Aim. D-dimer is a widely available serum test that detects fibrinolytic activities that occur during infection. Prior studies have explored its utility for diagnosis of chronic periprosthetic joint infections (PJI), but not explored its prognostic value for prediction of subsequent treatment failure. The purpose of this study was to: (1) assess the ability of serum D-dimer and other standard-of-care serum biomarkers to predict failure following reimplantation, and (2) establish a new cutoff value for serum D-dimer for prognostic use prior to reimplantation. Method. This prospective study enrolled 92 patients undergoing reimplantation between April 2015 and March 2019 who had previously undergone total hip/knee resection arthroplasty with placement of an antibiotic spacer for treatment of chronic PJI. Serum D-dimer level, erythrocyte sedimentation rate (ESR), and C-reactive protein (CRP) levels were measured preoperatively for all patients. Failure following implantation was defined per the Delphi consensus criteria. Optimal cutoffs for D-dimer, ESR, and CRP were calculated based on ROC curves and compared in their association with failure following reimplantation criteria at minimum 1-year follow-up. Results. 15/92(16.3%) patients failed reimplantation surgery at mean follow up of 2.9 years (range 1.0–4.8). Optimal thresholds for D-Dimer, ESR and CRP were determined to be 1300ng/mL, 30mm/hr, and 1mg/L, respectively. The failure rate in patient with positive D-dimer was significantly higher at 32.0% (8/25) compared to those with negative D-dimer 10.6% (7/66); p=0.024. In comparison, 17.8% (8/45) of patients with ESR above threshold failed, compared to 13.89% (5/41) below (p=0.555) and 16.0% (4/25) of patients with CRP above threshold failed, compared to 16.1% (10/62) below (p=1.000). Conclusions. Patients with elevated D-Dimer appear to be at higher risk of failure after reimplantation surgery. This serum marker may be used to generate an additional data point in patients undergoing reimplantation surgery, especially in circumstances when optimal timing of reimplantation cannot be determined based on clinical circumstances


Aim. The diagnosis of periprosthetic joint infection (PJI) remains a clinical dilemma, since presentations of PJI usually greatly overlap with aseptic failure (AF). The aim of this study is to evaluate the values of plasma fibrinogen, individually or in combination with CRP, ESR and WBC, for distinguishing PJI from AF. Method. We retrospectively enrolled 357 cases who underwent revision hip or knee arthroplasties in the Third Affiliated Hospital of Southern Medical University, Sun Yat-sen Memorial Hospital and the First Affiliated Hospital of Sun Yat-sen University from January 2013 to December 2021, including 197 AF, 116 PJI and 44 reimplantation. The diagnostic capacity of preoperative fibrinogen, CRP, ESR and WBC as well as their combinations for differentiating PJI from AF were assessed by ROC curves. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy were calculated according to the optimal cutoff value based on the Youden index. All biomarkers were further investigated for their potential ability to predict optimal timing of reimplantation as well as their diagnostic capacity in the subgroups of the knee and hip PJI. Furthermore, the correlations among fibrinogen, CRP and ESR in the patients with PJI and AF were analyzed to further evaluate the potential capacity of fibrinogen in the diagnosis of PJI. Results. The levels of fibrinogen, CRP, ESR and WBC were significantly higher in PJI group than in AF group. ROC analyses showed that the AUCs of fibrinogen, CRP, ESR and WBC were 0.879, 0.903, 0.879 and 0.685, respectively. The optimal threshold of fibrinogen is 4.04 g/L (74.1% sensitivity, 85.6% specificity, 76.1% PPV, 85.0% NPV and 81.8% accuracy). Combining fibrinogen with CRP and/or ESR (AUC: 0.903∼0.914) yielded almost equivalent diagnostic efficiency compared with the combination of CRP and ESR (AUC: 0.910). Besides, fibrinogen yielded AUCs of 0.869 (cutoff: 3.44 g/L) and 0.887 (cutoff: 4.12 g/L) in the hip and knee subgroups, with higher specificity and PPV of 93.1% and 96.1% in the knee PJI. Intriguingly, as for the cases with CRP < 10mg/L and ESR ≧ 30 mm/h, the specificity and NPV of fibrinogen for diagnosing PJI were 92.2% and 83.9%. Conclusions. Plasma fibrinogen is considered as a potential first-line screening marker for PJI detection and timing of reimplantation. As for the patients with an increased ESR but normal CRP, a low fibrinogen level (below 4.04 g/L) is more likely to rule out PJI

Aim. A two-stage exchange of an infected prosthetic joint (PJI) is considered the most effective surgical treatment of chronic PJIs, particularly in North America. However, reinfection rates are unacceptably high (10–20%). This could be the consequence of a persistent infection or a new infection introduced during the first or second stage of the exchange arthroplasty. We aimed to determine: i) the prevalence of positive cultures at reimplantation, ii) whether there is an association between positive cultures at reimplantation and reinfection during follow-up, and iii) if there is a microbiological correlation between primary infections, reimplantations and reinfections. Method. We retrospectively evaluated all two-stage exchange procedures performed at two academic centers between 2000 and 2015. Primary culture-negative PJIs and cases in whom no intraoperative cultures were obtained during reimplantation were excluded from the analysis. One or more positive intraoperative cultures during reimplantation were considered positive for infection. Reinfection was defined as the need for additional surgical intervention after reimplantation or the need for antibiotic suppressive therapy due to persistent clinical signs of infection. Results. A total of 424 cases were included in the final analysis with a mean follow-up of 48 months (SD 37). Eighty-eight cases (20.8%) had positive cultures during reimplantation (second stage) of which 68.1% (n=60) grew a different microorganism than during the first stage of the procedure. The percentage of positive cultures during reimplantation was higher for hips than for knees (26.5% vs 17.1%, p 0.02). For the total group, the reinfection rate during follow-up was 18.4% (78/424), which was 29.5% for the positive-culture group versus 15.5% for the culture-negative group at reimplantation (p=0.002). A positive culture during reimplantation was an independent risk factor for reinfection during follow-up in the multivariate analysis (OR 2.2 (95% CI 1.2 – 3.8), p 0.007). Reinfection was caused by a different microorganism than the primary infection (first stage) in 64.1% of cases (50/78). Conclusions. There is a very high rate of positive cultures at reimplantation, which are mostly attributed to a different microorganism than the primary infection and is associated with a worse outcome. These results stress the importance of developing treatment strategies for this particular population

It is strongly recommended that tissue and synovial fluid culture samples be obtained during reimplantation performed as part of a two-stage exchange arthroplasty. The incidence of positive cultures during reimplantation and the influence of positive cultures on subsequent outcome are unknown. This aim of this study was to determine the incidence of positive cultures during reimplantation and to investigate the association between positive cultures at reimplantation and the subsequent outcome. A retrospective review was conducted on 267 patients that met the Musculoskeletal Infection Society (MSIS) criteria for PJI that completed both stages of two-stage exchange arthroplasty (Table 1). Intraoperative culture results from tissue and/or synovial fluid were obtained. Cultures were positive in 33 cases (12.4%) undergoing reimplantation surgery (Figure 1). Treatment failure was assessed based on the Delphi consensus definition. Logistic regression analysis was performed to assess the predictors of positive culture and risk factors for failure of two-stage exchange arthroplasty. Treatment failure was 45.5% for those with a positive intraoperative culture and 20.9% in those with negative cultures at the time of reimplantation. When controlling for organism virulence, comorbidities, and other confounding factors, treatment failure was higher (odds ratio [OR]: 3.3; 95% confidence interval [CI]: 1.3–4.5) and occurred at an earlier time point (hazard ratio: 2.5; 95% CI: 1.3–4.5) in patients with a positive reimplantation culture. The treatment failure rate was not different between cases with two or more positive cultures (36.4%) and one positive culture (42.8%). Positive intraoperative cultures during reimplantation, regardless of the number of positive samples were independently associated with two times the risk of subsequent infection and earlier treatment failure. Surgeons should be aware that a positive culture at the time of reimplantation independently increases the risk of subsequent failure and needs to be taken seriously. Given the significance of these findings, future studies are needed to evaluate the optimal management of positive cultures during reimplantation surgery

Introduction. The goal of treating artificial joint infection is to relieve the infection quickly and re-establish joint function, but many patients have underlying diseases, and treatment is often made problematic by the diversity of the causative bacteria. In this study we assessed the factor that enabled revision arthroplasty in patients with infection after artificial hip arthroplasty, including bipolar hip arthroplasty, in our hospital. Subjectives and Methods. The subjects were the 16 patients (16 hips) with infection after hip arthroplasty who were treated in our hospital during the past 10 years. There were 7 males and 9 females, and their mean age was 69.8 years. Primary total hip arthroplasty had been performed in 6 hips, revision hip arthroplasty in 8 hips, and bipolar hip arthroplasty in 2 hips. Infected implants were removed as soon as possible and delayed reimplantations with an interval antibiotic spacer were attempted in all patients. 9 hips were successful in reimplantation (reimplantation group) and 7 hips were impossible (No reimplantation group). In this study we investigated age, complications, body mass index (BMI), body temperature, pain, rate of resistant bacteria, number of hip surgery prior to infection and clinical manifestations compared to two groups. Results. Age, rate of resistant bacteria, body temperature and number of surgery were not significantly different compared to two groups. In no reimplantation group BMI was significantly low. Also, local heat, redness and fistula as clinical manifestations had been observed in most no reimplantation hips. Conversely there were no associations with the presence or absence of swelling, tenderness and pain in initial consultation. Discussion. Garvin et al. have reported a two-stage reimplantation success rate for infected artificial joints of 91%. However, our results comparatively showed lower successful rate. It is suggested that lower BMI, presence of local heat, redness and fistulation were risk factors for reimplantation. Especially, these clinical manifestations means that the infection widely invade to subcutaneous tissue. Therefore, we should performed debridement and antibiotic treatment more carefully

Infection is a devastating complication following total hip arthroplasty. For chronically infected total hip arthroplasty; we utilise a 2-stage articulating antibiotic hip spacer technique. Our success rate with the technique is 90% with patients clinically free of infection at an average of five years postoperatively. 80% of the patients had a positive identification of the infecting organism. The other 20% had a positive frozen section and clinically infected joints. All patients receive a minimum of 6 weeks of intravenous antibiotics. Postoperative hip scores average 90 points. Some patients elected to keep their antibiotic spacers for up to 2 years before the 2nd stage. Advantages of this technique include improved patient function, maintenance of bone stock and soft tissue tension, thus simplifying reimplantation

Prosthetic joint infections (PJI) occur in 0.8–1.9 % of arthroplasties, but the absolute number is increasing because of the frequency of procedures. Two stage exchange is the most effective strategy, but failures are often described. Culture of perioperative tissues during removal of arthroplasty is a standard procedure but culture during second step is equally important to define a success or a failure. We retrospectively reviewed PJI treated with two stage-exchange from January 2011 and December 2012 at “Ospedale S. Maria Misericordia”, Albenga-Italy. The procedure calls for bacterial culture not only during first step but also during reimplantation. Antibiotic treatment is prolonged after reimplantation until the cultures availability. A failure was defined by persistence of infection for positive culture or reocurrence of infection during a follow up of at least 2 years in patients with negative cultures. Three positive cultures yielding phenotypically identical organisms, or a single specimen of a virulent microorganism (e.g. Staphylococcus aureus) were required to rule out false positive for contaminants. Patients with persistence of infection were treated for 3 months with antibiotics. 86 patients underwent the two stage treatment: 45 hip and 41 knee prosthesis. The average ESR before arthroplasty removal was 59 mm/ 1st h (range 5–120), the average CRP was 3.9 mg/dl (range 0.3 – 34). Coagulase-negative staphylococci were isolated in 31 cases, Staphylococcus aureus in 19, Streptococcus spp in 8 and enterococci in 4. Gram-negatives were isolated in 4 patients and polymicrobial infection in 6 patients. In 14 patients (16%) no pathogen was identified. A positive culture during reimplantation was documented in 11 (13%) cases: 8 coagulase-negative staphylococci, 2 Staphylococcus aureus, 1 Candida sp. All patients received 3 months of therapy after surgery and 6 of them were free of infection at 2 years of follow up after the end of treatment. Among the 75 patients with negative cultures, a relapse was documented in 2 (3%), after 5 and 24 months, respectively. These cases were treated with arthrodesis and 6 weeks antibiotic treatment, with resolution of infection but poor functional results. Overall the success rate of our strategy was 92% (79/86). In patients treated with two-stage exchange, the combination of cultures at reimplantation and antibiotic suppressive treatment for 3 months in presence of positive cultures, are associated with a high rate of success. Only a prolonged follow up can rule out a relapse and agree with a true resolution of infection

Aims. 1. A two-stage revision remains as the “gold standard” treatment for chronically infected total knee arthroplaties. 2. Evaluate technical challenges in two stage revision. Materials and Methods. Fourteen septic knee prostheses were revised with a minimum follow-up of 2 years. Static antibiotic-impregnated cement spacers were used in all cases. Intravenous antibiotics according to sensitivity test of the culture were applied during patients’ hospital stay and continued up to 3 weeks. Oral antibiotics were given for another 3 weeks. Second-stage surgery was undertaken after control of infection with normal erythrocyte sedimentation rate and C-reactive protein values. Extensile techniques were used if needed and metallic augments were employed for bone loss in revision of both femur and tibia components. Results. The average interval between the first-stage resection and reimplantation was 7 weeks. Significant improvement was obtained with respect to visual analog scale pain and clinical and functional scores, and infection was not seen in all cases following a two-stage revision total knee arthroplasty. Radiographic evaluation showed suitable alignment without signs of mechanical loosening. Average range of movement achieved was 0 to 100 degrees. Most common knee infected was the second knee in bilateral cases. Conclusions. This technique is a reasonable procedure in chronic infection in knee arthroplasty and provides proper functional and clinical results. However, it sometimes requires extensile surgical approaches that could imply arduous surgeries. Metallic augments with cement less stems available in most of the knee revision systems are a suitable alternative to handle bone deficiencies, avoiding the use of bone allograft with its complications

Different therapeutic options for prosthetic joint infections exist, but surgery remains the key. With a two-stage exchange procedure, a success rate above 90% can be expected. Currently, there is no consensus regarding the optimal duration between explantation and the reimplantation in a two-stage procedure. The aim of this study was to retrospectively compare treatment outcomes between short-interval and long-interval two-stage exchanges. Patients having a two-stage exchange of a hip or knee prosthetic joint infection at Lausanne University Hospital (Switzerland) between 1999 and 2013 were included. The satisfaction of the patient, the function of the articulation and the eradication of infection, were compared between patients having a short (2 to 4 weeks) versus a long (4 weeks and more) interval during a two-stage procedure. Patient satisfaction was defined as good if the patient did not have pain and bad if the patient had pain. Functional outcome was defined good if the patient had a prosthesis in place and could walk, medium if the prosthesis was in place but the patient could not walk, and bad if the prosthesis was no longer in place. Infection outcome was considered good if there had been no re-infection and bad if there had been a re-infection of the prosthesis. 145 patients (100 hips, 45 knees) were identified with a median age of 68 years (range 19–103). The median hospital stay was 58 days (range 10–402). The median follow-up was 12.9 months (range 0.5–152). 28 % and 72 % of the patients had a short-interval and long-interval exchange of the prosthesis, respectively. Patient satisfaction, functional outcome and infection outcome for patients having a short versus a long interval are reported in the Table. The patient satisfaction was higher when a long interval was performed whereas the functional and infection outcomes were higher when a short interval was performed. According to this study a short-interval exchange appears preferable to a long interval, especially in the view of treatment effectiveness and functional outcome

Aim

The effectiveness of mandatory joint aspiration prior to re-implantation in patients with a cement spacer already in place is unclear.

The aim of this study was to evaluate the role of culturing articular fluid obtained by joint aspiration prior to re-implantation in patients who underwent a two stage septic revision.

Method

A retrospective observational study was conducted, assessing51 patients that underwent a two stage septic hip or knee revision from 2010 to 2017.

According to the results of intraoperative cultures, after the first stage revision each patient was treated with an antibiotic protocol for 6–8 weeks. Following two weeks without antibiotics, a culture of synovial fluid was obtained. Synovial fluid was obtained by direct joint aspiration in cases of knee spacers by and by joint aspiration guided by fluoroscopy in the theatre room in cases of hip spacers. Synovial fluid was transferred into a Vacutainer ACD® flask. Samples were processed and analysed in the microbiology laboratory. Gram stains were performed and the sample was subsequently transferred into a BacALERT bottle (bioMérieux, France) and incubated in a BacALERT instrument for seven days.

Results of these cultures were recorded and compared with cultures obtained during re-implantation surgery.

Background

Well-fixed cementless stems are sometimes needed to be extracted in patients with complications including periprosthetic infection, stem-neck breakage or trunnionosis. The purpose of this study was to report the clinical outcome in patients undergoing re-implantation surgery following removal of a well-fixed porous-coated cementless stem by the femoral longitudinal split (FLS) procedure(Fig.1, Fig.2).

Methods

We conducted a retrospective study and radiographic review of 16 patients who had undergone re-implantation following the FLS procedure to remove a well-fixed stem due to periprosthetic infection, stem-neck breakage or trunnionosis. The study group consisted of 2 men and 14women with an average age of 68.4 years. Mean follow-up was 33.1± 25.0 months. Operation time, intraoperative bleeding, complications, causes of re-operation and clinical score were evaluated and the Kaplan-Meier method was used to evaluate the longevity of the stem.

Introduction

Periprosthetic infection is a serious complication after total hip arthroplasty (THA). Two stage procedure using antibiotic-impregnated cement spacer is one of the treatments for late chronic infection after THA. We investigated the effects of two stage procedure on the infection control and the recurrence of infection after revision THA.

Materials and Methods

We retrospectively reviewed a consecutive series of 10 cases of a periprosthetic infection after hip arthroplasty, including 3 THA and 7 bipolar hemiarthroplasty (BHA). They were treated with two stage procedure using antibiotic-impregnated cement spacer from 2004 to 2009. There were 4 women and 6 men with an average age of 68.4 years. The pathogens were methicilin-resistant Staphylococcus aureus (MRSA) in 3 cases, coagulase-negative Staphylococcus (CNS) in 2 cases, Enterococcus in 2 cases, Streptococcus in 2 cases, and unknown in 1 case.

After removal of the prosthesis, extensive debridement was performed to remove infected tissues and residual cement. After irrigation with iodine solution, antibiotic-impregnated cement spacer was inserted with proximal cement fixation to prevent dislocation and fracture of the cement spacer. The antibiotics that were impregnated in the cement spacer were VCM in 8 cases, MEPM in 1 case, and CAZ in 1 case. Intravenous antibiotics were administrated for 3 weeks after this first stage surgery, and then oral antibiotics were administrated until C-reactive protein (CRP) rates became negative. After confirming the culture of joint fluid contained no pathogens, second stage revision surgery was performed. The average follow-up period after revision THA was 2.3 years.

We investigated the period from first stage surgery to the day when CRP became negative, and to the day of second stage surgery. The incidence of recurrence of infection after revision THA was investigated.

Background. Postoperative dislocation is one of the main surgical complications and the primary cause for revision surgery after 2-stage implant exchange due to periprosthetic infection of a total hip arthroplasty. Objective. The aims of our study were (1) to determine the incidence of dislocation after two-stage THA reimplantation without spacer placement, (2) to evaluate relevant risk factors for dislocation and (3) to assess the final functional outcome of those patients. Method. We prospectively analyzed 187 patients who underwent a two-stage total hip arthroplasty (THA) revision after being diagnosed with periprosthetic joint infection (PJI) from 2013 to 2019. The mean duration of follow-up was 54.2 ± 24.9 months (>36 months). The incidence of postoperative dislocation and subsequent revision was estimated through Kaplan-Meier curves and potential risk factors were identified using Cox hazard regression. The functional outcome of the patients was assessed using the modified Harris Hip Score (mHHS). Results. The estimated cumulative dislocation-free survival was 87.2% (95% CI: 81.2%-91.3%) with an estimated 10% and 12% risk for dislocation within the first 6 and 12 months, respectively. The use of a dual-mobility construct had no significant impact on the dislocation rate. Increasing body mass index (BMI) (HR=1.11, 95% CI: 1.02-1.19, p=0.011), abductor mechanism impairment (HR=2.85, 95% CI: 1.01-8.01, p=0.047), the extent of elongation of the affected extremity between stages (HR=1.04, 95% CI: 1.01-1.07, p=0.017), the final leg length discrepancy (HR=1.04, 95% CI: 1.01-1.08, p=0.018) and PJI recurrence (HR=2.76, 95% CI: 1.00-7.62, p=0.049) were found to be significant risk factors for dislocation. Overall revision rates were 17% after THA reimplantation. Dislocated hips were 62% more likely to undergo re-revision surgery (p<0.001, Log-rank= 78.05). A significant average increase of 30 points in mHHS scores after second-stage reimplantation (p=0.001, Wilcoxon-rank) was recorded, but no difference was noted in the final HHS measurements between stable and dislocated hips. Conclusion. Dislocation rates after 2-stage THA reimplantation for PJI remain high, especially regarding overweight or re-infected patients. Careful leg length restoration and an intact abductor mechanism seem critical to ensure stability in these complex patients

Aim. Two-stage replacement is a frequent procedure in patients with chronic PJI. However, results in the literature after this procedure differ, ranging from 54% to 100% of infection eradication. Positive cultures at reimplantation, when performing the second stage, are perceived as a risk factor for reinfection. This study aims to determine the impact of positive cultures during the second stage on the outcome of patients undergoing a 2-stage septic replacement and the impact of antibiotic holidays between the first and the second stage. Method. We systematically searched four databases from inception to May 31, 2022. We combined terms related to PJI, joint replacement and culture results. We analysed the risk of failure when positive cultures at second stage and performed a subgroup analysis by antibiotic holiday period. Results. We included 24 studies with 2387 patients of which 432 had positive cultures during second stage (18.09%). Global failure rate was 18.01% (430 patients). When dividing failure by culture results during second stage, we found that failure in positive group was 37.01% (161/432 patients) and failure in negative group was 13.7% (269/1953 patients). In the meta-analysis (MA) the odds ratio (OR) was 4.047 (95% CI: 2.954–5.544). When performing the subgroup analysis by antibiotic holidays we found that the rate of positive cultures without and with holidays was 21.3% and 16.05%, respectively. Failure rate without holidays was 15% (90/600 patients) and with holidays was 17.3% (202/1165 patients) (p=0.21). Failure in each group was higher when cultures were positive (without holidays, 25% vs 12.2%, p=0.0003, and with holidays 41.1% vs 12.7%, p<0.0001). In the MA we found that those studies in which antibiotic holiday was reported had higher risk of failure when cultures were positive during second stage (OR 4.798 95%IC 3.142–7.325). When studies reported no antibiotic holidays also had a higher risk of failure when cultures where positive (OR 2.225 95%CI 1.103–4.489), though it was lower. Conclusions. There exists a higher risk of failure after a two-stage septic replacement when cultures are positive during reimplantation. Patients who followed no antibiotic holidays or antibiotic holidays before reimplantation have similar failure rate when cultures are negative. In both groups they have a higher risk of failure when cultures are positive, having higher risk those positive patients in the antibiotic holidays group. Therefore, importance in detecting these patients before reimplantation is crucial to ensure higher survival rates

Background. Two-stage revision arthroplasty is the standard treatment for chronic hip and knee periprosthetic joint infections (PJI). Accurate diagnosis of persistent infections at 2nd stage using established biomarkers and diagnostic criteria is of paramount importance. This study aimed to evaluate the diagnostic value of synovial calprotectin and alpha-defensin, and compare established diagnostic criteria from the International Consensus Meeting (ICM 2018) and the European Bone and Joint Infection Society (EBJIS 2021) to determine persistent PJI at the 2nd stage of a two-stage revision arthroplasty. Methods. We retrospectively analyzed 97 patients who underwent 100 two-stage revisions (hip: 39, knee: 61). Synovial fluid samples were assessed for calprotectin and alpha-defensin levels. ICM 2018 and EBJIS 2021 were applied to all patients undergoing 2nd stage revision. Receiver operating characteristic (ROC) curves and Youden Index were utilized to determine optimal cut-off values, and correlations between biomarkers were evaluated. The microbiological spectrum was analyzed at 2nd stage and re-revision surgery. Results. Calprotectin levels showed a sensitivity of 66.7%, specificity of 32.9%, and accuracy of 38.0% in predicting septic failure. Alpha-defensin showed sensitivity of 28.6%, specificity of 87.8%, and accuracy of 79.2%. Significant correlations included: calprotectin with PMN% (r = 0.471, p = 0.05) and alpha-defensin with WBC (r = 0.830, p < 0.01) in the successful cohort. For septic re-revisions, calprotectin and alpha-defensin were highly correlated (r = 0.969, p < 0.01). ICM correctly diagnosed persistent PJI in 26.7%, while EBJIS diagnosed 24.2%. The microbial spectrum shifted from gram-positive to gram-negative bacteria between reimplantation and re-revision surgeries. Conclusion. Synovial calprotectin and alpha-defensin demonstrated limited accuracy in ruling out persistent PJI at reimplantation. The low sensitivity of current diagnostic criteria, combined with the observed shift in microbial spectrum, underscores the challenges in diagnosing persistent PJI during 2nd stage of a two-stage revisions arthroplasty

Aim. Antibiotic loaded spacers are often used during a two-stage exchange for periprosthetic joint infections (PJI) both for its mechanical properties and as a means for local antibiotic delivery. The main goal of this study is to compare the rate of positive cultures during reimplantation with the use of different antibiotic loaded spacers: aminoglycoside only vs. combined glycopeptide/aminoglycoside vs. combined glycopeptide/carbapenem/aminoglycoside. Method. We retrospectively evaluated every two-stage exchange procedures for infected hip/knee arthroplasty between 2012–2018. Microbiological findings in the first and second stage were registered as well as the type of spacer and antibiotic(s) used. Cases in whom no cultures were obtained during reimplantation and cases without sufficient data on antibiotic(s) used in cement spacers were excluded. Results. Fifty-four cases were included (20THA and 34TKA), with an overall rate of positive cultures during reimplantation of 18.5% (10/54). The rate of positive cultures was statistically significant higher among spacers with monotherapy with aminoglycoside compared to spacers with combined antibiotic therapy- 35.7% (5/14) vs. 12.5% (5/40) respectively(p<0.05). Comparing those with combined glycopeptide/aminoglycoside (2/19) with triple glycopeptide/carbapenem/aminoglycoside therapy (3/21) there was no significant difference. Microorganisms present during the second stage were mostly staphylococci (coagulase-negative in four cases, S.aureus in three), Corynebacterium striatum, Enterococcus faecalis, C.albicans in one case each. In most cases (8/10), the isolated microorganism was the same as the first stage and was resistant to the antibiotic(s) used in the spacer in seven cases. Failure rate with the need for subsequent surgery was significantly higher at 60% (6/10) in cases with positive cultures at reimplantation compared to 4.5% (2/44) for those with negative cultures during reimplantation(p=0.0005). Conclusions. It has recently been suggested that adding a glycopeptide to the spacer may be advantageous when compared to spacers with aminoglycoside monotherapy, as it will produce significantly lower rates of positive cultures during reimplantation which have been shown to increase the risk of subsequent failure as is the case in our study. Local unavailability of obtaining powder aminoglycosides has driven us to manually add high doses of vancomycin and meropenem to commercially available low-dose gentamicin cement in many of our spacers and they seem to to perform just as well as commercially available vancomycin/gentamicin combination. Although many other variables not considered in this study may influence the rate of positive cultures during the second stage (quality of initial debridement, systemic antibiotic therapy, etc.), we believe these results portrait a sufficiently accurate picture of clinical results with the use of different spacers

Aim. Two stage revision is the most commonly used surgical treatment strategy for periprosthetic hip infections (PHI). The aim of our study was to assess the intra- and postoperative complications during and after two stage revision using resection arthroplasty between ex- and reimplantation. Method. In this retrospective cohort study, all patients treated with a two stage revision using resection arthroplasty for PHI were included from 2008 to 2014. During the first stage, the prosthesis was removed resulting in a resection arthroplasty without the use a PMMA spacer. During second stage, (cemented or uncemented) reimplantation of the hip prosthesis was performed. The cohort was stratified into two groups according to the length of prosthesis-free interval (≤10 weeks and >10 weeks). Data on complications during explantation, prosthesis-free interval, reimplantation, and after reimplantation was collected. The overall complication rate between both groups was compared using the chi-squared test. The revision-free and infection-free survival was estimated using Kaplan-Meier survival analysis. Results. Overall, 93 patients with hip PJI treated with two-stage revision performing resection arthroplasty were included, 49 had a prosthesis-free interval of ≤10 weeks, and 44 an interval of >10 weeks. A total of 146 complications was documented in the cohort. Patients were followed-up for a mean duration of 42.7 months, range: 13.1 – 104.6 months. Blood loss during reimplantation [n=25], blood loss during explantation [n=23], persistent infection during prosthesis-free interval [n=16], leg length discrepancy [n=13], and reinfection [n=9] were the most common complications. No complication showed a statistically significant difference between both groups except for wound healing disorder after reimplantation, which was more often reported in the group with > 10 weeks interval (p=0.009). A statistically significant increase of periprosthetic bone fractures (p=0.05), blood loss (p=0.039), and total number of complications (p=0.008) was seen with increasing acetabular bone defects (after Paprosky). Infection-free survival rate at 24 months was 93.9% (95% CI: 87.2 – 100) in the group with ≤10 weeks interval and 85.9% (95% CI: 75.4 – 96.4) with an interval of > 10weeks. Conclusions. After two years of follow-up, the infection-free survival rate using resection arthroplasty during two stage revision for PHI was higher in the group with ≤10 weeks interval compared to the group with >10 weeks interval. The most common complications during and after a two stage revision using resection arthroplasty were blood loss during the two surgeries, persistent infection during the prosthesis-free interval, leg length discrepancy, and reinfection

Traditional socket prosthesis (TSP) rehabilitation for amputees is associated with substantial dissatisfaction due to poor mobility and pain from soft tissue squeezing. Osseointegration (OI), eliminates skin compression and prosthesis fit issues, providing superior mobility and quality of life (QOL) for most amputees. The potential for OI to benefit patients with exceptionally poor mobility is understudied. To address this knowledge gap, this project investigated the mobility and QOL changes. A retrospective review was performed to identify transfemoral amputees who presented with wheelchair-bound mobility (K0) and had at least five years of post-osseointegration follow-up. Outcome measures included changes in mobility (K-level, daily prosthesis wear hours, Timed Up and Go (TUG), 6 minute walk test (6MWT)) and QOL (Questionnaire for persons with a Transfemoral Amputation (QTFA); Short Form Health Survey 36 (SF36). Adverse events recorded included debridement for infection, implant revision surgery, fractures, and implant failures. 9 patients with mean age 48.2±7.7 (range 34-59) years were included. The proportion of patients achieving K-level >2 improved from 0/9=0% to [9/9=100%], p<0.001. The 6MWT improved from 0±0 to 320.65±57 meters (p<0.001). The TUG improved from incapable to 10.68±2.7 seconds (p<0.001). Statistically significant QOL metrics included QTFA global score (33.33±31 vs 68.52±21, p=0.039), SF36 physical component score (30.03±6.3 vs 42.34±12, p=0.023) and the SF36 mental component score (47.89±8.8 vs 51.95±10.4, p=0.332). Six patients required operative intervention to address complications. Five developed infections: three had debridement, and 2 required implant removal and reimplantation. Osseointegration does confer significant sustained mobility and quality of life improvement for wheelchair bound transfemoral amputees. All 9 K0 patients had statistically improved mobility and QOL after osseointegrated reconstruction. This supports osseointegration as an effective rapid and long-term rehabilitation alternative for patients who have the neurologic and muscular capacity to walk but are unable due to issues related to their TSP

Aim. Periprosthetic joint infection (PJI) is a devastating complication that develops after total joint arthroplasty (TJA) whose incidence is expected to increase over the years. Traditionally, surgical treatment of PJI has been based on algorithms, where early infections are preferably treated with debridement, antibiotics, and implant retention (DAIR), while late infections with two-stage revision surgery. Two-stage revision is considered the “gold standard” for treatment of chronic PJI. In this observational retrospective study, we investigated the potential role of inflammatory blood markers (neutrophil-to- lymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR), platelet-to-lymphocyte ratio (PLR), systemic inflammatory index (SII)], systemic inflammatory response index (SIRI), and aggregate index of systemic inflammation (AISI)) as prognostic factors in two-stage exchange arthroplasty for PJI. Method. A single-center retrospective analysis was conducted, collecting clinical data and laboratory parameters from patients submitted to prosthetic explantation for chronic PJI. Laboratory parameters (PCR, NLR, MLR, PLR, SIRI, SII and AISI) were evaluated at the explantation time, at 4, 6, 8 weeks after surgery and at reimplantation time. Correlation between laboratory parameters and surgery success was evaluated, defined as infection absence/resolution at the last follow up. Results. 57 patients with PJI were evaluated (62% males; average age 70 years, SD 12.14). Fifty-three patients with chronic PJI were included. Nineteen patients completed the two-stage revision process. Among them, none showed signs of re-infection or persistence of infection at the last available follow up. The other twenty-three patients did not replant due to persistent infection: among them, some (the most) underwent spacer retention; others were submitted to Girdlestone technique or chronic suppressive antibiotic therapy. Of the patients who concluded the two-stage revision, the ones with high SIRI values (mean 3.08 SD 1.7, p-value 0.04) and MLR values (mean 0.4 SD 0.2, p-value 0.02) at the explantation time were associated with a higher probability of infection resolution. Moreover, higher variation of SIRI and PCR, also defined respectively delta-SIRI (mean −2.3 SD 1.8, p-value 0.03) and delta-PCR (mean −46 SD 35.7, p-value 0.03), were associated with favorable outcomes. Conclusions. The results of our study suggest that, in patients with PJI undergoing two-stage, SIRI and MLR values and delta-SIRI and delta-PCR values could be predictive of favorable outcome. The evaluation of these laboratory indices, especially their determination at 4 weeks after removal, could therefore help to determine which patients could be successfully replanted and to identify the best time to replant

Aim. The purpose of this study was to report on outcomes after stabilization of large skeletal defects following radical debridement of hip or knee infections and staged reimplantation using segmental antibiotic mega-spacers. Method. From 1998–2018, 39 patients (18 male, 21 female) were treated for musculoskeletal infections at the hip (14) or knee (25). Patients were treated for infection after a procedure related to oncology (20), arthroplasty (16), or trauma (3). Following debridement, defects were stabilized with antibiotic impregnated PMMA and intramedullary nails. All patients underwent a standardized protocol: 6 weeks of intravenous antibiotics followed by 6 weeks of oral antibiotics guided by intraoperative cultures. After a 6-week holiday of antibiotics, repeat intraoperative cultures and inflammatory markers were analysed for infection resolution. Success was defined by reimplantation without additional infection-related complications or requirement of suppressive antibiotics at latest follow-up. Results. Mean age was 50.5±19.4 years. Mean defect size was 20.4cm. Mean time from surgery until infection was 34.5 months, with 74% of patients presenting with infection greater than one year after their most recent surgery. Mean follow-up was 110±68 months. Most common organisms of infection were Staphylococcus Epidermidis (11) and Staphylococcus Aureus (10). Mean defect size was significantly different among oncology (28±8 cm), trauma (19±5 cm) and arthroplasty (12±6 cm) patients (p<0.0001), though outcomes were comparable. Two patients with antibiotic spacers have not underwent attempted reimplantation – one patient with clinical and laboratory signs of resolved infection; one patient with recent spacer placement. One patient died of oncologic disease shortly after spacer placement. These three patients were excluded from outcomes analysis. Twenty-nine (81%) patients were successfully re-implanted with a segmental endoprosthesis. Eight patients required an additional procedure prior to infection resolution, including additional antibiotic spacer and debridement due to sustained inflammatory markers and clinical signs of infection (5), antibiotic spacer exchange due to mechanical failure (2), and polyethylene exchange 9 months after reimplantation (1). Two patients have remained on chronic suppressive antibiotics, but have retained their limb, prosthesis, and pain-free function. Four (11%) patients ultimately required an amputation for infection control (3 above knee amputations; 1 hip disarticulation). Conclusions. Following radical debridement for infection, staged management of large segmental defects at the hip and knee with antibiotic cement and temporary intramedullary stabilization results in an 81% success-rate of limb salvage with infection control
