Introduction

Revision Total Knee Arthroplasty (TKA) is becoming increasingly prevalent as the number of TKA procedures grow in a younger, higher-demand population. Factors associated with patients requiring multiple revision TKAs are not yet well understood. The purpose of this study is to investigate the epidemiology of re-revision TKA, and identify risk factors that are associated with failure of re-revision TKA.

Methods

A retrospective analysis was performed on 358 patients who underwent revision TKA at a single institution between 1/2012 and 12/2013. Patients who underwent revision knee arthroplasty two or more times were included. Patients were excluded if their indication for the first revision was periprosthetic joint infection (PJI). Patient demographics, surgical indications, revision details, and available follow-up information were collected. Re-revision failure was defined as the need for any additional operative intervention. A logistic regression analysis was performed to assess for significant predictors of re-revision failure.

Introduction

The epidemiology of re-revision total hip arthroplasty (THA) is not well understood. The purpose of this study is to investigate the epidemiology of re-revision THA, and identify risk factors that are associated with failure of re-revision THA.

Methods

A retrospective analysis was performed on 288 patients who underwent revision THA at a single institution between 1/2012 and 12/2013. Patients who underwent revision hip arthroplasty two or more times were included. Patients were excluded if their indication for their first revision was due to periprosthetic joint infection (PJI). Patient demographics, surgical indications, revision details, and available follow-up information were collected through the electronic medical record. Re-revision failure was defined as the need for any additional return to the operating room, regardless of indication. A logistic regression analysis was performed to assess for significant predictors of re-revision failure.

Introduction

Reducing readmissions after total joint arthroplasty (TJA) is challenging. Pre-operative risk stratification and optimization pre surgical care may be helpful in reducing readmission rates after primary TJA. Assessment of the predictive value of individual modifiable risk factors without a tool to properly stratify patients may not be helpful to the surgical community to reduce the risk of readmission. We developed a scoring system: Readmission Risk Assessment Tool (RRAT) as part of a Perioperative Orthopaedic Surgical Home model that allows for risk stratification in patients undergoing elective primary TJA at our institution. We analyzed the relationship between the RRAT score and readmission following primary hip or knee arthroplasty.

Methods

The RRAT, which is scored incrementally based on the number and severity of modifiable comorbidities was used to generate readmission scores for a cohort of 207 readmitted and 2 cohorts of 234 (random and age-matched) non-readmitted patients each. Regression analysis was performed to assess the strength of association between individual risk factors, RRAT score and readmissions. We also calculated the odds and odds ratio (OR) at each level of RRAT score to identify patients with relatively higher risk of readmission.

The use of smart trial components is now allowing a better assessment of soft tissue balancing at the time of total knee replacement surgery. A balanced knee can be defined as one that possesses symmetry, ie. equal and centered lateral and medial forces through the full range of flexion. There is still a need for a standard reproducible surgical test to quickly confirm optimized balancing at surgery with such devices. The Heel Push test is the established standard, by pushing the foot in a cephalad direction while supporting the thigh and keeping the leg stable in the vertical plane. A common variation of this test is the Thigh Pull test where the foot is actively assisted during the cephalad pull of the thigh through deep flexion. The test is an open chain test. The Thigh Pull test may be an improvement since the weight of the leg is alleviated and no supplemental compressive forces are introduced. The directional changes of the lower extremity are thus a result of ligamentous tension and balances. The purpose of this study is to compare the two tests using a standard testing methodology and observe the variation in kinetic parameters in a controlled biomechanical setting.

A custom l rig was developed, which independently controls all six degrees of freedom about the knee joint. In addition a commercial navigation system was used to derive instantaneous alignment values and flexion angles between the tibia and femur. The pelvis was fixed to the table and the foot was fitted onto a low friction carriage along a slide rail. The knee design used was cruciate retaining. The pressure mapping system was a wireless tibial trial that provided magnitude of load per compartment.

The study is a preliminary cadaveric study reporting the data from two. In this experiment the leg was then tested with the Heel Push and Thigh Pull tests after obtaining optimum soft tissue balance of the cadaveric specimen. From this standard neutral state a series of single surgical variables were introduced to mimic common intra-operative surgical corrections. This was achieved through custom tibial liner and angle shims.

The results defied theoretical anticipation. Though the total contact forces with heel push were generally higher than with thigh pull, the relative load distribution between compartments did not follow a trend (see Figure 1). Furthermore in deeper flexion the persistence of relatively high contact pressures would suggest that ligaments still generate intra-articular forces despite the much weaker gravitational effect. The clinical relevance lies in the asymmetry of the load distribution between medial and lateral compartment for the two methods tested. The load asymmetry as tested by the Thigh Pull test may correspond to an open chain in swing phase. This asymmetry would force some axial rotation and tibial femoral alignment deviation that can significantly affect the forces at the time of heel strike. The Heel Push test would be more representative of the compressive forces in a closed chain mode as seen during the stance phase of gait.

Soft tissue balancing in total knee replacement may well be the determining factor in raising the fair patient satisfaction. The development of intelligent implants allows quantification of reactive loads to applied pressures. This can be tested in dynamic mode such as heel push test at surgery, or in static mode such as when testing for varus/valgus (VV) laxity of the collateral ligaments of the knee. We postulate that a well-balanced knee will have comparable if not equal load distribution across compartments in dynamic loading. When tested for laxity, we anticipate an equal or comparable response to VV applied loads under physiologic load range of 10–50N. This study sought to analyze the relationship between the kinematic (joint motion) and kinetic (force) effects to VV testing in the 0–15 degrees range of flexion. One goal was to demonstrate that testing the knee in locked extension (Screw Home effect) is unreliable and should be abandoned in favor of the more reliable VV testing at 10–15 degrees of flexion.

This is a preliminary cadaveric study utilizing data from two hemibodies. The pelvis was fixed in a custom test rig with open or closed chain lower leg testing capability along a sliding rail with foot VV translational. Forces were applied at the malleoli with a wireless hand held dynamometer. Kinematic analysis of the hip-knee-ankle (HKA) tibiofemoral angle was derived from a commercial navigation system with mounted infrared trackers. Kinetic analysis was derived from a commercially available sensor imbedded in a tibial trial liner. Balance was optimized by conventional methods with the use of the sensor feedback until loads were roughly symmetrical and VV testing yielded symmetrical rise in opposite compartments. The VV testing was then performed with the knees locked at the femoral side in axial rotation and translational motion in any plane. Sagittal flexion was pre-set at 0, 10, and 15 degrees and progressive load was applied.

Results

From the graphs one can observe significant differences between VV testing at 0 degrees (locked Screw Home), 10 degrees, and 15 degrees of flexion. The shaded area corresponds to the common range of VV stress testing loading pressure, typically less than 35N. The HKA deviates from neutrality no sooner than by the middle of the physiologic test zone. By 35N, the magnitude of the effect is also much less than that observed at 10 and 15 degrees (unlocked from Screw Home). From the kinetic analysis one can also note the significant difference in the High-Low spread throughout the testing range of applied pressure.

If the surgeon tests in the low range of applied loads, he/she may not observe the kinematic joint opening effect. The kinetic effect seems more reliable as sensed loads are detectable earlier on. It is clear however that testing at 10–15 degrees offers a much better sensitivity to the VV laxity or stiffness as exemplified in the bottom portions of the figure. Therefore testing in locked Screw Home full extension may lead to underestimation of the true coronal laxity of the joint.

Introduction

Acetabular component position is an important determinant of stability, wear and impingement following total hip arthroplasty (THA). Its optimum position and size in direct anterior approach (DAA) THA has not been clearly described in previous studies. Our aim was to study the evolution of the same with reference to stability and impingement as a part of a single surgeon's learning curve.

Methods

Clinical and radiographic records of first 300 consecutive DAA THAs performed by a single surgeon from April 2009 to April 2011 were reviewed from a prospective database at a single center. Radiographic analysis was done by two observers to determine acetabular inclination and anteversion on 6 week postoperative standing radiographs. Native femoral head size, measured on preoperative radiographs after adjusting for magnification, was used to calculate the native acetabular cup size. The study population was divided into three groups; Group A– 1st 100 DAA THA cases, Group B – 2nd 100 and Group C – 3rd 100 corresponding to the use of intraoperative anterior stability assessment (Group B and C) and change in the cup size strategy (Group C). The incidence of instability and psoas impingement (PI) –related groin pain at 2 year follow-up was determined for the three groups. Statistical analysis was done to see if there were differences in these clinical and radiographic outcome measures in the three groups.

Introduction:

Direct anterior approach (DAA) total hip arthroplasty (THA) has been reported to be a muscle sparing approach. The purpose of this study was to compare gait patterns over time between patients undergoing THA via DAA and posterior approach (PA).

Methods:

22 patients with unilateral primary hip osteoarthritis were prospectively enrolled and gait analysis was performed prior to, at 6 months and 1 year following THA via DAA and PA. All PA THA's were performed by a single surgeon from January 2008 to February 2009; all DAA THA's were performed by the same surgeon at the same institution from January 2010 to May 2011 with similar design of uncemented acetabular, femoral components and bearing surfaces. Reflective markers were placed on the lower extremity and motion data collected using six infrared cameras (Qtrac, Qualysis). Ground reaction forces were recorded with a multicomponent force plate (Kistler). A repeated-measures ANOVA was used to compare changes in gait parameters over time. Harris Hip Score was used to quantify pain and function.

The aim was to study the evolution of radiographic patterns of osteointegration of tapered wedge stems and determine if there is correlation with bony morphology and initial stem fit.

We reviewed primary total hip replacements performed by two surgeons using a single cementless tapered wedge design and that had a complete series of radiographs (defined as preoperative, 6 weeks, 1 year and 5 or more years).

Signs of bony remodeling were recorded at each Gruen zone. Calcar remodeling, changes in cortical thickness, evidence of subsidence and pedestal formation were recorded (Figure 1).

57 hips (50 patients) were available for analysis with a mean follow up of 5.03 years. Mean changes in cortical thickness were positive in zones 2(7.51) and 6(5.36) and negative in zones 1(−7.53) and 7(−13.51). Radiolucent lines were found in gruen zones 3,4(39%), and 5. Femoral neck cancellization was seen in zone 7 in 8 patients (14%) in year 1 and 36 (63%) by year 5.

Correlations were seen with proximal canal fill and radiolucent lines at zones 3 (0.278; p0.36) and 5 (0.258; p 0.05) and with distal canal fill and hypertrophy of the cortex in zone 3 (0.429; p0.001) and 5.

Cortical hypertrophy around the midstem, lack of radiolucent lines around the proximal stem and cancellization of the calcar are all radiographic patterns which occur routinely. A positive correlation with distal canal fill and hypertrophy of zones 3 and 5 was noted.

There was no significant correlation with preoperative boney morphology or initial stem fit proximally.

INTRODUCTION

Acetabular cup orientation is an important element of Total Hip arthroplasty (THA). The purpose of this retrospective case-control study was to compare variability of acetabular cup placement between THA performed via Direct Anterior Approach (DAA) with fluoroscopy in supine position and posterior approach (PA) in lateral position without use of fluoroscopy.

METHODS

Radiographic and clinical records of THAs performed by a single, high volume arthroplasty surgeon at one institution were reviewed. Patients with similar design of uncemented acetabular cup, femoral component and bearing surface were included to form two groups. PA group consisted of 300 THAs performed from May 2006 to June 2009. DAA group consisted of 300 THAs performed from Oct 2009 to Oct 2011 excluding first 100 cases to eliminate the influence of learning curve. Radiographic analysis was done by two independent blinded observers to determine cup inclination and anteversion (Liaw et al) on standardized, 6 week postoperative, standing anteroposterior pelvic radiographs using Picture Archiving and Communication System software (PACS).

Introduction:

The incidence of heterotrophic ossification after primary total hip arthoplasty (THA) has been reported to be between 8 to 90%. The incidence is higher in lateral approach because of extensive muscular trauma associated with it. There exists limited data on the incidence of heterotrophic ossification after direct anterior approach (DAA) THA. The purpose of this study was to assess the incidence of heterotrophic ossification after THA via the direct anterior approach and the influence of surgical technique and chemoprophylaxis.

Method:

A consecutive series of four hundred two primary uncemented direct anterior approach total hip arthoplasties in 378 patients were reviewed for incidence of heterotrophic ossification. In the first 200 total hip arthoplasties an anterior capsulectomy (Group 1) was done for exposure while in the subsequent 202 total hip arthoplasties a capsulotomy (Group 2) followed by complete release of supero-lateral flap of from its attachement to the gluteus minimus muscle and trochanter was performed (Figure 1). Group 1 received warfarin for thromboprophylaxis; while aspirin (thromboprophylaxis) and celecoxib (pain) was used in group 2. Heterotrophic ossification was classified according to Brooker's classification on plain radiographs.

INTRODUCTION:

Short femoral stems are receiving increasing attention for less invasive total hip replacement (THR). The purpose of this study is to report our initial experience with a short modular femoral (SMF) stem at a minimum two-year follow up.

METHODS:

Twenty-six primary total hip arthroplasties (THAs) using the SMF stem were performed between August 2009 and January 2010. Patients were evaluated clinically with Harris Hip Score (HHS) and radiographically up to two years. Radiographs were analyzed to determine the degree of stem migration from its initial postoperative position. These parameters were compared to those of a cohort of 54 patients implanted with a monolithic tapered wedge stem from the same manufacturer over the same time period.

Introduction:

Subsidence of cementless femoral stems in total hip arthroplasty (THA) has been associated with poor initial fixation and subsequent risk of aspectic loosening. There is limited literature on how subsidence of cementless, proximally porous coated, tapered wedge femoral stems impacts the patient clinically. The aim of our study was to assess whether subsidence with these stems is associated with a decline in clinical function.

Method:

A review of a prospectively collected database of THAs performed by a single surgeon at one institution using two cementless, tapered wedge stem designs from January 2006 to June 2010 was performed. Radiographic analysis using Picture Archiving and Communications System (PACS) was used to identify patients with greater than 1.5 mm of subsidence, and to document osseointegration. Preoperative and postoperative pain and Harris hip scores were recorded; and analyzed to identify if the clinical recovery pattern of the subsidence versus no subsidence groups differed. Protected weight bearing was recommended to all patients with subsidence.

Introduction

Patellar mobilisation methods used during total knee arthroplasty (TKA) have been debated in the literature, with some proponents of minimally invasive TKA suggesting that laterally retracting, rather than everting the patella may be beneficial. It was our hypothesis that by using randomised, prospective, blinded study methods, there would be no significant difference in clinical outcome measures based solely on eversion of the patella during total knee arthroplasty.

Methods

After an a priori power analysis was done, 120 primary total knee replacements indicated for degenerative joint disease were included in the study and randomised to one of two patella exposure techniques: lateral retraction or eversion. Short-term outcomes were evaluated during hospitalisation and included time to return of straight leg raise (SLR), ambulation distance, and length of hospital stay. Long-term outcome values were evaluated pre-operatively, at 6 weeks, 3 months, and 1 year post-operatively, and included leg extension strength measured by dynamometer, knee range of motion (ROM), Visual Analog Scale (VAS) pain before and after knee motion, circumferential thigh measurements, and SF-36 Physical and Mental Component Scores (PCS, MCS). All collaborating investigators were blinded to each other's data. Surgical techniques and perioperative arthroplasty management protocols were those routinely and currently used during total knee replacement surgery at our institution.

INTRODUCTION

Osteoarthritis (OA) of the hip and the knee has been found to affect sexual activity. Few retrospective studies have investigated the role of total hip replacement on sexual function. We designed a prospective study to evaluate the influence of total hip arthroplasty (THA) and total knee arthroplasty (TKA) on the physical and psychological aspects of sexuality of patients.

METHODS

Between April 2009 and April 2011, patients under 70 years of age scheduled for primary THA or TKA for OA were invited to anonymously participate in this study. All patients were recruited from the practices of 2 arthroplasty surgeons (at the same institution) by mailing the 2 questionnaires, 2 pre-paid self-addressed envelopes and a cover letter. Patients were instructed to mail back the pre-operative questionnaire prior to surgery and the post- operative questionnaire 6 months after surgery. Enrolled patients were also contacted after 1 year of surgery to fill a similar questionnaire. Responses were identified only by a unique code number on the questionnaire.

INTRODUCTION

Cadaveric studies have reported damage to the direct head of rectus femoris and tensor fascia lata muscles with direct anterior approach(DAA) and to the abductors, external rotators with posterior approach(PA). The aim of this prospective study was to evaluate differences in hip muscle strength recovery between DAA and posterior approach (PA) THA.

METHODS

Patients with unilateral hip osteoarthritis undergoing THA at a single institution from January 2011 to October 2011 were enrolled. All DAA THA's were performed by one surgeon, and all PA THA's were performed by another surgeon with similar design of components, pain management and rehabilitation protocols. Hip muscle strength was measured with a handheld dynamometer in all planes by a single observer preoperatively, at 6 weeks, 3 months and 6 months. Functional recovery was assessed with the motor component of Functional Independence Measure, UCLA activity score, Harris hip score, SF-12 score.
