation. The generation of items
for the questionnaire involved interviews with 26 patients. Psychometric
analysis (exploratory and confirmatory factor analysis and Rasch
analysis) guided the reduction of items and the generation of a
scale within a prospective study of 122 relatively young patients
(mean age 61.5 years (42 to 71)) prior to knee replacement. A total
of 99, completed pre-operative and six month post-operative assessments
(new items, OKS, Short-Form 36 and American Knee Society Score). The eight-item OKS-APQ scale is unidimensional, reliable (Cronbach’s
alpha 0.85; intraclass correlation coefficient (ICC) 0.79; or 0.92
when one outlier was excluded), valid (r >
0.5 with related scales)
and responsive (effect size 4.16). We recommend that it is used with the OKS with adults of all
ages when further detail regarding the levels of activity and participation

Intra-operative, peri-articular injection of
local anaesthesia is an increasingly popular way of controlling
pain following total knee replacement. The evidence from this study suggests that it is safe to use
peri-articular injection in combination with auto-transfusion of
blood from peri-articular drains during knee replacement surgery.
Previous studies from single centres or single-surgeon
series report good early and mid-term results for high tibial osteotomy
(HTO) in the treatment of osteoarthritis of the knee. However, the
survivorship of HTO at a national level is unknown. This registry-based
study included 3195 high HTOs performed between 1987 and 2008. Kaplan-Meier analysis
revealed an overall survivorship of 89% (95% confidence interval
(CI) 88 to 90) at five years and 73% (95% CI 72 to 75) at ten years,
when conversion to total knee replacement was taken as the endpoint.
Females and patients aged >
50 years had worse survivorship than
males or patients aged ≤ 50 years (hazard ratio (HR) 1.26 (95% CI
1.11 to 1.43) and HR 1.41 (95% CI 1.23 to 1.64), respectively).
The survivorship of HTOs performed between 1998 to 2008 was worse
than for those performed between 1987 and 1997.
Structural allografts may be used to manage uncontained
bone defects in revision total knee replacement (TKR). However,
the availability of cadaver grafts is limited in some areas of Asia.
The aim of this study was to evaluate the mid-term outcome of the
use of femoral head allografts for the reconstruction of uncontained
defects in revision TKR, focusing on complications related to the
graft. We retrospectively reviewed 28 patients (30 TKRs) with Anderson
Orthopaedic Research Institute (AORI) type 3 bone defects, who underwent
revision using femoral head allografts and stemmed components. The
mean number of femoral heads used was 1.7 (1 to 3). The allograft–host
junctions were packed with cancellous autograft. At a mean follow-up of 76 months (38 to 136) the mean American
Knee Society knee score improved from 37.2 (17 to 60) pre-operatively
to 90 (83 to 100) (p <
0.001). The mean function score improved
from 26.5 (0 to 50) pre-operatively to 81 (60 to 100) (p <
0.001).
All the grafts healed to the host bone. The mean time to healing
of the graft was 6.6 months
(4 to 16). There have been no complications of collapse of the graft,
nonunion, infection or implant loosening. No revision surgery was
required. The use of femoral head allografts in conjunction with a stemmed
component and autogenous bone graft in revision TKR in patients
with uncontained bone defects resulted in a high rate of healing
of the graft with minimal complications and a satisfactory outcome.

Ensuring correct rotation of the femoral component
is a challenging aspect of patellofemoral replacement surgery. Rotation
equal to the epicondylar axis or marginally more external rotation
is acceptable. Internal rotation is associated with poor outcomes.
This paper comprises two studies evaluating the use of the medial
malleolus as a landmark to guide rotation. We used 100 lower-leg anteroposterior radiographs to evaluate
the reliability of the medial malleolus as a landmark. Assessment
was made of the angle between the tibial shaft and a line from the
intramedullary rod entry site to the medial malleolus. The femoral
cut was made in ten cadaver knees using the inferior tip of the
medial malleolus as a landmark for rotation. Rotation of the cut
relative to the anatomical epicondylar axis was assessed using CT.
The study of radiographs found the position of the medial malleolus
relative to the tibial axis is consistent. Using the inferior tip
of the medial malleolus in the cadaver study produced a mean external
rotation of 1.6° (0.1° to 3.7°) from the anatomical epicondylar
axis. Using the inferior tip of the medial malleolus to guide the
femoral cutting jig avoids internal rotation and introduces an acceptable
amount of external rotation of the femoral component.
We evaluated the impact of pre-coating the tibial
component with polymethylmethacrylate (PMMA) on implant survival
in a cohort of 16 548 primary NexGen total knee replacements (TKRs)
in 14 113 patients. In 13 835 TKRs a pre-coated tray was used while
in 2713 TKRs the non-pre-coated version of the same tray was used.
All the TKRs were performed between 2001 and 2009 and were cemented.
TKRs implanted with a pre-coated tibial component had a lower cumulative
survival than those with a non-pre-coated tibial component (p =
0.01). After adjusting for diagnosis, age, gender, body mass index,
American Society of Anesthesiologists grade, femoral coupling design, surgeon
volume and hospital volume, pre-coating was an independent risk
factor for all-cause aseptic revision (hazard ratio 2.75, p = 0.006).
Revision for aseptic loosening was uncommon for both pre-coated
and non-pre-coated trays (rates of 0.12% and 0%, respectively).
Pre-coating with PMMA does not appear to be protective of revision
for this tibial tray design at short-term follow-up. Cite this article:
Improvements in the surgical technique of total
knee replacement (TKR) are continually being sought. There has recently
been interest in three-dimensional (3D) pre-operative planning using
magnetic resonance imaging (MRI) and CT. The 3D images are increasingly
used for the production of patient-specific models, surgical guides
and custom-made implants for TKR. The users of patient-specific instrumentation (PSI) claim that
they allow the optimum balance of technology and conventional surgery
by reducing the complexity of conventional alignment and sizing
tools. In this way the advantages of accuracy and precision claimed
by computer navigation techniques are achieved without the disadvantages
of additional intra-operative inventory, new skills or surgical
time. This review describes the terminology used in this area and debates
the advantages and disadvantages of PSI.
