This pilot study aims to investigate the utility and feasibility of a unique upright MR scan for imaging hips affected by Legg-Calve-Perthes Disease (LCPD) with patient standing up, in comparison to the standard supine scans.

Protocol development using this unique upright MRI included healthy adult and child volunteers. Optimum patient positioning in a comparable way between supine to standing was assessed. The balance between shorter scan time (to what a child can tolerate) and longer scan time (for better image acquisition). The study protocol has begun in 2 children with LCPD. Patient recruitment continues.

Early results indicate a dynamic deformity of the femoral head in early stage LCP disease. Femoral epiphysis height decreased on standing (7.8 to 6.8mm), width increased on standing (16.6 to 20.9mm) and lateral extrusion increased (3.5 to 4.1mm). Overall epiphyseal shape changed from trapezoidal (LCP femoral head when supine) to flattened triangular (LCP femoral head when standing). Differences were thus demonstrated in all parameters of bony epiphyseal height, width, extrusion and shape of a femoral head with LCP Disease when the child stood and loaded the affected hip.

Satisfactory image acquisition was possible with Coronal T1 GFE sequences, with both hips in the Field of View. 2.5min scans were performed with the child standing first, then supine. Hip position was comparable when standing and supine. Longer scans were not tolerated by younger children, more so those with LCP disease.

To our knowledge this is the first reported use of standing MRI in LCPD. A dynamic deformity has been demonstrated, with flattening, widening and worsened lateral extrusion when the child is standing compared to supine. This proof of concept investigation demonstrates the feasibility of upright MRI scanning and may demonstrate previously undetected deformity.

Aims. The treatment of tibial aplasia is controversial. Amputation represents the gold standard with good functional results, but is frequently refused by the families. In these patients, treatment with reconstructive limb salvage can be considered. Due to the complexity of the deformity, this remains challenging and should be staged. The present study evaluated the role of femoro-pedal distraction using a circular external fixator in reconstructive treatment of tibial aplasia. The purpose of femoro-pedal distraction is to realign the limb and achieve soft tissue lengthening to allow subsequent reconstructive surgery. Methods. This was a retrospective study involving ten patients (12 limbs) with tibial aplasia, who underwent staged reconstruction. During the first operation a circular hexapod external fixator was applied and femoro-pedal distraction was undertaken over several months. Subsequent surgery included reconstruction of the knee joint and alignment of the foot. Results. The mean follow-up was 7.1 years (2 to 10). The mean age of the patients at the time of the application of the fixator was 2.3 years (1.1 to 5.0). The mean time under distraction was 139.7 days (81.0 to 177.0). A mean fibular distalization of 38.7 mm (14.0 to 67.0) was achieved. Pin infections occurred in four limbs (33.3%) and osteitis in one. A femoral fracture occurred in one patient. Premature removal of the frame was not required in any patient. Sufficient realignment of the leg as well as soft tissue lengthening was achieved in all patients, allowing subsequent reconstruction. All patients were able to mobilize fully weight bearing after reconstruction. Functional outcome was limited in all limbs, and five patients (50.0%) required additional reconstructive operations. Conclusion. Regarding the functional results in the treatment of tibial aplasia, amputation remains superior to limb salvage. The latter procedure should only be performed in patients whose parents refuse amputation. Femoro-pedal distraction efficiently prepares the limb by realigning the leg and soft tissue lengthening. Minor complications are frequent, but usually do not hinder the continuation of distraction. Even though a fully weight-bearing limb is achieved, the functional outcome of reconstructive treatment remains limited. Recurrent deformities frequently occur and may require further operations. Purpose. We describe a novel method to encourage children to weight-bear after frame surgery using a whoopee cushion; the objective of this study is to assess the amount of force taken though a limb using this method. Design/participants. The amount force is required to activate the whoopee cushion is measured when a subject takes weight though a whoopee cushion on a force plate. The speed of the foot in vertical and horizontal planes is assessed with motion analysis to correlate this to the activation of the cushion and is assessed under different conditions, and with different whoopee cushions. Results. The average force required to reach the activation point was between 28N and 32N when only one cushion was strapped to the plantar surface of the foot. This is independent of the vertical speed of the foot, and of the horizontal walking speed. The use of shoes does not significantly alter the force required to reach the activation point. This force is a useful value to differentiate touch weight-bearing from partial weight-bearing. A child's weight-bearing status can be progressed if more than one whoopee cushion is used as the force needed increases. Conclusions. The whoopee cushion has been used in the rehabilitation of children to encourage weight-bearing after frame surgery. It will allow the child to distinguish between different forces expected for touch-and partial weight bearing, and can also be used to progress weight-bearing if more than one cushion is used

Purpose. To evaluate the efficacy of Kocher's criteria to differentiate between transient synovitis and septic arthritis in children. Methods and results. All children with a presentation of ‘atraumatic limp’ and a proven effusion on hip ultrasound between 2004 and 2009 were included. Patient demographics, details of the clinical presentation and laboratory investigations were documented to identify a response to each of the four variables (Weight bearing status, WCC >12,000 cells/m3, CRP >20mg/L and Temperature >38.5°C). SA was defined based upon culture and microscopy of the operative findings. 311 hips were included within the study. Of these 282 were considered to have transient synovitis. 29 patients met criteria to be classified as SA based upon laboratory assessment of the synovial fluid. The introduction of CRP eliminated the need for a four variable model as the prediction for two variables (CRP and weight bearing status) was of similar efficacy. Treating individuals who were non-weight-bearing and a CRP >20mg/L as SA correctly classified 94.8% individuals, with a sensitivity of 75.9%, specificity of 96.8%, positive predictive value of 71.0%, and negative predictive value of 97.5%. CRP was a significant independent predictor of septic arthritis. Conclusion. Mathematical models using common variables used in clinical practice are a useful way of considering ‘risk’ in SA. However, whilst such models correctly classify the majority of individuals they are by no means failsafe. Such models are more reliable in excluding SA, than including SA, and therefore a clinician's acumen remains important in identifying SA in those individuals with a single abnormal variable

Aims

This exploratory randomized controlled trial (RCT) aimed to determine the splint-related outcomes when using the novel biodegradable wood-composite splint (Woodcast) compared to standard synthetic fibreglass (Dynacast) for the immobilization of undisplaced upper limb fractures in children.

Methods

An exploratory RCT was performed at a tertiary paediatric referral hospital between 1 June 2018 and 30 September 2019. The intention-to-treat population consisted of 170 patients (mean age 8.42 years (SD 3.42); Woodcast (WCG), n = 84, 57 male (67.9%); Dynacast (DNG), n = 86, 58 male (67.4%)). Patients with undisplaced upper limb fractures were randomly assigned to WCG or DNG treatment groups. Primary outcome was the stress stability of the splint material, defined as absence of any deformations or fractures within the splint during study period. Secondary outcomes included patient satisfaction and medical staff opinion. Additionally, biomechanical and chemical analysis of the splint samples was carried out.

The Ponseti method of clubfoot management requires a period of bracing in order to maintain correction. This study compared the effectiveness of ankle foot orthoses and Denis Browne boots and bar in the prevention of recurrence following successful initial management. Between 2001 and 2003, 45 children (69 feet) with idiopathic clubfeet achieved full correction following Ponseti casting with or without a tenotomy, of whom 17 (30 clubfeet) were braced with an ankle foot orthosis while 28 (39 clubfeet) were prescribed with Denis Browne boots and bar. The groups were similar in age, gender, number of casts and tenotomy rates. The mean follow-up was 60 months (50 to 72) in the ankle foot orthosis group and 47 months (36 to 60) in the group with boots and bars. Recurrence requiring additional treatment occurred in 25 of 30 (83%) of the ankle foot orthosis group and 12 of 39 (31%) of the group with boots and bars (p < 0.001). Additional procedures included repeat tenotomy (four in the ankle foot orthosis group and five in the group treated with boot and bars), limited posterior release with or without tendon transfers (seven in the ankle foot orthosis group and two in the group treated with boots and bars), posteromedial releases (nine in the orthosis group) and midfoot osteotomies (five in the orthosis group, p < 0.001).

Following initial correction by the Ponseti method, children managed with boots and bars had far fewer recurrences than those managed with ankle foot orthoses. Foot abduction appears to be important to maintain correction of clubfeet treated by the Ponseti method, and this cannot be achieved with an ankle foot orthosis.
