Background:

For the past four decades controversy surrounds the decision to retain or sacrifice the posterior cruciate ligament during a total knee arthroplasty. To our knowledge no study has been done to describe the effect of releasing the PCL on the range of motion of the knee.

Background

When positioning and rotating the femoral cutting block (AP) on the femur it can either be done according to bony landmarks (measured resection) or by tensioning the flexion gap and positioning it parallel to the tibia (gap balanced technique.) Accurate rotation of the femoral component is essential to ensure a symmetric flexion gap to ensure optimal tibio-femoral kinematics and patello-femoral tracking.

Methods

74 consecutive total knee replacements were assessed intra-operatively for symmetry of the flexion gap by applying a varus and a valgus stress and digitally recording the opening with a computer assisted navigation system. External rotation of the femoral component according to the bony landmarks was measured radiologically. This was compared to the external rotation suggested by the navigation intra-operatively using a gap balanced workflow.

Background

Cup inclination is a major factor in the success of a total hip replacement. An open cup position can lead to dislocation or increased wear from rim loading and a closed cup position lead to impingement against the femoral neck or psoas. Although the ideal inclination for cup position is recommended as between 40 and 45 degrees, accurate positioning of the implant might be influenced by pelvic flexion and movement of the patient's pelvis during the procedure. We wanted to examine if the transvers acetabular ligament (TAL) could be used to determine cup inclination intra-operatively.

Methods

16 hips from 9 cadaveric specimens were used for the study. A computer navigation system (Brain lab) was used to measure and document the exact inclination and version of the acetabular trial component in three positions: flush with the transvers acetabular ligament (TAL), with the rim of the cup 5 mm from the TAL in a cranial direction and with the rim of the cup 5 mm caudally displaced. Statistical analysis of the results was performed by the Department of Biostatistics.

Background:

We studied the effect of posterior condylar offset on maximum knee flexion after a posterior stabilised total knee arthroplasty. We also looked at gender difference and the post-operative change in posterior condylar offset.

Methods:

Eighty consecutive computer navigated posterior stabilised total knee replacements were prospectively assessed intra-operatively for maximum knee flexion. The flexion angle was measured and recorded with an imageless computer navigation system (Brainlab) before and after implantation of the prosthesis. This was correlated with a radiological review of the posterior condylar offset pre- and post-operatively, as defined by posterior condylar offset ratio (PCOR) originally described by Soda (2007) and modified by the Bristol Knee Group (2010).

Background:

During the past two decades the medial Patellofemoral ligament has come to the fore as the essential lesion of acute patella dislocation and its reconstruction in cases of chronic instability seems logical. The femoral insertion of the medial Patellofemoral ligament (MPFL) is key to the isometry or desired anisometry of the reconstruction. Radiographic landmarks for the femoral insertion has been described in literature most notably by Schottle et al. AJSM 2007. We examined the consistency of these landmarks of the femoral insertion of the MPFL.

Methods:

24 unpaired knees of cadavaric specimen were dissected for the origin of the MPFL.

A radiographic marker was then placed in the centre of the femoral attachment of the MPFL and a direct lateral X-ray obtained of the distal femur. The sweet spot was defined according to the landmarks described by Schottle et al and deviation from the sweet spot was measured.

Background

Leg length discrepancy real or perceived remains an important source of patient dissatisfaction after a total hip replacement. Pre-operative templating and intra-operative measurement has to be used to ensure an accurate restoration of the normal centre of rotation of the hip as well as equal leg lengths. Theoretically more bone has to be resected from the femur to maintain the centre of the femoral head in the same location. This is due to a smaller size of the prosthetic femoral head compared to the native femoral head. It was postulated that this was an accurate predictor of leg length after a total hip replacement.

Methods

56 consecutive patients who underwent a total hip replacement (cemented, uncemented or hybrid) had intra-operative measurements documented of their femoral head-neck resection distance. This was compared with the measurement of the femoral prosthesis that was inserted. A telephonic survey of the patients perceived leg equality as well as a radiological measurement of their actual leg lengths on a standing AP X-ray at 6 weeks post-operatively was done.
