Introduction

Various biomaterials and bone graft substitute technologies for use in osteomyelitis treatment are currently used in clinal practice. They vary in mode of action (with or without antibiotics) and clinical application (one-stage or two-stage surgery). This systematic review aims to compare the clinical evidence of different synthetic antimicrobial bone graft substitutes and antibiotic-loaded carriers in eradicating infection and clinical outcome in patients with chronic osteomyelitis.

Methods

Systematic review according to PRISMA statement on publications 2002-2023. MESH terms: osteomyelitis and bone substitutes. FREE terms: chronic osteomyelitis, bone infection. A standardized data extraction form was be used to extract data from the included papers.

Aim

Prosthetic joint infections pose a major clinical challenge. Developing novel material surface technologies for orthopedic implants that prevent bacterial adhesion and biofilm formation is essential. Antimicrobial coatings applicable to articulating implant surfaces are limited, due to the articulation mechanics inducing wear, coating degradation, and toxic particle release. Noble metals are known for their antimicrobial activity and high mechanical strength and could be a viable coating alternative for orthopaedic implants [1]. In this study, the potential of thin platinum-based metal alloy coatings was developed, characterized, and tested on cytotoxicity and antibacterial properties.

Method

Three platinum-based metal alloy coatings were sputter-coated on medical-grade polished titanium discs. The coatings were characterized using optical topography and scanning electron microscopy with energy dispersive spectroscopy (SEM/EDS). Ion release was measured using inductively coupled plasma optical emission spectrometry (ICP-OES). Cytotoxicity was tested according to ISO10993-5 using mouse fibroblasts (cell lines L929 and 3T3). Antibacterial surface activity, bacterial adhesion, bacterial proliferation, and biofilm formation were tested with gram-positive Staphylococcus aureus ATCC 25923 and gram-negative Escherichia coli ATCC 25922. Colony forming unit (CFU) counts, live-dead fluorescence staining, and SEM-EDS images were used to assess antibacterial activity.

Aim

Magnetic resonance imaging (MRI) and 2-[18F]-fluoro-2-deoxy-D-glucose (18F-FDG) Positron Emission Tomography, paired with Computed Tomography (PET/CT) are two indicated advanced imaging modalities in the complicated diagnostic work-up of osteomyelitis. PET/MRI is a relatively novel hybrid modality with suggested applications in musculoskeletal infection imaging. The goal of this study was to assess the value of hybrid 18F-FDG PET/MRI for chronic osteomyelitis diagnosis and surgical planning.

Method

Five suspected chronic osteomyelitis patients underwent a prospective 18F-FDG single-injection/dual-imaging protocol with hybrid PET/CT and hybrid PET/MR. Diagnosis and relevant clinical features for the surgeon planning treatment were compared. Subsequently, 36 patients with 18F-FDG PET/MRI scans for suspected osteomyelitis were analysed retrospectively. Sensitivity, specificity, and accuracy were determined with the clinical assessment as the ground truth. Standardized uptake values (SUV) were measured and analysed by means of receiver operating characteristics (ROC).

This multicenter study compared computer-navigated TKA using either MIS or conventional surgical technique, using a CR fixed bearing knee, Stryker Navigation system and dedicated MIS instrumentation. It was hypothesized that patients would benefit from the MIS technique by shorter recovery periods, less blood loss, faster wound healing and improved mobility during early rehabilitation.

A prospective multicentre double-blind controlled trial included 69 patients matched for age, gender, BMI (MIS n=36, CONV n=33). Assessments at pre-op, 1 week, 3 and 6 months post-op included surgery time, bloodloss, range of motion, Knee Society Score (KSS) and WOMAC, Chair rise test and quadriceps strength. Radiographic analysis included radiographs for lucencies and CAT scans for alignment,

Four patients were lost to follow-up. The MIS group had significantly more prolonged surgery time and blood loss at 24 hours p<0.05. At 6 months mean flexion values for MIS (106,7°±12,91) and CONV 105,92 ±11,58) with no significant differences in flexion ROM between both groups at any time point. KSS scores showed a significant improvement (p<0,01) over time in both groups but no statistical significance between groups. WOMAC score also improved significantly (p<0,01) over time in both groups without reaching statistical significance. A significant decrease of anterior knee pain score was observed over time with no significant difference between both groups. Quadriceps strength recovery was not significant between groups but trended toward faster recovery in the MIS group. X-rays showed stable implants with no progressive radiolucent lines in all patients.

The hypothesis that patients benefit from the MIS technique in the short term was not confirmed by the results of this study. The MIS surgery technique resulted in more blood loss intra-op and in the first 24hours post op as well as an elongated surgery time. The MIS surgery technique also failed to generate clear advantages in clinical or functional outcome that persisted over time.
