Iliopsoas tendonitis occurs in up to 30% of patients after hip resurfacing arthroplasty (HRA) and is a common reason for revision. The primary purpose of this study was to validate our novel computational model for quantifying iliopsoas impingement in HRA patients using a case-controlled investigation. Secondary purpose was to compare these results with previously measured THA patients.

We conducted a retrospective search in an experienced surgeon's database for HRA patients with iliopsoas tendonitis, confirmed via the active hip flexion test in supine, and control patients without iliopsoas tendonitis, resulting in two cohorts of 12 patients. The CT scans were segmented, landmarked, and used to simulate the iliopsoas impingement in supine and standing pelvic positions. Three discrete impingement values were output for each pelvic position, and the mean and maximum of these values were reported. Cup prominence was measured using a novel, nearest-neighbour algorithm.

The mean cup prominence for the symptomatic cohort was 10.7mm and 5.1mm for the asymptomatic cohort (p << 0.01). The average standing mean impingement for the symptomatic cohort was 0.1mm and 0.0mm for the asymptomatic cohort (p << 0.01). The average standing maximum impingement for the symptomatic cohort was 0.2mm and 0.0mm for the asymptomatic cohort (p << 0.01). Impingement significantly predicted the probability of pain in logistic regression models and the simulation had a sensitivity of 92%, specificity of 91%, and an AUC ROC curve of 0.95.

Using a case-controlled investigation, we demonstrated that our novel simulation could detect iliopsoas impingement and differentiate between the symptomatic and asymptomatic cohorts. Interestingly, the HRA patients demonstrated less impingement than the THA patients, despite greater cup prominence. In conclusion, this tool has the potential to be used preoperatively, to guide decisions about optimal cup placement, and postoperatively, to assist in the diagnosis of iliopsoas tendonitis.

Abstract. Introduction. Recurrent groin pain following periacetabular osteotomy (PAO) is a challenging problem. The purpose of our study was to evaluate the position and dynamics of the psoas tendon as a potential cause for recurrent groin pain following PAO. Methods. Patients with recurrent groin pain following PAO were identified from a single surgeon series. A total of 13 patients with 18 hips (4.7%) out of a 386 PAO, had recurrent groin pain. Muscle path of the psoas tendon was accurately represented using 3D models from CT data were created with Mimics software. A validated discrete element model using rigid body springs was used to predict psoas tendon movement during hip circumduction and walking. Results. Five out of the 18 hips did not show any malformations at the osteotomy site. Thirteen hips (72%) showed malformation secondary to callus at the superior pubic ramus. These were classified into: osteophytes at the osteotomy site, hypertrophic callus or non-union and malunion at the osteotomy. Mean minimal distance of the psoas tendon to osteophytes was found to be 6.24 mm (n=6) and to the osteotomy site was 14.18 mm (n=18). Conclusions. Recurrent groin pain after PAO needs a thorough assessment. One need to have a high suspicion of psoas issues as a cause. 3D CT scan may be necessary to identify causes related to healing of the pubic osteotomy. Dynamic ultrasound of the psoas psoas tendon may help in evaluating for psoas impingement as a cause of recurrent groin pain in these cases

Psoas tendinopathy is a potential cause of groin pain after primary total hip arthroplasty (THA). The direct anterior approach (DAA) is becoming increasingly popular as the standard approach for primary THA due to being a muscle preserving technique. It is unclear what the prevalence is for the development of psoas-related pain after DAA THA, how this can influence patient reported outcome, and which risk factors can be identified. This retrospective case control study of prospectively recorded data evaluated 1784 patients who underwent 2087 primary DAA THA procedures between January 2017 and September 2019. Psoas tendinopathy was defined as (1) persistence of groin pain after DAA THA and was triggered by active hip flexion, (2) exclusion of other causes such as dislocation, infection, implant loosening or (occult) fractures, and (3) a positive response to an image-guided injection with xylocaine and steroid into the psoas tendon sheath. Complication-, re-operation rates, and patient-reported outcome measures (PROMs) were measured. Forty-three patients (45 hips; 2.2%) were diagnosed with psoas tendinopathy according to the above-described criteria. The mean age of patients who developed psoas tendinopathy was 50.8±11.7 years, which was significantly lower than the mean age of patients without psoas pain (62.4±12.7y; p<0.001). Patients with primary hip osteoarthritis were significantly less likely to develop psoas tendinopathy (14/1207; 1.2%) in comparison to patients with secondary hip osteoarthritis to dysplasia (18/501; 3.6%) (p<0.001) or FAI (12/305; 3.9%) (p<0.001). Patients with psoas tendinopathy had significantly lower PROM scores at 6 weeks and 1 year follow-up. Psoas tendinopathy was present in 2.2% after DAA THA. Younger age and secondary osteoarthritis due to dysplasia or FAI were risk factors for the development of psoas tendinopathy. Post-operatively, patients with psoas tendinopathy often also presented with low back pain and lateral trochanteric pain. Psoas tendinopathy had an important influence on the evolution of PROM scores


Abstract. Optimal acetabular component position in Total Hip Arthroplasty is vital for avoiding complications such as dislocation and impingement, Transverse acetabular ligament (TAL) have been shown to be a reliable landmark to guide optimum acetabular cup position. Reports of iliopsoas impingement caused by acetabular components exist. The Psoas fossa (PF) is not a well-regarded landmark for Component positioning. Our aim was to assess the relationship of the TAL and PF in relation to Acetabular Component positioning. A total of 22 cadavers were implanted on 4 occasions with the an uncemented acetabular component. Measurements were taken between the inner edge of TAL and the base of the acetabular component and the distance between the lower end of the PF and the most medial end of TAL. The distance between the edge of the acetabular component and TAL was a mean of 1.6cm (range 1.4–18cm). The distance between the medial end of TAL and the lowest part of PF was a mean of 1.cm (range 1,3–1.8cm) It was evident that the edge of PF was not aligned with TAL. Optimal acetabular component position is vital to the longevity and outcome following THA. TAL provides a landmark to guide acetabular component position. However we feel the PF is a better landmark to allow appropriate positioning of the acetabular component inside edge of the acetabulum inside the bone without exposure of the component rim and thus preventing iliopsoas impingement at the psoas notch and resultant groin pain

Introduction. Iliopsoas impingement is a well described cause of groin pain after direct anterior total hip arthroplasty (THA). We proposed to evaluate the incidence, natural history and response to treatment of iliopsoas impingement after direct anterior total hip arthroplasty. Methods. A retrospective chart review of 725 consecutive patients who underwent anterior approach total hip arthroplasty between 2009 and 2014 was conducted. All surgeries were performed by one of two surgeons. Patients were included if they underwent primary anterior approach THA and had a minimum of 2 years of follow up. Patients who had a posterior approach, revision surgery or had less than 2 years of follow up were excluded. Iliopsoas impingement was identified if patients reported groin pain at greater than 6 weeks of postoperative follow up and in association with pain with resisted seated hip flexion. The natural history and response to treatment was recorded for patients identified as having iliopsoas impingement. Results. 900 patients met inclusion criteria. Of these, 120 (13.4%) developed groin pain following direct anterior total hip arthroplasty. The average time of onset was at 21 months postoperatively. At 2 years postoperatively, 16% of patients had symptoms, whereas 84% had resolution. 28% of patients responded to structured physical therapy, 22% improved with home stretching, 19% improved after arthroscopic psoas release, 9% after psoas sheath injection, and 6% required acetabular component revision. Conclusion. In our study population, iliopsoas impingement is not an uncommon finding after direct anterior total hip arthroplasty, but nearly half of these patients responded well to home stretching or physical therapy. In some cases, psoas injection and arthroscopic release was necessary. Rarely, cup revision was required for symptomatic relief

Introduction. Acetabular component position is an important determinant of stability, wear and impingement following total hip arthroplasty (THA). Its optimum position and size in direct anterior approach (DAA) THA has not been clearly described in previous studies. Our aim was to study the evolution of the same with reference to stability and impingement as a part of a single surgeon's learning curve. Methods. Clinical and radiographic records of first 300 consecutive DAA THAs performed by a single surgeon from April 2009 to April 2011 were reviewed from a prospective database at a single center. Radiographic analysis was done by two observers to determine acetabular inclination and anteversion on 6 week postoperative standing radiographs. Native femoral head size, measured on preoperative radiographs after adjusting for magnification, was used to calculate the native acetabular cup size. The study population was divided into three groups; Group A– 1. st. 100 DAA THA cases, Group B – 2. nd. 100 and Group C – 3. rd. 100 corresponding to the use of intraoperative anterior stability assessment (Group B and C) and change in the cup size strategy (Group C). The incidence of instability and psoas impingement (PI) –related groin pain at 2 year follow-up was determined for the three groups. Statistical analysis was done to see if there were differences in these clinical and radiographic outcome measures in the three groups. Results. Mean values for abduction were similar in all the groups. Mean anteversion was significantly lower in Group B [12.5° (± 3.3°)] and C [13.6 °(± 2.3°)] as compared to group A [24.3°(± 7.5°)]. The difference between the implanted cup size and calculated native cup size was significantly higher in Group A [5.2(± 2.1) mm] and group B [5.8 (± 2.60 mm] as compared to group C [1.4 (± 1.4) mm] (Figure 1). There were 2 anterior dislocations in group A with none in the other groups. The incidence of PI-related groin pain was higher in group B (12%) as compared to group A (2%) and group C (2%). For PI related groin pain, an arthroscopic psoas release was performed in 3 patients and cup revision in 1 patient. Discussion. For optimum anterior stability, the target cup anteversion is lower in the DAA. Increasing the cup size makes the use of large heads possible. However, the implanted cup size should be as close to the native cup size with DAA THA to avoid anterior overhang (psoas impingement) or posterior overhang (cup-neck impingement and anterior instability)

Introduction. Dual Mobility (DM) implants have gained popularity for the treatment and prevention of hip dislocation, with increased stability provided by a large diameter mobile insert. However, distal regions of the insert may impinge on soft tissues like the iliopsoas, leading to groin pain. Additionally, soft-tissue impingement may trap the mobile insert, leading to excessive loading of the insert rim from engagement with the femoral neck and subsequent intra-prosthetic dislocation. To address this, an Anatomically Contoured Dual Mobility (ACDM) insert with a soft-tissue friendly distal geometry was developed (Fig.1). Previously, the ACDM insert was shown to maintain the femoroacetabular contact area and joint stability of a conventional DM insert [Duffy et al. BJJ 2013, 95-B:34, p298; Zumbrunn et al. BJJ 2013, 95-B:34, p605]. The goal of this study was to utilize cadaver specimens to verify whether the ACDM insert could reduce soft-tissue impingement relative to a conventional DM insert. Methods. Fluoroscopic imaging was used to evaluate soft-tissue interaction with ACDM and conventional DM inserts in four cadaver hips (Fig. 2). A metal wire was sutured to the deep fibers of the iliopsoas muscle/tendon, and metal wires were embedded in the inner head and the mobile insert for fluoroscopic visualization. All soft tissue except the anterior hip capsule and iliopsoas were removed, and a rope was attached to the iliopsoas to apply tension along its native orientation. A femoral stem and a DM acetabular shell were implanted sothe ACDM or conventional DM inserts, together with the inner heads, could be inserted. Fluoroscopic images of the hip joint were taken at maximum hyperextension, 0°, 15° and 30° hip flexion with the insert positioned in neutral and anteverted orientations (Fig. 2). Neutral orientation corresponded to the insert axis parallel to the femoral neck, while anteverted orientation corresponded to a flexed insert that contacted the femoral neck posteriorly. Results. In all hips, fluoroscopic images revealed iliopsoas tenting with the conventional DM insert, and impingement of the iliopsoas occurred at low hip flexion angles (hyperextension, 0°, 15°) with the insert in neutral and anteverted orientations (Fig. 2 and 3). Further, at certain low flexion positions during dynamic motion, the movement of the conventional DM insert was blocked due to trapping of the insert by the anterior soft tissue and the femoral stem (Fig. 2B). At flexion angles above 30°, the iliopsoas moved away from the mobile insert and no impingement was seen. In all hips, the soft-tissue impingement and insert trapping was significantly reduced with the ACDM insert (Fig. 2 and 3). The reduction in impingement occurred with the insert in both neutral and anteverted orientations, although it was more evident for the latter. Conclusion. This study showed that conventional DM inserts impinge against the iliopsoas in low flexion, and their motion can be blocked by soft-tissue impingement. The Anatomically Contoured Dual Mobility (ACDM) insert significantly reduced this undesirable soft-tissue impingement. Thus, the ACDM insert may reduce the risk of groin pain and intra-prosthetic dislocation resulting from soft-tissue impingement and entrapment of the mobile insert

Total hip arthroplasty (THA) is among the most successful interventions in all of medicine and has recently been termed “The Operation of the Century”. Charnley originally stated that “Objectives must be reasonable. Neither surgeons nor engineers will ever make an artificial hip joint that will last 30 years and at some time in this period enable the patient to play football.” and he defined an appropriate patient as generally being over 65 years of age. Hip rating scales developed during this time were consistent with this approach and only required relief of pain and return to normal activities of daily living to achieve a perfect score. Since this time, however, hip arthroplasty has been applied to high numbers of younger, more active individuals and patient expectations have increased. One recent study showed that in spite of a good hip score, only 43% of patients had all of their expectations completely fulfilled following THA. The current generation metal-metal hip surface replacement arthroplasty (SRA) has been suggested as an alternative to standard THA which may offer advantages to patients including retention of more native bone, less stress shielding, less thigh pain due to absence of a stem, less limb length discrepancy, and a higher activity level. A recent technology review by the AAOS determined that currently available literature was inadequate to verify any of these suggested potential benefits. The potential complications associated with SRA have been well documented recently. The indications are narrower, the implant is more expensive, the technique is more demanding and less forgiving, and the results are both highly product and surgeon specific. Unless a clinical advantage in the level of function of SRA over THA can be demonstrated, continued enthusiasm for this technique is hard to justify. To generate data on the level of function of younger more active arthroplasty patients, a national multicenter survey was conducted by an independent university medical interviewing center with a long track record of conducting state and federal medical surveys. All patients were under 60, high demand (pre-morbid UCLA score > 6) and had received a cementless stem with an advanced bearing surface or an SRA at one of five major total joint centers throughout the country. The detailed questionnaire quantified symptoms and function related to employment, recreation, and sexual function. Patients with SRA had a higher incidence of noises emanating from the hip than other bearing surfaces although this was transient and asymptomatic. SRA patients were much more likely to have less thigh pain than THA, less likely to limp, less likely to perceive a limb length difference, more likely to run for exercise, and more likely to run longer distances. In another study of over 400 THA and SRA patients at two major academic centers, patients completed pain drawings that revealed an equivalent incidence of groin pain between THA and SRA, but an incidence of thigh pain in THA that was three times higher than in SRA in young active patients. While some or most of the observed advantages of SRA over THA may be attributable to some degree of selection bias, the inescapable conclusion is that SRA patients are demonstrating clinical advantages that warrants continued utilization and investigation of this procedure

We herein report a case of isolated hip pain in a four year old boy. The importance of this project is the unusual history, presentation, ultrasound, MRI and blood culture results and treatment, which lead to the diagnosis of adductor pyomyositis with a rare organism (Streptococcus Mitis) in a temperate country. The patient presented with a one day history of malaise, fever, left groin pain and inability to weight bear on the left leg. There was no history of any predisposing infections or recent travel. A working diagnosis of transient synovitis / septic arthritis of the hip was made on clinical examination. Plain radiograph and ultrasound of the hip was normal with no effusion. Two consecutive blood cultures suggested Streptococcus Mitis bacteriaemia and MRI scan confirmed pyomyositis of the left hip adductors that was too small to drain. Streptococcus Mitis is a normal commensal organism of the oral cavity however it can lead to opportunistic infections particularly endocarditis. Echocardiogram revealed no cardiac complications, in particular no endocarditic vegetation. Patient was treated with intravenous benzylpenicillin for a week followed by oral phenoxymethylpenicillin for a week. As it was a soft tissue infection, a short course of antibiotics was sufficient and he made a complete recovery. Adductor pyomyositis must be considered as a differential diagnosis in a child with unusual presentation of hip pain. When an ultrasound is normal, MRI scan is warranted to confirm diagnosis. Septic screen should include blood cultures. The commonest organisms are the Staphylococcus family. However if Streptococcus Mitis is isolated, cardiac sources of infection resulting in septic emboli must be investigated. Repeated MRI scans are required particularly if the patient does not respond to medical management

Although there is strong evidence that bisphosphonates prevent certain types of osteoporotic fractures, there are concerns that they may be associated with rare atypical femoral fractures. 1480 patients of proximal femur and shaft fractures over a period of 2 years from Jan 2014 to Jan 2016 were retrospectively reviewed in Gloucestershire Hospitals NHS trust. Hospital trauma database was used.195 patients had fractures in subtrochancteric and femoral shaft area. 11 patients had atypical femur fractures as defined by American society for bone and mineral research (ASBMR) task force 2013, revised criteria. Ten were female, one was male. Patients were aged from 68 to 97. In 6 patients, fractures were in the shaft, 5 in subtrochancteric area and 4 patients out of these had bilateral fractures. 10 out of 11 patients were on bisphosphonates. 4 patients had delayed diagnosis. 5 out of 11 patients did not have contralateral femoral x-rays. Treatment, 9 patients had intramedullary nail, one blade plate, and one treated conservatively. One patient in the IM group, had bilateral nailing. Average follow up was 7.6 months (range 1 to 16 months). At the end of the study, only 4 had united, 6 had not united and one not followed up. 4 out of 7 had low Vitamin D levels, 3 out of 7 had their bisphosphonate treatment stopped and 2 had histology which showed necrotic bone with trabeculae surrounded by fibrosis. Increasing number of patients are on bisphosphonates for osteoporosis. Atypical femur fractures from bisphosphonates are often occult, often bilateral, with delayed healing. Patients on bisphosphonatetreatment should be advised to report any thigh or groin pain. Painful incomplete fractures need treatment with cephalomedullary nailing. Bone biology needs correcting by stopping bisphosphonatesand administering calcium & vitamin D supplements. Implications: We need to raise awareness amongst treating clinicians and have national guidelines

Large heads offer substantial advantages over small ones in hip arthroplasty, as they are far less likely to dislocate. This feature is of particular benefit in very dysplastic females who often have a degree of joint laxity making dislocation a real possibility. Large metal heads have a range of problems, so registries report that they are now being substituted by large ceramic heads, typically reducing in diameter by 15% or more from the native size. All current designs of the femoral ball heads, whether for resurfacing of replacement share a unique design characteristic: a subtended angle of 120° defining the proportion of a sphere that the head represents. A novel design has recently been proposed that might reduce conflict between the femoral ball head rim and the iliopsoas tendon. This paper explains the problem of iliopsoas impingement on femoral heads of native diameter, and the consequences. Material and Methods. Using MRI, we measured the contact area of the Iliopsoas tendon on the femoral head in sagittal reconstruction of 20 hips with symptoms of FAI. We also measured the Articular extent of the femoral head on 40 normal hips and 10 dysplastic hips. We then performed virtual hip resurfacing on normal and dysplastic type hips, attempting to avoid the overhang of the rim inferomedially. Results. The contact area of the Iliopsoas tendon on the femoral head in extension is well visualized (Figure 1). The femoral head articular surface has a subtended angle of 120° anteriorly and posteriorly, but only of 100° medially. Virtual surgery in a femoral head of a dysplastic hip showed that when the femoral head is resurfaced with an anatomic sized component, the femoral ball head has a 20° skirt of metal protruding medially where iliopsoas articulates (figure 2). Reducing this by 15%, (eg to put a 40mm ball head onto a hip that had a 46mm femoral diameter), completely avoids any chance of iliopsoas tendon using the femoral head as a fulcrum. MRI of a dysplastic hip with a 40mm ball shows that iliopsoas impingement is hard to substantiate (figure 3). Discussion. The excessive extent of the femoral components of anatomic proportions may contribute to the pain felt by many following their use. However, the 15% reduction in head size undertaken in total hip arthroplasty completely defunctions the femoral head as a fulcrum. Groin pain is a real issue following hip arthroplasty, but edge loading of the iliopsoas probably only occurs in hip resurfacing or large head metal on metal hip replacement. This may provide another explanation for these bearings are symptomatic in general. Hip resurfacing needs to be undertaken with great care in dysplasia to avoid iliopsoas impingement. In ceramic bearing hip replacement, where ball heads are typically reduced by 15% or more from the native diameter, iliopsoas edge loading on the head rim is unlikely

Introduction. Total hip arthroplasty (THA) is the most common surgery performed for complications of bipolar arthroplasty. The present study evaluated the functional results and complications associated with this surgery. Patients and Methods. Forty eight hips (48 patients) who had conversion of bipolar arthroplasty to THA between 1998 June and 2013 June, and who were followed-up for more than one year were evaluated. Twenty one hips had conversion surgery to THA using a Fitmore cup with metal-on-metal articulation (28 mm head). Six hips had surgery using the SecurFit cup and three hips, using the Lima LTO cup with ceramic-on-ceramic articulation (28 mm or 32 mm head). Eighteen hips had surgery using a large head metal-on-metal bearing: –MMC (seven hips), ACCIS (six hips) and Magnum (five hips). The average time of follow-up duration was 3.9 years (range, 1.0–11.3). There were 22 men and 26 women between the ages of 28 and 80 years (average, 68.9 years) at the time of conversion surgery. Conversion arthroplasty was performed for acetabular erosion without femoral stem loosening in 19 hips, acetabular cartilage erosion with femoral stem loosening in 13 hips, periprosthetic fracture in 12 hips, and recurrent dislocation in four hips. Results were evaluated using Harris hip score (HHS) and Western Ontario McMaster Universities Osteoarthritis Index (WOMAC) score. The radiographs were analyzed for evidence of acetabular and femoral osteolysis or loosening. The complications were evaluated. Results. The time interval between hemiarthroplasty and conversion total hip prosthesis was 6.7 years on average (range, 5 months to 12 years). Pain was the leading sign in all patients. Average HHS improved from 42 preoperatively (range, 34–67) to 86 (range, 65 – 97) postoperatively. The average total WOMAC score improved from 47 (range, 32–67) to 22 (range, 9–44) postoperatively. All the patients operated for groin pain reported significant improvement in their symptoms. Radiological evaluation showed good bony ingrowth and stability of all the femoral components. None of the acetabular component showed migration, loosening, wear, or osteolysis at last follow-up. Complications occurred in five hips. One dislocation and one recurrent dislocation were encountered in isolated acetabular revision hips; whereas one single dislocation, one recurrent dislocation, and one trochanteric nonunion occurred in the hips with revision of both components. All dislocations were occurred in hips with a femoral head size of 28 mm. Dislocations were managed by closed reduction, and none of the patients required revision for dislocation. Conclusions. Conversion THA after symptomatic bipolar arthroplasty can offer reliable pain relief and functional improvement. The perioperative complications approximate those of revision THAs

Polyethylene and femoral head exchange for wear or osteolysis is a common operation. The difficulty lies in the facts that wear and osteolysis are difficult to measure, wear does not always correlate with osteolysis, catastrophic failure (wear through, loosening, or fracture) is difficult to predict, and these problems are usually asymptomatic. I currently recommend this procedure when complete wear through of the polyethylene is present or impending, when the patient has obvious wear and symptoms, or if there is a rapidly enlarging osteolytic lesion. The surgical goals focus on management of debris generation and management of the osteolytic lesion. A third goal becomes avoidance of the know complications of this procedure. Management of debris generation basically involves modernising the head and polyethylene. Management of the osteolytic lesion includes debridement and when possible grafting. By far the most common complication after this procedure is dislocation. Prevention of dislocation should be accomplished by patient education, use of larger heads when possible, and capsular repair. Prerequisites to perform this procedure are a replacement liner of adequate thickness that can be locked or cemented in place. The acetabular component must be stable. Lastly the component must be properly oriented to minimise both wear and dislocation. Metal-on-metal liner conversion to metal-on-poly is becoming more common. Since patient satisfaction with THA is high, MoM patients may unknowingly minimise their symptoms because they are minor compared to the symptoms before surgery. The patient history should include specific questions about groin pain, swelling, hip noise, and asking the patient if they notice their hip on a daily basis. Patient symptoms, osteolysis and a pseudotumor are indications for modular conversion. Radiographically stable, well-oriented components that can accept a polyethylene liner are requirements for a successful conversion

Purpose. The purpose of the study was to determine the rate of conversion from RSA to THR in a number of Canadian centers performing resurfacings. Method. Retrospective review was undertaken in 12 Canadian Centers to determine the rate of revision and reason for conversion from RSA to THR. Averages and cross-tabulation with Chi-Squared analysis was performed. kaplan Meier survivorship was calculated. Results. A total of 2810 resurfacings were performed up to December 2008. 770 hips had a minimum of 5 year follow-up. The overall survivorship of this group was 97.8%. 80 patients underwent conversion to THR. Five resurfacing systems were used. The reason for failure is reported: 21 were for femoral neck fracture, 25 were for loosening, 9 were for deep infection, 3 foravn, 4 pseudotumors, 2 for impingement, 6 for groin pain and 10 for other reasons. The cumulative conversion rate is 2.8%. The survivorship was significantly different analyzed by gender. The 5 year female survivorship was 95.4% and for males was 97.7%. Surgeon experience proved to be a significant factor in conversion rates. Conclusion. The revision rate to date with this new technology suggests that with increasing experience hip resurfacing arthroplasty remains an acceptable option for the treatment of hip arthritis

[Background]. Bipolar hemiarthroplasty (following BHA) have historically had poor results in patients with idiopathic osteonecrosis of femoral head (OFNH). However, most recent report have shown excellent results with new generation BHA designs that incorporate advances in bearing technology. These optimal outcomes with bipolar hemiarthroplasty will be more attractive procedure for young patients who need bone stock for future total arthroplasty. The purpose of the current study was to evaluate the clinical and radiographic finding of this procedure for the treatment of OFNH at our institution after 7-to 21years follow-up. [Subjects and Methods]. We retrospectively reviewed a consecutive series of 29 patients (40 hips) who underwent primary bipolar hemiarthroplasty for ION (36 hips with stage III and 4 hips with stage IV) with a cementless femoral component between 1992 and 2006. Osteonecrosis was associated with corticosteroid use (23 patients), alcohol (16 patients), idiopathic (one patients). The mean follow-up duration was approximately 12 (range 7 to 21) years. Patients were evaluated according to the Japan Orthopaedic Association (JOA) hip score. We evaluate osteolysis and bone response of acetabulum or femur, and migration distance of outer head were calculated at the latest follow-up. Kaplan-Meier survivorship rate was investigated to examine implant failure rate. [Results]. The average the Japan Orthopedic Association (JOA) hip score significantly improved from 53.9±16.0 points (preoperative) to 89.6±8.0 points (final follow-up). At the time of the final follow-up, one patients (one hips) had undergone revision to total hip arthroplasty because of groin pain without mechanical failure and migration. The rate of implant survival, with revision because of any reason as the end point, was 100% for femoral components and 97.5% for bipolar head components at a average 12 years (139.9±51.1 months) respectively. A complication that include dislocation and disassembly was not occurred in this study group. Radiographically, all cases is acquired bone ingrowth fixation. Stress shielding of femoral-component was observed 13 cases (32.5%). No femoral-component loosening and osteolysis of femur and acetabular was occurred at final follow-up. Bipolar head shift greater than 2mm is 3cases (7.5%) at final follow-up point, but progressive outer head migration was not occurred (0 %). [Discussion]. Survivorship determined in our study was more favorable than that of previous studies. The our long-term postoperative clinical results of up to Stage3 without the acetabular cartilage surface damage showed more superior to those previously reported for BHA. Recently, larger oscillation angle, proximal porous coating, highly cross-linked PE liner, improved these BHA design concept. Thus, the current BHA can be one of reliable procedure. Based on our results, The BHA for stage III ION, with appropriate surgical indications, may be a good option

Between April 2008 and February 2012, we implanted 159 large-diameter MOM stemmed THA with head diameters of 38–50mm. There were 6–38mm, 22–40mm, 42–42mm, 42–44mm, 24–46mm, 13–48mm, 4–50mm, 5–52mm, and one-54mm heads implanted in 138 patients (21 males and 117 females). The pre-operative diagnoses included: 120 OAs, 12 IONs, 4 femoral neck fractures, one RA, and one post-traumatic OA. Their ages were 40–86 years (avg. 63.6 yrs). Follow up was 4 to 67 months post implantation (avg. 40.4 months). All implants were manufactured by one company (Wright Medical Technology, Arlington, TN, USA). The stems were of a standard titanium-aluminum alloy, either 44 ANCA-FIT or 115 PROFEMUR Z non-cemented stems. Acetabular components were all CONSERVE PLUS cobalt-chromium monoblock shells. Heads were also fabricated out of cobalt-chromium alloy, with modular junctions. Patients with complaints of groin pain and/or swelling or hip instability underwent MRI examination in order to detect the presence of fluid collections or soft tissue masses. The statistical correlation between abnormal findings on MRI and age, gender, head diameter, component position and duration post-surgery was performed. 35 hips in 31 patients (22.0%) were found to have either a fluid collection or “pseudotumor” on MRI. These were in 5 males and 26 female patients. According to Hart's MRI classification, they were classified 21 hips in Type 1, twelve hips in Type 2, and two hips in Type 3 (Fig. 1, 2 and 3). 8 hips in 8 patients who had any pseudotumors were undergone revision THA (Fig. 4, 5 and 6). All hips had corrosions at head-neck taper junctions (Fig. 7). There was no difference in age between these two groups of patients (63.7 vs. 63.6 yrs.), but a significant difference in duration from the time of implantation of two groups (23.9 vs. 44.8 months). There appeared to be no significant difference between the mean head diameter of the two groups, 43.2mm and 44.0mm respectively. There was no statistical difference between the two groups with regard to implant orientation: cup inclination 18–70 degrees (41.8 vs. 43.6 degrees); cup anteversion −13–49 degrees (15.1 vs. 14.7 degrees); stem anteversion 2–48 degrees (20.1 vs. 23.3 degrees); and stem offset 17.5–56.2mm (38.2 vs. 37.8mm). Furthermore, according to Lewinnek's safe zone, there was no difference in cup orientation between the two groups (Fig. 8). When we investigated the types of modular neck, the hips having any pesudotumors tended to have long or varus necks. In this study, it is important to emphasize that the appearance of symptoms and development of a pseudotumor occurred early after a MOM THA in some patients. Also it will be important to subject all patients to MRI examination to evaluate the possibility of “silent” fluid collections and pseudotumors. In large-diameter head metal-on-metal stemmed THAs, femoral stems having long or varus neck may contribute to head-neck junction failure

Perthes disease often leaves young adults with hip joint incongruency due to femoral head asphericity, (extra-articular extrusion and superior flattening). This causes femoro-acetabular impingement, a reduced range of movement and early degenerative change. We report a novel method for restoration of femoral head sphericity and femoro-acetabular congruency. Two males (aged 21 and 22 years) presented with groin pain and severe hip stiffness after childhood Perthes disease. Imaging confirmed characteristic saddle shaped deformities of the femoral head, with cartilage loss overlying a central depression in the superior section of the head. A new method of treatment was proposed. Both cases were treated in the same manner. A surgical dislocation was performed with a trochanteric flip osteotomy. The extra-articular bump was removed with osteotomes and a burr to reduce femoro- acetabular impingement. The sphericity of the femoral head was restored using a HemiCap partial re-surfacing (Arthrosurface, MA, USA). The radius of the implant was selected to match that of the acetabulum. Restoration of the height of the flattened portion of the weight-bearing surface of the femoral head reduces abnormal loading of the acetabular articular cartilage by improving congruency of the joint. Both patients recovered without incident and were mobilised with crutches, restricted to touch weight-bearing for six weeks to protect union of the trochanteric osteotomy. At a minimum of three year follow-up both patients had sustained improved range of movement, pain and Oxford hip score. Repeated imaging shows no evidence of joint space narrowing or loosening at this stage. We conclude that this novel treatment functions well in the short term. Further surveillance is on-going to confirm that this treatment results in improved long term durability of the natural hip joint after Perthes disease

Total hip arthroplasty is among the most successful interventions in all of medicine and has recently been termed “The Operation of the Century”. Charnley originally stated that “Objectives must be reasonable. Neither surgeons nor engineers will ever make an artificial hip joint that will last 30 years and at some time in this period enable the patient to play football.” and he defined the appropriate patient as generally being over 65 years of age. Hip rating scales developed during this time were consistent with this approach and only required relief of pain and return to normal activities of daily living to achieve a perfect score. Since this time, however, hip arthroplasty has been applied to high numbers of younger, more active individuals and patient expectations have increased. One recent study showed that in spite of a good hip score, only 43% of patients had all of their expectations completely fulfilled following THA. The current generation metal-metal hip surface replacement arthroplasty (SRA) has been suggested as an alternative to standard THA which may offer advantages to patients including retention of more native bone, less stress shielding, less thigh pain due to absence of a stem, less limb length discrepancy, and a higher activity level. A recent technology review by the AAOS determined that currently available literature was inadequate to verify any of these suggested potential benefits. The potential complications associated with SRA have been well documented recently. The indications are narrower, the implant is more expensive, the technique is more demanding and less forgiving, and the results are both highly product and surgeon specific. Unless a clinical advantage in the level of function of SRA over THA can be demonstrated, continued enthusiasm for this technique is hard to justify. To generate data on the level of function of younger more active arthroplasty patients, a national multicenter survey was conducted by an independent university medical interviewing center with a long track record of conducting state and federal medical surveys. All patients were under 60, high demand (pre-morbid UCLA score > 6) and had received a cementless stem with an advanced bearing surface or an SRA at one of five major total joint centers throughout the country. The detailed questionnaire quantified symptoms and function related to employment, recreation, and sexual function. Patients with SRA had a higher incidence of noises emanating from the hip than other bearing surfaces although this was transient and asymptomatic. SRA patients were much more likely to have less thigh pain than THA, less likely to limp, less likely to perceive a limb length difference, more likely to run for exercise, and more likely to run longer distances. In another study of over 400 THA and SRA patients at two major academic centers, patients completed pain drawings that revealed an equivalent incidence of groin pain between THA and SRA, but an incidence of thigh pain in THA that was three times higher than in SRA in young active patients. While some or most of the observed advantages of SRA over THA may be attributable to some degree of selection bias, the inescapable conclusion is that SRA patients are demonstrating clinical advantages that warrants continued utilization and investigation of this procedure
