Previous dye-infusion experiments on cadavers have suggested that the hindfoot should be divided into four muscle compartments including a newly described ‘calcaneal’ element containing quadratus plantae. Since there are no clinical data to support this proposed division, we re-examined the validity of the infusion experiment. We made infusions of dilute Omnipaque at a constant rate into flexor digitorum brevis of four cadaver feet. We monitored the spread of the infusate by real-time CT imaging and measured the pressures at the infusion site by side-ported needles. In all feet, the barrier between flexor digitorum brevis and quadratus plantae became incompetent at pressures of less than 10 mmHg. Pressure gradients in this range cannot be expected to affect tissue perfusion significantly and independently generate compartment syndromes. These results do not confirm those of previous studies carried out by uncontrolled and unmonitored injections made by hand. Injection studies in cadaver limbs can give dramatically different results depending upon the assumptions made when designing the experiment. The technique cannot adequately act as a model of the physiology of the compartment syndrome. As the existence of a physiologically significant compartmental boundary between flexor digitorum brevis and quadratus plantae is based solely on a cadaver infusion experiment the presence of a ‘calcaneal’ compartment has not been confirmed

Patients with diabetes mellitus may develop plantar flexion contractures (equinus) which may increase forefoot pressure during walking. In order to determine the relationship between equinus and forefoot pressure, we measured forefoot pressure during walking in 27 adult diabetics with a mean age of 66.3 years (sd 7.4) and a mean duration of the condition of 13.4 years (sd 12.6) using an Emed mat. Maximum dorsiflexion of the ankle was determined using a custom device which an examiner used to apply a dorsiflexing torque of 10 Nm (sd 1) for five seconds.

Simple linear regression showed that the relationship between equinus and peak forefoot pressure was significant (p < 0.0471), but that only a small portion of the variance was accounted for (R2 = 0.149). This indicates that equinus has only a limited role in causing high forefoot pressure. Our findings suggest caution in undertaking of tendon-lengthening procedures to reduce peak forefoot plantar pressures in diabetic subjects until clearer indications are established.

We reviewed 116 patients who underwent 118 arthroscopic ankle arthrodeses. The mean age at operation was 57 years, 2 months (20 to 86 years). The indication for operation was post-traumatic osteoarthritis in 67, primary osteoarthritis in 36, inflammatory arthropathy in 13 and avascular necrosis in two. The mean follow-up was 65 months (18 to 144). Nine patients (10 ankles) died before final review and three were lost to follow-up, leaving 104 patients (105 ankles) who were assessed by a standard telephone interview. The pre-operative talocrural deformity was between 22° valgus and 28° varus, 94 cases were within 10° varus/valgus. The mean time to union was 12 weeks (6 to 20). Nonunion occurred in nine cases (7.6%). Other complications included 22 cases requiring removal of a screw for prominence, three superficial infections, two deep vein thromboses/pulmonary emboli, one revision of fixation, one stress fracture and one deep infection. Six patients had a subtalar fusion at a mean of 48 months after ankle fusion. There were 48 patients with excellent, 35 with good, 10 with fair and 11 with poor clinical results.
