We report the effect of introducing a dedicated
Ponseti service on the five-year treatment outcomes of children
with idiopathic clubfoot. Between 2002 and 2004, 100 feet (66 children; 50 boys and 16
girls) were treated in a general paediatric orthopaedic clinic.
Of these, 96 feet (96%) responded to initial casting, 85 requiring
a tenotomy of the tendo-Achillis. Recurrent deformity occurred in
38 feet and was successfully treated in 22 by repeat casting and/or
tenotomy and/or transfer of the tendon of tibialis anterior, The
remaining 16 required an extensive surgical release. Between 2005 and 2006, 72 feet (53 children; 33 boys and 20 girls)
were treated in a dedicated multidisciplinary Ponseti clinic. All
responded to initial casting: 60 feet (83.3%) required a tenotomy
of the tendo-Achillis. Recurrent deformity developed in 14, 11 of
which were successfully treated by repeat casting and/or tenotomy
and/or transfer of the tendon of tibialis anterior. The other three
required an extensive surgical release. Statistical analysis showed that children treated in the dedicated
Ponseti clinic had a lower rate of recurrence (p = 0.068) and a
lower rate of surgical release (p = 0.01) than those treated in
the general clinic. This study shows that a dedicated Ponseti clinic, run by a well-trained
multidisciplinary team, can improve the outcome of idiopathic clubfoot

We present a review of litigation claims relating
to foot and ankle surgery in the NHS in England during the 17-year period
between 1995 and 2012. A freedom of information request was made to obtain data from
the NHS litigation authority (NHSLA) relating to orthopaedic claims,
and the foot and ankle claims were reviewed. During this period of time, a total of 10 273 orthopaedic claims
were made, of which 1294 (12.6%) were related to the foot and ankle.
1036 were closed, which comprised of 1104 specific complaints. Analysis
was performed using the complaints as the denominator. The cost
of settling these claims was more than £36 million. There were 372 complaints (33.7%) involving the ankle, of which
273 (73.4%) were related to trauma. Conditions affecting the first
ray accounted for 236 (21.4%), of which 232 (98.3%) concerned elective
practice. Overall, claims due to diagnostic errors accounted for
210 (19.0%) complaints, 208 (18.8%) from alleged incompetent surgery
and 149 (13.5%) from alleged mismanagement. Our findings show that the incorrect, delayed or missed diagnosis
of conditions affecting the foot and ankle is a key area for improvement,

The aim of this prospective multicentre study
was to report the patient satisfaction after total knee replacement (TKR),
undertaken with the aid of intra-operative sensors, and to compare
these results with previous studies. A total of 135 patients undergoing
TKR were included in the study. The soft-tissue balance of each
TKR was quantified intra-operatively by the sensor, and 18 (13%)
were found to be unbalanced. A total of 113 patients (96.7%) in
the balanced group and 15 (82.1%) in the unbalanced group were satisfied
or very satisfied one year post-operatively (p = 0.043). A review of the literature identified no previous study with
a mean level of satisfaction that was greater than the reported
level of satisfaction of the balanced TKR group in this study. Ensuring
soft-tissue balance by using intra-operative sensors during TKR

Instability is the reason for revision of a primary
total knee replacement (TKR) in 20% of patients. To date, the diagnosis
of instability has been based on the patient’s symptoms and a subjective
clinical assessment. We assessed whether a measured standardised
forced leg extension could be used to quantify instability. A total of 25 patients (11 male/14 female, mean age 70 years;
49 to 85) who were to undergo a revision TKR for instability of
a primary implant were assessed with a Nottingham rig pre-operatively
and then at six and 26 weeks post-operatively. Output was quantified
(in revolutions per minute (rpm)) by accelerating a stationary flywheel.
A control group of 183 patients (71 male/112 female, mean age 69
years) who had undergone primary TKR were evaluated for comparison. Pre-operatively, all 25 patients with instability exhibited a
distinctive pattern of reduction in ‘mid-push’ speed. The mean reduction

Down’s syndrome is associated with a number of
musculoskeletal abnormalities, some of which predispose patients
to early symptomatic arthritis of the hip. The purpose of the present
study was to review the general and hip-specific factors potentially
compromising total hip replacement (THR) in patients with Down’s
syndrome, as well as to summarise both the surgical techniques that
may anticipate the potential adverse impact of these factors and
the clinical results reported to date. A search of the literature
was performed, and the findings further informed by the authors’
clinical experience, as well as that of the hip replacement in Down
Syndrome study group. The general factors identified include a high
incidence of ligamentous laxity, as well as associated muscle hypotonia
and gait abnormalities. Hip-specific factors include: a high incidence
of hip dysplasia, as well as a number of other acetabular, femoral
and combined femoroacetabular anatomical variations. Four studies
encompassing 42 hips, which reported the clinical outcomes of THR
in patients with Down’s syndrome, were identified. All patients
were successfully treated with standard acetabular and femoral components.
The use of supplementary acetabular screw fixation to enhance component
stability was frequently reported. The use of constrained liners
to treat intra-operative instability occurred in eight hips. Survival
rates of between 81% and 100% at a mean follow-up of 105 months
(6 to 292) are encouraging. Overall, while THR in patients with
Down’s syndrome does present some unique challenges, the overall
clinical results are good, providing these patients with reliable

We report the five year outcomes of a two-stage
approach for infected total hip replacement. This is a single-surgeon
experience at a tertiary centre where the more straightforward cases
are treated using single-stage exchange. This study highlights the
vital role of the multidisciplinary team in managing these cases. A total of 125 patients (51 male, 74 female) with a mean age
of 68 years (42 to 78) were reviewed prospectively. Functional status
was assessed using the Harris hip score (HHS). The mean HHS improved
from 38 (6 to 78.5) pre-operatively to 81.2 (33 to 98) post-operatively.
Staphylococcus species were isolated in 85 patients (68%). The rate of control of infection was 96% at five years. In all,
19 patients died during the period of the study. This represented
a one year mortality of 0.8% and an overall mortality of 15.2% at
five years. No patients were lost to follow-up. We report excellent control of infection in a series of complex
patients and infections using a two-stage revision protocol supported
by a multidisciplinary approach. The reason for the high rate of

Fractures of the forearm (radius or ulna or both)
in children have traditionally been immobilised in plaster of Paris (POP)
but synthetic cast materials are becoming more popular. There have
been no randomised studies comparing the efficacy of these two materials.
The aim of this study was to investigate which cast material is
superior for the management of these fractures. We undertook a single-centre
prospective randomised trial involving 199 patients with acute fractures
of the forearm requiring general anaesthesia for reduction. Patients
were randomised by sealed envelope into either a POP or synthetic
group and then underwent routine closed reduction and immobilisation
in a cast. The patients were reviewed at one and six weeks. A satisfaction
questionnaire was completed following the removal of the cast. All
clinical complications were recorded and the cast indices were calculated.
There was an increase in complications in the POP group. These complications
included soft areas of POP requiring revision and loss of reduction
with some requiring re-manipulation. There was an increased mean
padding index in the fractures that lost reduction. Synthetic casts
were preferred by the patients. This study indicates that the clinical outcomes and patient satisfaction
are superior using synthetic casts with no reduction in safety. Cite this article:

Clinicians are often asked by patients, “When
can I drive again?” after lower limb injury or surgery. This question
is difficult to answer in the absence of any guidelines. This review
aims to collate the currently available evidence and discuss the
factors that influence the decision to allow a patient to return
to driving. There is currently insufficient evidence for any authoritative
body to determine fitness to drive. The lack of guidance could result
in patients being withheld from driving for longer than is necessary,
or returning to driving while still unsafe. Cite this article:
Previous studies have identified clinical and
demographic risk factors for recurrence in the treatment of idiopathic clubfoot
(congenital talipes equinovarus). Evertor muscle activity is not
usually considered amongst them. This study aimed to evaluate whether
recurrence could be predicted by demographic, clinical and gait
parameters. From a series of 103 children with clubfeet, 67 had
completed a follow-up of two years: 41 male and 26 female, 38 with idiopathic
and 29 with non-idiopathic deformities. The mean age was 3.2 years
(2.1 to 6.3). Primary correction was obtained in all 38 children
(100%) with an idiopathic deformity, and in 26 of 29 patients (90%)
with a non-idiopathic deformity. Overall, 60 children (90%) complied
with the abduction brace regime. At a mean follow-up of 31.4 months (24
to 62), recurrence was noted in six children (15.8%) in the idiopathic
and 14 children (48.3%) in the non-idiopathic group. Significant
correlation was found between poor evertor activity and recurrence
in both groups. No statistically significant relationship was found
between the rate of recurrence and the severity of the initial deformity,
the age at the time of treatment, the number of casts required or
the compliance with the brace. After correction of idiopathic and non-idiopathic clubfoot using
the Ponseti method, only poor evertor muscle activity was statistically
associated with recurrence. The identification of risk factors for
recurrent deformity allows clinicians to anticipate problems and
advocate early additional treatment to improve muscle balance around
