Aims. Our objective was describing an algorithm to identify and prevent vascular injury in patients with intrapelvic components. Methods. Patients were defined as at risk to vascular injuries when components or cement migrated 5 mm or more beyond the ilioischial line in any of the pelvic incidences (anteroposterior and Judet view). In those patients, a serial investigation was initiated by a CT angiography, followed by a vascular surgeon evaluation. The investigation proceeded if necessary. The main goal was to assure a safe tissue plane between the hardware and the vessels. Results. In ten at-risk patients undergoing revision hip arthroplasty and submitted to our algorithm, six were recognized as being high risk to vascular injury during surgery. In those six high-risk patients, a preventive preoperative stent was implanted before the orthopaedic procedure. Four patients needed a second reinforcing stent to protect and to maintain the vessel anatomy deformed by the intrapelvic implants. Conclusion. The evaluation algorithm was useful to avoid blood vessels injury during revision total hip arthroplasty in high-risk patients. Cite this article: Bone Jt Open 2022;3(11):859–866

Orthopaedic injuries in the knee are often associated with vascular injury. When these vascular injuries are missed devastating there are devastating outcomes like limb ablation. Pulse examination in these patients is not sensitive to exclude vascular injuries. That often lead to clinicians opting for Computed Tomography Angiogram (CTA) to exclude vascular. this usually leads to a burden in Radiology Department. This study aimed to evaluate the prevalence of vascular injury in patient with orthopaedic injury in the knee. The computed tomography (CT) done in patients with distal femur fracture, knee dislocation and proximal tibia fractures were retrieved from the picture archiving and comunication system (PACS). The CTs were done between June 2017 and June 2022. The computed tomography angiogram (CTA) reports were reviewed to determine cases that vascular injury. A sample size of 511 cases was collected. 386 cases were done CTA and 125 cases were not done CTA. There were 218 tibial plateau fractures, 79 knee dislocations, 72 distal metaphyseal femur fractures, 61 floating knees, 55 distal femure intraarticular and 26 proximal metaphyseal tibia fractures. The mechanisms of injury in these were gunshot, fall from standing height, fall from height, MVA, MBA, PVA and sports. Prevalance was 9.17% (47) of the total injuries in the knee. Prevalance in patients who were sent for CTA was 12.08%. Routine CTA in patients with injuries in the knee is not recomended. The use of ankle brachial index may decrease the number of CTA done

Background: Vascular injuries associated with total joint arthroplasty are the most feared complication. The arterial and venous injury can occur due to direct or indirect trauma. A high index of suspicion, recognition of the injury and prompt treatment, with the immediate availability of a vascular surgeon is vital for good outcome. Methods: Using prospectively collected data on 13624 patients undergoing total joint arthroplasty at our institution, all incidences of vascular injury were identified. Detailed data regarding the mode of presentation, the type and the outcome of intervention delivered, and the eventual functional outcome of the total joint arthroplasty were determined. Results: There were a total of 17 vascular injuries (0.1%). Majority (16/17) of these vascular injuries were detected in the postoperative recovery area. 9 injuries occurred after TKA and 8 occurred after THA. Indirect injury was the mechanism in TKA patients with popliteal artery thrombosis being the mechanism. In contrast direct injury was the mechanism in THA patients. Fasciotomy was performed in all TKA patients and none were needed in the THA patients. One patient died of complications related to vascular injury. 12 of 17 patients (70%) had launched a legal suit against the operating surgeon. Conclusions: After more than 80 years of accumulated experience and more than 35000 joint replacements performed by 5 surgeons in our center, the vascular complication still continues to occur. Patient awareness regarding this real problem may play a role in defraying the extremely high likelihood of legal suits associated with this complication

Vascular injuries during total hip arthroplasty (THA) are rare but when they occur, have serious consequences. These have traditionally been managed with open exploration and repair, but more recently there has been a trend towards percutaneous endovascular management. We performed a systematic review of the literature to assess if this change in trend has led to an improvement in the overall reported rates of morbidity and mortality during the last 22 years in comparison with the reviews of the literature published previously. . We found a total of 61 articles describing 138 vascular injuries in 124 patients. Injuries because of a laceration were the most prevalent (n = 51, 44%) and the most common presenting feature, when recorded, was bleeding (n = 41, 53.3%). Delay in diagnosis was associated with the type of vascular lesion (p < 0.001) and the clinical presentation (p = 0.002). Open exploration and repair was the most common form of management, however percutaneous endovascular intervention was used in one third of the injuries and more constantly during the last 13 years. The main overall reported complications included death (n = 9, 7.3%), amputation (n = 2, 1.6%), and persistent ischaemia (n = 9, 7.3%). When compared with previous reviews there was a similar rate of mortality but lower rates of amputation and permanent disability, especially in patients managed by endovascular strategies. Complete dislocation of the knee is a relatively rare condition. When it occurs as a result of high-velocity injury, such as in a road traffic accident, associated vascular injury is generally suspected. In low-velocity injuries, however, distal pulses are often maintained throughout, and the possibility of vascular injury may erroneously be discounted. We report four cases of low-velocity dislocation of the knee, only one of which had an overt vascular disruption, but three of which had arterial damage. On the basis of our experiences, we recommend arteriography in all cases of complete dislocation of the knee

We carried out a randomised, controlled trial in 157 patients who had isolated severe Gustilo type-IIIa and type-IIIb fractures of the metaphysis or diaphysis of the leg to determine the prevalence of vascular injuries and the role of vascular repair. All patients had stable vital signs and clinically adequate circulation in their legs before operation. In a control group of 64 patients we performed conventional surgery with systematic debridement and primary stabilisation of the fractures. In the trial group of 93 patients the major vessels and nerves adjoining the compound fracture were routinely explored and repaired when necessary after the initial procedure. Two of the 28 control patients (7.1%) with type-IIIb compound fractures had signs of inadequate circulation after the first operation. Both had major vascular injuries which were demonstrated at a second procedure. In the trial group, major vascular injuries were found in two of 54 patients (3.7%) with type-IIIa and 11 of 39 patients (28.2%) with type-IIIb compound fractures. Compared with the control group the trial group showed improved results at both the immediate and long-term follow-up. Routine exploration and early repair of injured major vessels of the leg in severe compound fractures gave encouraging results

1. Biopsies of muscle were taken during the course of operation from sixteen patients with vascular injuries to the limbs. Three types of histological change were found. 2. In the first, there was massive necrosis of muscle fibres—a group of cases in which there had always been serious damage to the main artery of the limb or to the vessel supplying the affected muscles. 3. In the second type there was dense interstitial fibrosis, the muscle fibres sometimes being normal and sometimes showing necrosis or denervation—a group of cases in which the vascular injury varied from severance of the vessels by gunshot wounds to trivial damage, causing slow haemorrhage within fascial-bound spaces. 4. The third type showed scattered foci of necrosis together with patchy interstitial fibrosis—due to the pressure of tight plasters, crushing of the limb, fractures with arterial contusion, or slow haemorrhage or extravascular transfusion within fascial planes. The rise of tension within the muscles was probably sufficient to occlude the smaller arterioles with resultant patchy necrosis. 5. The vulnerability of certain muscles to vascular damage is partly related to the intramuscular vascular pattern, of which five types have been described. 6. In ischaemic muscles the intramuscular nerve trunks may be normal or they may show evidence of degeneration or necrosis; but in favourable circumstances there may be regeneration of axons. 7. In some cases there was evidence of regeneration of muscle fibres in man, the regeneration being dependent to some extent upon the efficiency of intramuscular anastomoses. 8. The prognosis, in cases of ischaemia of human voluntary muscle, depends upon the extent and the reversibility of damage to both muscle and nerve fibres and upon the extent of regeneration of muscle fibres

We report the management and outcome of 35 lower limb fractures with associated severe vascular injuries treated over a 15-year period. Limb survival was related to the period of ischaemia. Management of the fractures by immediate open reduction and internal fixation was associated with a higher amputation rate than either external fixation or simple splintage, particularly for upper tibial injuries. External fixation is recommended as the method of choice for the stabilisation of the skeletal injury. A selective policy is advised for fasciotomy

Purpose of the study: Elbow dislocation is a frequent traumatic injury. Management is well described. An associated vascular lesion is exceptional, but must be searched for systematically. We report a short series and describe our therapeutic strategy for these complicated dislocations. Material and methods: Between 1999 and 2004, our emergency unit cared for 357 cases of elbow dislocation. For six, injury to the brachial artery was present. This series included four men and one woman, mean age 34.7 years. The causal trauma was a traffic accident for four, and a fall four two. The dislocation was open in three. The radial pulse could not be palpated in any of the patients but blood supply to the hand persisted in five. There was a clinically detected neurological deficit in three. After obtaining the necessary x-rays, the elbows were reduced in the emergency room. The radial pulse remained impalpable in all patients and arteriography was performed and revealed a rupture of the brachial artery in five and an arterial spasm in one. Vascular repair was undertaken in all six cases using an reversed venous graft. The elbow was stabilized with an external fixator in three patients. For the others, the elbow was immobilized for 21 days on average before remobilization. Results: AT 23 months follow-up, the overall outcome has been average with a Mayo clinic score of 69.2 points. Only four elbows were functional. Only one patient had a pain free elbow. Stiff joint was the most frequent functional impairment: mean flexion was 111.7° with a 41.7° extension deficit. The blood supply to the hand was correct in all cases, but one of the graft bridges became obstructed by a thrombus, the distal vascularization being supplied by the collateral circulation. The initial neurological lesions recovered partially. Discussion: Elbow dislocation with vascular injury is exceptional but must be systematically detected. Emergency management associates reduction of the dislocation, a vascular work-up and rapid surgical management of the elbow, using an articulated external fixator when necessary, to limit as much as possible posttraumatic stiffness

Aims. Vascular compromise due to arterial injury is a rare but serious complication of a proximal humeral fracture. The aims of this study were to report its incidence in a large urban population, and to identify clinical and radiological factors which are associated with this complication. We also evaluated the results of the use of our protocol for the management of these injuries. Methods. A total of 3,497 adult patients with a proximal humeral fracture were managed between January 2015 and December 2022 in a single tertiary trauma centre. Their mean age was 66.7 years (18 to 103) and 2,510 (72%) were female. We compared the demographic data, clinical features, and configuration of those whose fracture was complicated by vascular compromise with those of the remaining patients. The incidence of vascular compromise was calculated from national population data, and predictive factors for its occurrence were investigated using univariate analysis. Results. A total of 18 patients (0.5%) had a proximal humeral fracture and clinical evidence of vascular compromise, giving an annual incidence of 0.29 per 100,000 of the population. Their mean age was 68.7 years (45 to 92) and ten (56%) were female. Evidence of a mixed pattern neurological deficit (brachial plexus palsy) (odds ratio (OR) 380.6 (95% CI 85.9 to 1,685.8); p < 0.001), complete separation of the proximal shaft from the humeral head with medial displacement (OR 39.5 (95% CI 14.0 to 111.8); p < 0.001), and a fracture-dislocation (OR 5.0 (95% CI 1.6 to 15.3); p = 0.015) were all associated with an increased risk of associated vascular compromise. A policy of reduction and fixation of the fracture prior to vascular surgical intervention had favourable outcomes without vascular sequelae. Conclusion. The classic signs of distal ischaemia are often absent in patients with proximal injuries to major vessels. We were able to identify specific clinical and radiological ‘red flags’ which, particularly when present in combination, should increase the suspicion of a fracture with an associated vascular injury, and facilitate early diagnosis and appropriate combined orthopaedic and vascular intervention. This chamber is designed for use with conscious or unconscious patients requiring treatment with oxygen under high pressure. It can be pressurised to a maximum of 45 pounds per square inch above ambient (60 pounds per square inch absolute).

The unit consists of two parts, the pressure chamber and the control console, connected together by flexible hose. The chamber can be operated up to a distance of 100 feet from the control console and both parts are mobile.

The chamber consists of two concentrically mounted transparent cylinders, closed at the ends by domed metal caps. One end cap is detachable and can be removed, complete with the bed. The bed will accommodate a patient 6 feet 6 inches tall. The fully transparent chamber allows a complete view of the patient during treatment.

A communication system is provided between the patient and operators. A loud speaker and microphone are built into the door of the chamber and two external pick-up points are situated on the console and the fixed end cap of the chamber. Two-way or three-way conversation is possible and the patient can be temporarily isolated from the system if required.

Oxygen and electrical services must be coupled to the console. The chamber will operate directly from oxygen cylinders charged to a maximum of 1,800 pounds per square inch or from any piped supply with a minimum pressure of 75 pounds per square inch absolute.

A self-contained 12-volt direct current battery supplies the necessary electrical power. A charging unit is incorporated in the control console and this is normally connected to an alternating current supply to keep the battery fully charged. If initially the battery is fully charged the unit can be operated in a continuous pressurisation-depressurisation cycle for at least twenty-four hours without further charging.

Pressurisation and depressurisation are achieved by means of a motorised valve on the outlet of the pressure chamber. During pressurisation a variable speed motor applies a gradually increasing load to a helical spring. This in turn loads the outlet valve. By this means smooth pressure changes are obtained and the motor speed can be used to control the rate of pressure change.

Operating the pressure chamber is simple. After the patient has been placed in the chamber and the door sealed by screw-down knobs, pressurisation is started by turning a selector switch on the control console. The required working pressure can be preset or the pressure can be held at any point between ambient and the top working pressure of 45 pounds per square inch above ambient. Pressurisation stops automatically when the preset level is reached or at 45 pounds per square inch. The rate of pressurisation is selected on the console and can be varied up to a maximum of approximately 8 pounds per square inch per minute. In the same manner, depressurisation can be controlled at any rate up to 8 pounds per square inch per minute down to ambient pressure, or held at any intermediate pressure according to the wishes of the operator.

Coloured lights on the control panel indicate whether the chamber is ready for use, in the pressurisation or depressurisation state, holding a steady pressure or holding at the chosen maximum pressure.

This pressure chamber is run as an open circuit. A continuous ventilating flow of oxygen passes through the chamber to wash out carbon dioxide and water vapour.

In the event of an emergency affecting the patient, fast depressurisation controls are provided on both the chamber and the console. An electrically operated dumping valve can be triggered from the control console or the same valve operated manually from the end of the chamber. A pressure drop of one pound per square inch per second is obtained when this valve is used, and it is possible to reduce from maximum pressure and remove the patient from the chamber in approximately one minute. If a total power failure should occur, a manually operated, slow depressurisation valve can be opened on the control console.

A number of sealed electrical plugs are provided in the door of the chamber. With suitable apparatus these allow electro-physiological monitoring of the patient throughout the course of the treatment.

Aims

The ‘pink, pulseless hand’ is often used to describe the clinical situation in which a child with a supracondylar fracture of the humerus has normal distal perfusion in the absence of a palpable peripheral pulse. The management guidelines are based on the assessment of perfusion, which is difficult to undertake and poorly evaluated objectively. The aim of this study was to review the available literature in order to explore the techniques available for the preoperative clinical assessment of perfusion in these patients and to evaluate the clinical implications.

Methods

A systematic literature review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and registered prospectively with the International Prospective Register of Systematic Reviews. Databases were explored in June 2022 with the search terms (pulseless OR dysvascular OR ischaemic OR perfused OR vascular injury) AND supracondylar AND (fracture OR fractures).

Combined bony and vascular injuries present challenging problems to orthopaedic and vascular surgeons. The use of temporary intraluminal vascular shunts produces significant reductions in ischaemia time and allows fracture stabilisation to be performed before definitive, delicate vascular repair. We report our management of a five-week-old infant who sustained a comminuted fracture of the femur with arterial and venous injuries in a shooting incident. Paediatric nasogastric feeding tubes were used as temporary vascular shunts to re-establish the distal circulation. Stabilisation of the fracture was then followed by vascular reconstruction and soft-tissue surgery, with a good result. We emphasise the need for skills from several surgical disciplines in the management of complex combined injuries

Introduction. Knee dislocations, vascular injuries and floating knee injuries can be initially managed by a external fixator. Fixator design constructs include the AO pattern and the Diamond pattern. However, these traditional constructs do not adhere to basic principles of external fixation. The Manchester pattern knee-spanning external fixator is a new construct pattern, which uses beam loading and multiplanar fixation. There is no data on any construct pattern. This study compares the stability of these designs. Materials & Methods. Hoffman III (Stryker, USA) external fixation constructs were applied to articulated models of the lower limb, spanning the knee with a diamond pattern and a Manchester pattern. The stiffness was loaded both statically and cyclically with a Bose 3510 Electroforce mechanical testing jig (TA Instruments). A ramp to load test was performed initially and cyclical loading for measurement of stiffness over the test period. The results were analysed with a paired t-test and ANOVA. Results. The mean stiffness with the diamond pattern fixator was significantly less stiff than the Manchester pattern fixator – by a factor of 3 (40N/mm vs 115N/mm). Displacement increased in all patterns over simulated loading equating to six weeks. The diamond pattern demonstrated a 50%% increase in displacement over time. The Manchester pattern demonstrated only 20% increase in displacement over time. These are all statistically significant (p<0.01). Conclusions. The aim of an external fixator in knee dislocations and vascular injuries is to provide stability, prevent displacement and protect repairs. Vascular injuries often require fixation for several weeks to protect a repair. The Manchester pattern, applying the principles of external fixation, provides a stiffer construct and also confers greater stability over the time a fixator may be required. We commend this more informed design for the management of knee dislocations and vascular injuries

Abstract. Introduction. Knee dislocations (KDs) are complex injuries which are often associated with damage to surrounding soft tissues or neurovascular structures. A classification system for these injuries should be simple and reproducible and allow communication among surgeons for surgical planning and outcome prediction. The aim of this study was to formulate a list of factors, prioritised by high-volume knee surgeons, that should be included in a KD classification system. Methods. A global panel of orthopaedic knee surgery specialists participated in a Delphi process. A list of factors to be included in a KD classification system was formulated by 91 orthopaedic surgeons, which was subsequently prioritised by 27 experts from 6 countries. The items were analysed to find factors that had at least 70% consensus for inclusion in a classification system. Results. The four factors that reached consensus agreement and thus deemed critical for inclusion in a classification system were vascular injuries (89%), common peroneal nerve injuries (78%), number of torn ligaments (78%), and open injuries (70%). Conclusion. The wide geographic distribution of participants provides diverse insight and makes the results of the study globally applicable. The most important factors to include in a classification system as determined by the Delphi technique were vascular injuries, common peroneal nerve injuries, number of torn ligaments, and open injuries. The Schenck anatomic classification system most accurately identifies these patient variables with the addition of open injuries. The authors propose to update the Schenck classification system with the inclusion of open injuries as an additional modifier

Abstract. Introduction. Traumatic knee dislocations are devastating injuries and there is no single best accepted treatment. This study looked at functional outcome of a single surgeon case series of patients who underwent surgical management of their knee dislocation. Methods. Eighty-seven patients with knee dislocation were treated at a major trauma centre. Acute surgical repair and reconstruction with fracture fixation within 4 weeks was preferred unless the patient was too unstable (Injury severity score>16). The collaterals were repaired and augmented using a variety of autografts, allografts or synthetic grafts. PCL was primarily braced and reconstructed subsequently, if required. Outcome was collected prospectively using IKDC score, KOOS and Tegner score. Results. The mean age of the patients was 33.6yrs (SD 11.9), 66 males and 21 females. According to Schenck classification, 26 were KD-I (30%), 6 KD-II (7%), 13 were KD-III (15%), 7 were KD-IV (8%) and 35 were KD-V (40%). 7 patients had CPN injury (8%), 3 had vascular injury (3.4%), 2 had combined CPN and vascular injury (2.3%). Acute surgical treatment was done in 62 patients while 10 had staged reconstruction. 22 patients had delayed reconstruction. The mean follow-up period was 5.8 years (1- 12 yrs), mean IKDC score was 57.6 (SD 23.6). The mean Tegner activity scale postoperatively was 4.5(SD 2) and the mean KOOS score was 74.9 (SD 19.6). Conclusion. A large proportion of cases were KD-V but still had a good outcome with surgical management. Current classification systems do not cover all dislocation patterns and a modification is proposed
