Background. Jumper's knee is the result of violent and repeated contractions of quadriceps muscle caused by rapid acceleration and deceleration, jumping and kicking that load on patellar tendon stressing its mechanical resistance. The porpose of this retrospective study is to analyze the results, after the debridment of the patellar tendon and the patellar apex abrasion performed by arthroscopy, at a mean follow-up of seven years. Methods. From 1996 to 2006, sixty-four patients (seventy-three knees) affected by jumper's knee underwent surgical tretment after failure of nonoperative treatment. All knees were operated on by the same surgeon using the same surgical technique: arthroscopic debridement of the articular face of patellar tendon and arthroscopic abrasion of patellar apex. Pre-operative and post-operative evaluation was made according to IKDC score, Lysholm Knee Scale and VISA-P score. Results. The pre-operative subjective IKDC score was 52,96. This score significantly increased to 94,72 at 12 months post-operation, and has remained nearly constant at 10 years of follow-up. The mean pre-operative Lysholm Knee Scale was 51,57 and significantly increased to 86,48 at 12 months post-operation again remaining nearly constant at 10 years of follow-up. The score according to VISA-P which was pre-operatively 35,32 increased to 69,80 at 12 months post-operation and was 69,35 at 10 years of follow-up. Nineteen of the twenty-seven patients who were involved in competitive sport continued it at the same level and twenty-four were symptoms-free. Conclusions. For these satisfactory results, for the low aggressivity of this surgery, for the short recovery time and the faster return to sport we think that the debridment of the patellar tendon and the patellar apex abrasion performed by arthroscopy is the technique to prefer for surgical treatment of Jumper's Knee

Objectives. Osteochondral ankle defects (OCDs) mainly occur in a young, active population. In 63% of cases the defect is located on the medial talar dome. Arthroscopic debridement and microfracture is considered the primary treatment for defects up to 15 mm. To treat patients with a secondary OCD of the medial talar dome, a 15-mm diameter metal implant (HemiCAP ®) was developed. The set of 15 offset sizes was designed to correspond with the anatomy of various talar dome curvatures. Recently, two independent biomechanical cadaver studies were published, providing rationale for clinical use. The present study was undertaken to evaluate the clinical effectiveness and safety of the metal implantation technique for osteochondral lesions of the medial talar dome in a prospective study. Material and methods. Since October 2007, twenty patients have been treated with the implant. Four patients who did not meet the inclusion criteria and four patients who had less than one-year follow-up at the time of writing were left out of this analysis. Twelve patients are reported with one year (n=8) or two years (n=4) follow-up. All patients had had one or two earlier operations without success. On preoperative CT-scanning, the mean lesion size was 16 × 11 (range, 9–26 × 8–14) mm. Outcome measures were: Numeric Rating Scale pain (NRS) at rest and when walking, Foot Ankle Outcome Score (FAOS), American Orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot score, and clinical and radiographic complications. Data are presented as median and range. The Wilcoxon signed ranks test was used to calculate p-values. Results. All patients recovered well from surgery. The NRS at rest improved from 3 (0–7) preoperatively, to 0.5 (0–4) after 1 year and 1 (0–1) after 2 years follow-up (p < 0.05). The NRS when walking was 6.5 (4–8) preoperatively, improving to 1.5 (0–5) at 1 year and 1 (0–2) at 2 years follow-up (p < 0.05). The five subscales of the FAOS improved from 14–64 preoperatively, to 53–91 after 1 year and 63–100 after 2 years (p < 0.05). The AOFAS improved from 70 (42–75) before surgery to 86 (58–100) at 1 year, and 89 (82–90) at 2 years follow-up (p < 0.05). There were no clinical or radiographic complications. Conclusion. The metallic implantation technique appears to be a promising treatment for secondary OCDs of the talus, but more patients and longer follow-up are necessary to draw firm conclusions

In a randomised prospective study, 20 patients with intra-articular fractures of the distal radius underwent arthroscopically- and fluoroscopically-assisted reduction and external fixation plus percutaneous pinning. Another group of 20 patients with the same fracture characteristics underwent fluoroscopically-assisted reduction alone and external fixation plus percutaneous pinning. The patients were evaluated clinically and radiologically at follow-up of 24 months. The Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaire and modified Mayo wrist score were used at 3, 9, 12 and 24 months postoperatively. In the arthroscopically- and fluoroscopically-assisted group, triangular fibrocartilage complex tears were found in 12 patients (60%), complete or incomplete scapholunate ligament tears in nine (45%), and lunotriquetral ligament tears in four (20%). They were treated either arthroscopically or by open operation. Patients who underwent arthroscopically- and fluoroscopically-assisted treatment had significantly better supination, extension and flexion at all time points than those who had fluoroscopically-assisted surgery. The mean DASH scores were similar for both groups at 24 months, whereas the difference in the mean modified Mayo wrist scores remained statistically significant.

Although the groups are small, it is clear that the addition of arthroscopy to the fluoroscopically-assisted treatment of intra-articular distal radius fractures improves the outcome. Better treatment of associated intra-articular injuries might also have been a reason for the improved outcome.

A total of 179 adult patients with displaced intra-articular fractures of the distal radius was randomised to receive indirect percutaneous reduction and external fixation (n = 88) or open reduction and internal fixation (n = 91). Patients were followed up for two years. During the first year the upper limb musculoskeletal function assessment score, the SF-36 bodily pain sub-scale score, the overall Jebsen score, pinch strength and grip strength improved significantly in all patients. There was no statistically significant difference in the radiological restoration of anatomical features or the range of movement between the groups.

During the period of two years, patients who underwent indirect reduction and percutaneous fixation had a more rapid return of function and a better functional outcome than those who underwent open reduction and internal fixation, provided that the intra-articular step and gap deformity were minimised.
