Implantation of ultra-purified alginate (UPAL) gel is safe and effective in animal osteochondral defect models. This study aimed to examine the applicability of UPAL gel implantation to acellular therapy in humans with cartilage injury. A total of 12 patients (12 knees) with symptomatic, post-traumatic, full-thickness cartilage lesions (1.0 to 4.0 cm2) were included in this study. UPAL gel was implanted into chondral defects after performing bone marrow stimulation technique, and assessed for up to three years postoperatively. Debridement, antibiotics, and implant retention (DAIR) remains one option for the treatment of acute periprosthetic joint infection (PJI) despite imperfect success rates. Intraosseous (IO) administration of vancomycin results in significantly increased local bone and tissue concentrations compared to systemic antibiotics alone. The purpose of this study was to evaluate if the addition of a single dose of IO regional antibiotics to our protocol at the time of DAIR would improve outcomes. A retrospective case series of 35 PJI TKA patients, with a median age of 67 years (interquartile range (IQR) 61 to 75), who underwent DAIR combined with IO vancomycin (500 mg), was performed with minimum 12 months' follow-up. A total of 26 patients with primary implants were treated for acute perioperative or acute haematogenous infections. Additionally, nine patients were treated for chronic infections with components that were considered unresectable. Liposomal bupivacaine (LB) as part of a periarticular injection protocol continues to be a highly debated topic in total knee arthroplasty (TKA). We evaluated the effect of discontinuing the use of LB in a periarticular protocol on immediate postoperative pain scores, opioid consumption, and objective functional outcomes. On 1 July 2019, we discontinued the use of intraoperative LB as part of a periarticular injection protocol. A consecutive group of patients who received LB as part of the protocol (Protocol 1) and a subsequent group who did not (Protocol 2) were compared. All patients received the same opioid-sparing protocol. Verbal rating scale (VRS) pain scores were collected from our electronic data warehouse and averaged per patient per 12-hour interval. Events relating to the opiate administration were derived as morphine milligram equivalences (MMEs) per patient per 24-hour interval. The Activity Measure for Post-Acute Care (AM-PAC) tool was used to assess the immediate postoperative function.Aims

Metaphyseal cones with cemented stems are frequently used in revision total knee arthroplasty (TKA). However, if the diaphysis has been previously violated, the resultant sclerotic canal can impair cemented stem fixation, which is vital for bone ingrowth into the cone, and long-term fixation. We report the outcomes of our solution to this problem, in which impaction grafting and a cemented stem in the diaphysis is combined with an uncemented metaphyseal cone, for revision TKA in patients with severely compromised bone. A metaphyseal cone was combined with diaphyseal impaction grafting and cemented stems for 35 revision TKAs. There were two patients with follow-up of less than two years who were excluded, leaving 33 procedures in 32 patients in the study. The mean age of the patients at the time of revision TKA was 67 years (32 to 87); 20 (60%) were male. Patients had undergone a mean of four (1 to 13) previous knee arthroplasty procedures. The indications for revision were aseptic loosening (80%) and two-stage reimplantation for prosthetic joint infection (PJI; 20%). The mean follow-up was four years (2 to 11).Aims

To date, no study has demonstrated an improvement in postoperative outcomes following elective joint arthroplasty with a focus on nutritional intervention for patients with preoperative hypoalbuminaemia. In this prospective study, we evaluated differences in the hospital length of stay (LOS), rate of re-admission, and total patient charges for a malnourished patient study population who received a specific nutrition protocol before surgery. An analytical report was extracted from the electronic medical record (EMR; Epic, Verona, Wisconsin) of a five-hospital network joint arthroplasty patient data set between 2014 and 2017. A total of 4733 patients underwent joint arthroplasty and had preoperative measurement of albumin levels: 2220 at four hospitals and 2513 at the study hospital. Albumin ≤ 3.4 g/l, designated as malnutrition, was found in 543 patients (11.5%). A nutritional intervention programme focusing on a high-protein, anti-inflammatory diet was initiated in January 2017 at one study hospital. Hospital LOS, re-admission rate, and 90-day charges were compared for differential change between patients in study and control hospitals for all elective hip and knee arthroplasty patients, and for malnourished patients over time as the nutrition intervention was implemented.Aims

The treatment of patients with allergies to metal in total joint arthroplasty is an ongoing debate. Possibilities include the use of hypoallergenic prostheses, as well as the use of standard cobalt-chromium (CoCr) alloy. This non-designer study was performed to evaluate the clinical outcome and survival rates of unicondylar knee arthroplasty (UKA) using a standard CoCr alloy in patients reporting signs of a hypersensitivity to metal. A consecutive series of patients suitable for UKA were screened for symptoms of metal hypersensitivity by use of a questionnaire. A total of 82 patients out of 1737 patients suitable for medial UKA reporting cutaneous metal hypersensitivity to cobalt, chromium, or nickel were included into this study and prospectively evaluated to determine the functional outcome, possible signs of hypersensitivity, and short-term survivorship at a minimum follow-up of 1.5 years.Aims

This study investigated the role of multiple injections of allogeneic bone marrow-derived stem cells (BMSCs) to alleviate the progression of osteoarthritic changes in the various structures of the mature rabbit knee in an anterior cruciate ligament (ACL)-deficient OA model. The aim of this study was to evaluate the effects of using a
portable, accelerometer-based surgical navigation system (KneeAlign2)
in total knee arthroplasty (TKA) on the alignment of the femoral
component, and blood loss. A total of 241 consecutive patients with primary osteoarthritis
of the knee were enrolled in this prospective, randomised controlled
study. There were 207 women and 34 men. The mean age of the patients
was 74.0 years (57 to 89). The KneeAlign2 system was used for distal
femoral resection in 121 patients (KA2 group) and a conventional intramedullary

Multimodal infiltration of local anaesthetic provides effective
control of pain in patients undergoing total knee arthroplasty (TKA).
There is little information about the added benefits of posterior
capsular infiltration (PCI) using different combinations of local
anaesthetic agents. Our aim was to investigate the effectiveness
of the control of pain using multimodal infiltration with and without
infiltration of the posterior capsule of the knee. In a double-blind, randomised controlled trial of patients scheduled
for unilateral primary TKA, 86 were assigned to be treated with
multimodal infiltration with (Group I) or without (Group II) PCI.
Routine associated analgesia included the use of bupivacaine, morphine,
ketorolac and epinephrine. All patients had spinal anaesthesia and patient-controlled
analgesia (PCA) post-operatively. A visual analogue scale (VAS)
for pain and the use of morphine were recorded 24 hours post-operatively.
Side effects of the infiltration, blood loss, and length of stay

The Bereiter trochleoplasty has been used in our unit for 12
years to manage recurrent patellar instability in patients with
severe trochlea dysplasia. The aim of this study was to document
the outcome of a large consecutive cohort of patients who have undergone
this operation. Between June 2002 and August 2013, 214 consecutive trochleoplasties
were carried out in 185 patients. There were 133 women and 52 men
with a mean age of 21.3 years (14 to 38). All patients were offered
yearly clinical and radiological follow-up. They completed the following
patient reported outcome scores (PROMs): International Knee Documentation
Committee subjective scale, the Kujala score, the Western Ontario
and McMaster Universities Arthritis Index score and the short-form

The role of arthroscopy in the treatment of soft-tissue
injuries associated with proximal tibial fractures remains debatable.
Our hypothesis was that MRI over-diagnoses clinically relevant associated
soft-tissue injuries. This prospective study involved 50 consecutive
patients who underwent surgical treatment for a split-depression fracture
of the lateral tibial condyle (AO/OTA type B3.1). The mean age of
patients was 50 years (23 to 86) and 27 (54%) were female. All patients
had MRI and arthroscopy. Arthroscopy identified 12 tears of the
lateral meniscus, including eight bucket-handle tears that were
sutured and four that were resected, as well as six tears of the
medial meniscus, of which five were resected. Lateral meniscal injuries
were diagnosed on MRI in four of 12 patients, yielding an overall
sensitivity of 33% (95% confidence interval (CI) 11 to 65). Specificity
was 76% (95% CI 59 to 88), with nine tears diagnosed among 38 menisci
that did not contain a tear. MRI identified medial meniscal injuries
in four of six patients, yielding an overall sensitivity of 67%
(95% CI 24 to 94). Specificity was 66% (95% CI 50 to 79), with 15
tears diagnosed in 44 menisci that did not contain tears. MRI appears to offer only a marginal benefit as the specificity
and sensitivity for diagnosing meniscal injuries are poor in patients
with a fracture. There were fewer arthroscopically-confirmed associated

We studied whether the presence of lateral osteophytes
on plain radiographs was a predictor for the quality of cartilage
in the lateral compartment of patients with varus osteoarthritic
of the knee (Kellgren and Lawrence grade 2 to 3). The baseline MRIs of 344 patients from the Osteoarthritis Initiative
(OAI) who had varus osteoarthritis (OA) of the knee on hip-knee-ankle
radiographs were reviewed. Patients were categorised using the Osteoarthritis
Research Society International (OARSI) osteophyte grading system
into 174 patients with grade 0 (no osteophytes), 128 grade 1 (mild
osteophytes), 28 grade 2 (moderate osteophytes) and 14 grade 3 (severe
osteophytes) in the lateral compartment (tibia). All patients had
Kellgren and Lawrence grade 2 or 3 arthritis of the medial compartment.
The thickness and volume of the lateral cartilage and the percentage
of full-thickness cartilage defects in the lateral compartment was
analysed. There was no difference in the cartilage thickness or cartilage
volume between knees with osteophyte grades 0 to 3. The percentage
of full-thickness cartilage defects on the tibial side increased
from <
2% for grade 0 and 1 to 10% for grade 3. The lateral compartment cartilage volume and thickness is not
influenced by the presence of lateral compartment osteophytes in
patients with varus OA of the knee. Large lateral compartment osteophytes
(grade 3) increase the likelihood of full-thickness cartilage defects

The aim of this study was to compare the maximum
laxity conferred by the cruciate-retaining (CR) and posterior-stabilised
(PS) Triathlon single-radius total knee arthroplasty (TKA) for anterior
drawer, varus–valgus opening and rotation in eight cadaver knees
through a defined arc of flexion (0º to 110º). The null hypothesis
was that the limits of laxity of CR- and PS-TKAs are not significantly
different. The investigation was undertaken in eight loaded cadaver knees
undergoing subjective stress testing using a measurement rig. Firstly
the native knee was tested prior to preparation for CR-TKA and subsequently
for PS-TKA implantation. Surgical navigation was used to track maximal
displacements/rotations at 0º, 30º, 60º, 90º and 110° of flexion.
Mixed-effects modelling was used to define the behaviour of the
TKAs. The laxity measured for the CR- and PS-TKAs revealed no statistically
significant differences over the studied flexion arc for the two
versions of TKA. Compared with the native knee both TKAs exhibited
slightly increased anterior drawer and decreased varus-valgus and
internal-external roational laxities. We believe further study is required
to define the clinical states for which the additional constraint
offered by a PS-TKA implant may be beneficial. Cite this article:
In this paper, we will consider the current role
of simultaneous-bilateral TKA. Based on available evidence, it is
our opinion that bilateral one stage TKR is a safe and efficacious treatment
for patients with severe bilateral arthritic knee disease but should
be reserved for selected patients without significant medical comorbidities.
