The laterjet procedure is an important option in both primary and revision surgeries, especially in anterior shoulder instabilities that progress with glenoid bone loss. 12 patients who had a history of unsuccessful arthroscopic bankart repair and who underwent laterjet procedure in Ankara Atatürk Research and Training Hospital between 2013 and 2017 were included in the study and the patients were evaluated retrospectively. The mean age of the patients was calculated as 27.7 (range 21 to 38 years). Rowe and Walch-Duplay scores and operated shoulder (OS)-healthy shoulder (HS) range of motion (ROM) measurements were used to evaluate patients clinically and functionally. The mean follow-up time of the patients was calculated as 5.5±1.8 years. Firstly, no dislocation history was recorded in any patient afer the laterjet procedure. The mean Rowe score was calculated as 82.5 (range 60–100). Accordingly, 8 patients had excellent result, 3 patients had good result and 1 patient had fair result. The mean Walch-Duplay score was 81.4 (range 55–100). It was determined as excellent result in 6 patients, good result in 4 patients and fair result in 2 patients. Another data, joint ROM of the OS-HS of patients are shown in the table. There was some limitation in OS compared to the HS. p<0.05 value was accepted as statistically significant. While there was a statistically significant difference in external rotation (OS:35.2º, HS:56.4º)(p=0.003), internal rotation (OS:65.7º, HS:68.1º)(p=0.008) and flexion (OS:171.2º, HS:175.9º)(p=0.012) degrees but there was no statistically significant difference in abduction degrees (OS:164.3º, HS:170.4º) (p=0.089). Background. Innovative developments for total knee arthroplasty enhanced anatomical design and fixation in order to decrease particle-induced aseptic implant loosening. As hypersensitivity reactions to metallic implant materials have been recognized to possibly cause premature implant failure, ceramic materials might constitute a proper alternative solution. The aim of this prospective short-term study was the initial comparison of a completely metal-free ceramic with a geometrically identical metallic arthroplasty over a one-year follow-up period. The purpose of this study is to evaluate accuracy of tibia cutting and tibia implantation in UKA which used navigation system for tibia cutting and tibia component implantation, and to evaluate clinical results. We performed 72 UKAs using navigation system from November, 2012. This study of 72 knees included 56 females and 16 males with an average operation age of 74.2 years and an average body mass index (BMI) of 24.8 kg/m2. The diagnosis was osteoarthritis (OA) in 67 knees and osteonecrosis (ON) in 5 knees. The UKA (Oxford partial knee microplasty, Biomet, Warsaw, IN) was used all cases. We evaluated patients clinically using the Japanese orthopaedic association (JOA) score, range of motion (ROM), operation time, the amount of bleeding and complications. Patients were evaluated clinically at preoperation and final follow up in JOA score and ROM. As an radiologic examination, we evaluated preoperative and postoperative lower limb alignment in FTA (femoro-tibial angle) by weightbearing long leg antero-posterior alignment view X-rays. Also we evaluated a tibial component implantation angle by postoperative CT, and tibia cutting angle by intraoperative navigation system. We defined the tibial angle which a tibia functional axis and the tibia component made in coronal plane, also tibial posterior slope angle which a tibia axis and tibia component made in sagittal plane by CT. We measured tibial angle and tibial posterior slope angle by 3D template system. We performed UKA in all cases mini-midvastus approach. At first we performed osteotomy of the proximal medial tibia using CT-Free navigation. At this procedure we performed osteotomy to do re-cut if check did cutting surface in navigation, and there was cutting error (>3°), and then to do check again in navigation. Next we did not use navigation and went the osteotomy of the distal femur with an IM rod and drill guide of microplasty system. And then we performed a trial and decided bearing gap and moved to cementing. At first we went cementing of the tibia component. At this procedure we went to drive implant again if check did implant surface in navigation, and there was implantation error(>3°), and to do check. We checked did tibia cutting, tibia implantation carefully in navigation. In addition, We sterilize a clips and use it came to be in this way possible for the check of the first osteotomy side exactly. ROM was an average of 122.7° of preoperation became an average of 128.2° at final follow up, and JOA score was an average of 50.5 points of preoperation improved an average of 86.6 points at final follow up after UKA. There is no diagnostic, non-invasive method for the early detection of loosening after total hip arthroplasty. In a pilot study, we have analysed two serum markers of bone remodelling, procollagen I C-terminal extension peptide (PICP) and cross-linked N-terminal telopeptide (NTx), as well as the diagnostic performance of NTx for the assessment of osteolysis. We recruited 21 patients with loosening (group I), 18 with a well-fixed prosthesis (group II) and 17 at the time of primary arthroplasty for osteoarthritis (OA) (group III). Internal normal reference ranges were obtained from 30 healthy subjects (group IV). The serum PICP level was found to be significantly lower in patients with OA and those with loosening, when compared with those with stable implants, while the NTx level was significantly increased only in the group with loosening, suggesting that collagen degradation depended on the altered bone turnover induced by the implant. This hypothesis was reinforced by the finding that the values in the pre-surgery patients and stable subjects were comparable with the reference range of younger healthy subjects. There is controversy in the literatures regarding the best treatment for early knee osteoarthritis because there is a more controversy regarding the initiating factor of KOA The Objectives of this work were to evaluate the efficacy of various treatment regimens for the prevention of progression of early knee osteoarthritis (KOA). Also, to elucidate the factors for initiation and progression of KOA. Patients and Methods. Four groups of 50 patients with early KOA were treated with four treatment regimens. The first group (control) received analgesics as needed for one year. The second group received non steroidal anti-inflammatory drugs (NSAIDS) plus physiotherapy for one month; with analgesics as needed for the rest of the year. The third group received NSAIDS plus physiotherapy, plus vasoprotective and vasodilator drugs for one month; vasoprotective and vasodilator drugs for the next six months, and analgesics as needed for the rest of the year. The fourth group received NSAIDS plus physiotherapy, plus vasoprotective and vasodilator drugs plus alendronate for one month; vasoprotective and vasodilator drugs plus alendronate for the next six months; and analgesics as needed for the rest of the year. The age of the patients was from 40 to45years. There were 25 males and 25 females in each group. Patients with causes of secondary KOA (e.g. rheumatoid, gouty, traumatic, etc.) were excluded. There is little consensus regarding the regime for treatment of tuberculosis of spine, although WHO has laid down guidelines couple of years back classifying spinal tuberculosis in Category 1. This study proves the efficacy of WHO regime in spinal tuberculosis by clinico-radiological evaluation. Introduction. The medical fraternity is divided over the duration of chemotherapy in cases spinal tuberculosis. WHO clearly recommend spinal tuberculosis under Category I, but not accepted by most clinicians. Patient and Methods. In this prospective study during the period between August 2005 and July 2012, a total of 76 cases were diagnosed and evaluated clinico-radiologically to test the efficacy of WHO protocol (2HRZE+4HR) in our hospital with a mean follow up of 50 months (30 – 80 months). Results. Spinal tuberculosis was seen in 56% of all osteoarticular tuberculosis. Maximum population was between 11–50 years, females were involved more than males (66%), and regional distribution was different in males (Lumbar) and females (Thoracic). Skip and multifocal lesions were seen in 13% (6 cases), more common in immune compromised cases. Pain was the most common symptom (95%) followed by constitutional symptoms. Radiographic changes were nonspecific, appear late and suggestive of tuberculosis in 53%case, MRI is very useful in diagnosing in 95% cases especially when X ray is contributory. ESR is useful tool for follow up of patients, elevated in 94%cases. Results were evaluated on clinical, hematological and radiological basis. Of the total 64cases (after dropouts, lost in follow up, mortality), 50 patients (78%) received treatment for 6 months and14 cases for more than 6months (P value<0.001). No MDR cases were present. In 50 patients fall in ESR at the end of 2 months was found to be statistically significant (P value<0.05) and hence were given a treatment for 6 months, the fall at the end of 6 months was highly significant (P value<0.001). In rest of the 14 cases the duration of treatment was given for more than 6 months as the trend of fall of ESR was not significant. MRI changes were assessed in the form of osteitis, osteitis with discitis, abscess formation and granulation tissue on initiation of treatment, completion of treatment and 6 months after completion of treatment. The Nexgen LPS Uncemented Knee using a trabecular metal tibial component has performed well at minimum of 5 years’ follow-up. Introduction. Total Knee Arthroplasty prostheses most frequently used in today's practice have cemented components. These have shown excellent clinical results. The fixation can however weaken with time, and cement debris within the articulation can lead to accelerated wear. Cementless implants are less commonly used, but some have also shown good long-term clinical results. The potential advantages of cementless implants are retention of bone stock, less chance of third-body wear due to the absence of cement, shorter operative time, and easier treatment of periprosthetic fractures. The posterior stabilised knee replacement has been said to increase tangential shear stresses on the tibial component and increases contact stresses on the cam and post mechanism hence the great debate of cruciate retaining or cruciate sacrificing implants. Introduction. Very limited treatment options are available for osteoarthritis and most of them are for symptoms of osteoarthritis not for cause. Adult mesenchymal stem cells (MSCs), which have the ability to differentiate into cells of the chondrogenic lineage, have emerged as a candidate cell type with great potential for cell-based articular cartilage repair technologies. We conducted a study to see the effect of direct injection of stem cells on artificially created osteoarthritis model in rabbits. Methods. Surgical instability was created in 20 adult white rabbits over 16 weeks old and weighing over 2 kg using a modification of the technique of Hulth et al. Introduction. The purpose of this study was to evaluate the functional and radiographical mid-term follow-up results of a second generation metal-on-metal cementless total hip arthroplasty for the treatment of osteonecrosis of the femoral head in patients younger than 50 years. Methods. Twenty eight patients (35 hips) who underwent total hip arthroplasty with second generation metal-on-metal bearings for osteonecrosis of the femoral head at a minimum 5-year follow-up were included in this study. There were 5 women (6 hips) and 23 men (29 hips) who had a mean age of 40 years (range, 23 to 49 years) and a mean follow-up of 7 years. We used a Fitmore (Zimmer) cup and a 28mm Metasul femoral head in all cases. A CLS (Protek AG/Zimmer) femoral stem was used in 30 hips and a Cone prosthesis¯ (Protek AG) was used in 5 hips. Functional results were measured by Harris hip (HHS) and WOMAC scores. Radiographic evaluations were used to assess loosening and osteolysis according to Gruen and Delee and Charnley criteria. Results. The mean Harris hip score improved from 58 points (range, 35 to 67 points) pre-operatively to 92 points (range, 84 to 99 points) post-operatively, and the mean WOMAC score improved from 73 points (range, 63 to 89 points) to 30 points (range, 24 to 41 points). The purpose of this study was to evaluate the functional and radiographical results in patients who underwent a modified minimally invasive two-incision total hip arthroplasty using large-diameter ceramic-on-ceramic articulations for osteonecrosis of the femoral head. Methods. One hundred and one patients (135 hips) who underwent unilateral minimally invasive two-incision total hip arthroplasties using large diameter ceramic-on-ceramic bearings for osteonecrosis of the femoral head with a minimum 12-months follow-up were included in this study. There were 22 women and 79 men who had a mean age of 46 years (range, 22 to 82 years). The mean follow-up was 25 months (range, 12 to 36 months). All surgeries were done by a single hip surgeon. The authors modified the original minimally invasive two-incision total hip arthroplasty technique and used large-diameter (32mm, 36mm) ceramic-on-ceramic articulations. In the lateral position, an anterolateral approach between the gluteus medius and tensor fascia lata muscles and a posterior approach between the piriformis and gluteus medius muscles was used. Functional results were measured by Harris hip (HHS) and WOMAC scores. Radiographic evaluation was assessed for positioning of the components and complications were assessed. Results. The mean Harris hip score improved from 43 points (range, 13 to 58 points) pre-operatively to 96 points (range, 73 to 100 points) post-operatively and the mean WOMAC score improved from 67 points (range, 50 to 98 points) to 28 points (range, 26 to 39 points). Introduction. Hip resurfacing arthroplasty (HRA) with metal-on-metal bearing is currently emerging as a major evolution of hip arthroplasty. It could be an alternative to total hip arthroplasty. HRA also may allow young patients to perform high sports activities. It preserves proximal femoral bone stock and keeps the medullary canal intact. A large diameter of the femoral head provides wider range of the hip motion. Incidence of dislocation is very low. Most of HRA have been reported for young patients with stage of osteoarthritis. But, reports of HRA for osteonecrosis of the femoral head (ONFH) are rare and the outcomes are uncertain. Methods. This study was performed to introduce our indication and technique of HRA for the patients with ONFH and to assess the outcomes after a minimum 5-year follow up. After the biomechanical study, we set our own indications for HRA as follows: when the size of a lesion was less than 50% of the entire head and the rim at the head-neck junction was intact at least to a 5 mm above from the head-neck junction. From December 1998 to July 2004; 157 hips (139 patients) underwent HRA under the diagnosis of ONFH. Their mean age was 37 years. One hundred and fourteen hips were in men and 25 hips were in women. Methods. From December 2007 to July 2008, 45 hips (33 patients) underwent total hip arthroplasty for the treatment of osteonecrosis of the femoral head. There was 1 woman (2 hips) and 32 men (43 hips) who had a mean age of 39 years (range, 22 to 64 years). The minimum follow-up was 12 months (range, 12 to 19 months). The authors modified the original minimally invasive two-incision total hip arthroplasty technique and used large-diameter metal-on-metal articulations. In the lateral position, an anterolateral approach was used between the gluteus medius and tensor fascia lata muscles and for the posterior approach the muscle plane was between the piriformis and gluteus medius muscles. The acetabular components, Durom¯ (Zimmer) in 20 hips and Magnum¯ (Biomet) was used in 25 hips. M/L taper¯ (Zimmer) femoral stems were used in all cases. The size of the femoral heads were 38 mm (1 hip), 40 mm (3 hips), 42 mm (13 hips), 44 mm (18 hips), 46 mm (5 hips), 48 mm (4 hips) and 50 mm (1 hip). Postures such as excessive flexion or adduction which cause dislocation were not restricted, post-operatively. Functional results were measured by Harris hip scores (HHS), WOMAC scores, and range of motion. Radiographic evaluation was assessed for positions of components and post-operative complications were noted. Results. Mean operation time was 72 minutes (range, 54 to 94 minutes). The mean Harris hip score improved from 50 points (range, 38 to 73 points) pre-operatively to 96 points (range, 84 to 100 points) post-operatively, and the mean WOMAC score improved from 68 points (range, 50 to 93 points) to 28 points (range, 26 to 34 points). The mean flexion improved from 85° pre-operatively to 122° post-operatively. The mean internal rotation improved from 2.5° pre-operatively to 25.3° post-operatively. The mean external rotation improved from 31.8° pre-operatively to 60.1° post-operatively. The mean abduction improved from 24.0° pre-operatively to 41.6° post-operatively. Osteoarthritis (OA) is characterised by articular cartilage degradation. MicroRNAs (miRNAs) have been identified in the development of OA. The purpose of our study was to explore the functional role and underlying mechanism of miR-138-5p in interleukin-1 beta (IL-1β)-induced extracellular matrix (ECM) degradation of OA cartilage. Human articular cartilage was obtained from patients with and without OA, and chondrocytes were isolated and stimulated by IL-1β. The expression levels of miR-138-5p in cartilage and chondrocytes were both determined. After transfection with miR-138-5p mimics, allele-specific oligonucleotide (ASO)-miR-138-5p, or their negative controls, the messenger RNA (mRNA) levels of aggrecan (ACAN), collagen type II and alpha 1 (COL2A1), the protein levels of glycosaminoglycans (GAGs), and both the mRNA and protein levels of matrix metalloproteinase (MMP)-13 were evaluated. includes surgical debridement and filling the resultant void with antibiotic-loaded
polymethylmethacrylate cement, bone grafts or bone substitutes.
Recently, the use of bioactive glass to treat bone defects in infections
has been reported in a limited series of patients. However, no direct comparison
between this biomaterial and antibiotic-loaded bone substitute has
been performed. In this retrospective study, we compared the safety and efficacy
of surgical debridement and local application of the bioactive glass
S53P4 in a series of 27 patients affected by chronic osteomyelitis
of the long bones (Group A) with two other series, treated respectively
with an antibiotic-loaded hydroxyapatite and calcium sulphate compound
(Group B; n = 27) or a mixture of tricalcium phosphate and an antibiotic-loaded
demineralised bone matrix (Group C; n = 22). Systemic antibiotics
were also used in all groups. After comparable periods of follow-up, the control of infection
was similar in the three groups. In particular, 25 out of 27 (92.6%)
patients of Group A, 24 out of 27 (88.9%) in Group B and 19 out
of 22 (86.3%) in Group C showed no infection recurrence at means
of 21.8 (12 to 36), 22.1 (12 to 36) and 21.5 (12 to 36) months follow-up,
respectively, while Group A showed a reduced wound complication
rate. Our results show that patients treated with a bioactive glass
without local antibiotics achieved similar eradication of infection
and less drainage than those treated with two different antibiotic-loaded

The purposes of this study were to define the range of laxity of the interosseous ligaments in cadaveric wrists and to determine whether this correlated with age, the morphology of the lunate, the scapholunate (SL) gap or the SL angle. We evaluated 83 fresh-frozen cadaveric wrists and recorded the SL gap and SL angle. Standard arthroscopy of the wrist was then performed and the grades of laxity of the scapholunate interosseous ligament (SLIL) and the lunotriquetral interosseous ligament (LTIL) and the morphology of the lunate were recorded. Arthroscopic evaluation of the SLIL revealed four (5%) grade I specimens, 28 (34%) grade II, 40 (48%) grade III and 11 (13%) grade IV. Evaluation of the LTIL showed 17 (20%) grade I specimens, 40 (48%) grade II, 28 (30%) grade III and one (1%) grade IV. On both bivariate and multivariate analysis, the grade of both the SLIL and LTIL increased with age, but decreased with female gender. The grades of SLIL or LTIL did not correlate with the morphology of the lunate, the SL gap or the SL angle. The physiological range of laxity at the SL and lunotriquetral joints is wider than originally described. The intercarpal ligaments demonstrate an age-related progression of laxity of the SL and lunotriquetral joints. There is no correlation between the grades of laxity of the SLIL or LTIL and the morphology of the lunate, the SL gap or the SL grade. Based on our results, we believe that the Geissler classification has a role in describing intercarpal laxity, but if used alone it cannot adequately diagnose pathological instability. We suggest a modified classification with a mechanism that may distinguish physiological laxity from pathological instability.
We used interconnected porous calcium hydroxyapatite ceramic to bridge a rabbit ulnar defect. Two weeks after inducing the defect we percutaneously injected rabbit bone marrow-derived mesenchymal stromal cells labelled with ferumoxide. The contribution of an external magnetic targeting system to attract these cells into the ceramic and their effect on subsequent bone formation were evaluated. This technique significantly facilitated the infiltration of ferumoxide-labelled cells into ceramic and significantly contributed to the enhancement of bone formation even in the chronic phase. As such, it is potentially of clinical use to treat fractures, bone defects, delayed union and nonunion.
We evaluated the histological changes before and after fixation in ten knees of ten patients with osteochondritis dissecans who had undergone fixation of the unstable lesions. There were seven males and three females with a mean age of 15 years (11 to 22). The procedure was performed either using bio-absorbable pins only or in combination with an autologous osteochondral plug. A needle biopsy was done at the time of fixation and at the time of a second-look arthroscopy at a mean of 7.8 months (6 to 9) after surgery. The biopsy specimens at the second-look arthroscopy showed significant improvement in the histological grading score compared with the pre-fixation scores (p <
0.01). In the specimens at the second-look arthroscopy, the extracellular matrix was stained more densely than at the time of fixation, especially in the middle to deep layers of the articular cartilage. Our findings show that articular cartilage regenerates after fixation of an unstable lesion in osteochondritis dissecans.
This study was undertaken to elucidate the mechanism of biological repair at the tendon-bone junction in a rat model. The stump of the toe flexor tendon was sutured to a drilled hole in the tibia (tendon suture group, n = 23) to investigate healing of the tendon-bone junction both radiologically and histologically. Radiological and histological findings were compared with those observed in a sham control group where the bone alone was drilled (n = 19). The biomechanical strength of the repaired junction was confirmed by pull-out testing six weeks after surgery in four rats in the tendon suture group. Callus formation was observed at the site of repair in the tendon suture group, whereas in the sham group callus formation was minimal. During the pull-out test, the repaired tendon-bone junction did not fail because the musculotendinous junction always disrupted first. In order to understand the factors that influenced callus formation at the site of repair, four further groups were evaluated. The nature of the sutured tendon itself was investigated by analysing healing of a tendon stump after necrosis had been induced with liquid nitrogen in 16 cases. A proximal suture group (n = 16) and a partial tenotomy group (n = 16) were prepared to investigate the effects of biomechanical loading on the site of repair. Finally, a group where the periosteum had been excised at the site of repair (n = 16) was examined to study the role of the periosteum. These four groups showed less callus formation radiologically and histologically than did the tendon suture group. In conclusion, the sutured tendon-bone junction healed and achieved mechanical strength at six weeks after suturing, showing good local callus formation. The viability of the tendon stump, mechanical loading and intact periosteum were all found to be important factors for better callus formation at a repaired tendon-bone junction.
We have evaluated the effect of the short-term administration of low therapeutic doses of modern COX-2 inhibitors on the healing of fractures. A total of 40 adult male New Zealand rabbits were divided into five groups. A mid-diaphyseal osteotomy of the right ulna was performed and either normal saline, prednisolone, indometacin, meloxicam or rofecoxib was administered for five days. Radiological, biomechanical and histomorphometric evaluation was performed at six weeks. In the group in which the highly selective anti-COX-2 agent, rofecoxib, was used the incidence of radiologically-incomplete union was similar to that in the control group. All the biomechanical parameters were statistically significantly lower in both the prednisolone and indometacin (p = 0.01) and in the meloxicam (p = 0.04) groups compared with the control group. Only the fracture load values were found to be statistically significantly lower (p = 0.05) in the rofecoxib group. Histomorphometric parameters were adversely affected in all groups with the specimens of the rofecoxib group showing the least negative effect. Our findings indicated that the short-term administration of low therapeutic doses of a highly selective COX-2 inhibitor had a minor negative effect on bone healing.
External fixation of distal tibial fractures is often associated with delayed union. We have investigated whether union can be enhanced by using recombinant bone morphogenetic protein-7 (rhBMP-7). Osteoinduction with rhBMP-7 and bovine collagen was used in 20 patients with distal tibial fractures which had been treated by external fixation (BMP group). Healing of the fracture was compared with that of 20 matched patients in whom treatment was similar except that rhBMP-7 was not used. Significantly more fractures had healed by 16 (p = 0.039) and 20 weeks (p = 0.022) in the BMP group compared with the matched group. The mean time to union (p = 0.002), the duration of absence from work (p = 0.018) and the time for which external fixation was required (p = 0.037) were significantly shorter in the BMP group than in the matched group. Secondary intervention due to delayed healing was required in two patients in the BMP group and seven in the matched group. RhBMP-7 can enhance the union of distal tibial fractures treated by external fixation.
