Implant waste during total hip arthroplasty (THA) represents a significant cost to the USA healthcare system. While studies have explored methods to improve THA cost-effectiveness, the literature comparing the proportions of implant waste by intraoperative technology used during THA is limited. The aims of this study were to: 1) examine whether the use of enabling technologies during THA results in a smaller proportion of wasted implants compared to navigation-guided and conventional manual THA; 2) determine the proportion of wasted implants by implant type; and 3) examine the effects of surgeon experience on rates of implant waste by technology used. We identified 104,420 implants either implanted or wasted during 18,329 primary THAs performed on 16,724 patients between January 2018 and June 2022 at our institution. THAs were separated by technology used: robotic-assisted (n = 4,171), imageless navigation (n = 6,887), and manual (n = 7,721). This study aimed to assess the carbon footprint associated with total hip arthroplasty (THA) in a UK hospital setting, considering various components within the operating theatre. The primary objective was to identify actionable areas for reducing carbon emissions and promoting sustainable orthopaedic practices. Using a life-cycle assessment approach, we conducted a prospective study on ten cemented and ten hybrid THA cases, evaluating carbon emissions from anaesthetic room to recovery. Scope 1 and scope 2 emissions were considered, focusing on direct emissions and energy consumption. Introduction. This study aims to determine how the acetabular version changes during the key developmental stage of adolescence, and what contributes to this change. In addition, we examined whether patient factors (BMI, activity levels) or the femoral-sided anatomy contribute to any observed changes. Patients/Materials & Methods. This prospective longitudinal cohort study included 19 volunteers (38 healthy hips). The participants underwent clinical examination (BMI, range of movement assessment), MRIs of both hips at recruitment and at follow-up (6 ± 2 years) and HSS Paediatric Functional Activity Brief Scale (Pedi-FABS) questionnaire. MRI scans were assessed at both time points to determine change of the tri-radiate cartilage complex (TCC), the acetabular anteversion, the degree of anterior, posterior, and superior femoral head coverage by the acetabulum, and anterior and antero-superior alpha angles. We investigated if the change in anteversion and sector angles was influenced by the BMI, range of movement measurements, the Pedi-FABS or the alpha angle measurements. Results. At the baseline MRI, all hips had a Grade I (open) TCC; the TCC was Grade III (closed) by follow-up MRI in all of the hips. The acetabular anteversion increased moving caudally further away from the roof for both time-points. The mean anteversion increased from 7.4° ± 3.8 to 12.2° ± 4 (p < 0.001). The increase in version occurred universally on the acetabulum but was greatest at the rostral ¼ of the acetabulum. The change in version did not correlate with any of the patient factors tested (p = 0.1–0.6). Discussion. There is concern about necrosis of the femoral head after resurfacing which can result in fracture and loosening. Widespread use of XLPE has reduced the incidence of both revision surgery and dislocations. This paper aims to create gross estimates of the magnitude of the resulting cost savings. Data about decreasing rates of revision surgery and dislocations after THA, comparing XLPE versus CPE, were obtained from the literature along with figures for costs. Gross estimates were generated from these figures. AOA NJRR 17 reports that at 15 years XLPE reduced the “all cause” revision rate of ceramic on polyethylene by 6.8% and at 16 years reduced the rate for metal on polyethylene by 6.5%. Their average 15 year figure was 4.8%, nearly in half. Dislocations were reduced by 24%. Other data substantiate these trends. Inclusive costs of nonseptic revisions in US dollars, incorporating those relevant costs for the year prior to and the year following surgery, have been shown to average in the neighborhood of $55,000. Dislocations add further to the costs. The estimated savings from XLPE by 15 years on those total hip replacements done in one year in North America equal nearly $1 billion, even without incorporating any increase annually in the number done. While the specific savings in other countries with lower costs are less, similar proportions likely obtain. Despite all the assumptions, the magnitude is striking. In addition to major benefits to the patients, the surgeons, and the hospitals, XLPE affords a massive advantage economically.
Fixation of osteoporotic proximal humerus fractures remains challenging even with state-of-the-art locking plates. Despite the demonstrated biomechanical benefit of screw tip augmentation with bone cement, the clinical findings have remained unclear, potentially as the optimal augmentation combinations are unknown. The aim of this study was to systematically evaluate the biomechanical benefits of the augmentation options in a humeral locking plate using finite element analysis (FEA). A total of 64 cement augmentation configurations were analyzed using six screws of a locking plate to virtually fix unstable three-part fractures in 24 low-density proximal humerus models under three physiological loading cases (4,608 simulations). The biomechanical benefit of augmentation was evaluated through an established FEA methodology using the average peri-screw bone strain as a validated predictor of cyclic cut-out failure.Aims

Aims. The aim of this study was to evaluate the suitability of the tapered cone stem in total hip arthroplasty (THA) in patients with excessive femoral anteversion and after femoral osteotomy. Methods. We included patients who underwent THA using Wagner Cone due to proximal femur anatomical abnormalities between August 2014 and January 2019 at a single institution. We investigated implant survival time using the endpoint of dislocation and revision, and compared the prevalence of prosthetic impingements between the Wagner Cone, a tapered cone stem, and the Taperloc, a tapered wedge stem, through simulation. We also collected Oxford Hip Score (OHS), visual analogue scale (VAS) satisfaction, and VAS pain by postal survey in August 2023 and explored variables associated with those scores. Results. Of the 58 patients (62 hips), two (two hips) presented with dislocation or reoperation, and Kaplan-Meier analysis indicated a five-year survival rate of 96.7% (95% CI 92.4 to 100). Some have suggested that these fractures should be treated in community hospitals so as not to tax the resources of Level One trauma centers. Since many factors predictive of fixation failure are related to technical aspects of the surgery, the purpose of this study was to compare radiographic parameters after fixation comparing trauma fellowship trained surgeons to non-fellowship trained community surgeons to see if these fractures can be treated successfully in either setting.
