Tibiofemoral alignment is important to determine the rate of
progression of osteoarthritis and implant survival after total knee
arthroplasty (TKA). Normally, surgeons aim for neutral tibiofemoral
alignment following TKA, but this has been questioned in recent
years. The aim of this study was to evaluate whether varus or valgus
alignment indeed leads to increased medial or lateral tibiofemoral
forces during static and dynamic weight-bearing activities. Tibiofemoral contact forces and moments were measured in nine
patients with instrumented knee implants. Medial force ratios were
analysed during nine daily activities, including activities with
single-limb support (e.g. walking) and double-limb support (e.g.

It may not be possible to undertake revision total hip arthroplasty
(THA) in the presence of massive loss of acetabular bone stock using
standard cementless hemispherical acetabular components and metal
augments, as satisfactory stability cannot always be achieved. We
aimed to study the outcome using a reconstruction cage and a porous
metal augment in these patients. A total of 22 acetabular revisions in 19 patients were performed
using a combination of a reconstruction cage and porous metal augments.
The augments were used in place of structural allografts. The mean
age of the patients at the time of surgery was 70 years (27 to 85)
and the mean follow-up was 39 months (27 to 58). The mean number
of previous THAs was 1.9 (1 to 3). All patients had segmental defects
involving more than 50% of the acetabulum and seven hips had an

Pelvic reconstruction after the resection of a tumour around
the acetabulum is a challenging procedure due to the complex anatomy
and biomechanics. Several pelvic endoprostheses have been introduced,
but the rates of complication remain high. Our aim was to review
the use of a stemmed acetabular pedestal cup in the management of
these patients. The study involved 48 patients who underwent periacetabular reconstruction
using a stemmed pedestal cup (Schoellner cup; Zimmer Biomet Inc.,
Warsaw, Indiana) between 2000 and 2013. The indications for treatment included
a primary bone tumour in 27 patients and metastatic disease in 21
patients. The mean age of the patients at the time of surgery was

Hindfoot arthrodesis with retrograde intramedullary nailing has
been described as a surgical strategy to reconstruct deformities
of the ankle and hindfoot in patients with Charcot arthropathy.
This study presents case series of Charcot arthropathy patients
treated with two different retrograde intramedullary straight compression
nails in order to reconstruct the hindfoot and assess the results
over a mid-term follow-up. We performed a retrospective analysis of 18 consecutive patients
and 19 operated feet with Charcot arthropathy who underwent a hindfoot
arthrodesis using a retrograde intramedullary compression nail.
Patients were ten men and eight women with a mean age of 63.43 years
(38.5 to 79.8). We report the rate of limb salvage, complications requiring
additional surgery, and fusion rate in both groups. The mean duration

Until now, there has been no consensus as to whether stemmed
acetabular components are appropriate for use in patients undergoing
revision total hip arthroplasty (THA) who have major acetabular
defects or pelvic discontinuity. We wished to address this deficiency
in the literature. We carried out a retrospective study of 35 patients (six men
and 29 women) with a mean age of 68 years (37 to 87), with major
acetabular defects who underwent revision THA between 2000 and 2012.Aim

The objective of this study was to assess the association between whole body sagittal balance and risk of falls in elderly patients who have sought treatment for back pain. Balanced spinal sagittal alignment is known to be important for the prevention of falls. However, spinal sagittal imbalance can be markedly compensated by the lower extremities, and whole body sagittal balance including the lower extremities should be assessed to evaluate actual imbalances related to falls. Patients over 70 years old who visited an outpatient clinic for back pain treatment and underwent a standing whole-body radiograph were enrolled. Falls were prospectively assessed for 12 months using a monthly fall diary, and patients were divided into fallers and non-fallers according to the history of falls. Deformity of the proximal femur in fibrous dysplasia
leads to deviation of the mechanical axis of the hip, which may lead
to the development of secondary osteoarthritis (OA). This study
investigated the prevalence and predisposing factors for the development
of OA in patients with fibrous dysplasia of the proximal femur.
We reviewed the records of 209 patients from our institutional database
with fibrous dysplasia of the proximal femur, investigating possible predisposing
factors including patient demographics, the extent of the coxa vara
deformity, the presence of peri-articular disease, and the overall
burden of skeletal disease. Of the 209 patients, 24 (12%) had radiological
evidence of OA in the ipsilateral hip. The prevalence was significantly
higher in patients with polyostotic fibrous dysplasia compared with
those with monostotic disease (p <
0.001). In a subgroup analysis of
patients with polyostotic disease, the extent of deformity (quantified
using the neck–shaft angle), and the presence of peri-articular
disease (whether in the head of the femur or the acetabulum) were
significant predictors of osteoarthritis (neck–shaft angle likelihood
ratio (LR) = 0.847 per 1° increase, p = 0.004; presence of lesion
in the head of the femur LR = 9.947, p = 0.027; presence of lesion
in the acetabulum LR = 11.231, p = 0.014). Our data suggest that patients with polyostotic fibrous dysplasia
have a high risk of developing secondary OA of the hips. This risk
is higher in patients with peri-articular disease, and those with

The aim of this study was to compare early functional and health
related quality of life outcomes (HRQoL) in patients who have undergone
total hip arthroplasty (THA) using a bone conserving short stem
femoral component and those in whom a conventional length uncemented
component was used. Outcome was assessed using a validated performance
based outcome instrument as well as patient reported outcome measures
(PROMs). We prospectively analysed 33 patients whose THA involved a contemporary
proximally porous coated tapered short stem femoral component and
53 patients with a standard conventional femoral component, at a
minimum follow-up of two years. The mean follow-up was 31.4 months
(24 to 39). Patients with poor proximal femoral bone quality were
excluded. The mean age of the patients was 66.6 years (59 to 77)
and the mean body mass index was 30.2 kg/m2 (24.1 to
41.0). Outcome was assessed using the Oxford Hip Score (OHS) and
the University College Hospital (UCH) hip score which is a validated
performance based instrument. HRQoL was assessed using the EuroQol
5D (EQ-5D).Aims

Periprosthetic femoral fracture (PFF) is a potentially
devastating complication after total hip arthroplasty, with historically
high rates of complication and failure because of the technical
challenges of surgery, as well as the prevalence of advanced age
and comorbidity in the patients at risk. This study describes the short-term outcome after revision arthroplasty
using a modular, titanium, tapered, conical stem for PFF in a series
of 38 fractures in 37 patients. The mean age of the cohort was 77 years (47 to 96). A total of
27 patients had an American Society of Anesthesiologists grade of
at least 3. At a mean follow-up of 35 months (4 to 66) the mean
Oxford Hip Score (OHS) was 35 (15 to 48) and comorbidity was significantly
associated with a poorer OHS. All fractures united and no stem needed
to be revised. Three hips in three patients required further surgery
for infection, recurrent PFF and recurrent dislocation and three
other patients required closed manipulation for a single dislocation.
One stem subsided more than 5 mm but then stabilised and required
no further intervention. In this series, a modular, tapered, conical stem provided a versatile

Our aim was to examine the clinical and radiographic outcomes
in 257 consecutive Oxford unicompartmental knee arthroplasties (OUKAs)
(238 patients), five years post-operatively. A retrospective evaluation was undertaken of patients treated
between April 2008 and October 2010 in a regional centre by two
non-designing surgeons with no previous experience of UKAs. The
Oxford Knee Scores (OKSs) were recorded and fluoroscopically aligned
radiographs were assessed post-operatively at one and five years.Aims

The treatment of bone loss in revision total
knee arthroplasty has evolved over the past decade. While the management
of small to moderate sized defects has demonstrated good results
with a variety of traditional techniques (cement and screws, small
metal augments, impaction bone grafting or modular stems), the treatment of
severe defects continues to be problematic. The use of a structural
allograft has declined in recent years due to an increased failure
rate with long-term follow-up and with the introduction of highly
porous metal augments that emphasise biological metaphyseal fixation.
Recently published mid-term results on the use of tantalum cones
in patients with severe bone loss has reaffirmed the success of

An uncemented hemispherical acetabular component
is the mainstay of acetabular revision and gives excellent long-term
results. Occasionally, the degree of acetabular bone loss means that a
hemispherical component will be unstable when sited in the correct
anatomical location or there is minimal bleeding host bone left
for biological fixation. On these occasions an alternative method
of reconstruction has to be used. A major column structural allograft has been shown to restore
the deficient bone stock to some degree, but it needs to be off-loaded
with a reconstruction cage to prevent collapse of the graft. The
use of porous metal augments is a promising method of overcoming
some of the problems associated with structural allograft. If the defect
is large, the augment needs to be protected by a cage to allow ingrowth
to occur. Cup-cage reconstruction is an effective method of treating
chronic pelvic discontinuity and large contained or uncontained
bone defects. This paper presents the indications, surgical techniques and
outcomes of various methods which use acetabular reconstruction

This review summarises the technique of impaction
grafting with mesh augmentation for the treatment of uncontained
acetabular defects in revision hip arthroplasty. The ideal acetabular revision should restore bone stock, use
a small socket in the near-anatomic position, and provide durable
fixation. Impaction bone grafting, which has been in use for over
40 years, offers the ability to achieve these goals in uncontained
defects. The precepts of modern, revision impaction grafting are
that the segmental or cavitary defects must be supported with a
mesh; the contained cavity is filled with vigorously impacted morselised
fresh-frozen allograft; and finally, acrylic cement is used to stabilise
the graft and provide rigid, long-lasting fixation of the revised
acetabular component. Favourable results have been published with this technique. While
having its limitations, it is a viable option to address large acetabular

Several studies have reported the safety and efficacy of subcapital
re-alignment for patients with slipped capital femoral epiphysis
(SCFE) using surgical dislocation of the hip and an extended retinacular
flap. Instability of the hip and dislocation as a consequence of
this surgery has only recently gained attention. We discuss this
problem with some illustrative cases. We explored the literature on the possible pathophysiological
causes and surgical steps associated with the risk of post-operative
instability and articular damage. In addition, we describe supplementary
steps that could be used to avoid these problems.Aims

The Unified Classification System (UCS) emphasises
the key principles in the assessment and management of peri-prosthetic
fractures complicating partial or total joint replacement. We tested the inter- and intra-observer agreement for the UCS
as applied to the pelvis and femur using 20 examples of peri-prosthetic
fracture in 17 patients. Each subtype of the UCS was represented
by at least one case. Specialist orthopaedic surgeons (experts)
and orthopaedic residents (pre-experts) assessed reliability on
two separate occasions. For the pelvis, the UCS showed inter-observer agreement of 0.837
(95% confidence intervals (CI) 0.798 to 0.876) for the experts and
0.728 (95% CI 0.689 to 0.767) for the pre-experts. The intra-observer
agreement for the experts was 0.861 (95% CI 0.760 to 0.963) and
0.803 (95% 0.688 to 0.918) for the pre-experts. For the femur, the
UCS showed an inter-observer kappa value of 0.805 (95% CI 0.765
to 0.845) for the experts and a value of 0.732 (95% CI 0.690 to 0.773)
for the pre-experts. The intra-observer agreement was 0.920 (95%
CI 0.867 to 0.973) for the experts, and 0.772 (95% CI 0.652 to 0.892)
for the pre-experts. This corresponds to a substantial and ‘almost
perfect’ inter- and intra-observer agreement for the UCS for peri-prosthetic
fractures of the pelvis and femur. We hope that unifying the terminology of these injuries will

We report the incidence of radiolucent lines (RLLs) using two
flanged acetabular components at total hip arthroplasty (THA) and
the effect of the Rim Cutter. We performed a retrospective review of 300 hips in 292 patients
who underwent primary cemented THA. A contemporary flanged acetabular
component was used with (group 1) and without (group 2) the use
of the Rim Cutter and the Rimfit acetabular component was used with
the Rim Cutter (group 3). RLLs and clinical outcomes were evaluated
immediately post-operatively and at five years post-operatively.Aims

Cubitus varus is the most common late complication of a supracondylar
fracture of the humerus in children. Correction can be performed
using one of a number of techniques of osteotomy but each has disadvantages.
We describe a new technique for correcting post-traumatic cubitus
varus using a lateral closing wedge isosceles triangular osteotomy. A lateral closing wedge isosceles triangular osteotomy was performed
in 25 patients (15 male and ten female with a mean age of 9.5 years
(6 to 12)) between May 2010 and April 2013. All patients had cubitus
varus secondary to malunion of a supracondylar fracture, with good
function of the elbow and a full range of movement. The osteotomy
lines were marked on the bone with an isosceles triangular template
made before surgery, after which the osteotomy was performed leaving
the medial cortex intact. Fixation was performed using two lateral
2 mm Kirschner (K)-wires and patients were immobilised in an above-elbow
plaster. By six to eight weeks callus was present and the wires
and cast were removed. Patients were reviewed at four and six weeks,
three, six and 12 months and then every two years until skeletal
maturity. Clinical and radiographic outcomes were categorised as excellent,
good or poor.Aims

To investigate the longevity of uncemented fixation of a femoral
component in total hip arthroplasty (THA) in patients with Dorr
type C proximal femoral morphology. A total of 350 consecutive uncemented THA in 320 patients were
performed between 1983 and 1987, by a single surgeon using the Taperloc
femoral component. The 63 patients (68 hips) with Dorr type C proximal
femoral morphology were the focus of this review. The mean age of
the patients was 69 years (24 to 88) and mean follow-up was 16.6
years (ten to 29). Survival analysis included eight patients (eight
hips) who died without undergoing revision surgery prior to obtaining
ten years follow-up. All 55 surviving patients (60 hips) were available
for clinical assessment and radiographic review. As a comparator
group, the survival and implant fixation in the remaining 282 THAs
(257 patients) with Dorr type A and B morphology were evaluated.
