Iliopsoas tendonitis occurs in up to 30% of patients after hip resurfacing arthroplasty (HRA) and is a common reason for revision. The primary purpose of this study was to validate our novel computational model for quantifying iliopsoas impingement in HRA patients using a case-controlled investigation. Secondary purpose was to compare these results with previously measured THA patients. We conducted a retrospective search in an experienced surgeon's database for HRA patients with iliopsoas tendonitis, confirmed via the active hip flexion test in supine, and control patients without iliopsoas tendonitis, resulting in two cohorts of 12 patients. The CT scans were segmented, landmarked, and used to simulate the iliopsoas impingement in supine and standing pelvic positions. Three discrete impingement values were output for each pelvic position, and the mean and maximum of these values were reported. Cup prominence was measured using a novel, nearest-neighbour algorithm. The mean cup prominence for the symptomatic cohort was 10.7mm and 5.1mm for the asymptomatic cohort (p << 0.01). The average standing mean impingement for the symptomatic cohort was 0.1mm and 0.0mm for the asymptomatic cohort (p << 0.01). The average standing maximum impingement for the symptomatic cohort was 0.2mm and 0.0mm for the asymptomatic cohort (p << 0.01). Impingement significantly predicted the probability of pain in logistic regression models and the simulation had a sensitivity of 92%, specificity of 91%, and an AUC ROC curve of 0.95. Using a case-controlled investigation, we demonstrated that our novel simulation could detect iliopsoas impingement and differentiate between the symptomatic and asymptomatic cohorts. Interestingly, the HRA patients demonstrated less impingement than the THA patients, despite greater cup prominence. In conclusion, this tool has the potential to be used preoperatively, to guide decisions about optimal cup placement, and postoperatively, to assist in the diagnosis of iliopsoas tendonitis.

Reports of iliopsoas impingement caused by acetabular components exist. The Psoas fossa (PF) is not a well-regarded landmark for Component positioning. Our aim was to assess the relationship of the TAL and PF in relation to Acetabular Component positioning. A total of 22 cadavers were implanted on 4 occasions with the an uncemented acetabular component. Measurements were taken between the inner edge of TAL and the base of the acetabular component and the distance between the lower end of the PF and the most medial end of TAL. The distance between the edge of the acetabular component and TAL was a mean of 1.6cm (range 1.4–18cm). The distance between the medial end of TAL and the lowest part of PF was a mean of 1.cm (range 1,3–1.8cm) It was evident that the edge of PF was not aligned with TAL. Optimal acetabular component position is vital to the longevity and outcome following THA. TAL provides a landmark to guide acetabular component position. Charnley originally stated that “Objectives must be reasonable. Neither surgeons nor engineers will ever make an artificial hip joint that will last 30 years and at some time in this period enable the patient to play football.” and he defined an appropriate patient as generally being over 65 years of age. Hip rating scales developed during this time were consistent with this approach and only required relief of pain and return to normal activities of daily living to achieve a perfect score. Since this time, however, hip arthroplasty has been applied to high numbers of younger, more active individuals and patient expectations have increased. One recent study showed that in spite of a good hip score, only 43% of patients had all of their expectations completely fulfilled following THA. The current generation metal-metal hip surface replacement arthroplasty (SRA) has been suggested as an alternative to standard THA which may offer advantages to patients including retention of more native bone, less stress shielding, less thigh pain due to absence of a stem, less limb length discrepancy, and a higher activity level. A recent technology review by the AAOS determined that currently available literature was inadequate to verify any of these suggested potential benefits. The potential complications associated with SRA have been well documented recently. The indications are narrower, the implant is more expensive, the technique is more demanding and less forgiving, and the results are both highly product and surgeon specific. Unless a clinical advantage in the level of function of SRA over THA can be demonstrated, continued enthusiasm for this technique is hard to justify. To generate data on the level of function of younger more active arthroplasty patients, a national multicenter survey was conducted by an independent university medical interviewing center with a long track record of conducting state and federal medical surveys. All patients were under 60, high demand (pre-morbid UCLA score > 6) and had received a cementless stem with an advanced bearing surface or an SRA at one of five major total joint centers throughout the country. The detailed questionnaire quantified symptoms and function related to employment, recreation, and sexual function. Patients with SRA had a higher incidence of noises emanating from the hip than other bearing surfaces although this was transient and asymptomatic. Although there is strong evidence that bisphosphonates prevent certain types of osteoporotic fractures, there are concerns that they may be associated with rare atypical femoral fractures. 1480 patients of proximal femur and shaft fractures over a period of 2 years from Jan 2014 to Jan 2016 were retrospectively reviewed in Gloucestershire Hospitals NHS trust. Hospital trauma database was used.195 patients had fractures in subtrochancteric and femoral shaft area. 11 patients had atypical femur fractures as defined by American society for bone and mineral research (ASBMR) task force 2013, revised criteria. Ten were female, one was male. Patients were aged from 68 to 97. In 6 patients, fractures were in the shaft, 5 in subtrochancteric area and 4 patients out of these had bilateral fractures. 10 out of 11 patients were on bisphosphonates. 4 patients had delayed diagnosis. 5 out of 11 patients did not have contralateral femoral x-rays. Treatment, 9 patients had intramedullary nail, one blade plate, and one treated conservatively. One patient in the IM group, had bilateral nailing. Average follow up was 7.6 months (range 1 to 16 months). At the end of the study, only 4 had united, 6 had not united and one not followed up. 4 out of 7 had low Vitamin D levels, 3 out of 7 had their bisphosphonate treatment stopped and 2 had histology which showed necrotic bone with trabeculae surrounded by fibrosis. Increasing number of patients are on bisphosphonates for osteoporosis. Atypical femur fractures from bisphosphonates are often occult, often bilateral, with delayed healing. Large heads offer substantial advantages over small ones in hip arthroplasty, as they are far less likely to dislocate. This feature is of particular benefit in very dysplastic females who often have a degree of joint laxity making dislocation a real possibility. Large metal heads have a range of problems, so registries report that they are now being substituted by large ceramic heads, typically reducing in diameter by 15% or more from the native size. All current designs of the femoral ball heads, whether for resurfacing of replacement share a unique design characteristic: a subtended angle of 120° defining the proportion of a sphere that the head represents. A novel design has recently been proposed that might reduce conflict between the femoral ball head rim and the iliopsoas tendon. This paper explains the problem of iliopsoas impingement on femoral heads of native diameter, and the consequences. Material and Methods. Using MRI, we measured the contact area of the Iliopsoas tendon on the femoral head in sagittal reconstruction of 20 hips with symptoms of FAI. We also measured the Articular extent of the femoral head on 40 normal hips and 10 dysplastic hips. We then performed virtual hip resurfacing on normal and dysplastic type hips, attempting to avoid the overhang of the rim inferomedially. Results. The contact area of the Iliopsoas tendon on the femoral head in extension is well visualized (Figure 1). The femoral head articular surface has a subtended angle of 120° anteriorly and posteriorly, but only of 100° medially. Virtual surgery in a femoral head of a dysplastic hip showed that when the femoral head is resurfaced with an anatomic sized component, the femoral ball head has a 20° skirt of metal protruding medially where iliopsoas articulates (figure 2). Reducing this by 15%, (eg to put a 40mm ball head onto a hip that had a 46mm femoral diameter), completely avoids any chance of iliopsoas tendon using the femoral head as a fulcrum. MRI of a dysplastic hip with a 40mm ball shows that iliopsoas impingement is hard to substantiate (figure 3). Discussion. The excessive extent of the femoral components of anatomic proportions may contribute to the pain felt by many following their use. However, the 15% reduction in head size undertaken in total hip arthroplasty completely defunctions the femoral head as a fulcrum.
Introduction. Total hip arthroplasty (THA) is the most common surgery performed for complications of bipolar arthroplasty. The present study evaluated the functional results and complications associated with this surgery. Patients and Methods. Forty eight hips (48 patients) who had conversion of bipolar arthroplasty to THA between 1998 June and 2013 June, and who were followed-up for more than one year were evaluated. Twenty one hips had conversion surgery to THA using a Fitmore cup with metal-on-metal articulation (28 mm head). Six hips had surgery using the SecurFit cup and three hips, using the Lima LTO cup with ceramic-on-ceramic articulation (28 mm or 32 mm head). Eighteen hips had surgery using a large head metal-on-metal bearing: –MMC (seven hips), ACCIS (six hips) and Magnum (five hips). The average time of follow-up duration was 3.9 years (range, 1.0–11.3). There were 22 men and 26 women between the ages of 28 and 80 years (average, 68.9 years) at the time of conversion surgery. Conversion arthroplasty was performed for acetabular erosion without femoral stem loosening in 19 hips, acetabular cartilage erosion with femoral stem loosening in 13 hips, periprosthetic fracture in 12 hips, and recurrent dislocation in four hips. Results were evaluated using Harris hip score (HHS) and Western Ontario McMaster Universities Osteoarthritis Index (WOMAC) score. The radiographs were analyzed for evidence of acetabular and femoral osteolysis or loosening. The complications were evaluated. Results. The time interval between hemiarthroplasty and conversion total hip prosthesis was 6.7 years on average (range, 5 months to 12 years). Pain was the leading sign in all patients. Average HHS improved from 42 preoperatively (range, 34–67) to 86 (range, 65 – 97) postoperatively. The average total WOMAC score improved from 47 (range, 32–67) to 22 (range, 9–44) postoperatively. I currently recommend this procedure when complete wear through of the polyethylene is present or impending, when the patient has obvious wear and symptoms, or if there is a rapidly enlarging osteolytic lesion. The surgical goals focus on management of debris generation and management of the osteolytic lesion. A third goal becomes avoidance of the know complications of this procedure. Management of debris generation basically involves modernising the head and polyethylene. Management of the osteolytic lesion includes debridement and when possible grafting. By far the most common complication after this procedure is dislocation. Prevention of dislocation should be accomplished by patient education, use of larger heads when possible, and capsular repair. Prerequisites to perform this procedure are a replacement liner of adequate thickness that can be locked or cemented in place. The acetabular component must be stable. Lastly the component must be properly oriented to minimise both wear and dislocation. Metal-on-metal liner conversion to metal-on-poly is becoming more common. Since patient satisfaction with THA is high, MoM patients may unknowingly minimise their symptoms because they are minor compared to the symptoms before surgery. Purpose. The purpose of the study was to determine the rate of conversion from RSA to THR in a number of Canadian centers performing resurfacings. Method. Retrospective review was undertaken in 12 Canadian Centers to determine the rate of revision and reason for conversion from RSA to THR. Averages and cross-tabulation with Chi-Squared analysis was performed. kaplan Meier survivorship was calculated. Results. A total of 2810 resurfacings were performed up to December 2008. 770 hips had a minimum of 5 year follow-up. The overall survivorship of this group was 97.8%. 80 patients underwent conversion to THR. Five resurfacing systems were used. [Background]. Bipolar hemiarthroplasty (following BHA) have historically had poor results in patients with idiopathic osteonecrosis of femoral head (OFNH). However, most recent report have shown excellent results with new generation BHA designs that incorporate advances in bearing technology. These optimal outcomes with bipolar hemiarthroplasty will be more attractive procedure for young patients who need bone stock for future total arthroplasty. The purpose of the current study was to evaluate the clinical and radiographic finding of this procedure for the treatment of OFNH at our institution after 7-to 21years follow-up. [Subjects and Methods]. We retrospectively reviewed a consecutive series of 29 patients (40 hips) who underwent primary bipolar hemiarthroplasty for ION (36 hips with stage III and 4 hips with stage IV) with a cementless femoral component between 1992 and 2006. Osteonecrosis was associated with corticosteroid use (23 patients), alcohol (16 patients), idiopathic (one patients). The mean follow-up duration was approximately 12 (range 7 to 21) years. Patients were evaluated according to the Japan Orthopaedic Association (JOA) hip score. We evaluate osteolysis and bone response of acetabulum or femur, and migration distance of outer head were calculated at the latest follow-up. Kaplan-Meier survivorship rate was investigated to examine implant failure rate. [Results]. The average the Japan Orthopedic Association (JOA) hip score significantly improved from 53.9±16.0 points (preoperative) to 89.6±8.0 points (final follow-up). Between April 2008 and February 2012, we implanted 159 large-diameter MOM stemmed THA with head diameters of 38–50mm. There were 6–38mm, 22–40mm, 42–42mm, 42–44mm, 24–46mm, 13–48mm, 4–50mm, 5–52mm, and one-54mm heads implanted in 138 patients (21 males and 117 females). The pre-operative diagnoses included: 120 OAs, 12 IONs, 4 femoral neck fractures, one RA, and one post-traumatic OA. Their ages were 40–86 years (avg. 63.6 yrs). Follow up was 4 to 67 months post implantation (avg. 40.4 months). All implants were manufactured by one company (Wright Medical Technology, Arlington, TN, USA). The stems were of a standard titanium-aluminum alloy, either 44 ANCA-FIT or 115 PROFEMUR Z non-cemented stems. Acetabular components were all CONSERVE PLUS cobalt-chromium monoblock shells.
