Methods
Two Durasul highly crosslinked polyethylene liners were exchanged during revision surgery four and five years after implantation, respectively. The retrieved liners were evaluated macroscopically and surface analysis was performed using optical and electron microscopy. A sample of each liner was used to determine the oxidation of the material by Fourier transform infrared spectroscopy. Samples of the capsule were examined histologically. The annual wear rate was found to be 0.010 and 0.015 mm/year, respectively. Surface analysis showed very little loss of material caused by wear. Histological evaluation revealed a continuous neosynovial lining with single multinucleated foreign-body giant cells. Our findings showed no unexpected patterns of wear on the articulating surfaces up to five years after implantation and no obvious failure of material.

The Birmingham Hip Resurfacing (BHR) arthroplasty has been used as a surgical treatment of coxarthrosis since 1997. We present 20-year results of 234 consecutive BHRs performed in our unit. Between 1999 and 2001, there were 217 patients: 142 males (65.4%), mean age 52 years (18 to 68) who had 234 implants (17 bilateral). Dual-mobility (DM) components are increasingly used to prevent and treat dislocation after total hip arthroplasty (THA). Intraprosthetic dissociation (IPD) is a rare complication of DM that is believed to have decreased with contemporary implants. This study aimed to report incidence, treatment, and outcomes of contemporary DM IPD. A total of 1,453 DM components were implanted at a single academic institution between January 2010 and December 2021: 695 in primary and 758 in revision THA. Of these, 49 presented with a dislocation of the large DM head and five presented with an IPD. At the time of closed reduction of the large DM dislocation, six additional IPDs occurred. The mean age was 64 years (SD 9.6), 54.5% were female (n = 6), and mean follow-up was 4.2 years (SD 1.8). Of the 11 IPDs, seven had a history of instability, five had abductor insufficiency, four had prior lumbar fusion, and two were conversions for failed fracture management.Aims

Wear of the polyethylene (PE) tibial insert of total knee arthroplasty (TKA) increases the risk of revision surgery with a significant cost burden on the healthcare system. This study quantifies wear performance of tibial inserts in a large and diverse series of retrieved TKAs to evaluate the effect of factors related to the patient, knee design, and bearing material on tibial insert wear performance. An institutional review board-approved retrieval archive was surveyed for modular PE tibial inserts over a range of in vivo duration (mean 58 months (0 to 290)). Five knee designs, totalling 1,585 devices, were studied. Insert wear was estimated from measured thickness change using a previously published method. Knee arthroplasty surgery is a highly effective treatment for arthritis and disorders of the knee. There are a wide variety of implant brands and types of knee arthroplasty available to surgeons. As a result of a number of highly publicized failures, arthroplasty surgery is highly regulated in the UK and many other countries through national registries, introduced to monitor implant performance, surgeons, and hospitals. With time, the options available within many brand portfolios have grown, with alternative tibial or femoral components, tibial insert materials, or shapes and patella resurfacings. In this study we have investigated the effect of the expansion of implant brand portfolios and where there may be a lack of transparency around a brand name. We also aimed to establish the potential numbers of compatible implant construct combinations. Hypothetical implant brand portfolios were proposed, and the number of compatible implant construct combinations was calculated.Aims

Hip resurfacing arthroplasty (HRA) is typically indicated for young and active patients. Due to the longevity of arthroplasty, these patients are likely to undergo revision surgery during their lifetime. There is a paucity of information on the long-term outcome of revision surgeries performed after failed HRA. The aim of our study was to provide survivorship data as well as clinical scores after HRA revisions. A total of 42 patients (43 hips) were revised after HRA at our centre to a variety of devices, including four HRA and 39 total hip arthroplasties (THAs). In addition to perioperative complications, University of California, Los Angeles (UCLA) hip scores and 12-Item Short-Form Health Survey questionnaire (SF-12) quality of life scores were collected at follow-up visits after the primary HRA and after revision surgery.Aims

Adverse local tissue reactions associated with abnormal wear considerably slowed down the general use of metal-on-metal (MoM) hip resurfacing arthroplasty (HRA), now limited to a few specialized centres. In this study, we provide the clinical results of 400 consecutive MoM HRAs implanted more than 20 years ago in one such centre. A total of 355 patients (400 hips) were treated with Conserve Plus HRA between November 1996 and November 2000. There were 96 female (27%) and 259 male patients (73%). Their mean age was 48.2 years (SD 10.9). The University of California, Los Angeles (UCLA) hip scores and 12-item Short Form Survey (SF-12) quality of life scores were reported. The complex relationship between acetabular component position and spinopelvic mobility in patients following total hip arthroplasty (THA) renders it difficult to optimize acetabular component positioning. Mobility of the normal lumbar spine during postural changes results in alterations in pelvic tilt (PT) to maintain the sagittal balance in each posture and, as a consequence, markedly changes the functional component anteversion (FCA). This study aimed to investigate the in vivo association of lumbar degenerative disc disease (DDD) with the PT angle and with FCA during postural changes in THA patients. A total of 50 patients with unilateral THA underwent CT imaging for radiological evaluation of presence and severity of lumbar DDD. In all, 18 patients with lumbar DDD were compared to 32 patients without lumbar DDD. The rate of dislocation when traditional single bearing implants are used in revision total hip arthroplasty (THA) has been reported to be between 8% and 10%. The use of dual mobility bearings can reduce this risk to between 0.5% and 2%. Dual mobility bearings are more expensive, and it is not clear if the additional clinical benefits constitute value for money for the payers. We aimed to estimate the cost-effectiveness of dual mobility compared with single bearings for patients undergoing revision THA. We developed a Markov model to estimate the expected cost and benefits of dual mobility compared with single bearing implants in patients undergoing revision THA. The rates of revision and further revision were calculated from the National Joint Registry of England and Wales, while rates of transition from one health state to another were estimated from the literature, and the data were stratified by sex and age. Implant and healthcare costs were estimated from local procurement prices and national tariffs. Natural Language Processing (NLP) offers an automated method to extract data from unstructured free text fields for arthroplasty registry participation. Our objective was to investigate how accurately NLP can be used to extract structured clinical data from unstructured clinical notes when compared with manual data extraction. A group of 1,000 randomly selected clinical and hospital notes from eight different surgeons were collected for patients undergoing primary arthroplasty between 2012 and 2018. In all, 19 preoperative, 17 operative, and two postoperative variables of interest were manually extracted from these notes. A NLP algorithm was created to automatically extract these variables from a training sample of these notes, and the algorithm was tested on a random test sample of notes. Performance of the NLP algorithm was measured in Statistical Analysis System (SAS) by calculating the accuracy of the variables collected, the ability of the algorithm to collect the correct information when it was indeed in the note (sensitivity), and the ability of the algorithm to not collect a certain data element when it was not in the note (specificity).Aims

Earlier studies dealing with trends in the management of osteonecrosis of the femoral head (ONFH) identified an increasing rate of total hip arthroplasties (THAs) and a decreasing rate of joint-preserving procedures between 1992 and 2008. In an effort to assess new trends in the management of this condition, this study evaluated the annual trends of joint-preserving versus arthroplasties for patients aged < or > 50 years old, and the incidence of specific operative management techniques. A total of 219,371 patients with ONFH were identified from a nationwide database between 1 January 2009 and 31 December 2015. The mean age was 54 years (18 to 90) and 105,298 (48%) were female. The diagnosis was made using International Classification of Disease, Ninth revision, Clinical Modification (ICD-9-CM) and Tenth Revision, Clinical Modification (ICD-10-CM) procedure codes. The percentage of patients managed using each procedure during each year was calculated and compared between years. The trends in the use of the types of procedure were also evaluated.Aims

Instability continues to be a troublesome complication after total hip arthroplasty (THA). Patient-related risk factors associated with a higher dislocation risk include the preoperative diagnosis, an age of 75 years or older, high body mass index (BMI), a history of alcohol abuse, and neurodegenerative diseases. The goal of this study was to assess the dislocation rate, radiographic outcomes, and complications of patients stratified as high-risk for dislocation who received a dual mobility (DM) bearing in a primary THA at a minimum follow-up of two years. We performed a retrospective review of a consecutive series of DM THA performed between 2010 and 2014 at our institution (Hospital for Special Surgery, New York, New York) by a single, high-volume orthopaedic surgeon employing a single prosthesis design (Anatomic Dual Mobility (ADM) Stryker, Mahwah, New Jersey). Patient medical records and radiographs were reviewed to confirm the type of implant used, to identify any preoperative risk factors for dislocation, and any complications. Patients and Methods
The aim of this study was to determine the stability of a new
short femoral stem compared with a conventional femoral stem in
patients undergoing cementless total hip arthroplasty (THA), in
a prospective randomized controlled trial using radiostereometric
analysis (RSA). A total of 53 patients were randomized to receive cementless
THA with either a short femoral stem (MiniHip, 26 patients, mean
age: 52 years, nine male) or a conventional length femoral stem
(MetaFix, 23 patients, mean age: 53 years, 11 male). All patients
received the same cementless acetabular component. Two-year follow-up
was available on 38 patients. Stability was assessed through migration
and dynamically inducible micromotion. Radiographs for RSA were
taken postoperatively and at three, six, 12, 18, and 24 months.Aims

Fretting and corrosion at the modular head/neck junction, known
as trunnionosis, in total hip arthroplasty (THA) is a cause of adverse
reaction to metal debris (ARMD). We describe the outcome of revision
of metal-on-polyethylene (MoP) THA for ARMD due to trunnionosis
with emphasis on the risk of major complications. A total of 36 patients with a MoP THA who underwent revision
for ARMD due to trunnionosis were identified. Three were excluded
as their revision had been to another metal head. The remaining
33 were revised to a ceramic head with a titanium sleeve. We describe
the presentation, revision findings, and risk of complications in

This study aimed to evaluate implant survival of reverse hybrid
total hip arthroplasty (THA) at medium-term follow-up. A consecutive series of 1082 THAs in 982 patients with mean follow-up
of 7.9 years (5 to 11.3) is presented. Mean age was 69.2 years (21
to 94). Of these, 194 (17.9%) were in patients under 60 years, 663
(61.3%) in female patients and 348 (32.2%) performed by a trainee.
Head size was 28 mm in 953 hips (88.1%) or 32 mm in 129 hips (11.9%).
Survival analysis was performed and subgroups compared using log

The leading indication for revision total hip
arthroplasty (THA) remains aseptic loosening owing to wear. The younger,
more active patients currently undergoing THA present unprecedented
demands on the bearings. Ceramic-on-ceramic (CoC) bearings have
consistently shown the lowest rates of wear. The recent advances,
especially involving alumina/zirconia composite ceramic, have led
to substantial improvements and good results Alumina/zirconia composite ceramics are extremely hard, scratch
resistant and biocompatible. They offer a low co-efficient of friction
and superior lubrication and lower rates of wear compared with other
bearings. The major disadvantage is the risk of fracture of the
ceramic. The new composite ceramic has reduced the risk of fracture
of the femoral head to 0.002%. The risk of fracture of the liner
is slightly higher (0.02%). Assuming that the components are introduced without impingement,
CoC bearings have major advantages over other bearings. Owing to
the superior hardness, they produce less third body wear and are
less vulnerable to intra-operative damage. The improved tribology means that CoC bearings are an excellent

Large femoral heads have been used with increasing
frequency over the last decade. The prime reason is likely the effect
of large heads on stability. The larger head neck ratio, combined
with the increased jump distance of larger heads result in a greater
arc of impingement free motion, and greater resistance to dislocation
in a provocative position. Multiple studies have demonstrated clear
clinical efficacy in diminishing dislocation rates with the use
of large femoral heads. With crosslinked polyethylene, wear has
been shown to be equivalent between larger and smaller heads. However,
the stability advantages of increasing diameter beyond 38 mm have
not been clearly demonstrated. More importantly, recent data implicates
large heads in the increasing prevalence of groin pain and psoas impingement.
There are clear benefits with larger femoral head diameters, but
the advantages of diameters beyond 38 mm have not yet been demonstrated
clinically.
The primary aim of this independent prospective randomised trial
was to compare serum metal ion levels for ceramic-on-metal (CoM)
and metal-on-metal (MoM) bearing surfaces in total hip arthroplasty
(THA). Our one-year results demonstrated elevation in metal ion
levels above baseline with no significant difference between the
CoM and MoM groups. This paper reviews the five-year data. The implants used in each patient differed only in respect to
the type of femoral head (ceramic or metal). At five-year follow-up
of the 83 enrolled patients, data from 67 (36 CoM, 31 MoM) was available

We report the five-year outcome of a randomised
controlled trial which used radiostereometric analysis (RSA) to assess
the influence of surface oxidised zirconium (OxZr, Oxinium) on polyethylene
wear A total of 120 patients, 85 women and 35 men with a mean age
of 70 years (59 to 80) who were scheduled for primary cemented total
hip arthroplasty were randomly allocated to four study groups. Patients
were blinded to their group assignment and received either a conventional
polyethylene (CPE) or a highly cross-linked (HXL) acetabular component
of identical design. On the femoral side patients received a 28
mm head made of either cobalt-chromium (CoCr) or OxZr. The proximal head penetration (wear) was measured with repeated
RSA examinations over five years. Clinical outcome was measured
using the Harris hip score. There was no difference in polyethylene wear between the two
head materials when used with either of the two types of acetabular
component (p = 0.3 to 0.6). When comparing the two types of polyethylene
there was a significant difference in favour of HXLPE, regardless
of the head material used (p <
0.001). In conclusion, we found no advantage of OxZr over CoCr in terms
of polyethylene wear after five years of follow-up. Our findings
do not support laboratory results which have shown a reduced rate
of wear with OxZr. They do however add to the evidence on the better

Total knee arthroplasty (TKA) is a cost effective
and extremely successful operation. As longevity increases, the demand
for primary TKA will continue to rise. The success and survivorship
of TKAs are dependent on the demographics of the patient, surgical
technique and implant-related factors. Currently the risk of failure of a TKA requiring revision surgery
ten years post-operatively is 5%. The most common indications for revision include aseptic loosening
(29.8%), infection (14.8%), and pain (9.5%). Revision surgery poses
considerable clinical burdens on patients and financial burdens

We present a case of early retrieval of an Oxinium femoral head and corresponding polyethylene liner where there was significant surface damage to the head and polyethylene. The implants were retrieved at the time of revision surgery to correct leg-length discrepancy just 48 hours after the primary hip replacement. Appropriate analysis of the retrieved femoral head demonstrated loss of the Oxinium layer with exposure of the underlying substrate and transfer of titanium from the acetabular shell at the time of a reduction of the index total hip replacement. In addition, the level of damage to the polyethylene was extensive despite only 48 hours The purpose of this report is to highlight the care that is required at the time of reduction, especially with these hard femoral counter-faces such as Oxinium. To our knowledge, the damage occurring at the time of reduction has not been previously reported following the retrieval of an otherwise well-functioning hip replacement.
Surgical interventions consisting of internal
fixation (IF) or total hip replacement (THR) are required to restore
patient mobility after hip fractures. Conventionally, this decision
was based solely upon the degree of fracture displacement. However,
in the last ten years, there has been a move to incorporate patient
characteristics into the decision making process. Research demonstrating
that joint replacement renders superior functional results when compared
with IF, in the treatment of displaced femoral neck fractures, has
swayed the pendulum in favour of THR. However, a high risk of dislocation
has always been the concern. Fortunately, there are newer technologies
and alternative surgical approaches that can help reduce the risk
of dislocation. The authors propose an algorithm for the treatment
of femoral neck fractures: if minimally displaced, in the absence
of hip joint arthritis, IF should be performed; if arthritis is
present, or the fracture is displaced, then THR is preferred. Cite this article:
Non-traumatic osteonecrosis of the femoral head
is a potentially devastating condition, the prevalence of which
is increasing. Many joint-preserving forms of treatment, both medical
and surgical, have been developed in an attempt to slow or reverse
its progression, as it usually affects young patients. However, it is important to evaluate the best evidence that is
available for the many forms of treatment considering the variation
in the demographics of the patients, the methodology and the outcomes
in the studies that have been published, so that it can be used
effectively. The purpose of this review, therefore, was to provide an up-to-date,
evidence-based guide to the management, both non-operative and operative,

The use of large-diameter metal-on-metal (MoM)
components in total hip arthroplasty (THA) is associated with an increased
risk of early failure due to adverse local tissue reaction to metal
debris (ARMD) in response to the release of metal ions from the
bearing couple and/or head-neck taper corrosion. The aim of this
paper was to present a review of the incidence and natural history
of ARMD and the forms of treatment, with a focus on the need for
and extent of resection or debulking of the pseudotumour. An illustrative
case report is presented of a patient with an intra-pelvic pseudotumour
associated with a large diameter MoM THA, which was treated successfully
with revision of the bearing surface to a dual mobility couple and
retention of the well-fixed acetabular and femoral components. The

The ‘jumbo’ acetabular component is now commonly
used in acetabular revision surgery where there is extensive bone
loss. It offers high surface contact, permits weight bearing over
a large area of the pelvis, the need for bone grafting is reduced
and it is usually possible to restore centre of rotation of the
hip. Disadvantages of its use include a technique in which bone
structure may not be restored, a risk of excessive posterior bone
loss during reaming, an obligation to employ screw fixation, limited
bone ingrowth with late failure and high hip centre, leading to increased
risk of dislocation. Contraindications include unaddressed pelvic
dissociation, inability to implant the component with a rim fit,
and an inability to achieve screw fixation. Use in acetabulae with
<
50% bone stock has also been questioned. Published results
have been encouraging in the first decade, with late failures predominantly because
of polyethylene wear and aseptic loosening. Dislocation is the most
common complication of jumbo acetabular revisions, with an incidence
of approximately 10%, and often mandates revision. Based on published results,
a hemispherical component with an enhanced porous coating, highly
cross-linked polyethylene, and a large femoral head appears to represent

We reviewed the literature on the currently available
choices of bearing surface in total hip replacement (THR). We present
a detailed description of the properties of articulating surfaces
review the understanding of the advantages and disadvantages of
existing bearing couples. Recent technological developments in the
field of polyethylene and ceramics have altered the risk of fracture
and the rate of wear, although the use of metal-on-metal bearings has
largely fallen out of favour, owing to concerns about reactions
to metal debris. As expected, all bearing surface combinations have

The purpose of this study was to compare the
amount of acetabular bone removed during hip resurfacing (HR) and cementless
total hip replacement (THR), after controlling for the diameter
of the patient’s native femoral head. Based on a power analysis,
64 consecutive patients (68 hips) undergoing HR or THR were prospectively

compare the rates of wear of conventional and cross-linked (XL) polyethylene
(PE) in total hip arthroplasty (THA) have described their use with
a cementless acetabular component. We conducted a prospective randomised study to assess the rates
of penetration of two distinct types of PE in otherwise identical
cemented all-PE acetabular components. A total of 100 consecutive patients for THA were randomised to
receive an acetabular component which had been either highly XL
then remelted or moderately XL then annealed. After a minimum of eight years follow-up, 38 hips in the XL group
and 30 hips in the annealed group had complete data (mean follow-up
of 9.1 years (7.6 to 10.7) and 8.7 years (7.2 to 10.2), respectively).

The aim of this randomised controlled study was
to compare functional and radiological outcomes between modern cemented
and uncemented hydroxyapatite coated stems after one year in patients
treated surgically for a fracture of the femoral neck. A total of

to be treated with a cemented Exeter stem or an uncemented Bimetric
stem. The patients were reviewed at four and 12 months. The cemented group performed better than the uncemented group

We report patient-reported outcomes and complications
associated with contemporary periacetabular osteotomy (PAO) surgery
in treating symptomatic acetabular dysplasia and compare these outcomes
with total hip arthroplasty (THA) in patients with similar demographic
details. Two consecutive cohorts included patients between aged
18 to 40 years who had undergone either PAO (100 hips; 24 male,
76 female) or THA (55 hips; 18 male, 37 female). At a mean follow-up
of 5.9 years (2 to 13), there was significant improvement in the
modified Harris hip pain (p <

Bioengineering reasons for increased wear and failure of metal-on-metal (MoM) bearings in hip prostheses have been described. Low wear occurs in MoM hips when the centre of the femoral head is concentric with the centre of the acetabular component and the implants are correctly positioned. Translational or rotational malpositioning of the components can lead to the contact-patch of the femoral component being displaced to the rim of the acetabular component, resulting in a ten- to 100-fold increase in wear and metal ion levels. This may cause adverse tissue reactions, loosening of components and failure of the prosthesis.
A number of studies have reported satisfactory
results from the isolated revision of an acetabular component. However,
many of these studies reported only the short- to intermediate-term
results of heterogeneous bearing surfaces in a mixed age group. We present our experience of using a ceramic-on-ceramic (CoC)
bearing for isolated revision of an uncemented acetabular component
in 166 patients (187 hips) who were under the age of 50 years at
the time of revision. There were 78 men and 88 women with a mean
age of 47.4 years (28 to 49). The most common reason for revision
was polyethylene wear and acetabular osteolysis in 123 hips (66%),
followed by aseptic loosening in 49 hips (26%). We report the clinical and radiological outcome, complication
rate, and survivorship of this group. The mean duration of follow-up
was 15.6 years (11 to 19). The mean pre-operative Harris hip score was 33 points (1 to 58),
and improved to a mean of 88 points (51 to 100) at follow-up. The
mean pre-operative total Western Ontario and McMaster Universities
Osteoarthritis Index score was 63.2 (43 to 91) and improved to 19.8
points (9 to 61) post-operatively. Overall, 153 of 166 patients
(92%) were satisfied with their outcome. Kaplan–Meier survivorship
analysis, with revision or radiological evidence of implant failure
(13 patients, 8%) as end-points, was 92% at 15 years (95% confidence
interval 0.89 to 0.97). Isolated revision of a cementless acetabular component using

Increased femoral head size may reduce dislocation rates following total hip replacement. The National Joint Registry for England and Wales has highlighted a statistically significant increase in the use of femoral heads ≥ 36 mm in diameter from 5% in 2005 to 26% in 2009, together with an increase in the use of the posterior approach. The aim of this study was to determine whether rates of dislocation have fallen over the same period. National data for England for 247 546 procedures were analysed in order to determine trends in the rate of dislocation at three, six, 12 and 18 months after operation during this time. The 18-month revision rates were also examined. Between 2005 and 2009 there were significant decreases in cumulative dislocations at three months (1.12% to 0.86%), six months (1.25% to 0.96%) and 12 months (1.42% to 1.11%) (all p <
0.001), and at 18 months (1.56% to 1.31%) for the period 2005 to 2008 (p <
0.001). The 18-month revision rates did not significantly change during the study period (1.26% to 1.39%, odds ratio 1.10 (95% confidence interval 0.98 to 1.24), p = 0.118). There was no evidence of changes in the coding of dislocations during this time. These data have revealed a significant reduction in dislocations associated with the use of large femoral head sizes, with no change in the 18-month revision rate.
We carried out a prospective randomised study designed to compare the penetration rate of acetabular polyethylene inserts of identical design but different levels of cross-linking at a minimum of four years follow-up. A total of 102 patients (102 hips) were randomised to receive either highly cross-linked Durasul, or contemporary Sulene polyethylene inserts at total hip replacement. Dislocation remains among the most common complications
of, and reasons for, revision of both primary and revision total
hip replacements (THR). Hence, there is great interest in maximising
stability to prevent this complication. Head size has been recognised
to have a strong influence on the risk of dislocation post-operatively.
As femoral head size increases, stability is augmented, secondary
to an increase in impingement-free range of movement. Larger head
sizes also greatly increase the ‘jump distance’ required for the
head to dislocate in an appropriately positioned cup. Level-one
studies support the use of larger diameter heads as they decrease
the risk of dislocation following primary and revision THR. Highly cross-linked
polyethylene has allowed us to increase femoral head size, without
a marked increase in wear. However, the thin polyethylene liners
necessary to accommodate larger heads may increase the risk of liner
fracture and larger heads have also been implicated in causing soft-tissue
impingement resulting in groin pain. Larger diameter heads also
impart larger forces on the femoral trunnion, which may contribute
to corrosion, metal release, and adverse local tissue reactions.
Alternative large bearings including large ceramic heads and dual
mobility bearings may mitigate some of these risks, and several

Dysplasia of the hip, hypotonia, osteopenia,
ligamentous laxity, and mental retardation increase the complexity
of performing and managing patients with Down syndrome who require
total hip replacement (THR). We identified 14 patients (six males,
eight females, 21 hips) with Down syndrome and degenerative disease
of the hip who underwent THR, with a minimum follow-up of two years

The Oxford unicompartmental knee replacement (UKR) was designed to minimise wear utilising a fully-congruent, mobile, polyethylene bearing. Wear of polyethylene is a significant cause of revision surgery in UKR in the first decade, and the incidence increases in the second decade. Our study used model-based radiostereometric analysis to measure the combined wear of the upper and lower bearing surfaces in 13 medial-compartment Oxford UKRs at a mean of 20.9 years (17.2 to 25.9) post-operatively. The mean linear penetration of the polyethylene bearing was 1.04 mm (0.307 to 2.15), with a mean annual wear rate of 0.045 mm/year (0.016 to 0.099). The annual wear rate of the phase-2 bearings (mean 0.022 mm/year) was significantly less (p = 0.01) than that of phase-1 bearings (mean 0.07 mm/year). The linear wear rate of the Oxford UKR remains very low into the third decade. We believe that phase-2 bearings had lower wear rates than phase-1 implants because of the improved bearing design and surgical technique which decreased the incidence of impingement. Previous studies of failure mechanisms leading
to revision total knee replacement (TKR) performed between 1986 and
2000 determined that many failed early, with a disproportionate
amount accounted for by infection and implant-associated factors
including wear, loosening and instability. Since then, efforts have
been made to improve implant performance and instruct surgeons in
best practice. Recently our centre participated in a multi-centre evaluation
of 844 revision TKRs from 2010 to 2011. The purpose was to report
a detailed analysis of failure mechanisms over time and to see if
failure modes have changed over the past 10 to 15 years. Aseptic
loosening was the predominant mechanism of failure (31.2%), followed
by instability (18.7%), infection (16.2%), polyethylene wear (10.0%),
arthrofibrosis (6.9%) and malalignment (6.6%). The mean time to
failure was 5.9 years (ten days to 31 years), 35.3% of all revisions
occurred at less than two years, and 60.2% in the first five years.
With improvements in implant and polyethylene manufacture, polyethylene
wear is no longer a leading cause of failure. Early mechanisms of
failure are primarily technical errors. In addition to improving
implant longevity, industry and surgeons must work together to decrease
these technical errors. All reports on failure of TKR contain patients
with unexplained pain who not infrequently have unmet expectations.
Surgeons must work to achieve realistic patient expectations pre-operatively,

Total hip replacement in patients with Gaucher’s disease with symptomatic osteonecrosis of the femoral head is controversial because of the high early failure rates. We describe four patients who had an uncemented total hip replacement following enzyme replacement therapy for a median of two years and one month (1 to 9.8 years) prior to surgery, and who remained on treatment. At operation, the bone had a normal appearance and consistency. Histopathological examination showed that, compared with previous biopsies of untreated Gaucher’s disease, the Gaucher cell infiltrate had decreased progressively with therapy, being replaced by normal adipose tissue. The surfaces of viable bone beyond the osteonecrotic areas showed osteoblasts, indicating remodelling. In one case acetabular revision was carried out after 11 years and eight months. The three remaining patients had a mean follow-up of six years and four months (3.3 to 12 years). We recommend initiating enzyme replacement therapy at least one to two years prior to total hip replacement to facilitate bone remodelling and to allow implantation of uncemented components in these young patients.
There is little in the literature on the level
of participation in sports which patients undertake after total
hip replacement (THR). Our aims in this study were to determine
first, the level of sporting activity, second, the predictive factors
for returning to sporting activity, and third, the correlation between
participation in sports and satisfaction after THR. We retrospectively
identified 815 patients who had undergone THR between 1995 and 2005. All
were asked to complete a self-administered questionnaire regarding
their sporting activity. A total of 571 patients (71%) met the inclusion
criteria and completed the evaluation. At a mean follow-up of 9.8
years ( In conclusion, we found that most patients participate in sporting
activity after THR, regardless of the advice of their surgeon, and
that there is a correlation between the level of participation and
pre-operative function, motivation, duration of symptoms and post-operative

Large ceramic femoral heads offer several advantages
that are potentially advantageous to patients undergoing both primary
and revision total hip replacement. Many high-quality studies have
demonstrated the benefit of large femoral heads in reducing post-operative instability.
Ceramic femoral heads may also offer an advantage in reducing polyethylene wear

The treatment of hip dysplasia should be customised
for patients individually based on radiographic findings, patient
age, and the patient’s overall articular cartilage status. In many
patients, restoration of hip anatomy as close to normal as possible
with a PAO is the treatment of choice. Cite this article:
Symptomatic hip osteonecrosis is a disabling
condition with a poorly understood aetiology and pathogenesis. Numerous
treatment options for hip osteonecrosis are described, which include
non-operative management and joint preserving procedures, as well
as total hip replacement (THR). Non-operative or joint preserving
treatment may improve outcomes when an early diagnosis is made before
the lesion has become too large or there is radiographic evidence
of femoral head collapse. The presence of a crescent sign, femoral
head flattening, and acetabular involvement indicate a more advanced-stage
disease in which joint preserving options are less effective than
THR. Since many patients present after disease progression, primary
THR is often the only reliable treatment option available. Prior
to the 1990s, outcomes of THR for osteonecrosis were poor. However,
according to recent reports and systemic reviews, it is encouraging
that with the introduction of newer ceramic and/or highly cross-linked
polyethylene bearings as well as highly-porous fixation interfaces,
THR appears to be a reliable option in the management of end-stage
arthritis following hip osteonecrosis in this historically difficult

We have reviewed 70 Harris-Galante uncemented acetabular components implanted as hybrid hip replacements with cemented stems between 1991 and 1995 in 53 patients whose mean age was 40 years (19 to 49). The mean follow-up was for 13.6 years (12 to 16) with no loss to follow-up. We assessed the patients both clinically and radiologically. The mean Oxford hip score was 20 (12 to 46) and the mean Harris hip score 81 (37 to 100) at the final review. Radiologically, 27 hips (39%) had femoral osteolysis, 13 (19%) acetabular osteolysis, and 31 (44%) radiolucent lines around the acetabular component. Kaplan-Meier survival curves were constructed for the outcomes of revision of the acetabular component, revision of the component and polyethylene liner, and impending revision for progressive osteolysis. The cumulative survival for revision of the acetabular component was 94% (95% confidence interval 88.4 to 99.7), for the component and liner 84% (95% confidence interval 74.5 to 93.5) and for impending revision 55.3% (95% confidence interval 40.6 to 70) at 16 years. Uncemented acetabular components with polyethylene liners undergo silent lysis and merit regular long-term radiological review.
Arthritis of the hip in the young adult can be a disabling condition. Recent years have witnessed extensive research related to the management of this condition. This article reviews the current status with regard to aetiology, diagnosis and treatment of arthritis of the hip in the young adult.
Osteolysis remains a common reason for revision
after total hip arthroplasty (THA). For osteolysis associated with loose
cups, revision is indicated. For osteolysis around a well-fixed
cup, the decision is more controversial. The data available data
support retention of the cupwith lesional treatment, working through
screw holes and access channels for debridement and grafting. The
choice of graft material to fill defects, if any, remains controversial. Several
studies demonstrate good survivorship with cup retention strategies.
Complete revision allows more complete debridement of the lesion
and better graft fill, and allows implantation of a modern cup,
typically with a full line of liners and bearing surfaces available.
Additionally, revision allows fine tuning of the orientation of
the cup, which may be advantageous for optimising hip stability.
The author prefers to retain a well-fixed cup if it meets the following
criteria: it is well-fixed to intra-operative testing, it is well-positioned,
it is of sufficient size to allow insertion of a new liner with
a reasonable head size, new liners are available, and the hip is
stable to intra-operative trialing after liner insertion.
In this paper, we will consider the current role
of metal-on-metal bearings by looking at three subtypes of MoM hip
arthroplasty separately: Hip resurfacing, large head (>
36 mm) MoM
THA and MoM THA with traditional femoral head sizes.
A moderator and panel of five experts led an
interactive session in discussing five challenging and interesting patient
case presentations involving surgery of the hip. The hip pathologies
reviewed included failed open reduction internal fixation of subcapital
femoral neck fracture, bilateral hip disease, evaluation of pain
after metal-on-metal hip arthroplasty, avascular necrosis, aseptic
loosening secondary to osteolysis and polyethylene wear, and management
of ceramic femoral head fracture.
Between 1999 and 2001, 90 patients underwent
total hip replacement using the same uncemented acetabular and femoral
components with a 28 mm metallic femoral head but with prospective
randomisation of the acetabular liner to either Durasul highly cross-linked
polyethylene or nitrogen-sterilised Sulene polyethylene. We assessed
83 patients at a minimum follow-up of ten years. Linear penetration
of the femoral head was estimated at six weeks, six and 12 months
and annually thereafter, using the Dorr method, given the non-spherical
shape of the acetabular component. There was no loosening of any component; only one hip in the
Sulene group showed proximal femoral osteolysis. The mean penetration
of the femoral head at six weeks was 0.08 mm (0.02 to 0.15) for
the Durasul group and 0.16 mm (0.05 to 0.28) for the Sulene group
(p = 0.001). The mean yearly linear penetration was 64.8% lower
for the Durasul group at 0.05 mm/year ( Cite this article:
