Determine the prevalence, etiologies, and risk factors of unplanned return to the OR (UROR) in adult orthopaedic trauma patients. Retrospective review of a trauma prospective registry from 2014 – 2019 at a Level 1 academic hospital. An UROR was defined as a patient returning to OR unexpectedly following a planned definitive surgery to either readdress the presenting diagnosis or address a complication arising from the index procedure. Univariate and multivariate logistic regression was performed comparing those patients with an UROR versus those without. A total of 1568 patients were reviewed. The rate of UROR was 9.8% (153 patients). Symptomatic implant was the leading cause of UROR (60%). Other significant UROR causes were infection (15%) and implant failure (9%). The median time between index procedure and UROR was 301 days. For the univariate and multivariate analysis, open fracture (p< 0.05), fracture complexity (p<0.01), and weekend procedure (p< 0.01) were all associated with increased risk of UROR. All other variables were not statistically significant for any associations. Those patients with an UROR for reasons other than symptomatic implants were more likely to have polyorthopaedic injuries (p < 0.05), ISS > 15 (p < 0.05), osteoporosis (p < 0.01), ICU status (p < 0.05), psychiatric history (p < 0.05), compartment syndrome (p < 0.05), neurovascular injury (p < 0.01), open fracture (p < 0.05), and fracture complexity (p < 0.05). The rate of UROR in the orthopaedic trauma patient population is 10%. Most of these cases are due to implant-related issues. UROR for reasons other than symptomatic implants tend to be polytraumatized patients with higher-energy injuries, multiple complex fractures, and associated soft tissue injuries. Future focus on improved implant development and treatments for polytraumatized patients with complex fractures is warranted to decrease a relatively high UROR rate in orthopaedic trauma.

The timing of definitive fracture fixation after Damage Control Surgery (DCS) remains a problem. Our unit employs a pragmatic approach, timing definitive surgery when the patient's clinical condition is judged satisfactory. Previous data implies fixation may result in a significant ‘second hit’ if executed <5 days after admission and DCS. The response to definitive fracture fixation in adult major trauma patients requiring DCS (MT ISS>25, n= 11) with fractures of the femoral shaft, pelvis or acetabulum were studied in comparison to patients with those fractures in isolation (IF n=21) and uninjured comparable surgical controls (SC n=12). Interleukin-8 (IL-8), IL-6 and sIL-6R levels, and neutrophil CD11b & monocyte HLA-DR expression were studied at admission, preoperatively and on days 2 & 5 post-operatively. Patients were divided into those undergoing definitive surgery within the first 5 days of admission (MT1st5 & IF1st5) or later (MTL & IFL). IL-8 levels were elevated in MT patients throughout, suggesting a proinflammatory state, whereas IL-6 levels were elevated but then declined steadily. This was independant of timing of surgery. The only post-operative rise observed was in IL-6 in SC patients. sIL-6R levels were increased in MT compared to IF patients post surgery. This elevated state, following increased IL-6 levels may be associated with resolution of the inflammatory response. CD11b expression in the MT group was unaffected. HLA-DR expression was reduced in the MT1st5 group, and post surgery in SC and IF1st5 groups. No post op cases of ARDS/MODS were diagnosed. These data suggest there is no associated detrimental effect upon the systemic inflammatory response even when undertaken less than 5 days from admission & DCS, and thus support a pragmatic approach in timing definitive

Background Hip fractures are one of the leading causes of hospital admissions. Surgical treatment is often associated with significant blood loss, requiring post-operative erythrocyte transfusion. Aim To assess post-operative erythrocyte transfusion rates for hip fractures and evaluate the effect of tranexamic acid (TXA). Method. All patients undergoing surgical repair of a hip fracture at RLBUHT between 1st April 2013 – 30th April 2014 were selected. Data regarding transfusion status and peri-operative TXA use was collected from electronic databases and case notes. Results. 450 patients were admitted with a hip fracture, of whom 441 underwent surgery. Modalities of hip fixation included total hip replacement (THR, n=39), hemiarthroplasty (n=145), dynamic hip screw (DHS, n=116), cannulated hip screw (CHS, n=17) and long (n=71) and short (n=53) intramedullary hip screw (IMHS). 44.2% received a post-operative erythrocyte transfusion, with an average of 2.5 units per patient (range 1–9 units). With all study subjects included, the average transfusion rates for THR, hemiarthroplasty, DHS, CHS, long IMHS and short IMHS were 0.44 units, 0.78 units, 0.97 units, 0.47 units, 1.55 units and 1.19 units respectively. Data regarding intra-operative TXA administration was available for 318 (72.1%) patients, sixty-one of whom received TXA. 27.9% given TXA required a post-operative transfusion (range 0–6 units), compared to 35.4% of patient with no intra-operative TXA (range 0–11 units), p=0.142. Conclusion. 44.2% of patients required an erythrocyte transfusion, with an average of 2.5 units per patient of those transfused. This is higher than rates quoted by other studies including Shokoohi et al who had a transfusion rate of 32.6%. Adherence to local transfusion protocols must be reviewed. There is a tendency for TXA to reduce post-operative transfusion requirements, however in this study, this did not reach statistical significance (p=0.142).
