Acute kidney injury is a recognised post-operative complication in primary joint replacement. Recently it has been demonstrated that antibiotic regimen can significantly impact on the proportion of patients who develop acute kidney impairment post-operatively. Within our unit an increased rate of acute kidney injury had been noted post-operatively over the last 5 years. This increase followed the introduction of a rapid recovery protocol for arthroplasty patients. Our aim was determine whether we could identify a causative factor or those who were at increased risk of post-operative renal impairment. Data were collected for 413 patients initially retrospectively but continued prospectively. Univariable and multivariable analysis was performed to determine any causative factors. The primary increase was 150% increase in baseline creatinine, but as some authors recognise an increase in 125% this was also analysed.Introduction

Peritrochanteric femur fractures are common and impose major costs on the healthcare system. A fixed angle sliding hip screw is the principle method of treatment, but the rate of mechanical failure associated with these devices can be high; the usual mechanism being the collapse of the neck shaft angle leading to extrusion of the screw from the femoral head, commonly known as “cutout”. Many variables contribute to the risk of cutout, however there is substantial evidence that the “Tip Apex Distance” (a single number that summarises the position and depth of the lag screw on anteroposterior and lateral radiographs after controlling for magnification) is the single best predictor of risk of failure. There is a strong statistical relationship between an increasing tip apex distance (TAD) and the rate of cutout. Specifically, a TAD of 25 mm or less significantly reduces risk of failure. The aim of our study is to assess levels of compliance with a maximum TAD of 25 mm in peritrochanteric femur fractures treated with Dynamic Hip Screws and IM Nails. We retrospectively measured the TAD of 45 patients who had undergone DHS or IM nails in the previous 6 months.Introduction and Aims

The aim of this study is to assess the discrepancy between weight bearing long leg radiographs and supine MRI alignment. There is currently increasing interest in the use of MRI to assess knee alignment and develop custom made cutting blocks utilising this data. However in almost all units MRI scans are performed supine and it is recognised that knee alignment can alter with weight bearing. 46 patients underwent MRI scans as pre-operative planning for Biomet signature total knee replacement and the measure of varus or valgus deformity on MRI was obtained from the plan produced by Biomet Signature software system. 41 of these patients had long leg weight bearing radiographs performed. 37 of these radiographs were amenable to measuring the knee alignment on the picture archiving and communication system (PACS). These measurements were performed by two assessors and inter-observer reliability was satisfactory. There was a significant difference between the alignment as measured on supine MRI compared with weight bearing long leg films. In knee arthroplasty one of the aims is to correct the biomechanical axis of the knee and one of the appeals of custom made cutting blocks is that this can be achieved more easily. However it is important to realise that alignment is not a static value and thus correcting supine alignment may not necessarily result in correction of weight bearing alignment.
It is well recognised that patients with diabetes mellitus have a predisposition towards stenosing flexor tenosynovitis (FTS). However, recent research has suggested an association between the development of FTS and haemoglobin A1c (HbA1c) level which is used as a marker of glycaemic control. National guidelines on management of diabetes suggest treatment should aim to maintain HbA1c at <6.5%. The aim of our study is to quantify glycaemic control in patients undergoing surgical A1 pulley release. We retrospectively reviewed the blood results of 78 patients who underwent FTS surgery. 27 of these had an HbA1c checked within 6 months of their surgery and we therefore presumed these patients were diabetic. For diabetic patients the average HbA1c was 7.9% (range 5.3–11.4) and only 7 of the 27 patients had an HbA1c within the recommended range. In this cohort 33% of patients were presumed diabetic and 74% of these had a documented HbA1c above the national target suggesting a significant number presenting for surgery have poor glycaemic control. Therefore it may be of benefit to screen for this in patients undergoing FTS surgery.
Post-operative pain is well recognised in patients undergoing shoulder surgery. With the recent advances in arthroscopic shoulder surgery over the last decade, a larger number of cases are being performed in day surgery units. These procedures are generally performed under general anaesthetic with either an interscalene or suprascapular nerve block or local anaesthetic infiltration. The aim of our prospective audit was to investigate the adequacy of analgesia provided for patients, undergoing day case arthroscopic shoulder procedures in a rural district general hospital, to ensure best medical care and to tailor certain procedures to appropriate analgesic pathways in the future. Fifty consecutive patients, who underwent day case arthroscopic shoulder surgery, were contacted by telephone one week post surgery, to assess their post-operative pain scores and analgesic requirements. Patients who received a nerve block were found to have a significantly longer duration of pain relief (p < 0.001). These patients also had significantly less pain performing their usual activities of daily living in the immediate post-operative period (p = 0.05), compared to patients who only had local anaesthetic infiltration. There was no trend found between the type of procedure and post-operative pain scores. Our audit has confirmed that nerve blocks provide longer pain relief, but has also highlighted the need to take into consideration pre-operative pain and pain perception to enable analgesia to be tailored.
This case report describes a patient with thoracic plasmacytoma, an uncommon haematological malignancy, who presented with neck pain. Plasmacytoma is a neoplastic proliferation of B cell lineage but is much less common than multiple myeloma. The histological examination of multiple myeloma and plasmacytoma is identical however in plasmacytoma there is a solitary lesion with negative skeletal survey, negative bone marrow aspirate and little or no myeloma protein detected in the blood. This makes it more challenging to diagnose and a high index of suspicion is required.
We present a case of a 14 year old who sustained an isolated injury to her foot while horse riding. X-rays demonstrated a medial and plantar dislocation at the level of the talo-navicular and calcaneo-cuboid joint, with associated fractures of the cuboid and navicular. This was treated initially with open reduction and fixation with kirschner wires as the injury was grossly unstable and reduction difficult to maintain with casting alone. CT scan was then performed prior which confirmed satisfactory reduction of the dislocation and fixation with the k wires so these were left in situ and the navicular fracture reduced and fixed with a barouk screw. The Chopart joint was first described by French surgeon Francois Chopart as the talo-navicular and calcaneo-cuboid joints were a practical level for amputation. Injury here is a rare but missed in 40% at presentation. Pure dislocation occurs in 10–25% with most having concomitant fractures. The Chopart joint has critical role in balance and stability in normal gait. Early recognition allows prompt reduction and fixation of these injuries which has been associated with a better outcome. However these are severe injuries and patients should be counselled on potential long term functional impairment even with optimal management.

We aim to compare revision rates and functional outcome scores in between two contemporary total knee arthoplasty systems. A search was carried out of a prospectively maintained outcomes database to identify all patients receiving total knee replacement (TKR) in the department between 2005 and 2006, when evaluation of different knee arthroplasty systems was being undertaken. Data on the first 93 consecutive patients receiving a Scorpio (Stryker, Allendale, NJ) implant and the first 93 consecutively receiving a PFC (DePuy, Warsaw, IN) implant were reviewed. This study looks for correlation between pre-operative MRI and intra-operative surgical findings regarding the site of medial patellofemoral ligament (MPFL) rupture in first time patella dislocaters with the aim of confirming the reliability of this modality of imaging. A retrospective analysis of all patients who were referred for MRI following patella dislocation was performed. The surgical and MRI findings of those who subsequently underwent MPFL repair were compared to look for any correlation.Aim
Methods
Distal radial fractures are amongst the most common trauma referrals, however controversy remains regarding their optimum management. We undertook a retrospective review of the management of distal radial fractures in our department. The prospectively maintained trauma database was used to identify patients admitted for operative management of a dorsally displaced distal radial fracture between June 2008 and June 2009. Only extra-articular or simple intra-articular fractures were included (AO classification A2/A3/C1/C2). Operation notes were reviewed to determine the method of fixation. Patients were contacted by post and asked to complete a functional outcome score - Disabilities of the Arm, Shoulder and Hand (DASH). A further 12 patients with similar fractures who had been managed conservatively were also asked to complete a DASH score to provide a comparison between operative and non-operative management. 98 patients were identified - 67 female, 31 male. Mean age was 51 years, range 15-85 years. All patients were at least 1 year post-op. 26 patients had manipulation under anaesthesia (MUA). 48 patients had MUA and K-wire fixation, which was supplemented with synthetic bone substitute in 16 cases. 3 patients had MUA and bone graft and 21 patients had open reduction and internal fixation (ORIF) with a volar plate. 34 correctly completed DASH scores were returned. A lower score equates to a better functional outcome. Mean DASH scores were: MUA 14.8; MUA+K-wire 13.1; ORIF 13.6; conservative 47.1. This data would indicate that patients with a significantly displaced distal radial fracture have a better functional outcome with operative management to improve the fracture alignment. However, all of the methods of fixation used resulted in similar functional outcomes at one year.
The medial patellofemoral ligament is commonly torn in first patella dislocation and according to the literature occurs most frequently at the femoral insertion. As research has demonstrated the MPFL to be a primary soft tissue stabilizer of the patella there has been increased interest in acute repair of the ligament with the aim of reducing the risk of recurrent dislocation. MRI is generally used pre op to identify the site of rupture allowing planning of the surgical repair, however there is currently no published data regarding the correlation between surgical and MRI findings to confirm the reliability of this modality of imaging. A retrospective analysis of all patients who were referred for MRI following patella dislocation was performed. The surgical and MRI findings of those who subsequently underwent MPFL repair were compared to look for any correlation. 41 patients were first time patella dislocators and 38 had MPFL rupture diagnosed on MRI. Of these 19 progressed to surgical intervention and 17 had adequate data available from the notes and to permit comparison. All of the MPFLs ruptured at the patella insertion. In 76.5% (13/17) of patients the site of MPFL rupture was correctly identified on MRI scan. Of those that were discordant 3/4 had the site of rupture identified as the femoral insertion on MRI and on 1/4 the MRI suggested rupture at both the femoral and patella insertions. MPFL rupture is common in first time dislocations occurring in 93%% of this cohort. There was a clear predominance patella insertion rupture in our study which contrasts with the majority of the published literature. By correlating surgical and radiological findings we can conclude that while MRI can accurately diagnose MPFL rupture at the patella insertion but it is less accurate in identifying rupture at the femoral insertion.
