Aims. Tissue adhesives (TAs) are a commonly used adjunct to traditional surgical wound closures. However, TAs must be allowed to dry before application of a surgical dressing, increasing operating time and reducing intraoperative efficiency. The goal of this study is to identify a practical method for decreasing the curing time for TAs. Methods. Six techniques were tested to determine which one resulted in the quickest drying time for 2-octyle cyanoacrylate (Dermabond) skin adhesive. These were nothing (control), fanning with a hand (Fanning), covering with a hand (Covering), bringing operating room lights close (OR Lights), ultraviolet lights (UV Light), or prewarming the TA applicator in a hot water bath (Hot Water Bath). Equal amounts of TA were applied to a reproducible plexiglass surface and allowed to dry while undergoing one of the six techniques. The time to complete dryness was recorded for ten specimens for each of the six techniques. Results. Use of the Covering, OR Lights, and Hot Water Bath techniques were associated with a 25- (p = 0.042), 27- (p = 0.023), and 30-second (p = 0.009) reduction in drying time, respectively, when compared to controls. The UV Light (p = 0.404) and Fanning (p = 1.000) methods had no effect on drying time. Conclusion. Use of the Covering, OR Lights, and Hot Water Bath techniques present a means for reducing overall operating time for surgeons using TA for closure augmentation, which can increase intraoperative efficiency. Post-discharge surveillance of surgical site infection is necessary if accurate rates of infection following surgery are to be available. We undertook a prospective study of 376 knee and hip replacements in 366 patients in order to estimate the rate of orthopaedic surgical site infection in the community. The inpatient infection was 3.1% and the post-discharge infection rate was 2.1%. We concluded that the use of telephone interviews of patients to identify the group at highest risk of having a surgical site infection (those who think they have an infection) with rapid follow-up by a professional trained to diagnose infection according to agreed criteria is an effective method of identifying infection after discharge from hospital.

The posterior midline approach used in spinal surgery has been associated with a significant rate of wound dehiscence. This study investigates anatomical study of the arterial supply of the cervical and thoracic spinal muscles and overlying skin at each vertebral level. It aimed to provide possible anatomical basis for such wound complications.

A dissection and angiographic study was undertaken on 8 cadaveric neck and posterior torso from 6 embalmed and 2 fresh human cadavers. Harvested cadavers were warmed and hydrogen peroxide was injected into the major arteries. Lead oxide contrast mixture was injected in stepwise manner into the subclavian and posterior intercostal arteries of each specimen. Specimens were subsequently cross-sectioned at each vertebral level and bones elevated from the soft tissue. Radiographs were taken at each stage of this process and analysed.

The cervical paraspinal muscles were supplied by the deep cervical arteries, transverse cervical arteries and vertebral arteries. The thoracic paraspinal muscles were supplied by the superior intercostal arteries, transverse cervical arteries and posterior intercostal arteries. In the thoracic region, two small vessels provide the longitudinal connection between the segmental arteries and in the cervical region, deep cervical arteries provide such connection from C3 to C6. The arterial vessels supplying the paraspinal muscles on the left and right side anastomose with each other, posterior to the spinous processes in all vertebral levels. At cervical vertebral levels, source arteries travel near the surgical field and are not routinely cauterised; Haematoma is postulated to be the cause of wound complications. At thoracic levels, source arteries travel in the surgical field and tissue ischemia is a contributing factor to wound complications, especially in operations over extensive levels.

Post-operative wound complications is a multi-factorial clinical problem, the anatomical findings in this study provide possible explanations for wound dehiscence in the posterior midline approach. It is postulated that drain tubes may reduce the incidence of haematoma in the cervical level.

Background. Acute soft tissue defects (wound dehiscence or necrosis) after a total knee arthroplasty (TKA) may be the cause of the devasting complication of deep infection. When a medium (4–6cm) defect is present, in patellar or infra-patellar localization, a medial hemi-gastrocnemius flap is widely used to cover it, because of its low morbidity and high functional results. Normally, this coverage is not associated to a debridement, antibiotics and implant retention surgery (DAIR). When facing this situation, we should consider associating to the coverage treatment, like muscle flap, a DAIR procedure, in order to treat the possible acute infection, even when the diagnosis of infection is not clear. We could not find any studies comparing the benefice of this association in the same surgical act to isolated treatment of soft tissue defects. Our hypothesis was that when a TKA surgical wound defect is present, the risk of an acute infection is elevated and the patient would benefit from a muscle flap with DAIR procedure and polyethylene exchange. Methods. We performed a retrospective study to compare TKA infection clearance in patients with DAIR and flap in the same surgical act against those who received an isolated flap procedure for soft tissue coverage after an acute surgical wound defect. Patients were identified from a prospectively collated TKA database. Between 2005 and 2021, 19 patients met our inclusion criteria. A medial hemi-gastrocnemius flap was performed in 15 patients (78%). Healing or TKA infection clearance was defined as the presence of the original prosthesis after soft tissue coverage intervention, no need of DAIR after soft tissue coverage or no suppressive antibiotic treatment. Results. We obtained two groups. The first one, included those patients who had received the association of DAIR with polyethylene exchange and Flap (n=12). The other group included those who had received an isolated flap (n=7). We did not find differences in comorbidities and risk factors between both groups. In the combination treatment group 66,6% patients healed after treatment. In the other group, these favourable results decrease to 42,9%. Even though results were better in the combination treatment group, no significant differences were found. Conclusion. Although no significant statistical differences were found, probably due to small sample, the association of DAIR with polyethylene exchange and muscle flap is recommended in the coverage over an acute dehiscence or necrosis after TKA. More studies, with bigger sample are needed to extrapolate results in general population

The incidence of PJI in knee replacements is 2.8% and slightly lower with hip replacement surgery. PJI make up 15% (or even more) of knee revisions. To combat PJI, antibiotic laden bone cement has been used for many decades, but antibiotic stewardship dictates more prudent management of antimicrobials. Projected increase in infection rate, due to increased surgery and latent infection to be almost 5-fold up to 2035. Biofilm is a complex structure of bacteria and polysaccharide matrix and, is recognised as a major component in PJI and other orthopaedic infections. Biofilm is responsible for high incidence of resistance to antimicrobials and ineffective host immune response. Method. Stabilized hypochlorous acid has been reported to have a rapid kill rate on all pathogens, including MDR pathogens associated with chronic and acute wound infections. It destroys biofilm on contact, is not cytotoxic, reduces inflammation and stimulates wound healing. 0,038% of Hypochlorous acid was used as prophylaxis against infection and to treat PJI. We report on our experience with hypochlorous acid as a wound irrigation as prophylaxis against infection (more than 600 cases) and for PJI. We also report on a University study where a head to head analysis was done on the anti-biofilm efficacy between hypochlorous acid 0,038% (Trifectiv Surgical Wound Irrigation) and Product X (an industry-standard product for the prevention and treatment of biofilm infection. Hypochlorous acid offers a valuable addition to the armamentarium of wound antiseptics, with added anti-inflammatory value. An in vitro study demonstrated superior efficacy against biofilm when compared to Product X

Aim. The origin of surgical site and biomaterial-associated infection is still elusive. Microorganisms contaminating the wound may come from the air, the surgical team, or from the skin of the patient. Prior to surgery the skin of patients is disinfected, but bacteria deeper in the skin (e.g. in sweat glands or sebaceous glands), may not be reached. This study aims to assess a potential role of this intracutaneous bacterial reservoir in biomaterial-associated infection. Method. To study if cutaneous microbiota colonize the wound when released from the skin upon cutting, we isolated, quantified and identified aerobic and anaerobic bacteria from the skin of 99 patients undergoing trauma surgery, before and after skin disinfection, from the knife blades and from the wound directly after the first cut. Results. Ninety-nine percent of the patients were culture-positive before disinfection with chlorhexidine. Of these, 40% were still culture-positive after disinfection. Of these, 54% had a positive culture of the wound after the skin cut. Twenty percent of the patients with a negative culture after disinfection, nevertheless had a positive wound culture after cutting the skin. Staphylococcus epidermidis and Cutibacterium acnes were the most often cultured bacterial species. In 9%, more than 100 bacterial colonies were cultured from the wound, a dose that may cause biomaterial-associated infections. Conclusions. Bacteria residing in the skin and not eradicated by disinfection may enter the surgical wound upon cutting, resulting in contamination which may cause a biomaterial-associated infection. Use of two knifes likely reduces the risk of wound contamination

Introduction. Major trauma during military conflicts involve heavily contaminated open fractures. Staphylococcus aureus (S. aureus) commonly causes infection within a protective biofilm. Lactoferrin (Lf), a natural milk glycoprotein, chelates iron and releases bacteria from biofilms, complimenting antibiotics. This research developed a periprosthetic biofilm infection model in rodents to test an Lf based lavage/sustained local release formulation embedded in Stimulin beads. Method. Surgery was performed on adult rats and received systemic Flucloxacillin (Flu). The craniomedial tibia was exposed, drilled, then inoculated with S. aureus biofilm. A metal pin was placed within the medullary cavity and treatments conducted. Lf in lavage solutions: The defect was subject to 2× 50 mL lavage with 4 treatment groups (saline only, Lf only, Bactisure with Lf, Bactisure with saline). Lf embedded in Stimulin beads: 4 bead types were introduced (Stimulin only, Lf only, Flu only, Lf with Flu). At day 7, rats are processed for bioluminescent and X-ray imaging, and tibial explants/pins collected for bacterial enumeration (CFU). Results. Rats without treatments established a mean infection of 2×106 CFU/tibia. 4 treatment groups with a day 0, one-off lavage demonstrated >95% reduction in bacterial load 7 days post-op, with a reduction in CFU from 1×106/tibia down to 1×104/tibia. There was no statistically significant difference between each group (p = 0.55 with one way ANOVA). The stimulin bead experiments are ongoing and complete results will be obtained in the end of July. Conclusions. This research demonstrated a clinically relevant animal model of implanted metalware that establishes infection. No additional benefit was observed with a one-off, adjuvant Lf lavage during the initial decontamination of the surgical wound, compared with saline alone, and in combination with the antiseptic Bactisure. This animal model provides the foundation for future antibiofilm therapies

Aim. There are no studies in literature that analyze the effectiveness of closed-incisional negative pressure wound therapy (ciNPWT) in the treatment of bone and joint infections (BJI). The aim of the study was to evaluate the efficacy and the safety of the application of ciNPWT in the postsurgical wound management of patients with osteoarticular infections. Method. We conducted a perspective single-center study on patients with BJI treated between 01/2022 and 10/2022 with ciNPWT dressing application at the end of the surgical procedure. All patients were treated by a multidisciplinary team (MDT) approach and operated by the same surgical equipe. Inclusion criteria were: presence of periprosthetic joint infection (PJI), fracture-related infection (FRI), osteomyelitis (OM), septic arthritis (SA) surgically treated, after which ciNPTW was applied over the closed surgical wound. 30 patients (19M, 11F) have been analyzed with mean age of 56,10±17,11 years old; BJIs were all localized in the lower limb (16 PJI, 12 FRI, 1 SA, 1 OM). Results. We considered the following clinical local pre-operative parameters: presence of fistula (10 patients, 33,33%), presence of erythema (18 patients, 60%), presence of previous flap in the incisional site (7 patients, 23,33%). In 11 cases (36,67%) more than 3 previous surgical procedures were performed in the surgical site. The following surgical procedures were performed: 8 debridement and implants removal, 7 DAIR, 3 one-stage exchange, 6 two-stage exchange, 3 spacer exchange, 3 resection arthroplasty. Nineteen patients (63,34%) showed no occurrence of any local post-operative complication (erythema, hematoma, wound breakdown, wound blister, necrosis). Seven (23,33%) patients showed the presence of one or more postoperative complications that didn't require additional surgery. We observed four (13,33%) failures, defined as the need for further surgical procedures following the onset of a local complication: two patients had a wound breakdown before wound closure and two had a recurrence of infection after an uneventfully wound closure. All failures were within the group of joint infection (PJI+SA) and were affected by a multi drug resistant pathogen. Conclusions. In our series four patients required further surgery, but only two cases were related to incisional wound problems, that is consistent with aseptic joint revision surgeries data that are available in literature (3.4%-6.9%)[1-2]. Patients affected by BJI are a group with significant high risk of failure and therefore the use of ciNPWT should be considered. However, randomized clinical trials are needed to establish the superiority of the ciNPWT dressing over the standard one

The rate of arterial injury in trauma patients with pelvic ring fractures has been cited as high as 15%. Addressing this source of hemorrhage is essential in the management of these patients as mortality rates are reported as 50%. Percutaneous techniques to control arterial bleeding, such as embolization and REBOA, are being employed with increasing frequency due to their assumed lower morbidity and invasiveness than open exploration or cross clamping of the aorta. There are promising results with regards to the mortality benefits of angioembolization. However, there are concerns with regards to morbidity associated with embolization of the internal iliac vessels and its branches including surgical wound infection, gluteal muscle necrosis, nerve injury, bowel infarction, and thigh / buttock claudication. The primary aim of this study is to determine whether pelvic arterial embolization is associated with surgical site infection (SSI) in trauma patients undergoing pelvic ring fixation. This observational cohort study was conducted using US trauma registry data from the American College of Surgeons (ACS) National Trauma Database for the year of 2018. Patients over the age of 18 who were transported through emergency health services to an ACS Level 1 or 2 trauma hospital and sustained a pelvic ring fracture treated with surgical fixation were included. Patients who were transferred between facilities, presented to the emergency department with no signs of life, presented with isolated penetrating trauma, and pregnant patients were excluded from the study. The primary study outcome was surgical site infection. Multivariable logistic regression was performed to estimate treatment effects of angioembolization of pelvic vessels on surgical site infection, adjusting for known risk factors for infection. Study analysis included 6562 trauma patients, of which 508 (7.7%) of patients underwent pelvic angioembolization. Overall, 148 (2.2%) of patients had a surgical site infection, with a higher risk (7.1%) in patients undergoing angioembolization (unadjusted odds ratio (OR) 4.0; 95% CI 2.7, 6.0; p < 0 .0001). Controlling for potential confounding, including patient demographics, vitals on hospital arrival, open fracture, ISS, and select patient comorbidities, pelvic angioembolization was still significantly associated with increased odds for surgical site infection (adjusted OR 2.0; 95% CI 1.3, 3.2; p=0.003). This study demonstrates that trauma patients who undergo pelvic angioembolization and operative fixation of pelvic ring injuries have a higher surgical site infection risk. As the use of percutaneous hemorrhage control techniques increase, it is important to remain judicious in patient selection

Aim. Deadspace is the tissue and bony defect in a surgical wound after closure. This space is presumably poorly perfused favouring bacterial proliferation and biofilm formation. In arthroplasty surgery, an obligate deadspace surrounding the prosthesis is introduced and deadspace management, in combination with obtaining therapeutic prophylactic antibiotic concentrations, is important for limiting the risk of acquiring a periprosthetic joint infection (PJI). This study aimed to investigate cefuroxime distribution to an orthopaedic surgical deadspace in comparison with plasma and bone concentrations during two dosing intervals (8 h × 2). Method. In a setup imitating shoulder arthroplasty surgery, but without insertion of a prosthesis, microdialysis catheters were placed for cefuroxime sampling in a deadspace in the glenohumeral joint and in cancellous bone of the scapular neck in eighteen pigs. Blood samples were collected from a central venous catheter as a reference. Cefuroxime was administered according to weight (20 mg/kg). The primary endpoint was time above the cefuroxime minimal inhibitory concentration of the free fraction of cefuroxime for Staphylococcus aureus (fT > MIC (4 µg/mL)). Results. During the two dosing intervals, mean fT > MIC (4 µg/mL) was significantly longer in deadspace (605 min) compared with plasma (284 min) and bone (334 min). For deadspace, the mean time to reach 4 µg/mL was prolonged from the first dosing interval (8 min) to the second dosing interval (21 min), while the peak drug concentration was lower and half-life was longer in the second dosing interval. Conclusions. In conclusion, weight-adjusted cefuroxime fT > MIC (4 µg/mL) and elimination from the deadspace was longer in comparison to plasma and bone. Our results suggest a deadspace consolidation and a longer diffusions distance, resulting in a low cefuroxime turn-over. Based on theoretical targets, cefuroxime appears to be an appropriate prophylactic drug for the prevention of PJI. Acknowledgments. Aim. The most frequent mechanical failure in the osteosynthesis of intertrochanteric fractures is the cut-out. Fracture pattern, reduction quality, tip-apex distance or the position of the cervico-cephalic screw are some of the factors that have been associated with higher cut-out rates. To date, it has not been established whether underlying bacterial colonization or concomitant infection may be the cause of osteosynthesis failure in proximal femur fractures (PFF). The primary objective of this study is to assess the incidence of infection in patients with cut-out after PFF osteosynthesis. Method. Retrospective cohort study on patients with cut-out after PFF osteosynthesis with endomedullary nail, from January 2007 to December 2020. Demographic data of patients (such as sex, age, ASA), fracture characteristics (pattern, laterality, causal mechanism) and initial surgery parameters were collected (time from fall to intervention, duration of surgery, intraoperative complications). Radiographic parameters were also analyzed (tip-apex distance and Chang criteria). In all cut-out cases, 5 microbiological cultures and 1 anatomopathological sample were taken and the osteosynthesis material was sent for sonication. Fracture-related infection (FRI) was diagnosed based on Metsemakers et al (2018) and McNally et al (2020) diagnostic criteria. Results. Of the 67 cut-out cases, 16 (23.9%) presented clinical, analytical or microbiological criteria of infection. Of these sixteen patients, only in 3 of them the presence of an underlying infection was suspected preoperatively. A new osteosynthesis was performed in 24 cases (35.8%) and a conversion to arthroplasty in the remaining 43 (64.2%). A comparative analysis was performed between cases with and without infection. The groups were comparable in terms of demographic data and postoperative radiological data (using Chang criteria and tip-apex distance). Patients with underlying infection had a higher rate of surgical wound complication (56.3% vs 22%, p = 0.014), higher rates of leukocytes counts (11.560 vs 7.890, p = 0.023) and time to surgery (5.88 vs 3.88 days, p = 0.072). Conclusions. One out of four osteosynthesis failure in PFF is due to underlying FRI and in almost 20% were not unsuspected before surgery. In PFF osteosynthesis failures, underlying infection should be taken into account as a possible etiological factor and thus a preoperative and intraoperative infection study should be always performed

Aim. The efficacy of various irrigation solutions in removing microbial contamination of a surgical wound and reducing the rate of subsequent surgical site infection (SSI), has been demonstrated extensively. However, it is not known if irrigation solutions have any activity against established biofilm. This issue is pertinent as successful management of patients with periprosthetic joint infection (PJI) includes the ability to remove biofilm established on the surface of implants and necrotic tissues. The purpose of this study was to evaluate the efficacy of various irrigation solutions in eradicating established biofilm, as opposed to planktonic bacteria, in a validated in vitro model. Method. Established biofilms of Staphylococcus aureus and Escherichia coli were exposed to different irrigation solutions that included Polymyxin 500,000U/L plus bacitracin 50,000U/L, Vancomycin 1g/L, Gentamicin 80mg/L, Normal saline 0.9%, off-the-shelf Betadine 0.3%, Chlorhexidine 0.05%, Benzalkonium 1.3g/L, Sodium hypochlorite 0.125%, and Povidone-iodine 0.5%. Each experiment was conducted in a 96-well microtiter plate with a peg lid and standardized per the MBEC assay manufacturer's protocol. Following 2 minutes of solution exposure to the irrigation solution, residual biofilms were recovered by sonication. Outcome measures for antibiofilm efficacy were residual colony forming units (CFU) and optical density (690nm). Experiments were conducted in 24 replicates and the observations recorded by two blinded observers. Statistical analysis involved t-tests with Bonferonni adjustment. Results. Povidone-iodine 0.5%, Betadine 0.3%, Benzalkonium 1.3g/L, and Sodium hypochlorite 0.125% were significantly more efficacious against S.aureus biofilm versus all other solutions (p<0.001). Against E.coli biofilm, Povidone-iodine-0.5%, Benzalkonium-1.3g/L and Sodium hypochlorite-0.125% were also most effective compared to other irrigation solutions (p<0.001). Polymyxin-bacitracin, Gentamicin, Vancomycin, and Saline solutions had minimal activity against both E.coli and S.aureus biofilms (p<0.001). Similar trends were observed using both experimental endpoints (CFU and Turbidity) and both investigators (interrater reliability; r=0.99). Conclusion. This in vitro study observed that topical antibiotic solutions do not have any activity against established biofilms. Irrigations solutions containing adequate amount of povidone-iodine, betadine, sodium hypochlorite, and benzalkonium appear to have activity against established biofilm by gram positive and gram negative organisms. The use of these irrigation solutions may need to be considered in patients with established PJI

The incidence of MRSA infection is increasing worldwide. Costs incurred in treating MRSA infection are over twice that of normal patients, and the duration of hospital stay is up to 10 times longer. Risk factors are age, previous MRSA infection, prolonged hospitalization, patients from convalescent homes, immunocompromised states, vascular and pulmonary disease. Methods. A retrospective chart review was conducted on 14 patients who developed MRSA infection in our unit, over a period of six years. Data included: age, gender, neurological status, length of hospital and ICU admission, type of procedure performed, HIV status, co-morbidities, nutritional status, haemoglobin, sensitivities and treatment. Results. Age ranged from 2 to 52 (mean 15.75 years) and included four males, six females, and four children. Of the thirteen patients who developed Surgical Site Infection (SSI), nine were posterior surgical wounds. Two patients were HIV positive. Mean albumin and lymphocyte count was 34.88 and 2.37 respectively. The average wait to surgery was 23.8 hospital days, average length of ICU admission was 5.01 days. Signs of SSI developed at 11.75 days on average. Four cases showed sensitivity to Vancomycin, while ten were sensitive to Clindamycin. Patients were treated for a total of six weeks with antimicrobial therapy. Five patients required debridement, two required implant removal for chronic infection. Infection subsequently resolved in all patients. Conclusion. The risk factors were prolonged hospitalization, and posterior surgical wounds. Infection by community acquired MRSA was twice as common as nosocomial MRSA. Current recommendations are to treat superficial sepsis with topical Mupirocin, while systemic antibiotics are reserved for patients at risk for MRSA bacteraemia and who have prosthetic implants. Screening for patient colonization is recommended when risk factors are present, while staff screening is recommended following outbreaks. The cornerstone in preventing MRSA infection is strict hand hygiene

According to Webster's Dictionary, efficiency is defined as the capacity to produce desired results with a minimal expenditure of energy, money, time, and materials. For a surgeon performing an operative procedure this would mean “skillfulness in avoiding wasted time and effort.” (. www.webster-dictionary.org. ) The essential ingredient to becoming efficient is to promote a culture of efficiency. There are 10 elements: 1) proactive surgeon perspective; 2) effective utilization of preoperative holding area; 3) preoperative planning / templating; 4) development of preference cards; 5) operating room set-up protocols; 6) operating room team concept; 7) streamlined instrument sets; 8) consistent operative workflow; 9) standardised closure / dressings; and 10) prompt and meticulous room turnover. Efficient performance of an operative procedure requires skillfulness in avoiding wasted time and effort. Perioperative efficiencies are optimised by development of “swing,” “flip,” or “double occupancy” criteria, understanding of timing of when to initiate the anesthetic block for the next case, skin closure routine by physician assistant/nurse practitioner/private scrub, and marking the operative site of your first two patients upon arrival to the hospital or surgery center. Utilise a pro-active approach to prepare case carts the day before surgery. The operating room team turns over their own rooms, with a “clean as you go” mentality. Develop a formalised communication process for patient flow issues, such as real-time push-to-talk group calling phones. Determine in advance the number of instrument sets required for the day's caseload to mitigate flash sterilization and decrease room turnover time. The goal of the surgeon is to be out of the operating room for 5 minutes in between cases before the next incision, utilizing that time to enter orders, communicate with the family, dictate, and mark the operative site of the patient who will follow the one in the case about to start. Implant selection can help if consistent. Everyone must know the instrument trays including surgeon, scrubs, and nurses. Minimise both the number of trays and the redundancy of instrumentation. Templating should be done in advance of the day of surgery. Keep your surgery consistent and always deliver your best product. The workflow for inpatient and outpatient surgeries should be the same: same implant, same approach, and same closure. The culture of efficiency requires buy-in by all involved in the operative procedure. Every one entering the operating theatre should have proper body coverage – no hair visible, no nose visible. There should be a strict limit to needless activity: minimum opening of doors, no changing of personnel during an operation, and use of intercom/telephone to request equipment. As the surgeon and the team begin to embrace efficiency, surgical times will decrease. Multiple studies have demonstrated that increased surgical time is associated with a higher incidence of infection. This is secondary to time-dependent contamination of the surgical wound and field. The take home message is to develop and embrace efficiency. Operating room efficiency is the product of multiple factors including preoperative preparation, skilled anesthesia team, motivated operating room staff, choreographed surgery, and well-designed instrumentation. The surgeon is the captain of the ship and the staff follows his or her lead. Your operating room days will flow smoothly. Your operations will proceed with minimal stress. You will spend less time drinking coffee between cases and have more free time at the end of the day. However, most importantly, you will deliver a quality product to your patient

Corynebacterium Jeikeium is a pathogen rarely involved in orthopaedic infections. Till date only 14 cases of osteomyelitis are described in the literature, envolving the tibia, foot and prosthethic (hip and joint) infection. To our knowledge, Corynebacterium Jeikeium as not been reported as an infectious agent of the spine. Our goal is to describe a case of scoliosis surgical site infection by a Corynebacterium Jeikeium specimen. This is a retrospective and descriptive case report based on data from clinical records, patient observation and analysis of complementary exams. We present a 24 year old female with a history of premature birth, West syndrome, spastic cerebral palsy and spina bifida. She was sent to our consult for evaluation of dorsolombar scoliosis. In October of 2014, she was submitted to surgery – posterior spine arthrodesis and instrumentation (D10 to L5) with bilateral pedicle screws and two chromium-cobalt bars. The early post-operative period was without complications. She was discharge at the seventh day of internment and was seen, fifteen days postoperative, at the consultation office, where the dressings were changed, with no signs of surgical site infection. One month post-operative, she recurs to the office because of an apparent seroma at the surgical site wound. There was no reference to fever or other signs of local/systemic infection. A swabbing of the wound was done and the patient was medicated with Ciprofloxacine, 500mg 12/12 hours – the culture came back negative. Seven days later she was seen again, maintaining the seroma with purge of a serous-aspect fluid. Antibiotic therapy was maintained and another swabbing was collected – culture came back negative. Because of suspected surgical site infection, she was re-operated at December of 2014. Surgical wound debridement was performed; three tissue samples and one exudate were sent to the microbiological department. In all samples but one was identified a Corynebacterium Jeikeium. No sensitivity test was performed. Intravenous Vancomicine, 1 gram 12/12 hours was started and maintained during 8 days. Eleven days post-operative she was discharged with oral Vibramicin, 100 milligrams 12/12 hours for two weeks. She is currently being followed at the doctor's office, with no sign of reinfection of the surgical site. This is the first reported case describing an infection of the spine by a Corynebactereium Jeikeium. Isolation of the causative agent of infection and literature-based directed antibiotherapy are important for a successful outcome

Postoperative sepsis is a costly and potentially devastating problem in total joint arthroplasty. Airborne bacteria and other viable microorganisms shed from surgical staff are a source of deep prosthetic infection, and the density of airborne bacteria is correlated with the rate of postoperative joint sepsis in total joint arthroplasty surgery. Previous studies have also reported a positive relationship between the density of nonviable airborne particulate and viable CFU counts, both airborne and in the surgical wound, during surgery. The purpose of this study was to determine the extent to which a system that delivers a small field of local, directed HEPA-filtered air flow over the surgical field reduces airborne particulate and airborne bacteria during total hip arthroplasty. A minimum of 8 subjects per group provided 80% power (a = 0.05) to detect a =75% difference in bacterial density between groups. All patients who consented to undergo primary total hip arthroplasty were eligible. Thirty-six patients were prospectively randomized into three groups: directed air flow, air flow system present but turned off (sham), and control (standard) conditions. Airborne particulate and bacteria were continuously collected in consecutive 10 minute intervals within 5 cm of the surgical wound using an air sampling device. Data were analyzed using a generalized linear model for repeated measures. Particulate counts and bacterial density at the surgical site were 80% lower on average in the directed air flow group compared to the other two groups (p<0.001) (Figure 1). Density of particulate >10 μm in diameter was strongly related to bacterial density at the surgical site (p<0.001), as was staff count (p<0.001) and bacterial density at a control site that was remote from the surgical field (p<0.001). The directed air flow system's effectiveness in reducing bacteria appears to be related to its ability to reduce particulate that may carry and allow proliferation of bacteria. The directed air flow system is relatively simple to use and does not appear to hinder the function of the surgeon or operating room staff, impede access to the surgical site, or interfere with the surgical procedure. The directed air flow system can be used in any operating room environment to provide clean air equivalent to a properly-used, well-functioning laminar air flow system. The directed air flow system was effective in reducing airborne particulate and bacteria in the surgical field during total hip arthroplasty

Introduction. Ceramic ball heads are well known in hip arthroplasty for their superior tribology performance and high burst strength. To assess the ball head performance and the in-vivo fracture risk Pandorf et al 2008 examined the burst strength of BIOLOX®forte (pure aluminium oxide ceramic, CeramTec GmbH, Plochingen, Germany) ball heads on clean standard test tapers and contaminated test tapers. They found that fat tissue and scratches are reducing the burst strength to 40% and to 20% of the reference burst strength, respectively. The aim of this work is to investigate if BIOLOX®delta (alumina matrix ceramic, CeramTec GmbH, Plochingen, Germany) ball heads show a similar behaviour as BIOLOX®forte ball heads with respect to taper contamination. Materials and Methods. Each test series consisted of n=5 BIOLOX®delta 28–12/14 L ball heads and n=5 metal test tapers (Ti-6Al-4V, ISO 5832-3). For the reference series the metal tapers remained untouched representing the CeramTec standard test procedure. For the fluid series the ball heads were filled up with tap water or calf blood serum. For the solid series the metal test tapers were contaminated with small particles of bovine bone, commercially available bone cement and porcine fat tissue in the engagement zone. A chisel and a slight hammer tap were used to scratch the proximal region of the metal test taper. The ball heads were then manually attached to the contaminated metal test tapers without further force appliances. An apparatus according to ISO 7206-10 was used for burst testing. The tests were performed at CeramTec in-house test laboratory. Results. The contamination of the taper region showed an influence on the resulting burst strength which was qualitatively similar to the previously performed investigations with pure alumina ball heads. Discussion. Sterilized water is used for cleaning the surgical wound before attaching the ball head on the metal taper. A contamination of the metal taper with water is reducing the burst strength of BIOLOX®delta ball heads to 80% reference burst strength. Remains of blood and bone particles on the metal taper can lead to 63% of the reference burst strength. Remains of fat tissue on the metal taper can lead to 38% of the reference burst strength. The fat tissue is reducing the coefficient of friction, which leads in further consequence to a change of the stress distribution and a raise of the stress magnitude in the ceramic ball head. Scratches and grooves have the ability to reduce the burst strength to only 29% reference burst strength. The results confirm that the taper connection of the components have to be clean and dry in order to provide a sufficient and strong connection

Aim. According to Tsukuyama classification, late acute hematogenous prosthesis joint infections (PJI) should be treated with debridement and implant retention (DAIR). We report here a recurrent Salmonella Dublin hip prosthesis infection. Through this case, we show how a recurrence of chronic PJI may have an acute clinical presentation leading to an inadequate surgical treatment. Method. Case report. On May 2011, a 74-year-old woman with bilateral hip prostheses (implanted in 1998 (right) and 2001 (left)), was admitted to intensive care for sepsis and pain of her left hip. Blood cultures and a joint aspiration of the left hip yielded pure cultures of S.Dublin. The patient had a recent history of febrile diarrhea after consuming dubious meat. The patient underwent DAIR followed by a six-week antibiotherapy. Three years later, she presented to the emergency room for an acute onset febrile PJI of the right hip. The patient underwent DAIR of the right hip. Blood cultures, joint aspiration fluid, and all intraoperative periprosthetic tissue samples yielded S.Dublin. Colonoscopy and abdomen ultrasound were negative. The patient received two weeks of intravenous combined antibiotherapy followed by oral antibiotics for further 10 weeks. Six weeks post operatively, the surgical wound was healed and the patient walked normally. One year later, the patient was referred by her primary care practitioner for night fevers without local signs or dysfunction of her prostheses. Radioleucoscintigraphy showed right hip inflammation. Bilateral hip biopsies were nevertheless performed, yet S. Dublin was recovered solely from the right hip biopsy. A one-stage exchange of the right hip was performed. All intraoperative periprosthetic tissue samples yielded S.Dublin. A six-week-combined antibiotherapy was undertaken. One year later, the patient appeared free of infection and walked normally. Results. Whole-genome sequencing was carried out on the three patient's strains (2011/2014/2015). Their genomes differed by only six SNPs, suggesting that they derived from a single “infecting” strain. Conclusions. This is the first report of recurrent S.Dublin PJI proven by whole genome sequencing. In the absence of detectable gallbladder and/or intestinal carriage, it is most likely that S. Dublin was able to persist at the surface of prosthesis, leading to a chronic disease with recurrences occurring years after the initial episode. These recurrences were associated with a clinical acute onset of the infection, inducing an inadequate surgical treatment at the first time. A better knowledge of possible acutisation of chronic PJI by orthopedic surgeons may improve surgical management of these infections

Acute peri-operative blood loss warranting transfusion is a frequent consequence of major joint replacement (TJR) surgery. Significant peri-operative anemia can contribute to hypotension, dyspnea, coronary ischemia and other peri-operative medical events that can result in increased risk of peri-operative complications, readmissions and impair the patient's ability to mobilise after surgery resulting in a longer length of stay (LOS) and increase skilled nursing facility (SNF) utilization. The risks associated with allogeneic blood transfusions (ABT) administered to treat symptomatic peri-operative anemia are numerous and extend beyond the concerns of transmission of communicable disease (HIV, hepatitis, other). Patients receiving ABTs have been shown to have a longer hospital LOS, higher risk of infection, and higher mortality after TJR than those who do not require transfusion after surgery. As a result, many different pre-operative, peri-operative and post-operative strategies have been utilised to minimise peri-operative blood loss and transfusion need for patients undergoing TJR. Several studies have shown that the strongest predictor of the need for ABT in the TJR patient is the pre-operative hematocrit (Hct). As a result, all patients with unexplained pre-operative anemia should be evaluated for an underlying cause prior to elective TJR surgery. In recent years, focus has shifted towards peri-operative reduction of blood loss with the use of pharmacologic agents like tranexamic acid (TXA). These agents work by inhibiting fibrinolysis and activating plasminogen. Numerous studies have shown that TXA given IV, applied topically into the surgical wound or given orally have been shown to reduce peri-operative bleeding and ABT after both THA and TKR. Regardless of route of administration, all appear to be more efficacious and considerably more cost-effective in reducing the need for ABT than other methods discussed previously. Despite concerns about the potential increased thromboembolic risk in patients undergoing TJR, there does not appear to be any conclusive evidence suggesting an increased risk of venous thromboembolic disease (VTED) in TJR patients who receive peri-operative TXA. Although it may be unnecessary, many TJR surgeons still, however, avoid use of TXA in patients with a past history of VTED, stroke, coronary artery disease (including coronary stents), renal insufficiency, hypercoagulable state and seizure disorder. The use of topical TXA may be safer in some of these high risk patients since systemic absorption is minimal when administered via this route. Although the optimal method of administration (IV, topical, oral or combined) has not yet been determined based on safety, cost and reduction of need for ABT, incorporation of tranexamic acid into a blood conservation program is clearly the standard of care for all TJR programs that should nearly eliminate the need for ABT for patients undergoing TJR

The aim of this abstract is to show that acute osteomyelitis is one of the most feared complication of orthopedic surgery. A rapid and aggressive treatment is mandatory in order to avoid significant bone loss, joint destruction and, in most cases, salvage of the limb. After apparent cure of the infection, sequelae must be addressed. In this case, the joint destruction was important, so reconstruction procedures where impossible. In a superficial and relatively small joint such as the elbow, it is preferred to do an arthrodesis than an arthroplasty because the risk of reactivation of the infection with implant involvement is very important. We present a case report of a 69 years old woman, who had a supra-intra-condylar fracture (AO 13-C1) of the right humerus. She was treated with open reduction and internal fixation with 2 internal lag screws and 2 external lag screws. After 6 weeks, she was admitted with a dislocated elbow associated with pain, loss of limb function, cubita nerve palsy and a purulent discharge from the surgical wound. She started vancomycin and was submitted to surgery with debriment, hardware removal and fixation with an external fixator was used. The local signs of infection disappeared progressively. After normalization of the laboratory parameters of infection, the patient was submitted to an elbow arthrodesis using a posterior contour plate. The elbow achieved solid fixation and infection was eradicated, at least for the time being, allowing the patient to use the upper limb in her daily live activities. The treatment of post operative acute osteomyilits is challenging, In this case, after apparent solution of the infection, a solid fixation of the elbow was achieved, allowing the use of the upper limb in the patient daily activities
