National registers compare implants by their revision rates, but the validity of the method has never been assessed. The New Zealand Joint Registry publishes clinical outcomes (Oxford knee scores, OKS) alongside revision rates, allowing comparison of the two measurements. In the two types of knee replacement, unicompartmental (UKR) had a better knee score than total replacement (TKR), but the revision rate of the former was nearly three times higher than that of the latter. This was because the sensitivity of the revision rate to clinical failure was different for the two implants. For example, of knees with a very poor outcome (OKS <
20 points), only about 12% of TKRs were revised compared with about 63% of UKRs with similar scores. Revision therefore is not an objective measurement and should not be used to compare these two types of implant. Furthermore, revision is much less sensitive than the OKS to clinical failure in both types and therefore exaggerates the success of knee replacements, particularly of TKR.
The Oxford Hip and Knee Scores (OHS, OKS) have been demonstrated
to vary according to age and gender, making it difficult to compare
results in cohorts with different demographics. The aim of this
paper was to calculate reference values for different patient groups
and highlight the concept of normative reference data to contextualise an
individual’s outcome. We accessed prospectively collected OHS and OKS data for patients
undergoing lower limb joint arthroplasty at a single orthopaedic
teaching hospital during a five-year period.

Methods
Patient expectations and their fulfilment are
an important factor in determining patient-reported outcome and satisfaction
of hip (THR) and knee replacement (TKR). The aim of this prospective
cohort study was to examine the expectations of patients undergoing
THR and TKR, and to identify differences in expectations, predictors
of high expectations and the relationship between the fulfilment
of expectations and patient-reported outcome measures. During the
study period, patients who underwent 346 THRs and 323 TKRs completed
an expectation questionnaire, Oxford score and Short-Form 12 (SF-12)
score pre-operatively. At one year post-operatively, the Oxford
score, SF-12, patient satisfaction and expectation fulfilment were
assessed. Univariable and multivariable analysis were performed.
Improvements in mobility and daytime pain were the most important
expectations in both groups. Expectation level did not differ between
THR and TKR. Poor Oxford score, younger age and male gender significantly
predicted high pre-operative expectations (p <
0.001). The level
of pre-operative expectation was not significantly associated with
the fulfilment of expectations or outcome. THR better met the expectations
identified as important by patients. TKR failed to meet expectations
of kneeling, squatting and stair climbing. High fulfilment of expectation
in both THR and TKR was significantly predicted by young age, greater
improvements in Oxford score and high pre-operative mental health
scores. The fulfilment of expectations was highly correlated with satisfaction.
The aim of this study was to examine the loading
of the other joints of the lower limb in patients with unilateral osteoarthritis
(OA) of the knee. We recruited 20 patients with no other symptoms
or deformity in the lower limbs from a consecutive cohort of patients
awaiting knee replacement. Gait analysis and electromyographic recordings were
performed to determine moments at both knees and hips, and contraction
patterns in the medial and lateral quadriceps and hamstrings bilaterally.
The speed of gait was reduced in the group with OA compared with
the controls, but there were only minor differences in stance times
between the limbs. Patients with OA of the knee had significant
increases in adduction moment impulse at both knees and the contralateral
hip (adjusted p-values: affected knee: p <
0.01, unaffected knee
p = 0.048, contralateral hip p = 0.03), and significantly increased
muscular co-contraction bilaterally compared with controls (all
comparisons for co-contraction, p <

The objective of this study was to explore dimensionality of
the Oxford Hip Score (OHS) and examine whether self-reported pain
and functioning can be distinguished in the form of subscales. This was a secondary data analysis of the UK NHS hospital episode
statistics/patient-reported outcome measures dataset containing
pre-operative OHS scores on 97 487 patients who were undergoing
hip replacement surgery. Objective
Methods
Hypermobility is an acknowledged risk factor
for patellar instability. In this case control study the influence
of hypermobility on clinical outcome following medial patellofemoral
ligament (MPFL) reconstruction for patellar instability was studied. A total of 25 patients with hypermobility as determined by the
Beighton criteria were assessed and compared with a control group
of 50 patients who were matched for age, gender, indication for
surgery and degree of trochlear dysplasia. The patients with hypermobility
had a Beighton Score of ≥ 6; the control patients had a score of <
4. All patients underwent MPFL reconstruction performed using semitendinosus
autograft and a standardised arthroscopically controlled technique.
The mean age of the patients was 25 years (17 to 49) and the mean
follow-up was 15 months (6 to 30). Patients with hypermobility had a significant improvement in
function following surgery, with reasonable rates of satisfaction,
perceived improvement, willingness to repeat and likelihood of recommendation.
Functional improvements were significantly less than in control
patients (p <
0.01). Joint hypermobility is not a contraindication to MPFL reconstruction
although caution is recommended in managing the expectations of
patients with hypermobility before consideration of surgery.
The options for treatment of the young active patient with isolated symptomatic osteoarthritis of the medial compartment and pre-existing deficiency of the anterior cruciate ligament are limited. The potential longevity of the implant and levels of activity of the patient may preclude total knee replacement, and tibial osteotomy and unicompartmental knee arthroplasty are unreliable because of the ligamentous instability. Unicompartmental knee arthroplasties tend to fail because of wear or tibial loosening resulting from eccentric loading. Therefore, we combined reconstruction of the anterior cruciate ligament with unicompartmental arthroplasty of the knee in 15 patients (ACLR group), and matched them with 15 patients who had undergone Oxford unicompartmental knee arthroplasty with an intact anterior cruciate ligament (ACLI group). The clinical and radiological data at a minimum of 2.5 years were compared for both groups. The groups were well matched for age, gender and length of follow-up and had no significant differences in their pre-operative scores. At the last follow-up, the mean outcome scores for both the ACLR and ACLI groups were high (Oxford knee scores of 46 (37 to 48) and 43 (38 to 46), respectively, objective Knee Society scores of 99 (95 to 100) and 94 (82 to 100), and functional Knee Society scores of 96 and 96 (both 85 to 100). One patient in the ACLR group needed revision to a total knee replacement because of infection. No patient in either group had radiological evidence of component loosening. The radiological study showed no difference in the pattern of tibial loading between the groups. The short-term clinical results of combined anterior cruciate ligament reconstruction and unicompartmental knee arthroplasty are excellent. The previous shortcomings of unicompartmental knee arthroplasty in the presence of deficiency of the anterior cruciate ligament appear to have been addressed with the combined procedure. This operation seems to be a viable treatment option for young active patients with symptomatic arthritis of the medial compartment, in whom the anterior cruciate ligament has been ruptured.
In this study we randomised 140 patients who
were due to undergo primary total knee arthroplasty (TKA) to have the
procedure performed using either patient-specific cutting guides
(PSCG) or conventional instrumentation (CI). The primary outcome measure was the mechanical axis, as measured
at three months on a standing long-leg radiograph by the hip–knee–ankle
(HKA) angle. This was undertaken by an independent observer who
was blinded to the instrumentation. Secondary outcome measures were
component positioning, operating time, Knee Society and Oxford knee
scores, blood loss and length of hospital stay. A total of 126 patients (67 in the CI group and 59 in the PSCG
group) had complete clinical and radiological data. There were 88
females and 52 males with a mean age of 69.3 years (47 to 84) and
a mean BMI of 28.6 kg/m2 (20.2 to 40.8). The mean HKA
angle was 178.9° (172.5 to 183.4) in the CI group and 178.2° (172.4
to 183.4) in the PSCG group (p = 0.34). Outliers were identified
in 22 of 67 knees (32.8%) in the CI group and 19 of 59 knees (32.2%)
in the PSCG group (p = 0.99). There was no significant difference
in the clinical results (p = 0.95 and 0.59, respectively). Operating time,
blood loss and length of hospital stay were not significantly reduced
(p = 0.09, 0.58 and 0.50, respectively) when using PSCG. The use of PSCG in primary TKA did not reduce the proportion
of outliers as measured by post-operative coronal alignment. Cite this article:
We have performed a prospective double-blind, randomised controlled trial over two years to evaluate the efficacy and safety of an intra-operative peri-articular injection of triamcinolone acetonide in patients undergoing medial unicondylar knee replacement. We randomised 90 patients into two equal groups. The study group received an injection of triamcinolone acetonide, bupivacaine, and epinephrine into the peri-articular tissues at the end of the operation. The control group received the same injection mixture but without the addition of triamcinolone. The peri-operative analgesic regimen was standardised. The study group reported a significant reduction in pain (p = 0.014 at 12 hours, p = 0.031 at 18 hours and p = 0.031 at 24 hours) and had a better range of movement (p = 0.023 at three months). There was no significant difference in the rate of infection and no incidence of tendon rupture in either group. The addition of corticosteroid to the peri-articular injection after unicondylar knee replacement had both immediate and short-term benefits in terms of relief from pain, and rehabilitation with no increased risk of infection.
In this systematic review, our aim was to explore
whether or not patients are able to return to athletic activity
following lower limb joint replacement. We also investigated any evidence
as to whether participation in athletic activity post-joint replacement
increases complications and reduces implant survival. A PubMed, Embase and Sports Discus search was performed using
the MeSH terms ‘Sport’, ‘Athletic’, ‘Athlete’, ‘Physical’, ‘Activity’,
‘Arthroplasty’, ‘Total Hip Replacement’, ‘Hip Resurfacing’, ‘Total
Knee Replacement’, ‘Unicompartmental Knee Replacement’ and ‘Unicondylar
Knee Replacement’. From this search, duplications were excluded,
the remaining abstracts were reviewed and any unrelated to the search
terms were excluded. The remaining abstracts had their full papers
reviewed. Following joint replacement, participation in sporting activity
is common principally determined by pre-operative patient activity
levels, BMI and patient age. The type of joint replaced is of less
significance. Total time spent performing activity does not change
but tends to be at a lower intensity. There is little evidence in
the literature of an association between high activity levels and

Patellofemoral joint degeneration is often considered a contraindication to medial unicompartmental knee replacement. We examined the validity of this preconception using information gathered prospectively on the intra-operative status of the patellofemoral joint in 824 knees in 793 consecutive patients who underwent Oxford unicondylar knee replacement for anteromedial osteoarthritis. All operations were performed between January 1998 and September 2005. A five-point grading system classified degeneration of the patellofemoral joint from none to full-thickness cartilage loss. A subclassification of the presence or absence of any full-thickness cartilage loss was subsequently performed to test selected hypotheses. Outcome was evaluated independently by physiotherapists using the Oxford and the American Knee Society Scores with a minimum follow-up of one year. Full-thickness cartilage loss on the trochlear surface was observed in 100 of 785 knees (13%), on the medial facet of the patella in 69 of 782 knees (9%) and on the lateral facet in 29 of 784 knees (4%). Full-thickness cartilage loss at any location was seen in 128 knees (16%) and did not produce a significantly worse outcome than those with a normal or near-normal joint surface. The severity of the degeneration at any of the intra-articular locations also had no influence on outcome. We concluded that, provided there is not bone loss and grooving of the lateral facet, damage to the articular cartilage of the patellofemoral joint to the extent of full-thickness cartilage loss is not a contraindication to the Oxford mobile-bearing unicompartmental knee replacement.
Peri-prosthetic infection is amongst the most
common causes of failure following total knee replacement (TKR).
In the presence of established infection, thorough joint debridement
and removal of all components is necessary following which new components
may be implanted. This can be performed in one or two stages; two-stage revision
with placement of an interim antibiotic-loaded spacer is regarded
by many to be the standard procedure for eradication of peri-prosthetic
joint infection. We present our experience of a consecutive series of 50 single-stage
revision TKRs for established deep infection performed between 1979
and 2010. There were 33 women and 17 men with a mean age at revision
of 66.8 years (42 to 84) and a mean follow-up of 10.5 years (2 to
24). The mean time between the primary TKR and the revision procedure
was 2.05 years (1 to 8). Only one patient required a further revision for recurrent infection,
representing a success rate of 98%. Nine patients required further
revision for aseptic loosening, according to microbiological testing
of biopsies taken at the subsequent surgery. Three other patients
developed a further septic episode but none required another revision. These results suggest that a single-stage revision can produce
comparable results to a two-stage revision. Single-stage revision
offers a reduction in costs as well as less morbidity and inconvenience

Substantial healthcare resources have been devoted
to computer navigation and patient-specific instrumentation systems
that improve the reproducibility with which neutral mechanical alignment
can be achieved following total knee replacement (TKR). This choice of
alignment is based on the long-held tenet that the alignment of
the limb post-operatively should be within 3° of a neutral mechanical
axis. Several recent studies have demonstrated no significant difference

We report the mid-term results of a new patellofemoral arthroplasty for established isolated patellofemoral arthritis. We have reviewed the experience of 109 consecutive patellofemoral resurfacing arthroplasties in 85 patients who were followed up for at least five years. The five-year survival rate, with revision as the endpoint, was 95.8% (95% confidence interval 91.8% to 99.8%). There were no cases of loosening of the prosthesis. At five years the median Bristol pain score improved from 15 of 40 points (interquartile range 5 to 20) pre-operatively, to 35 (interquartile range 20 to 40), the median Melbourne score from 10 of 30 points (interquartile range 6 to 15) to 25 (interquartile range 20 to 29), and the median Oxford score from 18 of 48 points (interquartile range 13 to 24) to 39 (interquartile range 24 to 45). Successful results, judged on a Bristol pain score of at least 20 at five years, occurred in 80% (66) of knees. The main complication was radiological progression of arthritis, which occurred in 25 patients (28%) and emphasises the importance of the careful selection of patients. These results give increased confidence in the use of patellofemoral arthroplasty.
We report a prospective analysis of clinical
outcome in patients treated with medial patellofemoral ligament
(MPFL) reconstruction using an autologous semitendinosus graft.
The technique includes superolateral portal arthroscopic assessment
before and after graft placement to ensure correct graft tension
and patellar tracking before fixation. Between October 2005 and
October 2010, a total of 201 consecutive patients underwent 219 procedures.
Follow-up is presented for 211 procedures in 193 patients with a
mean age of 26 years (16 to 49), and mean follow-up of 16 months
(6 to 42). Indications were atraumatic recurrent patellar dislocation
in 141 patients, traumatic recurrent dislocation in 50, pain with
subluxation in 14 and a single dislocation with persistent instability
in six. There have been no recurrent dislocations/subluxations.
There was a statistically significant improvement between available pre-
and post-operative outcome scores for 193 patients (all p <
0.001).
Female patients with a history of atraumatic recurrent dislocation
and all patients with history of previous surgery had a significantly
worse outcome (all p <
0.05). The indication for surgery, degree
of dysplasia, associated patella alta, time from primary dislocation
to surgery and evidence of associated cartilage damage at operation
did not result in any significant difference in outcome. This series adds considerably to existing evidence that MPFL
reconstruction is an effective surgical procedure for selected patients
with patellofemoral instability.
We obtained pre-operative and six-month post-operative
Oxford hip (OHS) and knee scores (OKS) for 1523 patients who underwent
total hip replacement and 1784 patients who underwent total knee
replacement. They all also completed a six-month satisfaction question. Scatter plots showed no relationship between pre-operative Oxford
scores and six-month satisfaction scores. Spearman’s rank correlation
coefficients were -0.04 (95% confidence interval (CI) -0.09 to 0.01)
between OHS and satisfaction and 0.04 (95% CI -0.01 to 0.08) between
OKS and satisfaction. A receiver operating characteristic (ROC) curve
analysis was used to identify a cut-off point for the pre-operative
OHS/OKS that identifies whether or not a patient is satisfied with
surgery. We obtained an area under the ROC curve of 0.51 (95% CI
0.45 to 0.56) for hip replacement and 0.56 (95% CI 0.51 to 0.60)
for knee replacement, indicating that pre-operative Oxford scores
have no predictive accuracy in distinguishing satisfied from dissatisfied
patients. In the NHS widespread attempts are being made to use patient-reported
outcome measures (PROMs) data for the purpose of prioritising patients
for surgery. Oxford hip and knee scores have no predictive accuracy
in relation to post-operative patient satisfaction. This evidence
does not support their current use in prioritising access to care.
In the absence of patellar resurfacing, we have
previously shown that the use of electrocautery around the margin of
the patella improved the one-year clinical outcome of total knee
replacement (TKR). In this prospective randomised study we compared
the mean 3.7 year (1.1 to 4.2) clinical outcomes of 300 TKRs performed
with and without electrocautery of the patellar rim: this is an
update of a previous report. The overall prevalence of anterior knee
pain was 32% (95% confidence intervals [CI] 26 to 39), and 26% (95%
CI 18 to 35) in the intervention group compared with 38% (95% CI
29 to 48) in the control group (chi-squared test; p = 0.06). The
overall prevalence of anterior knee pain remained unchanged between
the one-year and 3.7 year follow-up (chi-squared test; p = 0.12). The
mean total Western Ontario McMasters Universities Osteoarthritis
Indices and the American Knee Society knee and function scores at
3.7 years’ follow-up were similar in the intervention and control
groups (repeated measures analysis of variance p = 0.43, p = 0.09
and p = 0.59, respectively). There were no complications. A total
of ten patients (intervention group three, control group seven)
required secondary patellar resurfacing after the first year. Our study suggests that the improved clinical outcome with electrocautery
denervation compared with no electrocautery is not maintained at
a mean of 3.7 years’ follow-up. Cite this article:
This prospective study compares the outcome of
157 hydroxyapatite (HA)-coated tibial components with 164 cemented
components in the ROCC Rotating Platform total knee replacement
in 291 patients. The mean follow-up was 7.6 years (5.2 to 11). There
were two revisions for loosening: one for an HA-coated and one for
a cemented tibial component. Radiological evaluation demonstrated
no radiolucent lines with the HA-coated femoral components. A total
of three HA-coated tibial components exhibited radiolucent lines
at three months post-operatively and these disappeared after three
further months of protected weight-bearing. With HA-coated components
the operating time was shorter (p <
0.006) and the radiological
assessment of the tibial interface was more stable (p <
0.01).
Using revision for aseptic loosening of the tibial component as
the end point, the survival rates at nine years was identical for
both groups at 99.1%. Our results suggest that HA-coated components perform at least
as well as the same design with cemented components and compare
favourably with those of series describing cemented or porous-coated
knee replacements, suggesting that fixation of both components with
hydroxyapatite is a reliable option in primary total knee replacement.
We reviewed the long-term clinical and radiological
results of 63 uncemented Low Contact Stress (LCS) total knee replacements
(TKRs) in 47 patients with rheumatoid arthritis. The mean age of
the patients at the time of surgery was 69 years (53 to 81). At
a mean follow-up of 22 years (20 to 25), 12 patients were alive
(17 TKRs), 27 had died (36 TKRs), and eight (ten TKRs) were lost
to follow-up. Revision was necessary in seven patients (seven TKRs, 11.1%)
at a mean of 12.1 years (0 to 19) after surgery. In the surviving
ten patients who had not undergone revision (15 TKRs), the mean
Oxford knee score was 30.2 (16 to 41) at a mean follow-up of 19.5
years (15 to 24.7) and mean active flexion was 105° (90° to 150°).
The survival rate was 88.9% at 20 years (56 of 63) and the Kaplan–Meier
survival estimate, without revision, was 80.2% (95% confidence interval
37 to 100) at 25 years. Cite this article:
