Despite the COVID-19 pandemic, incidence of hip fracture has not changed. Evidence has shown increased mortality rates associated with COVID-19 infection. However, little is known about the outcomes of COVID-19 negative patients in a pandemic environment. In addition, the impact of vitamin D levels on mortality in COVID-19 hip fracture patients has yet to be determined. This multicentre observational study included 1,633 patients who sustained a hip fracture across nine hospital trusts in North West England. Data were collected for three months from March 2020 and for the same period in 2019. Patients were matched by Nottingham Hip Fracture Score (NHFS), hospital, and fracture type. We looked at the mortality outcomes of COVID-19 positive and COVID-19 negative patients sustaining a hip fracture. We also looked to see if vitamin D levels had an impact on mortality.Aims

COVID-19 remains the major focus of healthcare provision. Managing orthopaedic emergencies effectively, while at the same time protecting patients and staff, remains a challenge. We explore how the UK lockdown affected the rate, distribution, and type of orthopaedic emergency department (ED) presentations, using the same period in 2019 as reference. This article discusses considerations for the ED and trauma wards to help to maintain the safety of patients and healthcare providers with an emphasis on more remote geography. The study was conducted from 23 March 2020 to 5 May 2020 during the full lockdown period (2020 group) and compared to the same time frame in 2019 (2019 group). Included are all patients who attended the ED at Raigmore Hospital during this period from both the local area and tertiary referral from throughout the UK Highlands. Data was collected and analyzed through the ED Information System (EDIS) as well as ward and theatre records.Aims
Methods
A significant portion of ankle arthroplasty and ankle arthrodesis procedures performed in British Columbia are funded by the public medical services plan (MSP). However, some patients are treated privately through self-pay or by the workers compensation board (WCB), with the latter two groups being more likely to receive treatment sooner. The potential effect of payer on patient-reported outcomes and reoperation rates has not been previously explored. A retrospective chart review was performed using data from the Canadian Orthopaedic Foot and Ankle Society Prospective Ankle Reconstruction Database. N=443 patients (393 MSP, 26 self-pay, 24 WCB), treated with total ankle replacement or ankle arthrodesis by three subspecialty-trained surgeons in Vancouver from 1999–2003, were analyzed. Outcomes were compared, by payer, preoperatively and at long-term follow-up (6.3 years, range 2–14 years). Function was assessed using the Ankle Osteoarthritis Scale (AOS) Total score (primary outcome) and the AOS Pain and Difficulty subscores. Expectation and satisfaction with symptoms was assessed using the Musculoskeletal Outcomes Data Evaluation and Management Scale, and physical and emotional quality of life was assessed using the Short Form-36 (SF-36) Health Survey in terms of the mental component summary (MCS) and physical component summary (PCS). Swelling and reoperation rates were also compared. AOS Total score was not significantly different between payers. WCB patients had significantly worse preoperative AOS Difficulty scores (73, 95%CI 65–80) compared to MSP (65, CI 63–67) and self-pay patients (56, CI 49–63)(p < 0 .008). Their SF-36 MCS scores were also significantly worse pre- and postoperatively (WCB: 43, CI 38–49, 45, CI 40–50, MSP: 51, CI 50–52, 51, CI 50–52, self-pay: 51, CI 46–56, 54, CI 49–58)(p < 0 .03). AOS Pain scores and SF-36 PCS scores were not different. Pre- and postoperatively, MSP patients reported more satisfaction with symptoms (1.31, CI 1.24–1.38, 3.21, CI 3.07–3.35), compared to WCB (1.13, CI 0.84–1.41, 2.83, CI 2.26–3.41) and self-pay patients (1.19, CI 0.91–1.47, 2.88, CI 2.33–3.44). Preoperatively, WCB patients had the lowest expectations (76, CI 69–84), the worst AOS Total (64, CI 57–71) and SF-36 scores (MCS 43, CI 38–49, PCS 28, CI, 25–32), and the most swelling (3.5, CI 3.1–4). Conversely, self-pay patients had the highest preoperative expectations (88, CI 81–95), the best AOS Total (53, CI 46–60) and SF-36 scores (MCS 51, CI 46–56, PCS: 34, CI 30–37) and the least swelling (3, CI 2.6–3.4). Postoperatively, WCB and self-pay patients had lower expectations met (35, CI 23–47 and 40, CI 28–51) and worse AOS Total scores (36, CI 27–45 and 35, 26–43), compared to MSP patients (Expectations: 29, CI 26–32, AOS Total: 31, CI 29–33). Reoperation rates were similar among groups. WCB patients had significantly more difficulty with symptoms prior to surgery and worse SF-36 MCS scores pre- and postoperatively. The preoperative expectations of WCB patients were lowest, while those of self-pay patients were highest. Both groups had lower expectations met postoperatively.
COVID-19 has changed the practice of orthopaedics across the globe. The medical workforce has dealt with this outbreak with varying strategies and adaptations, which are relevant to its field and to the region. As one of the ‘hotspots’ in the UK , the surgical branch of trauma and orthopaedics need strategies to adapt to the ever-changing landscape of COVID-19. Adapting to the crisis locally involved five operational elements: 1) triaging and workflow of orthopaedic patients; 2) operation theatre feasibility and functioning; 3) conservation of human resources and management of workforce in the department; 4) speciality training and progression; and 5) developing an exit strategy to resume elective work. Two hospitals under our trust were redesignated based on the treatment of COVID-19 patients. Registrar/consultant led telehealth reviews were carried out for early postoperative patients. Workflows for the management of outpatient care and inpatient care were created. We looked into the development of a dedicated operating space to perform the emergency orthopaedic surgeries without symptoms of COVID-19. Between March 23 and April 23, 2020, we have surgically treated 133 patients across both our hospitals in our trust. This mainly included hip fractures and fractures/infection affecting the hand.Aims

The adequate provision of personal protective equipment (PPE) for healthcare workers has come under considerable scrutiny during the COVID-19 pandemic. This study aimed to evaluate staff awareness of PPE guidance, perceptions of PPE measures, and concerns regarding PPE use while caring for COVID-19 patients. In addition, responses of doctors, nurses, and other healthcare professionals (OHCPs) were compared. The inclusion criteria were all staff working in clinical areas of the hospital. Staff were invited to take part using a link to an online questionnaire advertised by email, posters displayed in clinical areas, and social media. Questions grouped into the three key themes - staff awareness, perceptions, and concerns - were answered using a five-point Likert scale. The Kruskal-Wallis test was used to compare results across all three groups of staff.Aims

Advanced osteoarthritis of knee is associated with low-backache in a significant number of patients and adversely affects the quality of life. There is a paucity of literature describing outcomes of backache after total-knee-arthroplasty (TKA). We evaluated backache in patients of advanced knee-osteoarthritis and their functional and radiological outcomes after TKA after approval from Institutional ethics committee. Fifty-nine patients (40 females and 19 males) were included. Mean body-mass index was 28.7. Mean visual analogue score (VAS) for knee-pain was 7.98 preoperatively and 1.6 in follow-up. For chronic backache, the mean VAS score improved from 6.08 to 2.4, Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) improved from 67.5 to 37.8, Knee society score (KSS) from 49.8 to 76.6, Oswestry Disability Index (ODI) Score from 55.44 to 34.65 and SF-36 Quality-of-life score from 44.95 to 74.63. There was a significant correlation between in knee and low-back functional scores. Magnetic resonance imaging-based scoring of degenerative changes (Pfirrmann grading) showed improvement only in 13.5% patients; 56% showed no change and 30.5% showed deterioration of scores. Chronic low backache is a significant co-morbidity in advanced knee-osteoarthritis. TKA has the potential to relieve backache along with knee-pain and improves quality of life.
The objective of this study was to evaluate the intermediate term clinical and radiological results of a new short stem hip implant. In 20 consecutive patients suffering from osteoarthritis with 25 affected hip joints (five cases were bilateral), the clinical and radiological results of 25 hip arthroplasties performed in one hospital between October 2009 and May 2014 through a minimally invasive anterolateral approach using a cementless short stem prosthesis type Aida and a cementless cup type Ecofit with a ceramic on ceramic pairing were evaluated prospectively. The median age of patients at time of surgery was 60 years (range, 42–71 years), 15 male (4 were bilateral) and 5 female patients (one was bilateral) were included in the study. The median clinical follow up was 30 months (range, 2–88 months), and the median radiological follow up was 30 months (range, 2–88 months).Aim

Hip resurfacing arthroplasty (HRA) is an alternative to traditional total hip arthroplasty (THA) in young active patients. While comparative implant survival rates are well documented, there is a paucity of studies reporting the patient mortality rates associated with these procedures. The purpose of this study was to evaluate the mortality rates in patients age 55 years and younger who underwent HRA versus THA and to assess whether the type of operation was independently associated with mortality. The database of a single high-volume surgeon was reviewed for all consecutive patients age 55 years and younger who underwent hip arthroplasty between 2002 and 2010. HRA became available in the United States in 2006. This yielded 504 patients who had undergone HRA from 2006 to 2010 and 124 patients who had undergone a THA. Patient characteristics were collected from the electronic medical record including age, gender, body mass index, Charleston comorbidity index, smoking status, and primary diagnosis. Mortality was determined through a combination of electronic chart reviews, patient phone calls, and online obituary searches. Univariate analysis was performed to identify a survival difference between the two cohorts. Multivariable Cox-Regression analyses were used to determine whether the type of operation was independently associated with mortality.Introduction

Osteolysis has been reported following ACJ reconstruction with a synthetic graft. We present the first study into its prevalence and pattern, and its effect on patient outcome. Patients who underwent treatment of an unstable ACJ injury using the Surgilig/LockDown implant were identified via our database. Patients were invited to attend a dedicated outpatient clinic for clinical examination, radiographic evaluation, and completion of outcome scoring. Patients who were unable to attend were contacted by telephone. 49 patients were identified. We assessed 21 clinically at a mean of 7 years post-procedure (range 3–11 years). All had radiographic evidence of distal clavicle and coracoid osteolysis. We did not observe progression of osteolysis from the final post-operative radiographs. A further 13 were contacted by phone. The mean Oxford Shoulder Score was 43 (range 31–48) and mean DASH score was 8.5 (range 3–71). The average Patient Global Impression of Change score was 6 (range 2–7). Six patients underwent removal of a prominent screw at a mean of 2 years after surgery; the pattern of osteolysis was no different in this group. All patients had comparable abduction, forward flexion and internal rotation to their uninjured shoulder. We did not observe any relationship between patient demographics, position of implant or etiology and the pattern of osteolysis. Osteolysis of the distal clavicle and/or coracoid is always seen following synthetic reconstruction of the ACJ using this implant, but is non-progressive. Range of shoulder movement is largely unaffected and patient outcomes remain high.
Potential implant and technique related factors to improve patellofemoral (PF) kinematics in total knee arthroplasty (TKA) are design of trochlear geometry and patella, restoration of posterior offset, patellar tilt and avoid overstuffing. The primary aim of this prospective, matched pair study was to assess the radiographic features of PF kinematics with an anatomic patella. Between July 2012 and May 2013, 49 consecutive posterior stabilized cemented Attune TKAs (Depuy Synthes Warsaw Indiana) were matched to the 49 PFC Sigma (Depuy) based on age, gender, and body mass index (BMI). All surgeries were performed via medial parapatellar approach with patellar resurfacing. Radiographic analysis was performed prospectively with minimum 1-year follow-up and included overall limb alignment, anterior offset, posterior offset, joint line, patellar thickness, patellar tilt and patellar displacement by two independent observers.Introduction

Total hip arthroplasty (THR) with non-cemented or hybrid fixation remains one of the most successful procedures performed today. The aim of this study was to assess the safety and efficacy of a hydroxyapatite (HA) coated, hemispherical cup. Between 2003 and 2007, 223 THAs (210 patients) with peripheral self-locking (PSL) cup and highly cross-linked polyethylene (Crossfire, Stryker, Mahwah, NJ) with minimum 5 years clinical and radiographic follow-up (5–9 years) were analyzed. The mean age was 62.5 years ± 10.8 (range, 32.7 – 86.3) at the time of surgery and the predominant preoperative diagnoses was osteoarthritis (97.8%). 72% were solid cups without screw augmentation and 28% were multi-hole with screw. Clinical analysis included Hospital for Special Surgery (HSS) hip scores at latest follow-up. Detail radiographic analysis was carried out on anteroposterior and false profile views for evidence of osseointegration in all Charnley's zones. Osseointegration was assessed based on presence of Stress Induced Reactive Cancellous Bone (SIRCaB) with trabecular bone hypertrophy 5–15mm extending from the cup, and absence of radiolucency or demarcation. Highly cross-linked polyethylene (HCLPE) was introduced to reduce wear and osteolysis in total hip arthroplasty (THA). There is no reported data regarding wear rates and clinical performance of Crossfire HCLPE (Stryker, Mahwah, New Jersey) in young and active patients. The purpose of this prospective study is to assess minimum 10-year wear rates and survivorship of Crossfire in young and active patients. Between January 2001 to December 2003, 52 consecutive THAs (43 patients; 26 males and 17 females), 55 years and younger, with an average University of California Los Angeles activity (UCLA) score of 7.3 ± 1.5 (5 – 10) at the time of surgery were prospectively followed. The mean age of patients was 47.4 ± 7.8 years old (range 24 to 55 years). Indication for surgery included osteoarthritis in all cases. All operations were performed by the senior surgeon via a posterolateral approach. All components were non-cemented SecurFit femoral stem, 28 mm Cobalt-Chromium (Co-Cr) femoral head, and Trident PSL cup with Crossfire HCLPE acetabular liner (Stryker, Mahwah, New Jersey). At minimum 10-years follow-up (mean 11.5 ± 0.94 years), wear rates were assessed using the Roman software. Hospital for Special Surgery (HSS) scores and survivorship data were analyzed.Background

Obesity is a risk factor for acetabular malposition when total hip arthroplasty (THA) is performed with manual orientation techniques. However, conflicting evidence exists regarding the usefulness of computer-assisted surgery for performing THA in obese patients. The purpose of this study was to compare the precision and accuracy of imageless navigation for acetabular component placement in obese versus non-obese patients. After institutional review board approval, 459 THA performed for primary hip osteoarthritis were reviewed retrospectively. The same imageless navigation system was used for acetabular component placement in all THA. During surgery the supine anterior pelvic plane was referenced superficially. THA was performed via posterolateral approach in the lateral position. A hemispherical acetabular component was used, with target inclination of 40° and target anteversion of 25°. Computer software was used to determine acetabular orientation on postoperative anteroposterior pelvic radiographs. Obese patients (BMI ≥ 30 kg/m2) were compared to non-obese patients. A 5° difference in mean orientation angles was considered clinically significant. Orientation error (accuracy) was defined as the absolute difference between the target orientation and the measured orientation. Student's t test was used to compare means. Hartley's test compared variances of the mean differences (precision). Fisher exact tests examined the relationship between obesity and component placement in the target zone (target ± 10°) for inclination and version. All statistical tests were two-sided with a significance level of 0.05. Differences in mean inclination and anteversion between obese and non-obese groups were 1.1° (p=0.02 and p=0.08, respectively), and not clinically significant. Inclination accuracy trended toward improvement for non-obese patients (p=0.06). Inclination precision was better for non-obese patients (p=0.006). Accuracy and precision for anteversion were equal between the two groups (p=0.19 and p=0.95, respectively). There was no relationship between obesity and placement of the acetabulum outside of the target ranges for inclination (p=0.13), anteversion (p=0.39) or both (p=0.99), with a trend toward more inclination outliers in obese patients versus non-obese patients (7.3% versus 3.9%). The observed differences in mean acetabular orientation angles were not clinically significant (< 5°), although inclination orientation was less accurate and precise for obese patients. In contrast to existing literature, we found no difference in the accuracy and precision with regard to anteversion in obese and non-obese patients. We propose that accurate superficial registration of landmarks in obese patients is achievable, and the use of imageless navigation likely improves acetabular positioning in obese and non-obese patients.
Surgeons often target the Lewinnek zone (40°±10° of inclination; 15°±10° of anteversion) for acetabular orientation during total hip arthroplasty (THA). However, matching native anteversion (20°-25°) may achieve optimal stability. The purpose of this study was to (1) determine incidence of early dislocation with increased target acetabular anteversion, and (2) report the accuracy of imageless navigation for achieving target acetabular position in a large, single-surgeon cohort. A posterolateral approach with soft tissue repair was performed in the 553 THA meeting the inclusion criteria. The same imageless navigation system was used for acetabular component placement in all THA. Target acetabular orientation was 40° ± 10° of inclination and 25° ± 10° of anteversion. Computer software was used to measure acetabular positioning on 6-week postoperative anteroposterior pelvic radiographs. Incidence of dislocation within 6 months of surgery was determined. Repeated measures multiple regression using the Generalised Estimating Equations approach was used to identify baseline patient characteristics (age, gender, BMI, primary diagnosis, and laterality) associated with component positioning outside of the targeted ranges for inclination and anteversion. Fisher exact tests were used to examine the relationship between dislocation and component placement in either the Lewinnek safe zone or the targeted zone. All tests were two-sided with a significance level of 0.05. Mean inclination was 42.2° ± 4.9°, and mean anteversion was 23.9° ± 6.5°. 82.3% of cups were placed within the target zone. Variation in anteversion accounted for 67.3% of outliers. Only body mass index was associated with inclination outside the target range (p = 0.017), and only female gender was associated with anteversion outside the target range (p = 0.030). Six THA (1.1%) experienced early dislocation, and 3 THA (0.54%) were revised for multiple dislocations. There was no relationship between dislocation and component placement in either the Lewinnek zone (p = 0.224) or the target zone (p = 0.287). This study demonstrates that increasing target acetabular anteversion using the posterolateral approach does not increase the incidence of early THA dislocation. However, the long-term effects on bearing surface wear and stability must be elucidated. The occurrence of instability even in patients within our target zone emphasises the importance of developing patient-specific targets for THA component alignment.
The prevalence of Class III Obesity (BMI ≥ 40 kg/m25) in black women is 18%, three times the 6 national average. Class III obesity is associated with mobility limitations, particularly hip joint 7 deterioration. Therefore black women are highly likely to come to the attention of orthopedic 8 surgeons. Weight loss associated with bariatric surgery should lead to enhanced success of hip 9 replacements. However, we present a case of a black woman who underwent Roux-en-y gastric 10 bypass with the expectation that weight loss would improve her ambulation and if necessary 11 make her a better surgical candidate for hip replacement. Her gastric bypass was successful as her BMI declined from 52.0 kg/m2 to 33.7 kg/m212. However, her hip circumference post weight 13 loss remained persistently high. As a consequence, the soft tissue tunnel geometry presented 14 major challenges. The tunnel depth as well as the immobility of the soft tissue envelope 15 interfered with retractor placement, tissue reflection and adequate surgical access to the 16 acetabulum. Therefore a traditional cup placement could not be achieved. Instead, a 17 hemiarthroplasty was performed. Her pre-surgery Harris Hip Score was 17.0. In the first few 18 months post surgery there were improvements, specifically a decrease in pain and a decreased 19 reliance on external support. But her overall functional independence never improved. This case 20 is presented to raise awareness that improved BMI category post bariatric surgery is not 21 sufficient to guarantee that orthopedic risks have been minimized. Overall, weight loss does 22 improve both the metabolic profile and anesthesia risk, but the success rate of total hip 23 arthroplasties will be low if fat mass (i.e. high hip circumference) in the operative field remains 24 high. We are now repeatedly recognizing this problem but are not finding any case reports on 25 this issue. Therefore we provide a practical approach to evaluate these patients and describe 26 ways we have found to successfully address intra-operative challenges.
