The treatment of chronic osteomyelitis often
includes surgical debridement and filling the resultant void with antibiotic-loaded
polymethylmethacrylate cement, bone grafts or bone substitutes.
Recently, the use of bioactive glass to treat bone defects in infections
has been reported in a limited series of patients. However, no direct comparison
between this biomaterial and antibiotic-loaded bone substitute has
been performed. . In this retrospective study, we compared the safety and efficacy
of surgical debridement and local application of the bioactive glass
S53P4 in a series of 27 patients affected by chronic osteomyelitis
of the long bones (Group A) with two other series, treated respectively
with an antibiotic-loaded hydroxyapatite and calcium sulphate compound
(Group B; n = 27) or a mixture of tricalcium phosphate and an antibiotic-loaded
demineralised bone matrix (Group C; n = 22). Systemic antibiotics
were also used in all groups. After comparable periods of follow-up, the control of infection
was similar in the three groups. In particular, 25 out of 27 (92.6%)
patients of Group A, 24 out of 27 (88.9%) in Group B and 19 out
of 22 (86.3%) in Group C showed no
The purpose of this study was to examine the bactericidal efficacy of hydrogen peroxide (H2O2) on The effect of H2O2 was assessed by testing bactericidal effect, time course analysis, growth inhibition, and minimum bactericidal concentration. To assess the bactericidal effect, bacteria were treated for 30 minutes with 0%, 1%, 3%, 4%, 6%, 8%, or 10% H2O2 in saline or water and compared with 3% topical H2O2 solution. For time course analysis, bacteria were treated with water or saline (controls), 3% H2O2 in water, 3% H2O2 in saline, or 3% topical solution for 5, 10, 15, 20, and 30 minutes. Results were analyzed with a two-way analysis of variance (ANOVA) (p < 0.05).Objectives

In 78 consecutive patients, we performed fine-needle aspiration of the hip before revision surgery. At the revision operation biopsies were taken from the capsule and joint tissues. The aspirate and the homogenised soft-tissue specimens were cultured using a radiometric technique. Fifteen hips proved to be infected and 63 sterile on culture of the tissues taken during revision surgery; 94% of the aspiration results were correct. The sensitivity of the technique was 87% and the specificity 95%. Fine-needle aspiration combined with a radiometric culture technique is a simple and reliable method of diagnosing

There are no recent studies comparing cable with
wire for the fixation of osteotomies or fractures in total hip replacement
(THR). Our objective was to evaluate the five-year clinical and
radiological outcomes and complication rates of the two techniques.
We undertook a review including all primary and revision THRs performed
in one hospital between 1996 and 2005 using cable or wire fixation.
Clinical and radiological evaluation was performed five years post-operatively.
Cables were used in 51 THRs and wires in 126, and of these, 36 THRs
with cable (71%) and 101 with wire (80%) were evaluated at follow-up.
The five-year radiographs available for 33 cable and 91 wire THRs
revealed rates of breakage of fixation of 12 of 33 (36%) and 42
of 91 (46%), respectively. With cable there was a significantly
higher risk of metal debris (68% vs 9%; adjusted
relative risk (RR) 6.6; 95% confidence interval (CI) 3.0 to 14.1),
nonunion (36% vs 21%; adjusted RR 2.0; 95% CI 1.0
to 3.9) and osteolysis around the material, acetabulum or femur
(61% vs 19%; adjusted RR 3.9; 95% CI 2.3 to 6.5).
Cable breakage increased the risk of osteolysis to 83%. There
was a trend towards foreign-body reaction and increased infection
with cables. Clinical results did not differ between the groups. In conclusion, we found a higher incidence of complications and
a trend towards increased

Introduction. Negative pressure wound therapy (NPWT) and vessel loop assisted
closure are two common methods used to assist with the closure of
fasciotomy wounds. This retrospective review compares these two
methods using a primary outcome measurement of skin graft requirement. Methods. A retrospective search was performed to identify patients who
underwent fasciotomy at our institution. Patient demographics, location
of the fasciotomy, type of assisted closure, injury characteristics,
need for skin graft, length of stay and evidence of
In the neonate, Group B beta-haemolytic streptococcal osteomyelitis presents with few inflammatory signs, a mild clinical course, extensive bony destruction, and usually single bone involvement. Onset is late (10 to 60 days after birth) and obstetric trauma appears to predispose to
The interaction between surgical lighting and laminar airflow
is poorly understood. We undertook an experiment to identify any
effect contemporary surgical lights have on laminar flow and recommend
practical strategies to limit any negative effects. Neutrally buoyant bubbles were introduced into the surgical field
of a simulated setup for a routine total knee arthroplasty in a
laminar flow theatre. Patterns of airflow were observed and the
number of bubbles remaining above the surgical field over time identified.
Five different lighting configurations were assessed. Data were analysed
using simple linear regression after logarithmic transformation.Aims

A retrospective review of MRSA screening showed that of a total of 8911 patients screened pre-operatively between May 1996 and February 2001, 83 (0.9%) had MRSA isolated from one source or another. During the same period, 115 (13.6%) of 844 positive tissue samples taken during surgery grew
We compared the use of broth culture medium for
samples taken in theatre with the standard practice of placing tissue
samples in universal containers. A total of 67 consecutive patients
had standard multiple samples of deep tissue harvested at surgery
and distributed equally in theatre either to standard universal
containers or to broth culture medium. These samples were cultured
by direct and enrichment methods. The addition of broth in theatre to
standard practice led to an increase in sensitivity from 83% to
95% and an increase in negative predictive value from 77% to 91%.
Placing tissue samples directly into broth in the operating theatre
is a simple, inexpensive way to increase the sensitivity of cultures
from infected patients, and does not appear to compromise the specificity
of these cultures. Cite this article:
