The aim of this study was to analyse the gait
pattern, muscle force and functional outcome of patients who had undergone
replacement of the proximal tibia for tumour and alloplastic reconstruction
of the extensor mechanism using the patellar-loop technique. Between February 1998 and December 2009, we carried out wide
local excision of a primary sarcoma of the proximal tibia, proximal
tibial replacement and reconstruction of the extensor mechanism
using the patellar-loop technique in 18 patients. Of these, nine
were available for evaluation after a mean of 11.6 years (0.5 to
21.6). The strength of the knee extensors was measured using an
Isobex machine and gait analysis was undertaken in our gait assessment
laboratory. Functional outcome was assessed using the American Knee
Society (AKS) and Musculoskeletal Tumor Society (MSTS) scores. The gait pattern of the patients differed in ground contact time,
flexion heel strike, maximal flexion loading response and total
sagittal plane excursion. The mean maximum active flexion was 91°
(30° to 110°). The overall mean extensor lag was 1° (0° to 5°).
The mean extensor muscle strength was 25.8% (8.3% to 90.3%) of that
in the non-operated leg (p <
0.001). The mean functional scores
were 68.7% (43.4% to 83.3%) (MSTS) and 71.1 (30 to 90) (AKS functional
score). In summary, the results show that reconstruction of the extensor
mechanism using this technique gives good biomechanical and functional
results. The patients’ gait pattern is close to normal, except for
a somewhat stiff knee gait pattern. The strength of the extensor

We measured the isokinetic strength of abduction, adduction, internal rotation, and external rotation in ten patients with full-thickness tears of the supraspinatus and ten with partial-thickness tears. The measurements were repeated after intra-articular or intrabursal injection of local anaesthetic. Pain blocks produced significant increases in strength in both full and partial-thickness tears. After the block, the strength in full-thickness tears compared with the opposite side was 67% to 81% in abduction and 67% to 78% in external rotation, both significantly smaller than those on the uninvolved side (p = 0.0064, p = 0.0170). In partial-thickness tears the strength after the block ranged from 82% to 111%, with no significant differences between the involved and uninvolved sides. The decreases in strength of 19% to 33% in abduction and 22% to 33% in external rotation after full-thickness tears appear to represent the contribution of supraspinatus to the strength of the shoulder.
Closed tenotomy was used to treat triggering of the fingers and thumb in 54 patients. In 56 digits the method was successful; in seven it was a simple matter to proceed to open tenotomy. With experience, the closed procedure can be completed within minutes without risk of damaging the digital nerves.
1 . Twenty-two cases of transplantation of the tibialis anterior for relapsed club-foot have been reviewed. Results were good in sixteen cases, fair in two and bad in four. 2. The operation and routine treatment are described. 3. The reasons for the four failures are discussed. 4. It is concluded that the operation is well worth while, and will often avoid the need for bony stabilisation at a later date.
A method of treatment of Volkmann's ischaemic contracture is described which retains wrist movement and restores reasonably good function to the hand in suitable cases.
A carefully planned operation may be expected to check increasing deformity without doing harm, and to make subsequent bony stabilisation easier. In favourable cases it may be possible to restore muscle balance and stability, making further surgery unnecessary. A longer follow-up is necessary to determine to what extent this ideal can be achieved.
Cryopreserved patellar tendon allografts are often recommended for reconstruction of anterior cruciate ligaments (ACLs) because living donor fibroblasts are thought to promote repair. Animal studies, however, indicate that ligaments regenerate from recipient rather than donor cells. If applicable to man, these observations suggest that allograft cell viability is unimportant. We therefore used short tandem repeat analysis with polymerase chain reaction (PCR) amplification to determine the source of cells in nine human ACLs reconstructed with cryopreserved patellar tendon allografts. PCR amplification of donor and recipient DNA obtained before operation and DNA from the graft obtained two to ten months after transplantation revealed the genotype of cells and showed only recipient cells in the graft area. Rather than preserve the viability of donor cells, a technique is required which will facilitate the introduction of recipient cells into patellar tendon allografts.
The use of platelet-rich plasma (PRP) as an adjuvant
to tissue repair is gaining favour in orthopaedic surgery. Tunnel widening
after anterior cruciate ligament (ACL) reconstruction is a recognised
phenomenon that could compromise revision surgery. The purpose of
this study was to determine whether PRP might prevent tunnel widening
in ACL reconstruction. Patients undergoing ACL reconstruction using a hamstring graft
were randomly allocated either to have PRP introduced into the tunnels
peri-operatively or not. CT scanning of the knees was carried out
on the day after surgery and at three months post-operatively and
the width of the tunnels was measured. Patients were also evaluated
clinically at three months, when laxity was also measured. Each group comprised 25 patients, and at three months post-operatively
all were pain-free with stable knees, a negative Lachman test and
a good range of movement. Arthrometric results had improved significantly
in both groups (p <
0.001). Despite slightly less tunnel widening
in the PRP group, there was no significant difference between the
groups at the femoral opening or the mid-tunnel (p = 0.370 and p
= 0.363, respectively) nor at the tibial opening or mid-tunnel (p
= 0.333 and p = 0.177, respectively). We conclude that PRP has no significant effect in preventing

Aims. The Chopart joint complex is a joint between the midfoot and hindfoot. The static and dynamic support system of the joint is critical for maintaining the medial longitudinal arch of the foot. Any dysfunction leads to progressive collapsing flatfoot deformity (PCFD). Often, the tibialis posterior is the primary cause; however, contrary views have also been expressed. The present investigation intends to explore the comprehensive anatomy of the support system of the Chopart joint complex to gain insight into the cause of PCFD. Methods. The study was conducted on 40 adult embalmed cadaveric lower limbs. Chopart joint complexes were dissected, and the structures supporting the joint inferiorly were observed and noted. Results. The articulating bones exhibit features like a cuboid shelf and navicular beak, which appear to offer inferior support to the joint.
