Aims. The aim of this study was to compare patient-reported outcomes (PROMs) following isolated anterior cruciate ligament reconstruction (ACLR), with those following ACLR and concomitant meniscal resection or repair. Methods. We reviewed prospectively collected data from the UK National Ligament Registry for patients who underwent primary ACLR between January 2013 and December 2022. Patients were categorized into five groups: isolated ACLR, ACLR with medial meniscus (MM) repair, ACLR with MM resection, ACLR with lateral meniscus (LM) repair, and ACLR with LM resection. Linear regression analysis, with isolated ACLR as the reference, was performed after adjusting for confounders. Results. From 14,895 ACLR patients, 4,400 had two- or five-year Knee injury and Osteoarthritis Outcome Scores (KOOS) available. Implantation of ultra-purified alginate (UPAL) gel is safe and effective in animal osteochondral defect models. This study aimed to examine the applicability of UPAL gel implantation to acellular therapy in humans with cartilage injury. A total of 12 patients (12 knees) with symptomatic, post-traumatic, full-thickness cartilage lesions (1.0 to 4.0 cm2) were included in this study. UPAL gel was implanted into chondral defects after performing bone marrow stimulation technique, and assessed for up to three years postoperatively. To determine the relationship between articular cartilage status and clinical outcomes after medial opening-wedge high tibial osteotomy (MOHTO) for medial compartmental knee osteoarthritis at intermediate follow-up. Aims. The Oswestry-Bristol Classification (OBC) is an MRI-specific assessment tool to grade trochlear dysplasia. The aim of this study is to validate clinically the OBC by demonstrating its use in selecting treatments that are safe and effective. Methods. The OBC and the patellotrochlear index were used as part of the Oswestry Patellotrochlear Algorithm (OPTA) to guide the surgical treatment of patients with patellar instability. Anterior cruciate ligament (ACL) rupture commonly leads to post-traumatic osteoarthritis, regardless of surgical reconstruction. This study uses standing MRI to investigate changes in contact area, contact centroid location, and tibiofemoral alignment between ACL-injured knees and healthy controls, to examine the effect of ACL reconstruction on these parameters. An upright, open MRI was used to directly measure tibiofemoral contact area, centroid location, and alignment in 18 individuals with unilateral ACL rupture within the last five years. Eight participants had been treated nonoperatively and ten had ACL reconstruction performed within one year of injury. All participants were high-functioning and had returned to sport or recreational activities. Healthy contralateral knees served as controls. To compare time dependent functional improvement for patients with medial, respectively lateral knee osteoarthritis (OA) after treatment with opening wedge osteotomy relieving the pressure on the osteoarthritic part of the knee. In all, 49 patients (52 knees) with a mean age of 47 years (31 to 64) underwent high tibial osteotomies (HTO), and 24 patients with a mean age of 48 years (31 to 62) low femoral osteotomies (LFO) with opening wedge technique due to medial, respectively lateral knee OA with malalignment. All osteotomies were stabilized with a Puddu plate and bone grafting performed in the same time period (2000 to 2008). The patients were evaluated by the Knee Injury and Osteoarthritis Outcome Score (KOOS) pre-operatively and at six months, and at one, two, five, and ten years postoperatively. Different methods of anterior cruciate ligament (ACL) reconstruction
have been described for skeletally immature patients before closure
of the growth plates. However, the outcome and complications following
this treatment remain unclear. The aim of this systematic review
was to analyse the outcome and complications of different techniques
which may be used for reconstruction of the ACL in these patients. We performed a systematic review of the literature according
to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses
guidelines. This involved a comprehensive search of PubMed, Medline,
CINAHL, Cochrane, Embase and Google Scholar databases using the
following combinations of keywords, “knee”, “anterior cruciate ligament”,
“reconstruction”, “injury”, “children”, “adolescent”, “skeletally
immature”, “open physis” and “surgery”.Aims

The aim of this study was to investigate the long-term clinical
and radiological outcome of patients who suffer recurrent injuries
to the anterior cruciate ligament (ACL) after reconstruction and
require revision surgery. From a consecutive series of 200 patients who underwent primary
reconstruction following rupture of the ACL, we identified 36 who
sustained a further rupture, 29 of whom underwent revision surgery.
Patients were reviewed prospectively at one, two, seven, 15 and
about 20 years after their original surgery. Primary outcome measures
were the number of further ruptures, the posterior tibial slope
(PTS), and functional and radiological outcomes. These were compared
with a gender and age matched cohort of patients who underwent primary
ACL reconstruction only.Aims

The Bereiter trochleoplasty has been used in our unit for 12
years to manage recurrent patellar instability in patients with
severe trochlea dysplasia. The aim of this study was to document
the outcome of a large consecutive cohort of patients who have undergone
this operation. Between June 2002 and August 2013, 214 consecutive trochleoplasties
were carried out in 185 patients. There were 133 women and 52 men
with a mean age of 21.3 years (14 to 38). All patients were offered
yearly clinical and radiological follow-up. They completed the following
patient reported outcome scores (PROMs): International Knee Documentation
Committee subjective scale, the Kujala score, the Western Ontario
and McMaster Universities Arthritis Index score and the short-form

The purpose of this study was to report the experience of dynamic
intraligamentary stabilisation (DIS) using the Ligamys device for
the treatment of acute ruptures of the anterior cruciate ligament
(ACL). Between March 2011 and April 2012, 50 patients (34 men and 16
women) with an acute rupture of the ACL underwent primary repair
using this device. The mean age of the patients was 30 years (18
to 50). Patients were evaluated for laxity, stability, range of
movement (ROM), Tegner, Lysholm, International Knee Documentation Committee
(IKDC) and visual analogue scale (VAS) scores over a follow-up period

The aim of this study was to assess the effect
of injecting genetically engineered chondrocytes expressing transforming
growth factor beta 1 (TGF-β1) into the knees of patients with osteoarthritis.
We assessed the resultant function, pain and quality of life. A total of 54 patients (20 men, 34 women) who had a mean age
of 58 years (50 to 66) were blinded and randomised (1:1) to receive
a single injection of the active treatment or a placebo. We assessed
post-treatment function, pain severity, physical function, quality
of life and the incidence of treatment-associated adverse events. Patients
were followed at four, 12 and 24 weeks after injection. At final follow-up the treatment group had a significantly greater
improvement in the mean International Knee Documentation Committee
score than the placebo group (16 points; -18 to 49, This technique may result in improved clinical outcomes, with
the aim of slowing the degenerative process, leading to improvements
in pain and function. However, imaging and direct observational
studies are needed to verify cartilage regeneration. Nevertheless,

The aim of this study was to examine the functional
outcome at ten years following lateral closing wedge high tibial osteotomy
for medial compartment osteoarthritis of the knee and to define
pre-operative predictors of survival and determinants of functional
outcome. 164 consecutive patients underwent high tibial osteotomy between
2000 and 2002. A total of 100 patients (100 knees) met the inclusion
criteria and 95 were available for review at ten years. Data were
collected prospectively and included patient demographics, surgical
details, long leg alignment radiographs, Western Ontario and McMaster Universities
osteoarthritis index (WOMAC) and Knee Society scores (KSS) pre-operatively
and at five and ten years follow-up. At ten years, 21 patients had been revised at a mean of five
years. Overall Kaplan–Meier survival was 87% (95% confidence interval
(CI) 81 to 94) and 79% (95% CI 71 to 87) at five and ten years,
respectively. When compared with unrevised patients, those who had
been revised had significantly lower mean pre-operative WOMAC Scores
(47 (21 to 85) This study has shown that improved survival is associated with
age <
55 years, pre-operative WOMAC scores >

functional scores, survival can be good and functional outcomes

We have investigated whether shape of the knee
can predict the clinical outcome of patients after an anterior cruciate
ligament rupture. We used statistical shape modelling to measure
the shape of the knee joint of 182 prospectively followed patients
on lateral and Rosenberg view radiographs of the knee after a rupture

Between 1993 and 1994, 891 patients underwent
primary anterior cruciate ligament (ACL) reconstruction. A total
of 48 patients had undergone bilateral ACL reconstruction and
42 were available for review. These patients were matched to a unilateral
ACL reconstruction control group for gender, age, sport of primary
injury, meniscal status and graft type. At 15-year follow-up a telephone
interview with patients in both groups was performed. The incidence
of further ACL injury was identified through structured questions
and the two groups were compared for the variables of graft rupture
or further ACL injury, family history of ACL injury, International
Knee Documentation Committee (IKDC) subjective score and activity
level. There were 28 male and 14 female patients with a mean age of
25 years (13 to 42) at the time of first ACL injury. Subsequent
further ACL injury was identified in ten patients (24%) in the bilateral
ACL reconstruction study group and in nine patients (21%) in the
unilateral ACL reconstruction control group (p = 0.794). The mean
time from bilateral ACL reconstruction to further ACL injury was
54 months (6 to 103). There was no significant difference between
the bilateral ACL reconstruction study group and the matched unilateral
ACL reconstruction control group in incidence of further ACL injury
(p = 0.794), family history of ACL injury (p = 0.595), IKDC activity
level (p = 0.514), or IKDC subjective score (p = 0.824). After bilateral ACL reconstruction the incidence of graft rupture
and subjective outcomes were equivalent to that after unilateral

Hypermobility is an acknowledged risk factor
for patellar instability. In this case control study the influence
of hypermobility on clinical outcome following medial patellofemoral
ligament (MPFL) reconstruction for patellar instability was studied. A total of 25 patients with hypermobility as determined by the
Beighton criteria were assessed and compared with a control group
of 50 patients who were matched for age, gender, indication for
surgery and degree of trochlear dysplasia. The patients with hypermobility
had a Beighton Score of ≥ 6; the control patients had a score of <
4. All patients underwent MPFL reconstruction performed using semitendinosus
autograft and a standardised arthroscopically controlled technique.
The mean age of the patients was 25 years (17 to 49) and the mean
follow-up was 15 months (6 to 30). Patients with hypermobility had a significant improvement in
function following surgery, with reasonable rates of satisfaction,
perceived improvement, willingness to repeat and likelihood of recommendation.
Functional improvements were significantly less than in control
patients (p <
0.01). Joint hypermobility is not a contraindication to MPFL reconstruction
although caution is recommended in managing the expectations of
patients with hypermobility before consideration of surgery.
We continued a prospective longitudinal follow-up
study of 53 remaining patients who underwent open total meniscectomy
as adolescents and who at that time had no other intra-articular
pathology of the knee. Their clinical, radiological and patient-reported
outcomes are described at a mean follow-up of 40 years (33 to 50).
The cohort of patients who had undergone radiological evaluation
previously after 30 years were invited for clinical examination,
radiological evaluation and review using two patient-reported outcome
measures. A total of seven patients (13.2%) had already undergone total
knee replacement at the time of follow-up. A significant difference
was observed between the operated and non-operated knee in terms
of range of movement and osteoarthritis of the tibiofemoral joint,
indicating a greater than fourfold relative risk of osteoarthritis
at 40 years post-operatively. All patients were symptomatic as defined
by the Knee Injury and Osteoarthritis Outcome Score. This study represents the longest follow-up to date and it can
be concluded that meniscectomy leads to symptomatic osteoarthritis
of the knee later in life, with a resultant 132-fold increase in
the rate of total knee replacement in comparison to their geographical
and age-matched peers.
Our aim was to assess the use of intra-operative fluoroscopy
in the assessment of the position of the tibial tunnel during reconstruction
of the anterior cruciate ligament (ACL). Between January and June 2009 a total of 31 arthroscopic hamstring
ACL reconstructions were performed. Intra-operative fluoroscopy
was introduced (when available) to verify the position of the guidewire
before tunnel reaming. It was only available for use in 20 cases,
due to other demands on the radiology department. The tourniquet
times were compared between the two groups and all cases where radiological
images lead to re-positioning of the guide wire were recorded. The
secondary outcome involved assessing the tibial interference screw
position measured on post-operative radiographs and comparing with
the known tunnel position as shown on intra-operative fluoroscopic
images.Objectives
Methods
We report a prospective analysis of clinical
outcome in patients treated with medial patellofemoral ligament
(MPFL) reconstruction using an autologous semitendinosus graft.
The technique includes superolateral portal arthroscopic assessment
before and after graft placement to ensure correct graft tension
and patellar tracking before fixation. Between October 2005 and
October 2010, a total of 201 consecutive patients underwent 219 procedures.
Follow-up is presented for 211 procedures in 193 patients with a
mean age of 26 years (16 to 49), and mean follow-up of 16 months
(6 to 42). Indications were atraumatic recurrent patellar dislocation
in 141 patients, traumatic recurrent dislocation in 50, pain with
subluxation in 14 and a single dislocation with persistent instability
in six. There have been no recurrent dislocations/subluxations.
There was a statistically significant improvement between available pre-
and post-operative outcome scores for 193 patients (all p <
0.001).
Female patients with a history of atraumatic recurrent dislocation
and all patients with history of previous surgery had a significantly
worse outcome (all p <
0.05). The indication for surgery, degree
of dysplasia, associated patella alta, time from primary dislocation
to surgery and evidence of associated cartilage damage at operation
did not result in any significant difference in outcome. This series adds considerably to existing evidence that MPFL
reconstruction is an effective surgical procedure for selected patients
with patellofemoral instability.
