Introduction

The Center for Medicare and Medicaid Services (CMS) is faced with a challenge of decreasing the cost of care for total knee arthroplasty (TKA), but must make efforts to prevent patient selection bias in the process. Currently, no appropriate modifier codes exist for primary TKA based on case complexity. We sought to determine differences in perioperative parameters for patients with “complex” primary TKA with the hypothesis that they would require increased cost of care, prolonged care times, and have worse postoperative outcome metrics.

Methods

We performed a single center retrospective review from 2015 to 2018 of all primary TKA. Patient demographics, medial proximal tibial angle (mPTA), lateral distal femoral angle (lDFA), flexion contracture, cost of care, and early postoperative outcomes were collected. ‘Complex’ patients were defined as those requiring stems or augments, and multivariable logistic regression analysis and propensity score matching were performed to evaluate perioperative outcomes.

Introduction

Recent focus has shifted towards the utilization of deoxyribonucleic acid (DNA) sequencing modalities in periprosthetic joint infection (PJI) diagnosis and organism identification. The purpose of this study was to compare the diagnostic accuracy of next generation sequencing (NGS) to polymerase chain reaction (PCR) multiplex, culture, the Musculoskeletal Infection Society (MSIS) criteria, and the recently proposed criteria by Parvizi et al. [1] in the diagnosis of periprosthetic hip infections.

Methods

In this retrospective study, aspirate or tissue samples were collected in 23 revision and 19 primary hip arthroplasties for routine diagnostic workup for PJI and sent to the laboratory for NGS and PCR multiplex. Concordance along with statistical differences between diagnostic studies were calculated using Chi-squared test for categorical data.

Total knee arthroplasty is an excellent operation and the results have been well documented for both cemented and cementless techniques. It is generally accepted that the results for cemented total knee outpace the results for cementless total knees. Despite this there remains great interest in developing systems and techniques that might allow predictable biologic fixation for knee arthroplasty. There is a long list of requirements that must be met to predictably allow bone ingrowth. These include viable bone, optimal pore size, optimal pore depth, optimal porosity, minimal gaps between bone and implant and minimal micromotion. Implant design is critical but it is proposed that operative techniques can help with some of these issues. We will discuss these operative issues during the surgical demo. These technique issues include: replication of normal posterior slope of the tibia, irrigation of all cuts to avoid thermal necrosis, and application of autologous bone chips to interface - “bone slurry”.

These are obviously not all of the issues to consider but we feel they are some of the more important factors related to the cementless technique. The surgeon also has to be mindful of all of the other techniques that are essential to primary total knee arthroplasty. This demo will also utilise an ultracongruent bearing and with Vitamin E polyethylene.

Introduction

Hypoalbuminemia has previously been identified as an independent predictor of postoperative complications following total knee arthroplasty (TKA). Given the morbidity and financial burden associated with TKA complications, significant effort has gone into identifying patients at increased risk for perioperative complications. The American Society of Anesthesiologists (ASA) physical status score has been utilized for risk stratification of surgical patients for many years and is a measure of overall health. However, it is unclear how measures like albumin compare to the prognostic ability of this type of global health measure. This study aims to elucidate the utility of preoperative albumin compared with that of the ASA score in predicting complications following TKA.

Methods

Patients undergoing TKA between 2005 and 2015 were identified using the American College of Surgeons-National Surgical Quality Improvement Program (ACS-NSQIP) database. Patients were stratified based on preoperative hypoalbuminemia (<3.5 g/dL) and ASA score (≤ 2 vs. > 2). Multivariable regression analysis adjusted for age, sex, BMI, and smoking status was utilized to determine predictive potential of hypoalbuminemia and ASA score on each postoperative complication.

Introduction

The number of complex revision total hip arthroplasties (THA) is predicted to rise. The identification of acetabular bone defects prior to revision THA has important implications on technique and complexity of acetabular reconstruction. Paprosky et al. proposed a classification system including 3 main types with up to 3 subtypes focused on the integrity of the superior rim of the acetabulum and medial wall. However, the classification system is complex and its reliability has been questioned. The purpose of this study was to evaluate the effectiveness of different radiologic imaging modalities (plain radiographs, 2-D CT, 3-D CT reconstructions) in classifying acetabular defects in revision hip arthroplasty cases and their value of at different levels of orthopaedic training.

Methods

Patients treated with revision total hip arthroplasty for acetabular bone defects between 2002–2012 were identified and 22 cases selected that had plain radiographs, 2-D CT and 3-D reconstructions available. Bone defects were classified independently by two fellowship-trained adult reconstruction surgeons. Representative sections were chosen and compiled into a timed presentation. Thirty-five residents from PGY-1 to PGY-5 and 4 attending orthopaedic surgeons were recruited for this study and received a 15-minute introduction to the classification system. Chi square analysis was utilized to examine the influence of image modality and level of training on the correct classification of acetabular bone loss using the Paprosky classification system with alpha=0.05.

Elderly patients are at risk of developing cardiopulmonary and cognitive impairment following major orthopaedic surgery. One of the mechanisms believed to be responsible for such complications after total knee arthroplasty (TKA) is the release of embolic debris that may travel from the surgical site, through the lungs, and into the brain following tourniquet release. Removal of fat globules and marrow particulates from bone surfaces prior to pressurization and cementation of prosthetic components may reduce the number and size of embolic particles. We conducted a prospective, randomized clinical trial to compare the effect of carbon dioxide (CO2) gas versus saline lavage on the number and size of embolic particles observed during cemented TKA.

Twenty patients undergoing elective TKA were randomly assigned to one of two groups. In group A, standard high-pressure pulsatile saline lavage was used to clean the resected bone surfaces. In group B, the femoral canal was cleaned using CO2 lavage techniques and the resected bone surfaces were cleaned with a manual saline wash followed by CO2 lavage. All patients received the same TKA implant design. The presence of embolic particles in the heart and brain was intraoperatively monitored using transesophageal echocardiography (TEE) and transcranial Doppler (TCD) techniques, respectively. For each patient, TEE images were analyzed at tourniquet release and during the final range of motion (ROM) assessment prior to wound closure using the following five point cardiac echogenic scoring system: Grade 0: no emboli; Grade I: a few fine emboli; Grade II: a cascade of many fine emboli; Grade III: a cascade of fine emboli mixed with at least one embolus > 1 cm in diameter; and Grade IV: large embolic masses > 3 cm in diameter. The highest grade observed during either tourniquet release or ROM assessment was assigned to each patient. Cardiac emboli were then categorized according to embolic grade as follows: Grade 0 or I = Low; Grade II, III, or IV = High. For analysis of cerebral emboli, the total number of positive counts measured using TCD was recorded for each patient. TEE data were available for nine patients in group A and eight patients in group B. Comparative TCD data were available for seven patients in group A and six patients in group B. Fischer’s Exact Test was used to check for differences between groups.

For cardiac emboli, nine of nine (100%) patients in group A were in the High category based on their TEE grade, with eight patients being Grade II and one Grade III. In contrast, three of eight (37.5%) patients in group B were in the Low category, leaving only five (62.5%) in the High category (p = 0.08). All five group B patients in the High category were Grade II. No patients in group A had cerebral emboli detected using TCD. In group B, three of six patients had one cerebral embolus and the remaining three had none. Three patients in group B were excluded from the comparative TCD analysis due to the presence of a patent foramen ovale (PFO). These three patients with a PFO had one, three, and four cerebral emboli, respectively. No patients in group A had a PFO.

This study examines the effect of pulsatile saline versus CO2 gas lavage on intraoperative embolic events during TKA. Thirty-seven percent of patients in the CO2 lavage group had a Low cardiac echogenic score compared with 0% of patients in the standard pulsatile saline lavage group. A single cerebral embolus was detected in three of six patients in the CO2 lavage group compared with none in the seven patients in the standard pulsatile saline lavage group. Compared to published studies on cerebral emboli in TKA, the overall incidence of cerebral emboli in the current study was very low across both groups. The results of this study suggest that CO2 gas, as compared to pulsatile saline, lavage reduces the number of intraoperative cardiac emboli during total knee arthroplasty.
