Aims

Global literature suggests that female surgical trainees have lower rates of independent operating (operative autonomy) than their male counterparts. The objective of this study was to identify any association between gender and lead/independent operating in speciality orthopaedic trainees within the UK national training programme.

Methods

This was a retrospective case-control study using electronic surgical logbook data from 2009 to 2021 for 274 UK orthopaedic trainees. Total operative numbers and level of supervision were compared between male and female trainees, with correction for less than full-time training (LTFT), prior experience, and time out during training (OOP). The primary outcome was the percentage of cases undertaken as lead surgeon (supervised and unsupervised) by UK orthopaedic trainees by gender.

Australian Football (AF) is a popular sport in Australia, with females now representing one-third of participants. Despite this, the injury profiles of females versus males in largely unknown. The current study investigated fractures, dislocations and tendon ruptures in females versus males presenting to emergency departments (ED) with an AF injury.

All patients, regardless of age, presenting to one of 10 EDs in Victoria, Australia, with an AF injury were included. Data were prospectively collected over a 10-month period, coinciding with a complete AF season. Data were extracted from patient medical records regarding injury-type, body-part injured and treatments required. Female and male data were compared with chi-squared tests.

Of the 1635 patients presenting with an AF injury, 595 (36.0%) had a fracture, dislocation or tendon rupture, of whom 85 (14.3%) were female and the average age was 20.5 years (SD 8.0). Fractures accounted for most injuries (n=478, 80.3% of patients had a fracture), followed by dislocations (n=118, 19.8%) and tendon ruptures (n=14, 2.4%). Upper limb fractures were more common than lower limb fractures (71.1% v 11.5% of fractures). Females were more likely to fracture their hands or fingers than males (45.7% v 34.3%). Males were more likely to fracture ribs (5.4% v 0%). Most fractures (91.2%) were managed in the ED, with the remainder being admitted for surgery (GAMP/ORIF). Males were more likely to be admitted for surgery than females (11.2% v 5.9%). Regarding dislocations (n=118), females were more likely to dislocate the patella (36.8% v 8.1% of dislocations). Only males sustained a tendon rupture (n=14): finger extensor or flexor (57.1%), achilles tendon (28.6%) and patella tendon (14.3%).

Orthopaedic AF injuries are common presentations to EDs in Victoria, though few require specialist orthopaedic intervention. Injury profiles differed between genders suggesting that gender specific injury prevention and management might be required.

Osteoarthritis (OA) is a disease of the synovial joint with synovial inflammation, capsular contracture, articular cartilage degradation, subchondral sclerosis and osteophyte formation contributing to pain and disability. Transcriptomic datasets have identified genetic loci in hip and knee OA demonstrating joint specificity. A limited number of studies have directly investigated transcriptional changes in shoulder OA. Further, gene expression patterns of periarticular tissues in OA have not been thoroughly investigated. This prospective case control series details transcriptomic expression of shoulder OA by analysing periarticular tissues in patients undergoing shoulder replacement for OA as correlated with a validated patient reported outcome measure of shoulder function, an increasing (clinically worsening) QuickDASH score. We then compared transcriptomic expression profiles in capsular tissue biopsies from the OA group (N=6) as compared to patients undergoing shoulder stabilisation for recurrent instability (the control group, N=26). Results indicated that top ranked genes associated with increasing QuickDASH score across all tissues involved inflammation and response to stress, namely interleukins, chemokines, complement components, nuclear response factors and immediate early response genes. Some of these genes were upregulated, and some downregulated, suggestive of a state of flux between inflammatory and anti-inflammatory signalling pathways. We have also described gene expression pathways in shoulder OA not previously identified in hip and knee OA, as well as novel genes involved in shoulder OA.

In Australia nearly two-thirds of arthroplasty procedures are performed in the private setting, which is disproportionate to the dimensioning 43.5% of the population with private health cover. The rapid growth of shoulder arthroplasty surgery will be absorbed by both private and public sectors. This study aimed to assess the influence of healthcare setting on elective shoulder arthroplasty outcomes, defined by revision rate, and functional measures.

Data was collected on all primary procedures performed from 2004 – 2019 within a regional area of Victoria, Australia. Patients were categorised into private or public settings. Trauma cases for acute proximal humerus fractures were excluded. The primary outcome of revision surgery was recorded as a cumulative percentage, and survival analysis conducted to calculate a hazard ratio (HR). Functional outcomes were measured through range-of-motion (ROM) and multiple validated patient-reported-outcome-measures (PROMs).

458 patients were identified in the study: 290 private and 168 public. There was no difference in the revision rate (3.8% private, 4.8% public), with an adjusted HR of 1.25 (p=0.66) for public compared to private. Baseline and post-operative functional measures were significantly greater in the private setting for ROMs and PROMs analysis, in particular post-operative QuickDASH (15.9±14.7 to 32.7±23.5; p<0.001) and Oxford Shoulder Score (42.6±6.3 to 35.7±11.2; p<0.001). However, there was no significant difference for any of the functional measures in the amount of change from baseline to 12-months between settings.

Although healthcare setting does not appear to influence revision rate for shoulder arthroplasty, clear differences were demonstrated for functional measures both pre and post operatively. This may be attributed to factors such as access to perioperative rehabilitation and should be an area to target future investigations.

The progressive painful and disabling predicament of patients with severe osteoarthritis awaiting a total hip or knee arthroplasty (THA/TKA) results in a decline in muscle mass, strength and function also known as Sarcopenia.

We conducted a cross-sectional, prospective study of patients on the waiting-list for a THA/TKA in the South Australian public healthcare system and compared the findings to healthy participants and patients newly referred from their general practitioners. Participants with a history of joint replacements, pacemakers and cancers were excluded from this study. Outcomes of this study included (i) sarcopenia screening (SARC-F ≥4); (ii) sarcopenia, defined as low muscle strength (hand grip strength M<27kg; F<16kg), low muscle quality (skeletal muscle index M<27%, F<22.1%) and low physical performance (short physical performance battery ≤8). Additional outcomes include descriptions of the recruitment feasibility, randomisation and suitability of the assessment tools.

29 healthy controls were recruited; following screening, 83% (24/29) met the inclusion criteria and 75% (18/24) were assessed. 42 newly referred patients were recruited; following screening, 67% (30/45) met the inclusion criteria and 63% (19/30) were assessed. 68 waiting list patients were recruited; following recruitment, 24% (16/68) met the inclusion criteria and 75% (12/16) were assessed. Preliminary data shows increasing waiting time is associated with higher SARC-F scores, lower hand grip strength and lower muscle quality.

As a pilot study, preliminary data demonstrate that: (1) study subjects’ willingness to participate will enable a larger study to be conducted to establish the prevalence of sarcopenia and the diagnostic cut-off points for this patient group. (2) SARC-F is a suitable tool to screen for sarcopenia. (3) There is a positive correlation between waiting time for a THA/TKA and sarcopenia.


We compared the rate of revision of two classes of primary anatomic shoulder arthroplasty, stemmed (aTSA) and stemless (sTSA) undertaken with cemented all polyethylene glenoid components.

A large national arthroplasty registry identified two cohort groups for comparison, aTSA and sTSA between 1st January 2011 and 31st December 2020. A sub-analysis from 1 January 2017 captured additional patient demographics. The cumulative percentage revision (CPR) was determined using Kaplan-Meier estimates of survivorship and hazard ratios (HR) from Cox proportional hazard models adjusted for age and gender.

Of the 7,533 aTSA procedures, the CPR at 8 years was 5.3% and for 2,567 sTSA procedures was 4.0%. There was no difference in the risk of revision between study groups (p=0.128).

There was an increased risk of revision for aTSA and sTSA undertaken with humeral head sizes <44mm (p=0.006 and p=0.002 respectively). Low mean surgeon volume (MSV) (<10 cases per annum) was a revision risk for aTSA (p=0.033) but not sTSA (p=0.926).

For primary diagnosis osteoarthritis since 2017, low MSV was associated with an increased revision risk for aTSA vs sTSA in the first year (p=0.048). Conversely, low MSV was associated with a decreased revision risk for sTSA in the first 6 months (p<0.001). Predominantly aTSA was revised for loosening (28.8%) and sTSA for instability/dislocation (40.6%).

Revision risk of aTSA and sTSA was associated with humeral head size and mean surgeon volume but not patient characteristics. Inexperienced shoulder arthroplasty surgeons experience lower early revision rates with sTSA in the setting of osteoarthritis. Revision of aTSA and sTSA occurred for differing reasons.

Aims

The UK government declared a national lockdown on 23 March 2020 to reduce transmission of COVID-19. This study aims to identify the effect of lockdown on the rates, types, mechanisms, and mortality of musculoskeletal trauma across Scotland.

Methods

Data for all musculoskeletal trauma requiring operative treatment were collected prospectively from five key orthopaedic units across Scotland during lockdown (23 March 2020 to 28 May 2020). This was compared with data for the same timeframe in 2019 and 2018. Data collected included all cases requiring surgery, injury type, mechanism of injury, and inpatient mortality.

Nearly a quarter of screws cause damage during insertion by stripping the bone, reducing pullout strength by over 80%. Studies assessing surgically achieved tightness have predominately shown that variations between individual surgeons can lead to underpowered investigations. Further to the variables that have been previously explored, several basic aspects related to tightening screws have not been evaluated with regards to how they affect screw insertion. This study aims to identify the achieved tightness for several variables, firstly to better understand factors related to achieving optimal intraoperative screw purchase and secondly to establish improved methodologies for future studies.

Two torque screwdrivers were used consecutively by two orthopaedic surgeons to insert 60 cortical, non-locking, stainless-steel screws of 3.5 mm diameter through a 3.5 mm plate, into custom-made 4 mm thick 20 PCF sheets of Sawbone, mounted on a custom-made jig. Screws were inserted to optimal tightness subjectively chosen by each surgeon. The jig was attached to a bench for vertical screw insertion, before a further 60 screws were inserted using the first torque screwdriver with the jig mounted vertically, enabling horizontal screw insertion. Following the decision to use the first screwdriver to insert the remaining screws in the vertical position for the other variables, the following test parameters were assessed with 60 screws inserted per surgeon: without gloves, double surgical gloves, single surgical gloves, non-sterile nitrile gloves and, with and then without augmented feedback (using digitally displayed real-time achieved torque). For all tests, except when augmented feedback was used, the surgeon was blinded to the insertion torque. Once the stopping torque was reached, screws were tightened until the stripping torque was found, this being used to calculate tightness (stopping/stripping torque ratio). Screws were recorded to have stripped the material if the stopping torque was greater than the stripping torque. Following tests of normality, Mann-Whitney-U comparisons were performed between and combining both surgeons for each variable, with Bonferroni corrections for multiple comparisons.

There was no significant (p=0.29) difference in the achieved tightness between different torque screw drivers nor different jig positions (p=0.53). The use of any gloves led to significant (p < 0 .001) increases in achieved tightness compared to not using gloves for one surgeon but made no difference for the other (p=0.38–0.74). Using augmented feedback was found to virtually eliminate stripping. For one surgeon average tightness increased significantly (p < 0 .001) when torque values were displayed from 55 to 75%, whilst for the other, this was associated with significantly decreases (p < 0 .001), 72 to 57%, both surgeons returned to their pre-augmentation tightness when it was removed.

Individual techniques make a considerable difference to the impact from some variables involved when inserting screws. However, the orientation of screws insertion and the type of screwdriver did not affect achieved screw tightness. Using visual feedback reduces rates of stripping and investigating ways to incorporate this into clinical use are recommended. Further work is underway into the effect of other variables such as bone density and cortical thickness.

We introduce a novel active tensioning system that can be used for dynamic gap-based implant planning as well as for assessment of final soft tissue balance during implant trialing. We report on the concept development and preliminary findings observed during early feasibility testing in cadavers with two prototype systems.

System description

The active spacer (fig 1) consists of a motorized actuator unit with integrated force sensors, independently actuated medial and lateral upper arms, and a set of modular attachments for replicating the range of tibial baseplate and insert trial sizes. The spacer can be controlled in either force or position (gap) control and is integrated into the OMNIBoticsTM Robotic-assisted TKA platform (OMNI, MA, USA).

Cadaver Study

Two design iterations were evaluated on eleven cadaver specimens by seven orthopaedic surgeons in three separate cadaver labs. The active spacer was used in a tibial-first technique to apply loads and measure gaps prior to and after femoral resections. To determine the range of forces applied on the spacer during a varus/valgus assessment procedure, each surgeon performed a varus/valgus stress test and peak medial and lateral forces were measured. Surgeons also rated the feel of the stability of the knee at 50N and 80N of preload using the following scale: 1 – too loose; 2 – slightly loose; 3 – ideal; 4 slightly tight; 5 – too tight. Final balanced was assessed with the spacer and with manual trial components.

Introduction

Total hip arthroplasty has become an increasingly common procedure. Improper cup position contributes to bearing surface wear, pelvic osteolysis, dislocations, and revision surgery. The incidence of cup malposition outside of the safe zone (40° ± 10° abduction and 15° ± 10° anteversion) using traditional techniques has been reported to be as high as 50%. Our hypothesis is that computer assisted navigation will improve cup placement in total hip arthroplasty compared with traditional techniques.

Methods

This study retrospectively evaluated the position of 425 consecutive cups placed during primary total hip arthroplasty performed over a two-year period, from 8/1/2012 to 8/1/2014. All cups were placed with a direct-anterior muscle-sparing approach with computer-assisted imageless navigation by a single surgeon. Real-time intraoperative “screen shots” were taken of cup placement. Standard antero-posterior postoperative radiographs of the pelvis were taken within 6 weeks of surgery in the operating surgeon's office using the same standardized protocol for each patient. The radiographs were evaluated by two separate investigators for final abduction and anteversion utilizing the same method as other studies. Statistics were descriptive in nature.

Introduction

Knee arthroplasty is an effective intervention for painful arthritis when conservative measures have failed. Despite recent advances in component design and implantation techniques, a significant proportion of patients experience problems relating to the patella-femoral joint (PFJ).

Detailed knowledge of the shape and orientation of the normal and replaced femoral trochlea groove is critical when considering potential causes of anterior knee pain. Furthermore, to date it has proved difficult to establish a diagnosis due to shortcomings in current imaging techniques for obtaining satisfactory coronal plane motion data of the patella in the replaced knee.

The aim of this study was to correlate the trochlea shape of normal and replaced knees with corresponding coronal plane PFJ kinematic data.

Method

Bony and cartilagenous trochlea geometries from 3T MRI scans of 20 normal healthy volunteers were compared with both anatomical and standard total knee replacements (TKR) and patellofemoral joint replacement (PFJR) geometries. Following segmentation and standardized alignment, the path of the apex of the trochlea groove was measured using customized Matlab software. (Fig1).

Next, kinematic data of the 20 normal healthy volunteers (Normal) was compared with that of 20 TKR, and 20 PFJR patients using the validated MAUSTM system (Motion Analysis and UltraSound) comprising a 12-camera, motion capture system used to capture images of reflective markers mounted on subjects lower limbs and an ultrasound probe. A mapping between the ultrasound image and the motion capture system allows the ultrasound probe to be used to determine the locations of the patella relative to bony landmarks on the femur during a squat exercise.

The aim of this study was to document the survivorship and patient reported outcome of the Avon patello-femoral replacement in a consecutive series with follow up of 10 years or more.

All cases performed in Bristol from 1996 onwards were prospectively recorded. Follow up was at 1,2,5,7,10,12 and 15 years with the Bristol Patella Score, the Oxford and WOMAC scores and SF12. Implant survival was analysed using the Kaplan-Meier method.

There were 323 PFJ replacements (280 individuals). Follow up was available for 286 cases in 250 patients (89% follow up). The 10 year survival rate was 77%, falling to 67% at 15 years. The most common reason for revision was tibio-femoral progression (45/74 revisions), with loosening or polyethylene wear recorded in 8 cases. The best results were seen in the youngest and the oldest patients. Good improvements were seen in PROMs, with the mean OKS improving from 19.5 to 34.1 at 2 years and 32.7 at the 15 years.

The Avon patello-femoral knee replacement is a successful long-term treatment for isolated patello-femoral knee osteoarthritis, although further improvements are expected in subsequent series, particularly as indications for surgery have evolved over time.

To audit adherence to the British Orthopaedic Association and British Association of Plastic, Reconstructive and Aesthetic Surgeons 2009 review of the acute management of severe open lower limb fractures (BOAST4).

Audit: Retrospective study of adherence to BOAST4 standards of 37 cases treated at Derriford Hospital between March 2010 and 2011.

Intervention: a multidisciplinary approach towards the management of such injuries. Assigning department-specific roles across all specialities involved and establishing an early patient transfer pathway from non-specialist centres in the region.

Re-audit: Prospective data collection of adherence to BOAST4 of 18 cases between October 2011 and April 2012.

Eleven of 15 standards were comparable. Statistical analysis was performed using Fisher's exact test (p≤0.05). Six standards showed statistically significant improvement: antibiotic therapy; regular assessment of neurovascular status; urgent surgery for contaminated wounds; initial wound care; appropriate splinting and early patient transfers. One standard was adhered to in all cases. 3 standards showed improvement, although not statistically significant and 1 standard remained consistently low in adherence.

Although doctors are familiar with BOAST4, many are not aware of the specific details. A coordinated and informed multidisciplinary team has enabled the successful application of an evidenced-based approach to the management these injuries.

The WHO surgical safety checklist was introduced at Derriford Hospital in 2009. Evidence of the effect on efficiency has been slow to appear in the literature.

Using a standardised, locally modified WHO surgical safety checklist theatre list capacity, start and finish times as well as time between cases was measured in 4 elective orthopaedic theatres. Data from 3 successive years was analysed retrospectively: prior to checklist introduction, during checklist introduction and routine checklist use.

Data was analysed using the One-Way ANOVA with post-hoc Tukey test. The number of cases per list showed a statistical difference from Year 1–2 and this increase was sustained in Year 3. The number of delayed starts showed no difference between Year 1&2, followed by a statistically significant decrease in Year 3. The number of late finishes showed a statistical difference from Year 1–2 and this decrease was sustained in Year 3. However, the number of lost minutes between cases showed no difference between Year 1&2, followed by an increase in Year 3.

This study demonstrates that pre-list briefings combined with the WHO surgical checklist can improve theatre list capacity and prompt starts, reduce the number of overruns, however fails to improve turnaround time between cases.

Previous attempts to measure coronal plane patellofemoral kinematics following knee replacement have suffered from methodological drawbacks; the patella being obscured by the components, metal artefact and technical inaccuracies. The aim of this study was to assess whether there was any significant difference in the patellofemoral kinematics between normal, TKR and PFJR patients using the validated MAUS™ technique (combining motion analysis with ultrasound).

60 patients were recruited into three groups; normal healthy volunteers (Normal), TKR, and PFJR patients. The MAUS technique incorporates a 12 camera analysis system (providing gross alignment data for tibial and femoral segments) and an ultrasound probe (providing coordinates of bony landmarks on patella femur and tibia) during a squat exercise. 6 DOF kinematics were described between 0 and 90° flexion. The validated accuracy of the MAUS technique registering the ultrasound images within the motion capture system is 1.84 mm (2 × SD).

Movements of the Normal group were significantly different from the TKR group (p=0.03) and the PFJR group (p<0.01), whilst there was no significant difference between the TKR and PFJR groups (p=0.27).

Our data suggest that many aspects of patellofemoral kinematics are absent following TKR and PFJR, which could be addressed in future designs of knee TKR and PFJR.

As part of a prospective RSA study into a new design of short-stem, trabecular metal, Co-Cr femoral components with modular necks and metal on polyethylene articulation, three patients presented with almost identical symptoms of pain and a large avascular mass within the operated hip. All three cases came to exploration and similar findings of a solid pseudotumour within the hip joint were found in all thee cases. Infection was not present. All cases were submitted to histology and reviewed by a pathologist with specific interest in tissue reactions to foreign materials. All three cases showed histological features classical of published descriptions of ALVAL (Aseptic Lymphocyte-dominated Vasculitis Associated Lesion). A consistent finding was fretting corrosion of the neck-stem junction. These cases demonstrate that Co and Cr ions can provoke adverse tissue reactions even in the absence of metal/metal articulations. This has design implications for all modular Co-Cr necks. The stem used in this study has been voluntarily withdrawn from the Australian market but is still being implanted in Europe. We make recommendations that modular trunnion-style Co-Cr necks not be used and alternative materials and design solutions be sought.

Introduction

Lisfranc joint injuries account for only 0.2% of all fractures but early diagnosis improves the chances of a better outcom]. Radiographic signs, such as diastasis (>2mm) of the 1–2 interspace, are subtle and. often missed, leading to a poorer outcome. We present a new radiographic sign, less likely to result in missed diagnoses. Null nypothesis: The intermetarsal angle remains unchanged in Lisfranc injuries.

Method

A series of radiographs demonstrating Lisfranc injury were interspersed with normal and postoperative cases. Evidence of fixation was obscured. A selection of Foundation and Core Trainee medical staff measured the intermetatarsal angle (IMA) on two separate occasions. The measuring technique was demonstrated with no explanation for the reason behind the measurement.

Unicompartmental Knee Replacement (UKR) is associated with fewer complications, faster recovery and better function than Total Knee Replacement (TKR). However, joint registries demonstrate a higher revision rate in UKR, limiting its use. Currently most UKRs are cemented and performed using a minimally invasive technique. In joint registries, common reasons for revision include aseptic loosening and pain. These problems could potentially be addressed by using cementless implants, which may provide more reliable fixation.

The objectives of this study were to compare the quality of fixation (determined by the incidence and appearance of radiolucencies), and clinical outcomes of cemented and cementless UKR at five years.

A randomised controlled trial was established with 63 knees (62 patients) randomised to either cemented (32 patients) or cementless UKR (30 patients). Fixation was assessed with fluoroscopic radiographs aligned to the bone-implant interface at one and five years. Outcome scores were collected pre-operatively and at one, two and five years, including Oxford Knee Score (OKS), American Knee Society Score, objective and functional (AKSS-O/F) and Tegner Activity Scale (TAS), expressed as absolute scores and 0–5 year change (δ) scores.

Four patients died during the study period. There were no revisions. Mean operative time was 11 minutes shorter in the cementless group (p=0.029). At five years, there was no significant difference in any outcome measure except AKSS-F and δAKSS-F which were significantly better in the cementless group (both p=0.003). There were no femoral radiolucencies in either group. There were significantly more tibial radiolucencies in the cemented group (20/30 vs 2/27, p< 0.001). There were nine complete radiolucencies in the cemented group and none in the cementless group (p< 0.001).

Cementless fixation provides improved fixation at five years compared to cemented fixation in UKR, maintaining equivalent or superior clinical outcomes with a shorter operative time and no increase in complications.

Introduction

Subtle deformities of the acetabulum and proximal femur are recognised as biomechanical risk factors for the development of hip osteoarthritis (OA) as well as a cause of hip and groin pain. We undertook this study to examine relationships between a number of morphological measurements of the acetabulum and proximal femur and the hip pain in a 20-year longitudinal study.

Methods

In 1989 women of 45–64 years of age were recruited. Each had an AP-Pelvis radiograph at Year-2. These radiographs were analysed using a validated programme for measuring morphology. All morphological measurements were read blinded to outcome. At year 3 all participants were asked whether they experienced hip pain (side specific). This was repeated at visits up to and including 20-years. Logistic regression analysis (with robust standard errors and clustering by subject identifier) was performed using hip pain as a binary outcome. The model adjusted for baseline age, BMI and joint space and included only participants who were pain free on initial questioning.

Indications for Unicompartmental Knee Arthroplasty (UKA) vary between units. Some authors have suggested, and many surgeons believe, that medial UKA should only be performed in patients who localise their pain to the medial joint line. This is despite research showing a poor correlation between patient-reported location of pain and radiological or operative findings in osteoarthritis. The aim of this study is to determine the effect of patient-reported pre-operative pain location and functional outcome of UKA at one and five years.

Pre-operative pain location data were collected for 406 knees (380 patients) undergoing Oxford medial UKA. Oxford Knee Score, American Knee Society Scores and Tegner activity scale were recorded preoperatively and at follow-up. 272/406 (67%) had pure medial pain, 25/406 (6%) had pure anterior knee pain and 109/406 (27%) had mixed or generalised pain. None had pure lateral pain. The primary outcome interval is one year; 132/406 patients had attained five years by the time of analysis and their five year data is presented.

At one and five years, each group had improved significantly by each measure (mean δOKS 15.6 (SD 8.9) at year one, 16.3 (9.3) at year five). There was no difference between the groups, nor between patients with and without anterior knee pain or isolated medial pain.

We have found no correlation between preoperative pain location and outcome. We conclude that localised medial pain should not be a prerequisite to UKA and that it may be performed in patients with generalised or anterior knee pain.
