Due to the opioid epidemic, our service developed a cultural change highlighted by decreasing discharge opioids after lower extremity arthroplasty. However, concern of potentially increasing refill requests exists. As such, the goal of this study was to analyze whether decreased discharge opioids led to increased postoperative opioid refills. We retrospectively reviewed 19,428 patients undergoing a primary hip or knee arthroplasty at a single institution from 2016–2019. Patients that underwent secondary procedures within that timeframe were excluded. Two-thousand two-hundred and forty-one patients (12%) were on narcotics preoperatively or had chronic pain syndrome. Two reductions in routine discharge narcotics were performed over this timeframe. First, 8,898 patients routinely received 750 morphine milligram equivalents (MMEs). After the first reduction, 4,842 patients routinely received 520 MMEs. After a second reduction, 5,688 patients routinely received 320 MMEs. We analyzed refill rates, refill MMEs, and whether discharge MMEs were associated with refill MMEs in a multivariate model.Introduction

There remains confusion in the literature with regard to the spinopelvic relationship, and its contribution to ideal acetabular component position. Critical assessment of the literature has been limited by use of conflicting terminology and definitions of new concepts that further confuse the topic. In 2017, the concept of a Hip-Spine Workgroup was created with the first meeting held at the American Academy of Orthopedic Surgeons Annual Meeting in 2018. The goal of this workgroup was to first help standardize terminology across the literature so that as a topic, multiple groups could produce literature that is immediately understandable and applicable. This consensus review from the Hip-Spine Workgroup aims to simplify the spinopelvic relationship, offer hip surgeons a concise summary of available literature, and select common terminology approved by both hip surgeons and spine surgeons for future research. Variation in pelvic tilt during postural changes may affect functional alignment. The primary objective of this study was to quantify the changes in lumbo-pelvic-femoral alignment from sitting to standing in patients undergoing THA. 144 patients were enrolled. Standing and sitting radiographs using the EOS imaging system were analyzed preoperatively and 1-year postoperatively. Pelvic incidence (PI), lumbar lordosis (LL), sacral slope (SS), proximal femoral angle (PFA) and spine/femoroacetabular flexion were determined. 38 patients had multilevel DDD (26%). Following THA, patients sat with increased anterior pelvic tilt demonstrated by a significant increase in sitting lumbar lordosis (28° preop vs 35° postop; p<0.01) and sacral slope (18° vs 23°; p<0.01). Following THA, patients flexed less through their spines (preop 26° vs postop 19°; p<0.01) and more through their hips (femoroacetabular flexion) (preop 60° vs postop 67°; p<0.01) to achieve sitting position. Patients with multilevel DDD sat with less spine flexion (normal 22° vs spine 13°; p<0.01), less change in sacral slope (more relative anterior tilt) (17° vs 9°; p<0.01), and more femoroacetabular flexion (64° vs 71°; p<0.01). For the majority of patients after THA, a larger proportion of lumbo-pelvic-femoral flexion necessary to achieve a sitting position is derived from femoroacetabular flexion with an associated increase in anterior pelvic tilt and a decrease in lumbar spine flexion. These changes are more pronounced among patients with multilevel DDD. Surgeons may consider orienting the acetabular component with greater anteversion and inclination in patients identified preoperatively to have anterior pelvic tilt or significant DDD.
